1. Assessing Psychosocial Burden in Psoriasis Patients Using the PRISM-RII Tool: A Comprehensive Evaluation

Abstract

Introduction: Psoriasis is a chronic, immune-mediated inflammatory disease that significantly impacts patients' quality of life due to its visible lesions and associated stigma. Psychological comorbidities are prevalent among psoriasis patients. Objectives: This study aimed to evaluate the utility of the Pictorial Representation of Illness and Self Measure Revised II (PRISM-RII) tool in assessing the psychosocial burden, including internalized stigma, in psoriasis patients. Methods: A cross-sectional study was conducted with 190 psoriasis patients recruited from a university hospital's dermatology treatment center. Participants completed the PRISM-RII Psoriasis Internalized Stigma Scale (PISS), Hospital Anxiety and Depression Scale (HADS), and Dermatology Life Quality Index (DLQI). Clinical severity was measured using the Psoriasis Area and Severity Index (PASI). Results: Significant correlations were observed between PRISM-RII parameters and measures of quality of life, internalized stigma, anxiety, and depression. Our study revealed significant differences in PASI scores among different Illness Perception Measures (IPM). A negative correlation was found between PASI and Self-Illness Separation (SIS), suggesting that increased disease severity is associated with a higher perceived burden of illness. Additionally, significant negative correlations were observed between SIS and DLQI, HADS, and PISS. Patients with larger IPM reported higher levels of anxiety, depression, and internalized stigma, as well as lower treatment satisfaction. Conclusions: The PRISM-RII tool effectively captures the psychosocial burden of psoriasis including internalized stigmatization, highlighting the need to address both physical and psychological aspects in clinical practice. Incorporating PRISM-RII into routine assessments can enhance patient-centered care by identifying and addressing the psychosocial dimensions of psoriasis.

4. Turkish Validity and Reliability Study of the ECT Perception and Knowledge Scale

Abstract

INTRODUCTION: Electroconvulsive Therapy (ECT) is an effective and safe treatment method used in the treatment of various psychiatric diseases. However, negative attitudes associated with ECT are common. This causes many negative consequences, from the treatment preference to treatment response and stigma. In this study, we aimed to carry out a validity-reliability analysis of the ECT Perception and Knowledge Scale (ECT-PK), which was developed to determine the perception and knowledge levels related to ECT, and adapt it to Turkish. METHOD: The Turkish adaptation of the ECT-PK was made using the translation-retranslation method. Our study included 50 patients with schizophrenia, 50 patients with bipolar disorder, 50 patients with major depression who met the remission criteria determined separately for each disorder, and 150 healthy controls.. To measure test-retest reliability, the scale was re-applied to 30 patients randomly selected from the patient group 14–21 days after the first application of the scale. RESULTS: In our study, a significant difference was found in both the patient and control groups in terms of the history of ECT application in the past and the status of accepting ECT application when recommended, and the perception and knowledge subscales of the ECT-PK. These results support the construct and criterion validity of the ECT-PK. Cronbach’s alpha coefficient was found to be 0.85 for the perception subscale and 0.78 for the knowledge subscale. The intra-class correlation coefficient used to evaluate the test-retest reliability was 0.86 for the perception scale and 0.83 for the knowledge subscale. CONCLUSION: It has been shown that the ECT-PK is a valid and reliable measurement tool that can be used to measure the perception and knowledge levels related to ECT in both clinical and non-clinical groups.

5. Neutrophil to lymphocyte and monocyte to high-density lipoprotein ratios are promising inflammatory indicators of bipolar disorder

Abstract

The neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), monocyte to lymphocyte ratio (MLR), and monocyte to high-density lipoprotein (MHR) are indicators of inflammation. In this study, we aimed to examine the possible association between NLR, PLR, MLR, and MHR in the same patients with bipolar disorder (BD) during their manic, depressive, and euthymic episodes.The participants of this study consisted of 61 patients with BD, aged between 18 and 65, who were hospitalized with a diagnosis of BD. Patients who were hospitalized during their manic and depressive episodes and medication free for at least 1 month before hospitalization were included. White blood cell, neutrophil, lymphocyte, platelet, and monocyte counts, high-density lipoprotein (HDL) cholesterol, and C reactive protein (CRP) levels were recorded.Leukocytes (This study was the first study to examine the inflammatory markers such as NLR, MLR, PLR, and MHR levels in same patients with BD during their three episodes of disorder. Both NLR and MHR values in manic and depressive episodes were higher than euthymic episodes. NLR and MHR were useful inflammatory markers to evaluate inflammation in bipolar patients.

Abstract

Septum pellucidum, which forms the medial wall of the lateral ventricles, consists of two laminates. Cavum septum pellucidum (CSP) is defined when there is a space between these laminae. In some MRI studies have shown a higher rate of large CSP in patients with schizophrenia than in normal subjects. Looking at the literature on psychiatric disorders, CSP has been shown to be most associated with schizophrenia. Large CSP supports the neurodevelopmental model, which is one of the etiological explanations of schizophrenia. In our study, two patients with a diagnosis of CSP are mentioned. One of our patients is a first episode of schizophrenia, and the other one chronic schizophrenia patient with a history of multi-drug resistance. The first episode of schizophrenia is consistent with the information available in the literature in terms of the severity of symptoms, weak-response to treatment, and insufficiency of neuropsychological tests. The apparent deficit of the chronic schizophrenia patient suggests that CSP has a neurodevelopmental model in the etiology of schizophrenia, as well as the duration of the disease and non-compliance with treatment. There is no study in the literature comparing the response to treatment with large CSP in schizophrenia. It is thought that investigation of response to treatment in future studies is important for demonstrating the effects of neurodevelopmental model on the treatment of psychiatric disorders.

9. Can Comorbid Bipolar Disorder Be Associated with Atypical Depression in Patients with Social Anxiety Disorder ?

Abstract

Social anxiety disorder (SAD) is one of the most common psychiatric disorders and frequently co-exists with other psychiatric conditions, primarily with mood disorders. MD is the most common psychiatric comorbidity in patients with SAD, and the association of anxiety disorders and bipolar disorder with atypical depression, which is included in diagnostic guidelines for MD as a subgroup, has been well established. The present study aims to determine if SAD patients with comorbid atypical depression or bipolar disorder show differences in terms of symptomatology and disease course compared to SAD patients without bipolar disorder. We hypothesize that social phobia patients may have subgroups within themselves and the processes of these subgroups may be different from those of known SAD patients. In this study a retrospective chart review was performed for patients who had applied to the Psychiatry Outpatient Unit, Kartal Research and Training Hospital, during a 7-month period in 2018. The study had a retrospective design. A total of 82 patients diagnosed with Social Anxiety Disorder based on a SCID-I interview for DSM-IV were included in the study. Of the 82 SAD patients, 16 patients (19.5%) had also co-existing BPD. All SAD patients with comorbid BPD had at least one major depressive episode history, while 15 (93.7%) SAD patients with comorbid BPD exhibited atypical features in at least one episode. Thus, we identified an association between SAD/BPD and atypical depression and discussed the importance of this co-occurrence in terms of clinical evaluation.

10. Relationship between childhood trauma, dyadic adjustment, and sexual functions in patients with bipolar disorder

Abstract

Purpose: This study aimed to investigate the dyadic adjustment and sexual functions in patients with bipolar disorder (BD) during the remission period, and examine the effect of a history of childhood trauma on these. Materials and Methods: This cross-sectional study is consisted of 80 (46 female, 34 male) patients with bipolar disorder-1 and 40 (25 female, 15 male) healthy controls. The Dyadic Adjustment Scale (DAS), Childhood Trauma Questionnaire (CTQ-28), Arizona Sexual Questionnaire (ASEX) were administered to the participants. Results: The patient group showed significantly higher trauma scores in all subscales and total scores of the CTQ-28, and lower scores in the subscales and total score of DAS compared to the control group. 82.5% of bipolar patients and 57.5% of healthy controls were found to have sexual dysfunction (SED). According to the results of linear regression and multiple regression analyses, age and DAS-total score predicted SED, and male gender, absence of SED and CTQ-total score predicted DAS score. Conclusion: Patients with BD have more dyadic problems, sexual dysfunction, and trauma history than healthy controls. Sexual function and dyadic adjustment affect each other in two ways and a history of childhood trauma has a negative effect on both sexual function and dyadic adjustment.
