Resume Objectifs : comparer les donnes de la cytometrie en flux (ploidie et phases SG2M)) aux donnees cytologiques, anatomopathologiques dans les tumeurs de la vessie. Materiels et methodes : l'etude du contenu en ADN de 48 lavages de vessie par cytometrie en flux (CMF) a ete realisee sur un groupe temoin (n=20) et un groupe de patients atteints d'un cancer de la vessie (n=28). Resultats : nous avons montre que la specificite diagnostique (94 %) de la CMF est superieure a celle de la cytologie conventionnelle (75 %). En revanche, sa sensibilite (68 %) s'est revelee inferieure a celle de la cytologie (91 %). Si les deux techniques sont utilisees conjointement, on atteint 100 % de sensibilite. L'etude du groupe de patients a montre une augmentation de la frequence des anomalies cytometriques (aneuploidies et/ou activite de proliferation : SG2M > 10 %) en fonction du grade et du stade de la tumeur. En effet le test cytometrique est positif dans 80 % des G3 et dans 68 % des G2. Concernant le stade, il est positif dans 46 % des pTa-pT1, 89 % des pT2 et 100 % des pT4. Par ailleurs, la comparaison des temoins avec les patients a montre une difference significative entre les groupes suivants : temoins/G1-G2 (p Conclusion : nous avons, par cette etude, confirme l'utilite de la cytometrie en flux comme element de diagnostic et de pronostic des tumeurs de la vessie, et nous avons mis le point sur l'importance de la classification des histogrammes afin de faciliter leur interpretation et eviter les pieges des fausses aneuploidies.

Resume L'hyperplasie thymique est un phenomene qui a ete decrit apres des situations de stress. Ce phenomene a ete egalement decrit chez des enfants cancereux traites par une chimiotherapie. Observation. – Cette patiente a ete prise en charge a l'âge de 18 mois pour un nephroblastome gauche non metastatique traite par association de chimiotherapie et de chirurgie. La chimiotherapie comportait des cures de vincristine, actinomycine et adriamycine. Le bilan de fin de traitement etait normal. Quatre mois apres la fin du traitement, la radiographie pulmonaire montrait un elargissement mediastinal, qui se revelait d'origine thymique a la tomodensitometrie thoracique. Cet aspect a fait suspecter une recidive. La biopsie a montre cependant un aspect d'hyperplasie thymique sans envahissement tumoral. L'evolution s'est faite vers une regression en 2 mois. Conclusion. – L'hyperplasie thymique survenant en phase de remission d'un cancer traite par chimiotherapie, pose un probleme de diagnostic differentiel avec une recidive tumorale mediastinale. La cytologie a l'aiguille fine peut etre un moyen peu invasif pour poser le diagnostic. La corticotherapie ne peut etre utilisee qu'apres confirmation histologique de la nature de l'hypertrophie thymique.

Thymic hyperplasia in response to stress is a well known phenomenon. Thymic hyperplasia has also been described after chemotherapeutic treatment for malignancies in children.A three-year-old girl was followed up from the age of 18 months for a left kidney nephroblastoma treated by combination of chemotherapy (vincristin, actinomycin and adriamycin) and surgery. Assessment at the end of treatment was normal. Four months after the end of treatment, pulmonary radiography showed mediastinal enlargement, which was shown to originate in the thymus at thoracic CT scan. A recurrence of the disease was suspected. Biopsy showed thymic hyperplasia without evidence of tumor cells. Mediastinal enlargement then disappeared spontaneously 2 months later.Thymic hyperplasia occurring during remission of a cancer treated by chemotherapy is a diagnostic dilemma as it suggests mediastinal reccurence of the disease. Needle aspiration cytology is an appropriate investigation in thymic hyperplasia. No steroid therapy should be used before histologic diagnosis of thymic hyperplasia.

To compare flow cytometric data (ploidy and proliferative activity or percentage SG2M-phase cells) to cytologic and histologic data of the bladder carcinomas.Cytologic and flow cytometric analysis of DNA content were performed on 48 bladder washings: 28 bladder washings from patients being followed for urothelial carcinomas and 20 control washings from individuals undergoing cytoscopy for other reasons.Cytological sensitivity and specificity of bladder washing were 75% and 91% respectively. Specificity was increased to 94% using flow cytometric DNA analysis whereas sensibility was moderately decreased to 68%. Combination of flow cytometry and cytology increased the diagnostic yield to 100%. The study of the patient group showed an increased abnormalities (aneuploidy and/or proliferate activity SG2M10%) according to the tumor grading and tumor staging. A cytometric test was positive in 80% for G3 tumours and in 68% for G2 tumours. The staging tumor was positive in 46%, 89% and 100% of the pTa-pT1, pT2 and pT4 tumours respectively. Otherwise the comparison of control group with patients showed a statistical correlation between cytometric test, staging tumour and tumoral grading as showed in the following groups: control/G1-G2 (p0.05), control/G3 (p0.001), control/pTa-pT (p0.05), control/pT2-pT4 (p0.001).We confirmed through this study the interest of the flow cytometric DNA analysis in the diagnosis and prognosis of bladder carcinomas, and we showed the importance of the histogram classification in order to facilitate their interpretation and to avoid the trap of false aneuploidy.
