Idiopathic pulmonary fibrosis (IPF) is the most widely studied, arguably the most severe etiology of ILD and the most common IIP diagnosis. The objective of this narrative review is to outline the current evidence on optimal perioperative management of IPF. PubMed, Embase and Web of Science were analyzed for appropriate peer-reviewed references by utilizing key word search (“interstitial lung disease” OR “idiopathic pulmonary fibrosis” OR “idiopathic interstitial pneumonitis” OR “ILD” OR “IPF” AND “surgery” OR “anesthesia” OR “perioperative”) within the past thirty years (1990-current). Non-English language references were excluded. A total of 205 references were curated by the authors. Eighty-seven consensus statements, clinical trials, retrospective cohort studies or case series met criteria and were incorporated into the findings of this narrative review.Conclusion: After review, we conclude that complications, dominated by postoperative pulmonary complications, pose a significant barrier to safe perioperative care of patients with IPF. Ensuring that the preoperative IPF patient has been medically optimized is important for minimizing this risk. Initial assessment of the ARISCAT score, pulmonary function studies and cardiopulmonary exercise testing may identify IPF patients at particularly high perioperative pulmonary risk. Identifying IPF patients with 6– 12-month declines in DLCO of > 15%, V02max < 8.3 mL/kg/min, < 80% predicted value FVC, a 50-meter reduction in the 6MWT or preoperative home oxygen use may be helpful in preoperative risk stratification. Medically optimizing treatable co-morbidities should be a priority in preoperative assessment. Regional or neuraxial anesthesia should be considered an optimal technique for the avoidance of general anesthesia related complications when indicated. Acute exacerbation and postoperative pneumonia have been identified as important postsurgical complications in both thoracic and nonthoracic surgical populations.Keywords: interstitial lung disease, lung fibrosis, perioperative, anesthesia, surgery

The positive features of neural networks and fuzzy logic are combined together for the detection of stator inter-turn insulation and bearing wear faults in single-phase induction motor. The adaptive neural fuzzy inference systems (ANFISs) are developed for the detection of these two faults. These faults are created experimentally on a single-phase induction motor in the laboratory. The experimental data is generated for the five measurable parameters, viz, motor intakes current, speed, winding temperature, bearing temperature, and the noise of the machine. Earlier, the ANFIS fault detectors are trained for the two input parameters, i.e., speed and current, and the performance is tested. Later, the three remaining parameters are added and the five input ANFIS fault detector is trained and tested. It observed from the simulation results that the five input parameter system predicts more accurate results. Sportswear textile is the interface between consumers, clothing and the environment in conjunction with human physiology. In this study, multi-response optimization of sensorial comfort properties of terry and fleece knitted sportswear fabrics was planned by varying areal densities (GSM) and fiber content under Taguchi-based gray rational technique. The L36 Taguchi orthogonal array was used to develop thirty-six samples out of which 18 samples were washed in order to determine the effect of washing, areal density and fiber content on sensorial comfort of fabrics. From the results it was revealed that washing process caused positive influence on crease recovery rate, smoothness score, and resilience while slight reduction in drapability and softness was observed after washing process. It was found that as the polyester content was increased from 20 to 40% the sensory characteristics of both terry and fleece fabrics were increased. Similarly, the areal density of fabric also caused significant influence on sensory attributes of clothing comfort. Based on Taguchi gray rational optimization, the optimum fabric parameters were found to be terry fabric, after wash, 240 GSM and 60/40 cotton: polyester content for simultaneously achieving good sensorial comfort properties for sportswear fabric.

The water, air permeability and thermal resistance of fabrics are important attributes that have a significant impact on the thermal comfort properties of sportswear fabrics in different environmental conditions. In this work, terry and fleece fabrics were developed by varying the fibre content and mass per unit area of fabrics. Moreover, the thermo-physical properties of the developed fabrics, including air permeability, water vapor permeability and thermal resistance, were analysed before and after washing. The multi-response optimization of the thermal comfort properties of knitted fabrics was performed using principal component analysis (PCA) and the Taguchi signal-to-noise ratio (PCA-S/N ratio) to achieve optimal properties. It was determined that the selected parameters (fabric type, finishing, fibre content and fabric mass per unit area) had a significant effect on the thermal comfort properties of knitted fabrics. The PCA analysis showed that 100% cotton terry fabric before washing with an aerial weight of 220 g/m2 had higher air and water vapor permeability value, but a lower thermal resistance value.

Kapok fiber is emerging as a cheaper natural fiber having excellent thermal insulation properties. Recycled polyester/kapok blended needle punched nonwoven waddings are prepared where the effect of kapok fiber %, sheet areal density, and needle punch density on thermal resistance and air permeability is measured and analyzed with the help of MINITAB®18.1. Three percentages of kapok fiber (20, 40, 60%), four levels of fabric areal density (150, 300, 450, 600 g.m-2) and two levels of punch density (15, 20 punches.cm-2) were chosen. The statistical analysis revealed that kapok fiber % and areal density had a significant effect on both thermal resistance and air permeability of nonwovens. The thermal resistance was decreased significantly with the increase in kapok fiber %, especially at higher punch density. A significant interaction between kapok fiber % and areal density was observed for air permeability. It may be concluded that, in the selected range of input variables, recycled PET/kapok nonwovens with less % of kapok fibers and punch density display better thermal insulation performance, especially at higher areal density. The prepared nonwovens may find applications in winter coats and wadding layers for quilts, etc., and will also help to reduce the pollution from the environment.

Piezoelectric materials have attracted more attention than other materials in the field of textiles. Piezoelectric materials offer advantages as transducers, sensors, and energy-harvesting devices. Commonly, ceramics and quartz are used in such applications. However, polymeric piezoelectric materials have the advantage that they can be converted into any shape and size. In smart textiles, polyvinylidene fluoride (PVDF) and other piezoelectric polymers are used in the form of fibers, filaments, and composites. In this research, PVDF nanofibers were developed and integrated onto a knitted fabric to fabricate a piezoelectric device for human body angle monitoring. Scanning electron microscopy and X-ray diffraction analyses were used to study the morphology and to confirm the beta phase in fibers. The results reveal that the nanofibers made from solutions with high concentration were smooth and defect-free, compared to the fibers obtained from solutions with low concentration, and possess high crystallinity as well. Under high dynamic strain more output voltage is generated than under low dynamic strain. The maximum current density shown by the device is 172.5 nA/cm2. The developed piezoelectric nanofiber sensor was then integrated into a knitted fabric through stitching to be used for angle measurement. With increasing bending angle, the output voltage increased. The promising results show that the textile-based piezoelectric sensor developed in this study has a great potential to be used as an angle measuring wearable device for the human body due to its high current density output and flexibility.

Aim The burden of Type 2 diabetes is alarmingly high in South Asia, a region that has many genetically diverse ethnic populations. Genome-wide association studies ( GWAS) conducted largely in European populations have identified a number of loci predisposing to Type 2 diabetes risk, however, the relevance of such genetic loci in many South Asian sub-ethnicities remains elusive. The aim of this study was to replicate 49 single nucleotide polymorphisms ( SNPs) previously identified through GWAS in Punjabis living in Pakistan. Methods We examined the association of 49 SNPs in 853 Type 2 diabetes cases and 1945 controls using additive logistic regression models after adjusting for age and gender. Results Of the 49 SNPs investigated, eight showed a nominal association ( P < 0.05) that also remained significant after controlling for the false discovery rate. The most significant association was found for rs7903146 at the TCF7L2 locus. For a per unit increase in the risk score comprising of all the 49 SNPs, the odds ratio in association with Type 2 diabetes risk was 1.16 (95% CI 1.13-1.19, P < 2.0E-16). Conclusion These results suggest that some Type 2 diabetes susceptibility loci are shared between Europeans and Punjabis living in Pakistan. [ABSTRACT FROM AUTHOR]

The dosimetric inter-comparison studies carry pertinent significance to ensure uniform radiation dose delivery for clinical trials. This paper investigates the comparative performance analysis of an X-ray high power linear accelerator performed by the International Dosimetry Survey Mission and institute of Nuclear Medicine & Oncology (INMOL), Lahore. The measurements were made using cylindrical ionization chambers based on the International Atomic Fnergy Agency (IAHA) TRS-398 protocol for absorbed dose-to-water dosimetric standards, and the percentage deviation was found to be between 0. 5% - 1 % . The dosimetric analysis concerning linear X-ray accelerator output performed by INMOL, Lahore was found to be in good agreement with the results of I AHA Dosimtery Survey Mission. Antimicrobial viroblock/polyacrylonitrile nanofiber webs fabricated using the electrospinning method were assessed in terms of the antimicrobial activity against infectious agents as a potential material used in mask production. Viroblock (VB) is an amalgam of lipid vesicle and silver. Lipid vesicle depletes the virus outer membrane, which contains cholesterol, while silver ions penetrate the virus, interact with sulfur-bearing moieties, and possess the virus bactericidal property. VB, having anti-coronavirus and anti-influenza properties, was prepared in four different concentrations, 0.5 wt%, 1 wt%, 1.5 wt%, and 2 wt%, in regard to nanofiber weight. The resultant nanofibers were characterized by scanning electron microscope (SEM), Fourier transform infrared (FTIR), thermogravimetric analysis (TGA), water contact angle, water content, and thermogravimetric analysis (TGA). Moreover, all nanofibrous samples were evaluated for cell proliferation assay and ATCC antibacterial tests. Based on characterization results and cytotoxicity, the developed composite nanofibers-based webs showed good promise for utilization in anti-viral masks. Particularly, 2 wt% VB/PAN nanofibers have the highest antibacterial properties against negative and positive bacteria along with excellent cell viability.

Russia is one of the 8 member states of the Arctic Council, so the Arctic is one of the priority areas of Russia’s foreign policy. Another player in the Arctic territories in cooperation with Russia is Belarus, which can provide assistance in many areas related to technological progress. In this article, the author examines which areas of cooperation between Russia and Belarus are the most successful, and what platforms (The Union State of Russia and Belarus or the EAEU) and institutions exist for this purpose. In addition, it is important to consider the problems that arise in the process of cooperation, as well as the question of whether a confrontation is possible within the integration association. The author also notes further lines of cooperation and opportunities for both states in the Arctic.

Anastomotic stenosis of an arteriovenous fistula is often amenable to percutaneous intervention (angioplasty and stenting) and unlikely to be complicated by infection. A 69-year-old man underwent pre-emptive arteriovenous fistula construction that required interval placement of a covered stent for juxta-anastomotic stenosis. The patient presented 1 year after the intervention with systemic sepsis that required stent graft explantation and revision. This is a unique case report showing an infected stent graft, placed to restore secondary patency, that was later found to be the source of bacteremia and septic pulmonary emboli. Acquired diaphragmatic perforation leading to massive hepatic hydrothorax and respiratory failure is a rare complication of microwave ablation (MWA) of hepatocellular carcinoma (HCC). Imaging modalities to detect pleuroperitoneal communication remain poorly described. We report a nuclear imaging technique used to efficiently diagnose and locate diaphragmatic defects. A 57-year-old male with cirrhosis and HCC presented with respiratory distress after undergoing MWA of a HCC lesion. He was admitted to the intensive care unit for noninvasive positive pressure ventilator support. Chest radiography revealed a new large right pleural effusion. Large-volume thoracentesis was consistent with hepatic hydrothorax. The fluid reaccumulated within 24 hours; therefore an acquired diaphragmatic perforation induced by the ablation procedure was suspected. To investigate, 99mTechnetium-labeled albumin was injected into the peritoneal cavity. The tracer accumulated in the right hemi thorax almost immediately. The patient then underwent transjugular intrahepatic portosystemic shunting in efforts to relieve portal hypertension and decrease ascites volume. Unfortunately, the patient deteriorated and expired after few days. Although diaphragmatic defects develop in cirrhotic patients, such small fenestrations do not normally lead to rapid development of life-threatening pleural effusion. MWA procedures can cause large diaphragmatic defects. Immediate detection of this complication is essential for initiating early intervention.

Spodoptera exigua (Hübner) (Lepidoptera: Noctuidae) is a highly dispersive, polyphagous insect pest that severely defoliates crops. Excessive reliance on synthetic insecticides leads to ecological pollution and resistance development, urging scientists to probe eco-friendly biopesticides. Here, we explore the virulence of an entomopathogenic fungus, Beauveria bassiana , against S. exigua , resulting in 88% larval mortality. Using an age-stage, two-sex life table, we evaluated the lethal and sublethal effects of B. bassiana on the demographic parameters of S. exigua , including survival, development, and reproduction. Sublethal (LC 20 ) and lethal concentrations (LC 50 ) of B. bassiana impacted the parental generation (F 0 ), with these effects further influencing the demographic parameters of the first filial generation (F 1 ). The infected F 1 offsprings showed a reduced intrinsic rate of increase ( r ), mean generation time ( T ), and net reproduction rate ( R 0 ). Larval developmental duration varied significantly between the control (10.98 d) and treated groups (LC 20 : 10.42; LC 50 : 9.37 d). Adults in the treated groups had significantly reduced lifespans (M: 8.22; F: 7.32 d) than the control (M: 10.00; F: 8.22 d). Reduced fecundity was observed in the B. bassiana -infected groups (LC 20 : 313.45; LC 50 : 223.92 eggs/female) compared to the control (359.55 eggs/female). A biochemical assay revealed elevated levels of detoxification enzymes (esterases, glutathione S-transferases, and acetylcholinesterase) in the F 0 generation after B. bassiana infection. However, the enzymatic activity remained non-significant in the F 1 generation likely due to the lack of direct fungal exposure. Our findings highlight the enduring effects of B. bassiana on the biological parameters and population dynamics of S. exigua , stressing its use in eco-friendly management programs.

Nicotine, a naturally occurring alkaloid found in tobacco, is a potent neurotoxin extensively used to control Nilaparvata lugens (Stål), a destructive insect pest of rice crops. The insect gut harbors a wide array of resident microorganisms that profoundly influence several biological processes, including host immunity. Maintaining an optimal gut microbiota and immune homeostasis requires a complex network of reciprocal regulatory interactions. However, the underlying molecular mechanisms driving these symbiotic exchanges, particularly between specific gut microbe and immunity, remain largely unknown in insects. Our previous investigations identified and isolated a nicotine-degrading Burkholderia cepacia strain (BsNLG8) with antifungal properties. Building on those findings, we found that nicotine intake significantly increased the abundance of a symbiotic bacteria BsNLG8, induced a stronger bacteriostatic effect in hemolymph, and enhanced the nicotine tolerance of N. lugens. Additionally, nicotine-induced antimicrobial peptides (AMPs) exhibited significant antibacterial effects against Staphylococcus aureus. We adopted RNA-seq to explore the underlying immunological mechanisms in nicotine-stressed N. lugens. Bioinformatic analyses identified numerous differentially expressed immune genes, including recognition/immune activation (GRPs and Toll) and AMPs (i.e., Defensin, Lugensin, lysozyme). Temporal expression profiling (12, 24, and 48 hours) of immune genes revealed pattern recognition proteins and immune effectors as primary responders to nicotine-induced stress. Defensin A, a broad-spectrum immunomodulatory cationic peptide, exhibited significantly high expression. RNA interference-mediated silencing of Defensin A reduced the survival, enhanced nicotine sensitivity of N. lugens to nicotine, and decreased the abundance of BsNLG8. The reintroduction of BsNLG8 improved the expression of immune genes, aiding nicotine resistance of N. lugens. Our findings indicate a potential reciprocal immunomodulatory interaction between Defensin A and BsNLG8 under nicotine stress. Crystal (Cry) toxins, produced by Bacillus thuringiensis , are widely used as effective biological pesticides in agricultural production. However, insects always quickly evolve adaptations against Cry toxins within a few generations. In this study, we focused on the Cry1Ac protoxin activated by protease. Our results identified PxTrypsin-9 as a trypsin gene that plays a key role in Cry1Ac virulence in Plutella xylostella larvae. In addition, P. xylostella miR-2b-3p, a member of the micoRNA-2 (miR-2) family, was significantly upregulated by Cry1Ac protoxin and targeted to PxTrypsin-9 downregulated its expression. The mRNA level of PxTrypsin-9, regulated by miR-2b-3p, revealed an increased tolerance of P. xylostella larvae to Cry1Ac at the post-transcriptional level. Considering that miR-2b and trypsin genes are widely distributed in various pest species, our study provides the basis for further investigation of the roles of miRNAs in the regulation of the resistance to Cry1Ac and other insecticides.

MicroRNAs (miRNAs) play a pivotal role in important biological processes by regulating post-transcriptional gene expression and exhibit differential expression patterns during development, immune responses, and stress challenges. The diamondback moth causes significant economic damage to crops worldwide. Despite substantial advancements in understanding the molecular biology of this pest, our knowledge regarding the role of miRNAs in regulating key immunity-related genes remains limited. In this study, we leveraged whole transcriptome resequencing data from Plutella xylostella infected with Metarhizium anisopliae to identify specific miRNAs targeting the prophenoloxidase-activating protease1 (PAP1) gene and regulate phenoloxidase (PO) cascade during melanization. Seven miRNAs (pxy-miR-375-5p, pxy-miR-4448-3p, pxy-miR-279a-3p, pxy-miR-3286-3p, pxy-miR-965-5p, pxy-miR-8799-3p, and pxy-miR-14b-5p) were screened. Luciferase reporter assays confirmed that pxy-miR-279a-3p binds to the open reading frame (ORF) and pxy-miR-965-5p to the 3' untranslated region (3' UTR) of PAP1. Our experiments demonstrated that a pxy-miR-965-5p mimic significantly reduced PAP1 expression in P. xylostella larvae, suppressed PO activity, and increased larval mortality rate. Conversely, the injection of pxy-miR-965-5p inhibitor could increase PAP1 expression and PO activity while decreasing larval mortality rate. Furthermore, we identified four LncRNAs (MSTRG.32910.1, MSTRG.7100.1, MSTRG.6802.1, and MSTRG.22113.1) that potentially interact with pxy-miR-965-5p. Interference assays using antisense oligonucleotides (ASOs) revealed that silencing MSTRG.7100.1 and MSTRG.22113.1 increased the expression of pxy-miR-965-5p. These findings shed light on the potential role of pxy-miR-965-5p in the immune response of P. xylostella to M. anisopliae infection and provide a theoretical basis for biological control strategies targeting the immune system of this pest.

Background: Gastroparesis is a serious condition that can be caused by diabetes, surgery or infection, or can be idiopathic. When there is no mechanical obstruction, gastroparesis is characterized by delayed stomach emptying. Itopride, a prokinetic drug, inhibits acetylcholinesterase activity in addition to antagonizing dopamine D2 receptors., Methods: This prospective, multicentre study is based on real-world data from 988 patients with a diagnosis of diabetic gastroparesis for index (PAGI-SYM2) evaluation at baseline and week 4 of treatment for upper gastrointestinal disorder symptoms., Results: Upper gastrointestinal symptom severity scores improved significantly after 4 weeks of treatment ( p <0.001), with significant improvement across all categories of gastroparesis (very mild (37-58.6%), mild degree (24.6-31.6%), moderate (29.3-7.3%) and severe (8.8-2.6%)., Conclusion: Itopride SR (Nogerd SR) in a 150 mg once-daily dose showed promising results in reducing the severity of upper gastrointestinal disorder symptoms associated with diabetic gastroparesis. Both statistical and clinical effectiveness were observed. Spodoptera frugiperda is a highly destructive migratory pest that threatens various crops globally. Autographa californica multiple nucleopolyhedrovirus (AcMNPV) is an effective biocontrol agent against lepidopteran pests. Here, we explored the molecular mechanisms underlying the immune response to AcMNPV infection in S. frugiperda . RNA-seq and quantitative reverse transcription-polymerase chain reaction (qRT-PCR) analyses identified the Toll, IMD, and apoptosis pathways as primary immune responses. Investigation into AcMNPV-induced apoptosis in the S. frugiperda cell line (Sf9) revealed that the Toll pathway activated the JNK via the TRAF6 (TNF receptor-associated factor 6) adapter. In addition, AcMNPV-induced the differential expression of several host-encoded microRNAs (miRNAs), with significant negative regulatory effects, on S. frugiperda antiviral immune genes. RNAi and miRNA-mimic mediated silencing of these genes resulted in increased AcMNPV proliferation. Our findings reinforce the potential of AcMNPV as a potent biocontrol agent and further our understanding of developing biotechnology-based targeted pest control agents.

Bacterial symbionts exhibiting co-evolutionary patterns with insect hosts play a vital role in the nutrient synthesis, metabolism, development, reproduction, and immunity of insects. The brown planthopper (BPH) has a strong ability to adapt to various environmental stresses and can develop resistance to broad-spectrum insecticides. We aimed to investigate whether gut symbionts of BPH play a major role in the detoxification of insecticides and host fitness in unfavorable environments. Nicotine-treated rice plants were exposed to BPH (early stage) and the gut microbiome of the emerging female adults were analyzed using high throughput sequencing (HTS). Nicotine administration altered the diversity and community structure of BPH symbionts with significant increases in bacterial members such as Microbacteriaceae, Comamondaceae, Enterobacteriaceae, and these changes may be associated with host survival strategies in adverse environments. Furthermore, the in-vitro study showed that four intestinal bacterial strains of BPH (Enterobacter NLB1, Bacillus cereus NL1, Ralstonia NLG26, and Delftia NLG11) could degrade nicotine when grown in a nicotine-containing medium, with the highest degradation (71%) observed in Delftia NLG11. RT-qPCR and ELISA analysis revealed an increased expression level of CYP6AY1 and P450 enzyme activities in Delftia NLG11, respectively. CYP6AY1 increased by 20% under the action of Delftia and nicotine, while P450 enzyme activity increased by 18.1%. After CYP6AY1 interference, nicotine tolerance decreased, and the mortality rate reached 76.65% on the first day and 100% on the third day. Moreover, Delftia NLG11 helped axenic BPHs to increase their survival rate when fed nicotine in the liquid-diet sac (LDS) feeding system. Compared with axenic BPHs, the survival rate improved by 25.11% on day 2% and 6.67% on day 3. These results revealed an altered gut microbiota and a cooperative relationship between Delftia NLG11 and CYP6AY1 in nicotine-treated BPH, suggesting that insects can adapt to a hostile environment by interacting with their symbionts and providing a new idea for integrated pest management strategies., Competing Interests: Declaration of Competing Interest The authors declare no conflicts of interest., (Copyright © 2023 The Authors.
