We compared the efficacy and safety of secukinumab in patients aged < 65 years (adult patients) versus patients aged ≥ 65 years (elderly patients) in a post-hoc analysis of the SUPREME study.Patients and Methods: Patients with moderate to severe plaque psoriasis received subcutaneous secukinumab 300 mg per week for the first 5 weeks, then 300 mg per month. We compared the following outcomes in patients aged ≥ 65 years vs < 65 years: baseline characteristics; PASI50/75/90/100 response rates (improvements ≥ 50%/75%/90%/100% in Psoriasis Area and Severity Index (PASI) from baseline); changes in Dermatology Life Quality Index (DLQI); Hospital Anxiety and Depression Scale (HAD-A, HAD-D) score changes; treatment-emergent adverse events (TEAEs).Results: Secukinumab was slightly less effective in elderly patients than in adult patients (response rates at week 16: PASI90, 69.4% vs 80.9%, p = 0.4528; PASI100, 44.4% vs 56.7%, p = 0.8973). Elderly and adult patients showed a similar time course of changes in absolute PASI scores. Patients aged ≥ 65 years had a statistically significantly lower improvement in quality of life (mean DLQI reduction) than patients aged < 65 years at week 16 [− 5.4 (± 4.3) vs − 8.8 (± 6.9), p = 0.0065] and at week 24 [− 5.3 (± 4.4) vs − 9.2 (± 7.1), p = 0.0038]. Secukinumab treatment resulted in comparable mean reductions in anxiety and depression scores in both cohorts at 24 weeks [HAD-A, − 1.3 (± 3.3) vs − 2.1 (± 3.8), p = 0.9004; HAD-D, − 1.0 (± 3.3) vs − 1.5 (± 3.1), p = 0.4598]. The frequency of TEAEs in the two cohorts was similar (16.7% vs 14.6%, p = 0.7391).Conclusion: Secukinumab is a valid option for the management of moderate to severe psoriasis in elderly patients.Keywords: psoriasis, biologics, interleukin-17A, secukinumab, elderly

In the current study, the authors qualitatively investigate, through the theoretical perspective of cognitive dissonance, the relationship between teacher beliefs and their associated teacher practices at two public elementary schools with diverse student populations. They argue that while teachers may hold theoretical beliefs about culturally proficient teaching, they may also hold deficit beliefs associated with pre-existing cognitions about the reasons for disparities in the academic outcomes of diverse student populations. Three research questions guided this study: (1) What beliefs (culturally proficient beliefs, deficit beliefs) do teachers hold about teaching students from diverse populations; (2) What are the classroom practices of teachers with diverse students; and (3) What is the relationship among culturally proficient beliefs, deficit beliefs, and teacher practices in the classroom? The authors present and discuss the findings in light of the relevant literature and cognitive dissonance theory. The study concludes with several recommendations for teacher practice and future research.

Several guidelines concerning AKs treatment have been published in the past years. Among destructive procedures, cryotherapy is today considered a standard first-line approach in case of single lesions. ABSTRACT In stream ecosystems, invertebrates are a principal source of prey for many fishes, but their abundance as a food source depends on the predator's ability to exploit them in time and space. Although salmonid fishes are commonly thought of as daytime drift‐feeding predators of invertebrates in streams, they exhibit great flexibility in foraging behaviour and may also consume prey from the benthos and at night. Char species, (Salvelinus sp.), in particular, may be better adapted to forage under low light conditions due to greater scotopic sensitivity than other salmonids. In this study, we used in situ or in place videography to quantify the foraging behaviour of bull trout (Salvelinus confluentus) and determine when and where they capture prey. We found that bull trout were primarily daytime predators of invertebrates drifting in the water column, and that they rarely captured prey from the benthos or foraged at night. We did observe size‐related differences and seasonal declines in foraging rate from summer to fall months. A modelled estimate of energy intake indicates that daytime drift‐feeding by small size classes of bull trout was above that needed for a maximum daily ration under a range of assumptions about prey size and prey retention rates. Energy intake rates for larger fish were more dependent on assumptions of preferred prey size and prey retention rates. Our data indicate that daytime measures of invertebrate drift abundance and size are the most inclusive measure of food abundance for stream‐dwelling bull trout. [ABSTRACT FROM AUTHOR]

The first assortment of achiral pentafluorosulfanylated cyclobutanes (SF5‐CBs) are now synthetically accessible through strain‐release functionalization of [1.1.0]bicyclobutanes (BCBs) using SF5Cl. Methods for both chloropentafluorosulfanylation and hydropentafluorosulfanylation of sulfone‐based BCBs are detailed herein, as well as proof‐of‐concept that the logic extends to tetrafluoro(aryl)sulfanylation, tetrafluoro(trifluoromethyl)sulfanylation, and three‐component pentafluorosulfanylation reactions. The methods presented enable isolation of both syn and anti isomers of SF5‐CBs, but we also demonstrate that this innate selectivity can be overridden in chloropentafluorosulfanylation; that is, an anti‐stereoselective variant of SF5Cl addition across sulfone‐based BCBs can be achieved by using inexpensive copper salt additives. Considering the SF5 group and CBs have been employed individually as nonclassical bioisosteres, structural aspects of these unique SF5‐CB "hybrid isosteres" were then contextualized using SC‐XRD. From a mechanistic standpoint, chloropentafluorosulfanylation ostensibly proceeds through a curious polarity mismatch addition of electrophilic SF5 radicals to the electrophilic sites of the BCBs. Upon examining carbonyl‐containing BCBs, we also observed rare instances whereby radical addition to the 1‐position of a BCB occurs. The nature of the key C(sp3)−SF5 bond formation step – among other mechanistic features of the methods we disclose – was investigated experimentally and with DFT calculations. Carcinosarcoma of the kidney and renal pelvis (CSKP) is a rare and highly-aggressive malignancy characterized by rapid progression and widespread metastases. To date, few studies describe the natural history of the disease. We present a patient placed on pembrolizumab therapy for suspected metastatic colon cancer. The patient was found to have a right renal mass with caval extension on surveillance and ultimately underwent radical surgery revealing carcinosarcoma with positive PD-L1 expression with no evidence of recurrence to date. To our knowledge, this is the first case describing PD-L1 expression in CSKP and presents a novel pathway for future treatment algorithms.

In this interview, Curry Malott and Derek R. Ford discuss their new book, "Marx, Capital, and Education: Towards a Critical Pedagogy of Becoming." The interview begins with a general inquiry into their intellectual backgrounds as scholar-activists and, importantly, comrades. The authors then discuss the process of collaborating together on the book itself and its content. In writing the book, the authors were "particularly interested in the role of education in revolution" and, thus, the role of comradeship for resisting the reproduction of capitalist social relations. In this regard, the book is an intervention linking Marxist educational theory with critical pedagogy for a revolutionary critical pedagogy of becoming that reclaims the radical roots of critical pedagogy. The authors contextualize revolutionary critical pedagogy with examples from the Ferguson rebellion in the United States. Lastly, Malott and Ford share their current and future research projects.

Some physiological concepts, such as physiology of filtration and absorption in the different nephron segments, are so detailed that they can be a challenge to be memorized. This article describes an exercise that solidifies learning as students manipulate, using paper models, "transporters" and "electrolytes" in the basolateral and luminal membranes of "nephron cells." Most important is the opportunity for the students to apply and test their own knowledge related to changes that occur in physiological processes, such as excess of salt or water intake or blood volume alterations, or pathology, such as syndrome of inappropriate antidiuretic hormone secretion, among many other applications. This nephron manipulative model is simple and inexpensive to implement. It may be particularly useful in educational settings with limited financial resources that seek to implement small- or large-group active learning sessions. The manipulative also gives instructors a great deal of flexibility in its use.

Genetic immunization is an attractive approach to generate antibodies because native proteins are expressed in vivo with normal posttranscriptional modifications, avoiding time-consuming and costly antigen isolation or synthesis. Hydrodynamic tail or limb vein delivery of naked plasmid DNA expression vectors was used to induce antigen-specific antibodies in mice, rats, and rabbits. Both methods allowed the efficient generation of high-titer, antigen-specific antibodies with an overall success rate of Western detectable antibodies of 78% and 92%, respectively. High-titer antibodies were typically present after 3 hydrodynamic tail vein plasmid DNA deliveries, 5 weeks after the initial injection (i.e., prime). For hydrodynamic limb vein plasmid DNA delivery, two deliveries were sufficient to induce high-titer antibody levels. Tail vein delivery was less successful at generating antibodies directed against secreted proteins as compared with limb vein delivery. Material for screening was generated by transfection of the immunization vector into mammalian cell lines. The cell line (COS-7) that produced the highest level of antigen expression performed best in Western blot analysis screens. In summary, intravenous delivery of antigen-expressing plasmid DNA vectors is an effective genetic immunization method for the induction of antigen-specific antibodies in small and large research animals.
