As the US federal government pursues immigration reform, changes to the federal public-charge rule have triggered confusion and concerns among patients who are immigrants. Although federal judges temporarily blocked implementation, a decision by the Supreme Court in January 2020 allowed the proposed changes to take effect. These policy changes have resulted in many legal immigrants and their family members becoming more reluctant to apply for health insurance, food, housing, and other benefits for which they are qualified. This article summarizes the changes and exclusions. Family physicians can effectively respond to patient and immigrant community concerns about these changes by providing outreach education, access to primary health care, and referrals to legal and social services.

Psychometric properties of the Levenson's Self-Report Psychopathy Scale (LSRP) are evaluated in incarcerated (n = 300) and non-incarcerated (n = 248) samples in the Mexican population. The models that have been reported in previous studies were tested by means of a confirmatory factor analysis (CFA). Brinkley's trifactorial model was the one presenting the best fit, whose indicators were similar to those reported in previous studies. Regarding reliability, a Cronbach's alpha =.766 and McDonald's omega =.761 was obtained for the total scale. The observed results provide theoretical and practical support on the dimensionality of the psychopathy construct. Implications for both research and assessment of psychopathy in correctional facilities and in the general population are discussed. [ABSTRACT FROM AUTHOR]

The objective of the present study is to analyze the differences in performance in conventional verbal (semantic and phonological) and emotional fluency tests (total, positive and negative) between among adolescents with and without probable conduct disorder. at the same time, to extract the emotional resilience index, the specific indexes according to the valence of the emotion (positive and negative) and to analyze the relationship with callousunemotional traits. Participation: a total of 242 adolescents, 102 with conduct disorder symptoms and 140 without conduct disorder. aged between 11 and 18 years (M=15.9; sd=1.75), male (62%) and female (38%). Results: no significant differences were observed between the groups. However, there is a positive relationship between phonological verbal fluency and conduct disorder and a negative relationship with callus-unemotional traits. positive relationship between semantic verbal fluency and conduct disorder. negative relationship between emotional verbal fluency and callus-unemotional traits and the unemotional dimension. positive emotional verbal fluency was negatively related to inexpressiveness, while negative emotional verbal fluency was negatively related to uncaring. Finally, the index of emotional resistance was positively related to conduct disorder and callous-unemotional traits, positive index to conduct disorder, and negative index to callous-unemotional traits and uncaring. the results suggest that emotional verbal fluency is differentially related to conduct disorder and, callousunemotional traits. the possible implications are discussed. Pro-inflammatory and coagulation factors serve as biomarkers of aging and functional reserve. The purpose of this study was to determine if pro-inflammatory (interleukin-6 [IL-6], C-reactive protein [CRP]), and coagulation (D-dimer) factors were associated with pre-chemotherapy functional status in women with stage I-III breast cancer. Patients and Methods. Prior to chemotherapy initiation in patients with stage I-III breast cancer, the following was captured: IL-6, CRP, D-dimer blood levels, and physical function measures including activities of daily living (ADL, subscale of Medical Outcomes Study Physical Health); instrumental activities of daily living (IADL, subscale of the Older Americans Resources and Services Program); Timed Up and Go (TUG); physician-rated Karnofsky Performance Status (KPS); and self-rated KPS. The association of these biomarkers with physical function measures was evaluated. Results. One hundred sixty patients (mean age 58.3 years, range 30-81 years) with stage I-III breast cancer (stages I [n = 34; 21.5%], II [n = 88; 55.7%], III [n = 36; 22.8%]) were enrolled. The group with poorest physical function (defined by ADL <70, IADL <14, and TUG ≥10 seconds) had higher levels of IL-6 (p = .05), D-dimer (p = .0004), and CRP (p = .05). There was no significant association between these biomarkers and KPS. Patients with at least two biomarkers in the highest quartile were more likely to have poorer physical function (odds ration [OR] 18.75, p < .001). In multivariate analysis adjusting for age, stage, number of comorbidities, and body mass index, the association remained (OR 14.6, p = .002). Conclusion. Objective In older men with prostate cancer, aging is associated with reduced anxiety and increased depression. The purpose of this study was to examine the association among age, anxiety, and depression in a cohort of older adults receiving chemotherapy. Methods This is a secondary analysis of a prospective longitudinal study investigating chemotherapy toxicity in older adults with cancer. Baseline data (pre-chemotherapy) included: age, sociodemographics, tumor and treatment factors, functional status, comorbidities, psychological state (measured by the Hospital Anxiety and Depression Scale), and social support. Univariate and multiple regression analyses were conducted to test the relationship between age, anxiety, and depression. Results The average age of the 500 patients (56% females) was 73.1. The majority had late stage disease: 22% Stage III and 61% stage IV. Clinically significant depression was reported in 12.6%. Clinically significant anxiety was reported in 20.9%. In univariate analyses, there was no association between anxiety and age, or depression and age. In multivariable analyses, older age ( p=0.05) was associated with decreased anxiety, as well as lack of social support ( p<0.01) and increased number of comorbidities ( p<0.01). In multivariable analysis, depression was associated with lack of social support ( p<0.01), increased number of comorbidities ( p<0.01), and advanced stage ( p<0.01). Conclusions This study supports previous research that anxiety decreases with age in older adults with cancer. However, depression remained constant with increasing age. Introduction: Buschke-Löwenstein tumor (TBL) or Giant Condyloma semimaligna verrucosa is a neoplasm with local invasion, slow growth, recurrent and tendency to form fistulas and abscesses. Despite its benign histology and low risk for metastasis is considered an intermediate lesion between condyloma acuminatum and squamous cell carcinoma. Case report: Female patient 12 years of age with perianal tumor seven months of development, accompanied by rectal bleeding and painful defecation two-month history Physical examination, foul smelling distance, abdomen was normal. In the perianal verrucous exophytic tumor, colifloriforme, bleeding, fetid, protruding to the genital region. Tumor culture with Candida species, E. Colli and Staphylococcus coagulase negative. Surgical resection was performed in two stages. The histopathological study reported condyloma acuminata intrepitelial hyperplasia without atypia. In a year of evolution without recurrence, with good functional and aesthetic development. Discussion: Buschke Löwenstein Tumor anorectal level represents 10% of cases. We report malignant transformation of TBL at 30% to 50%. In the pediatric age is an extremely rare tumor, only Attipou et al. reported the case of a patient of 14 years with a fatal descenlace. Because most of them were case reports there are no controlled studies to establish treatment guidelines. This article describes a 15-year roadmap for service-oriented multiagent system (MAS) research. The key MAS concepts are reflected directly in those of service-oriented computing: ontologies, process models, choreography, directories and facilitators and service-level agreements and quality-of-service. As of 2004, a sizable body of MAS research has not yet made many inroads into conventional system development: simulated evolution, stigmergic methods, consensus software for robustness, frameworks for describing and controlling societal-level computations and engineering tools and frameworks for managing MAS.
