ObjectiveThe neuroanatomical profile of behavioral variant frontotemporal dementia (bvFTD) suggests a common biological etiology of disease despite disparate pathologic causes; we investigated the genetic underpinnings of this selective regional vulnerability to identify new risk factors for bvFTD.MethodsWe used recently developed analytical techniques designed to address the limitations of genome-wide association studies to generate a protein interaction network of 63 bvFTD risk genes. We characterized this network using gene expression data from healthy and diseased human brain tissue, evaluating regional network expression patterns across the lifespan as well as the cell types and biological processes most affected in bvFTD.ResultsWe found that bvFTD network genes show enriched expression across the human lifespan in vulnerable neuronal populations, are implicated in cell signaling, cell cycle, immune function, and development, and are differentially expressed in pathologically confirmed frontotemporal lobar degeneration cases. Five of the genes highlighted by our differential expression analyses, BAIAP2, ERBB3, POU2F2, SMARCA2, and CDC37, appear to be novel bvFTD risk loci.ConclusionsOur findings suggest that the cumulative burden of common genetic variation in an interacting protein network expressed in specific brain regions across the lifespan may influence susceptibility to bvFTD.

Background: Unplanned hospital readmissions are serious medical adverse events, stressful to patients, and expensive for hospitals. This study aims to develop a probability calculator to predict unplanned readmissions (PURE) within 30-days after discharge from the department of Urology, and evaluate the respective diagnostic performance characteristics of the PURE probability calculator developed with machine learning (ML) algorithms comparing regression versus classification algorithms. Methods: Eight ML models (i.e. logistic regression, LASSO regression, RIDGE regression, decision tree, bagged trees, boosted trees, XGBoost trees, RandomForest) were trained on 5.323 unique patients with 52 different features, and evaluated on diagnostic performance of PURE within 30 days of discharge from the department of Urology. Results: Our main findings were that performances from classification to regression algorithms had good AUC scores (0.62–0.82), and classification algorithms showed a stronger overall performance as compared to models trained with regression algorithms. Tuning the best model, XGBoost, resulted in an accuracy of 0.83, sensitivity of 0.86, specificity of 0.57, AUC of 0.81, PPV of 0.95, and a NPV of 0.31. Conclusions: Classification models showed stronger performance than regression models with reliable prediction for patients with high probability of readmission, and should be considered as first choice. The tuned XGBoost model shows performance that indicates safe clinical appliance for discharge management in order to prevent an unplanned readmission at the department of Urology. Plain Language Summary: Study need and importance: Unplanned readmissions form a consistent problem for many hospitals. Unplanned readmission rates can go up as high as to 35%, and may differ significantly between respective hospital departments. In addition, in the field of Urology readmission rates can be greatly influenced by type of surgery performed and unplanned readmissions in patients can go up as high as 26%. Although predicting unplanned readmissions for individual patients is often complex, due to multiple factors that need to be taken into account (e.g. functional disability, poor overall condition), there is evidence that these can be prevented when discharge management is evaluated with an objective measuring tool that facilitate such risk stratification between high and low risk patients. However, to the best of our knowledge, the latter risk stratification using ML driven probability calculators in the field of Urology have not been evaluated to date. Using ML, calculated risk scores based on analysing complex data patterns on patient level can support safe discharge and inform concerning the risk of having an unplanned readmission. What we found: Eight ML models were trained on 5.323 unique patients with 52 different features, and evaluated on diagnostic performance. Classification models showed stronger performance than regression models with reliable prediction for patients with high probability of readmission, and should be considered as first choice. The tuned XGBoost model shows performance that indicates safe clinical appliance for discharge management in order to prevent an unplanned readmission at the department of Urology. Limitations of our study were the quality and presence of patient data on features, and how to implement these findings in clinical setting to transition from predicting to preventing unplanned readmissions. - Abstract
Sparsely populated areas tend to be poorly served by Fixed Line and Schedule (FLS) public transport systems as the operation of a regular bus line is not economically viable for such areas. Therefore, introducing a Demand Responsive Transport (DRT) to partially replace FLS can result in increasing mobility service accessibility and inclusion. In this paper, a mixed-integer linear problem (MILP) is proposed to design an integrated FLS and DRT network for a transport operator. Passengers behavior is implicitly incorporated in our proposed approach via a discrete choice model. In addition, a tailored Adaptive Large Neighborhood Search (ALNS) coupled with tabu search and simulated annealing are introduced. We test our algorithm on real instances from a public transport operator in the Netherlands. The proposed algorithm can solve the problem up to 170 times faster than the MILP within 4% to 10% gap. Our proposed resolution approach investigates the temporal and spatial feasibility of deploying these integrated mobility systems based on the service level and provides recommendations to public transport operators., Transport and Planning

- Abstract
The analysis of the results of the use of whole-body hyperthermia (WBH) in patients with locally advanced, recurrent and / or metastatic tumors, which, as a rule, are radio- and chemo-resistant. Most patients were incurable, in connection with which the treatment was mainly carried out with a palliative purpose. The analysis showed that the method alone or in combination with chemo- and radiation therapy has pronounced direct and palliative effects, in some cases it provides long-term control. However, WBH can have side effects. This necessitates further improvement of the method, development of a set of preventive measures, as well as improvement of equipment for heating and thermometry.

Here we describe the LifeTime Initiative, which aims to track, understand and target human cells during the onset and progression of complex diseases, and to analyse their response to therapy at single-cell resolution. This mission will be implemented through the development, integration and application of single-cell multi-omics and imaging, artificial intelligence and patient-derived experimental disease models during the progression from health to disease. The analysis of large molecular and clinical datasets will identify molecular mechanisms, create predictive computational models of disease progression, and reveal new drug targets and therapies. The timely detection and interception of disease embedded in an ethical and patient-centred vision will be achieved through interactions across academia, hospitals, patient associations, health data management systems and industry. The application of this strategy to key medical challenges in cancer, neurological and neuropsychiatric disorders, and infectious, chronic inflammatory and cardiovascular diseases at the single-cell level will usher in cell-based interceptive medicine in Europe over the next decade., The LifeTime initiative is an ambitious, multidisciplinary programme that aims to improve healthcare by tracking individual human cells during disease processes and responses to treatment in order to develop and implement cell-based interceptive medicine in Europe.

The review examines the general principles of capacitive electromagnetic hyperthermia (EMHT), the distribution of electromagnetic energy in various experimental models and in patients’ tumors, the design features of applicators from various capacitive hyperthermic systems and their role in achieving hyperthermic mode in tumors of deep localization. In classical capacitive EMHT, the main obstacle in achieving the required temperature in such tumors is overheating of the subcutaneous fatty tissue under the electrodes. For some capacitive hyperthermic systems, the heating of adipose tissues is enhanced due to the fact that the applicator design does not conform to certain technical requirements. In capacitive EMHT at frequencies of 8–13.56 MHz, obtaining the minimum hyperthermic mode is possible with output powers of 500–800 W, maximum – 1000–1200 W and above. The results of the use of various hyperthermic capacitive systems in patients with malignant tumors of internal organs are analyzed.

- Abstract
The classification of podocytopathies, including minimal change disease (MCD) and focal segmental glomerulosclerosis (FSGS), has historically been based on limited glomerular features. This study used supervised machine learning methods to identify the most important clinical and histopathologic predictors of disease progression, complete proteinuria remission, and treatment response in MCD/FSGS. The top predictors included conventional and novel glomerular and tubulointerstitial features. Biopsy reporting for podocytopathies should be standardized by including these prognostic morphologic features to inform risk stratification.

Discovery of disease age-of-onset modifiers is important for clinical trials and drug design. Zhang et al. perform a genome-wide analysis of epigenetic functional polymorphisms and identify an association between the C6orf10/LOC101929163 locus and age of FTD/ALS onset. The risk allele may be associated with a pro-inflammatory state in the brain., The G4C2-repeat expansion in C9orf72 is the most common known cause of amyotrophic lateral sclerosis and frontotemporal dementia. The high phenotypic heterogeneity of C9orf72 patients includes a wide range in age of onset, modifiers of which are largely unknown. Age of onset could be influenced by environmental and genetic factors both of which may trigger DNA methylation changes at CpG sites. We tested the hypothesis that age of onset in C9orf72 patients is associated with some common single nucleotide polymorphisms causing a gain or loss of CpG sites and thus resulting in DNA methylation alterations. Combined analyses of epigenetic and genetic data have the advantage of detecting functional variants with reduced likelihood of false negative results due to excessive correction for multiple testing in genome-wide association studies. First, we estimated the association between age of onset in C9orf72 patients (n = 46) and the DNA methylation levels at all 7603 CpG sites available on the 450 k BeadChip that are mapped to common single nucleotide polymorphisms. This was followed by a genetic association study of the discovery (n = 144) and replication (n = 187) C9orf72 cohorts. We found that age of onset was reproducibly associated with polymorphisms within a 124.7 kb linkage disequilibrium block tagged by top-significant variation, rs9357140, and containing two overlapping genes (LOC101929163 and C6orf10). A meta-analysis of all 331 C9orf72 carriers revealed that every A-allele of rs9357140 reduced hazard by 30% (P = 0.0002); and the median age of onset in AA-carriers was 6 years later than GG-carriers. In addition, we investigated a cohort of C9orf72 negative patients (n = 2634) affected by frontotemporal dementia and/or amyotrophic lateral sclerosis; and also found that the AA-genotype of rs9357140 was associated with a later age of onset (adjusted P = 0.007 for recessive model). Phenotype analyses detected significant association only in the largest subgroup of patients with frontotemporal dementia (n = 2142, adjusted P = 0.01 for recessive model). Gene expression studies of frontal cortex tissues from 25 autopsy cases affected by amyotrophic lateral sclerosis revealed that the G-allele of rs9357140 is associated with increased brain expression of LOC101929163 (a non-coding RNA) and HLA-DRB1 (involved in initiating immune responses), while the A-allele is associated with their reduced expression. Our findings suggest that carriers of the rs9357140 GG-genotype (linked to an earlier age of onset) might be more prone to be in a pro-inflammatory state (e.g. by microglia) than AA-carriers. Further, investigating the functional links within the C6orf10/LOC101929163/HLA-DRB1 pathway will be critical to better define age-dependent pathogenesis of frontotemporal dementia and amyotrophic lateral sclerosis.
