Hem saray hem de saray dışında yaşayan Osmanlı kadınlarının yaşam tarzıher daim herkesin merakını celbeden bir konu olmuştur, olmaya da devam etmektedir. Bu çalışmada da 19.yüzyıl (1825-1850) Osmanlı kadınının gündelik yaşam tarzı özellikle kıyafetler üzerinden kısmî olarak ortaya çıkartılmaya çalışıldı. Bu çerçevede beşi Müslüman seksen dokuz kadın terekesi incelendi. Terekeler kadın kıyafetleri özelindeincelenirken kıyafetlerden mütevellid giysilerde kullanılan kumaş türleri de kısmen ele alındı. Toplum içinde aktif şekilde varolan kadınların içerideki (ev) ve dışarıdaki (sokakta) halleri zaman zaman klasik ve modern dönem karşılaştırması yapılarak ortaya konuldu. Kıyafetler özelindekadının toplum içindeki varoluş şekillerinin, şekilsel biçimlerinin ve dolayısıyla görüntülerinin gün yüzüne çıkartılması amaçlandı. Tespitlerimiz terekelerin daha çok gayrimüslimlere ve dahi yönetici sınıf ve zengin kesim dışındakilere ait olmasından dolayı halk içindeki gayrimüslim kadınlara dair oldu. Tüm bunlar İstanbul Şerʻiye Sicilleri Arşivi’ndeki Mülga Beledî Kassamlığı esas alınarak yapıldı.

AimTo evaluate the clinical properties and treatment results of patients with glomus tumor which is one of the rare tumors of the hand originating from the neuromyoarterial glomus body.Materials and MethodsBetween years 2012-2014, a total of 9 patients (6 women and 3 men) diagnosed with glomus tumor of the hand were included in this retrospective study. Age, sex, localization of the lesion, symptoms and the initiation time of the symptoms, time of first clinical evaluation and diagnosis, radiological imaging methods, the treatment applied, pathological diagnosis, follow-up results and the presence of complications were evaluated for each patient.ResultsThe mean age of the patients were 39.6 years. Glomus tumor was observed to be subungual in all patients. Two of the lesions were in the thumb, 3 were in the index finger, 3 were in the middle finger, and 1 was in the fourth finger. The common complaint in all patients was pain in the finger and the nail and tenderness. Cold intolerance was prominent in 4 patients. There was blue discoloration in 2 patients and protuberance and deformation of the nail was observed in 3 patients. The average time between the initiation of the symptoms and the diagnosis was 10.7 months. No erosion in the bony segment of the distal phalanx was present in plain radiography except 1 patient. The masses were observed in the Magnetic Resonance imaging. In 8 of the 9 patients the diagnosis was made clinically and radiologically, and only one patient was diagnosed clinically. All patients received surgical treatment. No recurrence was observed in the 12 month follow-up period. Minimal nail deformities were observed in 4 patients postoperatively.ConclusionIn cases of severe pain localized in the finger, especially fingertip, of unknown cause, glomus tumor should be suspected as a prediagnosis. MRI is the most important step in the diagnosis of the mass.

Fournier gangrene is a progressive clinical picture of necrotizing fasciitis with a high rate of morbidity and mortality, involving external genital organs and the perineal region, and may rapidly spread to other areas such as the abdomen, lower extremities and chest. It is a polymicrobial infection caused most frequently by aerobic and anaerobic bacteria. Treatment is achieved through the removal of dead tissues by serial debridement and broad spectrum parenteral antibiotic administration. In this case report, we present a successful penis reconstruction achieved by pedicled inguinal flap and urethra reconstruction in a patient with approximately 6x3 cm wide full-thickness skin and urethra defect secondary to Fournier gangrene..

Tendon sheath fibromas are slowly growing, firm, immobile, and painless masses that are frequently seen in fingers. They are commonly observed in middle-aged males. In our report, we discuss a case of a painless mass in the palmar area that was diagnosed as a pleomorphic fibroma of tendon sheath on histopathologic examination in light of the literature.

AimCutaneous malignancies often develop in traumatized, degenerative, and chronically inflamed skin or scar tissues. The observations indicated that cutaneous malignancies often arise in burn scars (Marjolin's ulcer), venous ulcers, compression wounds, cystostomy sites, chronic pilonidal sinus, hidradenitis suppurativa, lupus scars, chronic lymphedema, traumatic wounds, and sites of chronic osteomyelitis. The current study presents a differential diagnosis and treatment approach in patients who received wound care and treatment due to the presence of chronic wounds and in whom biopsy revealed malignancy.Material and MethodsFive patients who presented to the clinic due to chronic wounds between 2008 and 2012 and in whom biopsy revealed carcinomas were included in the study. The patients were retrospectively reviewed in terms of previously administered therapies, pathological diagnosis, predisposing factors, time to malignant degeneration, reconstruction techniques, complications, and recurrence.ResultsThe pathological examination revealed verrucous carcinoma in three patients and squamous cell carcinoma in two patients. The mean of malignant degeneration time was 8 years (5- 12).ConclusionChronic skin irritation and exposure of the soft tissues to different growth factors, and cytotoxic factors arising due to inflammation are thought to play an important role in malignant transformation. Providing good wound care, avoiding traumas, and obtaining biopsy from non-healing wounds to establish early diagnosis are very important in patients with chronic wounds. Squamous cell carcinoma was the most commonly encountered histological tumor type. Moreover, it must also be remembered that cutaneous malignancies can develop in patients with sustained open wounds and risk factors.

Aim: In this study, it was aimed to investigate the epidemiological and localization characteristics of the patients diagnosed with nonmelanocytic skin cancer (NMSC). Materials and Methods: Histopathologic results of patients diagnosed with NMSC in the Pathology Department of Necmettin Erbakan University, Meram School of Medicine, between 2007 and 2017 were retrospectively reviewed. The age, sex, tumor type, and localization of the patients were investigated. Results: A totals of 400 patients diagnosed with NMSC were identified. 220 of the patients were male (55%) and 180 were female (45%). The male to female ratio was 220/180 (1.22). The age range was 10–105-year-old and the mean age was 67.8. Basal cell carcinoma (BCC) was found in 263 patients (65%), squamous cell carcinoma (SCC) in 114 patients (28%) and baso SCC (BSCC) in 12 patients (3%). BCC and SCC were observed together in 9 patients. BCC was most commonly detected in the nasal region with (31%) 82 patients (50%). SHC was most commonly detected in the lower lip region with 26 patients (22%). The localization of the BSCC was most common in the nasal region with 6 patients (%50). The rate of BCC/SCC in the nasal region was 82/11. The BCC/SCC ratio in the extremity region was 4/14. Conclusion: In our region, the most frequent histopathologic diagnosis was found BCC in patients prediagnosed with NMSC, and most cases were seen in male patients. In cases of BCC a BSCC, the most common tumor localization was nose area while it was lower lip area in SCC cases. In addition, BCC and BSCC were not detected in the lower lip.

Objective: Different treatment principles have been applied in the reconstruction of partial or full layer defects of the lower eyelid. The use of the most similar tissue for eyelid reconstruction is important for both functional and esthetic results. This study aims to investigate the reconstruction methods performed in lower eyelid defects and to evaluate their esthetic and functional results. Patients and Methods: In this study, patients who underwent reconstructive surgery from 2012 to 2016 in our clinic were investigated. Cases of primary repairs after skin tumors located in the lower eyelids were excluded from the study. The sociodemographic characteristics of patients, the type and location of the tumor, defect size after surgery, anterior and posterior lamellar defects, and reconstruction methods used were retrospectively reviewed. Results: Thirty-seven patients were included in the study. Fifteen were male and 22 were female. There was only anterior lamellar defect in 29 patients and full-thickness lower eyelid defect in 8 patients. Anterior flaps used in lamellar defects were identified as glabellar flap, Limberg flap, advancement, transposition flap, nasolabial flap, forehead flap, and cheek flap. Chondromucosal graft, palatal mucosal graft, and buccal mucosal graft were used for repairing posterior lamellar defects. Conclusion: Separate reconstruction of the posterior and anterior lamellae is important to provide good functional and esthetic results in lower eyelid reconstruction. Depending on the size of the defect, using a single local flap or a combined flap with posterior lamella repair provides highly acceptable results.

The availability of local tissue in distal 1/3 of the leg and around the ankle is limited, and the difficulty in re-establishing soft-tissue coverage for these defects continues to challenge surgical procedures. In this study, two cases who had the soft-tissue defects around the lateral malleol reconstructed with the distally pedicled peroneus brevis muscle flap were discussed. The first case was a 30-year-old male patient, who had 4 cm × 2 cm soft-tissue defect with bony exposure, inferior to the lateral malleol after a gunshot injury. The second case was a 26-year-old male patient who had 5 cm × 3 cm soft-tissue defect with exposed metal implant, distal to the lateral malleolus after a traffic accident. Both of the patients were quite satisfied with the esthetic results. The distal pedicled peroneus brevis muscle flap should be kept in mind as a reliable and relatively easy flap option for the reconstruction of the soft-tissue defects around the lateral malleol with good results.

Background Although the use of temporary shunts in proximal extremity amputations has been reported, no study has described the use of temporary shunts in distal extremity amputations that require vein grafting. Moreover, the total volume of blood loss when temporary shunts are used has not been reported. The aim of this study was to investigate the applicability of a temporary shunt for distal extremity amputations requiring repair by vessel grafting with an ischemia time of >6 hours. This study also aimed to determine the total volume of blood loss when temporary shunts were used. Methods Patients who underwent distal major extremity replantation and/or revascularization with a vessel graft and who experienced ischemia for 6–8 hours between 2013 and 2014 were included in the study. A 6-Fr suction catheter was cut to 5 cm in length after the infusion of heparin, and secured with a 5-0 silk suture between the distal and the proximal ends of the artery. While bleeding continued, the bones were shortened and fixed. After the complete restoration of circulation, the arterial shunt created using the catheter was also repaired with a vein graft. Results Six patients were included in this study. The mean duration of ischemia was 7.25 hours. The mean duration of suction catheter use during limb revascularization was 7 minutes. The mean transfusion volume was 7.5 units. No losses of the extremity were observed. Conclusions This procedure should be considered in distal extremity amputations requiring repair by vessel grafting during critical ischemia.

The Viperidae family is responsible for most of the venomous snakebites in Anatolia. Toxicity symptoms may be local such as edema, hyperemia, blisters, necrosis, lymphadenopathy, and ecchymosis or hematological, including high white blood cell (WBC), neutrophilia, decrease in hemoglobin, thrombocytopenia, international normalized ratio (INR) elongation, and increase in creatine kinase (CK), tumor necrosis factor alpha (TNF and #945;), and lactate dehydrogenase (LDH). Antivenom thera- py should be provided if hyperemia continues and/or edema does not regress. Complications are very low in envenomation cases related to Viperidae bites with proper treatment and follow-up. Otherwise, complications can progress to necrosis, acute renal failure, compartment syndrome, amputation, and death may be seen. However, no report in the literature has described osteomyelitis as a late complication of snakebite. In this study, we present an osteomyelitis case caused by snakebite culminating in amputation because of the inadequate treatment. [Arch Clin Exp Surg 2016; 5(3.000): 180-183]

BACKGROUND AND OBJECTIVE: Reconstructing defects related to the leg soft tissue may be quite difficult because the soft tissue over the bone is rather thin and the tendons lie right under the skin. Distal pedicle sural neurocutaneous flap with its long pedicle composed of skin, subcutaneous tissue and fascia is suitable for superficial defects. This study aimed to determine the usability of the delayed reverse-flow (distally based) islanded sural flap for correcting tibial and ankle defects. DESIGN AND SETTINGS: A prospective clinical trial conducted from 2012 to 2013 in Plastic & Reconstructive and Aesthetic Surgery clinic, Necmettin Erbakan University, Turkey. METHOD: Eleven patients with pretibial defects and a visible open bone who underwent reconstruction with reverse-flow islanded sural flap between 2012 and 2013 were included in the study. All patients who had defects between the middle of the tibia and the foot underwent surgery in 2 sessions under spinal anesthesia. In the first session, necrotic tissues were debrided and cultures were taken, and the flap was delayed. Reconstruction was performed in the second session. RESULTS: The biggest flap was 16×11 cm and the smallest one was 5×6 cm. The longest pedicle was 27 cm long and the shortest one 21 cm. A total of 6 patients were smokers and 3 had diabetes mellitus. One patient had partial necrosis of the flap, and the necrosis was healed secondarily. No complications were seen in other patients. CONCLUSION: Delayed reverse-flow islanded sural flaps can be used as an easy, quick, and secure surgical alternative to free flaps for correcting in leg defects involving an exposed bone between the middle portion of the tibia and the heel.
