The receiver operator characteristic curve (ROC) was used to calculate the optimal cut-off values of parameters such as NLR. The Kaplan-Meier curve and Cox proportional risk model were used to evaluate the effects of NLR and other variables on 2-year all-cause mortality.Results: The area under the ROC for NLR was 0.634, with an optimal cutoff value of 4.9. Two-year mortality rates for patients with high (≥ 4.9) and low (< 4.9) NLR were 22.1% and 9.3%, respectively (Log rank test: P = 0.002). After correcting for confounders, multivariate analysis demonstrated that NLR ≥ 4.9 (HR = 2.741, 95% CI 1.467– 5.121, P = 0.002), age ≥ 63 (HR = 3.403, 95% CI 1.835– 6.310, P < 0.001), and gender (male) (HR = 2.842, 95% CI 1.366– 5.912, P = 0.001) were independent risk factors for the mortality outcome. Considering the stratification of early and selective TIPS treatment, high NLR still significantly increased the risk of mortality for patients (Log rank test: P = 0.007, HR = 2.317, 95% CI 1.232– 4.356).Conclusion: NLR can help to predict survival in EGVB patients after TIPS, and the type of TIPS should also be considered in practical applications.Keywords: neutrophil-to-lymphocyte ratio, transjugular intrahepatic portosystemic shunt, esophagogastric variceal bleeding, prognostic factor

Previous studies have reported some associations between sarcopenia and depressive symptoms. Recently, intracellular water (ICW) and total body water (TBW) have been found to reflect muscle function and muscle mass. ICW/TBW ratio is a marker of sarcopenia that is simple to assess. However, the relationship between ICW/TBW ratio and depression has not been explored in MHD patients.Methods: In our cross-sectional and multi-center study, 3300 adult MHD patients were included from June 1, 2021, to August 30, 2021. Depressive symptoms were evaluated using the Beck Depression Inventory-II (BDI-II). TBW and ICW were measured by Body Composition Monitor (BCM). Multivariable logistic regression, stratified analyses, and interactive analyses were conducted to assess the relationship between ICW/TBW ratio and depression.Results: About 16.5% of the 3300 MHD patients were found to have depressive symptoms. The prevalence of depression increased with decreasing quartiles of ICW/TBW ratios, and decreased ICW/TBW ratio was independently associated with depression after adjusting for potential confounders. Patients in Quartile 1 of ICW/TBW ratios were more likely to have depressive symptoms (odds ratio 1.55, 95% confidence interval 1.07– 2.22; p=0.002) than those in Quartile 4. History of diabetes and education status had interactive roles in the relationship between depression and ICW/TBW ratios (p < 0.05). The association of ICW/TBW ratios and depression existed in patients of both genders and different education levels, but only in non-diabetic patients.Conclusion: In MHD patients, the decreased ratio of ICW/TBW was independently related to high depression rates.Keywords: depression, hemodialysis, intracellular water, total body water

Here, using a high-throughput screening approach, we investigated the anti-necroptosis effects of piperlongumine, an alkaloid isolated from the long pepper plant, in vitro and in a mouse model of systemic inflammatory response syndrome (SIRS).Methods: A natural compound library was screened for anti-necroptosis effects in cellular. The underlying mechanism of action of the top candidate piperlongumine was explored by quantifying the necroptosis marker phosphorylated receptor-interacting protein kinase 1 (p-RIPK1) by Western blotting. The anti-inflammatory effect of piperlongumine was assessed in a tumor necrosis factor α (TNFα)-induced SIRS model in mice.Results: Among the compounds investigated, piperlongumine significantly rescued cell viability. The half maximal effective concentration (EC50) of piperlongumine for inhibiting necroptosis was 0.47 μM in HT-29 cells, 6.41 μM in FADD-deficient Jurkat cells, and 2.33 μM in CCRF-CEM cells, while the half maximal inhibitory concentration (IC50) was 95.4 μM in HT-29 cells, 93.02 μM in FADD-deficient Jurkat cells, and 161.1 μM in CCRF-CEM cells. Piperlongumine also significantly inhibited TNFα-induced intracellular RIPK1 Ser166 phosphorylation in cell lines and significantly prevented decreases in body temperature and improved survival in SIRS mice.Conclusion: As a potent necroptosis inhibitor, piperlongumine prevents phosphorylation of RIPK1 at its activation residue Ser166. Piperlongumine thus potently inhibits necroptosis at concentrations safe enough for human cells in vitro and inhibits TNFα-induced SIRS in mice. Piperlongumine has potential clinical translational value for the treatment of the spectrum of diseases associated with necroptosis, including SIRS.Keywords: piperlongumine, natural product, drug repositioning, necroptosis, RIPK1

This study was to explore the role of SNHG10 in AML.Methods: The expression of SNHG10 and miR-621 in BM mononuclear cells (BMMNCs) isolated from 60 AML patients and 60 healthy controls were determined by RT-qPCR. Correlation between SNHG10 and miR-621 was analyzed by Pearson correlation coefficient. The overexpression of SNHG10 and miR-621 in transfected AML cells was detected by RT-qPCR, and the regulatory relationship between them was explored. Methylation-specific PCR (MSP) was applied to detect the methylation of miR-621 induced by the overexpression of SNHG10. CCK-8 assay was conducted to evaluate cell proliferation.Results: In this study, we found that the expression of SNHG10 was upregulated and the expression of miR-621 was downregulated in AML samples. Moreover, SNHG10 and miR-621 were inversely correlated across AML samples, and a high level of SNHG10 predicted poor survival of AML patients. Bioinformatics analysis showed that SNHG10 could be targeted by miR-621. In AML cells, miR-621 overexpression downregulated the expression of SNHG10, while SNHG10 overexpression could not affect the expression of miR-621. However, it was found that the reduction of miR-621 caused by SNHG10 overexpression might be due to the increase of miR-621 methylation. In addition, SNHG10 overexpression significantly promoted BMMNC proliferation, whereas miR-621 overexpression inhibited BMMNC proliferation and abolished the effect of SNHG10 overexpression on BMMNC proliferation.Conclusion: miR-621 targets SNHG10 to suppress cell proliferation in AML.Keywords: acute myeloid leukemia, miR-621, SNHG10, proliferation

Then, we calculated Pearson’s correlation coefficients between the resting-state time courses of each seed and the time series of the rest of the brain. Lastly, we computed correlations between the altered zALFF, zReHo, and functional connectivity with Japanese Orthopaedic Association scores, Neck Disability Index score, and the duration of symptoms in patients with CSM.Results: zALFF and zReHo values were increased in the left medial superior frontal gyrus (lSFGmed) and left supramarginal gyrus (lSMG) in patients with CSM compared with those in the HC group. Selecting lSFGmed as the seed, we observed increased functional connectivity between it and the left postcentral gyrus (lPoCG) and left rolandic operculum and decreased functional connectivity with the right medial superior frontal gyrus in patients with CSM. In addition, there was a significant increase in the functional connectivity between the lSMG (seed) and the left calcarine and lPoCG in patients with CSM. However, we did not find any significant correlation between the resting-state findings and the clinical performance of patients with CSM.Conclusion: These observed intrinsic functional changes in the patients with CSM may be related to functional reorganization and reflect the innate cortical plasticity in patients with CSM. Notably, the increased connectivity between the lPoCG and the two seed ROIs indicates the adaptive changes in patients with CSM. After 8 weeks, the type 2 diabetes nephropathy model was successfully set up the different drugs were administrated to mice with diabetes (insulin 1–2 U/day, benazepril 10 mg/kg per day intragastrically, IGF-1R inhibitor 30 mg/kg per day intragastrically). After 8 weeks drugs administration, all mice were collected the kidney tissue, measured levels of inflammatory factor (F4/80, TLR4and CD68) and fibrosis markers(αSMA, E-cadherin and SR) using immunohistochemistry and in situ hybridization.Results: The type 2 diabetes nephropathy model was built successfully, which along with increased urinary protein excretion rate and increased inflammatory infiltration, and the correlation was characterized by increased CD68+, F4/80+ cells and increased TLR4, αSMA, SR expression. IGF-1R inhibitors reversed this changes, but benazepril and insulin were without significant changes. The insulin decreased the expression level of IGF-1, and increased the levels of suppressor of cytokine signaling 2 (SOCS2). Benazepril and IGF-1R inhibitor were no significant changes like insulin.Conclusion: Inhibition of IGF1R was a more effective choice for inflammation treatment than Ben or Ins in diabetic kidney disease (DKD). The IGF1R inhibitor blocked pathological changes induced by the over-expression of IGF1 in DKD without up-regulating SOCS2 protein levels. This study evaluated the concordance and feasibility of using various creatinine-based equations for estimating GFR in elderly Chinese patients with type 2 diabetes mellitus (T2DM).Methods: A cross-sectional analytical study was conducted in 21,723 older diabetic patients (≥60 years) based on electronic health records (EHR) for Minhang District, Shanghai, China. The concordance of chronic kidney disease (CKD) classification among different creatinine-based equations was assessed based on Kappa values, intraclass correlation coefficient (ICC) statistics, and the eGFR agreement between the equations was tested using Bland–Altman plots. The GFR was estimated using the Cockcroft–Gault (CG), Berlin Initiative Study 1 (BIS1), simplified Modification of Diet in Renal Disease (MDRD), MDRD modified for Chinese populations (mMDRD), chronic kidney disease epidemiology collaboration (CKD-EPI), CKD-EPI in Asians (CKD-EPI-Asia), and Ruijin equations.Results: Overall, the proportion of CKD stages 3–5 (eGFR

ObjectiveTo investigate the mutation and genetic evolution of drug resistance gene of A（H1N1） pdm09 influenza pandemic strain in 2023 in Huzhou City， Zhejiang Province.MethodsRespiratory tract specimens from 2 influenza monitoring hospitals were collected for A（H1N1） pdm09 influenza virus nucleic acid detection. Positive specimens were inoculated with MDCK cells for influenza virus isolation and sequencing. DNA Star 7.1 software and Mega 4.0 software were used to analyze the neuraminidase （NA） enzyme active site and the amino acid sites related to drug resistance in M2 protein.ResultsNucleotide homology and amino acid homology of NA between the isolated and the vaccine strains were 98.87%‒99.22% and 98.94%‒99.36%， respectively. The nucleotide homology range of M gene was 99.07% to 99.85%， and the amino acid homology range was 99.02%‒99.94%. The isolates and vaccine strains belong to the evolutionary clades of 6B.1A.5a.2a. The amino acids at the key sites of the enzyme activity center of NA were still highly conserved， and the 9 key amino acid sites related to NA inhibitor resistance did not change， but some mutations occurred at the non-enzyme active sites in some popular strains. The 5 amino acid sites related to drug resistance of M2 protein were not replaced， but the 31st amino acid sites changed from serine to asparagine.ConclusionThe A（H1N1） pdm09 pandemic strain in Huzhou in 2023 has high homology with the 2023‒2024 vaccine strain recommended by WHO. All endemic strains are resistant to amantadines.

Objective: To evaluate the efficacy of using additional locking plate-derived Poller screws to treat post-intramedullary nailing for femoral non-union was investigated. Methods: Between January 2009 and April 2012, six patients who underwent post-intramedullary nailing for femoral non-union were studied. Three, one, and two patients had femoral fractures in the proximal one-third, middle third and distal third, respectively. While the original intramedullary nails were retained, eight-to-nine hole locking plates were used for fixation and two-to-three cortical bone screws were applied to both sides of the fracture to ensure the stability of the intramedullary nail sagittal plane. One-to-two pieces of locking nails were inserted tightly next to the intramedullary nails to ensure a stable coronal plane. Autologous iliac bone grafts were performed around the fractures in all cases. Results: Follow-up evaluations were conducted between 10 and 17 months (mean, 13.8 months). The operative time was 110-160 minutes and the blood loss was 300-500 mL. Bone pain was relieved in 1 month. Continuous callus was observed after 4-6 months (mean, 4.83 months) based on imaging. There were no infections, loosening of internal fixation, or rupture. All patients were able to walk bearing weight within 3 months. Conclusion: An additional locking plate and the derived Poller screw technique effectively improved local rotation instability and is an effective and simple treatment method for femoral non-union after intramedullary nailing. 【Background and Objective】 Irrigation in the Hetao irrigation district relies on water diversion from the Yellow River. However, its flat terrain coupled with suboptimal irrigation and drainage practices over the past decades has resulted in groundwater table rise, leading to significant soil salinization. This paper develops a water balance model to calculate groundwater recharge and drainage in the irrigation district. 【Method】 A comprehensive water balance model was established by accounting for seasonal freezing and thawing, as well as groundwater drainage. It was then applied to the Yichang area in the irrigation district. Sensitivity of the model was analyzed using the Latin Hypercube-One-At-a-Time (LH-OAT) method; parameters in the model were estimated using the ensemble square-root method of data assimilation (ES-MDA). 【Result】 Sensitivity analysis reveals that change in groundwater depth is sensitive to parameters including the critical depth of phreatic groundwater table below which groundwater evaporation ceases, coefficient of phreatic groundwater evaporation and coefficient of irrigation water recharge. The model accurately reproduces the variation in groundwater depth and groundwater drainage in the studied area. Groundwater recharge primarily emanates from irrigation water infiltration, while drainage is predominantly through groundwater evaporation. The analysis identifies seasonal fluctuations in groundwater storage, which is influenced by factors such as irrigation water diversion, groundwater depth and groundwater drainage rate. 【Conclusion】 The proposed water balance model accounts for freezing and thawing, as well as groundwater discharge. Practical application shows the model is robust for modelling groundwater dynamics in the Yichang area in the Hetao irrigation district. Monthly change in groundwater storage is influenced intractably by irrigation practices, groundwater dynamics and climatic condition.

Deep learning based reconstruction (DLR) technology is currently one of the most cutting-edge technological advancements in the field of MRI image reconstruction. Compared to conventional MRI image reconstruction techniques, DLR technology redefines a new boundary between signal-to-noise ratio, spatial resolution, and scanning time on MRI. Its outstanding technical advantage is the effective removal of image noise and artifacts, significantly reducing scanning time, and also has potential advantages in improving the detection rate and accuracy qualitative diagnosis of lesions. With the continuous optimization of algorithms and the improvement of model generalization, DLR has been widely used in MRI examinations for multiple parts, such as the nervous system, musculoskeletal system, and heart. Its applicable scanning sequences and clinical application scenarios are also constantly expanding. DLR technology, while maintaining the original spatial resolution, reduces the number of signal acquisition times and increases the parallel acquisition acceleration factor to shorten the imaging time by more than 50%, achieving rapid imaging of the musculoskeletal system, and obtaining significantly better image quality than traditional reconstructed images. Currently, DLR is widely used in MRI exa-minations of musculoskeletal systems, such as the knee, shoulder, wrist, and spine, and has demonstrated its outstanding performance in shortening imaging time, improving image signal-to-noise ratio and resolution.
