BackgroundThe application of telemedicine in home pulmonary rehabilitation interventions for the management of patients with chronic obstructive pulmonary disease (COPD) has achieved promising results. ObjectiveThis study aimed to develop a WeChat official account (Pulmonary Internet Explorer Rehabilitation [PeR]) based on social media. It further evaluated the effect of PeR on the quality of life, symptoms, and exercise self-efficacy of patients with COPD. MethodsThe functional modules of PeR were developed by a multidisciplinary team according to the electronic health–enhanced chronic care model (eCCM) components. A total of 106 patients were randomly selected (53 in the PeR group and 53 in the outpatient face-to-face group [FtF]). Pulmonary rehabilitation intervention was conducted for 3 months, and the outcome was observed for 3 months. The primary outcome was patient quality of life measured with the COPD assessment test (CAT). The secondary outcomes were evaluated using the modified Medical Research Council scale (mMRC), exercise self-regulatory efficacy scale (Ex-SRES), and St George’s Respiratory Questionnaire (SGRQ). ResultsThe intention-to-treat analysis was used in the study. A total of 94 participants completed the 6-month pulmonary rehabilitation program. Introduction: Polygonum amplexicaule D. Don var. sinense Forb (PAF), a medicinal plant, has the effect of promoting blood circulation and removing blood stasis. However, the active compounds and targets of its anticoagulant effect are still unclear. Objectives: This study aims to establish an effective reversely thrombin‐targeted screening method for anticoagulant active components in PAF by affinity ultrafiltration (AUF) coupled with ultrahigh‐performance liquid chromatography–quadrupole time‐of‐flight mass spectroscopy (UPLC‐Q‐TOF‐MS). Methods: Different polar parts of PAF were screened for potential thrombin ligands by AUF‐HPLC and identified by UPLC‐Q‐TOF‐MS. After studying the affinity between ligands and thrombin by molecular docking, the antithrombotic activity of ligands was detected in vivo by zebrafish thrombus model, and in vitro by chromogenic substrate method. The mechanism of such ligands on thrombin was further studied by coagulation factor assay. Results: Eleven potential thrombin ligands from PAF were screened by the AUF‐UPLC‐Q‐TOF‐MS method, and two compounds (butyl gallate and β‐sitosterol) with significant anticoagulant activity were discovered via in vitro and in vivo activity testing. Conclusion: A method system based on AUF‐UPLC‐Q‐TOF‐MS, molecular docking and in vivo and in vitro experiments also provided a powerful tool for further exploration of anticoagulant active components in PAF. The molecular docking results showed that five compounds had good affinity with THR. This study chosen six compounds from potential ligands to validate their antithrombotic activities for the zebra fish experiments. After further validation through in vitro thrombin inhibition and clotting factor assays, the results showed that two compounds (Butyl gallate and β‐sitosterol) had significantly anticoagulant activity. Polygonum Amplexicaule Radix is a traditional Chinese medicine for promoting blood circulation and removing blood stasis, which is often used to treat thrombotic diseases. However, its potential anticoagulant active ingredients have been unexplored. In this study, a method for rapid screening of anticoagulant active ingredients from P. amplexicaule by affinity-ultrafiltration (AUF) and high-performance – tandem mass spectrometry (HPLC-MS/MS) was established, which using thrombin as an affinity receptor target. 5 potential anti-thrombin active components were screened: gallic acid, procyanidin B2-3''O-gallate, 11-O-galloylbergenin, (-)-epicatechin gallate and di-galloyl-O-bergenin by AUF and HPLC-MS/MS. The inhibitory ability of these active ingredients to thrombin was confirmed by in vitro activity experiments and molecular docking, hence justifying that P. amplexicaule is a traditional Chinese medicine with excellent anticoagulation effects. IM: To explore the mechanism by which Chaijin-Jieyu-Anshen tablet (CJJY)-medicated serum prevents synaptic injury in rat anterior cingulate cortex (ACC) neurons using an in vitro depression model. METHODS: Cells (astrocytes, microglia and neurons) were isolated from the ACC of SD rats. The isolated cells were characterized by immunofluorescence staining. An in vitro depression model was developed using 1 mg/L lipopolysaccharide (LPS) combined with 200 imol/L corticosterone (CORT). These cells were divided into control group, model group (CORT+LPS), glucocorticoid receptor (GR) blocker (GR-) group (CORT+LPS+RU486), GR agonist (GR+) group (CORT+LPS+dexa-methasone), CX3C chemokine receptor 1 (CX3CR1) blocker (CX3-) group (CORT+LPS+AZD8797), CX3CR1 agonist (CX3+) group (CORT+LPS+fractalkine), CJJY group (CORT+LPS+CJJY-medicated serum), CJJY/GR+ group (CORT+ LPS+CJJY-medicated serum+dexamethasone), and CJJY/CX3+ group (CORT+LPS+CJJY-mediated serum+fractalkine). The morphological characteristics of all ACC cells were observed by high-content analysis. The levels of neuroendocrine-related factors, adrenocorticotropic hormone (ACTH), corticotropin-releasing hormone (CRH) and CORT, and neuroin-flammatory mediators, tumor necrosis factor-a (TNF-a ), interleukin-1 p (IL-1p), IL-6 and glutamate (Glu), in the cell supernatants were quantified by ELISA. Immunofluorescence staining was used to analyze the protein expression of GR and vesicular glutamate transporter 1 (VGluTl) in astrocytes, as well as CX3CR1 and adenosine A2A receptor (A2AR) in microglia. Immunofluorescence staining with Nissl and p -tubulin was performed to evaluate synaptic damage in ACC neurons. RESULTS: In an in vitro model of depression, CJJY-medicated serum prevented morphological damage to ACC neurons, microglia and astrocytes. Moreover, CJJY-medicated serum reversed abnormal increases in the levels of ACTH, CRH, CORT, TNF-a, IL-1p, IL-6 and Glu in cell supernatants (P<0. 05 or P<0. 01). It was also found that CJJY-medicated serum reduced abnormal expression of GR, VGluT1, CX3CR1 and A2AR (P<0. 05 or P<0. 01), alleviating damage to the neuronal dendrites and dendritic spines of ACC neurons. CONCLUSION: The CJJY-medicated serum regulates GR/CX3CR1 double signaling in glia, and attenuates rat ACC neuronal synaptic damage in an in vitro depression model, indicating that CJJY-medicated serum controls depression by affecting the GR/CX3CR1 double signaling.
