Purpose Bulimia nervosa (BN)is characterized by recurrent binge-eating episodes and inappropriate compensatory behaviors. This study investigated alterations in resting-state surface-based neural activity in BN patients and explored correlations between brain activity and eating behavior. Methods A total of 26 BN patients and 28 healthy controls were enrolled. Indirect measurement of cerebral cortical activity and functional connectivity (FC) analyses were performed in Surfstat. A principal component analysis (PCA) model was used to capture the commonalities within the behavioral questionnaires from the BN group. Results Compared with the healthy control group, the BN group showed decreased surface-based two-dimensional regional homogeneityin the right superior parietal lobule (SPL). Additionally, the BN group showed decreased FC between the right SPL and the bilateral lingual gyrus and increased FC between the right SPL and the left caudate nucleus and right putamen. In the FC-behaviorassociation analysis, the second principal component (PC2) was negatively correlated with FC between the right SPL and the left caudate nucleus. The third principal component (PC3) was negatively correlated with FC between the right SPL and the left lingual gyrus and positively correlated with FC between the right SPL and the right lingual gyrus. Conclusion We revealed that the right SPL undergoes reorganization with respect to specific brain regions at the whole-brain level in BN. In addition, our results suggest a correlation between brain reorganization and maladaptive eating behavior. These findings may provide useful information to better understand the neural mechanisms of BN. Level of evidence Level V, descriptive study.

The brain receives sensory information about food, evaluates its desirability and value, and responds with approach or withdrawal. The evaluation process of food in the brain with obesity may involve a variety of neurocircuit abnormalities in the integration of internal and external information processing. There is a lack of consistency of the results extant reported for aberrant changes in the brain with obesity that prohibits key brain alterations to be identified. Moreover, most studies focus on the observation of neural plasticity of function or structure, and the evidence for functional and structural correlations in the neuronal plasticity process of obesity is still insufficient. The aims of this article are to explore the key neural structural regions and the hierarchical activity pattern of key structural nodes and evaluate the correlation between changes in functional modulation and eating behavior. Forty-two participants with obesity and 33 normal-weight volunteers were recruited. Gray matter volume (GMV) and Granger causality analysis (GCA) were performed using the DPARSF, CAT12, and DynamicBC toolbox. Compared with the normal weight group, the obesity group exhibited significantly increased GMV in the left parahippocampal gyrus (PG). The obesity group showed decreased causal inflow to the left PG from the left orbitofrontal cortex (OFC), right calcarine, and bilateral supplementary motor area (SMA). Decreased causal outflow to the left OFC, right precuneus, and right SMA from the left PG, as well as increased causal outflow to the left middle occipital gyrus (MOG) were observed in the obesity group. Negative correlations were found between DEBQ-External scores and causal outflow from the left PG to the left OFC, and DEBQ-Restraint scores and causal inflow from the left OFC to the left PG in the obesity group. Positive correlation was found between DEBQ-External scores and causal outflow from the left PG to the left MOG. These results show that the increased GMV in the PG may play an important role in obesity, which may be related to devalued reward system, altered behavioral inhibition, and the disengagement of attentional and visual function for external signals. These findings have important implications for understanding neural mechanisms in obesity and developing individual-tailored strategies for obesity prevention.

OBJECTIVE. The purpose of this study was to investigate the utility of radiomics for predicting the malignancy of pulmonary nodules (PNs) of different sizes using unenhanced, thin-section CT images. MATERIALS AND METHODS. Patients with a single PN (n = 373) who underwent a preoperative chest CT were recruited retrospectively at Beijing Friendship Hospital from March 2015 to March 2018. Of the 373 PNs studied, 192 were benign and 181 were malignant. The lesions were classified into three groups (T1a, T1b, or T1c according to the 8th edition of the TNM staging system for lung cancer) on the basis of lesion diameters: T1a (diameter, 0-1 cm), T1b (1 cm < diameter ≤ 2 cm) and T1c (2 cm < diameter ≤ 3 cm). A total of 1160 radiomic features were extracted from PN segmentation on unenhanced CT images. We developed three radiomic models to predict PN malignancy in each group on the basis of the extracted radiomic features. Fivefold cross-validation was used to estimate AUC, accuracy, sensitivity, and specificity for indicating the performance of prediction models. RESULTS. The AUC, accuracy, sensitivity, and specificity for predicting PN malignancy in each group were 0.84, 0.77, 0.89, and 0.74 with the T1a model; 0.78, 0.73, 0.74, and 0.71 with the T1b model, and 0.79, 0.76, 0.77, and 0.73 with the T1c model, respectively. The most contributive radiomic features for predicting PN malignancy for groups T1a, T1b, and T1c were LoG_X_Uniformity, Intensity_Minimum, and Shape_SI9, respectively. CONCLUSION. Radiomic features based on unenhanced CT images can be used to predict the malignancy of pulmonary nodules. The radiomic T1a model showed superior prediction performance to the T1b and T1c models, and the best performance in terms of AUC and sensitivity was found for predicting the malignancy of T1a PN.

Objective The objective of this study was to investigate the changes in the blood oxygen level-dependent (BOLD) response in the ipsilateral primary somatosensory cortex (SI) and thalamus of patients with moyamoya disease (MMD) during sensory stimulation. Methods Sixty-four MMD patients, and 15 healthy volunteers were enrolled. Thirty-three MMD patients exhibited paroxysmal numbness or hypoesthesia in the unilateral limbs. Fifteen patients with acroparesthesia underwent unilateral encephaloduroarteriosynangiosis (EDAS). All volunteers underwent BOLD functional magnetic resonance imaging (BOLD-fMRI) under median nerve electrical stimulation (MNES). Blood oxygen level-dependent fMRI data were processed to obtain time-signal intensity curves in the activation areas of the bilateral SI and thalamus. Processed dynamic susceptibility contrast-enhanced magnetic resonance imaging data were used to measure the time to peak of the BOLD response in the regions of interest, including the bilateral SI, thalamus, and cerebellum. Changes in the time-signal intensity curve-related hemodynamic parameters in the ipsilateral SI and thalamus were examined between healthy controls, nonacroparesthesia patients, and asymptomatic and symptomatic sides of unilateral acroparesthesia patients during MNES. Changes in these parameters in MMD patients before and after EDAS were examined. Results Compared with healthy volunteers, 3 groups of MMD patients exhibited an increased peak of the positive BOLD response in the ipsilateral thalamus during MNES (0.65 ± 0.24 vs 0.79 ± 0.35, 0.94 ± 0.57, and 0.89 ± 0.50; P = 0.0335). The positive response peak in the ipsilateral SI markedly increased in MMD patients with acroparesthesia during MNES on the asymptomatic side (0.56 ± 0.37 vs 0.38 ± 0.27, P = 0.0243). The time to peak negative response in the ipsilateral SI was prolonged during MNES on the symptomatic side after EDAS (12.14 ± 8.90 seconds vs 18.86 ± 9.20 seconds, P = 0.0201). Conclusions During sensory stimulation treatment, BOLD response changes occurred in the ipsilateral SI and thalamus of MMD patients. These changes enabled the contralateral hemisphere of the brain to better deal with sensory stimuli.

The Liver Imaging Reporting and Data System (LI-RADS) is widely adopted for noninvasive diagnosis of hepatocellular carcinoma (HCC). It's updated to version 2018 recently, with some major changes compared with v2017. However, the diagnostic performance of LI-RADS v2018 and its difference with v2017 are yet to be validated.To compare the diagnostic performances of LI-RADS on MR for diagnosing HCC between v2017 and v2018.Retrospective.In all, 181 patients with 217 hepatic observations (146 HCCs, 16 non-HCC malignancies and 55 benign lesions) with liver MRI and pathological or follow-up imaging diagnoses.1.5 T or 3 T MRI. Dual-echo TSensitivity, specificity, accuracy, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (+LR), and Youden index.When adopting LR-5 as a predictor of HCC, the sensitivity (80.8% vs. 71.2%), NPV (69.6% vs. 60.7%), and accuracy (83.9% vs. 77.9%) were all increased for LI-RADS v2018 compared with v2017, with a greater Youden index (0.709 vs. 0.627). However, the diagnostic performances of MRI for diagnosing HCC were not changed while adopting LR-4/5 as a predictor. The threshold growths of 76% (19/25) observations in v2017 were revised to subthreshold growth in v2018, and 16 LR-4 observations in v2017 were changed to LR-5 based on v2018.The diagnostic performance of LI-RADS v2018 for diagnosing HCC is superior to v2017, with a greater sensitivity, NPV, and accuracy. The reward-related regions have been considered a crucial component in the regulation of eating behavior. Furthermore, appetite-related regions associated with reward can influence eating behaviors through altered functional activity related to food in brain areas associated with emotion, memory, sensory processing, motor function, and cognitive control.To investigate the key nodes in obese females of reward-related regions and, based on key nodes, to evaluate the directionality of functional connectivity between key nodes and appetite-related regions.Prospective.Twenty-eight obese and 28 normal-weight female controls of similar age.3.0 T MRI and echo planar imaging (EPI) sequence, 3D BRAVO sequence.The fMRI data preprocessing was based on the Data ProcessingAnalysis of Brain Imaging and Statistical Parametric Mapping 12. Statistical Tests: Independent two-sample t-tests were used to assess the differences in demographic and clinical data between two groups. Two-sample t-tests were conducted to test the difference in degree centrality and effective connectivity of key nodes between two groups.Compared with normal-weight controls, obese females showed an increased degree centrality in the left ventral striatum/caudate (t = 2.96808, P0.05) and decreased degree centrality in right orbitofrontal cortex (OFC) (t = -3.3558, P0.05). The obese females showed directional effective connectivity between left ventral striatum/caudate and several regions (left inferior temporal gyrus, fusiform gyrus, postcentral gyrus, and right precentral gyrus) (P0.05). Directional effective connectivity was also observed between the right OFC and several regions (left middle temporal gyrus, cuneus, OFC, superior temporal gyrus, middle frontal gyrus, and right inferior parietal lobule) (P0.05).The left ventral striatum/caudate and right OFC are key nodes in reward-related regions. Purpose To evaluate the reproducibility of the pancreatic apparent diffusion coefficient (ADC) measured at different MR scanners. Materials and Methods Twenty-four healthy volunteers underwent three consecutive diffusion-weighted imaging (DWI) at a GE 1.5 Tesla (T), a Siemens 1.5T and a Philips 3.0T (session 1), and imaged again using the same protocol at the same GE 1.5T (session 2) 12 days later. The ADC values of pancreas were measured at all three MR scanners. Paired-sample t-test and the Bland-Altman method were used for ADC data analysis. Results The individual mean ADC values of pancreatic head, body, and tail (in 10−3mm2/s) measured at GE 1.5T (2.24, 2.01, 1.88 for observer 1 and 2.23, 2.00, 1.92 for observer 2) and Siemens 1.5T (2.24, 2.04, 1.84 for observer 1 and 2.20, 1.98, 1.84 for observer 2) were significantly higher than those at Philips 3.0T (2.06, 1.80, 1.56 for observer 1 and 2.02, 1.79, 1.60 for observer 2) (P = 0.000–0.008). There was no significant difference of ADC values either between GE 1.5T and Siemens 1.5T (P = 0.115–0.966), or between imaging session 1 and 2 at GE 1.5T (P = 0.072–0.938). The range of mean difference ± limits of agreement (in 10−3mm2/s) was −0.07–0.04 ± 0.39–0.53 between two 1.5T scanners, and −0.04–0.04 ± 0.24–0.47 between two imaging sessions at GE 1.5T. Conclusion The measured ADC values of pancreas are affected by the field strength of scanner, but show good reproducibility between different MR systems with same field strength and at the same MR system over time. Background: Subarachnoid hemorrhage (SAH) from a ruptured cerebral aneurysm is a devastating disease. Despite the risk factors, including hypertension, cigarette smoking and alcohol use, are more common in men, aneurysmal SAH belongs to a few diseases which the incidence is higher in women than in men. Sex hormones, especially estrogen, might be protective against this condition. Hormone replacement therapy (HRT) seems to be associated with a reduced risk for aneurysmal SAH. This study aims to know the prevalence of aneurysmal SAH of men and women at different ages. Methods: The age and gender information of 4,895 case of aneurysmal SAH (3,016 females, 1,879 males) were collected retrospectively from eight institutions in mainland China. The prevalence of aneurysmal SAH of men and women at different ages was analyzed. Results: The data showed women had a higher incidence of aneurysmal SAH than men starting at late thirties, and men might have a higher incidence of aneurysmal SAH than women only before 37-year-old. Conclusions: Menopause may not be the only dominant factor causing higher incidence of aneurysmal SAH in women than in men.

Rationale and Objectives We sought to investigate the value of diffusion-weighted MR imaging in evaluating focal hepatic nodules in an experimental hepatocellular carcinoma (HCC) rat model. Materials and Methods Forty rats with chemically induced primary hepatic nodules ranging pathologically from regenerative nodules (RNs) to dysplastic nodules (DNs) to HCC were examined with diffusion-weighted imaging. The apparent diffusion coefficient (ADC) values of hepatic nodular lesions were calculated. Tukey’s HSD post hoc test was used to compare the difference in ADC values between different hepatic nodular lesions. Results Eight RNs, 16 DNs, 7 well-differentiated HCCs (HCC well ), 11 moderately differentiated HCCs (HCC mod ), and 14 poorly differentiated HCCs (HCC poor ) were evaluated. There was no significant difference between RNs and DNs ( P > 0.05). Although the ADC values of HCC well were slightly lower than those of DNs, there was no significant difference between them ( P > 0.05). The ADC values of HCC mod and HCC poor were significantly higher ( P mod and HCC poor ( P > 0.05). Conclusion Diffusion-weighted magnetic resonance imaging can be useful in characterizing focal hepatic nodular lesions, but ADC values cannot be used efficiently to distinguish HCC well from DNs.

Rationale and Objectives We used a 1.5-T MR scanner to investigate in vivo hydrogen 1 (1H) MRS to evaluate metabolic changes in the hepatocarcinogenesis experimental rat model. Materials and Methods Hepatocellular carcinoma (HCC) was induced by diethylnitrosamine in 70 treated rats with 20 normal rats used as controls. Single-voxel 1H MRS is performed to obtained the relative choline-to-lipid (Cho/lipid) ratio. The liver and tumor tissues are incised for the histologic examination. Based on the histologic result, the progression of hepatocarcinogenesis of the animal model was divided into three stages: fibrosis stage, cirrhosis stage, and HCC stage. The mean (±SD) ratio values are calculated and compared at various stages between the treated group and the control group. Results In control group, the calculated mean (±SD) Cho/lipid ratio was 0.15 ± 0.05. With the progression of hepatocarcinogenesis, the Cho/lipid ratio increased significantly, to 0.18 ± 0.05, 0.24 ± 0.07, and 0.38 ± 0.19, respectively. Conclusion The 1H MRS is technically feasible for evaluation of the metabolic changes in the animal model. A significant increase in choline-containing compounds level was observed in the HCC stage in the treated group.

Apparent diffusion coefficient (ADC) measurement in diffusion-weighted imaging (DWI) has been reported to be a helpful biomarker for detection and characterization of lesion. In view of the importance of ADC measurement reproducibility, the aim of this study was to probe the variability of the healthy hepatic ADC values measured at 3 MR scanners from different vendors and with different field strengths, and to investigate the reproducibility of normalized ADC (nADC) value with the spleen as the reference organ. Thirty enrolled healthy volunteers received DWI with GE 1.5T, Siemens 1.5T, and Philips 3.0T magnetic resonance (MR) systems on liver and spleen (session 1) and were imaged again after 10 to 14 days using only GE 1.5T MR and Philips 3.0T MR systems (session 2). Interscan agreement and reproducibility of ADC measurements of liver and the calculated nADC values (ADCliver/ADCspleen) were statistically evaluated between 2 sessions. In session 1, ADC and nADC values of liver were evaluated for the scanner-related variability by 2-way analysis of variance and intraclass correlation coefficients (ICCs). Coefficients of variation (CVs) of ADCs and nADCs of liver were calculated for both 1.5 and 3.0-T MR system. Interscan agreement and reproducibility of ADC measurements of liver and related nADCs between 2 sessions were found to be satisfactory with ICC values of 0.773 to 0.905. In session 1, the liver nADCs obtained from different scanners were consistent (P = 0.112) without any significant difference in multiple comparison (P = 0.117 to >0.99) by using 2-way analysis of variance with post-hoc analysis of Bonferroni method, although the liver ADCs varied significantly (P

Using the functional magnetic resonance imaging (fMRI) to observe the distributed characteristic of excited cerebral cortical areas that induced by acupuncture-stimulating the Zhongzhu (TE 3) of the meridian of Hand-Shaoyang and Yanglingquan (GB 34) of the meridian of Foot-Shaoyang, and investigate the central neural mechanism on the effect of meridians and acupoints.Forty-two right handed healthy volunteers were randomly divided into Zhongzhu (TE 3) group and Yanglingquan (GB 34) group. The functional cortical changes during acupuncture-stimulating the Zhongzhu (TE 3) and Yanglingquan (GB 34) were successively scanned with fMRI, and the effected areas were determined through analysing the obtained data with SPM2 software.The main excited areas were bilateral frontal lobes, temporal lobes, cerebellum and occipital lobes successively in Zhongzhu (TE 3) group, and bilateral occipital lobes, cerebellum, frontal lobes and temporal lobes in Yanglingquan (GB 34) group in contrast.Acupuncture-stimulating both Zhongzhu (TE 3) and Yanglingquan (GB 34) can excite bilateral acoustic, visual and somatomotor cortices, which might be the central neural basis for clinical treatment on related diseases.

To investigate the role of perfusion-weighted magnetic resonance imaging (MRI) in evaluation of cirrhotic liver.With a 4F catheter, 1% diluted carbon tetrachloride (1 ml/kg) was selectively injected into right or left hepatic artery of 12 dogs fortnightly. The half liver into which carbon tetrachloride was injected was called as study side (SS), while the other half liver without carbon tetrachloride injection was called as study control side (SCS). Conventional and perfusion-weighted MRI were performed in every 4 weeks. Via a 4F catheter, 5ml gadolinium diethylentriamine pentaaceti acid (Gd-DTPA) dilution was injected into superior mesenteric artery at the 5th scan. The signal intensity-time curves of SS, SCS, and portal vein were completed in MR workstation. The maximal relative signal increase (MRSI), peak time (tp), and slope of the curves were measured.On conventional MR images, no abnormalities of externality and signal intensity were observed in both SS and SCS of liver at each stage. The mean tp, MRSI, and slope of intensity-time curves in normal liver were 10.56 seconds, 1.01, and 10.23 arbitrary unit (au)/s, respectively. Three parameters of curves didn't show obvious change in SCS of liver at every stage. Abnormal perfusion curves occurred in SS of liver at the 12th week after the 1st injection. The abnormality of perfusion curve in SS was more and more serious as the times of injection increased. The mean tp, MRSI, and slope intensity-time curves in SS of liver were 19.45 seconds, 0.43, and 3.60 au/s respectively at the 24th week.Perfusion-weighted imaging can potentially provide information about portal perfusion of hepatic parenchyma, and to some degree, reflect the severity of cirrhosis.

To determine the magnetic resonance (MR) imaging findings of an ovarian mass which are most predictive of malignancy and assess the value of intravenous gadolinium administration in the characterization of an ovarian mass.Totally 74 consecutive patients with a clinically or sonographically indeterminate adnexal mass underwent MR imaging, of whom 59 had subsequent surgical resection of 70 adnexal masses. These 59 patients formed the study population. MR imaging studies were prospectively and independently reviewed by a senior and a junior radiologist. The senior radiologist also reevaluated the studies in a blind fashion after a minimum 6 months interval. The sensitivity, specificity, positive predictive value, and negative predictive value of contrast-enhanced and unenhanced MR imaging were evaluated.The most predictive MR imaging findings for malignancy were presence of vegetations in a cystic lesion and presence of necrosis in a solid lesion. The odds ratio was even higher when the ancillary finding of peritoneal metastasis or ascites was present. Contrast media contributed significantly to lesion characterization. Total 70 ovarian masses were detected by contrast-enhanced MR imaging including 37 malignant ovarian masses and 33 benign ovarian masses with 87% (61/70) accuracy, 86% (32/37) sensitivity, 88% (29/33) specificity, 89% (32/36) positive predictive value, and 85% (29/34) negative predictive value, whereas 70 ovarian masses were detected by unenhanced MR imaging with 74% (52/70) accuracy, 73% (27/37) sensitivity, 76% (25/33) specificity, 77% (27/35) positive predictive value, and 71% (25/35) negative predictive value. There were significant differences in accuracy (P0.01), sensitivity (P0.01), specificity (P0.01) between contrast-enhanced and unenhanced MR imaging.Contrast-enhanced MR imaging is highly accurate in detection and characterization of complex adnexal masses.
