Abstract

Study objectiveThe transfusion of older packed RBCs may be harmful in critically ill patients. We seek to determine the association between packed RBC age and mortality among trauma patients requiring massive packed RBC transfusion.MethodsWe analyzed data from the Pragmatic, Randomized Optimal Platelet and Plasma Ratios trial. Subjects in the parent trial included critically injured adult patients admitted to 1 of 12 North American Level I trauma centers who received at least 1 unit of packed RBCs and were predicted to require massive blood transfusion. The primary exposure was volume of packed RBC units transfused during the first 24 hours of hospitalization, stratified by packed RBC age category: 0 to 7 days, 8 to 14 days, 15 to 21 days, and greater than or equal to 22 days. The primary outcome was 24-hour mortality. We evaluated the association between transfused volume of each packed RBC age category and 24-hour survival, using random-effects logistic regression, adjusting for total packed RBC volume, patient age, sex, race, mechanism of injury, Injury Severity Score, Revised Trauma Score, clinical site, and trial treatment group.ResultsThe 678 patients included in the analysis received a total of 8,830 packed RBC units. One hundred patients (14.8%) died within the first 24 hours. On multivariable analysis, the number of packed RBCs greater than or equal to 22 days old was independently associated with increased 24-hour mortality (adjusted odds ratio [OR] 1.05 per packed RBC unit; 95% confidence interval [CI] 1.01 to 1.08): OR 0.97 for 0 to 7 days old (95% CI 0.88 to 1.08), OR 1.04 for 8 to 14 days old (95% CI 0.99 to 1.09), and OR 1.02 for 15 to 21 days old (95% CI 0.98 to 1.06). Results of sensitivity analyses were similar only among patients who received greater than or equal to 10 packed RBC units.ConclusionIncreasing quantities of older packed RBCs are associated with increased likelihood of 24-hour mortality in trauma patients receiving massive packed RBC transfusion (≥10 units), but not in those who receive fewer than 10 units.

23. Effect of stress unloading rate on fine-scale deformation mechanism of rock under high osmotic pressure.

Abstract

To explore the influence of the working face excavation rate on the rock deformation mechanism and seepage characteristics, deformation and seepage tests of sandstone under different loading and unloading stress paths, such as constant axial pressure unloading confining pressure and loading axial pressure unloading confining pressure, were carried out. Particle Flow Code in 3 Dimensions (PFC3D) and Python were used to realize fluid-solid coupling, and numerical simulation calculations were performed along the test path to analyze the influence of the unloading rate on the fine-scale deformation mechanism and permeability characteristics of sandstone, and the relationship between crack type and permeability was obtained. A sandstone fracture mechanics model is established to analyze the stress concentration degree at the end of the branch crack of the test path. The results show that the rate of confining pressure unloading is inversely proportional to the strain. Additionally, permeability correlates with the principal stress difference in an exponential manner. Interestingly, the sensitivity of permeability to stress shows an inverse trend with the unloading rate of confining pressure. Furthermore, there exists a linear relationship between permeability and the number of cracks. During the unloading process, tensile cracks predominate, and the propagation of shear cracks lags behind that of tensile cracks. The proportion of tensile cracks decreases with the increase of the unloading rate when the axial pressure is unchanged but increases when axial pressure is added, resulting in axial compression deformation and expansion deformation along the unloading direction. These research outcomes offer theoretical insights for the prudent selection of mining rates, and they hold significant implications for mitigating water inrush disasters in deep mining operations. 31. Grain-growth Inhibitor with Three-section-sintering for Highly Dispersed Single-crystal NCM90 Cubes

Abstract

Single crystallization of LiNixCoyMn1-x-yO2(NCM) is currently an effective strategy to improve the cycling life of NCM cathode, owing to the reduced surface area and enhanced mechanical strength, but the application of single crystal NCM(SC-NCM) is being hindered by severe particle agglomeration and poor C-rate performance. Here, a strategy of three-section-sintering(TSS) with the presence of grain-growth inhibitor was proposed, in which, the TSS includes three sections of phase-formation, grain-growth and phase-preservation. While, the addition of MoO3 inhibits the grain growth and restrains the particle agglomeration. With the help of TSS and 1 mol % of MoO3, highly dispersed surface Mo-doped SC-NCM(MSC-NCM) cubes are obtained with the average diameter of 1.3 mu m. Benefiting from the surface Mo-doping and the reduced surface energy, the Li+-migration preferred (1 0 4) crystalline facet is exposed as the dominant plane in MSC-NCM cubes, because of which, C-rate performance is significantly improved compared with the regular SC-NCM. Furthermore, this preparation strategy is compatible well with the current industrial production line, providing an easy way for the large-scale production of SC-NCM. Highly dispersed single-crystal NCM90 cubic particles were prepared through a three-section-sintering strategy with the presence of grain-growth inhibitor, which show improved cycling stability, superior rate performance and enhanced safety performance.image

Abstract

AbstractClinical trials are an essential component of the drug development process, providing crucial data on the efficacy and safety of new treatments. However, traditional clinical trial designs can be inefficient and ineffective, leading to increased costs and a higher risk of failure. The FDA Oncology Center of Excellence (OCE) has recently initiated Project Optimus to reform the dose selection paradigm for oncology treatments. We propose the adaptive seamless phase II/III clinical trial designs (ASD) with the sequential estimation-adjusted urn (SEU) model for randomization to achieve efficient and ethical objectives. However, the combination of ASD and SEU poses a challenge in controlling the type I error rate: ASD exerts a dual influence of multiplicity and selection; all the responses and treatment assignments are not independent due to SEU. In this paper, we investigated how to overcome these difficulties, utilize the two adaptive approaches’ advantages, and control the type I error rate. We provide a theoretical foundation for this procedure, and numerical studies demonstrate that our methods can assign more people to better treatments, leading to fewer failures while still controlling the type I error rate and preserving power. Abstract

The principle of independence is a fundamental yet often disregarded assumption in statistical inference. It is observed that the implications of correlations, if not considered, can lead to a conservative estimation of Type I error in the presence of positive linear correlations when utilizing the Kolmogorov-Smirnov (KS) test. Conversely, negative linear correlations may engender a liberal estimation of Type I error. To address the impact of spatial autocorrelation in the analysis of Positron Emission Tomography (PET) images, we have proposed an innovative methodology to reconstruct a grid map of human heart scans using spherical coordinates. We have examined the distribution of the KS test statistic under spatial autocorrelation through Monte Carlo (MC) simulation and have introduced a KS test with a spatial adjustment. The newly proposed KS test with spatial adjustment demonstrates a controlled Type I error and power that is not inferior when compared to the original KS test. Abstract

The microstructure, mineral composition, total organic carbon content, etc., of gas shale are crucial parameters for shale reservoirs, which can directly/indirectly affect shale brittleness, fracturing effect, adsorption ability and production efficiency. The study proposed a workflow to characterize the physical and mechanical parameters of Lower Silurian Longmaxi shale outcrop samples extracted from the favorable block in Changning, Sichuan, southwest China. This study elaborated on the influence of these physical and mechanical characteristics and proposed a corresponding brittleness index on shale extraction. In addition, it put forward corresponding suggestions for development and risk control. For a better understanding the mechanisms of shale gas storage and production, XRD, XRF, SEM, low temperature Nitrogen adsorption method, nuclear magnetic resonance and other measurements were employed to analyze and study the mineral composition, microstructure, and adsorption performance of shale. The results demonstrated that the pores of shale are mainly slit pores; there are diverse pore types in shale, mainly including intergranular pores, mineral particle dissolution pores, and internal pores of organic matter; The samples with relatively low porosity also noticeably exhibit ultra-low permeability, and the nanopore structure is remarkably significant, with distribution primarily in range of 5-237 nm. Finally, a brittleness index considering the influence of water content and the mechanical properties was proposed, and the coupling interaction of various minerals components and mechanical properties on the brittleness index can more objectively reflect the brittleness characteristics of deep shale formation. 50. Sequential monitoring of response-adaptive randomized clinical trials

Abstract

Clinical trials are complex and usually involve multiple objectives such as controlling type I error rate, increasing power to detect treatment difference, assigning more patients to better treatment, and more. In literature, both response-adaptive randomization (RAR) procedures (by changing randomization procedure sequentially) and sequential monitoring (by changing analysis procedure sequentially) have been proposed to achieve these objectives to some degree. In this paper, we propose to sequentially monitor response-adaptive randomized clinical trial and study it's properties. We prove that the sequential test statistics of the new procedure converge to a Brownian motion in distribution. Further, we show that the sequential test statistics asymptotically satisfy the canonical joint distribution defined in Jennison and Turnbull (\citeyearJT00). Therefore, type I error and other objectives can be achieved theoretically by selecting appropriate boundaries. These results open a door to sequentially monitor response-adaptive randomized clinical trials in practice. We can also observe from the simulation studies that, the proposed procedure brings together the advantages of both techniques, in dealing with power, total sample size and total failure numbers, while keeps the type I error.
