26. Abstract 11534: Cost Effectiveness of Cascade Testing for Familial Hypercholesterolemia in the US: A Simulation Analysis

Abstract

Background:Familial hypercholesterolemia (FH) is an autosomal dominant disorder that significantly increases the risk of premature coronary heart disease (CHD). Unfortunately, no coordinated screening programs currently exist in the U.S.Objective:The goal of this study was to evaluate the contemporary cost-effectiveness of cascade testing of relatives of FH probands with an identifiable pathogenic variant and to assess whether cost-effectiveness is affected by the age at which testing is initiated in family members and the degree of relatedness to the proband.Methods:A Markov Decision Tree model was created in Arena?simulation software (Rockwell Automation, Coraopolis, PA) to simulate multiple family trees starting with progenitor individuals carrying a pathogenic variant for FH who were followed through multiple generations. This approach allowed us to examine a family tree that had grown sufficiently enough to have large numbers of relatives across multiple degrees of relatedness. The model identified FH probands and estimated costs and life years gained when cascade genetic testing was done in their family members. Costs were valued in 2017 U.S. dollars. Future costs and life years projected by the model were discounted at an annual rate of 3%.Results:For first-degree relatives of an FH proband, the average number of life years gained per person was 4.4 when genetic testing was done in relatives who were 10 years of age or older. At this age threshold, the average discounted cost per life year gained per person was approximately $1047. Results were similar for second-degree relatives. However, beginning with third-degree relatives, the average number of life years gained per person dropped to less than 1 and the average discounted cost per life year gained implied no benefit for treatment for many age thresholds.Conclusion:Cascade genetic testing of first-degree and second-degree relatives of FH probands with a known pathogenic variant is more cost-effective than testing relatives with more distant degrees of relatedness. The age threshold at which testing relatives appears to be most cost-effective is 10 years of age. These findings support establishing an FH cascade testing program in the US.

27. Abstract 13807: Prevalence and Control of Primary Severe Hypercholesterolemia in Olmsted County, Minnesota: A Community-Based Study

Abstract

Introduction:Severe hypercholesterolemia (SH), defined as plasma low-density lipoprotein cholesterol (LDL-C) level ? 190 mg/dl, is a well-established risk factor for atherosclerotic cardiovascular disease. The prevalence and control of primary SH in the community are unknown. We, therefore, conducted a community-based study to estimate the prevalence and control of primary SH, in Olmsted County, MN.Methods:We used the Rochester Epidemiology Project (REP) records linkage system to identify individuals with LDL-C ? 190 mg/dl between 1998-2015. We excluded individuals with mixed hyperlipidemia (triglycerides ?400 mg/dl on 2 occasions). Primary SH was defined as the absence of an identifiable secondary etiology (hypothyroidism, biliary obstruction, nephrotic syndrome, uncontrolled diabetes, pregnancy, liver disease, or renal failure). Clinical characteristics were ascertained from the electronic health record.Results:SH was identified in 3852 individuals of whom 522 had a secondary cause for SH and 3330 had primary SH. As of December 31, 2015, 2294 of the 3330 patients were living Olmsted County residents with primary SH representing prevalent cases. The point prevalence per 100,000, stratified by age and sex, is summarized in Table 1. The total age- and sex-adjusted prevalence of primary SH was 1.7% (95% CI 1.63-1.77). Prevalence estimates varied with age and sex. Of the 3330 individuals, nearly 1 in 10 met the Dutch Lipid Clinic Network (DLCN) criteria for probable/definite familial hypercholesterolemia. Only 58.6% (n=1946) of patients with primary SH were receiving lipid-lowering therapy (LLT) within 18 months of follow-up after diagnosis.Conclusions:These data highlight that primary SH is relatively common in the community (?2 in 100), suggesting that ?6 million US individuals have this phenotype and are eligible for LLT as per the ACC/AHA guideline. Our results indicate that primary SH may be significantly undertreated in the US.
