A negatívnyomás-terápia (NPWT) széles körben alkalmazott módszer, elsősorban problémás sebek, lágyrész-fertőzések, szeptikus hasi folyamatok, abdominalis kompartment szindróma, nyelőcső- és végbélanastomosis-elégtelenség kezelésére. Sajnos a klinikai gyakorlatban sokszor csak mint „ultimum refugiumot” alkalmazzák. A kezelést a betegek jól tolerálják, az életminőség jobb, mint a konvencionális kezelés esetén. Az NPWT alkalmazása, kedvező hatásai miatt lerövidíti a kezelési időt, jobb korai és késői eredményekkel bír és költséghatékonyabb. A szerzők felhívják a figyelmet arra, hogy a vákuumkezelést megfelelő indikációk alapján minél hamarabb el kell kezdeni. A kezelés alatti életminőség, a kezelési idő, a költséghatékonyság, az eredményesség és az evidencia kérdéseinek elemzésével világítanak rá arra, hogy az NPWT sok esetben első kezelésként javasolt.

Absztrakt: Bevezetés: Akut pancreatitis miatti necrectomiát ideálisan a betegség kezdetétől számított 4–6. héten javasolt elvégezni, amikor az ún. demarkált necrosis (walled-off pancreatic necrosis –WOPN) már kialakult. A szerzők ismertetik a kiterjedt WOPN miatt végzett sebészi transgastricus necrectomiák során nyert tapasztalataikat. Betegek és módszer: Kiterjedt WOPN miatt 2012. január 1-től 2017. december 31-ig terjedő időszakban 17 (12 férfi, 5 nő, átlag életkor 64,00 ± 15,14 év) betegnél végeztek sebészi transgastricus necrectomiát. Valamennyi betegnél a műtétet megelőzően konzervatív és szemikonzervatív kezelés történt átlagosan 61,00 ± 33,47 napig. A WOPN mérete átlagosan 14,00 ± 5,23 cm volt és valamennyi esetben a retrocolicus, retroduodenalis terekbe is beterjedt. A necrosis minden esetben bakteriálisan kontaminált volt. Eredmények: A műtétekkel kapcsolatos szövődmény nem volt. Az ápolási napok száma a műtétet követően 10,00 ± 3,08 nap volt. A mortalitás 5,9% volt. Késői műtét vagy egyéb beavatkozás pseudocystaképződés, vagy pancreas fistula miatt nem volt. Két betegnél a WOPN és a gyomor közötti anastomosis-endoszkópos tágítása és öblítése történt lázas állapot miatt. Frissen kialakult diabetest nem, a meglévő súlyosbodását 6,3%-ban észleltük. Következtetés: A kiterjedt, fertőzött demarkált necrosis kezelésében a sebészi transgastricus necrectomia jó eredménnyel alkalmazható. A műtét előnye, hogy a necrectomia után nem alakulhat ki külső pancreas sipoly és pseudocysta.

Recent SARS-CoV-2 pandemic leading to a rapidly increasing number of hospitalizations enforced reevaluation of wound management strategies. The optimal treatment strategy for patients with chronic wounds and those recovering from emergency and urgent oncological surgery should aim to minimize the number of hospital admissions, as well as the number of surgical procedures and decrease the length of stay to disburden the hospital staff and to minimize viral infection risk. One of the potential solutions that could help to achieve these goals may be the extensive and early use of NPWT devices in the prevention of wound healing complications. Single-use NPWT devices are helpful in outpatient wound treatment and SSI prevention (ciNPWT) allowing to minimize in-person visits to the health care center while still providing the best possible wound-care. Stationary NPWT should be used in deep SSI and perioperative wound healing disorders as soon as possible. Patient’s education and telemedical support with visual wound healing monitoring and video conversations have the potential to minimize the number of unnecessary in-person visits in patients with wounds and therefore substantially increase the level of care.

In surgical pathophysiology ischemia-reperfusion, inflammatory processes, sepsis, vascular interventions, tissue trauma, shock, all mean conditions in which hemorheological parameters show alterations. Despite of numerous clinical and experimental studies, the in vivo hemorheology is not completely understood yet, and several fundamental questions still need to be answered. Investigating these issues, experimental surgical models are important, in point of view of the translational research as well. In this paper we aimed to make an attempt on summarizing the possible factors and conditions that might have an effect on hemorheological results in experimental surgical studies. Hemorheological parameters show alterations in surgical pathophysiological processes in a complex way. However, the changes are dominantly non-specific. Standardized experimental conditions, related to the experimental animal (species, animal welfare) anesthesia-medications, operation, sampling and, if applicable, conditions of the postoperative period, are inevitable for a safe assessment of valuable (hemorheological) results. Parallel investigations - such as microcirculatory monitoring, imaging techniques, other laboratory methods, histomorphology- have great importance, together with individual analysis of changes, for a better understanding of the changes and for comparability with clinical results.

Primary pancreatic hydatid disease is extremely rare. Diagnosis of the disease is difficult because hydatid cysts can be confused with a pseudocyst or neoplastic cystic diseases. Authors report a case of a surgically treated hydatid disease of the uncinate process of pancreas. In a 34-year-old patient with minor symptoms, a cystic disease of the pancreas was accidentally identified. CT scan revealed a multivesicular cystic mass with a maximum of 13-cm diameter and with a calcificated wall. During laparotomy, the uncinate process of pancreas was resecated and the cystic lesion was enucleated. Patient was recovered without complications and recurrence of the disease. There must be a suspicion of hydatid disease when cysts are identified in good conditioned, asymptomatic patients, or in case of wall calcification or multivesicular cysts revealed by radiological images. Surgical procedures are recommended in uncertain diagnoses too, because differentiation preoperatively between cystic pancreatic lesions is often impossible.

In severe acute pancreatitis the timing of necrosectomy is ideally should be postponed 4-6 weeks after the onset of the disease when the walled-of pancreatic necrosis (WOPN) has developed. The authors present their experience with open transgastric necrosectomy for extended WOPN.The authors performed 17 (12 male, 5 female with a mean age of 61.6 ± 15.1 years) open transgastric necrosectomies for extended WOPN in a period of 1, January 2012 and 31, December 2017. Before the operations conservative and semiconservative therapy was used for an average of 74.6 ± 83.1 days. The mean size of the WOPNs was 13.8 ± 5.2 cm with localisation of the retrocolic and retroduodenal regions. All necroses were septic.Complications related to the operation were not observed. The mean time of hospitalization after the surgery was 11.6 ± 12.8 days. The mortality rate was 5.9%. Late operation or other interventions for pseudocyst or pancreas fistula formation was not performed. Two patients needed endoscopic dilatation with lavage in the early postoperative period because of fever. New diabetes mellitus was not observed but worsening of previously existed diabetes developed in 6.3% of the cases.The open transgastric necrosectomy is safe and effective for extended WOPN. Walled-off pancreatic necrosis is a late complication of severe acute pancreatitis that generally needs some interventions. The aim of this review is to analyse the indications, technical aspects and limits of these therapeutic options. The development of the walled-off pancreatic necrosis needs 4–6 weeks from the onset of the disease. The necrosectomy is recommended after this time. Endoscopic necrosectomy offers good results if the necrosis is in retrogastric or retroduodenal localisations. Open necrosectomy can be performed through the gastrocolic ligament or the mesocolon. It is suggested to complete necrosectomy with open or closed omental bursa drainage or packing. The transgastric necrosectomy does not need external drainage. Surgical procedures can be performed with laparoscopy either. In localized necrosis other minimal invasive approaches can be used. Conclusions: In addition to the transluminal endoscopic or minimal invasive necrosectomies different types of surgical procedures has an important role in the treatment of walled-off pancreatic necrosis. Background Pancreatic ductal HCO 3 - secretion is critically dependent on the cystic fibrosis transmembrane conductance regulator chloride channel (CFTR) and the solute-linked carrier 26 member 6 anion transporter (SLC26A6). Deterioration of HCO 3 - secretion is observed in chronic pancreatitis (CP), and CFTR mutations increase CP risk. Therefore, SLC26A6 is a reasonable candidate for a CP susceptibility gene, which has not been investigated in CP patients so far. Methods As a first screening cohort, 106 subjects with CP and 99 control subjects with no pancreatic disease were recruited from the Hungarian National Pancreas Registry. In 60 non-alcoholic CP cases the entire SLC26A6 coding region was sequenced. In the Hungarian cohort variants c.616G > A (p.V206M) and c.1191C > A (p.P397=) were further genotyped by restriction fragment length polymorphism analysis. In a German replication cohort all exons were sequenced in 40 non-alcoholic CP cases and variant c.616G > A (p.V206M) was further analyzed by sequencing in 321 CP cases and 171 controls. Results Sequencing of the entire coding region revealed four common variants: intronic variants c.23 + 78_110del, c.183-4C > A, c.1134 + 32C > A, and missense variant c.616G > A (p.V206M) which were found in linkage disequilibrium indicating a conserved haplotype. The distribution of the haplotype did not show a significant difference between patients and controls in the two cohorts. A synonymous variant c.1191C > A (p.P397=) and two intronic variants c.1248 + 9_20del and c.-10C > T were detected in single cases. Conclusion Our data show that SLC26A6 variants do not alter the risk for the development of CP.

Previously we have investigated the cerulein-induced acute pancreatitis and provided data on its micro-rheological impact in the rat. We hypothesized that non-steroid anti-inflammatory agent flunixin, the xanthine-derivate pentoxifylline and the low molecular weight heparin enoxaparin may have various beneficial effects improving microcirculatory and rheological parameters. In female rats, under general anesthesia, 10 μg/kg cerulein s.c. was administered and 2 hours afterwards microcirculation was tested by laser Doppler flowmetry on the tongue and after performing laparotomy on the small intestine, liver and pancreas prior to terminal blood sampling. From blood samples hematological parameters, blood pH, lactate concentration, erythrocyte deformability, osmoscan parameters and erythrocyte aggregation were tested. Compared to normal control in acute pancreatitis group we found severe deterioration in tissue microcirculation together with impaired erythrocyte deformability and enhanced aggregation, accompanied by acidic pH and increasing lactate concentration. Improvement was found when using flunixin (s.c.), pentoxifylline (i.p.) or enoxaparin (s.c.). These drugs could partly improve the blood flux on the surface of the investigated organs, and the flunixin had the most expressed improving effects on micro-rheological parameters. Surprisingly, the improving effect of pentoxifylline on micro-rheological parameters was not obvious (red blood cell deformability did not improved better than in the other treated groups), however, microcirculatory parameters improved.

Absztrakt Bevezetés/célkitűzés: 1968-ban R. E. Fear először közölt tanulmányt a laparoscopos „Port site” herniáról (Trocar site hernia, TSH), jelenlegi előfordulási gyakorisága eléri a 0,65–2,80%-ot. A sebészi laparoscopia elterjedésével a beavatkozások során szokássá vált, hogy a 10 mm-nél kisebb trokársebeket nem zárják be. Ugyanakkor egyre több közlemény foglalkozik az 5–6 mm-es trokár (továbbiakban: KT) helyén keletkező sérvek elzáródásával. Anyag és módszer: A szerzők oktatási célból 60 kutyán végzett laparoscopos cholecystectomia műtét után 2 és 4 héttel relaparoscopiát végeztek, és posztoperatív adhaesiók után kutatva azt is figyelték, hogy a laparoscopos trokárok intraabdominalis behatolási helyein vannak-e sérvek, különös tekintettel a KT-ra. Eredmények: A 60 kísérleti állat 20%-ában volt látható intraabdominalis sérv, és a sérvek 70%-a a KT helyein helyezkedett el. Következtetések: A megszokott gyakorlattal ellentétben az irodalomban egyre többen hangoztatják a nagy trokárok és a KT ejtette teljes vastagságú hasfali sebek zárásának szükségességét, megelőzendő a sérvek kialakulását és kizáródásuk lehetőségét. Tanulmányunkban a technikai és klinikai lehetőségeket mutatjuk be.

AbsztraktMájműtétek során gyakran szükséges az ellátó erek hosszabb-rövidebb idejű leszorítása, ezáltal a szerv vérellátása átmenetileg zavart szenved, majd az ezt követő reperfusio során további károsodások jöhetnek létre. Ezek hatására az adott szervben is és más lokalizációban is anyagcsere- és morphologiai változások jöhetnek létre, amelyek meghatározzák a szervek, szövetek további sorsát, életképességét, regenerációját, ezáltal a műtét kimenetelét is. Állatkísérletes modellben vizsgáltuk a Báron/Pringle manőver hatását a májfunkció és szövettani károsodások szempontjából. 12 beagle kutyán 3×15 perces ligamentum hepatoduodenale leszorítás során, majd 30 perces reperfusio után vettünk vér- és szövettani mintákat. Markáns szövettani változásokat észleltünk mind a máj, mind a vékonybelek területén, valamint a májenzimek közül. Legkorábban a GPT-emelkedés jelentkezett, majd a 30 perces reperfusiót követően a GOT és a LDH is megemelkedett. Állatkísérletes modellen hosszúnak bizonyult a harmadik 15 perces leszorítás, legérzékenyebben a GPT-emelkedés jelezte az ischaemiát.

Az enyhe akut pancreatitis kezelése alapvetően konzervatív, míg súlyos esetekben a sebészi kezelésnek jelentős szerepe van. Módszer: A szerzők az utóbbi 10 évben megjelent, a műtét kérdéseit taglaló közlemények alapján elemzik a sebészi kezelés indikációit, időzítését és a műtét technikai kérdéseit. Eredmények/következtetések: A steril pancreasnecrosis csak ritkán, a szeptikus necrosis azonban műtéti indikációt képez, ha a konzervatív kezelés eredménytelen. A terápiarezisztens sokszervi elégtelenség, a súlyos hasi kompartmentszindróma és egyéb sebészi szövődmény, mint a vérzés, perforáció is műtéti beavatkozást tesz szükségessé. Pancreatitist okozó epekövesség talaján kialakult cholestasis esetén sürgős endoszkópos sphincterotomia és később cholecystectomia javasolt. Pancreasnecrosis esetén a műtét ideális időpontja a betegség kezdetétől számított 21. nap utánra tehető, mivel a „korai” műtétek szövődmény- és halálozási aránya magas. Fontos a gondos necrectomia, amelynek a retrocolicus és retroduodenalis terekre is ki kell terjednie. A nyitott has kezelésének több a szövődménye, ezért csak válogatott esetekben javasolják. A műtétet posztoperatív bursa omentalis lavage-zsal érdemes kiegészíteni. Orv. Hetil., 2010, 41, 1697–1701.

Absztrakt Bevezetés: A spontán pneumothorax kezelésében a mellkasdrainage mellett a videothoracoscopia jelentős szerepet játszik. Jelen közlemény célja a különböző terápiás eljárások értékelése, a korai eredmények szempontjából. Betegek és módszer: A szerzők 5 év alatt 243 beteget (184 férfi, 59 nő) 302 esetben kezeltek spontán pneumothorax miatt. A primer pneumothorax miatt kezelt 114 beteg átlagéletkora (23,1 ± 5 év) szignifikánsan alacsonyabb volt a 129 szekunder ptx (51,5 ± 15 év) miatt kezeltekénél. Partialis pneumothorax esetén observatio 24 (8%), aspiráció 6 (2%) esetben történt. A mellkas csövezését 241 (67,6%) esetben végezték. Műtéti indikáció volt a recidív, a korábbi ellenoldali, valamint a drainage hatására nem gyógyuló pneumothorax. Videothoracoscopiát 71 (23,5%), minithoracotomiát 5 (1,7%), axillaris thoracotomiát 13 (4,3%) esetben indikáltak. A műtétet 25 esetben partialis pleurectomiával, 32 esetben talcumos, 3 esetben mechanikus pleurodesisszel egészítették ki. Eredmények: A konzervatív kezelés 80%-ban, a drainage 84,7%-ban vezetett gyógyuláshoz. A 71 VATS során 8 esetben (11,3%) konvertálták a műtétet. VATS, thoracotomia és minithoracotomia után valamennyi beteg meggyógyult. A posztoperatív szövődmények aránya VATS után 6,3%, thoracotomia után 7,7% volt. Az ápolási napok száma drainage után 8,5, VATS után 9,1, thoracotomia után 11,3 nap volt. A szekunder ptx-os csoportban drainage utáni ápolási idő (9,2 vs. 7,4 nap) szignifikánsan, a halálozás (2,6 vs. 0%) nem volt szignifikánsan magasabb, mint a primer ptx-os csoportban. Következtetés: A szekunder spontán légmell az idősebb korosztályt érinti, az ápolási idő és mortalitás magasabb, mint a primer ptx miatt kezelteknél. Kezelés alapvetően a mellkasi drainage. Recidíva, a drainage eredménytelensége és ellenoldali korábbi pneumothorax esetén elsősorban VATS javasolt.

The authors aimed to elucidate the indications of transcervical surgical approach and also to review the literature in the treatment of symptomatic Zenker’s diverticulum (ZD), focusing on clear indications for open surgery (diverticulectomy with cricopharyngeal myotomy). The authors analyzed the efficacy of open surgery versus the current standard endoscopic diverticulotomy, focusing on the size of the diverticulum, surgical approach, and postoperative morbidity..Following a retrospective search in the literature using Medline and PuBMed database of the past 5 years, the authors found 79 articles about the treatment of symptomatic Zenker’s diverticulum (keywords: Zenker’s diverticulum, open surgery, endoscopic diverticulectomy). 25 papers were included, the exclusion criteria were small number of patients, non-english publications.Although endoscopic diverticulectomy is the preferred method of treatment in symptomatic Zenker’s diverticulum, in those cases, in which the patient is not eligible for endoscopic treatment, open surgery is a safe and reliable choice when treating a small ZD.

Background Abdominal compartment syndrome (ACS) is a life-threatening condition, of which pathomechanism hasn't been completely clarified, yet. Furthermore, surgical therapy still needs optimization. Objective We aimed to investigate microcirculatory and micro-rheological alterations in ACS, using various temporary abdominal closure methods, including three settings of vacuum-assisted closure technique (negative pressure wound therapy, NPWT). Methods On anesthetized pigs, by intraabdominally placed and filled-up silicone bags, intraabdominal pressure at 30 mmHg was maintained for 3 hours, and afterwards, decompressive laparotomy happened. In different experimental groups Bogota-bag or Vivano abdominal sets were applied (-50, -100, -150 mmHg) for 2 hours. Pressure monitoring was done by implanted sensors, hemorheological parameters were determined, and laser Doppler flowmetry tests were performed on the surface of intraabdominal organs. Results Treatment with Bogota-bag and -150 mmHg vacuum increased erythrocyte aggregation, while deformability declined. Blood viscosity increased after treatment with -150 mmHg vacuum. The microcirculatory parameters of the NPWT groups were better in small intestine. Conclusions ACS resulted in impairment of macro- and micro-rheological parameters and abdominal organs' microcirculation. All of the used techniques improved the results, however, applying Bogota-bag or -150 mmHg vacuum set showed worse microcirculatory and micro-rheological data than the settings at -100 or -50 mmHg.

A súlyos akut pancreatitist kísérő folyadékgyülem-képződéssel járó lokális szövődmények perkután drenázskezelése, főleg steril esetekben, nem széles körben alkalmazott eljárás. Jelen közlemény célja a perkután drenázs indikációjának, technikájának, korlátjainak és eredményének elemzése az irodalom alapján. A perkután drenázskezelés a súlyos akut pancreatitist kísérő akut folyadékgyülem, az akut pseudocysta és a hasnyálmirigytályog, valamint az elfolyósodott nekrózis kezelésében jelentős szerepet játszik. A fertőzött folyadékgyülemek kezelésére a perkután drenázs mint első kezelés ajánlható. A steril akut folyadékgyülem és pseudocysta kezelésében a drenázskezelés, a sterilitás szabályainak fokozott betartása mellett, a iatrogén fertőzés relatív gyakorisága miatt csak komoly panaszokat okozó esetekben ajánlott. Az elfolyósodott steril és szeptikus nekrózis kezelésében a legjobb eredménnyel a 20–30F vastagságú katéterekkel végzett drenázskezelés, a „sinus tract endoscopy” és a laparoszkóppal asszisztált necrectomia kecsegtet. Az esetek több mint 25%-ában a drenázs a konzervatív kezelés mellett a beteg végleges gyógyulásához vezet. Más esetekben a műtét időpontjának prolongálására, a „korai” műtét elkerülésére hasznos eljárás.

Absztrakt Esetismertetés: A vesetumorok áttétet általában a májba és a tüdőbe adnak. Más hasüregi szervben (duodenum, másik oldali vese, mellékvese, colon-transversum) való előfordulásuk ritka, a pancreasba történő áttétképződés irodalmi ritkaságnak számít. A szerzők sikeresen operált 82 éves férfi beteg esetét ismertetik, akinek világos sejtes vesetumor pancreas-metastasisát távolították el. A beteg anamnaesisében 8 évvel korábban bal oldali vesetumor miatt nephrectomia szerepel. Magas subileusra utaló tünetek miatt kezelt és kivizsgált betegnél a hasi CT-vizsgálat a pancreasfarok területén térfoglalást igazolt. Az elváltozást splenectomiával kiegészített distalis pancreasresectióval távolították el. A szövettani vizsgálat a világos sejtes vesetumor metastasisát igazolta. Megbeszélés: A pancreas szekunder daganata ritka. Bár a vesetumorok túlnyomórészt máj- és tüdőáttétet képeznek, a világos sejtes vesetumor a pancreasba is adhat áttétet, ezért a hasnyálmirigyben megjelenő térfoglalás esetén gondolni kell az áttétképződésre.

Intubation of the tracheobronchial tree is the most common type of malposition during the placement of narrow-bore enternal tubes.In addition to other treatment components in a 65-year-old female, nasojejunal feeding was started to treat her for severe acute pancreatitis. After the placement of the narrow-bore feeding tube, she developed dyspnea and huskiness. On auscultation and X-ray investigation, the right pneumothorax was detected and the tube was found in the chest cavity. The diagnosis was confirmed by bronchoscopy. Videothoracoscopic resection and closure of the lacerated lung, using a tube thoracostomy, were performed. The patient recovered after postoperative conservative treatment for her pancreatitis.Pneumothorax and laceration of the lung-caused by the malposition of narrow-bore enternal tube-can be successfully treated by applying videothoracoscopy.

The intercostal hernia of the lung is a very rare extraordinary disease that requires operation because of the complaints and potential complications. The authors review cases of their operations and analyze the subsequence and treatment. Three patients have been treated for intercostal lung hernia in our treatment. The causes of this disease were a previous thoracotomy in one case and fits of coughing in the other two cases. The diagnosis was set up on the grounds of the specific clinical symptoms, thoracic X-ray and CT scan. The hernia was dissolved with percostal stitches and with the suture of the thoracic musculature in two cases. Plastic operation of the thoracic wall by implanting a polypropylene surgical mesh (Prolen, Ethicon, Johnson & Johnson) was performed in the case of the third patient and later in the first two patients due to recrudescence. In one case the authors were constrained to resect the dystelectasial lung in the hernial sac. The three patients had been operated five times. Relapse of hernia was detected in two patients, in whom the intercostal space had been reconstructed with percostal stitches. We did not detect any relapsing in those two patients at 33 and 66 months after the second operation with mesh implantation. The third patient who got mesh implant immediately did not relapse 12 months after the operation. Intercostal lung hernia is an indication of operation. A plastic operation of the thoracic wall combined with the implantation of a surgical mesh is recommended to close the hernial orifice, which is suitable for treating both primary and relapsed hernias. Recurrence is rare in those patients treated with this method.

Primary angiosarcoma of the pancreas is an extremely rare neoplasm that often mimicks severe acute pancreatitis. A 58-year-old man was admitted with clinical and laboratory signs of severe acute pancreatitis. Contrast enhanced CT scan demonstrated haemorrhagic necrotizing inflammation of the pancreas involving the pancreatic tail, splenic hilum and small bowels with multiple peripancreatic and free abdominal fluid collection. Percutaneous drainage was performed. After 13 days, laparotomy was indicated because of persistent intra-abdominal bleeding, fever and a palpable, rapidly growing mass in the left upper quadrant of the abdomen. During the operation a necrotic, haemorrhagic mass was found in the pancreatic tail; a frozen section showed malignancy, although the tumour was unresectable. Despite all conservative and surgical therapeutic attempts, the patient died within four weeks after diagnosis. Final histology justified primary angiosarcoma of the pancreas. If a patient with signs of severe acute pancreatitis has fever without elevated PCT, the presence of a malignant tumour of the pancreas should be considered.

In 1968 R. E. Fear first reported a trocar site hernia (TSH) in his large series on laparoscopy. Currently, the incidence of TSH is estimated to be 0.65-2.80%. Ports ≥10-mm are usually closed, but ports of the 5-mm trocars are always left open, which may lead to herniation.Authors guided teaching courses for hands-on animal laparoscopic cholecystectomy (LC) operations, where trainees performed LC-s on 60 animals. Two and four weeks following the operations the animals underwent second look laparoscopy to detect adhesion formation.Trocar site herniation was observed, and in 20% of the animals herniation was found. 70% of the hernias were situated in the 5-mm ports and 30% in the 10-mm ports.Port sites should be closed to prevent the formation of TSH. Attention should be payed on the closure of 5-mm trocar sites as well.Bevezetés/célkitűzés: 1968-ban R. E. Fear először közölt tanulmányt a laparoscopos „Port site” herniáról (Trocar site hernia, TSH), jelenlegi előfordulási gyakorisága eléri a 0,65–2,80%-ot. A sebészi laparoscopia elterjedésével a beavatkozások során szokássá vált, hogy a 10 mm-nél kisebb trokársebeket nem zárják be. Ugyanakkor egyre több közlemény foglalkozik az 5-6 mm-es trokár (továbbiakban: KT) helyén keletkező sérvek elzáródásával. Anyag és módszer: A szerzők oktatási célból 60 kutyán végzett laparoscopos cholecystectomia műtét után 2 és 4 héttel relaparoscopiát végeztek, és posztoperatív adhaesiók után kutatva azt is figyelték, hogy a laparoscopos trokárok intraabdominalis behatolási helyein vannak-e sérvek, különös tekintettel a KT-ra. Eredmények: A 60 kísérleti állat 20%-ában volt látható intraabdominalis sérv, és a sérvek 70%-a a KT helyein helyezkedett el. Következtetések: A megszokott gyakorlattal ellentétben az irodalomban egyre többen hangoztatják a nagy trokárok és a KT ejtette teljes vastagságú hasfali sebek zárásának szükségességét, megelőzendő a sérvek kialakulását és kizáródásuk lehetőségét. Tanulmányunkban a technikai és klinikai lehetőségeket mutatjuk be.

Intermittent compression of the structures of the hepatoduodenal ligament, is often performed during liver surgery. As a result, changes in hepatic blood supply and consequent reperfusion induced tissue damages will develop. Ischemia-reperfusion injury, which occur in local and distant regions, influence outcome of hepatic surgery, and it is in close correlation with the duration of hypoxia during the intervention. In animal model the effect of Baron/Pringle manoeuvre was investigated in terms of changes in liver function tests and histology. The study was carried out on 12 Beagle dogs, clamping of the hepatoduodenal ligament for 3×15 minutes then half an hour reperfusion was performed followed by blood and tissue sampling. Significant histological changes were observed both in the liver as well as the small intestine. In terms of liver function changes, GPT elevation occurred the earliest, GOT and LDH were also increased at the end of the 30 minutes reperfusion. In this animal model, the third 15 minutes compression turned out to be too long. Elevation in GPT levels was the most sensitive marker.Májműtétek során gyakran szükséges az ellátó erek hosszabb-rövidebb idejű leszorítása, ezáltal a szerv vérellátása átmenetileg zavart szenved, majd az ezt követő reperfusio során további károsodások jöhetnek létre. Ezek hatására az adott szervben is és más lokalizációban is anyagcsere- és morphologiai változások jöhetnek létre, amelyek meghatározzák a szervek, szövetek további sorsát, életképességét, regenerációját, ezáltal a műtét kimenetelét is. Állatkísérletes modellben vizsgáltuk a Báron/Pringle manőver hatását a májfunkció és szövettani károsodások szempontjából. 12 beagle kutyán 3×15 perces ligamentum hepatoduodenale leszorítás során, majd 30 perces reperfusio után vettünk vér- és szövettani mintákat. Markáns szövettani változásokat észleltünk mind a máj, mind a vékonybelek területén, valamint a májenzimek közül. Legkorábban a GPT-emelkedés jelentkezett, majd a 30 perces reperfusiót követően a GOT és a LDH is megemelkedett. Állatkísérletes modellen hosszúnak bizonyult a harmadik 15 perces leszorítás, legérzékenyebben a GPT-emelkedés jelezte az ischaemiát.

The incidence of intraabdominal hypertension or abdominal compartment syndrome, as the more severe form is called, is relatively high in patients with severe acute pancreatitis, and therefore more attention is needed to the topic. If conservative treatment fails, immediate surgical decompression is indicated. The most commonly used operation is a full thickness median laparotomy, but a transversal laparotomy may also be effective. Although subcutaneous linea alba, or bilateral anterior rectus fasciotomy is safe and effective, decompressive laparotomy is indicated in failure of these methods. The open abdomen therapy is not advised due to high morbidity. Primary closure of the abdomen is preferable.

According to the Atlanta Classification in many cases of acute pancreatitis there are three well-defined fluid collections: acute peripancreatic fluid collection (APFC), the so-called postnecrotic pancreatic/peripancreatic fluid collection which develops in the region of liquified pancreatic necrosis, and the pseudocyst that develops in the late phase of the illness. In many cases, these anatomic entities can be succesfully treated with radiological interventional methods. It is difficult to correctly interpret the articles that review the treatment of numerous patients because the nomenclature is unclear. The aim of this article is to analyse the indications, limits and results of the listed complications treated by percutaneous drainage (PD) which aggravate acute pancreatitis on the basis of the literature.

Although microcirculatory disturbances play pivotal role in the pathomechanism of acute pancreatitis (AP), very few papers can be found which had been tested any of hemorheological parameters. The aim of our study was to analyze the hemorheological changes in cerulein-induced experimental acute pancreatitis in rat in two doses (5 and 10 μg/kg, s.c.). Male and female rats were subjected to Control group, or AP with 5 or 10 μg/kg cerulein groups. Blood samplings (lateral caudal vein) were completed before cerulein administration, and 1, 2 and 24 hours later. Hematological parameters, amylase activity, erythrocyte deformability (ektacytometry) and aggregation (light-transmission method) were tested. The presence of AP could be confirmed by amylase testing and histological examination. The earliest impairment of the red blood cell deformability could be observed 1 hour after cerulein administration in 10 μg/kg dosage. Female animals had the worst rheological results with high mortality. In conclusion, subcutaneously administrated cerulein in dosage of 5 and 10 μg/kg resulted in AP in rats, with significant changes in red blood cell deformability and alterations in erythrocyte aggregation. This model seems to be suitable for further comparative studies.

Absztrakt A súlyos acut pancreatitist gyakran kíséri hasűri nyomásfokozódás és annak súlyos formája, az abdominalis compartment-syndroma, ezért a betegség kezelésekor gondolni kell a kialakulására. Mivel a betegség korai stádiumában végzett műtétek morbiditása és mortalitása magas, csak a konzervatív kezelés eredménytelensége esetén, de mielőbb el kell végezni a dekompressziós műtétet. A műtéti dekompresszióra általában median laparotomiát alkalmaznak, de a haránt laparotomia is hasonlóan eredményes. A subcutan linea alba vagy a kétoldali anterior rectus fasciotomia is jó eredménynyel végezhető, azonban ennek eredménytelensége esetén dekompressziós laparotomia javasolt. A nyitotthas-kezelés a gyakori szövődmények miatt nem ajánlott. Törekedni kell a hasfal mielőbbi végleges zárására.

Authors present two cases of successfully operated patients with retrohepatic inferior vena cava (IVC) injury. In the first case a 79 year old female patient suffered from multiple stab wounds in the area of the 4th segment of the right lung as well as in the retrohepatic region in close proximity to the IVC. At the time of the first surgery the IVC injury was not identified. During a second operation, however, the IVC was isolated from the liver using an anterior dissection of the parenchyma with finger-fracture technique and the injury was oversawn finally. Successful haemostasis of the liver was achieved by packing of the perihepatic space, which was removed three days later. In the second case a 25 year-old male patient had suffered blunt abdominal trauma. He underwent laparotomy and packing on the site of the hepatic injury, which had to be repeated seven more times. Later on, another urgent laparotomy was carried out for recurring intraabdominal bleeding and bile leakage, and cholecystectomy was performed due to gallbladder perforation. Another two days later a further emergency laparotomy was indicated for ongoing intraabdominal bleeding, when the bleeding source - an injury (3 mm in diameter) of the retrohepatic IVC - was oversawn. After relatively long postoperative stay both patients were fully recovered and discharged from hospital.penetrating injuries of the IVC are associated with high mortality rate of approximately 78%. Proper management of these injuries requires experience in both vascular and liver surgery. Retrohepatic IVC injury needs to be considered in the differential diagnosis of ongoing bleeding in penetrating or blunt liver trauma. Packing of the liver is a reliable haemostatic method if bleeding persists due to division of the liver parenchyma.

Absztrakt Esetismertetés: A szerzők két sikeresen operált beteg esetét ismertetik. A 79 éves anticoagulált nőbeteg többszörös mellkasi és hasi szúrt sérülést szenvedett, a jobb tüdő IV. szegmentumának és a vena cava inferior retrohepaticus szakaszának szúrt sérülését látták el. Az első műtét során a cava sérülését nem ismerték fel. A második műtét során a máj anterograd, finger-fracture technikával történt szétválasztását követően sikerült a cava sérülését feltárni és elvarrni. A máj állományának sebét perihepaticus packing módszerrel tamponálták. A 3. posztoperatív napon újabb műtét során a tampon eltávolítását végezték. A beteg meggyógyult. Egy másik, 25 éves férfi beteg tompa hasi sérülést szenvedett. Első ellátásként a májruptura tamponálása történt. Ezt követően sorozatos (7×) tamponcserékre került sor. Ismételten jelentkező hasüregi vérzés és epefolyás miatti reoperációkor az epehólyagon lévő kb. 1 mm-es perforatiós nyílás miatt cholecystectomia történt. Két nappal később ismételt vérzés miatt végzett haematomaevakuálás után a vena cava inferior retrohepaticus szakaszán lévő, kb. 3 mm-es sérülés elvarrása történt. Posztoperatív epefolyás miatt EST történt. A beteg gyógyultan otthonába távozott. Megbeszélés: Az áthatoló retrohepaticus vena cava sérülések mortalitása magas, mintegy 78%-ra tehető. A sérülés ellátása nemcsak érsebészeti, hanem májsebészeti jártasságot is igényel. A máj szúrt és tompa sérülésének ellátása során gondolni kell a vena cava inferior retrohepaticus szakaszának sérülésére is. A sérülés elvarrása gyógyuláshoz vezet. A parenchyma szétválasztása utáni vérzés perihepaticus „packing” módszerrel megbízhatóan kezelhető. A posztoperatív epefolyás EST-vel sikeresen kezelhető.

Distant spread from renal cell carcinoma is commonly found in the liver and lung. Metastatic involvement of any other gastro-intestinal organ (duodenum, other kidney, adrenal gland) is unexpected. However, clear cell renal carcinoma is known to cause pancreatic metastasis.The authors present the case of a successfully operated 82- year-old man, who was operated for a metastatic tumor in his pancreas. 8 years prior to his current hospitalization, a left sided nephrectomy was performed for renal cell carcinoma. The CT scan revealed a tumor localised in the tail of the pancreas. Distal pancreas resection was performed with splenectomy. Histology revealed clear cell renal carcinoma metastasis.pancreatic tumors are mostly primaries. Renal cell carcinoma generally gives hepatic and pulmonary metastases. However, clear cell renal carcinoma is known to give pancreatic metastasis, too.

Videothoracoscopy plays a leading role in the management of spontaneous pneumothorax. This study evaluates various treatment strategies based on early results.In a five-year period 243 (184 male, 59 female, mean age: 37.1 years) patients were treated with spontaneous pneumothorax in 302 cases. In case of partial ptx observation was used in 24 (8%) and aspiration in 6 (2%) patients. Chest drain was inserted in 241 (67.6%) cases. Indications for surgery were recurrence of disease, previous contralateral pneumothorax and failure of drainage. Videothoracoscopy was indicated in 71 (23.5%), Jakoscopy in 5 (1.7%), thoracotomy in 13 (4.3%) cases. The operation was completed with partial pleurectomy in 25, talcum or mechanical pleurodesis in 32 and 3 cases, respectively.Conservative treatment was successful in 24 (80%) of the 30 conservatively treated cases, while chest drainage succeeded in 204 (84.7%) of the 241 cases. Conversion was needed in 8 (11.3%) cases of the 71 VATS. The remaining 63 patients recovered. Thoracotomy and Jakoscopy were successful in all cases. The postoperative complication rate was 6.3% after VATS, and 7.7% after thoracotomy. Reoperation was performed because bleeding in one case after VATS and thoracotomy. In one case empyema, and in another patient pneumonia developed after VATS. Postoperative bleeding occurred in one case after thoracotomy. The mean hospitalization was 8.5 days after drainage, 9.1 days after VATS and 11.3 days after thoracotomy. The postoperative mortality rate was 1.3% (4 patients).In case of spontaneous pneumothorax the first choice of therapy is chest tube drainage. VATS is indicated in case of recurrence, failure of drainage and previous contralateral pneumothorax.

Az akut pancreatitis viszonylag gyakran előforduló betegség. Évenként 5 és 80 közötti új megbetegedés esik 100 000 lakosra, és ez a szám az utóbbi években növekedést mutat a különböző tanulmányok szerint. A két fő etiológiai faktor az alkohol és az epekövesség. Az alkoholos eredetű hasnyálmirigy-gyulladás a férfiak körében, a biliaris pancreatitis a nők körében gyakoribb. Célkitűzés: a szerzők súlyos akut pancreatitis miatt kezelt betegek adatait elemezik, arra keresve a választ, hogy milyen eltérő sajátságokkal rendelkezik a biliaris és a más etiológiájú pancreatitis. Betegek és módszer: a szerzők 139, súlyos akut pancreatitisben szenvedő beteget kezeltek, akiket etiológiai szempontból két csoportra osztottak: az epeköves (A csoport) és a nem epeköves (B csoport) talajon kialakult súlyos hasnyálmirigy-gyulladásban szenvedők csoportjára. Összehasonlították a két csoport nem és kor szerinti sajátosságait, a mortalitási és a morbiditási adatokat, a műtétek és az ápolási napok számát. A statisztikai analízishez a χ 2 -tesztet alkalmazták. Eredmények: a szövődmények az epés (A) csoportban súlyosabbak voltak, mint a nem epés (B) csoportban. A halálozás az A csoportban 17,8%-nak, a B csoportban 13,8%-nak bizonyult, míg az átlagos 15,1% volt. A mortalitás a nők körében az A csoportban szignifikánsan magasabb volt. Következtetések: a súlyos epés akut pancreatitisben szenvedő nőbetegek esetében súlyosabb lefolyással, magasabb halálozási aránnyal kell számolnunk. A szerzők az idős, egyéb betegségekkel terhelt epeköves nőknél még a szövődmények jelentkezése előtt elektív cholecystectomiát javasolnak.

The authors analyze the possibilities for the delay of surgery with special consideration regarding percutaneous peripancreatic drainage in the treatment of acute necrotizing pancreatitis.In addition to intensive care therapy, 61 patients were also given antibiotic prophylaxis, and early nasojejunal enteral feeding was commenced. In a total of 22 cases where peripancreatic fluid was found, percutaneous drainage was performed. Septic necrosis, sepsis, multi-organ failure not resolving with conservative treatment, gastrointestinal perforation, and bleeding were the indications for operation. Only 9 patients underwent surgery within one week and in 40 patients delayed (more than 7 days) necrectomy was performed. Following surgery, closed omental bursa rinsing was performed.Five patients were cured with only conservative therapy and 7 others were cured under the influence of percutaneous drainage. In 15 patients it was possible to delay surgery using percutaneous drainage with combination of conservative treatment. A total of 39 reoperations occurred due to septic focus, bleeding, colonic necrosis and gastric perforation. The average days of nursing care was 43.3 (3-120). Mortality was 16.4% (10/61 patients).The number of early operations can be reduced with the use of antibiotic prophylaxis, nasojejunal feeding and percutaneous drainage.

Successful treatment of penetrating chest trauma largely depends on the accurate and rapid diagnostic work-up, as well as the adequate surgical management. The authors discuss current issues in the diagnosis and the treatment of penetrating chest injuries based on the analysis of 109 cases.82 men and 27 women with penetrating chest trauma were studied. The average age of the patients was 37.8 years. The injury was caused by stabbing in 104 cases (95.4%), gunshot in 4 patients (3.7%) and explosion in one case (0.9%). 41 patients had cardiac and pericardial injuries. In those, 19 (46.3%) patients had a chest X-ray, echocardiography was done in nine cases (22%), while CT scan and diagnostic VATS were performed in two patients, respectively. All patients underwent surgery except one, who was treated conservatively.In all of the 68 patients, who had no cardiac injuries, a chest X-ray was performed. Echocardiography was done in six (8.8%) cases, diagnostic VATS in four (5.9%) patients, and abdominal ultrasound scan in 3 (4.4%) cases. Chest tube was inserted in 13 patients (19.1%), an open surgery was performed in 51 cases, while in 4 cases VATS was carried out.In the group of patients with cardiac and pericardial injuries, the sensitivity of the chest X-ray, echocardiography and VATS were 57.9%, 88.9% and 100%, respectively. Further, specificity of the above were 26.3%, 88.9% and 100%, respectively. However, in patients with non-cardiac injuries, the sensitivity of the chest X-ray was 100%, and both the specificity and sensitivity of VATS was 100%. Postoperative complication rate was 12.6% overall (15% in cases with cardiac injury and 10.9% in the non-cardiac subgroup). Mortality rate was 7.3% among the patients with cardiac injury, while there was no mortality detected in the non-cardiac subgroup. The average mortality rate was 2.8%.Patients with penetrating chest trauma should undergo a rapid and accurate diagnostic work-up followed by an adequate surgical management in order to keep their prognosis relatively good.

Video assisted thoracoscopy (VATS) is a more and more frequently used method in the diagnosis and treatment of blunt thoracic trauma. In some cases it has diagnostic and in others therapeutic effect. The authors analyze the role of VATS in the diagnosis and the treatment of 83 patients treated with haemothorax.There were 83 patients treated (60 male, 23 female, mean age of 54.4 years) with haemothorax caused by blunt thoracic trauma. Chest tube drainage was used in 31 (37.3%) cases. Urgent thoracotomy was performed in only two (2.4%) cases. Elective, planned VATS was used in 11 (13.3) cases. All of these 3 were diagnostic and 8 were therapeutic procedures. In three cases rupture of the diaphragm was diagnosed with the use of VATS which were treated through thoracotomy. In the remaining 8 cases haematoma evacuation and in 3 cases intercostal artery bleeding were treated with VATS.No complications related to the procedure were observed. The thoracotomy in all 3 cases verified the diaphragmatic injuries. The 8 patients undergone therapeutic VATS recovered. The mean hospital stay after VATS was 7.8 days and 11.3 days after thoracotomy in the patients with diaphragmatic injury.The VATS has a significant role in the diagnosis of blunt thoracic, especially of diaphragmatic, injuries. In other cases the VATS has good therapeutic effect.
