1. Reasons and extent of delay in the diagnosis of pulmonary tuberculosis after the appearance of symptoms

Abstract

Objective: Delay in the diagnosis of tuberculosis (TB) is a significant problem at both individual and community levels. Delayed diagnosis of TB contributes to more severe disease manifestations, higher risk of death, and higher disease transmission in the community. We conducted this study to assess the extent and associated reasons for delay in diagnosis of pulmonary TB. Methods: This study was conducted in the Department of TB and Respiratory Diseases, J. N. Medical College, Aligarh, from June 2020 to May 2022. A total of 2053 new pulmonary TB patients, who first consulted any private healthcare provider (HCP) for treatment, were enrolled in the study. The required information was collected by interview technique using a predesigned questionnaire. Results: A total of 2053 patients were enrolled in the study. There was a significant delay of more than 2 weeks in the diagnosis of pulmonary TB after the onset of symptoms in 94% of patients. The extent of delay ranged from 8 days to 240 days with a mean of 36.33 days. The delay in visiting the HCPs by the patient was not significant. Only 5.85% of patients had a significant delay of more than 2 weeks in seeking any health care after the appearance of symptoms. A delay by HCP contributed to a greater portion of the total delay, with a mean of 31.77 days. The main reason for the delay by HCPs was not investigating TB. The hospital delay was not significant. The mean hospital delay was 5.82 days. Conclusion: The delay in the diagnosis of TB in India is very high. A delay by HCPs contributes to a greater portion of the total delay. Information, Education and Communication (IEC) activities will play an important role in reducing the delay. All HCPs should be actively involved in subjecting the suspects to TB diagnosis at the earliest possible as per National Tuberculosis Elimination Programme (NTEP) guidelines.

Abstract

Autoimmune Haemolytic Anaemia (AIHA) is a rare haematological disorder characterised by autoantibodies directed against autologous red blood cells. It can be idiopathic or secondary and classified as Warm type AIHA (WAIHA), cold, or mixed. The primary treatment for WAIHA is a long course of steroid administration, which has an early response rate of 80-90%. However, up to 20-30% of patients require second-line therapy. In the last decade, rituximab has replaced splenectomy as the first-choice therapy for refractory WAIHA patients due to its efficacy and safety. There is a paucity of reported cases of WAIHA refractory to both steroids and rituximab that have responded to splenectomy. This is a case of a 35-year-old female with a history of jaundice and anaemia for the past two years. The patient was diagnosed with WAIHA with a Direct Antibody Test (DAT) positive, IgG positive, and C3d negative results, and massive splenomegaly that was initially refractory to steroids and rituximab but responded to splenectomy. Significant splenomegaly may be an indication for choosing splenectomy over rituximab as a second-line treatment in steroid-refractory WAIHA.

Abstract

Endobronchial tuberculosis (EBTB) is characterized by tuberculosis infection of the tracheobronchial tree. It has variable presentation but tumorous growth-like presentation in bronchus is very rare. The clinical and radiological features are non-specific, which creates a diagnostic dilemma. Bronchoscopy and biopsy of the lesion are mandatory to confirm the diagnosis. In this case series, we are presenting three unique cases of endobronchial growth diagnosed as EBTB after biopsy and evaluation of bronchoalveolar lavage (BAL) with cartridge-based nucleic acid amplification test (CBNAAT) and other ancillary investigations for tuberculosis. Four patients presented to the outpatient department with non-specific symptoms of fever, cough, hoarseness of voice, and hemoptysis. They were evaluated with chest radiograph (CXR), contrast-enhanced computed tomography (CECT) thorax, and bronchoscopy. Bronchoscopy revealed growth in the bronchus in all three cases. A biopsy was taken and BAL was performed. All cases turned out to be EBTB in histopathological examination and BAL CBNAAT. They were treated with anti-tubercular drugs and all responded well to treatment. Endobronchial tuberculosis presenting as tumorous growth in the tracheobronchial tree is rare. There should be a high index of suspicion while dealing with patients with non-specific clinical and radiological features of tuberculosis. EBTB can be misdiagnosed as malignancy in most cases. Therefore, it should be kept as a differential diagnosis while encountering a mass lesion in the trachea or bronchus during bronchoscopy.

4. Prosocial and Active Learning (PAL) Classrooms Evaluation. Final Summative Report

Abstract

Prosocial and Active Learning (PAL) Classrooms is a year-long teacher professional development program designed to increase students' prosocial behavior and engagement in 5th grade mathematics and science classrooms that use active, team-based lessons by altering the way that teachers interact with students using research-based strategies. The project is funded by a 6-year Education Innovation and Research grant and implemented by the eMINTS National Center and the Prosocial Development and Education Research Lab (ProsocialEd Lab) at the University of Missouri. The American Institutes for Research® (AIR®) as an independent evaluator, has completed an implementation and impact study of PAL Classrooms. The evaluation involves a mixed-methods study to assess the implementation of the program and a multicohort, school-level randomized controlled trial (RCT) to examine the impact of the program. The evaluation took place in 41 districts in three states--Arkansas, Kansas, and Missouri. For the impact study, AIR randomly assigned two cohorts (2021-22 and 2022-23) of elementary schools to receive PAL Classrooms immediately (treatment) or conduct treatment as usual and receive PAL Classrooms a year later (control). A total of 41 schools (21 treatment and 20 control), with 65 teachers and their 1,399 students, participated in the RCT. This final report summarizes the PAL Classrooms program and AIR's evaluation methods; findings on the extent to which PAL Classrooms' key components were implemented with fidelity; and the impact of PAL Classrooms on teacher outcomes (i.e., student-reported use of strategies, instructional quality), proximal student outcomes (i.e., prosocial behavior, engagement, perceived classroom climate, collaboration, teacher-student relationship), and student math and science achievement. Results of implementation analyses indicated that PAL Classrooms was generally implemented with fidelity despite occurring during the COVID-19 pandemic The program delivered key professional development activities and teachers generally participated as intended. In addition, teachers implemented strategies that promoted student prosocial behavior. Results were consistent across both cohorts. Results of impact analyses indicated that teachers assigned to PAL Classrooms had higher levels of overall instructional quality, student engagement, emotional support, and instructional support, measured by classroom observation. However, most of the impact estimates were not statistically significant, except for the estimate of impact on emotional support (p = 0.05), with an effect size of 0.58. In contrast, impacts on teachers' use of strategies such as praise or induction measured by student report were smaller, but still positive. Results reveal that PAL Classrooms had statistically significantly positive impact on proximal student outcomes measured by self-report surveys. Students assigned to PAL Classrooms reported higher levels of prosocial behavior in their peers and teachers, with effect sizes of 0.23 and 0.20 standard deviations (SD), respectively, compared with students in control schools. Analyses of students' self-report of prosocial behavior, engagement, classroom climate, and student-teacher relationships also yielded positive results, although the impact estimates were not statistically significant. Impacts on student-reported collaboration and more distal student achievement in math and science were less conclusive, with the impact estimates being smaller and not statistically significant. Additionally, the impact results on student achievement did not vary by student background characteristics (i.e., free or reduced-price lunch eligibility, or receipt of academic services under an individualized education plan).

Abstract

Stable $^{205}$Tl ions have the lowest known energy threshold for capturing electron neutrinos ($\nu_e$) of ${ E}_{\nu_e}\ge50.6$\,keV. The Lorandite Experiment (LOREX), proposed in the 1980s, aims at obtaining the longtime averaged solar neutrino flux by utilizing natural deposits of Tl-bearing lorandite ores. To determine the $\nu_e$ capture cross section, it is required to know the strength of the weak transition connecting the ground state of $^{205}$Tl and the 2.3 keV first excited state in $^{205}$Pb. The only way to experimentally address this transition is to measure the bound-state beta decay ($\beta_{b}$) of fully ionized $\mathrm{^{205}Tl^{81+}}$ ions. After three decades of meticulous preparation, the half-life of the $\beta_{b}$ decay of $\mathrm{^{205}Tl^{81+}}$ has been measured to be $291_{-27}^{+33}$ days using the Experimental Storage Ring (ESR) at GSI, Darmstadt. The longer measured half-life compared to theoretical estimates reduces the expected signal-to-noise ratio in the LOREX, thus challenging its feasibility.

Abstract

Introduction: Lung cancer is one of the most common malignancies to occur worldwide. Two main subtypes of lung cancer include small cell lung cancer and Non Small Cell Lung Cancer (NSCLC). Patients with advanced stage NSCLC who achieve good response with Tyrosine Kinase Inhibitors (TKI) have been found to have Epidermal Growth Factor Receptor (EGFR) mutation. The first biomarker identified for targeted treatment in lung cancer was EGFR and patients of NSCLC with EGFR mutation have superior survival outcome when treated with targeted therapy as compared to conventional chemotherapy. Aim: To compare the outcome of targeted therapy to mutation to EGFR and conventional therapy in non mutant lung cancer patient of NSCLC. Materials and Methods: The present longitudinal study was conducted in the Department of TB and Respiratory Diseases, Jawaharlal Nehru Medical College and Hospital, Aligarh, Uttar Pradesh, India from July 2017 to November 2019 on a sample size of 80. Patients diagnosed with NSCLC and EGFR mutation status were included in the study. They were started on TKI if tumour was EGFR positive and on conventional chemotherapy (cisplatin plus paclitaxel) if no mutation was detected on histopathology. Among the study group, 35 patients were EGFR positive and started on geftinib (group I), 45 were EGFR negative and received platinum-based chemotherapy (group II). Outcomes were measured in terms of progression-free survival, Overall Survival (OS), and toxicities. Statistical analysis of data was done using Statistical Package for the Social Sciences (SPSS) version 20.0. Results: Among the study group, 35 patients were EGFR positive and started on geftinib (Group I), 45 were EGFR negative and received platinum-based chemotherapy (group II). The mean age of EGFR positive patients was 58.91 years and for EGFR negative patients was 60.11 years. In group I, there was no complete response while 28.5% had partial response, 45.5% had stable disease and 25.7% had progressive disease. In group II, 15.5% patients had complete response, 33.3% had partial response,17.7% had stable disease and 33.3% had progressive disease. Mean progression-free survival in group I (5.65 months) was significantly higher than group II (4.26 months). The mean OS in group I (7.85 months) was slightly higher than group II (6.72 months). Both haematological and non haemaotlogical toxicities were significantly higher in group II. Conclusion: Patients with EGFR positive expression subjected to Gefitinib had significant mean progression-free survival with an acceptable range of non haematological toxicities and no haematological toxicities, as compared to the EGFR negative patients on conventional chemotherapy.

Abstract

In the Felsenkeller shallow-underground site, protected from cosmic muons by a 45 m thick rock overburden, a research laboratory including a 5 MV Pelletron ion accelerator and a number of radioactivity-measurement setups is located. The laboratory and its installations are described in detail. The background radiation has been studied, finding suppression factors of 40 for cosmic-ray muons, 200 for ambient neutrons, and 100 for the background in germanium $\gamma$-ray detectors. Using an additional active muon veto, typically the background is just twice as high as in very deep underground laboratories. The properties of the accelerator including its external and internal ion sources and beam line are given. For the radioactivity counting setup, detection limits in the 10$^{-4}$ Bq range have been obtained. Abstract

Increasing complications due to bacterial and viral infections require novel antimicrobial approaches. One emerging strategy is that based on catalysts able to selectively deliver reactive oxygen species (ROS) without leaching of other substances. In particular, metal oxide thin films activated by daylight can produce ROS by simply catalyzing oxygen and water molecules. This study examines plasma technology, combining deposition and oxidation processes, as well as plasma polymerization, to obtain functionalized AgOx-doped titanium oxide (TiOx) catalytic materials. The high-energy conditions in the reactive, ionized gas enable the intrinsic formation of a large number of reactive sites at defects and interfaces between the metal oxide nanostructures. Furthermore, plasma functionalization with nanoporous SiOx films (up to 100 nm thick) allows to precisely control the ROS delivery as well as unravel ROS formation mechanism at the metal oxide interface. Combining fluorescence spectroscopy and electron paramagnetic resonance, the controlled delivery of superoxide anion and singlet oxygen has been tuned based on the thickness of the nanoporous functional layer. 9. Comparative Study of Clinical and Radiological Profile and Outcome of COVID-19 Patients with and without Co-morbidities: A Cross-sectional Study

Abstract

Introduction: Coronavirus Disease-2019 (COVID-19) had devastating effects on the healthcare and economic sector worldwide. India stands second in the list of most number of COVID-19 cases. Most of the deaths due to COVID-19 were seen in patients with associated co-morbidities like hypertension, diabetes, chronic kidney disease and obesity. This study would like to examine specific co-morbidities in relation to the COVID-19 disease progression and outcomes. Aim: To compare the clinicoradiological profile and outcome of COVID-19 in patients with and without co-morbidities (diabetes and hypertension). Materials and Methods: The present observational, cross-sectional study was conducted at Jawaharlal Nehru Medical College, Aligarh, Uttar Pradesh between June 2020 to September 2020, after obtaining Institutional Ethics Committee (IEC) approval. A total of 148 patients with COVID-19 were included in the study. The COVID-19 patients admitted in the hospital were divided into four groups as those having only diabetes, only hypertension, both diabetes and hypertension, and those without any co-morbidities. Those with any other co-morbidity were excluded from the study. The general clinical characteristics, laboratory parameters, disease severity, morbidity and mortality were compared among various groups and the data was analysed. Categorical data were analysed using the Chi-square test. Results: A total of 148 patients with COVID-19 were included in the study, of which 26 patients were diabetic, 36 were hypertensive, 24 were both hypertensive and diabetic and 62 patients did not had any significant co-morbidity. Severe COVID-19 disease was most commonly observed in those with diabetes 14 (53.8%). The highest proportion of patients requiring oxygen (84.6%) and Non Invasive Ventilator (NIV) support (46.1%) was also seen among diabetics. The presence of diabetes, severe disease and leukocytosis at presentation increased the risk of mortality. The association of hypertension with COVID-19 does not seem to affect the in-hospital mortality. Conclusion: The COVID-19 in diabetics is associated with both increased risk of severe disease and increased odds of death. In diabetics, those with uncontrolled diabetes were more prone to severe disease and death than those with good glycaemic control. Hypertension, on the other hand, showed no association.

Abstract

The XLZD collaboration is developing a two-phase xenon time projection chamber with an active mass of 60 to 80 t capable of probing the remaining WIMP-nucleon interaction parameter space down to the so-called neutrino fog. In this work we show that, based on the performance of currently operating detectors using the same technology and a realistic reduction of radioactivity in detector materials, such an experiment will also be able to competitively search for neutrinoless double beta decay in $^{136}$Xe using a natural-abundance xenon target. XLZD can reach a 3$\sigma$ discovery potential half-life of 5.7$\times$10$^{27}$ yr (and a 90% CL exclusion of 1.3$\times$10$^{28}$ yr) with 10 years of data taking, corresponding to a Majorana mass range of 7.3-31.3 meV (4.8-20.5 meV). XLZD will thus exclude the inverted neutrino mass ordering parameter space and will start to probe the normal ordering region for most of the nuclear matrix elements commonly considered by the community., Comment: 29 pages, 7 figures

Abstract

This report describes the experimental strategy and technologies for a next-generation xenon observatory sensitive to dark matter and neutrino physics. The detector will have an active liquid xenon target mass of 60-80 tonnes and is proposed by the XENON-LUX-ZEPLIN-DARWIN (XLZD) collaboration. The design is based on the mature liquid xenon time projection chamber technology of the current-generation experiments, LZ and XENONnT. A baseline design and opportunities for further optimization of the individual detector components are discussed. The experiment envisaged here has the capability to explore parameter space for Weakly Interacting Massive Particle (WIMP) dark matter down to the neutrino fog, with a 3$\sigma$ evidence potential for the spin-independent WIMP-nucleon cross sections as low as $3\times10^{-49}\rm cm^2$ (at 40 GeV/c$^2$ WIMP mass). The observatory is also projected to have a 3$\sigma$ observation potential of neutrinoless double-beta decay of $^{136}$Xe at a half-life of up to $5.7\times 10^{27}$ years. Abstract

We present a novel deep learning pipeline to perform a model-independent, likelihood-free search for anomalous (i.e., non-background) events in the proposed next generation multi-ton scale liquid Xenon-based direct detection experiment, DARWIN. We train an anomaly detector comprising a variational autoencoder and a classifier on extensive, high-dimensional simulated detector response data and construct a one-dimensional anomaly score optimised to reject the background only hypothesis in the presence of an excess of non-background-like events. We benchmark the procedure with a sensitivity study that determines its power to reject the background-only hypothesis in the presence of an injected WIMP dark matter signal, outperforming the classical, likelihood-based background rejection test. We show that our neural networks learn relevant energy features of the events from low-level, high-dimensional detector outputs, without the need to compress this data into lower-dimensional observables, thus reducing computational effort and information loss. For the future, our approach lays the foundation for an efficient end-to-end pipeline that eliminates the need for many of the corrections and cuts that are traditionally part of the analysis chain, with the potential of achieving higher accuracy and significant reduction of analysis time., Comment: 10 Figures, 3 Tables, 23 Pages (incl. references)

13. Bronchial Hyper-responsiveness in Post-tubercular Patients: A Case-control Study

Abstract

Introduction: Bronchial hyper-responsiveness is the manifestation of excessive bronchoconstriction in response to diverse types of stimuli both physical and chemical. It is the most characteristic feature of bronchial asthma; it also occurs in a spectrum of other diseases like Chronic Obstructive Pulmonary Disorders (COPD) and reactive airway syndrome and may be provoked by a variety of stimuli like histamine and methacholine. Patients of healed pulmonary Tuberculosis (TB) show varying extent of lung impairment such as fibrosis, collapse, emphysema and broncho alveolar destruction. Aim: To assess the incidence and severity of obstructive airway diseases in previously treated TB patients. Materials and Methods: This was a prospective case control study, carried out in the Department of Tuberculosis and Respiratory Diseases, Jawaharlal Nehru Medical College, Aligarh Muslim University, Aligarh, Uttar Pradesh, India, from September 2017 to September 2019, this study included 120 patients of previously treated TB, who were divided into two groups. The case group (71) included patients who completed treatment for TB and had dyspnoea. The control group (49) consisted of patients who completed the treatment but did not have dyspnoea. The patients were subjected to spirometry, histamine bronchial challenge test and computed chest tomogram along with routine investigations including sputum for Acid Fast Bacilli (AFB). Results: The present study showed significant histamine hypersensitivity among post TB patients. In the case group, 34 (56%) patients showed positive response to bronchial challenge test with histamine; while in control group only 7 (14%) showed a positive response (p-value=0.004). Pulmonary function test (spirometry) showed an obstructive pattern in 35 (49%) case group patients, while normal pattern was the most common finding seen in 23 (47%) in the control group followed by obstruction in 10 (21%) patients. Among the cases, the mean Forced Expiratory Volume in the first second (FEV1) was 65.77±15.98, while among the controls, it was 80.02±8.81. The case group had a mean Forced Expiratory Volume in the first second/Forced Vital Capacity (FEV1/FVC) of 78.09±15.75, as against 81.33±16.79 in the control group. Conclusion: Airway bronchial hyper-reactivity is a prominent feature in previously treated tubercular patients. This underlines the need for proper attention towards post-tubercular lung function impairment and proper treatment of such patients so as to lessen the impact of bronchial hyper-reactivity on patient symptoms and their quality of life.

Abstract

The SNO+ detector operated initially as a water Cherenkov detector. The implementation of a sealed covergas system midway through water data taking resulted in a significant reduction in the activity of $^{222}$Rn daughters in the detector and allowed the lowest background to the solar electron scattering signal above 5 MeV achieved to date. This paper reports an updated SNO+ water phase $^8$B solar neutrino analysis with a total livetime of 282.4 days and an analysis threshold of 3.5 MeV. Abstract

The SNO+ collaboration reports its first spectral analysis of long-baseline reactor antineutrino oscillation using 114 tonne-years of data. Fitting the neutrino oscillation probability to the observed energy spectrum yields constraints on the neutrino mass-squared difference $\Delta m^2_{21}$. In the ranges allowed by previous measurements, the best-fit $\Delta m^2_{21}$ is (8.85$^{+1.10}_{-1.33}$) $\times$ 10$^{-5}$ eV$^2$. This measurement is continuing in the next phases of SNO+ and is expected to surpass the present global precision on $\Delta m^2_{21}$ with about three years of data.

Abstract

A search for full energy depositions from bosonic keV-scale dark matter candidates of masses between 65 keV and 1021 keV has been performed with data collected during Phase II of the GERmanium Detector Array (GERDA) experiment. Our analysis includes direct dark matter absorption as well as dark Compton scattering. With a total exposure of 105.5 kg yr, no evidence for a signal above the background has been observed. The resulting exclusion limits deduced with either Bayesian or Frequentist statistics are the most stringent direct constraints in the major part of the 140-1021 keV mass range. As an example, at a mass of 150 keV the dimensionless coupling of dark photons and axion-like particles to electrons has been constrained to $\alpha$'/$\alpha$ < 8.7x10$^{-24}$ and g$_{ae}$ < 3.3x10$^{-12}$ at 90% credible interval (CI), respectively. Additionally, a search for peak-like signals from beyond the Standard Model decays of nucleons and electrons is performed. We find for the inclusive decay of a single neutron in $^{76}$Ge a lower lifetime limit of $\tau_n$ > 1.5x10$^{24}$ yr and for a proton $\tau_p$ > 1.3x10$^{24}$ yr at 90% CI. For the electron decay e$^-\rightarrow\nu_e\gamma$ a lower limit of $\tau_e$ > 5.4x10$^{25}$ yr at 90% CI has been determined., Comment: 20 pages, 12 figures, 7 tables

Abstract

In stars that lie on the main sequence in the Hertzsprung-Russel diagram, like our sun, hydrogen is fused to helium in a number of nuclear reaction chains and series, such as the proton-proton chain and the carbon-nitrogen-oxygen cycles. Precisely determined thermonuclear rates of these reactions lie at the foundation of the standard solar model. This review, the third decadal evaluation of the nuclear physics of hydrogen-burning stars, is motivated by the great advances made in recent years by solar neutrino observatories, putting experimental knowledge of the proton-proton chain neutrino fluxes in the few-percent precision range. The basis of the review is a one-week community meeting held in July 2022 in Berkeley, California, and many subsequent digital meetings and exchanges. The relevant reactions of solar and stellar hydrogen burning are reviewed here, from both theoretical and experimental perspectives. Recommendations for the state of the art of the astrophysical S-factor and its uncertainty are formulated for each of them. Several other topics of paramount importance for the solar model are reviewed, as well: recent and future neutrino experiments, electron screening, radiative opacities, and current and upcoming experimental facilities. Abstract

Preduodenal Portal Vein (PDPV) is a rare congenital anomaly. The presence of PDPV carries the risk of injury to Portal Vein (PV) during operations involving biliary duct, duodenum and pancreas. This report is about a 50-year-old female patient with PDPV associated with midgut malrotation and left sided Inferior Vena Cava (IVC). The patient was operated for Recurrent Pyogenic Cholangitis (RPC) and associated biliary stones. The patient sustained iatrogenic injury to PV during surgery which was subsequently repaired with Polytetrafluoroethylene (PTFE) graft doppler showed patent graft at three months of follow-up. This report highlights the fact that pre-existing inflammatory conditions of bile duct and hepatoduodenal ligament further increase the risk of injury to PDPV during surgery.

Abstract

The generalized Bilal (GB) distribution can be defined as the distribution of the median of three independent random variables drawn from the Weibull distribution. Its failure rate function can be monotonic (decreasing or increasing) or upside-down bathtub-shaped. In this study, we aim to reveal some important properties of the GB distribution that have not been considered before. The findings are both theoretical and practical. From the theoretical viewpoint, we present explicit expressions for both single and product moments of order statistics from the GB distribution. The L-moments are derived as well. From the practical viewpoint, the parameter estimations are accomplished using the maximum likelihood (ML) method, which is based on two different sampling schemes: simple random sampling (SRS) and ranked set sampling (RSS) schemes. Furthermore, the asymptotic confidence intervals for the SRS and RSS estimators are discussed. For the sake of comparison and illustration, a simulation study and a real data example are presented. Concluding remarks are given at the end.

Abstract

Introduction: Amplification of airway inflammation and its destruction due to oxidative stress is a major step in the pathogenesis of chronic obstruction pulmonary disease (COPD). Exhaled carbon monoxide (eCO) may be quantified to evaluate the airway inflammation and oxidative stress in such patients. Objectives: To assess the disease severity of COPD and treatment response by measuring eCO as a biomarker. Materials and Methods: COPD patients diagnosed according to the global initiative for chronic obstructive lung disease guidelines and healthy individuals as controls were selected. One hundred and fifty patients with COPD and 125 controls were included in the study. Participants were further subdivided on the basis of their smoking habits. Clinical examinations and spirometry were done to diagnose COPD by following the standard protocol. eCO was measured using a piCO + Smokerlyzer (Breath CO Monitor, Bedfont Scientific Ltd., Kent, UK). It was a single-center cross-sectional study. Results: Mean (± standard error of mean) CO levels in ex-smokers with COPD were higher (5.21 ± 1.546 ppm; P < 0.05) than in nonsmoking controls (1.52 ± 0.571 ppm) but were lower than in current smokers with COPD (12.55 ± 4.514 ppm; P < 0.05). eCO levels were higher in current smokers with COPD (12.55 ± 4.514 ppm; P < 0.05) compared to healthy smokers (9.71 ± 5.649). There was a negative correlation between eCO and forced expiratory volume in 1 s (FEV1) in COPD (r = −0.28; P < 0.05). The mean eCO level was decreased (6.291–4.332; P < 0.001) with improvement in lung function (FEV1 38.75%–50.65%: P < 0.05) after treatment with inhaled steroid. Conclusion: Our study concludes that quantification of eCO level in COPD varies with different grades of airway obstruction and to measure the treatment response. Measuring the level of eCO can be used to assess the indirect assessment of airway inflammation, oxidative stress, and severity of airway obstruction in COPD patients.

Abstract

Objective: The objective of the study was to find the prevalence and incidence of tuberculosis among household contacts of adult Multi Drug Resistant-tuberculosis (MDR-TB) patients and to evaluate their response to first line anti-tubercular drugs. Materials and methods: This prospective observational study was conducted from January 2014 to August 2015 at a tertiary care center in India. We included child contacts of ≤14 year age of recently diagnosed adult MDR-TB patients. Detailed demographic, socio-economic and treatment history including previous treatment, MDR suspect criterion, and HIV status of the index case was collected. Enrolled children were screened for tuberculosis at initial screening as well as during each follow up visit. Extra-pulmonary or sputum negative pulmonary cases were treated in category I. Sputum positive patients were treated as per sensitivity report. All children were followed up for a period of 1 year. Results: Eighty contacts of 21 index cases were enrolled. Disease prevalence at initial screening was 11.3% (n-9) and incidence on follow up was 1.4% (n-1). Prevalence of latent infection was 23.8% with incidence of 15.4% on follow up. Two patients were found to be sputum positive both sensitive to first line drugs. Failure rate of initial treatment was 10% (n-1). This child was initially sputum positive and sensitive to first line drug, but was later found to have MDR-TB. Conclusion: Mild disease in child contacts of MDR-TB can still show good respond to first line treatment especially in areas with high disease burden.

Abstract

The onset of collision dynamics between an ion and a Rydberg atom is studied in a regime characterized by a multitude of collision channels. These channels arise from coupling between a non-polar Rydberg state and numerous highly polar Stark states. The interaction potentials formed by the polar Stark states show a substantial difference in spatial gradient compared to the non-polar state leading to a separation of collisional timescales, which is observed in situ. For collision energies in the range of $k_\textrm{B}\cdot\mu$K to $k_\textrm{B}\cdot$K, the dynamics exhibit a counter-intuitive dependence on temperature, resulting in faster collision dynamics for cold - initially "slow" - systems. Dipole selection rules enable us to prepare the collision pair on the non-polar potential in a highly controlled manner, which determines occupation of the collision channels. The experimental observations are supported by semi-classical simulations, which model the pair state evolution and provide evidence for tunable non-adiabatic dynamics.

Abstract

In the era of generative AI and specifically large language models (LLMs), exemplified by ChatGPT, the intersection of artificial intelligence and human reasoning has become a focal point of global attention. Unlike conventional search engines, LLMs go beyond mere information retrieval, entering into the realm of discourse culture. Its outputs mimic well-considered, independent opinions or statements of facts, presenting a pretense of wisdom. This paper explores the potential transformative impact of LLMs on democratic societies. It delves into the concerns regarding the difficulty in distinguishing ChatGPT-generated texts from human output. The discussion emphasizes the essence of authorship, rooted in the unique human capacity for reason - a quality indispensable for democratic discourse and successful collaboration within free societies. Highlighting the potential threats to democracy, this paper presents three arguments: the Substitution argument, the Authenticity argument, and the Facts argument. These arguments highlight the potential risks that are associated with an overreliance on LLMs. The central thesis posits that widespread deployment of LLMs may adversely affect the fabric of a democracy if not comprehended and addressed proactively and properly. In proposing a solution, we advocate for an emphasis on education as a means to mitigate risks. We suggest cultivating thinking skills in children, fostering coherent thought formulation, and distinguishing between machine-generated output and genuine, i.e. human, reasoning. The focus should be on responsible development and usage of LLMs, with the goal of augmenting human capacities in thinking, deliberating and decision-making rather than substituting them.

Abstract

The GERmanium Detector Array (GERDA) experiment operated enriched high-purity germanium detectors in a liquid argon cryostat, which contains 0.33% of $^{36}$Ar, a candidate isotope for the two-neutrino double-electron capture (2$\nu$ECEC) and therefore for the neutrinoless double-electron capture (0$\nu$ECEC). If detected, this process would give evidence of lepton number violation and the Majorana nature of neutrinos. In the radiative 0$\nu$ECEC of $^{36}$Ar, a monochromatic photon is emitted with an energy of 429.88 keV, which may be detected by the GERDA germanium detectors. We searched for the $^{36}$Ar 0$\nu$ECEC with GERDA data, with a total live time of 4.34 yr (3.08 yr accumulated during GERDA Phase II and 1.26 yr during GERDA Phase I). No signal was found and a 90% C.L. lower limit on the half-life of this process was established T$_{1/2}$ > 1.5x10$^{22}$ yr, Comment: 10 pages, 5 figures, 1 table

Abstract

We obtain explicit expressions for single and product moments of the order statistics of an omega distribution. We also discuss seven methods to estimate the omega parameters. Various simulation results are performed to compare the performance of the proposed estimators. Furthermore, the maximum likelihood method is adopted to estimate the omega parameters under the type II censoring scheme. The usefulness of the omega distribution is proven using a real data set.
