Aim. To evaluate the clinical effectiveness, safety and compare the incidence of hospitalization during treatment with antiviral drugs with a direct mechanism of action (riamilovir and umifenovir) in people with acute respiratory viral infections (ARVI) in an outpatient setting. Materials and methods. The study included 150 outpatients with ARVI aged 18–27 years: 50 patients received riamilovir 250 mg 3 times a day for 5 days, the second group included 50 patients who received umifenovir 200 mg 4 times a day for 5 days, 50 individuals received only pathogenetic treatment. Results. The use of direct-acting antiviral drugs was characterized by the least severity of pain and aches in the body, general weakness, and in the group of patients receiving riamilovir, the lowest severity of rhinitis, cough, and the lowest morning body temperature were recorded compared to other groups. In riamilovir group reduction in the duration of the disease was observed. The lowest frequency of ARVI pathogens detection was observed on the 6th day in riamilovir group. Outpatient treatment with riamilovir was accompanied by a minimal number of cases of the disease requiring hospitalization. Conclusion. The use of direct antiviral drugs contributes to the rapid relief of ARVI symptoms in patients receiving medical care on an outpatient basis. The antiviral drug riamilovir showed the most pronounced effectiveness (clinical and laboratory).

According to the WHO, 90-95% of all infectious diseases are acute respiratory viral infections of various etiologies and influenza. The purpose of the research work is to study some epidemiological characteristics of acute infections of the upper respiratory tract with multiple or uncertain MULAIURT localization during the Covid-19 pandemic in the regions of Baku. Intensive (per 1000 population %) and extensive (0%) indicators of morbidity with acute upper respiratory tract infections were determined in the regions of Baku. During the research period, for the years 2020-2022, in 12 districts of Baku city (Binagadi, Khazar, Garadag, Narimanov, Nasimi, Pirallahi, Nizami, Sabail, Sabunchu, Surakhani, Yasamal, Khatai), statistics on morbidity with acute upper respiratory tract infections data were analyzed, analytical epidemiological analysis of absolute indicators among the population (0-17 years, including 0-1 years, 1-4 years, 5-13 years, 14-17 years and 18 years and older), by gender (women and men), was conducted based on the study of the dynamics of registration by years (2020-2022) and by months (seasonality) within the country. In 2022, the level of infection with MULAIURT in Baku was 2.5 times higher than in 2020. In 2022, the number of cases was already twice as high among both women and men. In 3 years, the incidence rates of MULAIURT were higher in the 18-year-old age group compared to the 0-17 age group. In 2020, the intensity indicator of the incidence of MULAIURT was 29.53 people per 1000 people in Sabail region (2.95%), in 2021 it was 82.29 people per 1000 people in Khazar region (8.22%), in 2022 90.20 people per 1000 people per year was set at the maximum level in Garadag district (9.02%). Binagadi, Khazar and Yasamal regions are the regional risk factor for getting sick with MULAIURT. In 2020, the maximum level of morbidity with MULAIURT was determined in December in Sabunchu district, in December in Khazar district in 2021, and in January in Yasamal district in 2022. Prevention of respiratory infections during epidemic periods is an important task of modern health care in Russia and worldwide. One of such approach may rely on use of volatile substances capable of activating nonspecific resistance of the nasal mucosa. Long-term application of synthetic analogues of female pheromones to young males leads to lowered morbidity of respiratory diseases, increased levels of secretory immunoglobulins, as well as decrease peripheral blood of eosinophilia levels in subjects with allergic diseases. Based on this, the aim of the study was set to study an impact of anamnesis factors and indicators reflecting the functioning of nasal mucosal immunity, its nonspecific resistance, on emerging frequent respiratory morbidity; as well as to evaluate effectiveness of natopherine- and andropherine-containing odorants regarding dynamics of such indicators and to reduce incidence of respiratory infections in young subjects. Materials and methods. There were examined 46 apparently healthy boys and girls aged 18 to 22 years. The frequency of respiratory morbidity, anamnesis factors, and the results of non-invasive studies (cytokine concentrations in the nasopharyngeal aspirate and rhinocytograms). Within 30 days, all young subjects were applied to the nasolabial fold synthetic analogues of male (andropherine) and female (natopherine) pheromones (andropherine for young female, natopherine for young male). A re-examination with the above mentioned studies was carried out on day 14 and day 30 of the study. The incidence of respiratory infections was reassessed one year after the onset. Results. The study showed identified a number of predictors and protectors of frequent and long-term respiratory morbidity in young subjects. The predictors of this condition were presented by catamnestics markers of allergic anomalies of the constitution, behavioral factors (smoking), as well as immune indicators of allergy (IL-4) and mucociliary clearance deficiency. A monthly course of odorant applications on the skin of the nasolabial fold using synthetic analogues of male and female pheromones (steranes) has a positive effect both on the nonspecific resistance of the nasal mucosa as well as on the immune parameters. After a month of using steranes, the nasal mucosa is completely decontaminated from coccal microorganisms, and over the next 12 months, the frequency of respiratory infections decreases. This indicates about a prominent clinical effect of the odorants applied.

Objectives: In Egypt, an integrated surveillance for acute respiratory infections (ARIs) was established in 2016 to identify the causes of ARIs. The surveillance system includes 19 governmental hospitals. In response to the coronavirus disease 2019 (COVID-19) pandemic, the World Health Organisation (WHO) requested surveillance adaptation to address the emerging challenges. This study aims to describe the experience in Egypt of adapting ARI surveillance to the COVID-19 pandemic. Methods: WHO case definitions were used to identify patients with ARIs. Nasopharyngeal/oropharyngeal swabs were collected for SARS-CoV-2 and influenza testing. Demographic and clinical information were obtained by interviewing patients at the hospitals. During the COVID-19 pandemic, the first two outpatients daily and every fifth admitted patient were enrolled in the study. To determine the status of ARIs in Egypt during the pandemic, patient demographic, clinical and laboratory data from 2020 to 2022 were obtained and descriptive analyses were performed. Results: Overall, 18,160 patients were enrolled in the study, including 7923 (43.6%) seen at outpatient clinics and 10,237 (56.4%) inpatients. Of the study participants, 6453 (35.5%) tested positive for ARIs, including 5620 (87.1%) for SARS-CoV-2, 781 (12.1%) for influenza and 52 (0.8%) for SARS-CoV-2/influenza coinfection. SARS-CoV-2 was the cause for 95.3% of admitted patients and 65.4% of outpatients. Influenza subtypes included A/H3 (55.7%), Influenza-B (29.1%) and H1/pdm09 (14.2%). Objective: The high incidence of respiratory diseases has dramatically increased the medical burden under the COVID-19 pandemic in the year 2020. It is of considerable significance to utilize a new generation of information technology to improve the artificial intelligence level of respiratory disease diagnosis. Methods: Based on the semi-structured data of Chinese Electronic Medical Records (CEMRs) from the China Hospital Pharmacovigilance System, this paper proposed a bi-level artificial intelligence model for the risk classification of acute respiratory diseases. It includes two levels. The first level is a dedicated design of the "BiLSTM+Dilated Convolution+3D Attention+CRF" deep learning model that is used for Chinese Clinical Named Entity Recognition (CCNER) to extract valuable information from the unstructured data in the CEMRs. Incorporating the transfer learning and semi-supervised learning technique into the proposed deep learning model achieves higher accuracy and efficiency in the CCNER task than the popular "Bert+BiLSTM+CRF" approach. Combining the extracted entity data with other structured data in the CEMRs, the second level is a customized XGBoost to realize the risk classification of acute respiratory diseases. Results: The empirical study shows that the proposed model could provide practical technical support for improving diagnostic accuracy. Conclusion: Our study provides a proof-of-concept for implementing a hybrid artificial intelligence-based system as a tool to aid clinicians in tackling CEMR data and enhancing the diagnostic evaluation under diagnostic uncertainty. Relevance. Acute respiratory diseases (ARD) refer to mass infectious diseases, cause significant economic damage by provoking the development of diseases of the circulatory system, and the seasonal incidence of ARI is a predictor of mortality from cardiovascular diseases, which requires timely statistical accounting using the medical information system (MIS).The aim of the study was to quantitatively assess the risk of exposure to ARIs on the occurrence of diseases of the circulatory system using statistical data processing using MIS.Materials and methods. The epidemiological study was carried out in the design of a retrospective analytical cohort study of cases of temporary disability in the adult population, using statistical and analytical methods with the identification of a causal relationship using MIS, which provides the primary collection, storage, selection and grouping of data on the incidence of patients, with subsequent calculations of the relative risk and its 95% confidence interval.Results and discussion. Using the MIS, 88,735 cases of incapacity for work issued for the period from 2014 to 2019 were recorded and analyzed. Thanks to the reference architecture of the MIS, a sample of patients with acute respiratory infections was carried out, and their subsequent grouping according to the incidence rate into the main and control groups. The main group (frequently ill) was represented by patients who were ill more than 3 times per season, and the control group (rarely ill) was represented by patients who were ill from 1 to 2 times per season. The sample consisted of 4,980 patients, of which 1,461 were the main group and 3519 were the control group. A statistically significant relationship was revealed between the incidence of ARI and the development of angina pectoris with documented spasm (I20.1), chronic ischemic heart disease (I25.1), cerebrovascular disease (I67.9), atherosclerosis of the arteries of the extremities (I70.2), phlebitis and thrombophlebitis (I80), varicose veins of the lower extremities (I83), as well as hemorrhoids (I84).Conclusions. The introduction into the practice of MIS, which has a predominantly reference architecture, will optimize the epidemiological surveillance of infectious and non-infectious diseases, using the statistical registration of cases of temporary disability, mortality and hospitalization for taking timely and effective anti-epidemic and preventive measures.

Acute infectious and inflammatory diseases of the upper respiratory tract today belong to the category of the most common pathological conditions. Most often they occur in the form of acute nasopharyngitis, acute rhinosinusitis and acute laryngitis, the clinical of which consists of such symptoms as general weakness, hyperthermia, nasal blockage and nasal discharge, dysosmia, pain and tickling in the throat, cough, dysphonia. The need to select a safe and effective mucoactive therapy justifies the use of the phytopreparation in this pathology, the main active ingredient of which is an extract of ivy leaves. Also in the composition of the drug includes anise oil, peppermint oil, eucalyptus oil and levomenthol. The mechanism of action of is associated with α-hederin contained in ivy leaves. This substance, binding to the beta-adrenoreceptors of the bronchi, causes relaxation of their smooth muscles. In addition, thanks to the saponins contained in the leaves of common ivy, a mucolytic and expectorant effect is achieved. Literature data show that ivy leaf preparations significantly reduce the severity of acute respiratory infections of the upper respiratory tract. Virtually none of the patients had severe side effects. Allergic reactions were recorded in less than 0.5% of cases, with no mention of a severe allergic reaction of the anaphylaxis type. Thus, the drug of plant origin in the form of drops can be successfully used in patients with symptoms of acute respiratory diseases.

Abstract Background and Aim: Air pollution is one of the most significant environmental problems that has a remarkable impact on the incidence of cardiovascular disease and associated mortality. It is essential to comprehend air pollution effects and the ways of emission and predict the number of patients with acute respiratory problems to eliminate and reduce air pollutants and associated mortality. This study aimed to investigate the relationship between different air pollutants and the number of cardiovascular disease patients in Mashhad. Materials and Methods: This study applied a neural network to model and analyze the relationship between CO, NO2, SO2, PM2.5, and PM10 and the number of patients with acute respiratory problems. The inputs were average temperature, humidity, wind direction, and wind speed and the output was the number of people referred per day by gender and age. The data set used included meteorological data from the Iran Meteorological Organization, air pollution data from the Mashhad Meteorological Organization, and the number of daily referrals of heart disease patients to the emergency department of Mashhad. Results: According to this study, the most effective air pollutants in Mashhad were PM2.5 and PM10, followed by NO2, CO, and SO2, respectively. Conclusion: Neural networks can be applied in the modeling of the relationship between environmental parameters and cardiovascular disease patients because they have a high ability to model nonlinear phenomena. These models show that the more airborne particles, the more rate of cardiovascular diseases in Mashhad

The article presents an analysis of the available sources of scientific literature on modern aspects of laboratory diagnosis of acute respiratory diseases and influenza.

The article presents data on the relevance of differential diagnosis of acute respiratory diseases and pneumonia in military personnel. The results of studying the cytomorphological changes in the pool of neutrophils in the servicemen with acute respiratory infections (ARI) and atypical pneumonia (AP) were obtained during Anti-Terrorist Operation/Joint Forces Operation. These results showed the possibility of using cytomorphological changes of peripheral blood neutrophils as additional criteria for the differential diagnosis of AP and ARI in the first days of the disease. Moreover, only changes in the content of neutrophil pools, in particular for AP, can serve as additional diagnostic criteria in the differential diagnosis of AP and ARI, because in contrast to ARI, it is an increase in the relative content of hyposegmented neutrophils to 1/3 of the population or more. The obtained results can be used in the comprehensive development of additional criteria for the differential diagnosis of acute respiratory diseases and atypical pneumonia in the first days of the disease.

Two hundred fifty patients, including 100 children with frequent and prolonged diseases at the age of 4 to 7 years, 76 children at the age of 7 to 18 years and 74 subjects at the age of 22 to 57 years were observed. The patients were treated with cycloferon in two courses with a 2-week interval according to the standard scheme. The tonsil surface microflora and its susceptibility to antibiotics were determined. Cycloferon lowered the Staphylococcus aureus titre and increased the culture susceptibility to benzylpenicillin, oxacillin, rifampicin, and erythromycin, reducing the variety of the fauces nonpathogenic microflora. The use of cycloferon induced no adverse (pathologic) reactions in 94.8% of the cases. In 4.4% of the children under school age the adverse reactions were transitory and did not require discontinuation of the drug use. Unforeseen reactions were recorded in 0.8% of the children and the use of the drug in them was discontinued. The use of cycloferon in two courses with a 2-week interval according to the standard scheme is recommended for prophylaxis of acute respiratory diseases in the group of children with frequent and prolonged diseases during epidemiologically unfavourable periods and for complex therapy of rhinopharinx infections as an agent increasing efficacy of other antibacterials.

Acute respiratory diseases (ARDs) rank first in the structure of the total incidence as confirmed by the World Health Organization newsletters, and remain a serious problem for the healthcare and economy of different countries nowadays. Therefore, a rational approach to their treatment is important for each practicing pediatrician. Every 15–20 years, there are major changes in the structure of the main pathogens taking place in the etiologic structure of ARDs, which undoubtedly has a significant effect on the existing diagnostic and therapeutic measures for the treatment of ARDs. Clinical presentation of the disease of almost any etiology includes symptoms of pharyngitis and tonsillopharyngitis. The issue of effective treatment and prevention of acute respiratory viral infections is relevant worldwide due to the high prevalence, tendency to recurrent course and significant expenses on treatment. The main criterion for the choice of therapy is minimization of medical burden along with sufficient effectiveness of the treatment. Given the challenging choice of drugs prescribed as etiotropic therapy in the treatment of acute upper respiratory diseases in children, drugs having local anti-inflammatory and sanogenetic effects are of particular importance. The use of combination drugs is most expedient as they have a comprehensive (antimicrobial, anti-inflammatory and painkilling) effect at the same time. The article presents modern researches, as well as experience of practicing pediatricians on the choice of an optimal form of local antibacterial drugs for children of 6 years of age and older in the treatment of upper respiratory tract infections in pediatric patients.

Acute respiratory disease (ARD) is a common pathology. Untimely diagnosis and treatment, co-morbidities, old age and children under 1 year of age, pregnancy can increase the severity of the condition and lead to complications and death. According to IDSA recommendations, vaccination is an effective way to prevent complications and fatal influenza cases. Vaccination is primarily for people at risk of serious illness. In cases of suspected influenza, molecular diagnostics is recommended. At the same time, treatment of suspected influenza should be prescribed immediately, without waiting for laboratory confirmation of the diagnosis. Etiotropic treatment aimed at suppressing virus replication should be the first priority in the treatment of acute respiratory viral infections. Bacterial agents may be the primary cause of respiratory diseases or may complicate viral diseases. In the presence of a bacterial pathogen, antibacterial agents are prescribed. ARD symptoms worsen the quality of life of patients. Complex treatment of respiratory diseases includes the prescription of symptomatic therapy: antipyretics, antitussives, vasoconstrictor drugs in rhinorrhea. Cough is the most common respiratory symptom. In various diseases, cough has its own peculiarities, which allows you to carry out differential diagnosis. The American Thoracic Society has proposed schemes of differential search for acute and chronic coughs, which allow rational diagnosis of diseases. The use of antitussive drugs has its own peculiarities: when the cough is non-productive, drugs are prescribed to suppress cough, when the cough is productive - drugs are aimed at facilitating the evacuation of sputum. One of the preparations possessing expectorant, mucolytic and antispasmodic action, is Hedelix syrup on the basis of ivy.

Abstract Evidence shows that pulmonary problems in coronavirus disease 2019 (COVID-19) may set off from vascular injury that progresses to physiological disturbances through a compromised gas exchange, following an infection with the severe acute respiratory syndrome coronavirus 2. In this process, inefficient gas exchange in the alveolar could precipitate silent nonclinical hypoxemia. Unfortunately, patients with “silent hypoxemia” do not necessarily experience any breathing difficulty (dyspnea) at the early stage of COVID-19 while the disease progresses. As a result, several asymptomatic, presymptomatic and patients with mild symptoms may escape quarantine measure and thus continue to spread the virus through contacts. Therefore, early diagnosis of “silent hypoxemia”, which attracts no clinical warnings, could be an important diagnostic measure to prevent acute respiratory distress syndrome from the risk of pulmonary failure among the presymptomatic and as a screening tool in the asymptomatic who are hitherto potential spreaders of the virus.

The article provides data on the relevance of acute respiratory diseases and pneumonia among militaries. The purpose of this work was to study the status of the immune system in military personnel during anti-terrorist operation, patients with acute respiratory diseases and atypical pneumonia. Studies have shown that in the first days of the disease, patients with atypical pneumonia compared to those with acute respiratory diseases have higher content of leukocytes due to neutrophil granulocytes, among which the frequency of detection and content of young cells (metamyelocytes and band neutrophils) is increasing. In 100 % of patients, there is a failure of the immune system in terms of the content of young and atypical lymphocytes. The obtained results can be used as an additional criterion for the differential diagnosis of acute respiratory diseases and atypical pneumonia in the first days of the disease.

The article presents data on the relevance of acute respiratory diseases, influenza, clinical manifestations, complications, antiviral therapy.

Introduction. Modern studies show that the presentation of smell of female mice before male mice infection with the influenza virus activates non-specific immune response which protects male mice from infection of the upper respiratory tract and reducing their mortality. However the researches have not studied the adaptive effect of female sexual pheromone on men.The aim of this work was to study the effect of odor of synthetic analogue of natural female pheromone (Osmopherine) on the functional state of young men often suffering from acute respiratory infections (ARI) and allergic diseases.Materials and methods. We studied four-week effect of Osmopherine on the psycho-emotional state, the concentration of hormones (testosterone, cortisol) and immunoglobulins (sIgA, total IgG), the percentage of leukocytes in the blood and the individual smell of young men, who have ARI more than 4 times a year or an allergic disease. The study involved 117 boys aged 19–23 years. Olfactory testing of individual odor samples of young men was carried out by young women (n = 32) aged 18–22 years.Results. It was found that female pheromone has a positive effect on the functional state of men with different immune systems. However its effect is caused by the pathogenesis of the disease. Application of Osmopherine in men with reduced immunity causes an increased activity, improved mood, reduced anxiety and stabilized testosterone level. Men with allergic diseases did not have the same effect. A significant 2 times decrease in the content of eosinophils was found, which indirectly indicates the decrease in the activity of the delayed phase of allergic reactions of immediate type.Conclusion. The immune system functions according to its internal laws and programs. However its activity for the benefit of the whole body is integrated and regulated by the nervous and endocrine systems. Therefore modulating effect on the functioning of the immune system can be mediated through the presentation of pheromones.

Background: The economic impact of acute respiratory disease pandemics has yet to be specifically systematically reviewed. The aim of this scoping review is to identify and classify the economic impacts and its values and ranges. Materials and Methods: We conducted a literature search across three key databases using an extensive list of keywords. Then, we included studies which explored direct and indirect costs as well as broader economic impact associated with different nine acute respiratory diseases, i.e., pandemic and seasonal influenza, avian influenza, equine influenza, swine influenza, severe acute respiratory syndrome, coronavirus disease 2019, Middle East respiratory syndrome, H1N1, and H7N9. Results: We included 62 studies in English language between 1987 and 2020, mostly from the countries of East Asia and Pacific pertinent. We classified the economic impact into 5 main categories and 18 subcategories. The main categories were macroeconomic impacts, impacts on health cost, industry, businesses and trade, and education. Conclusion: Respiratory disease pandemics have widely impacted different sectors of economy such as the direct cost on macroeconomic, providing and receiving health services, disease management, industries, business and trade, education, and indirect costs due to productivity losses. However, lots of the reviewed studies were unable to quantify the actual economic cost of these impacts. This made it challenging to conduct any kind of quantitative comparison of the results. A key priority for future research is to develop standard methods to quantify the broader economic costs of respiratory disease pandemics. Understanding the total economic impact of respiratory disease pandemics is a key step to inform national and international priority setting for disease prevention and pandemic control interventions.

Acute infectious rhinosinusitis is the most common clinical form of respiratory pathology in childhood. Despite the fact that in 90 % of cases the main etiological factor leading to the development of rhinosinusitis is viral infection, 13 % of them are complicated by bacterial inflammation. Acute respiratory di­seases with manifestations of rhinosinusitis constantly recurring in the child can lead to the formation of allergic, non-allergic rhinitis and chronic rhinosinusitis — multifactorial chronic inflammatory diseases of the upper respiratory tract. The pathogenesis of acute infectious rhinosinusitis, in spite of the abundance of data, has been underinvestigated. The article highlights the problem of acute infectious rhinosinusitis treatment in children from the standpoint of modern ideas about the development of inflammatory process in the nasal mucosa, the role of innate immunity, and the functioning of mucociliary clearance. Data on the clinical manifestations of acute infectious rhinosinusitis are presented, clinical and diagnostic criteria for its detection are given. The mechanisms of action of nasal decongestants are described, as well as the features of their use, the immunological and anti-inflammatory effects of these drugs. Attention is focused on the mechanisms of action of oxymetazoline. Recommendations for the use of oxymetazoline in pediatric practice are given.

Acute respiratory diseases (ARD) are widespread diseases and are of great social importance. Diagnosis of acute respiratory infections implies the damage to the respiratory system caused by a wide range of pathogens. Among the pathogens of acute respiratory infections are viruses of acute respiratory viral infection (ARVI) group, including influenza, as well as fungal and bacterial pathogens, which include intracellular bacteria: mycoplasma and chlamydia. Diseases caused by intracellular pathogens are treated by antibacterial drugs that can easily penetrate the cells and create there high concentrations.

Background: It is assumed that the severity of the multiple organ dysfunctions syndrome (MODS) in children who have experienced critical conditions in the neonatal period is a risk factor for the development of acute respiratory infections (ARI). Objective.The Aim was to study the relationship between the severities of organ dysfunctions in children who had undergone critical conditions in the neonatal period, with an ARI frequency at preschool age.Methods: The study included children with MODS, the severity of which was assessed on the NEOMOD scale (moderate dysfunction 4 points, severe 5 points). An infectious index (II) was calculated since the discharge from the hospital till the age of 7: the ratio of the number of cases ARI during the past year to the age of the child that year, and the proportion of often ill children (OIC) — the number of ARI 4, 6 and 5 cases per year at the age of 1, 1–3 and 4– 5 years respectively. Data on cases of ARI are obtained from the history of child development (form № 112/y).Results: The study included 198 children, 100 of them with severe manifestations of MODS and 98 with moderate manifestations of MODS. The groups were comparable by sex, age, social factors (age and level of education of parents), place of residence (city / village). II (median and 95% confidence interval) in children with severe and moderate manifestations of MODS under 1 year were 2 (2; 3) and 1.5 (1; 2) respectively (p=0.006); 1–2 years — 1.5 (1–2) and 1 (1– 1.5) (p=0.008); 2–3 years — 1 (0.7–1.2) and 0.7 (0.5–1) (p=0.006); 3–4 years old — 1,1 (0,8–1,3) and 0,8 (0,6–0,8) (р=0.003); 4–5 years — 0,6 (0.6–0,7) and 0,4 (0.4–0,5) (р=0.001); 5–6 years — 0.5 (0.3–0.5) and 0.3 (0.2–0.3) (p=0.001); 6–7 years — 0.3 (0.3–0.3) and 0.1 (0.1–0.3) (p=0.025). The OIC in the groups for the entire follow-up period was 60 (60%) and 42 (43%) respectively (p=0.011).Сonclusion: Severe manifestations of MODS in the neonatal period are associated with a higher susceptibility of children to ARI.

This study focuses on identifying environmental health risk factors related to acute respiratory diseases using deep learning method. Based on respiratory disease data, air pollution data and meteorological environmental data, cross-domain risk factors of acute respiratory diseases were identified in Beijing, China. We conducted age and gender stratified deep neural network models in air pollution epidemiology. We ranked risk factors of respiratory diseases in stratified populations and conducted quantitative comparison. People ≥50 years were more sensitive to PM2.5 pollution than <50 years people, especially women ≥50 years. Compared with women, both men ≥50 years and <50 years were more susceptible to PM10. Young women <50 years were more sensitive to general air pollutants such as SO2 and NO2 than <50 years young men. Meteorological factors such as wind speed and precipitation could promote the diffusion of fine particulate matter and general air pollutants (SO2, NO2, etc.), which could help to reduce the incidence of acute respiratory diseases. This study represents a quantitative analysis of environmental health risk factors identification related to acute respiratory diseases based on deep neural network method. The article covers the history of aromatherapy, the mechanisms of essential oils influence on the human body, and the specific features of their therapeutic effect. The main principles of the use of essential oils, dosage features and modes of application for children are considered. The methods of using essential oils for the prevention and treatment of children’s acute respiratory infections are described.

Analyzed organization of preventive measures and patient care in Velikiy Novgorod in connection with the exceeding of the epidemic threshold for influenza and acute respiratory viral infections in 2016. The epidemic rise of influenza and other acute respiratory viral infections exceeded epidemiological and economic thresholds among the cumulative population on the territory of the Novgorod region started from the 3rd week 2016 (18.01.2016–24.01.2016) and continued for 4 weeks (until mid February). The peak of the epidemic was passed on the 4th calendar week of 2016, when the weekly prevalence rate was 135,7 per 10 thousand population (8447), the epidemic threshold was exceeded in 2 times. In terms of conversion to the infectious diseases hospital, a multidisciplinary clinic № 2 «Central city clinical hospital» carried out specialized medical assistance 301 the patient with acute respiratory diseases and influenza, 67 of which were confirmed influenza caused by the H1N1 virus, of which 28 patients would, if hospitalized in serious and critical condition and needed to be held-Institute the intensive care manual.

Background: Human adenovirus (HAdV) is a common cause of acute respiratory disease (ARD) and has raised significant concerns within the Korean military. Here, we conducted a comprehensive epidemiological analysis of HAdV-associated ARD by evaluating its prevalence, clinical outcomes, and prognosis., Methods: We reviewed data from multiple sources, including the New Defense Medical Information System, Defense Medical Statistical Information System, Ministry of National Defense, Army Headquarters, Navy Headquarters, Air Force Headquarters, and Armed Forces Medical Command. We analyzed data of patients who underwent polymerase chain reaction (PCR) testing for respiratory viruses between January 2013 and July 2022 in all 14 Korean military hospitals. The analysis included the PCR test results, demographic characteristics, health care utilization, and prognosis including types of treatments received, incidence of pneumonia, and mortality., Results: Among the 23,830 individuals who underwent PCR testing at Korean military hospitals, 44.78% (10,670 cases) tested positive for respiratory viruses. Across all military types and ranks, HAdV was the most prevalent virus, with a total of 8,580 patients diagnosed, among HAdV, influenza virus, human metapneumovirus, human parainfluenza virus, and human respiratory syncytial virus. HAdV-infected patients exhibited higher rates of healthcare use compared to non-HAdV-infected patients, including a greater number of emergency visits (1.04 vs. 1.02) and outpatient visits (1.31 vs. 1.27), longer hospitalizations (8.14 days vs. 6.84 days), and extended stays in the intensive care unit (5.21 days vs. 3.38 days). Furthermore, HAdV-infected patients had a higher proportion of pneumonia cases (65.79% vs. 48.33%) and greater likelihood of receiving advanced treatments such as high flow nasal cannula or continuous renal replacement therapy., Conclusion: Our findings indicate that HAdV posed a significant public health concern within the Korean military prior to the coronavirus disease 2019 (COVID-19) pandemic. Objective: To analyze the effects and tolerability of physiotherapeutic methods with optical radiation (phototherapy) in acute respiratory diseases (ARD) on the basis of the modern scientific literature data and the results of doctors and patients survey., Material and Methods: An analysis of regulatory sources and modern scientific literature on the subject of research, survey of 200 patients with ARD and 100 primary care physicians of the Central Federal District on their sociomedical status and awareness of phototherapeutic treatment methods were conducted., Results: Phototherapy in ARD have demonstrated chromogenic, immunostimulating, photosensitizing, vitamin-forming, trophostimulating, anti-inflammatory, analgesic, desensitizing, bactericidal and mycocidal, metabolic, coagulo-correcting therapeutic effects. Patients and doctors have been insufficiently aware of phototherapy methods and used them in practice relatively rare. A significant proportion of patients had ARD risk factors, namely teamwork, tobacco smoking and chronic diseases., Conclusion: 1. The therapeutic effects of all types of phototherapy in acute respiratory infections are interrelated with their etiopathogenesis. 2. Patients and doctors are insufficiently informed and relatively rarely use phototherapy methods. 3. A significant proportion of patients have risk factors for acute respiratory infections: teamwork (88%), tobacco smoking (68%) and chronic diseases (52%).

This study aimed to investigate the effects of temperature inversions on the concentration of some pollutants in the atmosphere in Hanoi City, Vietnam, during the period from 2011 to 2015. This work also aimed to evaluate relationships between the thermal inversion and health effects that are associated with air pollution. During this period, the temperature inversions were most frequently presenting from November to March in Hanoi City. Air quality data was gathered from air quality monitoring stations located in the study area. The data showed that levels of NO2, SO2, PM10 and PM2.5 increased when the inversions strengthened. Cases of two types of diseases (acute respiratory diseases and cardiovascular diseases), which are linked to atmospheric air pollution, were considered on number of patients under 15 and above 60 years old at National Geriatric Hospital and National Otorhinolaryngology Hospital. There was significant increase in the daily average number of hospital visits with increasing surface-based inversions. The statistical analysis showed that the temperature inversions correlated with concentration of air pollutants and the number of patients in 5 years. [ABSTRACT FROM AUTHOR]

Evidence shows that pulmonary problems in coronavirus disease 2019 (COVID-19) may set off from vascular injury that progresses to physiological disturbances through a compromised gas exchange, following an infection with the severe acute respiratory syndrome coronavirus 2. In this process, inefficient gas exchange in the alveolar could precipitate silent nonclinical hypoxemia. Unfortunately, patients with "silent hypoxemia" do not necessarily experience any breathing difficulty (dyspnea) at the early stage of COVID-19 while the disease progresses. As a result, several asymptomatic, presymptomatic and patients with mild symptoms may escape quarantine measure and thus continue to spread the virus through contacts. Therefore, early diagnosis of "silent hypoxemia", which attracts no clinical warnings, could be an important diagnostic measure to prevent acute respiratory distress syndrome from the risk of pulmonary failure among the presymptomatic and as a screening tool in the asymptomatic who are hitherto potential spreaders of the virus. Community-acquired pneumonia remains one of the most pressing problems of bronchopulmonary pathology among military personnel. Risk group are newly organized military units, where outbreaks of infectious diseases occur, in particular acute respiratory ones, that caused the outbreaks of community-acquired pneumonia. Despite the fact that considerable progress was achieved in the diagnosis and treatment of community-acquired pneumonia and prevention of its complications, on the one hand, there was a significant evolution in the views on the understanding of the pathogenesis of infectious process, on the other — the indices of morbidity and mortality remain its growth dynamics around the world. We have examined separately 50 patients with community-acquired pneumonia, who were treated in April 2015. Of these, in 44 (88 %) patients we have detected a viral agent: adenovirus — in 34 patients, rhinovirus — in 14, influenza A — in 3, parainfluenza — in 1, coronavirus 229E — in 1, coronavirus OC43 — in 1 patient ; viral agent has not been detected in 6 patients.

Acute lower respiratory tract infections are the leading cause of global morbidity and mortality in children under five years. Verification of the etiology of acute lower respiratory tract infections is necessary for definition of treatment and direction of prevention. Respiratory syncytial virus, influenza A and B, parainfluenza 1, 2, and 3 and adenovirus are considered the main reasons of acute lower respiratory tract infections. The importance of different viruses depends on countries, district, seasons and ages of children. Analysis of the results of studies from different regions of the world showed fluctuations in frequency of etiology definition of respiratory viruses from 25 to 90%. Respiratory syncytial virus is the main reason of acute lower respiratory tract infections, especially in the group of children up to 1 year.

Local treatment of 30 preschool children suffe­ring from acute respiratory diseases, acute catarrhal tonsillopha­ryngitis was conducted using the drug Horlospas for Children, which is a metered dose spray containing sea salt, colloidal silver, chlorhexidine bigluconate, marigold and sage extracts, eucalyptus and mint essential oils. A notable acceleration of inflammation regression and a significant decrease in the number of complications after acute respiratory disease were registered. The use of combined drug Horlospas for Children has reduced the number of catarrhal tonsillopharyngitis episodes in the winter, even in sickly children of preschool age.

Summary: The article reveals current data on the epidemiology of acute respiratory infections and antiviral defense mechanisms with regard to the anatomical and physiological characteristics of children's immune system and options for etiopathogenetic therapy. The focus is on the antiviral and immunomodulatory drug pranobex inosine (Isoprinosine), with data from literature on the use of inosine pranobex in children with respiratory infections. The efficacy and safety of inosine pranobex (Isopinosine) in pediatric practice is demonstrated.

Respiratory infectious diseases represent one of the most common pathologies in outpatient care. Use of immunomodulators in multimodal treatment of infectious and inflammatory diseases has been widely discussed recently. Since immune modulation therapy is auxiliary and could not substitute etiotropic drugs, the limits for the use of immunomodulators in the treatment of infectious diseases should be clearly defined. Patients with immune deficiencies constitute a special risk group for infectious diseases. Both etiotropic and immunomodulatory therapy for such patients should be selected very carefully. The review summarizes data on immunomodulators used in Russia and discusses conditions for prescribing immunomodulatory therapy.

Respiratory diseases account for about 25% of all pediatric consultations, and 10% of these are for asthma. The other main pediatric respiratory diseases, in terms of incidence, are bronchiolitis, acute bronchitis and respiratory infections. Oral corticosteroids, in particular prednisolone, are often used to treat acute respiratory diseases given their anti-inflammatory effects. However, the efficacy of treatment with oral corticosteroids differs among the various types of pediatric respiratory diseases. Notably, also the adverse effects of corticosteroid treatment can differ depending on dosage, duration of treatment and type of corticosteroid administered -- a case in point being growth retardation in long-course treatment. A large body of data has accumulated on this topic. In this article, we have reviewed the data and guidelines related to the role of oral corticosteroids in the treatment and management of pediatric bronchiolitis, wheezing, asthma and croup in the attempt to provide guidance for physicians. Objectives: In Egypt, an integrated surveillance for acute respiratory infections (ARIs) was established in 2016 to identify the causes of ARIs. The surveillance system includes 19 governmental hospitals. In response to the coronavirus disease 2019 (COVID-19) pandemic, the World Health Organisation (WHO) requested surveillance adaptation to address the emerging challenges. This study aims to describe the experience in Egypt of adapting ARI surveillance to the COVID-19 pandemic., Methods: WHO case definitions were used to identify patients with ARIs. Nasopharyngeal/oropharyngeal swabs were collected for SARS-CoV-2 and influenza testing. Demographic and clinical information were obtained by interviewing patients at the hospitals. During the COVID-19 pandemic, the first two outpatients daily and every fifth admitted patient were enrolled in the study. To determine the status of ARIs in Egypt during the pandemic, patient demographic, clinical and laboratory data from 2020 to 2022 were obtained and descriptive analyses were performed., Results: Overall, 18,160 patients were enrolled in the study, including 7923 (43.6%) seen at outpatient clinics and 10,237 (56.4%) inpatients. Of the study participants, 6453 (35.5%) tested positive for ARIs, including 5620 (87.1%) for SARS-CoV-2, 781 (12.1%) for influenza and 52 (0.8%) for SARS-CoV-2/influenza coinfection. SARS-CoV-2 was the cause for 95.3% of admitted patients and 65.4% of outpatients. Influenza subtypes included A/H3 (55.7%), Influenza-B (29.1%) and H1/pdm09 (14.2%). Compared with influenza, SARS-CoV-2 tended to infect the elderly, in warm weather and in urban governorates, and resulted in more hospitalisations, longer hospital stays and higher case fatalities (16.3% vs 6.6%, p < 0.001)., Conclusions: ARI surveillance in Egypt was successfully adapted to the COVID-19 pandemic and effectively described the clinical characteristics and severity of circulating viruses. Surveillance reported the re-emergence of influenza with a severe course and high fatality.
