Contracaecum rudolphii Hartwich,1964 (Nematoda: Ascaridoidea, Anisakidae) is a typical anisakid recorded globally. Consumption of undercooked seafood, raw or infected, contains the nematode larvae that cause human anisakidosis. Fish (Planiliza abu) specimens were obtained from a local market in Karbala, Iraq. This type of fish came from the Razzaza Lake. The prevalence of Contracaecum spp. parasites were done over eight months from June 2022 until January 2023, using ITS1, morphological and molecular analysis of the nematodes Contracaecum spp from fish. Of 395 fish, 124 (31.3%) had visceral infections caused by Contracaecum spp. larval type (L3). The morphological and genetic identification of Contracaecum spp. was validated in the parasitology laboratory of the Veterinary Medicine College at Kerbala University. The result showed that the infection rate in January 2023 was (46.6%) and increased while it was (20%) in September 2022. With the use of ITS1 gene, the molecular analysis for Contracaecum spp. was to investigate Contracaecum spp. and to confirm it. However, the nematode count, number of infected fish, and length were all substantially different at the P≤0.05. This study detected the isolate=(a1) at the locus=OP787071 and sequenced the parasites. The isolates were confirmed as Contracaecum rudolphii, isolate a1 internal transcribed spacer 1, partial sequence. In conclusion, molecular genotyping might be a useful technique for identifying the Contracaecum L3 larval species, life-cycle biology, transmission methods, and types of intermediate hosts. Human anisakiasis (or anisakidosis) is a disease caused by the ingestion of marine fish or cephalopods infected with anisakid nematode larvae of the genera Anisakis, Pseudoterranova, Contracaecum, and Hysterothylacium. Anisakiasis is a clinically important disease that often manifests as an acute abdominal syndrome requiring emergency medical attention and care. In Korea, at least several thousand clinical cases have been diagnosed to date; however, only a small proportion of them have been reported in the literature (1971-2022). The most common etiological agents were Anisakis pegreffii (reported as Anisakis sp., Anisakis type I, or erroneously Anisakis simplex), followed by Pseudoterranova decipiens, Contracaecum sp., and Anisakis simplex sensu stricto (s.s.). Most cases involved the stomach and small or large intestine, with a few involving the oral cavity (oral mucosa, pharynx, and tonsils), esophagus, omentum, and mesocolic lymph nodes. Anisakis allergies and host immune responses have been studied in humans and experimental animals. Marine fish and cephalopods, including sea eel (Astroconger myriaster), squid (Todarodes pacificus), yellow corvina (Pseudosciaena manchurica), Japanese flounder (Paralichthys olivaceus), codfish (Gadus macrocephalus), yellowtail (Seriola quinquaradiata), and rockfish (Sebastes spp.), are the most common infection sources. Surveys were performed on anisakid nematode larvae in marine fish and cephalopods caught in the western, eastern, and southern seas of Korea. The larvae recovered from fish or cephalopods caught from the western and southern seas were predominantly A. pegreffii larvae; however, the larvae from the eastern sea were either A. pegreffii larvae (in the chub mackerel, Japanese flounder, and rockfish) or A. simplex s.s. (in the salmon and pollock; these fish migrate through the northern North Pacific Ocean and Bering Sea and come to Korea). Health education to avoid eating raw or improperly cooked marine fish and cephalopods (particularly the viscera) is crucial for preventing human anisakidosis in Korea.

Consumption of raw and mildly processed seafood, in the context of modern Western world eating trends, is recognized as a major driver for human fish-borne infections. However, these zoonoses and their unfamiliar risks remain neglected and underappreciated among European diagnosticians. In contemporary Europe anisakidosis is one of the most important fish-borne zoonoses. It is caused by ingesting the third-stage infective larvae of the nematode parasites that belong to the family Anisakidae. The case described herein, is an intestinal and ectopic form of anisakiosis (Anisakis spp.), causing symptoms of subacute abdomen and masquerading as an intraperitoneal malignancy. It is the first anisakidosis case reported in Greece, affecting a young patient who had been repeatedly exposed to the parasite by consuming homemade raw fish. Right hemicolectomy, omentectomy and excision of a descending colon nodule were uneventfully performed. The pathology report confirmed granulomatous tissue with eosinophilic infiltration and parasites that were morphologically and molecularly identified as Anisakis spp. Although challenging, acquiring an accurate diagnosis of anisakidosis can prevent unnecessary surgery, as the infection typically is self-resolving, and if treatment is deemed necessary, it can be limited to antiparasitic medication. However, in rare cases, extra-gastrointestinal migration of larvae can cause severe damage with practically unknown risks, posing a diagnostic and therapeutic dilemma. In such a clinical case scenario, surgical exploration can decisively contribute to a definitive diagnosis and early identification of intraabdominal complications necessitating surgical intervention. Purpose: To determine species composition and localization of anisakidosis pathogens, infestation of fish species in the Zaporizhzhia NPP cooling reservoir, and identify trends in their infestation compared to monitoring epizootic studies of the last year. Methodology. In the course of the work, we used the generalized results of the studies conducted in the summer and autumn of 2021. The fish species composition in the ZNPP cooling reservoir as well as the species composition of parasites of Anisakidae family and their localization were determined. The studies were carried out using the method of incomplete parasitological dissection of fish at the laboratory. Nematode larvae were detected by visual inspection. The parasites were identified using identification keys. The detected parasites were washed with a saline and stored in 70% ethanol. For the quantitative analysis of the parasitofauna, we used such parameters as prevalence (%) and intensity (ind./ fish). Findings. The parasitological analysis showed that Contracaecum (Ascaris) larvae were found in European, channel and redtail catfishes, which were localized as cysts or free-living in the abdominal cavity. The infestation intensify reached 86 ind./fish, and prevalence reached 100%. The parasites caused hyperemia (redness) and inflammation of internal organs. At the same time, no Contracaecum larvae were found in common and silver carps. Compared to 2020, the infestation of fish was less pronounced, namely: no monogenean Dactylogirus were found on the gills of carps as well as ciliates including Trichodina and Apiosoma, which were found in small amounts on the body surface and gills of common and silver carps. The Contracaecum larvae invasion intensity in catfish decreased by 17 %. Originality. For the first time, a large tendency to the spread of Contracaecum larvae among fish of the ZNPP cooling reservoir was detected, namely in European, channel and redtail catfishes. The importance of the threat of this invasion for fish was established. Practical value. The obtained results can be used to identify timely trends in the infestation of fish by Anisakidae and to develop sanitary and preventive measures in the reservoir.

A review was conducted to identify the most common causative agents of anisakidosis, the methods used for identification of the causative agents, and to summarize the sources of infection, and patients' demographics. A total of 762 cases (409 articles, inclusive of all languages) were found between 1965 and 2022. The age range was 7 months to 85 years old. Out of the 34 countries, Japan, Spain, and South Korea stood out with the highest number of published human cases of anisakidosis, respectively. This raises the question: Why are there few to no reports of anisakidosis cases in other countries, such as Indonesia and Vietnam, where seafood consumption is notably high? Other than the gastrointestinal tract, parasites were frequently found in internal organs such as liver, spleen, pancreas, lung, hiatal and epigastric hernia, and tonsils. There are also reports of the worm being excreted through the nose, rectum, and mouth. Symptoms included sore throat, tumor, bleeding, gastric/epigastric/abdominal/substernal/lower back/testicular pain, nausea, anorexia, vomiting, diarrhea, constipation, intestinal obstruction, intussusception, blood in feces, hematochezia, anemia, and respiratory arrest. These appeared either immediately or up to 2 months after consuming raw/undercooked seafood and lasting up to 10 years. Anisakidosis commonly mimicked symptoms of cancer, pancreatitis, type I/II Kounis syndrome, intussusception, Crohn's disease, ovarian cysts, intestinal endometriosis, epigastralgia, gastritis, gastroesophageal reflux disease, hernia, intestinal obstruction, peritonitis, and appendicitis. In these cases, it was only after surgery that it was found these symptoms/conditions were caused by anisakids. A range of not only mainly marine but also freshwater fish/shellfish were reported as source of infection. There were several reports of infection with >1 nematode (up to >200), more than one species of anisakids in the same patient, and the presence of L4/adult nematodes. The severity of symptoms did not relate to the number of parasites. The number of anisakidosis cases is grossly underestimated globally. Using erroneous taxonomic terms, assumptions, and identifying the parasite as Anisakis (based solely on the Y-shaped lateral cord in crossed section of the parasite) are still common. The Y-shaped lateral cord is not unique to Anisakis spp. Acquiring a history of ingesting raw/undercooked fish/seafood can be a clue to the diagnosis of the condition. This review emphasizes the following key points: insufficient awareness of fish parasites among medical professionals, seafood handlers, and policy makers; limited availability of effective diagnostic methodologies; and inadequate clinical information for optimizing the management of anisakidosis in numerous regions worldwide. Objective: Nematodes of the Anisakidae family are parasites found in aquatic organisms. The lack of studies on anisakidosis and Anisakis in Colombia has meant this type of parasitosis is not widely known by health personnel and underreporting of the disease is highly likely. The objective of this study was to identify anisakid nematodes in the armed snook fish (Centropomus armatus) obtained by artisanal fishing and sold commercially in the coastal port city of Buenaventura. Material and methods: Morphological identification of these worms was performed using taxonomic keys and supplemented with microscopic study using the histochemical Hematoxylin-Eosin technique. Results: Nematodes of the genus Anisakis were found in 42% and the mean abundance was 2.8 in the C. armatus. Conclusions: The findings confirm the presence of Anisakis sp. in fish for human consumption in Buenaventura, the main fishing port in the Colombian Pacific region. This finding in itself warrants further investigation into the possibility of an emerging disease in Colombia.

Abstract Background The whitespotted conger Conger myriaster (Brevoort) (Anguilliformes: Congridae) is an extremely marketable food fish, commonly consumed as sashimi or sushi in some Asian countries (i.e. Japan, Korea and China). Conger myriaster is also suspected as being an extremely important source of human anisakidosis. However, there is currently very little information on the levels of infection with ascaridoid nematode parasites in this economically important marine fish. The aims of the present study are to determine the species composition, prevalence and mean intensity of ascaridoid parasites of C. myriaster caught in the Zhoushan Fishery. Results A total of 1142 third-stage ascaridoid larvae were isolated from 204 C. myriaster. The overall prevalence of infection was 100% (mean intensity 5.6). Nine species of such larvae were accurately identified using integrative taxonomic techniques involving both morphological and genetic data; these included Anisakis pegreffii, A. typica and A. simplex (sensu stricto) × A. pegreffii, Hysterothylacium fabri, H. aduncum, H. sinense, H. amoyense, H. zhoushanense and Raphidascaris lophii. Although high levels of infection and species richness were revealed in C. myriaster, most of the ascaridoid parasites (1135 individuals) were collected from the body cavity and visceral organs of the fish and only seven individuals of A. pegreffii were found in the musculature. Conclusions This study represents the first report C. myriaster from the Zhoushan Fishery being heavily infected with third-stage ascaridoid larvae. Among the ascaridoid larvae parasitic in this fish, an important etiological agent of human anisakidosis, A. pegreffii (L3), represents the predominant species. The genus Hysterothylacium has the highest species richness, with H. fabri (L3) being the most prevalent species. This high level of infection of A. pegreffii (L3) in C. myriaster suggests a high risk of anisakidosis or associated allergies for people consuming raw or poorly cooked fish originating from this marine area. These findings provide important basic information on the occurrence and infection parameters of ascaridoid nematodes in this economically important marine fish. They also have significant implications for the prevention and control of human anisakidosis when conger eels from the Zhoushan Fishery are consumed.

The Anthropocene has brought substantial change to ocean ecosystems, but whether this age will bring more or less marine disease is unknown. In recent years, the accelerating tempo of epizootic and zoonotic disease events has made it seem as if disease is on the rise. Is this apparent increase in disease due to increased observation and sampling effort, or to an actual rise in the abundance of parasites and pathogens? We examined the literature to track long‐term change in the abundance of two parasitic nematode genera with zoonotic potential: Anisakis spp. and Pseudoterranova spp. These anisakid nematodes cause the disease anisakidosis and are transmitted to humans in undercooked and raw marine seafood. A total of 123 papers published between 1967 and 2017 met our criteria for inclusion, from which we extracted 755 host–parasite–location–year combinations. Of these, 69.7% concerned Anisakis spp. and 30.3% focused on Pseudoterranova spp. Meta‐regression revealed an increase in Anisakis spp. abundance (average number of worms/fish) over a 53 year period from 1962 to 2015 and no significant change in Pseudoterranova spp. abundance over a 37 year period from 1978 to 2015. Standardizing changes to the period of 1978–2015, so that results are comparable between genera, we detected a significant 283‐fold increase in Anisakis spp. abundance and no change in the abundance of Pseudoterranova spp. This increase in Anisakis spp. abundance may have implications for human health, marine mammal health, and fisheries profitability. The infestation of Atlantic salmon with larvae of nematode Anisakis simplex has been studied in recreational catches in the River Ponoi of the Murmansk Region by using data from field research. The level and dynamics of infestation of autumn Atlantic salmon in 2009–2016 have been determined in comparison with the historical data. The results of research have shown that the proportion of salmon infested with parasite (extensiveness of infestation) varied from 2,1 to 59,8 % and a number of parasites in a single fish (intensity of infestation) – from 0–1 to 0–28. Indexes of infestation have varied also among different groups of autumn salmon but related neither with a number of years salmon spent at sea nor with salmon run timing into the river. The Red Vent Syndrome (RVS) has never been confirmed for Ponoi Atlantic salmon. The larvae of nematode have been found only in the mesentery and on the internal organs of the abdominal cavity of salmon and never in the muscles. Observations of the behavior of parasite larvae found in the abdominal cavity of the salmon have shown that when storing unbetted fish during the day at different ambient temperatures, migration of larvae to muscle tissue has not been observed. These findings have an important practical application as larvae of nematode Anisakis simplex are infective to humans and cause Anisakidosis. After having analyzed the data on dynamics of salmon infestation the assumption has been made that the level of Atlantic salmon infestation with Anisakis simplex larvae depends mostly on the role of different salmon food species in the parasite life cycle and their availability in different years of salmon feeding migration in ocean.

Abstract Parasites from the family Anisakidae are widely distributed in marine fish populations worldwide and mainly nematodes of the three genera Anisakis, Pseudoterranova and Contracaecum have attracted attention due to their pathogenicity in humans. Their life cycles include invertebrates and fish as intermediate or transport hosts and mammals or birds as final hosts. Human consumption of raw or underprocessed seafood containing third stage larvae of anisakid parasites may elicit a gastrointestinal disease (anisakidosis) and allergic responses. Excretory and secretory (ES) compounds produced by the parasites are assumed to be key players in clinical manifestation of the disease in humans, but the molecules are likely to play a general biological role in invertebrates and lower vertebrates as well. ES products have several functions during infection, e.g. penetration of host tissues and evasion of host immune responses, but are at the same time known to elicit immune responses (including antibody production) both in fish and mammals. ES proteins from anisakid nematodes, in particular Anisakis simplex, are currently applied for diagnostic purposes but recent evidence suggests that they also may have a therapeutic potential in immune-related diseases.

The article presents a study which provided a review of anisakidosis-related hospitalizations describing epidemiological trends and patient characteristics. Topics discussed include clinical features of anisakidosis, temporal and special trends on anisakidosis hospitalization in Spain, and characteristics of anisakidosis-related hospitalizations.

Nematodes of the Anisakidae family have the ability to infest a wide variety of aquatic hosts during the development of their larval stages, mainly marine mammals, aquatic birds, such as pelicans, and freshwater fish, such crucian carp, these being the hosts where the life cycle is completed. The participation of intermediate hosts such as cephalopods, shrimp, crustaceans and marine fish, is an important part of this cycle. Due to morphological changes and updates to the genetic information of the different members of the family, the purpose of this review was to carry out a bibliographic search of the genus and species of the Anisakidae family identified by molecular tests, as well as the geographical area in which they were collected. The Anisakidae family is made up of eight different parasitic genera and 46 different species. Those of clinical importance to human health are highlighted: Anisakis pegreffi, A. simplexsensu stricto, Contracaecumosculatum, Pseudoterranova azarazi, P. cattani, P. decipiens and P. krabbei. The geographical distribution of these genera and species is located mainly in the European continent, Asia and South America, as well as in North and Central America and Australia. Based on the information collected from the Anisakidae family, it was determined that the geographical distribution is affected by different environmental factors, the host and the ability of the parasite itself to adapt. Its ability to adapt to the human organism has led to it being considered as a zoonotic agent. The disease in humans manifests nonspecifically, however the consumption of raw or semi-raw seafood is crucial information to link the presentation of the parasite with the disease. The use of morphological and molecular tests is of utmost importance for the correct diagnosis of the genus and species of the Anisakidae family.

A 46-year-old female presented with an acute abdomen. A CT scan revealed a long segment of distal ileal wall thickening with adjacent fat stranding and mesenteric haziness. An exploratory laparotomy found a mass at the transition point causing intramural obstruction. Pathology showed an anisakid nematode within the ileal submucosa. [ABSTRACT FROM AUTHOR]

Anisakiosis, also known as Anisakis larvae infestation, is an increasing parasitic infestation due to the worldwide spread of raw fish and shellfish consumption habits. We present a rare presentation of intestinal intussusception as a preoperative diagnosis and noticed it postoperatively due to Anisakis larvae. A 43-year-old man presented to the emergency department with abdominal pain around the umbilicus and vomiting for several hours. On physical examination at presentation, he had tenderness in the lower abdomen. His radiological studies showed a right-sided pseudo-kidney sign and ileo-colonic intussusception on ultrasound echography. His computed tomography images added findings of submucosal edema and wall thickening in the terminal ileum, swollen regional lymph nodes, and ascites. An urgent laparotomy was performed for ileo-colonic intussusception of an unknown cause. During the laparotomy, the ileocecal intussusception was manually reduced after dissecting the adhesion due to the previous appendectomy, and a partial ileotomy was undertaken using the Endo-GIA automatic anastomosis device. At the resected ileal wall surface, the presence of Anisakis larvae was noticed, and anisakidosis was diagnosed. The dietary history taken post-operatively revealed that he had eaten salmon, bonito, and squid sashimi four days prior to his emergency department visit. His postoperative course was uneventful, and he was discharged from the hospital on the fifth day postoperatively. Abstract Serodiagnosis of human anisakidosis is presently hampered by the current lack of standardised serological assays that allow sensitive and specific detection of Anisakidae-specific antibodies in human patients. In the present study, we comparatively evaluated the diagnostic value (by IgG-ELISA) of excretory-secretory antigens (ESAgs) of Anisakis simplex , Pseudoterranova decipiens and Contracaecum osculatum , representing the most frequently found genera responsible for human infection. In addition, we tested also a mix of the three ES preparations (Mix-ESAgs) as well as two recombinant allergens of A. simplex , rAni s 1 and rAni s 7. ES antigen from C. osculatum yielded the best diagnostic performance in IgG-ELISA-based serodiagnosis of the Spanish anisakidosis patients investigated in this study (relative serodiagnostic sensitivity 100%; specificity 89%) as compared to A. simplex ES-antigen (93% versus 57%) and P. decipiens (67% versus 93%) or a mix of the three ES antigens (100% versus 44%), respectively. Cross-reactions of C. osculatum ES antigen with serum-antibodies from patients suffering from other helminth infections were rare and were exclusively found with few sera from toxocariasis, ascariasis, and filariasis patients. The two recombinant allergens rAni s 1 and rAni s 7 did not prove sufficiently sensitive and specific in order to justify a further evaluation of these antigens regarding their suitability in IgG-ELISA-based serodiagnosis of human anisakidosis. In conclusion, the C. osculatum -ESAg-ELISA remains as key candidate to be further assessed for the serodiagnosis of symptomatic anisakidosis in different endemic regions. In recent years, scientific evidence from clinical case studies has been accumulated to suggest that consumption of homemade raw anchovies ( Engraulis encrasicolus ) in vinegar (referred to locally as “boquerones en vinagre”) represented the main potential risk source for anisakiasis and IgE sensitization to Anisakis in Spain. Conversely, most epidemiological surveys for this fish species in fishing grounds along the Spanish coast and in Spanish markets showed parasite-free fish lots or parasite prevalence below 10%. In order to update the anchovy infection data and to re-evaluate the importance of this species as a potential source of risk exposure to zoonotic and allergy diseases, 956 anchovies collected by Spanish commercial fishing vessels operating in NE Atlantic fishing grounds were analyzed, using a high performance methodology. Results revealed an overall mean prevalence for zoonotic nematodes (genera Anisakis and Hysterothylacium combined) of 64.33% (14.75% in the flesh). Infection values reached 87% in one of the lots. Prevalence in anchovy muscle was 28.65%, which represents the highest value of parasitization of the flesh ever reported for European stocks of this species. An enhanced risk communication plan focused on control measures, such as removal of anchovy viscera by retailers, and educational campaigns stressing the importance of freezing anchovies that are intended for raw consumption, may clearly contribute to avoid this emergent biohazard in seafood. Abstract: Australia is a multicultural country surrounded by water where seafood is regularly consumed. Literature suggests that some popular edible fish sold in fish markets may be infected with parasites transmissible to humans (notably, anisakids and other helminths); however the number of reported human cases due to these parasites is low. In this article we critically review topical publications to understand whether the low number of human infection is due to lack of expertise in Australia to identify and diagnose accurately seafood‐borne parasitic infections. The risk these parasites pose to humans may be underestimated due to: (i) errors or inability of diagnosing these infections, primarily due to less sensitive and specific serological tests and misidentifying parasites without a taxonomist in the diagnostic team; and (ii) medical practitioners not being aware of these parasites or not considering them in the differential diagnosis even in patients with history of regular raw or undercooked seafood consumption. Parasites from the family Anisakidae are widely distributed in marine fish populations worldwide and mainly nematodes of the three genera Anisakis, Pseudoterranova and Contracaecum have attracted attention due to their pathogenicity in humans. Their life cycles include invertebrates and fish as intermediate or transport hosts and mammals or birds as final hosts. Human consumption of raw or underprocessed seafood containing third stage larvae of anisakid parasites may elicit a gastrointestinal disease (anisakidosis) and allergic responses. Excretory and secretory (ES) compounds produced by the parasites are assumed to be key players in clinical manifestation of the disease in humans, but the molecules are likely to play a general biological role in invertebrates and lower vertebrates as well. ES products have several functions during infection, e.g. penetration of host tissues and evasion of host immune responses, but are at the same time known to elicit immune responses (including antibody production) both in fish and mammals. ES proteins from anisakid nematodes, in particular Anisakis simplex, are currently applied for diagnostic purposes but recent evidence suggests that they also may have a therapeutic potential in immune-related diseases. El objetivo de la investigación fue determinar la prevalencia e intensidad media de infección por larvas del nemátode Anisakis physeteris y otros helmintos según talla y sexo. El estudio se realizó en 57 especímenes de Coryphaena hippurus seleccionados al azar en el Mercado Pesquero del distrito de Ventanilla, Callao, Perú, en mayo de 2002. La prevalencia e intensidad media para larvas de A. physeteris fueron de 33,33% y 1,37; para larvas y adultos de Hysterotylacium sp., fueron 100% y 88,93; para metacestodes Tentacularia coryphaenae fue 71,92% y 6,44; para Nybelinia sp. fueron 19,29% y 2,64; para los tremátodes Bathycotyle coryphaenae fueron 28,07% y 2,25 y para Dinurus spp., fueron 22,81% y 3,42, respectivamente. Los peces cuyo tamaño está entre 127-130 cm son los más parasitados por larvas de A. physeteris. Esta especie es prevalente en la población evaluada y a pesar de la baja intensidad media, existe riesgo de adquirir anisakidosis si se consume C. Hippurus insuficientemente cocido o crudo.

Anisakidosis is caused by Anisakis simplex and other anisakids larvae parasitizing marine fish and cephalopods. A lot of case reports about anisakidosis have been published in Korea because of raw fish eating habits. Recently seafood consumption has continued to increase due to health concerns and thus, it increases the risk for infectious diseases including anisakidosis. The aim of this review is to analyze the clinical and epidemiological characteristics of anisakidosis during the last 10 years in Korea, based on the case reports published from 2000 to 2010. The incidence age was changed from 30s and 40s to 50s. The young generation was considered to consume seafood in various ways, including raw fish as well. The most noticeable change was the appearance of Anisakis allergy patients over the last decade. The patients showed abdominal pain, urticaria after eating sea food. It reaffirmed that anisakid infection induces not only gastric and intestinal anisakidosis but also cause allergic reaction. Anisakid should be considered as a possible causative food allergen provoking allergic responses after eating raw fish.

Introducción: Anisakis simplex puede producir síntomas digestivos y alérgicos. En este trabajo se evalúan los antecedentes epidemiológicos y los resultados inmunológicos para diferenciar entre pacientes con anisakidosis y aquellos con otras patologías digestivas que cursan con dolor abdominal. Pacientes y método: estudio de cohortes realizado con 134 pacientes: 52 fueron diagnosticados de anisakidosis por los hallazgos quirúrgicos y anatomopatológicos y/o seroconversión específica frente a A. simplex (grupo A) y en 82 pacientes la anisakidosis fue excluida como diagnóstico (grupo NA: no-anisakidosis). Se han evaluado el antecedente de ingesta de pescado crudo, la prueba cutánea en prick (PC) y el inmunoblot IgE como elementos de diagnóstico. Resultados: los pacientes de los grupos A y NA mostraron resultados diferentes respecto a la de ingesta de pescado crudo (p < 0,0001) y la PC (p < 0,0001), con valores predictivos negativos (VPN) del 98,39 y 95,56% y positivos (VPP) del 70,83 y 87,50%, respectivamente. En el inmunoblot, se halló una banda de aproximadamente 60 kDa en el 86,2 y 19,2% de los pacientes del grupo A y NA, respectivamente (VPP: 62,50%; VPN: 94,03%). Conclusiones: en pacientes con dolor abdominal, la ingesta de pescado crudo o poco cocinado tiene elevada sensibilidad y VPN (98,39%) pero menor VPP (70,83%), por lo que nos ayuda principalmente a descartar la anisakidosis. La ausencia de sensibilización cutánea al extracto crudo de A. simplex apoya la ausencia de anisakidosis con una probabilidad alta (95,56%). La presencia de una banda de 60 kDa en el inmunoblot podría ser útil para su diagnóstico.Introduction: Anisakis simplex can be a cause of digestive symptoms. Our aim was to evaluate the epidemiological antecedents and immunological data available for a differentiation between patients with anisakidosis and those with other acute abdominal problems. Patients and methods: this is a prospective cohort study involving 134 patients with acute abdominal problems: 52 patients were diagnosed with anisakidosis by means of surgical and pathological findings and/or specific IgE seroconversion against Anisakis simplex (group A), and in 82 patients anisakidosis had been ruled out (group NA: non-anisakidosis). We evaluated the antecedent of raw fish ingestion, the skin prick test, and IgE immunoblotting as diagnostic tools. Results: patients in groups A and NA differ in terms of prior raw fish ingestion (p < 0.0001) and positive SPT (p < 0.0001), with their respective negative predictive values (NPV) being 98.39% (95%CI: 90.17-99.92) and 95.56% (95%CI: 83.64-99.23). Regarding immunoblotting, in 86.2% of patients in group A a band of 60 kDa was detected, which was also detected in 19.2% of patients in group NA. Conclusions: a negative answer to the question about raw or undercooked fish ingestion has very high sensitivity and NPV (98.39%), and is thus reasonably reliable to rule out anisakidosis. The absence of cutaneous sensitization to crude A. simplex extract gives a high probability (95.56%) that the illness is absent. The presence of a band of about 60 kDa in immunoblotting would be useful for diagnosis.

The cinnamon flounder Pseudorhombus cinnamoneus is a frequently consumed marine fish in China. However, the occurrence of ascaridoid larvae in P. cinnamoneus remains unclear. In the present study, a total of 85 P. cinnamoneus caught from the Yellow Sea (off Shidao, 36°52′57″N, 122°26′42″E) in 2011, which is located between mainland China and the Korean Peninsula, was investigated for ascaridoid larval infection. Four ascaridoid larval types, including Anisakis type I of Berland (1961), Hysterothylacium type of Smith (1983), Hysterothylacium type HL of Guo et al. (2014) and Raphidascaris type of Zhao et al. (2016), were detected in this important food fish. These larval types were identified as Anisakis pegreffii , Hysterothylacium aduncum , H. sinense and Raphidascaris lophii , respectively, using PCR-based restriction fragment length polymorphism (PCR–RFLP) analysis and sequencing of the ITS region of nuclear ribosomal DNA (rDNA). The third-stage larvae of H. sinense are reported from Chinese waters for the first time. The prevalence of H. sinense was 100% and represents the predominant species of the ascaridoid larvae found in P. cinnamoneus . The prevalences of A. pegreffii and H. aduncum were 44.7% and 81.2%, respectively. Phylogenetic analyses based on ITS sequences were performed to elucidate the genetic relationships of these ascaridoid nematodes. The present study increases the knowledge and distribution of ascaridoid larvae in this area of Yellow Sea. The high prevalence of ascaridoid larvae in P. cinnamoneus shows that an assessment needs to be undertaken to assess the risk these parasites may pose to public health. [ABSTRACT FROM AUTHOR]

Background. Sushi is a traditional Japanese cuisine enjoyed worldwide. However, using raw fish to make sushi may pose risk of certain parasitic infections, such as anisakidosis, which is most reported in Japan. This risk of infection can be eliminated by freezing fish; however, Japanese people are hesitant to freeze fish because it is believed that freezing ruins sushi's taste. Methods. A randomized double-blind trial with discrimination testing was conducted to examine the ability of Japanese individuals to distinguish between frozen and unfrozen sushi. A pair of mackerel and squid sushi, one once frozen and the other not, was provided to the participants, and they were asked to answer which one tasted better. Results. Among 120 rounds of discrimination testing involving the consumption of 240 pieces of mackerel sushi, unfrozen sushi was believed to taste better in 42.5% (51 dishes) of cases, frozen sushi was thought to taste better in 49.2% (59 dishes), and the participants felt the taste was the same in 8.3% (10 dishes). The odds ratio for selecting unfrozen sushi as "tastes better" over frozen sushi was 0.86 (95% confidence interval [CI], .59-1.26; P = .45). For squid, unfrozen sushi was believed to be superior 48.3% of the time (58 dishes), and frozen sushi, 35.0% of the time (42 dishes). They were felt to be the same in 16.7% (20 dishes) (odds ratio, 1.38; 95% CI, .93-2.05; P = .11). Conclusions. Freezing raw fish did not ruin sushi's taste. The incidence of anisakidosis continues to increase worldwide due to increased consumption of undercooked fish. Although anisakidosis can be categorized into four clinical phenotypes (gastric, intestinal, ectopic, and allergic), ectopic forms of the condition are much less common than gastric or intestinal forms when caused by Pseudoterranova spp. We report the case of a 5-year-old patient who presented with a rapidly growing left inguinal mass and was subsequently diagnosed with extra-gastrointestinal anisakidosis caused by Pseudoterranova azarasi . OBJECTIVE: Anisakidosis is a parasitic infection caused by the ingestion of row or uncooked fish, containing larval nematodes from the Anisakidae family. Intestinal anisakidosis represents about 4% of all cases, the majority being localized in the small bowel, with rare colonic involvement. Here we present an infrequent case of chronic anisakidosis, presenting with intestinal intussusception. CASE REPORT: A 52 years old woman, chronically treated with immunosuppressants, presented to our Institution with acute abdominal pain and vomiting, due to colocolic intussusception. Colonoscopy successfully reduced the intussusception and revealed the presence of a voluminous colonic submucosal mass, near the hepatic flexure. Therefore, the patient underwent laparoscopic right hemicolectomy. The diagnosis of anisakidosis was made when the histological examination of the surgical specimen revealed the infestation of the intestinal wall by a nematode of the Anisakidae family, with an intense erosive-inflammatory adjacent reaction. Anisakidosis is a human zoonotic disease caused by the ingestion of raw or undercooked or not adequately salted, pickled, or smoked fish containing larval nematodes of the Anisakis species. The incidence of this infection is higher in geographical zones with traditional consumption of raw fish. However, in the last years, prevalence raised in low risk zones due to the increase popularity of Asian cuisine. According to where the larvae settle in the gastrointestinal tract, anisakidosis may have different clinical symptoms. In particular, intestinal anisakidosis may mimic several surgical conditions, including appendicitis, ileitis, diverticulitis or inflammatory bowel disease. For this reason, diagnosis is often histopathological. In the present paper, we describe the clinico-pathological findings of six novel cases of intestinal anisakidosis occurred in southern Italy, and provide clues for the differential diagnosis toward Crohn's disease and eosinophilic enteritis, which have similar histopathological characteristics. Awareness of the existence of intestinal anisakidosis may facilitate its recognition and correct diagnosis, which is of fundamental importance for appropriate therapeutic approach. Anisakidosis is an emerging infection associated with a wide range of clinical syndromes in humans caused by members of the family Anisakidae. Anisakid nematodes have a cosmopolitan distribution and infect a wide range of invertebrates and vertebrates during their life cycles. Since the first report of these parasites in humans during the early 60s, anisakid nematodes have attracted considerable attention as emerging zoonotic parasites. Along with rapid development of various molecular techniques during last several decades, this has caused a significant change in the taxonomy and systematics of these parasites. However, there are still huge gaps in our knowledge on various aspects of the biology and ecology of anisakid nematodes in Australia. Although the use of advanced morphological and molecular techniques to study anisakids had a late start in Australia, great biodiversity was found and unique species were discovered. Here an updated list of members within the family and the current state of knowledge on Australian anisakids will be provided. Given that the employment of advanced techniques to study these important emerging zoonotic parasites in Australia is recent, further research is needed to understand the ecology and biology of these socio economically important parasites. The article describes a study which showed the effects of different culinary treatments on the anatomical organization of Contracaecum multipapillatum L3 sensu lato Anisakidae Larvae. Topics covered include materials and methods used in describing the damage caused to the external and internal structures of the L3 larvae subjected to different treatments, the results obtained from the analyzed surviving larvae after the treatment, and discussions relating to observed nematodes.

The occurrence of Genus Anisakis nematode larvae in marine fishes and cephalopods is epidemiologically important because Anisakis simplex larval stage can cause a clinical disease in humans when infected hosts are consumed raw. Common squid ( Todarodes pacificus) from Korean waters were investigated for anisakid nematodes infection during 2009∼2011. In total, 1,556 larvae were collected from 615 common squids and 732 of them were subsequently identified by PCR-RFLP analysis of ITS rDNA. Depending on the sampling locations, the nematode larvae from common squid showed different prevalence, intensity and species distribution. A high prevalence (P) and mean intensity (MI) of infection were observed in the Yellow Sea (n = 250, P = 86.0%, MI = 5.99 larvae/host) and the southern sea of Korea (n = 126, P = 57.1%, MI = 3.36 larvae/host). Anisakis pegreffii was dominantly found in common squid from the southern sea (127/ 140, 90.7%) and the Yellow Sea (561/565, 98.9%). In contrast, the P and MI of infection were relatively low in the East Sea (n = 239, P = 8.37%, MI = 1.25 larvae/host). A. pegreffii was not found from the East Sea and 52.0% (13/25) of the nematodes were identified as A. simplex. Most of them were found in the body cavity or digestive tract of common squid, which are rarely consumed raw by humans. Considering the differenences in anisakid nematode species distribution and their microhabitat in common squid, it remains unclear whether common squid plays an important role in the epidemiology of human anisakis infection in Korea. Abstract: The accurate identification of anisakid nematodes at any life cycle stage is important both to deepen the knowledge on their taxonomy, ecology, epidemiology and for diagnosis and control, as larval stages cause a clinical disease in humans known as anisakidosis. With the aim to investigate the presence of anisakid larvae, specimens of horse mackerel, Trachurus trachurus (Linnaeus, 1758), silver scabbardfish, Lepidopus caudatus (Euphrasen, 1788), European anchovy, Engraulis encrasicolus (Linnaeus, 1758) and opah fish, Lampris guttatus (Brunnich, 1788), were collected by trawling at depths ranging from 50 to 400m. A molecular approach based on restriction profiles obtained after digestion of the nuclear ribosomal ITS region was used to identify Anisakis spp. larvae recovered in fish samples. Restriction profiles showed three banding patterns, corresponding to Anisakis pegreffii, Anisakis physeteris and to heterozygote pattern between A. pegreffii and Anisakis simplex s.s. Specimens showing the heterozygote restriction pattern were also analyzed by sequencing of the entire ITS region, to confirm the heterozygote status. [Copyright &y& Elsevier]

The article presents a study that deals with the presence of proteases with collagenolytic activity in the fish nematode Hysterothylacium aduncum during in vitro development. All studied developmental stages of the nematode showed collagenolytic activity, maximum at pH 6.5 and in the other stages, at 7.0. With no pepsin added to the culture medium, collagenolytic activity was found to be greater. Metalloproteases was also cited as the factor for most of the collagenolytic activity detected in all the developmental stages.

Abstract: Proteinases play an important role as virulence factors both in the life-cycle of parasites and in the pathogen–host relationship. Hysterothylacium aduncum is a worldwide fish parasite nematode which has been associated with non-invasive anisakidosis and allergic responses to fish consumption in humans. Cysteine proteinases have been associated with allergy to plant pollens, detergents and dust mites. In this study the presence of two types of cysteine proteinases (cathepsin B and cathepsin L) during in vitro development of H. aduncum is investigated. Specific fluorescent substrates were used to determine cathepsin activities. The activity detected with substrate Z-FR-AMC was identified as cathepsin L (optimum pH=5.5; range 3.5–6.5). Cathepsin B activity was only identified with Z-RR-AMC (optimum pH=7.0–7.5; range 5.0–8.0). The start of cultivation led to increased activity of both cathepsins (1.8-fold for cathepsin B and 6.3-fold for cathepsin L). These activities varied according to the developmental stage. Cathepsin B activity decreased after M4, returning to its initial level. Cathepsin L activity also decreased after M4, but still maintained a high level (4–6 times the initial level) in adult stages. Having considered these activity variations and the optimum pH values, we suggest that cathepsin L has a role in digestive processes while cathepsin B could be involved in cuticle renewal, among other possible functions. Members of the genera Anisakis and Pseudoterranova are the main causative agents of human anisakidosis: the disease is worldwide distributed, with major impact in countries with a large consumption of raw fish. Because of unspecific symptoms and limited diagnostic tools, incidence and burden of disease are probably underestimated. In Italy, all human infestations where the etiological agent has been properly identified, have been associated to the parasitic species Anisakis pegreffii , the most frequent anisakid in the Mediterranean area. Here, an invasive human case of pseudoterranoviasis is described for the first time in Italy: in 2015, a woman was found infected during a colonoscopy scheduled after the occurrence of nonspecific clinical symptoms. The nematode was found penetrating the ascending colon. The identification was performed by sequencing the mitochondrial region cox 2 and by comparison to GenBank retrieved material using the BLAST search tool. The sample showed a 99% identity with Pseudoterranova decipiens sensu stricto. The record underlines the potential risk due to the consumption of raw or undercooked imported fishes.
