Axial spondyloarthritis is a chronic inflammatory joint disease mainly involving the sacroiliac joints (ISG) and the spine. The diagnosis can be made early due to acute inflammatory changes in the ISG on magnetic resonance imaging (MRI). Radiographs of the ISG do not help in early diagnosis because structural damage is not apparent on radiographs until an advanced stage. In recent years, however, several studies have shown that bone marrow edema – hyperintense signals (= bright spots) as a possible MRI correlate for inflammation – does not specifically occur in axial spondyloarthritis, but can also be seen in healthy people, athletes, people with high mechanical stress (e.g. military recruits) and postpartum women. The diagnosis of axial spondyloarthritis should therefore never be based solely on an MRI finding, but must always include the overall clinical context. [ABSTRACT FROM AUTHOR]

4. Assessments zur Erhebung des Fitnesszustandes in Therapiegruppen für Personen mit axialer Spondyloarthritis : eine Machbarkeitsstudie
- Abstract
Hintergrund: Die Schweizerische Vereinigung Morbus Bechterew hat die Empfehlungen für körperliche Aktivität der europäischen Rheumaliga in einem Gruppentherapie-Konzept für Personen mit axialer Spondyloarthritis (axSpA) umgesetzt. Dieses Konzept beinhaltet neben der Anleitung zum Training in allen 4 Fitnessdimensionen (Ausdauer, Kraft, Beweglichkeit, Neuromotorik) auch Bewegungsberatung und regelmäßige Fitness-Assessments für die Planung und Dokumentation des individuellen Therapiefortschritts sowie für die Qualitätsevaluation der Therapiegruppen. Ziel: Evaluation der Durchführbarkeit der Fitness-Assessments, indem Physiotherapeut*innen (PT) und Gruppenteilnehmende mit axSpA (TNaxSpA) Akzeptanz, Praktikabilität und Integration beurteilen. Methode: Machbarkeitsstudie in 4 Pilottherapiegruppen. PT führten die Assessmentbatterie, bestehend aus a) Chester Step Test (CST) für die aerobe Fitness, b) modifizierter Rumpfkrafttest für die Rumpfkraft, c) Bath Ankylosing Spondylitis Metrology Index (BASMI) für die Beweglichkeit der Wirbelsäule und d) Single Leg Stance Test (SLST) für das Gleichgewicht, mit den TNaxSpA durch. Anschließend bewerteten die PT und TNaxSpA mittels Fragebögen die Akzeptanz, Durchführbarkeit und Integration. Die Durchführbarkeit der Bewertungen wurde anhand von 3 Stufen (I–III) definiert: Bei einer positiven Bewertung von > 80 % (Stufe I) wurde eine bestimmte Bewertung als geeignet angesehen, bei einer positiven Bewertung von 50–80 % (Stufe II) waren Anpassungen erforderlich, bei einer positiven Bewertung von < 50 % (Stufe III) wurde eine bestimmte Bewertung als nicht geeignet für die Gruppenübungstherapie angesehen und es musste eine neue Auswahl getroffen werden. Ergebnisse: Der BASMI wurde von TNaxSpA und PT mit Stufe I bewertet, es waren keine Anpassungen erforderlich. Der CST, SLST und modifizierte Rumpfkrafttest wurden von TNaxSpA mit Stufe I und von PT mit Stufe II bewertet, notwendige Anpassungen wurden entsprechend definiert. Schlussfolgerung

Bechterew) implementiert. Ein wichtiger Bestandteil des BeFit-Konzepts ist eine gezielte Bewegungsberatung zur Förderung von selbstständigem Training. Die Kommunikationskompetenz von Physiotherapeut*innen (PT) gilt dabei als Voraussetzung für eine gelungene und wirksame Patientenedukation (PE), ist bisher aber wenig untersucht. Ziel: Analyse, welche Kommunikationstechniken PT während der Bewegungsberatung mit Personen mit axSpA anwenden. Methode: Es wurde eine Querschnittstudie mit PT und Personen mit axSpA der SVMB-Gruppentherapien durchgeführt. Die Bewegungsberatungen wurden mit Audio aufgenommen, die Kommunikationstechniken anhand des Manuals „The Calgary Cambridge Referenced Observation Guide One“ identifiziert. Das Manual enthält 5 Gesprächsabschnitte (1. Einleiten der Sitzung, 2. Informationssammlung, 3. Strukturierung der Konsultation, 4. Beziehungsaufbau, 5. Beenden der Sitzung) mit 38 Techniken, wovon 6 nicht analysiert werden konnten. Eingeschlossen wurden die Techniken, die mindestens einmal identifiziert wurden. Je strukturierter die Beratung war und je mehr Techniken identifiziert wurden, desto besser konnte die Kommunikation der PT während der Beratungsgespräche bewertet werden. Ergebnisse: Es nahmen 12 PT und 41 Personen mit axSpA teil. Somit wurden 41 Bewegungsberatungen ausgewertet. Insgesamt wurden bei 10 von 12 (85 %) der PT in allen Beratungen die 32 eingeschlossenen Kommunikationstechniken mit einer Häufigkeit der einzelnen Techniken zwischen einmal und 41-mal identifiziert. Der Gesprächsabschnitt „Strukturierung der Konsultation“ zeigte die höchste Häufigkeit zwischen 29- und 41-mal. 6. Feasibility of fitness assessments in group exercise therapy for individuals with axial Spondyloarthritis
- Abstract
Background: Assessments should be standard in individual as well as group physical therapy. The Ankylosing Spondylitis Association of Switzerland has translated the EULAR recommendations for physical activity (PA) in a concept for group exercise therapy, which, in addition to guidance on exercising in all four fitness dimensions (aerobic, strength, flexibility, neuromotor function), also includes PA counselling and regular fitness assessments for the planning and documentation of the individual therapy progress as well for quality monitoring of the exercise groups. Objectives: To evaluate the feasibility of the selected assessments across all four fitness dimensions, rating the acceptance, practicability, and integration by physiotherapists (PTs) and people with axSpA. Methods: A feasibility study was conducted in four pilot exercise therapy groups. PTs performed the assessment battery, consisting of a) the Chester Step Test (CST) for aerobic fitness; b) the modified Core Strength Test for core strength, c) the Bath Ankylosing Spondylitis Metrology Index (BASMI) for spinal mobility and d) the Single Leg Stance Test (SLST) for balance, with the participants of the exercise therapy groups. - Abstract
Inflammatory bowel diseases (IBD) are frequently accompanied by non-inflammatory joint pain and inflammatory spondyloarthritides. Spondyloarthritides can restrict joint function and typically manifest with inflammatory back pain with nightly pain and morning stiffness that improves upon exercising. In other patients, small or large peripheral joints are predominantly involved. Treatment comprises pain medication including COX-II selective non-steroidal anti-inflammatory drugs (NSAID), since non-selective NSAID can aggravate IBD. For axial manifestations, physiotherapy and tumor necrosis factor (TNF) inhibitors are effective, while for peripheral manifestations steroid injections, sulfasalazine and TNF inhibitors are useful. Osteopenia and osteoporosis may result from inflammation, malabsorption and/or steroids. Long-lasting disease activity or steroid treatment should prompt osteoporosis screening. Adequate calcium and vitamin D intake must be ensured and treatment with bisphosphonates evaluated. [ABSTRACT FROM AUTHOR]

Axial spondyloarthritis (axSpA) is a chronic inflammatory disease, related to the HLA-B27 gene, that mainly affects the sacroiliac joints and the spine. In a subsequent chapter of this thesis, we describe a study on whether axSpA patients show retinal vascular abnormalities, as a potential biomarker of cardiovascular disease. In this study, two retinal imaging techniques (fundus photography with Singapore I Vessel Assessment software, and Ocular Coherence Tomography Angiography) were used in axSpA patients between 50-75 years. This proof of concept study was the first to extensively study the retinal microvasculature in axSpA patients and showed a microvascular profile that has been associated with CVD in population studies. The last chapter focusses on the effects of structural collaboration between rheumatologists and ophthalmologists in the management of ocular inflammatory diseases, which consisted of a biweekly multidisciplinary meeting and an ophthalmology-dedicated rheumatology clinic. This work offers practical and valuable insights into how a collaboration can be beneficial for the management of these complex diseases. It emphasizes that it should provide easily accessible support for diagnostic work-up and guidance in the application of systemic immunosuppressives, in particular aimed to reduce the corticosteroid dosages. Overall, this thesis emphasizes that the disease burden in axSpA is still considerable and that improvement of the structural collaboration between rheumatologists and ophthalmologists is crucial to improve the awareness, early diagnosis and care of axSpA in patients with AAU.

In dieser Arbeit wurden die SpA-Erkrankungen in Bezug auf ihre peripheren Manifestationen, ihre genauen Lokalisationen und ihre zeitlichen Verläufe untersucht. Die meisten Patienten hatten sowohl axiale als auch periphere Manifestationen. Bei der klinischen Untersuchung der Gelenke zeigte sich bei Patienten mit peripherer Beteiligung zum größten Teil eine asymmetrische Oligoarthritis. Die Enthesitis manifestierte sich überwiegend an den Achillessehnen und den Plantarfaszien. Die Daktylitis betraf überwiegend die Zehen der Füße und trat zusammen mit Psoriasis auf. Die Uveitis war hingegen mit axSpA u. HLA B27 assoziiert. Auch die CED traten häufiger gemeinsam mit axialer Manifestation auf. Die Psoriasis war hingegen mit peripherer Gelenkbeteiligung verknüpft. Patienten mit perSpA wurden früher diagnostiziert als Patienten mit axialer Beteiligung. Die axialen Manifestationen traten vor peripheren Manifestationen auftraten. Die Psoriasis trat hauptsächlich vor der Gelenkmanifestation auf.

Aufgezeigt wurde der Anteil aller Patienten (50 mit ankylosierender Spondyloarthritis und 50 mit nicht- röntgenologischer axialer Spondyloarthritis), die nach international empfohlener vierwöchiger Behandlung mit Nicht- steroidalen Antirheumatika (NSAR) die Einschlusskriterieren für eine Behandlung mit Biologika erfüllten. Die Patienten wiesen eine hohe Krankheitsaktivität im BASDAI auf und hatten noch keine Behandlung mit der Maximaldosis eines NSAR und keine Biologika erhalten. MRT der Sakroilialgelenke und der Wirbelsäule wurden durchgeführt, CRP wurde bestimmt. Die meisten klinischen Parameter verbesserten sich im Verlauf. CRP und MRT Befunde blieben konstant. Nur wenige Patienten nahmen die empfohlene Maximaldosis über die gesamte Dauer ein. ASAS40-Ansprechen wurde von 35 % der Patienten erreicht. Mit einem BASDAI ≥ 4 erfüllten 44 % der Patienten am Studienende die Einschlusskriterien für Biologika- Behandlung. Beide Subgruppen sprachen vergleichbar gut auf die Behandlung an.

Diese Arbeit beinhaltet neben einem Übersetzungsprozess zwischen Deutschland, Österreich und Schweiz auch einen Feldtests des ASAS Gesundheitsindex zur Überprüfung der Augenscheinvalidität und eine Validierung. Letztere erfolgte zur Überprüfung der internen Konsistenz, Zuverlässigkeit, Konstruktvalidität und Änderungssensitivität. Augenscheinvalidität konnte nachgewiesen werden. Es zeigte sich eine gute interne Konsistenz und Zuverlässigkeit. Die Konstruktvalidität zeigte eingeschränkte bis gute Korrelationen. Die Änderungssensitivität wies einen geringen Effekt auf. Es zeigte sich eine gute Diskriminationsfähigkeit. Der ASAS Gesundheitsindex ist dazu in der Lage, die globale Funktionsfähigkeit in der Gesamtgruppe der SpA zu erfassen.

Ziel ist es, herauszufinden in wie weit mittels MRT eine valide Aussage über strukturelle Veränderungen an den SIG im Vergleich zur Radiographie (CR) und Computertomographie (CT) gelingt. Es erfolgte eine retrospektive Auswertung von Patienten mit axSpA vs. eine Kontrollgruppe. Es wurde zudem eine spezielle 3D VIBE MR-Sequenz genutzt. Es konnte gezeigt werden, dass sich die spezifischen Veränderungen mittels CR, CT und MR nachweisen lassen. Mittels MRT wurden gegenüber der CT signifikant mehr Erosionen in der Altersgruppe 45J oder mit einer Krankheitsdauer >3J ergaben sich keine Unterschiede. Die lineare Regression zeigte eine Überlegenheit der MRT bei der Detektion von Erosionen bei jungen Patienten und einer kurzen Krankheitsdauer gegenüber der CT, insbesondere mit der VIBE. Das MRT ist ein verlässliches Verfahren für den Nachweis von strukturellen Veränderungen, speziell von Erosionen und ist der CT ebenbürtig.

Patienten, welche unter Spondyloarthritis (SpA), einer Gruppe entzündlich-rheumatischer Erkrankungen leiden, entwickeln häufig extraspinale Krankheitsmanifestationen wie Psoriasis vulgaris, Uveitis anterior oder Enthesitis. Die Prävalenz der extraspinalen Manifestationen in einer deutschen SpA-Kohorte wurde in einer Querschnittsstudie erfasst. Zudem wurden die Auswirkungen der Krankheitsmanifestationen auf die Therapie der Patienten und ihre Lebensqualität untersucht. Hierzu wurden 200 Patienten mit einer gesicherten SpA-Diagnose befragt und untersucht. Mit 58,5% hat die periphere Arthritis die höchste Prävalenz unter den extraspinalen Manifestationen, gefolgt von der Enthesitis (35%) und Psoriasis vulgaris (32,5%). Die Krankheitsaktivität in der untersuchten Kohorte war insgesamt hoch, was Einschränkungen der Lebensqualität und der Erwerbstätigkeit zur Folge hatte. Als wichtigste medikamentöse Bestandteile der SpA-Therapie wurden NSAR und Biologika ermittelt.

Die axialen Spondyloarthritiden betreffen primär die Wirbelsäule und werden in eine nicht-radiographische Form und eine ankylosierende Form unterteilt. Da Rückenschmerzen eine Volkskrankheit sind, vergeht leider häufig eine lange Zeit, bis die Krankheit diagnostiziert wird. Die heute zur Verfügung stehende Technik, insbesondere das MRT, kann aber wenn richtig und früh genug eingesetzt diese deutlich reduzieren. Auch die Erstellung einer genauen Anamnese ist essenziell. Unmittelbar nach Diagnosestellung sollte von einem Rheumatologen eine adäquate Therapie, die sowohl eine medikamentöse Säule als auch physiotherapeutische Maßnahmen umfasst, eingeleitet werden.

47. Früherkennung von Patienten mit axialer Spondyloarthritis in der Primärversorgung

Bei Patienten mit axialer Spondyloarthritis besteht eine bekannte Diagnoseverzögerung von > 5 Jahren nach Erstmanifestation von typischen Symptomen, meistens sind das entzündliche Rückenschmerzen. In der Routineversorgung stellen sich Patienten mit Rückenschmerzen gewöhnlich zuerst in primär versorgenden Einrichtungen vor. Daher war das wesentliche Ziel der Studie die Entwicklung eines praktikablen Früherkennungsinstrumentes, um eine optimale Vorselektion zu ermöglichen. Deshalb wurden Patienten < 45 Jahre mit chronischen Rückenschmerzen (n=950) in orthopädischen Praxen eingeschlossen, und dann randomisiert zu einem ortsnahen Rheumatologen überwiesen (n=322). Die erhobenen Daten erlaubten eine Analyse und Validierung der klinischen Parameter und von HLA B27 allein und in Kombination. Durch statistisches Modeling konnte letztlich eine praktikable Zwei-Phasen-Strategie entwickelt werden (Sensitivität 80%, Spezifität 75%). Diese Früherkennungsstrategie wird bald prospektiv validiert.
