Introduction: Functional Abdominal Pain (FAP) is a major concern in developing countries, especially in school-going children. Recently, some probiotics have shown clinical evidence in managing abdominal diseases. The role of Lactobacillus rhamnosus GG (LGG) (ATCC 53103) for FAP in children has not been studied in India. Aim: To evaluate the safety and efficacy of LGG (ATCC 53103) in children with FAP in terms of frequency, severity, and adverse effects. Materials and Methods: This double-blinded, placebocontrolled study was conducted at Geetanjali Medical College and Hospital, Udaipur, Rajasthan, India, from February 2021 to July 2022, after obtaining permission from the Institutional Ethical Committee (IEC). Children aged 5-18 years with FAP were included in the study. A total of 82 children were included in the study and were randomised into the LGG group and placebo group. The LGG group received LGG (ATCC 53103), and the placebo group received sugar powder for a period of four weeks. These patients were followed-up after 2, 4, and 8 weeks for evaluation. Student’s t-test (unpaired) was used for quantitative data, and Chi-square test was used for qualitative data. Statistical Package for the Social Sciences (SPSS) PC-20.0 version was used to evaluate the data. Results: Out of 82 children, a total of 77 children were enrolled during the study period, with 38 in the LGG group and 39 in the placebo group. The severity of abdominal pain at four weeks and eight weeks was significantly less compared to placebo (p-value=0.009 and p-value=0.01, respectively). The frequency of abdominal pain at four weeks and eight weeks was also significantly less compared to placebo (p-value ≤0.001 and p-value≤0.001, respectively). There was no statistically significant difference in adverse effects between the two groups (p-value =0.115). Conclusion: This study concluded that LGG decreases both the severity and frequency of abdominal pain in children aged 5-18 years without any significant side-effects. Thus, it can be safely used in the management of childhood FAP.

Background. The use of hypercaloric formulas in cystic fibrosis patients has the risks of negative effects on carbohydrate and lipid metabolism. Thus, it is interesting to analyze the efficacy of malnutrition correction and the safety of hypercaloric enteral products with a low glycemic index and with medium-chain triglycerides content of 50%.Objective. The aim of the study is analyze the efficacy of malnutrition correction with therapeutic hypercaloric product for enteral nutrition in children with cystic fibrosis.Methods. The study included patients aged from 3 to 18 years with malnutrition (BMI < 50 percentile) who were prescribed therapeutic hypercaloric formula to correct the malnutrition. Anthropometric indicators (height, body weight), actual nutritional status, pancreatin doses, lung function, carbohydrate metabolism rates, and cholestasis marker (bile acid concentration) were measured initially and after 1 month of using formula nutrition.Results. The children’ body weight (Me) has increased from 24.5 (21.2; 38.7) to 25.3 kg (21.6; 39.7) (p = 0.001), growth (Me) — from 133.5 (120.2; 146.5) to 136.5 cm (123.0; 148.5) (p < 0.001) after 1 month of using hypercaloric formula. The growth percentile increased from 33 to 40, the z-criterion values — from –0.5 to –0.3 SD (p < 0.001). There was no increase in BMI in dynamics due to the fact that the growth of children was ahead of body weight increase. The daily energy intake increased by an average of 450 kcal that was 21.8% regarding physiological need.Conclusion. The inclusion of hypercaloric formula in the diet of children with cystic fibrosis for 1 month significantly increases the indicators of linear growth and positively affects the overall physical development. There were no negative effect of formula on carbohydrate and lipid metabolism.

The irritable bowel syndrome (IBS) is a common debilitating functional disorder of the gastrointestinal tract. In the article are described, for the first time, the biopsychosocial model and the vicious circle of IBS development. Therapeutic route of maintenance IBS patients that includes pharmacological and non-pharmacological approaches is presented. Based on the analysis of foreign recommendations for the treatment of IBS, it is shown that the "gold standard" of non-drug treatment is full-time and remote cognitive behavioral psychotherapy (CBT). The advantages of the remote form of cognitive-behavioral psychotherapy of IBS are presented, psychotherapeutic targets are systematized, and the structure is described in detail. A protocol for monitoring symptoms of irritable bowel is presented. Based on the analysis of foreign studies, in the article are presented the effectiveness and limitations of remote-control protocols in the treatment of IBS.

The article presents the history of the study of intestinal lesions of viral origin. The literature data was generalized, which regarding changes in the gastrointestinal tract viral infections transmitted not only by the fecal-oral route, but also airborne. The description of the clinical picture of viral infection of the intestine, morphological data, new approaches to treatment was presented.

Chronic abdominal pain is a common complaint in childhood and adolescence. Despite decades of clinical observations and research, it still poses a challenge to pediatric health care professionals. The aim of this review is to highlight the epidemiology of pediatric chronic abdominal pain and to describe the pathogenesis of this disorder, its clinical manifestations, evaluation and therapeutic options. Articles on chronic abdominal pain in the recent years from PubMed, MEDLINE, and reference textbooks were reviewed. Chronic abdominal pain, a functional gastrointestinal disorder (FGID), is a multifactorial condition that results from a complex interaction between psychosocial and physiologic factors via the brain-gut axis. A thorough history coupled with a complete physical examination and normal screening studies rule out an organic cause in 95% of the cases. It is highly important for the physician to establish a trusting relationship with the child and parents because successful treatment including modification of physical and psychological stress factors, dietary changes, and drug therapy depends greatly on education, reassurance and active psychological support. FGIDs are a cause of great anxiety, distress and morbidity in children as well as adults. As our understanding of these conditions improves, our therapeutic interventions will progress not only to overcome them but also to intervene early in the disease course so as to limit long-term impact. Irritable bowel syndrome (IBS) is a widespread functional bowel disorder and its prevalence in Western societies ranges from 3-20%. The present study aimed to determine the prevalence of IBS in eastern Turkey, particularly in urban parts of Elazig where 250 000 people live. Of the 18 primary care centers, four were randomly selected for this study. All individuals (aged >17 years) living around the service area of these health centers were included. A total of 1900 subjects were randomly selected using the personal health records from the primary care centers. The total number of subjects that could be contacted was 1766. A questionnaire with multiple choice questions was applied to the subjects by medical students using the face-to-face technique. Prevalence of IBS and distribution of symptoms were evaluated by the Rome II criteria. Of the 1766 subjects, 45.4% were male and 54.6% were female. Prevalence of IBS was 5% in males, 7.4% in females and 6.3% overall. The percentage of subjects with IBS in the 17–30 years age group was 26.2%; 52.3% in the 30–50 years age group and 21.6% in the above 50 years age group. Prevalence of IBS was highest (10.2%) in persons who were illiterate and lowest (3.0%) in university graduates. A positive correlation was determined between low economical status and prevalence of IBS ( P < 0.05). Although IBS is widely present in Turkey, its prevalence is lower than that reported in Western communities. In the region where this study was carried out, IBS was more prevalent in females and in individuals with low educational and economical status. [ABSTRACT FROM AUTHOR]

Irritable bowel syndrome (IBS) is one of the most common functional gastro-intestinal disorders which significantly impacts the quality of life of affected children. Abdominal pain improved by defecation, associated with a change in stool form and frequency, represents its specific clinical marker. Even if a number of potential patho-physiological mechanisms have been described, the exact underlying etiology of IBS is so far unclear. Likewise, no optimal treatment has ever been found neither for adult nor for pediatric patients. Current therapeutic options include drugs, dietary interventions and biopsychosocial therapies. Fibrotic strictures are one of the most severe complications of Crohn's Disease (CD). They occur in about 50% of patients at five years and in 70% at ten years of the diagnosis. The only treatment available for symptomatic fibrotic strictures is surgical resection and endoscopic dilation. Both strategies are associated with a high rate of recurrence, and with multiple surgical resections, which pose the threat of surgical morbidity and short bowel syndrome. Therefore, it is crucial to identify, early, the patients more prone to develop intestinal fibrosis to intensify follow-ups, switch to more aggressive treatments, and suggest lifestyle modifications. Scarce data are available concerning biomarkers and genetic determinants to predict which patient will develop intestinal fibrosis. Biologic or clinical markers would be useful to determine this subgroup of CD patients and to predict the onset of intestinal fibrosis and, ideally, its severity. Furthermore, the identification of environmental risk factors may suggest lifestyle changes aimed at modifying the natural course, thus decreasing the risk of complicated CD.
