Multiple Sklerose (MS) ist eine chronisch inflammatorische ZNS-Erkrankung, bei der neben Demyelinisierung, axonale und neuronale Schädigung für das Ausmaß der Erkrankung maßgebend sind. Oxidativer Stress, mitochondriale Dysfunktion und Dysregulation des Eisenstoffwechsels scheinen als Bindeglied zwischen entzündlichen und neurodegenerativen Prozessen zu fungieren. Antiinflammatorische oder immunmodulatorischen Therapieansätze zeigen kaum einen klinischen Nutzen bei chronisch progredienten Erkrankungsverläufen. Zur Entwicklung neuer Therapieoptionen ist ein besseres Verständnis der Ätiopathogenese und der pathogenetischen Rolle verschiedener Immunzellpopulationen unabdingbar. In meiner Arbeit habe ich zur Charakterisierung von Natürlichen Killer-(NK)-Zellen im Liquor von MS- Patienten und Patienten mit anderen inflammatorischen und nicht- inflammatorischen neurolo-gischen Erkrankungen beigetragen. Wir konnten zeigen, dass NK-Zellen im Liquor von Patienten mit neuroinflammatorischen Erkrankungen einen unreifen Phänotyp aufweisen, der im Kontrast zu den ausdifferenzierten NK-Zellen des Blutes steht, was auf eine selektive Rekrutierung unreifer NK-Zellen in den Liquor hindeutet. Das Hauptaugenmerk meiner Arbeit lag auf der Entwicklung neuer Therapiestrategien für die Behandlung progredienter MS-Verlaufsformen in Hinblick auf oxidativen Stress als begünstigenden Mechanismus der Neurodegeneration. Die antioxidative Substanz Idebenon, welche sich bei Erkrankungen, die primär durch eine mitochondriale Dysfunktion gekennzeichnet sind, als wirksam erwiesen hat, verfehlte im Mausmodell der progredienten MS, der chronisch experi-mentellen autoimmunen Enzephalomyelitis (EAE) jeglichen therapeutischen Effekt. Vielversprechender war die Untersuchung der Kombinationstherapie von Epigallocatechin-3-gallat (EGCG) und der immunmodulatorischen Substanz Glatirameracetat (GA) bei chronischer EAE. Grundlage waren vorausgegangene Untersuchungen unserer Arbeitsgruppe, welche synergistische neuroprotektive Wirkungen von EGCG und GA bei schubförmig verlaufender EAE nachgewiesen haben. In einem therapeutischen Behandlungsansatz wurde EGCG täglich mittels oraler Gavage verabreicht. Die Injektion einer suboptimalen GA-Dosierung erfolgte einmalig subkutan. Überraschenderweise verfehlte die Kombinationstherapie jeglichen therapeutischen Effekt. GA und EGCG in Monotherapie verringerten jedoch signifikant die Anzahl entzündlicher ZNS-Infiltrate. Allein EGCG konnte vor Demyelinisierung und axonaler Schädigung schützen. Die zusätzliche Verabreichung von GA hingegen neutralisierte diese Effekte. Als mögliche Interaktionsebene untersuchten wir die Proliferation enzephalitogener T-Zellen, die Beeinflussung anti- und proinflammatorischer Parameter, die durch EGCG-vermittelte Eisenkomplexierung sowie die Expression neurotropher Faktoren. Interessanterweise führte GA in Kombination mit EGCG zu einer gesteigerten Expression des durch oxidativen Stress und inflammatorische Stimuli induzierbaren Enzyms Hämoxygenase-1 (HO-1), wohingegen EGCG diese reduzierte. Die neuroprotektive Wirkung von EGCG scheint vornehmlich auf einer Beeinflussung oxidativer Prozesse zu beruhen. Es bleibt unklar, ob die Hochregulation der HO-1 einen entscheidenden Beitrag zur Krankheitsentwicklung leistet. Ein besseres Verständnis des Redox-Gleichgewichts ist zwingend erforderlich für die Entwicklung neuer Therapieoptionen bei progredienter MS., Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system (CNS), in which axonal and neuronal damage appear to be responsible for increasing clinical disability. Oxidative stress, mitochondrial dysfunction and dysregulation in iron metabolism seem to link inflammatory and neurodegenerative processes. Anti-inflammatory or immuno- modulatory therapeutic agents show minimal benefit in preventing disease progression. A better understanding of disease etiopathogenesis and immunology is indispensable to develop new therapeutic options. I have contributed to the characterization of natural killer (NK) cells in the CSF of patients with MS and other neuroinflammatory or non-inflammatory neurological diseases. In contrast to blood, NK cells in the CSF display an immature phenotype during neuroinflammation, suggesting a selective recruitment of immature NK cells from the periphery into the CSF. The main focus of this work was to examine the therapeutic potential of novel oxidative stress-targeting drugs on neurodegenerative processes in the mouse model of progressive MS. The antioxidant agent idebenone, which shows beneficial effects in disorders caused by mitochondrial alterations, failed to show therapeutic efficacy in chronic experimental autoimmune encephalomyelitis (EAE). A more promising strategy was the combination of another antioxidant agent, epigallocatechin-3-gallate (EGCG) and the immunomodulatory agent glatiramer acetate (GA) in chronic EAE. Basis for this approach was previous investigations from our group, which showed synergistic neuroprotective effects of the combination in relapsing-remitting EAE. Using a therapeutic treatment strategy, EGCG was administered daily by oral gavage. A suboptimal GA dose was injected once. Unexpectedly, the combined therapy of GA+EGCG did not improve EAE. However, GA and EGCG single treatments led to significant reductions of spinal cord inflammation. While treatment with EGCG alone preserved axons and myelin from damage, the neuroprotective effects of EGCG were abolished when GA was applied in combination. To investigate mechanisms of druginterference, we examined the encephalitogenic T cell re-sponse, anti- and proinflammatory parameters, the iron chelator activity of EGCG and the expression of neurotrophic factors. Interestingly, EGCG induced a downregulation of the gene expression of the stress-inducible enzyme heme oxygenase-1 (HO-1), while the combination of GA+EGCG led to increased HO-1 expression. Altogether, our data indicate that neuroprotection by EGCG in chronic EAE involve regulation of oxidative processes. It remains unclear, whether upregulation of HO-1 may contribute to disease progression. Further investigations of the redox balance are warranted to design novel treatment strategies for progressive MS

Multiple Sclerosis (MS) is a crippling chronic disease of the Central Nervous System caused by the presence of self-antibodies which progressively damage axonal myelin sheath, leading to axonal transmission impairment and to the development of neurological symptoms. MS is characterized by a Relapsing-Remitting course, and current therapies rely only on the use of immunosuppressive drugs, which are however unable to reverse disease progression. Encouraging results have been obtained in preclinical studies with the administration of Mesenchymal Stem Cells (MSCs) before disease onset (Zappia et al., 2005). Here, we investigate the therapeutic potential of MSC administration after disease onset into an animal model of MS, represented by Dark Agouti rats affected by chronic Relapsing-Remitting Experimental Autoimmune Encephalomyelitis (EAE) (Cavaletti et al., 2004). 106 MSC were intravenously injected in EAE rats after disease onset. Clinical score was assessed daily, and after 45 days rats were sacrificed and histological analysis of spinal cords performed to evaluate the demyelinating lesions. After the first peak of disease, no further relapses were observed in EAE rats treated with MSCs, differently from what observed in EAE group. Histological analysis demonstrated the presence of demyelinated plaques in spinal cords of EAE rats, (Luxol fast Blue staining and anti-MBP immunohystochemistry). On the contrary the therapeutic schedule with MSCs significantly reduces the number and the extension of demyelinated areas in the spinal cords, confirming clinical score evaluations. These results demonstrated that MSCs ameliorate the clinical course of EAE and hamper the disease relapsing by reducing the areas of demyelinated lesions.
