6. Validation of a Behavioral Marker System for Rating Cadet's Non-Technical Skills.

Simulator-based training assumes a very important role in the maritime domain, particularly in the education of Officers Of the Watch (OOW). In the Portuguese Navy, most of the cadet's skills as future OOW rely on the success of this training. Beyond theory and technical training, the development of non-technical skills is a key factor for obtaining officers capable of identifying and solving problems. To optimize the training and development of non-technical skills, using the Portuguese Naval Academy Simulator, a previously designed Behavioral Marker System model was further assessed. The revised model, which comprises new parameters such as the effectiveness of the task, was validated through a set of simulated sessions, where 11 instructors and 48 students participated. After each session, data was collected with questionnaires and focus group discussion, focusing on the quality and usability of the model and on the design of the scenario. The results show that the revised model, positively addresses the limitations found on the previous version, and it has received encouraging feedback from both instructors and cadets. This new model is now under implementation in all the Naval Academy course programs, and future research aims to digitalize the behavior markers. [ABSTRACT FROM AUTHOR]

Despite the adoption of crew resource management training for bridge teams over the last decades, the training is still heavily focussed on technical achievements. In an educational context, the situation is more problematical, since with requirement of developing the technical skills, there is a need to build and evaluate the non-technical skills of cadets with little experience in bridge team management. In parallel with the application of team leadership models, the Portuguese Naval Academy conducted a research to improve the development and assessment of non-technical skills in bridge simulators. This paper describes the method used to identify the key non-technical skills required for naval cadets and to develop a behavioural marker system for their measurement. A literature review of behavioural marker systems was supplemented with an analysis of interviews conducted with students and simulator instructors. Additionally, further analysis of Portuguese Navy accidents reports was made, applying the HFACS framework to identify the relevant nontechnical skills involved in the accidents. The resulting rating system covers five skill categories (leadership, situational awareness, communication, team work and decision making), each one with three rating elements. In order to deliver the minimum safety conditions for movement of the ships towards restricted waters, urgent survey operations are required whenever we deal with natural disasters; unreliable chart information or uncharted areas requires. In recent years, there has been a huge development in positioning and survey technology. Simultaneously, charts production techniques and GIS software are easily accessible. In this circumstance, a research project was made in order to assess the possibility of developing existing capabilities of emergency hydrographic survey. The toolkit was designed to allow the swift production of usable bottom representation and survey of navigational aids, with a focus on navigational safety, rather than bottom contour accuracy. Trabalho de revisão do 6º ano médico com vista à atribuição do grau de mestre (área científica de clínica geral) no âmbito do ciclo de estudos de Mestrado Integrado em Medicina. Foi realizado um estudo para averiguar a diferença de opiniões entre utentes e médicos sobre os pedidos de exames complementares de diagnóstico, sua quantidade e motivos, e se há boa comunicação nos mesmos. Este estudo foi aplicado através de inquéritos a uma amostra de 34 utentes e 20 médicos das consultas de Saúde do Adulto da especialidade de Medicina Geral e Familiar, com uma média de idades de 46,2 anos. Foi usada análise descritiva e testes não paramétricos na interpretação dos dados. Constatou-se que há pontos de aproximação nas opiniões, no uso de exames complementares pelos médicos, assim como há pontos de divergência, no que respeita a pedidos por parte dos utentes. Demonstrou-se que não há divergência de opinião na questão dos médicos explicarem os seus motivos de prescrição aos utentes (p=0,788). Concluiu-se que os utentes confiam nos seus médicos, fruto da boa comunicação e relação médico-doente. A study was conducted to investigate the difference between the opinions of patients and doctors about the requests for diagnostic tests, its quantity and motives, and if there is good doctor-patient communication. This study was implemented through surveys to a sample of 34 patients and 20 doctors from Adult Health appointments of the Family Medicine specialty, with an average age of 46,2 years. Descriptive statistics and non-parametric tests were used in the interpretation of the data. It was found that there are approaching points in the opinions, such as in the use of diagnostic tests by doctors, as much as there are divergent points, as in the patients’ requests for tests. It has been shown no difference in opinions on the issue of doctors explaining their motives for prescribing to the patients (p = 0,788). It could be concluded that patients trust their doctors from their good communication and doctor-patient relationship.

This master project concerns the development of a communication plan for Club Med Portugal, the Portuguese division of Club Med an international ‗all-inclusive‘ vacation resorts company. The current socio-economic situation lived in Portugal, alongside with technological developments, have forced companies to rethink its communication strategy and adapt to the changing consumer‘s needs and expectations. Club Med Portugal wants to be in the frontline of this new shift in the way companies reach and communicate with consumers, and therefore proposed the development of this communication plan. Club Med Portugal‘s low brand knowledge and awareness (mostly brand recall), were another reason why the company proposed this communication plan. This master project analyzes the external and internal factors that influence the company and affect its communications effectiveness. In order to have a better insight on the problems faced by Club Med Portugal was done an interview to David Delgado (Marketing Manager at Club Med Portugal). It was also done a study regarding Club Med Portugal target customers (vacation purchase habits, preferences, Club Med Portugal‘s communication effectiveness, brand awareness and knowledge) in order to develop marketing activities and actions, to this target, in a more effective way. This master project will therefore, consist of the development of a coherent and realistic communication plan that takes into account the new trends in marketing communications (i.e. emergence of internet and interactive marketing). At the same time, it also expects to be innovative, meet Club Med Portugal‘s objectives (increase brand awareness and knowledge), and consumers‘ needs and expectations. Este projecto de mestrado consiste no desenvolvimento de um plano de comunicação para o Club Med Portugal, a divisão Portuguesa do Club Med, uma cadeia internacional de resorts ‗tudo-incluído‘. A situação socioeconómica vivida actualmente em Portugal, assim como as novas tendências no campo do marketing e desenvolvimentos tecnológicos, têm forçado as empresas a repensar as suas estratégias de comunicação e a adaptarem-se às mudanças nas necessidades e expectativas dos consumidores. O Club Med quer estar à frente desta mudança, e por essa razão propôs o desenvolvimento deste plano de comunicação. A baixa notoriedade (principalmente brand recall) e conhecimento que os consumidores alvo têm sobre a marca, também levaram o Club Med a propor este plano. Neste projecto de mestrado são analisados os factores externos e internos que influenciam a empresa e a eficácia da sua comunicação. De forma a obter um melhor conhecimento sobre a realidade da empresa foi feita um entrevista a David Delgado (Director de Marketing do Club Med Portugal). Também foi feito um estudo ao consumidor alvo do Club Med (comportamento de compra, preferências, notoriedade e conhecimento sobre o Club Med), de forma a desenvolver actividades e acções para este consumidor alvo, de uma forma mais eficaz. - Abstract
Introduction: Exposure to ambient air pollution may play a role in the onset of common mental disorders like depressive and anxiety disorders. The association of long-term exposure to particles smaller than 10 μm (PM10) with these diseases remains unclear. This study aimed to estimate the association of long-term exposure to PM10 with mental well-being and the frequency of probable diagnosis of common mental disorders., Methods: A nationally representative cross-sectional study was done in mainland Portugal. Long-term exposure was estimated through one-year average concentrations of PM10, calculated with data from the Portuguese Environment Agency, attributed individually considering individuals' postal codes of residence. The mental well-being and the probable diagnosis of common mental disorders were ascertained through the five-item Mental Health Inventory scale. Linear and Robust Poisson regression models were computed to estimate change percentages, prevalence ratios (PR), and their 95% confidence intervals (95% CI)., Results: The median (interquartile range) concentration of PM10 was 18.6 (15.3 - 19.3) μg/m3. The mental well-being score was 72 (56 - 84) points, on a scale from 0 to 100. A probable diagnosis of common mental disorders was found in 22.7% (95% CI: 20.0 to 25.6). Long-term exposure to PM10 was associated with a non-statistically significant decrease in the mental well-being score [for each 10 μg/m³ increment in one-year average PM10 concentrations, there was a 2% (95% CI: -8 to 4) decrease], and with a non-statistically significant increase in the common mental health frequency (PR = 1.012, 95% CI: 0.979 to 1.045)., Conclusion: We did not find statistically significant associations between long-term exposure to PM10 and mental well-being or the frequency of probable diagnosis of common mental disorders. These results may be explained by the reduced variability in the exposure values, given the geographical distribution and functioning of the network of air quality monitoring stations. This study contributes with evidence for low levels of air pollutants, being one of the first to adjust for individual and aggregate-level variables. Moreover, to the best of our knowledge, this was the first nationally representative, population-based study conducted on the Portuguese population using real-life data. Maintaining a robust and nationwide air quality monitoring network is essential for obtaining quality exposure data.

12. Effects of a stigma reduction intervention on help-seeking behaviors in university students: A 2019-2021 randomized controlled trial.

Introduction: Stigma is one of the most frequently identified help-seeking barriers, but there is a lack of research on the effects of stigma reduction interventions on actual mental health help-seeking behaviors during crucial academic years. This research explores the effects of stigma on university students' mental health care help-seeking behaviors before and during the pandemic., Methods: A randomized control trial spanned from 2019 to 2021 at the University of Porto, along five evaluation moments, with students into one control group and two intervention groups-the interventions aimed to reduce depression stigma., Results: Among the 702 participants (mean age 18.87, 59.4 % female), the intervention groups significantly increased help-seeking behaviors. In 2020, the intervention groups, having reduced stigma, continued to demonstrate to be more prompt to seek mental health help. In 2021, 22 months, the effects of the intervention on help-seeking were no longer significant; however, participants in the intervention groups showed less severe symptomatology., Conclusion: Stigma reduction interventions have a pronounced effect on enhancing help-seeking behaviors among university students, even during times of crisis. 13. Predictors in ASD: The Importance of Parents' Perception.

- Abstract
Several predictors may influence children's developmental trajectories with Autism Spectrum Disorder (ASD), and parents' concerns may play an important role. This study aimed to investigate developmental trajectories of two groups of children with ASD to understand predictive factors, including parental perception. We examined the clinical features of a sample of 55 children with ASD at 3 and 6 years of age in two moments of evaluation to understand this process. We used the Autism Diagnostic Observation Schedule, (ADOS) in both moments. We selected two groups based on ADOS results at moment two: one group with a worse outcome (ADOS results above 8) and one group with a better outcome (ADOS results below 8 in the second moment). We also selected questions from a questionnaire (elaborated by the authors and used in clinical practice) applied to parents to understand if early parents' concerns may help to predict ASD prognosis. We found a significant association between imitation and playability and the child's prognostic. Also, Interactive Gestures, Beginning of Joint Attention, Reciprocity, and Pleasure in Interaction might help identify positive case evolution. - Abstract
Background: Leishmaniasis is a neglected tropical parasitic disease endemic in South Asia, East Africa, Latin America and the Middle East. It is associated with low socioeconomic status (SES) and responsible for considerable mortality and morbidity. Reports suggest that patients with leishmaniasis may have a higher risk of mental illness (MI), psychosocial morbidity (PM) and reduced quality of life (QoL), but this is not well characterised. The aim of this study was to conduct a systematic review to assess the reported impact of leishmaniasis on mental health and psychosocial wellbeing., Methods: A systematic review of the literature was carried out. Pre-specified criteria were applied to identify publications including observational quantitative studies or systematic reviews. Two reviewers screened all of the titles, abstracts and full-studies and a third reviewer was consulted for disagreements. Data was extracted from papers meeting the criteria and quality appraisal of the methods was performed using the Newcastle-Ottawa Scale or the Risk of Bias in Systematic Review tool., Results: A total of 14 studies were identified from 12,517 records. Nine cross-sectional, three case-control, one cohort study and one systematic review were included. Eleven assessed MI outcomes and were measured with tools specifically designed for this; nine measured PM and 12 measured QoL using validated measurement tools. Quality appraisal of the studies showed that six were of good quality. Cutaneous leishmaniasis and post kala-azar dermal leishmaniasis showed evidence of associated MI and PM including depression, anxiety and stigma, while all forms of disease showed decreased QoL. The findings were used to inform a proposed model and conceptual framework to show the possible links between leishmaniasis and mental health outcomes., Conclusion: There is evidence that leishmaniasis has an impact on MI, PM or QoL of patients and their families and this occurs in all the main subtypes of the disease. There are however large gaps in the evidence. - Abstract
Although studies have provided significant evidence about the role of RAS in mediating cancer risk in type 2 diabetes mellitus (DM), conclusions about the central molecular mechanisms underlying this disease remain to be reached, because this type of information requires an integrative multi-omics approach. In the current study, meta-analysis was performed on type 2 diabetes and breast, bladder, liver, pancreas, colon and rectum cancer-associated transcriptome data, and reporter biomolecules were identified at RNA, protein, and metabolite levels using the integration of gene expression profiles with genome-scale biomolecular networks in diabetes samples. This approach revealed that RAS biomarkers could be associated with cancer initiation and progression, which include metabolites (particularly, aminoacyl-tRNA biosynthesis and ABC transporters) as novel biomarker candidates and potential therapeutic targets. We detected downregulation and upregulation of differentially expressed genes (DEGs) in blood, pancreatic islets, liver and skeletal muscle from normal and diabetic patients. DEGs were combined with 211 renin-angiotensin-system related genes. Upregulated genes were enriched using Pathway analysis of cancer in pancreatic islets, blood and skeletal muscle samples. It seems that the changes in mRNA are contributing to the phenotypic changes in carcinogenesis, or that they are as a result of the phenotypic changes associated with the malignant transformation. Our analyses showed that Ctsg and Ednrb are downregulated in cancer samples. However, by immunohistochemistry experiments we observed that EDNRB protein showed increased expression in tumor samples. It is true that alterations in mRNA expression do not always reflect alterations in protein expression, since post-translational changes can occur in proteins. In this study, we report valuable data for further experimental and clinical analysis, because the proposed biomolecules have significant potential as systems biomarkers for screening or for therapeutic purposes in type 2 diabetes and cancer-associated pathways., (Copyright © 2019 Elsevier B.V. - Abstract
The study elaborated evaluation patterns of the components of the surgical scrub in hospitals of the district of São Paulo. The most important difficulty was the lack of rules and consent on the types of the fabrics, as a microbiological and contact with blood and other fluids barrier. However, it was possible to identify positive and problematic aspects. It was observed that the components are similar among the hospitals, considering the design and the way of fabrics production. Gloves were the ones with the best quality results, while shootweares were the worst. The most problematic aspect was referred to the acquisition controls and the re-use process of the components.

- Abstract
The study elaborated approaches for adequate use of the surgical scrub and it evaluated, through systematized observation, the performance of the surgical team during the surgery. The adopted procedure was very valid, allowing to recognize positive and problematic aspects. Except the protecting glasses, all the other components of the surgical scrub were used. The subject located in the use form, where caps and masks were more problematic among anaesthesists and circulate nurses; apron and gloves among surgeons and, mainly, scrub nurses.
