Hintergrund und Ziel der Studie Die nekrotisierende Enterokolitis ist ein entzündliches Krankheitsbild mit schwerwiegendem Verlauf, welches nicht selten zum Tod des jungen Patienten führen kann. Um eine optimale Therapie zu gewährleisten ist es essentiell, frühzeitig die NEC zu erkennen und gegebenenfalls chirurgisch zu intervenieren. Das Ziel der Studie war es, Faktoren zur Vorhersage eines BELL III Stadiums zu identifizieren. Methoden Untersucht wurden in einem retrospektiven Studiendesign die Daten aller Neugeborener im Zeitraum von 2010 bis 2020, welche auf der Neugeborenen-Intensivstation der Kinderklinik Erlangen des Universitätsklinikums der Friedrich-Alexander-Universität Erlangen-Nürnberg (FAU, Perinatalzentrum I) mit den Symptomen einer NEC behandelt wurden. Aus der Studie ausgeschlossen wurden Patienten mit unzureichender Datenlage sowie anderen gastrointestinalen Krankheitsbildern, wie z.B. Volvulus. Klinische und diagnostische Parameter wurden über eine multivariate Regressionsanalyse auf Assoziationen zu fortgeschrittenen Krankheitsstadien geprüft. Ergebnisse und Beobachtungen Aus der multivariaten Regressionsanalyse ergab sich für die folgenden Faktoren ein signifikantes Ergebnis: Auffälligkeiten in der sonographischen Untersuchung (p=0,037; OR=6,496), die Gabe von EK-Transfusionen (p=0,027; OR=3,298) und das Vorliegen eines Herzfehlers (p=0,015; OR=1,922), wobei CHD (OR= 5,478) von PDA (OR=2,850) unterschieden werden muss. Darauf basierend konnte ein Score zur Vorhersage eines BELL III Stadiums mit den drei Einflussgrößen erstellt werden, der sich im klinischen Alltag anwenden lässt. Schlussfolgerungen und Diskussion Klinische Faktoren scheinen maßgeblich an dem Krankheitsbild der NEC beteiligt zu sein und können innerhalb eines klinischen Scores zur praktischen Anwendung korreliert werden. Gerade die Bedeutung der Sonographie als zentrales diagnostisches Kriterium muss hierbei geprüft werden. Dieser Score sollte in Zukunft in größer angelegten klinischen Studien getestet werden, um seine Aussagekräftigkeit zu validieren. Background and aim of the study Necrotizing enterocolitis is an inflammatory disease of the gut which can have severe progression and lead to death. To ensure optimal therapy it is essential to diagnose NEC early and if necessary, start surgical intervention. The goal of our study was to identify risk factors that predict NEC in Bell III stages. Methods In this retrospective study, we researched all data of newborns in a period from 2010 to 2020, who were treated with the symptoms of NEC at the neonatal intensive care unit of the children’s hospital of the university hospital of the Friedrich-Alexander-Universität Erlangen-Nürnberg (FAU, Perinatalzentrum I). We excluded all patients with insufficient data and other concomitant diseases of the gut. Clinical and diagnostical variables were assessed regarding their association with severe clinical courses using a multivariate logistic regression analysis. Results and observations The multivariate regression analysis shows significant results for the following variables: Sonographical abnormalities (p=0,037; OR=6,496), red blood cell transfusion (p=0,027; OR=3,298), cardiac anatomy (p=0,015; OR=1,922), for patients with CHD (OR=5,478) and PDA (OR=2,850). Based on that we could establish a score to predict Bell III stages. The score is applicable in daily clinical routine. Conclusions and discussion Clinical variables may influence the clinical course of NEC substantially. They were associated within a score for daily application in clinical routine. Ultrasound was hereby seen as an important method to assess the patient’s status. The score has to be verified in larger scale clinical studies to assess its clinical value.

Untersuchung des Überlebens dentaler schmaler Implatate aus einer Titan-Zirkonium-Legierung gegenüber Implantaten normalen Durchmessers als retrospektive Studie

Hintergrund und Ziele Sportklettern erfreute sich in den letzten Jahrzehnten wachsender Beliebtheit. Neue wissenschaftliche Erkenntnisse hinsichtlich Trainingsmethodik, ein sich verändernder Kletterstil und die Tatsache, dass Sportklettern und vor allem Bouldern immer mehr zum Breitensport geworden ist, haben in den letzten Jahren ein verändertes kletterspezifisches Verletzungsmuster entstehen lassen. Während die Finger nach wie vor am häufigsten von Kletterverletzungen betroffen sind, rangiert die Kapsulitis innerhalb der Fingerverletzungen mittlerweile auf Rang drei hinter Ringbandverletzungen und Sehnenscheidenentzündungen. Die beiden Letzteren waren bereits öfter Bestandteil wissenschaftlicher Untersuchungen, über die Entität und Therapie der Kapsulitis der Fingergelenke hingegen fehlen derartige Studien bislang. Somit ist der Zweck der vorliegenden Arbeit, Entität und Genese der Kapsulitis zu untersuchen, sowie Ergebnisse der Kapsulitistherapie nach einem institutsinternen Behandlungsalgorithmus auszuwerten. Material und Methode Die Studie untersucht Kletterer, die zwischen 2015 und 2018 in der Abteilung Sportorthopädie, -traumatologie und -medizin des Klinikum Bamberg, Deutschland, aufgrund einer Kapsulitis vorstellig wurden. Alle Patienten wurden gemäß eines institutsinternen Behandlungsschemas therapiert, welches sich am Schweregrad der Verletzung und der Dauer der Symptome bei Erstvorstellung orientiert. Patienten mit Stadium I (Symptome seit < 6 Wochen) wurden konservativ behandelt; Patienten mit Stadium II (Symptome > 6 Wochen) mittels zweimaliger Kortison-Injektion im Abstand von 7-10 Tagen; bei Patienten mit Stadium III (keine Verbesserung der Beschwerden nach Kortison-Injektion) wurde eine Radiosynoviorthese (RSO) durchgeführt. Die Patienten wurden 28,5 ± 11,8 (15-71) Monate nach Abschluss der Behandlung anhand eines Fragebogens re-evaluiert, welcher u.a. die Erhebung eines Scores nach Buck-Gramcko, sowie eines funktionellen und eines sportartspezifischen Scores beinhaltet. Die deskriptive Statistik wurde mit Microsoft Excel (Microsoft Office Home and Student 2019, Redmond, Washington, USA) erhoben; GNU PSPP Statistical Analysis Software (Free Software Foundation Inc. 2007, Version 1.4.1-g79ad47, Boston, Massachusetts, USA) wurde für die vergleichende statistische Analyse verwendet. Ergebnisse 69 Finger von 50 Patienten wurde therapiert. Der am häufigsten betroffene Finger war der Mittelfinger (60,9%), das PIP-Gelenk das am häufigsten betroffene Gelenk (82,6%). 29 Finger erhielten eine konservative, non-invasive Behandlung, 41 Finger Kortison-Injektionen und 5 Finger wurden einer RSO unterzogen. Der Buck-Gramcko-Score war in 72,5% der Fälle exzellent, in 24,6% gut. 21 Finger waren zum Zeitpunkt der Befragung in ihrer Bewegung leicht eingeschränkt. 87% aller Finger erzielten ein sehr gutes oder gutes allgemein-funktionelles Ergebnis, 84% ein sehr gutes oder gutes sportartspezifisches Ergebnis. Es wurde keine signifikante Veränderung des Kletterlevels vor und nach Therapie der Kapsulitis festgestellt. 60% aller Kletterer waren zum Zeitpunkt der Befragung schmerzfrei, 40% hatten noch leichte Schmerzen (VAS 0,8 ± 1,4 (0-8)) während oder nach dem Klettern. Schlussfolgerungen und Diskussion Die guten bis ausgezeichneten Ergebnisse rechtfertigen die Kapsulitistherapie gemäß des Behandlungsschemas. Recherchen zur Entität und Genese der Kapsulitis kamen zu dem Schluss: die Kapsulitis ist das klinische Korrelat einer histopathologischen Synovialitis. Es wird angenommen, dass unphysiologischer Stress auf die Fingergelenke durch den Klettersport, wie Mikrotraumata oder wiederholte Überlastung, in Signale auf molekularer Ebene umgewandelt wird, die das angeborene Immunsystem aktivieren und zu einer Entzündung des Synoviums führen. Ob eine Kapsulitis mit einer Früharthrose gleichzusetzen ist oder sich aus ihr – ohne iatrogene Intervention – eine Arthrose entwickeln kann, bedarf weiterer wissenschaftlicher Auseinandersetzung. Setting and Objections Sportclimbing has become very popular within the past decades. Due to scientific enhancement of training methods, a developing style of motion and the fact that more and more recreational athletes perform climbing or bouldering, climbing specific injury patterns have changed over the years. While fingers remain the most common injured body part in sportclimbing, the capsulitis of the finger joints is now the third most frequent differential diagnosis among climbing related finger injuries after pulley injuries and tenosynovitis. For the latter having been well-examined, no scientific data exist on the entity and therapy of the capsulitis of the interphalangeal finger joints. Thus, the purpose of this study is to evaluate the outcome of patient treatment according to a certain regimen and to research the genesis of climbing-induced capsulitis. Material and Methods Rock climbing patients that were treated in the Department of Sports Orthopedics, Sports Medicine and Sports Traumatology at the Klinikum Bamberg in Germany in between 2015 and 2018 suffering from capsulitis were included in the study. All patients underwent stage-related therapy according to severity and time of onset of symptoms. Stage-1-cases that had symptoms for less than 6 weeks were treated conservatively; stage-2-cases (symptoms > 6 weeks) received twice a steroid injection within an interval of 7-10 days; patients who failed injection therapy (stage 3) underwent radiosynoviorthesis (RSO). Re-evaluation was performed 28,5 ± 11,8 (15-71) months after final treatment using a comprehensive questionnaire including Buck-Gramcko-Score as well as functional and sport specific score. Microsoft Excel (Microsoft Office Home and Student 2019, Redmond, Washington, USA) was used for data collection and GNU PSPP Statistical Analysis Software (Free Software Foundation Inc. 2007, Version 1.4.1-g79ad47, Boston, Massachusetts, USA) for statistical analysis. Results 69 fingers of 50 patients were treated. The middle finger was the most frequently involved finger (60,9%), the proximal interphalangeal (PIP) joint the most frequently involved joint (82,6%). 29 fingers had conservative, non-invasive treatment, 41 steroid injection and 5 underwent RSO . The Buck-Gramcko-Score was excellent in 72,5% and good in 24,6% of the cases. 21 fingers maintained a slightly reduced range of motion. 87% of the fingers treated had a very good or good functional outcome and 84% a very good or good sport specific outcome. There was no statistically significant difference between the climbing level before and after capsulitis therapy. 60% of all climbers were pain-free at the time of the re-evaluation, 40% still had slight pain (VAS 0,8 ± 1,4 (0-8)) during or after climbing. Conclusions The good to excellent results justify the capsulitis therapy performed according to the treatment regimen. As for researching the entity of the capsulitis, it is the clinical equivalent of a histological synovitis. It is assumed that unphysiological stress on finger joints through rock climbing, such as microtrauma or repetitive overuse, can be converted into molecular signals that activate the innate immune system and cause inflammation of the synovium. Whether capsulitis is an early stage of osteoarthritis or may lead to osteoarthritis if untreated will need further investigation.

Zusammenfassung Hintergrund und Ziele Die Fallot`sche Tetralogie ist einer der häufigsten angeborenen Herzfehler. Das Ziel der vorliegenden Arbeit war es die Langzeitergebnisse und die Freiheit von einem Pulmonalklappenersatz nach Korrektur einer Fallot`schen Tetralogie zu evaluieren und mit internationalen Studien zu vergleichen. Zusätzlich wurde der Einfluss möglicher anatomischer Variationen und der verwendeten chirurgischen Techniken am rechtsventrikulären Ausflusstrakt (RVOT = right ventriclar outflow tract) analysiert. Die Obstruktion im rechtsventrikulären Ausflusstrakt wurde entweder mit einem transanulärem Patch, einem klappenerhaltenden Verfahren oder mit der primären Implantation eines Conduits vom rechten Ventrikel zur Pulmonalarterienbifurkation beseitigt. Methoden Krankenhausaufzeichnungen und Operationsprotokolle von allen Patienten, die zwischen April 1980 und Juni 2017 operiert wurden, wurden geprüft. Insgesamt 306 Patienten erfüllten die Einschlusskriterien und wurden anhand ihrer anatomischen Gegebenheiten in verschiedene Untergruppen eingeteilt. Entscheidend hierfür war die Morphologie des RVOT und des Ventrikelseptumdefektes und ob eine Fallot`sche Tetralogie mit Pulmonalstenose, Pulmonalatresie oder die double-outlet-right-ventricle (DORV) Variante der Fallot`schen Tetralogie vorlag. Ergebnisse und Beobachtung Die Patientenunterlagen von 306 behandelten Kindern wurden untersucht und analysiert. Es gab 21 Todesfälle (6,9%), von denen 14 im Krankenhaus nach der Primärkorrektur verstarben (4,6%). Vier Todesfälle ereigneten sich nach einem Pulmonalklappenersatz (1,3%) und ein Patient starb nach einer gastroenterologischen Operation (0,3%). Es gab zwei späte Todesfälle (0,7%), mehr als 30 Tage nach der Operation, zu verzeichnen. Die langjährige Perspektive in der Behandlung führte dazu, dass keine Frühmortalität mehr nach einer Primärkorrektur in den letzten 12 Jahren beobachtet wurde. Im Verlauf erhielten 91 Patienten (29,7%) einen Pulmonalklappenersatz nach 12,1 ± 7,0 Jahren mit einer Frühmortalität von 4,4% (n = 4). Nach Pulmonalklappenersatz konnte keine Spätmortalität mehr beobachtet werden. Es gab signifikante Unterschiede in der Freiheit von einer Reoperation nach den verschiedenen Korrekturverfahren. Nach einer transanulären Korrektur betrug sie 17,0 ± 2,2 Jahre, nach einem klappenerhaltendem Verfahren 25,7 ± 3,5 Jahre und nach einer Korrektur mit Verwendung eines Conduits 10,9 ± 1,0 Jahre (p-Wert < 0,001). Als signifikante Risikofaktoren für einen Pulmonalklappenersatz zeigten sich nach multivariater Varianzanalyse die Notwendigkeit einer transanulären Korrektur (p-Wert = 0,016), eine primäre Palliation vor einer Korrektur (p-Wert < 0,001), das Vorhandensein von MAPCAs (p-Wert = 0,023) und ein Z-Score der Pulmonalarterie mit Pulmonalklappenring unter -4,0 (p-Wert = 0,040). Der einzige prädisponierende Risikofaktor für eine höhere Mortalität stellte in unserer Studie das Vorhandensein einer Trisomie-21 dar (p-Wert < 0,001). Schlussfolgerung Die Korrektur der Fallot`schen Tetralogie hat bis heute eine positive Langzeitprognose mit niedriger Morbidität und Mortalität. Das Vorhandensein einer Trisomie-21 ist ein Risikofaktor für eine höhere Mortalität. Ein Pulmonalklappenersatz ist eine häufig benötigte Sekundäroperation mit einem beachtlich niedrigen Risiko, sogar in Fällen mit mehreren vorgegangenen Operationen. Primäre palliative Eingriffe, eine transanuläre Korrektur, das Vorhandensein von MAPCAs und ein Z-Score der Pulmonalklappe unter -4,0 sind assoziiert mit einem früheren Pulmonalklappenersatz. Extremvarianten der Pulmonalklappenanomalie, die zu einer Funktionslosigkeit des Ventrikels führen (klappendurchtrennende Verfahren/transanulärer Patch) oder einen Klappenersatz erfordern (Fallot`sche Tetralogie mit Pulmonalatresie) führen zu früheren Reoperationen zeigen aber keine negative Auswirkung auf das Überleben.

Hintergrund und Ziele Eine der häufigsten Komplikationen nach viszeralchirurgischen Operationen mit Anlage einer Darmanastomose ist die Anastomoseninsuffizienz. Allgemein gesprochen kann eine Anastomoseninsuffizienz als ein Leck zwischen zwei miteinander verbundenen Hohlgefäßen bezeichnet werden. Sie erhöht die postoperative Mortalität und setzt die Lebensqualität herab. Der postoperative Krankenhausaufenthalt verlängert sich auf bis zu mehrere Wochen und die zusätzlichen Kosten belaufen sich zum Teil auf das Fünffache des Ursprünglichen. Eine frühe Diagnose der Erkrankung ist essenziell, um schwere lebensbedrohliche Komplikationen zu vermeiden. Ziel dieser Arbeit ist es, anhand von Veränderungen bei Routinelaborparametern, welche standardmäßig bei Patienten im postoperativen Verlauf erfasst werden, auf eine Anastomoseninsuffizienz schließen zu können. Hierzu wurden typische, chemische Laborparameter im Tagesverlauf vor und nach der Diagnosestellung der Erkrankung retrospektiv analysiert und erörtert. Methoden Die retrospektive Auswahl betraf Patienten der Chirurgischen- und Anästhesiologischen Klinik des Universitätsklinikums Erlangen, welche im Zeitraum vom 01.01.2013 bis 31.12.2018 kolorektal operiert wurden und eine Darmanastomose erhalten haben. Folgende Laborparameter wurden erhoben: Albumin, anorganisches Phosphat, Chlorid, C-reaktives Protein, Gesamteiweiß, Kalium, Leukozyten, Magnesium, Natrium, Procalcitonin, Quick, Thrombozyten, partielle Thromboplastinzeit, Laktat. Insgesamt wurden 2180 Datensätze erfasst. Am Ende dieser Datenaufbereitung enthielt die Fallgruppe insgesamt 37, die Kontrollgruppe insgesamt 1530 Patienten. Alle Daten wurden anonymisiert und anschließend bearbeitet. Statistisch ausgewertet wurden die Laborwerte vier Tage vor der Diagnosestellung und die ersten zwei Tage nach der Diagnosestellung. Ergebnisse und Beobachtungen Die Rate der kolorektalen Anastomoseninsuffizienzen lag bei 2,4 Prozent. Bei einer Anastomoseninsuffizienz zeigte sich vor allem bei dem Entzündungsparameter CRP bereits drei Tage vor der Diagnosestellung ein signifikanter Anstieg (p=0,022). Der Leukozyten-Spiegel war ab zwei Tage vor Diagnosestellung signifikant erhöht (p=0,021). Der Gerinnungsparameter Quick fiel in dieser Studie im postoperativen Verlauf kontinuierlich ab und war bereits zwei Tage vor Diagnosestellung signifikant (p=0,036) erniedrigt gegenüber der Kontrollgruppe. Bei den Plasmaeiweißen Gesamteiweiß und Albumin fiel der Spiegel ebenso wie bei dem Gerinnungsparameter Quick im postoperativen Verlauf kontinuierlich ab und war einen Tag vor Diagnosestellung im Vergleich zur Kontrollgruppe signifikant erniedrigt (p=0,024 und p=0,007). Der Natriumspiegel zeigte am vierten und dritten Tag vor der Diagnosestellung einen signifikanten Unterschied zwischen der Fall- und der Kontrollgruppe (p=0,001 und p=0,033). Für alle anderen erhobenen Laborparameter konnten keine signifikanten Unterschiede im postoperativen Verlauf zwischen der Fall- und der Kontrollgruppe festgestellt werden. Schlussfolgerungen In dieser Studie konnte gezeigt werden, dass ein Anstieg der Laborparameter CRP und Leukozyten zu einer drei bzw. zwei Tage früheren Detektion von kolorektalen Anastomoseninsuffizienz geeignet sind. Ebenso bietet ein Anstieg des Natriumspiegels bereits vier Tage vor Diagnose einen ersten frühen Hinweis auf die Entwicklung einer kolorektalen Anastomoseninsuffizienz. Ein kontinuierlicher Abfall des Plasmaeiweiß- und Albuminspiegels kann als Frühwarnsystem zu einer im Schnitt ein Tag früheren Detektion einer kolorektalen Anastomoseninsuffizienz dienen. Zusätzlich kann ein Abfall des Quick Wertes zu einer zwei Tage früheren Diagnose einer Anastomoseninsuffizienz führen. Zur weiteren Verifizierung dieser Ergebnisse sind weiterführende Studien und Diagnostiken mit höheren Fallzahlen einer Anastomoseninsuffizienz nötig. Abstract Background and Aims An anastomotic insufficiency is one of the most common complications after visceral surgery including the creation of an intestinal anastomosis. Generally speaking, an anastomotic insufficiency is defined as a leak between two interconnected hollow vessels. Such a leak increases the postsurgical mortality rate and lowers the quality of life. The postoperative duration of hospitalization is extended to several weeks and the additional costs are partly up to five times higher than that of the original. An early diagnosis of this condition is crucial to prevent serious, life-threatening complications. This paper’s aim is the inference of an anastomotic insufficiency on the basis of altered routine laboratory parameters, which are collected and measured by default in patients’ postoperative course. For this purpose, typical chemical laboratory parameters were retrospectively analyzed and discussed in the course of the days before and after the diagnosis of the disease. Methods The retrospective selection included patients of the surgical and anesthesiologic clinic of the University Hospital of Erlangen who underwent a colorectal surgery and received an intestinal anastomosis between January 1, 2013 and December 31, 2018. The following laboratory parameters were recorded: albumin, inorganic phosphate, chloride, c-reactive protein, total protein, potassium, leukocytes, magnesium, sodium, procalcitonin, quick, thrombocytes, partial thromboplastin time, and lactate. 2180 data sets were recorded in total. At the end of this phase of data preparation, the case group contained 37 and the control group 1530 patients. All data has first been anonymized and then processed. The laboratory values of the four days prior to the diagnosis and the two days after the diagnosis have been statistically evaluated. Results and Observation The rate of colorectal anastomotic insufficiency was 2.4%. In case of an anastomotic insufficiency, the inflammation parameter CRP in particular was significantly elevated (p=0.022) three days prior to diagnosis. The level of leukocytes was significantly increased from two days before the diagnosis (p=0.021). Quick, a coagulation parameter, decreased in this study in the course of the postoperative process steadily and was already significantly lower two days before diagnosis compared to the control group. The level of plasma proteins, more precisely, total protein and albumin, also decreased in the postoperative course continually and was significantly lower than the control group’s result one day before the diagnosis (p=0.024 and p=0.007). The case group’s and control group’s sodium levels showed a significant difference on day four and three before diagnosis (p=0.001 and p=0.033). All other laboratory parameters considered did not show any significant difference when comparing postsurgical courses of patients belonging to the case and control group. Conclusion This study has shown that two laboratory parameters, CRP and Quick, are highly suitable for a three respectively two days earlier detection of an anastomotic insufficiency. A steady rise in the leukocyte level or a continuous fall of plasma protein and albumin can also serve as an early warning system in order to detect a colorectal anastomotic insufficiency. In addition, an increase of the sodium level on the four days preceding the diagnosis serves as an early indication for the development of a colorectal anastomotic insufficiency. For further verification of these results, additional studies and diagnostic investigations with a higher number of cases suffering from an anastomotic insufficiency are required.

Die bariatrische Chirurgie ist momentan die einzige Therapieoption der morbiden Adipositas mit der eine langfristige Gewichtsreduktion erreicht werden kann. Unter den Operationsmethoden gilt der RYGB als eine der wirksamsten Behandlungen bezogen auf den Gewichtsverlust und die Verbesserung von Begleiterkrankungen wie dem Diabetes mellitus Typ 2. Darüber hinaus belegt eine wachsende Zahl an Veröffentlichungen, dass der RYGB den Zustand chronischer Entzündung, wie er typischerweise mit Adipositas einhergeht, verringern kann. Diese sogenannte Endotoxämie geht unter anderem mit einer gestörten Integrität der intestinalen Epithelbarriere einher. Ziel der vorliegenden Arbeit war es eine Analyse der Veränderungen von für die Darmbarriere wichtigen Junktionsproteinen vorzunehmen, um eine Grundlage für künftige mechanistische Untersuchungen zu schaffen. Dafür wurden die Veränderungen von Barriereproteinen in Vollwandresektaten des Duodenums, des Jejunums, des Ileums sowie des Kolons von Ratten, die einen RYGB erhalten hatten, mittels Western Blot Untersuchungen quantifiziert. Als Kontrollgruppe dienten schein-operierte Ratten. Es kam zu tiefgreifenden Veränderungen der analysierten Barriereproteine in den Vollwandresektaten. Interessanterweise unterschieden sich die Verteilungsmuster der Veränderungen der Barriereproteinte deutlich zwischen den einzelnen Darmregionen. Um herauszufinden, ob diese Veränderungen durch regionale Veränderungen der Mikroumgebung nach RYGB- Operation induziert wurden, wurden im reduktionistischen Zellkultursystem Stuhl- Transferexperimente durchgeführt. Caco2-Zellkulturen dienten hierbei als Modell für die intestinale epitheliale Barriere. Es wurden funktionelle Messungen und quantitative Analysen der Veränderungen der Barriereproteine der Zellkultur durchgeführt. Die Funktionsmessungen zeigten, dass der Inhalt des Duodenums, des Jejunums sowie des Kolons deutliche barrierestabilisierende Effekte auf die Caco2-Zellmonolayer hatte. Zudem zeigten sich tiefgreifende Veränderungen der untersuchten Barriereproteine. Zusammenfassend wurde in der vorliegenden Arbeit erstmals eine regionenspezfische Regulation der intestinalen Barriereproteine in Korrelation mit funktionellen Messungen nach RYGB nachgewiesen., Bariatric surgery is currently the only treatment option for morbid obesity that can achieve long-term weight loss. Among surgical methods, RYGB is considered one of the most effective treatments in terms of weight loss and improvement of associated diseases such as type 2 diabetes mellitus. In addition, a growing number of publications demonstrate that RYGB can reduce the state of chronic inflammation typically associated with obesity. This so-called endotoxemia is associated with impaired integrity of the intestinal epithelial barrier. The aim of the present work was to perform an analysis of changes in junctional proteins which are important for the intestinal barrier in order to provide a basis for future mechanistic studies. For this purpose, the changes of barrier proteins in whole-wall resections of the duodenum, jejunum, ileum, and colon of rats that had received an RYGB were quantified by Western blot studies. Sham-operated rats served as the control group. Profound changes in the analyzed barrier proteins occurred in the whole-wall resectates. Interestingly, the distribution patterns of barrier protein changes differed markedly between intestinal regions. To determine whether these changes were induced by regional changes in the microenvironment after RYGB- surgery, stool transfer experiments were performed in the reductionist cell culture system. Caco2 cell cultures served here as a model for the intestinal epithelial barrier. Functional measurements and quantitative analyses of the changes in the cell culture barrier proteins were performed. The functional measurements showed that the contents of the duodenum, jejunum, as well as the colon had significant barrier-stabilizing effects on the Caco2 cell monolayers. In addition, profound changes in the barrier proteins examined were evident. In summary, the present work was the first to demonstrate region-specific regulation of intestinal barrier proteins in correlation with functional measurements by RYGB.

Background and objectives: Homeostasis of the corneal epithelium, which is an important prerequisite for corneal transparency and visual function, relies on a population of unipotent stem / progenitor cells at the corneoscleral limbus of the eye. These stem cells reside in a highly specialized microenvironment known as the limbal niche, which comprises several types of supporting niche cells and a complex extracellular matrix mediating biochemical and biophysical signals to regulate stem cell quiescence, proliferation and differentiation. Depletion of this stem cell reservoir and / or destruction of its niche microenvironment can cause severe ocular surface disease and significant visual deterioration up to blindness. Transplantation of ex-vivo expanded limbal epithelial stem / progenitor cells (LEPCs) is an established therapeutic strategy to transiently regenerate the damaged corneal surface, although long-term clinical outcomes are less satisfactory. It became increasingly clear that reconstitution of a functional stem cell pool and long-term clinical success depend on the maintenance of LEPCs in the graft, which is however not ensured by standard culture techniques that largely disregard the physiological niche microenvironment. Therefore, current developments in stem cell-based tissue engineering aim to mimic the native stem cell niche and to present appropriate microenvironmental cues (matrix components, niche cells, soluble effectors, mechanical stimuli) in order to maintain stem cell function within the graft. Thus, the purpose of the present study was to analyze the effects of different extracellular matrix (ECM) components, which are preferentially expressed in the limbal niche, on the differentiation and adhesion, as reflected by expression of integrins, of LEPCs in-vitro. These analyses might give some insights into the suitability of specific matrix components for LEPC cultivation and for corneal epithelial tissue engineering. Methods: To achieve this objective, an in-vitro LEPC cell culture model was established. Primary LEPCs were isolated from human donor corneas with appropriate research consent and cultivated in defined keratinocyte serum-free medium. Purified LEPCs at passage 1 (n=4) were seeded into 24 well-plates coated with human recombinant matrix proteins (collagen IV, fibronectin, vitronectin, tenascin-C, agrin, nephronectin, chondroitin sulfate, JAM-C, laminin 111, laminin 332, laminin 511-E8 and laminin γ3) and grown for 3 to max. 5 days until proximate confluency. Standard tissue culture treated plates were used as control. All experiments were performed in triplicates. Cell growth, proliferation and changes in morphology were monitored daily by phase contrast microscopy. Messenger RNA expression of differentiation-related marker genes (cytokeratin (CK) CK3 and CK12) and stem / progenitor cell-associated genes (CK15, N-cadherin, ABCG2 and p63α) was analysed by quantitative real-time PCR. Expression levels of integrin subunits α1, α2, α3, α4, α5, α6, αV, β1, β2, β4, β5, β6 and β8 were analysed on a subset of selected matrix substrates (agrin, chondroitin sulfate, laminin 511-E8, collagen IV and tenascin-C) in addition. Protein expression of integrin subunits α2, α3 and αV was assessed by immunocytochemistry. Statistical comparisons between groups were performed using an unpaired two-tailed t-test. Results: Specific ECM proteins differently affected mRNA expression levels of differentiation and stemness markers in LEPCs without showing a consistent pattern. However, most of those proteins which downregulated expression of the cornea-specific differentiation markers CK3 (i.e., collagen type IV, fibronectin, agrin, laminin 332, and laminin γ3) and CK12 (i.e., vitronectin, nephronectin, laminin 111 and chondroitin sulfate) also upregulated at least one of the stemness-related genes CK15, ABCG2, and N-cadherin, suggesting the maintenance of a stem cell phenotype. Compared to controls, laminin 511-E8 and chondroitin sulfate were found to downregulate the proliferation marker p63α, which may also indicate promotion of a non-proliferative stem cell phenotype. The integrin subunits α2, α3, α6, αV, β1, β4, β5, β6 and β8 were most highly expressed in LEPCs, indicating that LEPCs mainly adhere to the ECM by the laminin-binding integrins α3β1, α6β1, and α6β4 as well as the fibronectin-binding integrin αVβ6. Most of the proteins used, particularly agrin, collagen type IV and laminin 511-E8, downregulated expression levels of integrin subunits compared to uncoated controls. Notably, all of the most highly expressed integrin subunits, i.e., integrin α3, α6, β1, β4, and β6, were consistently downregulated in LEPCs by collagen type IV. These findings may indicate that LEPCs expanded on collagen type IV, agrin and laminin 511-E8 may lose their firm attachment to the culture substrate and thereby acquire a migratory, proliferative phenotype. In contrast, expression levels of integrins α2, αV and β8 were individually upregulated by laminin 511-E8, tenascin-C, agrin and chondroitin sulfate, suggesting increased cell-matrix adhesion. On the protein level, laminin 511-E8 and collagen IV induced increased cell membrane staining for integrin α2 and α3, but reduced cytoplasmic staining for integrin α3 and αV. Conclusion and discussion: ECM molecules are widely believed to provide important stimuli for stem cell maintenance within their native niche, but also for their correct differentiation. Therefore, new tissue engineering strategies aim at providing niche-associated molecules in order to preserve the stem cell phenotype during cultivation and after transplantation, which is the primary objective in stem cell-based regenerative therapies. A defined ECM environment is a central part of the creation of an authentic in-vitro niche for an advancement of our current therapeutic strategies for ocular surface reconstruction. In order to advance such novel approaches, this study analysed the effects of niche-specific matrix components on the differentiation status of LEPCs in-vitro. The present findings showed a differential influence of matrix components on LEPC differentiation and integrin expression reflecting their adhesive properties. The data may provide a foundation for further functional studies and for the use of different ECM components to prevent premature differentiation of stem cells in tissue engineering strategies. The incorporation of human recombinant matrix proteins, such as agrin, laminin 332, laminin γ3, vitronectin and fibronectin, or chemically defined fragments, such as laminin 511-E8, into biosynthetic scaffolds represents a promising approach for xenobiotic-free corneal epithelial tissue engineering. Translation of such novel approaches into clinical settings is expected to improve long-term outcomes of limbal stem cell transplantation for ocular surface reconstruction. Nevertheless, a more detailed characterization of the molecular composition of the human limbal niche and examination of a broader panel of matrix components will be important to analyze cell function, behavior, phenotype and long-term survival of LEPCs in further studies. In addition to extended in-vitro investigations, advanced (pre-)clinical trials are necessary to obtain reliable experimental and clinical application data.

Zusammenfassung Hintergrund und Ziele Eine Mastektomie ist eine häufige Folge der Diagnose Mammakarzinom und stellt eine physische und psychische Belastung für die Patientin dar. Die Rekonstruktion der Brust trägt zur Wiedererlangung der weiblichen Körperintegrität bei. Die autologe Brustrekonstruktion mittels DIEP-Lappenplastik (deep inferior epigastric perforator flap) oder die freie ms-TRAM-Lappenplastik (muscle-sparing transverse rectus abdominis myocutaneous flap) bietet sich als Option an. Im Rahmen dieser Studie wurden retrospektiv intraoperative Perfusionsanalysen im Rahmen freier DIEP- und ms-TRAM-Lappenplastiken ausgewertet. Es wird der Frage nachgegangen, welche neuen Erkenntnisse die Indocyaningrün gestützte Laserangiographie im Vergleich zu bisher bekannten Perfusionsmustern liefert und welche Auswirkungen diese Erkenntnisse auf das Operationsverfahren hat. Methoden Zwischen Februar 2014 und Februar 2019 wurden 100 Patientinnen der Plastisch- und Handchirurgischen Klinik des Universitätsklinikum Erlangen in die Studie aufgenommen. Es erfolgte eine Perfusionsanalyse von DIEP- und ms-TRAM-Lappenplastiken nach vollständiger Lappenhebung und Gefäßpräparation. Videos wurden mit einer SPY Elite Laserkamera (Stryker Corporation, 2825 Airview Boulevard Kalamazoo, MI 49002, former “Novadaq Technologies Inc., Toronto, Canada“) nach Gabe von Indocyaningrün erstellt und Contourlevel von 20 oder 30 % analysiert. Ergebnisse Bei einem Contourlevel von 20 % zeigten sich keine signifikanten Unterschiede zwischen den Untergruppen im Perfusionsmuster. Im Gegensatz dazu zeigte sich bei 30%-igem Contorulevel die Konstellation der DIEP-Lappenplastiken mit medialen Perforatoren als signifikant unterschiedlich zu den DIEP-Lappenplastiken mit einem lateralen Perforator. Sowohl bei einem Contourlevel von 20 % (p = 0,068) als auch 30 % (p = 0,058) ließ sich kein signifikanter Unterschied zwischen den kontralateral-medialen Durchblutungsmustern der DIEP-Lappenplastik feststellen. Schlussfolgerung und Zusammenfassung Die aktuellen Daten bestätigen die hohe Sensitivität der ICG-Angiographie hinsichtlich arteriell minderperfundiertem Gewebe und bieten neue Erkenntnisse zu Perfusionsmustern in Abhängigkeit von der Perforatorlokalisation und Lappenauswahl. Zusätzlich gibt die Studie Hinweise auf die im Rahmen der ICG-Angiographie anwendbaren Contourlevel.

Die Ausbildung eines adäquaten Gefäßnetzwerkes ist elementar für die Knochenentwicklung, -heilung und -regeneration, da neben der Versorgung des Knochens mit Sauerstoff und Mineralien, die Endothelzellen mittels zellkontaktspezifischer Mechanismen Einfluss auf die Knochenphysiologie ausüben. Um ein besseres Verständnis des Zusammenhangs zwischen Angiogenese und Osteogenese zu generieren und um die Erkenntnisse für die Entwicklung künstlicher Knochenersatzgewebe zu nutzen, fokussieren sich viele Arbeiten auf die Interaktion zwischen Osteoprogenitor- und Endothelzellen. Ein vielversprechender Ansatz, stellt in diesem Zusammenhang die direkte Kokultur aus humanen Umbilikalvenenendothelzellen (HUVEC) und Stammzellen des Fettgewebes (adipose derived stem cell = ADSC) dar. Das Ziel dieser Doktorarbeit mit dem Titel „Züchtung bioartifizieller Knochenersatzgewebe mit Stammzellen aus dem Fettgewebe und Endothelzellen: Einfluss des Zellverhältnisses" ist die Untersuchung des optimalen Zell-Zell-Verhältnisses aus ADSC und HUVEC im Hinblick auf Proliferation, Apoptose, osteogene Differenzierung und Angiogenese. Zu diesem Zweck wurden ADSC mit HUVEC im Verhältnis 75%:25%, 50%:50% und 25%:75% kokultiviert und mit den jeweiligen Monokulturen verglichen. Die ADSC wurden aus Fettgewebe isoliert, das im Rahmen von autologen Brustrekonstruktionen verwendet wurde. Nach einer 14-tägigen osteogenen Prädifferenzierung wurden die ADSC für die eigentlichen Kokulturversuche mit den HUVEC verwendet. Mit Hilfe eines WST-8 Assays konnte gezeigt werden, dass die Proliferationsrate in den Kokulturen im Vergleich zu den Monokulturen über einen Zeitraum von 14 Tagen zunahm. Nach 7 Tagen zeigte sich bereits eine erhöhte Proliferationsrate in den Kokulturen, welche 75% oder 25% ADSC enthielten. Nach insgesamt 14 Tagen war die Proliferationsrate in allen Kokulturen, verglichen mit den Monokulturen, statistisch signifikant höher. Mittels negativer Immunoselektion wurden die kokultivierten ADSC und HUVEC separiert, um Zelltyp-spezifische Effekte der Kokultivierung im Hinblick auf die Apoptose und Genexpression osteogener Differenzierungsmarker, sowie angiogener Signalmoleküle untersuchen zu können. Mit Hilfe eines ELISAs wurde die Apoptose in den Zellkulturen bestimmt. Hierbei konnte gezeigt werden, dass die ADSC einen anti-apoptotischen Effekt auf die HUVEC haben. Im Gegensatz dazu, konnte in den kokultivierten ADSC mit Zunahme des HUVEC-Anteils eine erhöhte Apoptose nachgewiesen werden. Um die intrazelluläre Regulation der Apoptose in den Kokulturen untersuchen zu können, wurde der Phosphorylierungsstatus des proapoptotischen Proteins BAD analysiert. Übereinstimmend mit einer vermehrten Phosphorylierung von BAD, und einer damit einhergehenden Inaktivierung, wurde eine geringere Apoptoserate in kokultivierten HUVEC gemessen. Hinsichtlich der osteogenen Differenzierung wurde in den Kokulturen die Mineralisierung mittels Alizarin Red Assay und die Aktivität der alkalischen Phosphatase (ALP), eines frühen osteogenen Differenzierungsmarkers, bestimmt. Die ALP Aktivität und die Matrixmineralisierung war in den Kokulturen höher. Des Weiteren wurde eine PCR Analyse durchgeführt. Hierbei konnte demonstriert werden, dass HUVEC in den ADSC, insbesondere in der 50%:50% und 25%:75% Kokultur, eine verstärkte Genexpression von ALP induzieren. Zudem wurde eine erhöhte, wenn auch statisch nicht signifikante Expression, des osteogenen Transkriptionsfaktors RUNX2 in den 50% kokultivierten ADSC detektiert. Zur Untersuchung des angiogenen Potenzials der Kokultur aus ADSC und HUVEC wurde ein Matrigel Assay durchgeführt. Dabei lässt sich das angiogene Potenzial durch die Anzahl und Länge der tubulären Zellausläufer (sog. Tubes) im Matrigel quantifizieren. Es zeigte sich, dass die undifferenzierten ADSC Tubes formten. Die bereits osteogen prädifferenzierten ADSC formten hingegen keine Tubes. In der Kokultur konnte nachgewiesen werden, dass die Anzahl und Länge der geformten Tubes proportional zu dem Anteil der HUVEC zu nimmt, hinweisend auf eine verstärkte Angiogenese. Mit Hilfe eines ELISA, der die Menge an VEGF quantifiziert, konnte festgestellt werden, dass ADSC die größte Menge an Vascular Endothelial Growth Factor (VEGF) in der Monokultur nach 3 und 7 Tagen sezernieren. In der Kokultur, die 50% HUVEC beinhaltete, konnte nach 3 Tagen eine vermehrte VEGF Sekretion gemessen werden. Die Genexpression angiogener Wachstumsfaktoren, wie beispielsweise der Endothelial Nitric Oxide Synthase (eNOS) und der Matrix Metalloproteinase 3 (MMP 3) zeigte eine Zunahme in den kokultivierten HUVEC. In den ADSC induzierten hingegen die HUVEC eine vermehrte Genexpression von eNOS, Angiopoetin 2 (Ang 2) und VEGF Rezeptor 1 (Flt). Die Genexpression von MMP3 zeigte sich in den kokultivierten ADSC herunterreguliert. Zusammenfassend konnte demonstriert werden, dass die Kokultur aus ADSC und HUVEC im Hinblick auf die positiven Effekte auf Proliferation, Zellüberleben, osteogene Differenzierung und Angiogenese einen vielversprechenden Ansatz zur Generierung bioartifizieller Knochenersatzgewebe darstellt. Dabei ist das Verhältnis von 50% ADSC zu 50% HUVEC besonders vorteilhaft. The microvascular endothelial network is essential for bone formation and regeneration. In this context, endothelial cells not only support vascularization but also influence bone physiology via cell contact-dependent mechanisms. In order to improve vascularization and osteogenesis in tissue engineering applications, several strategies have been developed. One promising approach is the coapplication of endothelial and adipose derived stem cells (ADSCs). The aim of this doctoral thesis “Bone tissue engineering using adipose-derived stem cells and endothelial cells: Effects of the cell ratio” was the investigation of the best ratio of human umbilical vein endothelial cells (HUVECs) and osteogenic differentiated ADSCs regarding proliferation, apoptosis, osteogenesis and angiogenesis. For this purpose, cocultures of ADSCs and HUVECs with ratios of 25%:75%, 50%:50% and 75%:25% were performed. ADSCs were isolated from adipose tissue of five patients undergoing autologous breast reconstruction and predifferentiated in osteogenic medium for 14 days, before direct cocultures with HUVECs where accomplished. By using a WST-8 assay, it was demonstrated, that cocultures containing ADSCs and HUVECs promote proliferation over a 14-day period of cultivation. After 7 days, the rate of proliferation appeared higher in the cocultures, containing 75% or 25% ADSCs. After 14 days, proliferation was significantly increased in all cocultures compared to the corresponding monocultures. To examine cell type-specific effects of cocultivation on apoptosis and gene expression of osteogenic differentiation markers and angiogenic signal molecules, a negative immunoselection was performed, to separate cocultured ADSCs and HUVECs. Regarding apoptosis, a cell death detection ELISA was performed. The results indicated, that ADSCs had an anti-apoptotic effect on cocultivated HUVECs. In contrast to HUVECs, apoptosis was increased in cocultivated ADSCs. To enlighten a putative mechanism, a phospho-BAD ELISA was performed. The proapoptotic protein BAD is a key mediator of the regulation of apoptosis. Phosphorylation of BAD leads to inhibition of apoptosis. Correlating with increased phosphorylation status of BAD, a lower apoptosis rate was measured in cocultured HUVECs. By using an Alizarin Red assay, it was shown that a coculture of ADSCs and HUVECs led to increased matrix mineralization. Furthermore, enhanced alkaline phosphatase (ALP) activity was examined in cocultures by using an ALP assay. Alkaline phosphatase is generally known as an early osteogenic differentiation marker. After negative immunoselection and PCR analysis, it was demonstrated that HUVEC induce enhanced expression of ALP in the ADSC, especially in 50%:50% and 25%:75% cocultures. Furthermore the 50%:50% cocultured ADSCs displayed an higher, however not statistically significant expression of the transcription factor RUNX2. Concerning angiogenesis, a Matrigel assay was performed. By quantifying the number and length of formed tubes in the Matrigel, a conclusion on the angiogenic potential of the respective cocultures can be made. Interestingly it was demonstrated that undifferentiated ADSC formed tubes, contrary to predifferentiated ADSCs. In coculture, the number of formed tubes correlated to the amount of HUVECs. By using a VEGF ELISA, the highest amount of Vascular Endothelial Growth Factor (VEGF) was detected in ADSCs monocultures after 3 and 7 days. In the 50%:50% coculture, enhanced VEGF secretion was measured after 3 days. Gene expression of angiogenic growth factors, such as endothelial nitric oxide synthase (eNOS) and matrix metalloproteinase 3 (MMP 3), was increased in cocultured HUVECs after 3 days correlating to the amount of ADSCs. Furthermore, HUVECs induced enhanced expression of eNOS, angiopoietin 2 (Ang 2), and VEGF receptor 1 (Flt) in ADSCs. Whereas the expression of MMP 3 in ADSCs was downregulated. In conclusion, it was demonstrated that a 50%:50% coculture of ADSCs and HUVECs is a promising approach to improve proliferation, cell survival, osteogenesis and angiogenesis in terms of tissue engineering.

Hintergrund und Ziele Das Ziel dieser Arbeit war es, die Auswirkungen einer verkürzten Polymerisationszeit von 3 Sekunden mit einer Hochleistungs-LED auf Komposite im Bezug auf ihre biaxia-le Biegefestigkeit und Ermüdungsbeständigkeit zu prüfen und diese mit einer Stan-dardbelichtung von 20 Sekunden mit einer Halogenlampe, sowie mit industriell polyme-risierten CAD/CAM-Kompositblöcken zu vergleichen. Methoden Im Rahmen eines biaxialen Ball-on-three-Balls Testverfahren wurden das Bulk-Fill Komposit Tetric® PowerFill (TPF, Ivoclar Vivadent, Schaan, Liechtenstein), das gängige Komposit Tetric® EvoCeram (TEC, Ivoclar Vivadent) und das für CAD/CAM-Verfahren geeignete Tetric® CAD (TC, Ivoclar Vivadent) untersucht. Polymerisiert wurden die stopfbaren Komposite einmalig mit der Bluephase® PowerCure (Ivoclar Vivadent) bei 3050 mW/cm2 und mit der Elipar® TriLight (3M ESPE AG, Seefeld, Deutschland) bei 800 mW/cm2. Die Belichtungszeiten betrugen bei der Bluephase® PowerCure 3 Sekunden, sowie in Unterbelichtung 2 Sekunden, bei der Elipar® TriLight 20 Sekunden und in Unterbelichtung 10 Sekunden. Für das Prüfverfahren wurden scheibenförmige Proben (n=30) mit einem Durchmesser von 11,5 mm und einer Dicke von 2 mm hergestellt und im Anschluss bei 37 °C in Aqua dest. für 14 Tage gelagert. Dafür wurden die stopfbaren Komposite in eine Form appliziert und mit der entsprechenden Polymerisationslampe, die in eine Halterung mit einem konstanten Abstand eingespannt war, belichtet. Die Blöcke von TC wurden durch die Präzisions-säge (Präzisionstrenner ISOMET 5000, Buehler GmbH, Düsseldorf, Deutschland) in Scheiben geschnitten, mit der Präzisions-Flachschleifmaschine (MPS 1, G&N Genau-igkeits Maschinenbau Nürnberg GmbH, Erlangen, Deutschland) auf die gleiche Dicke gebracht und mit dem Poliermotor (Phoenix beta, Buehler GmbH, Düsseldorf, Deutsch-land) poliert. Die biaxiale Biegefestigkeit wurde an der Universal-Prüfmaschine (Zwick 2.5, Zwick Roell AG, Ulm, Deutschland) bestimmt. 60 % des ermittelten Wertes wurde als 1. Niveau für das Staircase-Verfahren an der DYNAdent (DYNAdent 14801, DYNA-MESS Prüfsysteme GmbH, Aachen/Stolberg, Deutschland) zur Messung der Ermü-dungsbeständigkeit bei 5 Hz und mit 10 000 Zyklen eingestellt. Die statistische Auswer-tung erfolgte über den Kolmogorov-Smirnov-Test, einer einfaktorielle Varianzanalyse sowie den Post-hoc-Test mit einem Signifikanzniveau von α=0,05. Ergebnisse und Beobachtungen Im Ulbrichtkugelversuch konnte eine ausreichende Bestrahlungsstärke und das vom Hersteller beschriebene Wellenlängenspektrum bestätigt werden. Die höchste Biegefestigkeit erreichte TC mit 236,33 MPa, den niedrigsten Wert wies TEC mit 89,11 MPa auf, das 3 Sekunden belichtet wurde. Es zeigte sich kein signifi-kanter Unterschied zwischen den Vergleichsgruppen TPF 3 Sekunden mit LED belich-tet und einer Standardbelichtung mit 20 Sekunden dieses Bulk-Fill-Komposits, ebenso wie kein signifikanter Unterschied, wenn TPF und TEC in Standardbelichtung polyme-risiert wurden. Allerdings konnte gegenläufig dazu der niedrigste Weibullmodul für TC mit m=13,5 nachgewiesen werden und war damit deutlich niedriger als das TPF, das 3 Sekunden belichtet wurde, mit m=23. Auch für die Ermüdungsbeständigkeit konnte TC mit 150,01 MPa den höchsten Wert erzielen. Ebenso war kein signifikanter Unterschied zwischen den beiden direkten Kompositen TPF und TEC, wenn sie in der vom Herstel-ler empfohlenen Belichtung (TPF 3 Sekunden, TEC 20 Sekunden) ausgehärtet wur-den, zu finden. Die kleinste Abnahme der Biegefestigkeit zeigte TEC in Unterbelich-tung mit 32,66 %. Insgesamt befand sich die prozentuale Reduktion der Biegefestigkeit bei zyklischer Belastung zwischen 32,66 % und 38,76 %. Schlussfolgerungen und Diskussion Alle Testergebnisse der direkten Komposite überstiegen den Grenzwert der ISO-Norm 4049 für die Biegefestigkeit von 80 MPa und auch das indirekte Kompositmaterial für das CAD/CAM-Verfahren lag über der Mindestbiegefestigkeit von 100 MPa. Das Pro-duktsystem aus der LED-Lampe und dem Bulk-Fill-Komposit kann als geeignet ange-sehen werden. Ein Off-Label-Use anderer Materialien zusammen mit der LED-Lampe im 3s Cure-Modus ist nicht indiziert, da es zu einer Abnahme der biaxialen Biegefestig-keit und Ermüdungsbeständigkeit führt und im Gegensatz zu dieser Laborstudie in der Praxis zusätzlich durch Überlagerung verschiedener weiterer Einflussfaktoren der Er-folg einer Restauration beeinflusst wird. Background and objectives The aim of this work was to test the effects of a shortened polymerisation time of 3 se-conds with a high-power LED on composites in terms of their biaxial flexural strength and fatigue resistance and to compare these with a standard exposure of 20 seconds with a halogen lamp, as well as with industrially polymerised CAD/CAM composite blocks. Methods The bulk-fill composite Tetric® PowerFill (TPF, Ivoclar Vivadent, Schaan, Liechten-stein), the commonly used composite Tetric® EvoCeram (TEC, Ivoclar Vivadent) and Tetric® CAD (TC, Ivoclar Vivadent), which is suitable for CAD/CAM procedures, were tested in a biaxial ball-on-three-balls test procedure. The direct composite was polymeri-sed once with Bluephase® PowerCure (Ivoclar Vivadent) at 3050 mW/cm2 and with Elipar® TriLight (3M ESPE AG, Seefeld, Germany) at 800 mW/cm2. The exposure times were 3 seconds with the Bluephase® PowerCure and 2 seconds in underexposure, 20 seconds with the Elipar® TriLight and 10 seconds in underexposure. For the test pro-cedure, disc-shaped samples (n=30) with a diameter of 11.5 mm and a thickness of 2 mm were prepared and subsequently stored at 37 °C in distilled water for 14 days. For this purpose, the composites were applied in a mould and exposed to light with the ap-propriate polymerisation lamp clamped in a holder at a constant distance. The blocks of TC were cut into slices by the precision saw (ISOMET 5000 precision cutter, Buehler GmbH, Düsseldorf, Germany), brought to the same thickness by the precision surface grinder (MPS 1, G&N Genauigkeits Maschinenbau Nürnberg GmbH, Erlangen, Germa-ny) and polished by the polishing motor (Phoenix beta, Buehler GmbH, Düsseldorf, Germany). The biaxial flexural strength was determined on the universal testing machi-ne (Zwick 2.5, Zwick Roell AG, Ulm, Germany). 60 % of the determined value was set as the 1st level for the staircase method on the DYNAdent (DYNAdent 14801, DY-NA-MESS Prüfsysteme GmbH, Aachen/Stolberg, Germany) for measuring the fatigue re-sistance at 5 Hz and with 10 000 cycles. Statistical analysis was carried out using the Kolmogorov-Smirnov test, a single factor analysis of variance, as well as the post-hoc test with a significance level of α=0,05. Results and Observations The Ulbricht sphere test confirmed sufficient irradiance and the wavelength spectrum described by the manufacturer. The highest bending strength was achieved by TC with 236.33 MPa, the lowest value was shown by TEC with 89.11 MPa, which was exposed for 3 seconds. There was no significant difference between the comparison groups TPF exposed for 3 seconds with LED and a standard exposure of 20 seconds of this bulk-fill composite, as well as no significant difference when TPF and TEC were polymerised in standard exposure. However, in contrast, the lowest Weibull modulus for TC was found to be m=13.5, which was significantly lower than the TPF exposed for 3 seconds with m=23. TC also achieved the highest value for fatigue resistance with 150.01 MPa. Simi-larly, no significant difference was found between the two direct composites TPF and TEC when cured at the manufacturer's recommended exposure time (TPF 3 seconds, TEC 20 seconds). TEC showed the smallest decrease in flexural strength in underex-posure at 32.66%. Overall, the percentage reduction in flexural strength under cyclic loading was between 32.66% and 38.76%. Conclusion All test results of the direct composites exceeded the ISO standard 4049 limit for flexural strength of 80 MPa and the indirect composite for the CAD/CAM procedure also excee-ded the minimum flexural strength of 100 MPa. The product system consisting of the LED lamp and the bulk-fill composite can be considered suitable. Off-label use of other materials together with the LED lamp in 3s Cure mode is not indicated, as it leads to a decrease in biaxial flexural strength and fatigue resistance and, in contrast to this labo-ratory study, the success of a restoration is additionally influenced in practice by the superimposition of various other influencing factors.

This study was performed to study the effects of different pressure levels in topical negative pressure application on healthy tissue and analysing changes in perfusion parameters using multimodal imaging techniques. In addition to recent imaging devices a new and potential helpful device detecting venous flow patterns was evaluated and compared to hitherto known perfusion measuring devices. 2.1 Background and Objective In modern wound treatment, negative pressure wound therapy (NPWT) has been established as a standard option. It is used for different indications such as wound bed preparation, treatment of chronic or infected wounds or as a temporary wound closure after operations. Although it is used widely and often, debates about the exact mechanisms of topical negative pressure application (TNPA) are still ongoing. In particular, the influence of topical negative pressure on microperfusion depending on the different strengths of pressure applied to the tissue needs further clarification. Since measurement of microperfusion in deep wounds under a TNPA is practically impossible for technical reasons, skin microcirculation could serve as an auxiliary parameter to further explore the effects of TNPA. This study therefore aims to understand the changes in microcirculation and perfusion induced by different levels of negative pressure, using new non-invasive imaging devices. 2.2 Methods To exclude patient specific potential pathologic skin and perfusion conditions 27 healthy volunteers with no history of relevant diseases effecting skin circulation participated in the study, randomly assigned into 3 groups. 3 participants had to be removed from data sample due to technical problems with one of the imaging devices and hence incomplete data acquisition. Results were calculated using data from the remaining 24 volunteers. Right anterolateral thigh region of each participant was selected as control region for the TNPA and -125mmHg negative pressure was applied as the standard use in clinical practice. Left anterolateral thigh region was used as the 12 study region and 3 groups were established for 3 different pressure levels. Group 1 had no negative pressure but the sponge and dressing of the TNPA was applied to see the effect of the air-tight dressing on the region. Group 2 and Group 3 had - 25mmHg and -175mmHg negative pressure application, respectively, to understand the deviation of the effect compared to the common clinical standard of -125mmHg negative pressure. A 30 minutes period of pressure application was followed by 30 minutes of resting. Tissue oxygen saturation was measured using a near-infrared spectroscopy (NIS), tissue temperature was measured using a thermographic camera and changes in venous structure was observed using a vein illuminator. Basal levels with each device were recorded and changing levels after applications were recorded at predetermined time points for each device. 2.3 Results and Observations An increase in oxygen saturation was observed in the control group, group 2 and group 3 over the 30 minutes following the removal of TNPA. Oxygen saturation and skin temperature increased by 8.07% and 1.67°C for the control group, 4.00% and 1.65°C for Group 2, and 8.45% and 1.68°C for Group 3. Group 1 with only foam and dressing also showed increase in oxygen saturation on average but it was not statistically meaningful and the increases and decreases in saturation were sporadic. Other groups had a similar pattern of gradually decreasing oxygen saturation during the observation period but the values did not fall under the base level readings, showing a positive increase with TNPA in oxygen saturation overall. Increase in skin temperature was observed for all groups after the removal of TNPA, 1.67°C for the control group, 1.65°C for Group 2, 1.68°C for Group 3. Group 1 with no pressure has shown 2.7°C increase. During the observation period, the observed region cooled gradually and at different rates for all groups. The observed area of group 1 without TNPA cooled the fastest, followed by group 2 with -25 mmHg, the control group with - 125 mmHg - and in group 3 with -175 mmHg the cooling of the region was the slowest. Throughout the study period, no significant measurable changes in vein structure and blood flow were observed in the subcutaneous veins studied in any of the groups. 2.4 Conclusions In conclusion, this study shows that TNPA lead to an increased oxygen saturation in the underlying tissue and increased the tissue temperature but did not cause changes in the venous structure and flow. Groups with TNPA were able to keep the increase in temperature for a longer time compared to group 1 with only sponge and dressing without TNP. Group 1 had no meaningful increase in oxygen saturation compared to other study groups which showed meaningful increase throughout the observation period and group 3 had the most increase with the highest negative pressure, showing a positive correlation between oxygen saturation and TNPA level. This study also showed that NIS is an effective and non-invasive tool to determine changes in microcirculatory skin blood flow. Thermographic images are helpful to see the increase in tissue temperature resulted by increased blood flow. Higher negative pressure did not have a negative effect on blood flow in the design of the experiment performed here. The determination of a possible maximum negative pressure causing a decrease in oxygen saturation and inhibiting blood flow could be established in a further study in the future. 1. Zusammenfassung Diese Studie wurde durchgeführt, um die Auswirkungen verschiedener Druckniveaus bei der topischen Anwendung von Unterdruck auf gesundes Gewebe zu untersuchen und die Veränderungen der Perfusionsparameter mithilfe multimodaler Bildgebungsverfahren zu analysieren. Zusätzlich zu den bisher bereist bekannten berührungslosen bildgebenden Verfahren wurde ein neues und potentiell wertvolles Verfahren zur Erkennung venöser Durchblutungsmuster evaluiert und der mögliche Erkenntnisgewinn mit bisher bekannten Perfusionsmessgeräten verglichen. 1.1 Hintergrund und Zielsetzung In der modernen Wundversorgung hat sich die topische Unterdruckbehandlung von vollschichtigen Wunden (Negative Pressure Wound Therapy / NPWT) als ein Standardverfahren etabliert. Sie wird bei verschiedenen Indikationen wie der Präparation des Wundbetts, der Behandlung chronischer oder infizierter Wunden oder als temporärer Wundverschluss nach Operationen eingesetzt. Obwohl die NPWT weit verbreitet ist und häufig angewandt wird, ist die Erforschung der genauen Mechanismen der topischen Unterdruckanwendung (Topical Negative Pressure Application / TNPA) noch nicht abgeschlossen. Insbesondere der Einfluss des topischen Unterdrucks auf die Mikroperfusion in Abhängigkeit von den unterschiedlichen Stärken des ausgeübten Drucks auf das Gewebe bedarf noch weiterer Klärung. Nachdem die Messung der Mikroperfusion in tiefen Wunden unter einer TNPA aus technischen Gründen praktisch kaum möglich ist, könnte die Mikrozirkulation der Haut als Hilfsgröße dienen, um die Auswirkungen von TNPA weiter zu erforschen. Diese Studie zielt daher darauf ab, die Veränderungen der Mikrozirkulation und der Perfusion zu verstehen, die durch unterschiedliche Unterdruckwerte hervorgerufen werden, wobei neue, nicht-invasive Bildgebungsgeräte zum Einsatz kommen. 1.2 Methodik Um patientenspezifische, potenziell pathologische Haut- und Perfusionsbedingungen 8 auszuschließen, nahmen 27 gesunde Probanden ohne relevante, die Hautdurchblutung beeinflussende Erkrankungen in der Krankengeschichte an der Studie teil, die nach dem Zufallsprinzip in 3 Gruppen eingeteilt wurden. 3 Teilnehmer mussten aufgrund technischer Probleme mit einem der Bildgebungsgeräte und der damit verbundenen unvollständigen Datenerfassung aus der Stichprobe ausgeschlossen werden. Die Ergebnisse wurden anhand der Daten der verbleibenden 24 Probanden berechnet. Die rechte anterolaterale Oberschenkelregion jedes Teilnehmers wurde als Kontrollregion für die TNPA ausgewählt und es wurde ein Unterdruck von -125 mmHg angelegt, wie er in der klinischen Praxis üblich ist. Die linke anterolaterale Oberschenkelregion wurde als Studienregion verwendet und es wurden 3 Gruppen für 3 verschiedene Druckstufen gebildet. In Gruppe 1 wurde kein Unterdruck, sondern lediglich der Schwamm und der Verband der TNPA angelegt, um die Wirkung des luftdichten Verbandes auf die Perfusion der Region zu beobachten. In Gruppe 2 und Gruppe 3 wurde ein Unterdruck von -25 mmHg respektive -175 mmHg angewendet, um die Abweichung der Wirkung im Vergleich zum üblichen klinischen Standard von -125 mmHg Unterdruck zu untersuchen. Auf eine 30-minütige Druckanwendung folgte eine 30-minütige Ruhephase. Die Sauerstoffsättigung des Gewebes wurde mit einer Nahinfrarotspektroskopie (NIS) gemessen, die Gewebetemperatur mit einer Thermografiekamera und die Veränderungen der Venenstruktur mit einem Venenbeleuchtungsgerät beobachtet. Die Ausgangswerte mit jedem Gerät wurden aufgezeichnet und die Veränderungen der Werte nach den Anwendungen wurden zu vorher festgelegten Zeitpunkten für jedes Gerät aufgezeichnet. 1.3 Resultate und Beobachtungen In der Kontrollgruppe, der Gruppe 2 und der Gruppe 3 wurde ein Anstieg der Sauerstoffsättigung während der 30 Minuten nach der Entfernung der TNPA beobachtet. Die Sauerstoffsättigung und die Hauttemperatur stiegen in der Kontrollgruppe um 8,07% und 1,67 °C, in der Gruppe 2 um 4,00% und 1,65 °C und in der Gruppe 3 um 8,45 % und 1,68 °C. In Gruppe 1, die nur mit Schaumstoff und Verband behandelt wurde, stieg die Sauerstoffsättigung im Durchschnitt ebenfalls an, war jedoch statistisch nicht signifikant und die Anstiege und Rückgänge der Sättigung waren unregelmäßig. Die anderen Gruppen wiesen ein ähnliches Muster einer allmählich abnehmenden Sauerstoffsättigung während des Beobachtungszeitraums auf, aber die Werte fielen nicht unter die Basiswerte, was insgesamt einen positiven Anstieg der Sauerstoffsättigung mit TNPA zeigte. Bei allen Gruppen wurde nach der Entfernung von TNPA ein Anstieg der Hauttemperatur beobachtet: 1,67°C bei der Kontrollgruppe, 1,65°C bei Gruppe 2 und 1,68°C bei Gruppe 3. Gruppe 1, die nicht mit Druck behandelt wurde, wies einen Anstieg von 2,7°C auf. Während des Beobachtungszeitraums kühlte die beobachtete Region bei allen Gruppen allmählich und mit unterschiedlicher Geschwindigkeit ab. Die Untersuchungsregion der Gruppe 1 ohne TNPA kühlte am schnellsten ab, gefolgt von Gruppe 2 mit -25 mmHg, der Kontrollgruppe mit -125 mmHg - und bei Gruppe 3 mit -175 mmHg verlief die Abkühlung der Region am langsamsten. Während der gesamten Studiendauer wurden bei keiner der Gruppen signifikante messbare Veränderungen der Venenstruktur und des Blutflusses in den untersuchten subkutanen Venen beobachtet. 1.4 Fazit Zusammenfassend zeigte diese Studie, dass TNPA zu einer erhöhten Sauerstoffsättigung im darunter liegenden Gewebe führte und die Gewebetemperatur erhöhte, aber keine Veränderungen in der Venenstruktur und im Venenfluss bewirkte. Die Gruppen mit TNPA waren in der Lage, den Temperaturanstieg über einen längeren Zeitraum aufrechtzuerhalten, verglichen mit Gruppe 1, die nur Schwamm und Verband ohne TNP erhielt. In Gruppe 1 kam es zu keinem nennenswerten Anstieg der Sauerstoffsättigung im Vergleich zu den anderen Studiengruppen, die während des gesamten Beobachtungszeitraums einen signifikanten Anstieg aufwiesen, und Gruppe 3 hatte den stärksten Anstieg mit dem höchsten Unterdruck, was eine positive Korrelation zwischen Sauerstoffsättigung und TNPA-Niveau zeigt. Diese Studie zeigte auch, dass die NIS ein wirksames und nicht-invasives Instrument zur Bestimmung von Veränderungen der Mikrozirkulation der Haut ist. Thermografische Bilder sind hilfreich, um den Anstieg der Gewebetemperatur infolge des erhöhten Blutflusses zu erkennen. Höherer Unterdruck wirkte sich im hier durchgeführten Versuchsdesign nicht negativ auf den Blutfluss aus. Die Bestimmung eines möglichen maximalen Unterdruckes, der eine Abnahme der Sauerstoffsättigung verursacht und den Blutfluss hemmt, könnte zukünftig in einer weiteren Studie etabliert werden.

Eine prämature Kraniosynostose bezeichnet eine vorzeitige Verknöcherung einer oder mehrerer Schädelnähte. Ihre Entstehung ist von multiplen Faktoren abhängig. So scheinen genetische Faktoren, das Rauchen der Mutter oder die Einnahme bestimmter Medikamente in der Schwangerschaft, Schilddrüsen- und Stoffwechselerkrankungen einen Einfluss zu haben. Die Koronarnahtsynostose stellt mit einer Inzidenz von 20 % die zweithäufigste Form der prämaturen Synostosen dar. Bei dem vorzeitigen unilateralen Nahtverschluss kommt es zur Entwicklung eines anterioren Plagiozephalus. Bei einer beidseitigen Koronarnahtsynostose entsteht ein brachy-turrizephaler Schädel. Die frühzeitige Diagnose ist wichtig, damit die betroffenen Kinder frühestmöglich in ein optimales Betreuungs- und Therapiekonzept eingebunden werden können. Bei Einzelnahtsynostosen sind meist bereits die untersuchten klinischen Parameter zur Diagnosestellung ausreichend und sollten um eine Sonographie und Röntgenaufnahmen in zwei Ebenen erweitert werden. Eine Indikation zur operativen Intervention stellt der Nachweis einer pathologischen intrakraniellen Drucksteigerung dar. Das Frontoorbitale Advancement ist die Operationstechnik der Wahl bei der Koronarnahtsynostose. Ziel der vorliegenden Dissertationsarbeit war die Weiterentwicklung bestehender kephalometrischer und kraniometrischer Messverfahren nach Slomic et al.. Dabei sollten der operative Therapieerfolg und der weitere Verlauf hinsichtlich einer Rezidivgefahr bewertet werden. In der vorliegenden Arbeit wurden Röntgenbilder des Carniofacialen Centrums Würzburg kraniometrisch ausgewertet. Das Patient*innenkollektiv wurde in zwei Gruppen untergliedert, und zwar Patient*innen mit einseitiger, nonsyndromaler Koronarnahtsynostose und Patient*innen mit beidseitiger, syndromaler Koronarnahtsynostose. Zur statistischen Auswertung erfolgte in beiden Patient*innengruppen die Untersuchung der Röntgenbilder zu vier festgelegten Zeitpunkten (00, 01, 02, 03). Die statistische Auswertung erfolgte mit dem Programm SPSS. Untersucht wurden der Gruppeneffekt und der Zeiteffekt hinsichtlich der 13 Strecken (LI, BRSt, BRPa, NO, PIS; SN, PIN, HI, NSt, SBR, PIBR, WI, AS) und fünf Winkel (ANS, SNBR, PIBRPa, BRNST, PISN). Da es in der Literatur eine unzureichende Erfassung von Strecken und Winkeln gibt, die die Veränderungen des Schädelwachstums erfassen, wurden die Strecken BRSt, BRPa, NSt, PIBR und AS sowie die fünf oben genannten Winkel neu definiert und entwickelt. Für die Röntgenzeitpunkte 00 und 01 zeigten sich für die Strecken und Winkel LI, BRSt, HI, NSt, SBR, PIBR, WI, PIBRPa und BRNST signifikante Unterschiede. Dies kann als OP-Erfolg gewertet werden. Der Kopf wird intraoperativ flacher und schmäler. Im weiteren Verlauf zeigte sich bei den Strecken BRSt, HI, PIBR und WI sowie bei den Winkeln PIBRPa und BRNST ein signifikanter Unterschied. Der Kopf wächst rezidivierend turrizephal. Im weiteren Untersuchungszeitraum wurde lediglich für die Strecken BRPa und AS ein signifikanter Unterschied ausgemacht. Zum einen Anzeichen eines im Wachstumsverlauf einsetzenden Rezidivs. Der Kopf wird wieder turrizephaler (BRPa). Zum anderen ist es Ausdruck einer beginnenden Mittelgesichtshypoplasie (AS). Weiterhin konnte über die Strecken SBR und PIBR gezeigt werden, dass Patient*innen mit beidseitiger Synostose eine turrizephalere Kopfform als die Vergleichsgruppe mit einseitiger Synostose aufweisen. Auffällig war außerdem das Ergebnis der beiden Winkel ANS und SNBR. Sie belegen, dass Patient*innen mit beidseitiger Synostose und Syndrom eine Mittelgesichtshypoplasie aufweisen. Als Fazit lässt sich sagen, dass die Strecken LI, BRSt, BRPa, HI, SBR, WI und AS sowie die Winkel SNBR, PIBRPa und ANS für weitere Untersuchungen geeignet scheinen., Premature craniosynostosis refers to premature ossification of one or more cranial sutures. Its development depends on multiple factors. Genetic factors, maternal smoking or the use of certain medications during pregnancy, thyroid and metabolic diseases seem to have an influence. Coronary suture synostosis is the second most common form of premature synostosis, with an incidence of 20%. Premature unilateral suture closure results in the development of anterior plagiocephaly. Bilateral coronal suture synostosis results in a brachy-turricephalic skull. Early diagnosis is important so that affected children can be integrated into an optimal care and treatment concept as early as possible. In the case of single-suture synostosis, the clinical parameters examined are usually sufficient to make a diagnosis and should be supplemented by sonography and x-rays in two planes. An indication for surgical intervention is the detection of a pathological intracranial pressure increase. Frontoorbital advancement is the surgical technique of choice for coronary suture synostosis. The aim of the present dissertation was to further develop existing cephalometric and craniometric measurement procedures according to Slomic et al.. The aim was to evaluate the success of the surgical therapy and the further course with regard to the risk of recurrence. In the present study, X-ray images from the Carniofacial Centre Würzburg were evaluated craniometrically. The patient collective was divided into two groups, namely patients with unilateral, nonsyndromal coronary suture synostosis and patients with bilateral, syndromal coronary suture synostosis. For statistical evaluation, the X-ray images of both groups of patients were examined at four fixed points in time (00, 01, 02, 03). The statistical analysis was performed with the SPSS programme. The group effect and the time effect were investigated with regard to the 13 distances (LI, BRSt, BRPa, NO, PIS; SN, PIN, HI, NSt, SBR, PIBR, WI, AS) and five angles (ANS, SNBR, PIBRPa, BRNST, PISN). As there is an insufficient coverage of distances and angles in the literature that capture the changes in cranial growth, the BRSt, BRPa, NSt, PIBR and AS distances and the five angles mentioned above were redefined and developed. For radiographic time points 00 and 01, there were significant differences for the LI, BRSt, HI, NSt, SBR, PIBR, WI, PIBRPa and BRNST distances and angles. This can be interpreted as a surgical success. The head becomes flatter and narrower intraoperatively. In the further course, there was a significant difference in the distances BRSt, HI, PIBR and WI as well as in the angles PIBRPa and BRNST. The head grows recurrently turricephalic. In the further investigation period, a significant difference was only found for the distances BRPa and AS. On the one hand, there are signs of recurrence during the course of growth. The head becomes more turricephalic again (BRPa). On the other hand, it is an expression of an incipient midface hypoplasia (AS). Furthermore, the SBR and PIBR sections showed that patients with bilateral synostosis have a more turricephalic head shape than the comparison group with unilateral synostosis. The results of the ANS and SNBR angles were also striking. They show that patients with bilateral synostosis and syndrome have midface hypoplasia. In conclusion, the LI, BRSt, BRPa, HI, SBR, WI and AS routes and the SNBR, PIBRPa and ANS angles seem suitable for further investigation.

Hintergrund: Bei vielen Patienten mit Marfan- oder Loeys-Dietz-Syndrom wird die Übergewichtigkeit klinisch oft übersehen. Die moderne Bioelektrische Impedanzanalyse scheint den anthropometrischen Routineparametern für die Erfassung einer zuverlässigen nicht-invasiven Bewertung der Körperzusammensetzung von Patienten überlegen zu sein. Ziel der Studie war es, die Körperzusammensetzung von Patienten mit Marfan-/Loeys-Dietz-Syndrom mittels Bioelektrische Impedanz Analyse zu beurteilen, um okkulte Adipositas zu erkennen, die ein Risikomarker für aortale und/oder vaskuläre Komplikationen sein kann. Methoden: Zwischen Juni 2020 und Februar 2022 wurden in dieser explorativen Querschnittsstudie 50 Patienten (66 % weiblich; Durchschnittsalter: 37,7 ± 11,7 [Spanne: 17-64] Jahre) mit einer molekulargenetisch (n = 45; 90 %) oder klinisch (n = 5; 10 %) nachgewiesenen Diagnose von Marfan- oder Loeys-Dietz-Syndrom aufgenommen. Alle Bioelektrische Impedanz Analyse-Messungen wurden mit dem Multifrequenz-Impedanz-Analysator Nutriguard-MS (Data Input, Poecking, Deutschland) durchgeführt. Ergebnisse: Das Marfan-/Loeys-Dietz-Syndrom-Kollektiv unterschied sich signifikant von einer alters-, geschlechts- und Body Mass Index-angepassten Kontrollgruppe in Bezug auf Körperfett, prozentuale Zellzahl, Körperzellmasse, Extrazellmasse/Körperzellmasse-Index und Phasenwinkel (alle p < 0,05). Der mittlere mit der Bioelektrische Impedanz Analyse gemessene Körperfettanteil betrug 31,7 ± 8,7 % [Bereich: 9,5-53,5 %], während der mittlere berechnete Body Mass Index der eingeschlossenen Patienten 23,0 ± 4,8 kg/m 2 [Bereich: 15,2-41,9 kg/m2] betrug. Unter Verwendung der Adipositas-Cut-off-Werte für den Körperfettanteil von 25 % bei Männern und 35 % bei Frauen stuft die Bioelektrische Impedanz Analyse daher immerhin 28 Patienten (56,0 %) als fettleibig ein. Im Gegensatz dazu waren nur 12 Patienten (24,0 %) präadipös bzw. 3 (6,0 %) adipös nach Body Mass Index. Der signifikante Unterschied (p < 0,001) hatte eine Übereinstimmung von 42,7%. Insgesamt hatten 15 Patienten (13 Marfan Syndrom; 2 Loeys-Dietz Syndrom) eine vorherige Aortenoperation (n = 14) und/oder eine interventionelle Behandlung (n = 2) auf Grund von Aortenkomplikationen (Aneurysma, Aortendissektion). 11 von den 15 (73,3 %) operierten Patienten wurden durch den Bioelektrische Impedanz Analyse als fettleibig eingestuft. Schlussfolgerung: Es ist weitgehend unbekannt, dass viele Patienten mit Marfan- oder Loeys-Dietz-Syndrom fettleibig sind. Die Fettleibigkeit ist mit einer beeinträchtigten vaskulären Endothelfunktion und einem erhöhten Risiko für kardiovaskuläre Komplikationen verbunden. Die Bioelektrische Impedanz Analyse ermöglicht bei Patienten mit Marfan-/Loeys-Dietz-Syndrom eine zuverlässige Bewertung der Körperzusammensetzung über die normalen anthropometrischen Parameter, wie den Body Mass Index hinaus. In Zukunft könnten Bioelektrische Impedanz Analyse-Daten neben den Aortendurchmessern möglicherweise von besonderer Bedeutung für die Beurteilung des Risikos für Gefäßkomplikationen von Marfan-/Loeys-Dietz-Syndrom-Patienten sein. Background: Obesity is often clinically overlooked in many patients with Marfan syndrome or Loeys-Dietz syndrome. Modern bioelectrical impedance analysis appears to be superior to routine anthropometric parameters for recording reliable non-invasive assessment of body composition in patients with connective tissue disorders, such as Marfan syndrome and Loeys-Dietz syndrome. The aim of this study was to assess the body composition of patients with Marfan syndrome/Loeys-Dietz syndrome using Bioelectrical Impedance Analysis to detect occult obesity, which may be a risk marker for aortic and/or vascular complications. Methods: Between June 2020 and February 2022, 50 patients (66% female; mean age: 37.7 ± 11.7 [range: 17-64 years]) with a molecularly (n = 45; 90%) or clinically (n = 5; 10%) proven diagnosis of Marfan syndrome or Loeys-Dietz syndrome were enrolled in this exploratory cross-sectional study. All Bioelectrical Impedance Analysis measurements were performed with the Nutriguard-MS multifrequency impedance analyzer (Data Input, Poecking, Germany). Results: The Marfan syndrome/Loeys-Dietz syndrome collective differed significantly from an age-, sex-, and Body Mass Index-matched control group in terms of body fat, percent cell count, body cell mass, extracellular mass/body cell mass index, and phase angle (all p < 0.05). The mean body fat percentage measured by Bioelectrical Impedance Analysis was 31.7 ± 8.7% [range: 9.5-53.5%], while the mean calculated Body Mass Index of the included patients was 23.0 ± 4.8 kg/m2 [range: 15.2-41.9 kg/m2]. Therefore, using the obesity cut-off values for body fat percentage of 25% in men and 35% in women, the Bioelectrical Impedance Analysis still classified 28 patients (56.0%) as obese. In contrast, only 12 patients (24.0%) were pre-adipose and 3 (6.0%) were obese according to Body Mass Index. The significant difference (p < 0.001) had an agreement of 42.7%. A total of 15 patients (13 with Marfan syndrome; 2 with Loeys-Dietz syndrome) had prior aortic surgery (n = 14) and/or interventional treatment (n = 2) due to aortic complications (aneurysm, aortic dissection). 11 of the 15 (73.3%) operated patients were classified as obese by Bioelectrical Impedance Analysis. Conclusions: It is largely unknown that many patients with Marfan syndrome or Loeys-Dietz syndrome are obese. Obesity is associated with impaired vascular endothelial function and increased risk of cardiovascular complications. Bioelectrical Impedance Analysis provides a reliable assessment of body composition in patients with Marfan syndrome/Loeys-Dietz syndrome beyond normal anthropometric parameters, such as Body Mass Index. In the future, Bioelectrical Impedance Analysis data, in addition to aortic diameters, may be of particular importance in assessing the risk for vascular complications in Marfan syndrome/Loeys-Dietz syndrome patients.

Background and Objective: The application of prefabricated tissue-engineered flaps could be a promising alternative for treating large tissue defects. One of the critical aspects of building successful tissue engineering-based therapeutic techniques is ensuring sufficient vascular growth in the generated construct for a decent blood supply. The arteriovenous (AV) loop model has emerged as a key tissue engineering technique that can potentially address the major challenge of providing appropriate circulatory support to a large tissue construct. However, the underlying mechanisms and factors influencing vascularization and de novo tissue generation in the AV loop remain poorly understood. These underlying mechanisms and contributing factors can be thoroughly investigated using intravital microscopy (IVM). Therefore, the first part of this study focuses on establishing a novel in vivo IVM chamber for the AV loop model. Moreover, an application of vascular grafts having a strong angiogenic potential can aid in tissue development and neovascularization in the AV loop model. Therefore, the angiogenic capacity of vascular grafts from various places in the rat was compared in the second portion of this investigation. Methods: We applied three different chamber designs: A, B, and C to establish the rat AV loop IVM chamber model. Design A is an oval-shaped, two-part chamber. To prevent any manipulation by the animal, we applied a handmade cone around the animal's neck and used a protective cover over the lid. Moreover, we tried different skin incisions and installation procedures to minimize skin damage. In addition, several minor modifications were made to design A to enhance the chamber stability, loop functionality, and to reduce blood accumulation in the chamber. Design B is a single-piece chamber. It was installed using the purse-string method. In addition, we created free space near the entrance of the chamber to enhance the loop functionality. Moreover, repeated IVM was performed. The chamber design C is also a single-piece chamber developed for subcutaneous installation. IVM was performed on days 0,7,14 and 21. Microvascular growth was quantified using image editing software and FIJI. We used two different types of matrices: Gelatin methacryloyl (GelMA) and fibrin. Animals were perfused with India ink, and the chamber was explanted at the end of the study. The weight of the generated tissue constructs was measured. Moreover, the generated construct was further examined using histological stainings (H & E, CD68, MMP3, and Masson's trichrome staining). Further information such as the number of microvessels, connective tissue area, total area, and distance of microvessels from the main vessels was collected from the H & E stained slides. The angiogenic potential of blood vessels from various locations was assessed using an ex vivo 3D setup. Each blood vessel was cut into 1 mm segment heights and immersed between two layers of fibrin matrix in 96 well-plates. The microvascular sprouting was examined every second day for two weeks. Vascular rings were stained with live-cell tracker dye to confirm the proliferation of microvessels. Microvascular growth was quantified using image editing software and FIJI. Results: Initially, we did not get the anticipated results with chamber design A. Unfortunately, animals could not freely move, feed, and drink because of the neck cone. Therefore, we stopped using the neck cone. Also, the medication schedule was revised. Moreover, long incisions resulted in skin necrosis and damaged loop because of animal manipulation. Therefore, we used a smaller skin incision. The accumulation of blood in the chamber was minimized by making free space near the entrance and increasing the vessel connecting sutures. The skin surrounding the chamber was becoming loose and escaped the suture thread. We tried thicker suture thread, more numbers of screws, and stitches, interrupted as well as continuous suturing methods, but it could not improve the chamber grip on the skin. Moreover, the loop functionality was limited to 3-4 days. In design B, the purse-string method significantly improved chamber stability and restricted animal manipulation. The chamber was stable on the leg for more than two weeks. The loop functionality was achieved for two weeks as well. The loop functionality was further improved with chamber design C. Initial microvascular sprouting from the AV loop was observed in GelMA on day 14. On day 21, the vessel network showed clear signs of maturation and reorganization. However, microvascular sprouting was not observed in fibrin. Histological examination exhibited higher numbers of sprouted microvessels in GelMA than in fibrin. The AV loop in all fibrin constructions was thrombosed, perhaps owing to matrix degradation. However, the fibrin AV loop construct exhibited higher connective tissue and lower construct weight. The maximum microvascular sprouting from the ex vivo vascular grafts occurred between days 10 and 12. The vascular grafts obtained from the upper extremity showed greater outgrowth compared to the lower extremity grafts. Moreover, the arterial grafts exhibited stronger microvascular outgrowth as compared with the venous grafts. Conclusion: We successfully established two distinct AV loop IVM chamber methods in the first in vivo part of our study. The first chamber (design B) enables direct in vivo imaging without additional surgical intervention following AV loop surgery. The design C chamber requires surgical exposure of the subcutaneously implanted AV loop chamber. Chamber design B allows IVM for 14 days, and chamber design C provides for 21 days. GelMA is a suitable matrix for IVM investigations in the AV loop model due to its unique properties, such as low degradation rate and high transparency. This IVM model can be used as a novel tool in tissue engineering for real-time analysis, and hence, a faster understanding of crucial regenerative mechanisms. Moreover, our ex vivo study indicates that arterial vessels from the upper extremity possess a stronger angiogenic capacity than other location-specific blood vessels. Therefore, applying the upper extremity arterial graft can greatly enhance tissue neovascularization and tissue development. Hintergrund und Ziel: Die Verwendung von mittels Tissue-Engineering generierten künstlichen Gewebskonstrukten könnte eine vielversprechende Alternative für die Behandlung großer Gewebedefekte in der rekonstruktiven Chirurgie darstellen. Einer der bedeutensten Aspekte bei der Entwicklung erfolgreicher therapeutischer Techniken auf der Grundlage des Tissue-Engineering ist die Gewährleistung einer adäquaten Gefäßversorgung für ein Gewebekonstrukt. Das Modell des arteriovenösen (AV) loop hat sich als eine wichtige Tissue-Engineering-Technik herauskristallisiert, mit der die große Herausforderung einer angemessenen Durchblutung eines großen Gewebekonstrukts bewältigt werden kann. Die zugrundeliegenden Mechanismen und Faktoren, welche die Vaskularisierung und De novo-Gewebebildung im AV loop beeinflussen, sind jedoch noch weitgehend unklar. Diese zugrundeliegenden Mechanismen und Einflussfaktoren können mithilfe der Intravitalmikroskopie (IVM) im Detail untersucht werden. Daher konzentriert sich der erste Teil dieser Studie auf die Etablierung einer neuartigen in vivo IVM-Kammer für das AV loop Modell. Darüber hinaus kann die Verwendung von Gefäßtransplantaten mit einem starken angiogenen Potenzial die Gewebeentwicklung und Neovaskularisierung im AV loop Modell unterstützen. Daher wurde im zweiten Teil dieser Untersuchung die angiogene Kapazität von Gefäßtransplantaten aus verschiedenen Entnahmelokalisationen an der Ratte verglichen. Methoden: Es kamen drei verschiedene Kammerdesigns zum Einsatz, um das AV loop IVM-Kammermodell der Ratte zu etablieren. Design A ist eine ovalförmige, zweiteilige Kammer. Um jegliche Manipulation an der Kammmer durch das Tier zu verhindern, kam eine Automutilationsprophylaxe in Form einer Halskrause sowie eine zusätzliche Schutzabdeckung über der Kammer zum Einsatz. Außerdem wurden verschiedene Hautschnittführungen und Implantationsverfahren getestet, um die Haut so wenig wie möglich zu verletzen. Darüber hinaus wurden mehrere kleinere Änderungen an Design A vorgenommen, um die Stabilität der Kammer und die Funktionalität des AV loop zu verbessern und Einblutungen und Blutansammlung in der Kammer zu verringern. Design B ist eine einteilige Kammer. Sie wurde mittels einer Tabaksbeutelnaht implantiert. Darüber hinaus wurde eine Ausspaarung im Eingangsbereich der Kammer realisiert, um die Funktionalität des AV loop zu verbessern. Außerdem wurde eine wiederholte IVM durchgeführt. Das Kammerdesign C ist ebenfalls eine einteilige Kammer, die für die subkutane Implantation entwickelt wurde. Die IVM wurde an den Tagen 0, 7, 14 und 21 durchgeführt. Das mikrovaskuläre Wachstum wurde mit einer Bildbearbeitungssoftware und FIJI quantifiziert. Es kamen zwei verschiedene Arten von Matrizes zum Einsatz: Gelatine-Methacryloyl (GelMA) und Fibrin. Die Tiere wurden nach Abschluss der in vivo versuche mit india ink perfundiert und die Kammer wurde am Ende der Studie explantiert. Das Gewicht der generierten Gewebekonstrukte wurde gemessen. Darüber hinaus wurde das Gewebskonstrukt mittels histologischer Färbung (H & E, CD68, MMP3 und Masson's Trichromfärbung) weiter untersucht. Weitere Parameter wie die Anzahl der Mikrogefäße, die Bindegewebsfläche, die Gesamtfläche und der Abstand der Mikrogefäße zu den Hauptgefäßen wurden anhand der H & E-Färbung ermittelt. Das angiogene Potenzial von Blutgefäßen von verschiedenen Donorlokalisationen wurde mit einem ex-vivo-3D-Setup untersucht. Jedes Blutgefäß wurde in 1 mm hohe Segmente geschnitten und zwischen zwei Schichten Fibrinmatrix in 96 Well-Platten eingebracht. Die mikrovaskuläre Sprossung wurde zwei Wochen lang jeden zweiten Tag untersucht. Die Gefäßrsegmente wurden mit Live-Cell-Tracker-Farbstoff angefärbt, um die Proliferation der Mikrogefäße zu bestätigen. Das mikrovaskuläre Wachstum wurde mit Hilfe einer Bildbearbeitungssoftware und FIJI quantifiziert. Ergebnisse: Anfänglich erzielten wir mit Kammerdesign A nicht die erwarteten Ergebnisse. Leider konnten sich die Tiere wegen der Halskrause nicht frei bewegen, fressen und trinken. Deshalb haben wir die Halskrause nicht mehr verwendet. Auch der Medikamentenplan wurde überarbeitet. Außerdem führten die langen Hautschnitte am Implantationssitus zu Hautnekrosen und der AV loop wurde durch die Manipulation durch die Tiere kompromittiert. Daher verwendeten wir einen kleineren Hautschnitt. Eine verbesserte Nahttechnik an der Gefäßanastomose sowie eine Ausspaarung zur Verbesserung der Bewegungsfreiheit der Stielgefäße des AV loop im Kammerboden konnte Einblutungen in die Kammer verhindern. Die Stabilität der Haut im Implantatbereich ließ über den Versuchszeitraum nach und der Nahtfaden zur Kammerbefestigung riss aus. Der Halt der Kammer am Implantationsbereich konnte leider weder durch die Fadenstärke, die Anzahl der Befestigungsschrauben und die Nahttechnik verbessert werden. Zudem war die Perfusion des AV loops auf eine Versuchsdauer von 3-4 Tagen begrenzt. Bei Design B wurde die Stabilität der Kammer durch Einsatz einer Tabaksbeutelnaht zur Kammerbefestigung deutlich verbessert und damit Manipulationen an der Naht durch die Versuchstiere eingeschränkt. Dieses Kammermodel lies sich über einen Zeitraum von 2 Wochen stabil am Implantationsort halten, zudem war der AV loop über den gesamten Versuchszeitraum fuktional perfundiert. Die Funktionalität des AV loop in der IVM-Kammer wurde mit Kammerdesign C weiter verbessert. Erste mikrovaskuläre Aussprossungen ausgehend vom AV loop wurden in GelMA an Tag 14 beobachtet. Am 21. Tag zeigte das Gefäßnetz deutliche Anzeichen von Gefäßreifung und Restrukturierung des Gefäßnetzwerkes. In der Fibrinmatrix wurde jedoch kein mikrovaskuläres Wachstum beobachtet. Bei der histologischen Untersuchung zeigte sich, dass in GelMA mehr Mikrogefäße ausgesprosst waren als in Fibrin. Der AV loop zeigte sich in allen Fibrinkonstruktionen thrombosiert, was möglicherweise auf die Degradierung der Fibrinmatrix zurückzuführen ist. Das Fibrin-Gewebskonstrukt wies jedoch mehr Bindegewebe und ein geringeres Konstruktgewicht auf. In-vitro zeigte sich die maximale Gefäßaussprossung der untersuchten Gefäßssegmente zwischen Tag 10 und 12 nach Versuchsbeginn. Die Gefäßssegmente von Lokalisationen in der oberen Extremität wießen im Vergleich zur unteren Extremität ein größeres angiogenes Potential auf. Darüber hinaus zeigte sich bei arteriellen Gefäßssegmenten gegenüber den entsprechenden venösen Segmenten eine ausgeprägtere mikrovaskuläre Aussprossung. Schlussfolgerung: Im ersten In-vivo-Teil unserer Studie konnten wir erfolgreich zwei verschiedene AV loop-IVM-Kammermethoden etablieren. Die erste Kammer (Design B) ermöglicht eine direkte In-vivo-Bildgebung ohne zusätzlichen chirurgischen Eingriff nach einer AV loop-Operation. Die Kammer des Designs C erfordert eine chirurgische Freilegung der subkutan implantierten AV loop-Kammer. Kammerdesign B ermöglicht eine IVM für 14 Tage und Kammerdesign C für 21 Tage. GelMA ist aufgrund seiner einzigartigen Eigenschaften, wie z. B. der geringen Degradationsrate und der hohen Transparenz eine geeignete Matrix für IVM-Untersuchungen im AV loop-Modell. Dieses IVM-Modell kann als neuartiges Werkzeug im Tissue-Engineering eingesetzt werden, um wichtige Regenerationsmechanismen in Echtzeit zu verstehen. Außerdem zeigt unsere Ex-vivo-Studie, dass die arteriellen Gefäße der oberen Extremitäten ein stärkeres angiogenes Potential im Vergleich zu Gefäßssegmenten von anderen Entnahmelokalisationonen haben. Daher kann die Anwendung eines arteriellen Transplantats der oberen Extremität möglicherweise die Neovaskularisierung eines Gewebskonstruktes sowie die de-novo Gewebeentwicklung erheblich verbessern.

Hintergrund und Ziele: Der Begriff Craniomandibuläre Dysfunktion (CMD) beschreibt mehrere Symptome, die sowohl die Kaumuskulatur als auch die Kiefergelenke und weitere angrenzende Strukturen betreffen, und fasst diese Symptome überbegrifflich zusammen. Zur Beurteilung der Kiefergelenke hat sich die Magnetresonanztomographie in den letzten Jahren als Goldstandard etabliert. Hierbei wird vornehmlich die ossäre und chondrale Komponente betrachtet. Eingehende Untersuchungen bezüglich des Weichgewebes respektive der Kaumuskeln liegen dagegen kaum vor. Somit war es Ziel dieser Studie etwaige Zusammenhänge zwischen muskulären Alterationen der Kaumuskeln, radiologischen Befunden des Kiefergelenks sowie weiteren Variablen wie Lebensalter oder Schmerzintensität nachzuweisen. Methoden: Diese klinische Studie berücksichtigte Patienten, die sich im Zeitraum vom 01.01.2015 bis 11.05.2017 im Rahmen der Kiefergelenksprechstunde der Klinik für Mund-, Kiefer- und Gesichtschirurgie des Universitätsklinikums Erlangen vorstellten. Die Studienteilnehmer wurden klinisch und radiologisch untersucht. Es wurden 53 Patienten (46 weiblich, 7 männlich) in die Studie eingeschlossen. Zunächst erfolgte anhand der magnetresonanztomographischen Bildgebung die Messung der maximalen Ausdehnung sämtlicher Kaumuskeln. Über geometrische Näherungsformeln konnten schließlich die Muskelvolumina idealisiert berechnet und mit pathologischen Veränderungen des Diskus, pathologischen Veränderungen der knöchernen Gelenkstrukturen sowie weiteren Variablen wie Lebensalter und Schmerzintensität verglichen werden. Die Korrelationen wurden mittels Spearman‘s-Rho analysiert. Ergebnisse und Beobachtungen: Zwischen pathologischen Veränderungen des rechten Diskus und dem Volumen des linken M. masseter im geschlossenen Zustand ließ sich eine statistisch signifikante Korrelation auf schwachem Niveau nach Cohen (r = -0,298; p = 0,037) nachweisen. Dagegen ergab sich zwischen pathologischen Veränderungen der knöchernen Gelenkstrukturen rechts und dem Volumen des rechten M. pterygoideus medialis im geschlossenen Zustand eine statistisch signifikante Korrelation auf moderatem Niveau (r = 0,32; p = 0,025). Ebenso zeigten sich statistisch signifikante Korrelationen auf moderatem Niveau zwischen pathologischen Veränderungen des rechten Diskus und dem Volumen des linken M. masseter im geöffneten Zustand (r = -0,308; p = 0,047). Das Lebensalter der Patienten korrelierte zudem statistisch signifikant auf moderatem Niveau mit dem Volumen des M. masseter beidseits im geschlossenen Zustand (r = -0,309/-0,325; p = 0,031/0,023), pathologischen Veränderungen der ossären Gelenkstrukturen beidseits (r = 0,463/0,44; p < 0,001/ = 0,001), und pathologischen Veränderungen des linken Diskus (r = 0,322; p = 0,19). Außerdem ließen sich zahlreiche statistisch signifikante Korrelationen zwischen pathologischen Veränderungen ossärer Kiefergelenkstrukturen und pathologischen Veränderungen der Disken berechnen. Schlussfolgerungen und Diskussion: Insgesamt zeigten sich bei Probanden mit ausgeprägten pathologischen Veränderungen des rechten Diskus reduzierte Volumina des linken M. masseter im geschlossenen und geöffneten Zustand. Ausgeprägte pathologische Veränderungen der knöchernen Gelenkstrukturen rechts gingen dagegen mit vergrößerten Volumina des rechten M. pterygoideus medialis im geschlossenen Zustand einher. Zudem ließ sich der Trend erkennen, dass ausgeprägte pathologische Veränderungen des Diskus von ausgeprägten pathologischen Veränderungen der ossären Kiefergelenkstrukturen begleitet werden. Ein reduziertes Volumen des M. masseter im geschlossenen Zustand war bei steigendem Lebensalter der Probanden zu beobachten. Ebenso gingen mit steigendem Lebensalter der Probanden sowohl höhergradige pathologische Veränderungen der ossären Gelenkstrukturen als auch der Disken einher. Die durch die Probanden angegebene individuelle Schmerzintensität zeigte sich dagegen unabhängig von muskulären Alterationen, pathologischen Veränderungen der ossären Kiefergelenkstrukturen oder pathologischen Veränderungen der Disken.

Hintergrund und Ziele: Schilddrüsenerkrankungen sind ein häufiger Befund in Deutschland, Operationen an der Schilddrüse stellen ca. 12 % aller viszeralchirurgischen Eingriffe hierzulande dar. Bei der Frage nach dem korrekten Resektionsausmaß muss zwischen ausreichender Rezidivprophylaxe und dem Belassen von funktionalem Restgewebe abgewogen werden. Die Hemithyreoidektomie stellt in manchen Situationen eine Alternative dar. Die Studienlage zum Verlauf von Schilddrüsengewebe nach Hemithyreoidektomie und dem Auftreten von Knotenrezidiven in der verbliebenen Seite liefert heterogene Ergebnisse bezüglich Faktoren, welche die Entstehung von Knotenrezidiven begünstigen. Die Identifikation solcher Faktoren könnte in der präoperativen Planung des Resektionsausmaßes bei Schilddrüsenoperationen unterstützend sein. Methoden: In der vorliegenden Untersuchung wurden Patienten ausgewählt, die sich im Zeitraum 2000-2015 im Krankenhaus Martha-Maria in Nürnberg, Referenzzentrum für Schilddrüsenchirurgie, einer Hemithyreoidektomie unterzogen haben. Dabei wurden Patienten ausgewählt, die sich zur prä- und postoperativen Diagnostik im Radiologisch-Nuklearmedizinischen Zentrum Nürnberg-Lauf-Rummelsberg vorgestellt hatten. Einschlusskriterium war dabei ein sonografisch evident knotenfreier Schilddrüsenlappen kontralateral des führenden Befundes. Die in Krankenhaus- und Radiologie-Informationssystemen archivierten Befunde wurden gesichtet, Daten in Microsoft EXCEL® extrahiert und mittels IBM SPSS Statistics® verarbeitet. Ergebnisse und Beobachtungen: Der endgültige Patientenpool umfasste 106 Patienten, 74 Frauen (70 %) und 32 Männer (30 %). Das durchschnittliche Alter bei Operation betrug 48 Jahre [13–74 Jahre]. In der präoperativen Szintigrafie waren 83 % der Knoten kalt, 7 % heiß, 10 % indifferent. Histologisch fanden sich Kolloidknoten in 48 (45 %), Adenom in 42 (40 %), Zufallsbefund Hashimoto in 9 (8 %), Zufallsbefund papilläres Mikrokarzinom in 7 (7 %). Zu postoperativen Komplikationen kam es bei 12 Patienten (11 %). Dazu zählten 7 (transiente) postoperative Paresen des N. Laryngeus Recurrens (7 %), es trat in 2 Fällen ein Wundinfekt auf (2 %), ebenso kam es zu 2 Fällen einer symptomatischen Hypocalciämie (2 %), in einem Fall kam es zu einer revisionsbedürftigen Nachblutung (1 %). Weder die Recurrensparesen noch die Hypocalciämien stellten sich im Verlauf als permanent heraus. Im mittleren Beobachtungszeitraum von 53 Monaten traten bei 27 Patienten (25 %) Knoten im kontralateralen, vormals knotenfreien Lappen auf. Keiner der Patienten im beobachteten Kollektiv wurde im Beobachtungszeitraum einer erneuten Operation an der Schilddrüse unterzogen. Die statistische Berechnung der Daten ergab im Kollektiv eine berechnete Wahrscheinlichkeit zur Knotenbildung nach 10 Jahren von 0,6. Faktoren, die eine Entwicklung von Knotenrezidiven kontralateral begünstigen waren das Vorliegen mehrerer Knoten präoperativ (p = 0,001) sowie ein Patientenalter > 45 Jahren (p = 0,041). Das Risiko eines Knotenrezidivs war bei Patienten dieser Altersgruppe um den Faktor 2,5 erhöht. Der Nutzen einer postoperativen Hormonsubstitution bleibt in der vorliegenden Studie ungeklärt. Schlussfolgerungen und Diskussion: Die Hemithyreoidektomie ist bei evident einseitigem Befund das adäquate Resektionsausmaß, es bestätigt sich das Verfahren der seitengetrennten Indikationsstellung bezüglich der Schilddrüsenlappen. Der Einfluss einer Substitutionstherapie kann aufgrund einer fehlenden Vergleichsgruppe nicht abschließend beurteilt werden. Ein Vergleich mit anderen Studien zeigt jedoch niedrige Rezidivraten und keine Reoperation.

Zielsetzungen: Defekte in der Kopf- und Halsregion werden standardmäßig mit mikrovaskulären Transplantaten wie dem freien Scapula- (SFF) sowie dem Fibula-Transplantat (FFF) behandelt, die im Laufe der Zeit einer gewissen Knochenresorption ausgesetzt sind. Ziel dieser Studie war die dreidimensionale Bewertung dieses Knochenresorptionsvolumens beider Transplantate. Material und Methoden: Über einen Zeitraum von 10 Jahren wurden Computertomographie (CT)-Aufnahmen von Patienten mit Unterkieferrekonstruktionen durch SFFs und FFFs untersucht. Die jeweiligen Transplantate wurden sowohl segmentiert als auch dreidimensional vermessen. Darüber hinaus wurden Faktoren wie Geschlecht, Alter, Nikotinabusus, Vorerkrankungen mit Typ-2-Diabetes und adjuvante Therapien auf ihren Einfluss untersucht. Ergebnisse: 211 CT-Aufnahmen von 67 Patienten (40 SFFs und 27 FFFs) wurden in die Studie aufgenommen. SFFs wiesen im Median ein etwas höheres Knochenvolumen auf (87,60% nach 730 Tagen und 86,55% nach 1500 Tagen) als FFFs (84,40% nach 730 Tagen und 82,10% nach 1500 Tagen). Bei Betrachtung des Endvolumens zeigten sich bei FFFs höhere mittlere Volumenwerte (88,22%) als bei SFFs (83,82%), wobei eine signifikante Korrelation zwischen Resorptionsvolumen und zeitlichem Verlauf bestand (r = 0,357, p = 0,024). Alle zuvor genannten Faktoren hatten keinen signifikanten Einfluss auf das Knochenvolumen. Schlussfolgerungen: Die Knochenvolumina von FFFs zeigten postoperative Volumenreduktionen, die denen von SFFs ähnlich waren, wobei die Volumenwerte von SFFs sich vereinzelt deutlich niedriger darstellten. Die Wahl eines mikrovaskulären Transplantats für Rekonstruktionen im Unterkiefer erweist sich im Hinblick auf die knöcherne Resorption als schwierig. Die vorgestellten Ergebnisse können eine Entscheidungshilfe für zukünftige Transplantationen bieten.

Im Rahmen der Progression des klarzelligen Nierenzellkarzinoms kann es zur Invasion der Vena cava durch einen Tumorthrombus (ccRCC/TT) kommen. Allerdings besteht auch in diesem fortgeschrittenen Stadium eine deutliche Heterogenität bezüglich des klinischen Verlaufs. Während sich mit bekannten Verfahren die Prognose bislang unzureichend vorhersagen ließ, gelang es in Vorarbeiten mittels im Tumorgewebe erfasster miRNA-Expressionen, ein Überlebensklassifikationsmodell auf Basis eines Kombinierten Risikoscores (miR-21, miR-126, miR-221) zu konzipieren. Hierdurch konnte das postoperative Überleben von ccRCC/TT Patienten des Würzburger Universitätsklinikums retrospektiv vorhergesagt werden. In der vorliegenden Arbeit war es möglich, mit Hilfe molekularbiologischer und biostatistischer Methoden das vorbeschriebene Modell erfolgreich an einem unabhängigen, größeren Regensburger ccRCC/TT Patientenkollektiv zu validieren. Am Tumor verstorbene Patienten konnten erneut einer klinisch relevanten High-Risk-Gruppe bzw. einer prognostisch günstigeren Gruppe zugeordnet werden. MiR-21 und miR-126 waren erneut statistisch signifikant mit der Fernmetastasierung und dem tumorbedingten Versterben assoziiert. MiR-21 präsentierte sich sowohl in der am Tumor verstorbenen als auch in der fernmetastasierten Patientengruppe deutlich überexprimiert, während die Expression von miR-126 stark vermindert war. Die neu untersuchte miR-205 zeigte sich in der fernmetastasierten sowie nodal positiven Patientengruppe hochreguliert, ein geringer Zusammengang mit dem tumorbedingten Versterben konnte hergestellt werden. Im zweiten Ansatz gelang es relevante miRNA-Expressionsunterschiede zwischen Seren Würzburger ccRCC-Patienten mit und ohne Invasion des Gefäßsystems sowie tumorfreien Kontrollen zu identifizieren. Die langfristige Herausforderung besteht darin, das validierte Überlebensklassifikationsmodell derart weiterzuentwickeln, dass es supportive klinische Anwendung in der Therapieplanung finden kann., Advanced clear cell renal cell carcinoma extending into the inferior vena cava (ccRCC/TT) represents a clinical high-risk setting. However, there is substantial heterogeneity within this high-risk patient subgroup regarding the survival outcomes. Previously, members of our group developed a combined risk score (CRS) – containing tumor tissue miR-21, miR-126 and miR-221 expression – which allowed the retrospective prediction of postoperative cancer-specific survival (CSS) in ccRCC/TT patients. In the present study, we successfully validated the CRS within an independent, larger cohort of ccRCC/TT patients from Regensburg. The combined risk score significantly stratified the ccRCC/TT cohort according to CSS. MiR-21 and miR-126 expression were significantly associated with the development of distant metastasis during follow-up, lymphonodal status at the time of surgery and cancer-related death. The newly investigated miR-205 showed an upregulation in the subgroup with distant metastasis and lymph node involvement and was associated to cancer-related mortality. In a second approach we measured the expression levels of serum-miR-21, miR-126, miR-145 and miR-221 using qRT-PCR in a ccRCC patient cohort from Würzburg. In this examination significant relative expression differences were identified between sera of ccRCC patients with and without vascular invasion, as well as controls. Conclusion: In our retrospective analysis, we successfully validated the CRS within an independent ccRCC/TT cohort.

1. Abstract Background and aims: Pancreatic ductal adenocarcinoma (PDAC), generally known as pancreatic cancer (PC), remains a deadly malignancy, with few symptoms showing before the disease reaches its advanced stage. Adjuvant chemotherapy after surgical resection is typically the preferred option to treat early pancreatic cancer. Although gemcitabine and nab-paclitaxel can profoundly improve the prognosis of advanced pancreatic cancer, the development of chemoresistance still leads to poor clinical outcomes. Our aim was to screen out sensitive targets for gemcitabine and paclitaxel chemotherapy through the CRISPR-CAS9-mediated protein kinase library. Combining the target gene inhibitor may enhance the effect of chemotherapy drugs. This research is expected to develop a novel strategy to increase the cytotoxic effects of gemcitabine and nab-paclitaxel, eventually achieving a significant survival benefit. Methods: In this study, based on CRISPR/Cas9 protein kinase library knockout screening, pancreatic cancer cells with a protein kinase library, and transfected cells were selected by puromycin treatment, followed by continuous exposure to vehicle control or gemcitabine/paclitaxel chemotherapy for 21 days. Next-generation sequencing as well as MAGeCK-VISPR analysis were performed to the identify candidate genes. After that, we used CRISPR-cas9 to knock out the target gene, and verified the screening hits for overcoming chemoresistance through in vivo and in vitro experiments. Results: We identified several cell cycle checkpoint kinases and DNA damage-related kinases as targets for overcoming chemoresistance, including cyclin-dependent kinase 7 (CDK7), whose loss of function made PDAC cells sensitive to drug combination chemotherapy. We further showed that the targeted inhibition of CDK7 regulated the cell cycle, mediated cell apoptosis, and synergized with gemcitabine and paclitaxel in vivo and in vitro, leading to more effective anti-tumor effects. Conclusion: Our results suggest a new therapeutic combination for PDAC, and support the application of CRISPR/Cas9-based functional screening in identifying novel therapeutic targets to potentiate existing cancer therapies. Hintergrund Die Ausbreitung durch Lufträume (STAS) ist ein kürzlich beschriebener Weg der Tumorinvasion, der mit einer schlechten Prognose bei primärem Lungenkrebs verbunden ist. Ziel dieser Studie war es, das Vorhandensein von STAS zu untersuchen und seine prognostische Bedeutung bei Patienten zu bewerten, die sich einer pulmonalen Metastasektomie (PM) bei solitären Metastasen von kolorektalem Darmkrebs (CRC) unterzogen werden. Materialien und Methoden Alle 49 Darmkrebspatienten (30 Männer und 19 Frauen, mittleres Alter 66 Jahre), die sich zwischen Januar 2008 und Dezember 2015 einer PM unterzogen, wurden retrospektiv analysiert. Ergebnisse STAS wurde in 26,5 % (n=13) der resezierten Proben identifiziert. Die Lage der pulmonalen Läsionen (zentral vs. peripher) wurde anhand der verfügbaren Computertomographie- Aufnahmen (n=47, 96%) beurteilt. STAS wurde bei allen fünf Patienten mit zentralen Metastasen (100 %) gegenüber 7 von 42 (17 %) mit peripheren Metastasen nachgewiesen (p=0,0001). Ein lokoregionales Rezidiv trat bei STAS-positiven Patienten auf (n=4 von 13 vs. n=0 von 36), alle STAS-negativen Patienten blieben rezidivfrei (p=0,003). Die mediane Anzahl der Alveolen mit STAS-Beteiligung betrug vier (Spanne von 2 bis 9). Es bestand ein statistisch positiver Zusammenhang zwischen der Anzahl der mit STAS befallenen Alveolen und lokoregionalem Wiederauftreten von Metastasen (p=0,0001). Das Vorhandensein von STAS ist kein Faktor, der die 5-Jahres-Gesamtüberlebensrate beeinflusst (p=0,6651). Schlussfolgerung Wir haben STAS als häufigen Befund bei resezierten Lungenmetastasen des KRK identifiziert und einen unbedeutenden Zusammenhang mit dem Ergebnis festgestellt.

Die hier vorliegende Untersuchung beschäftigt sich mit dem Vergleich zweier minimalinvasiver operativer Therapiestrategien der distalen Unterarmfraktur als häufigster Fraktur im Kindesalter. Für die vorliegende Studie erfolgte eine umfassende retrospektive Auswertung von 122 Fällen distaler kindlicher Unterarmfrakturen aus einem Zeitraum von elf Jahren, die in der kinderchirurgischen Abteilung des Universitätsklinikums Würzburg osteosynthetisch versorgt wurden. Ziel der Untersuchung war die Betrachtung der Epidemiologie des Verletzungsbildes und des Therapieerfolges in Abhängigkeit von der gewählten Osteosynthesetechnik. Die Kinder wurden dabei entweder allein durch eine Kirschner-Draht-Osteosynthese des Radius oder durch eine zusätzliche Fixierung der Ulna mittels elastisch stabiler intramedullärer Nagelung versorgt. Durch beide Operationsstrategien konnte für die meisten Patientinnen und Patienten ein gutes Therapieergebnis erreicht werden. Hinsichtlich der Operations- und Hospitalisierungsdauer zeigte sich ein Vorteil für Kinder, die osteosynthetisch lediglich durch Kirschner-Drähte versorgt wurden. In anderen Studien beobachtete Komplikationen in Form sekundärer Dislokationen der Fraktur auch nach Osteosynthese oder Infektionen im Bereich der Drahtenden konnten im hier untersuchten Kollektiv seltener festgestellt werden. Die vorliegende Untersuchung zeigt, dass, sofern die Indikation zur operativen Versorgung besteht, die Fixierung des Radius allein mittels Kirschner-Drähten eine adäquate Therapie distaler Frakturen von Ulna und Radius im Kindesalter darstellt. Von einer kombinierten Osteosynthese mittels Kirschner-Drähten und ESIN könnten besonders Patientinnen und Patienten profitieren, bei denen initial große Dislokationen der Fraktur, Begleitverletzungen des Arms oder eine Lokalisation im Bereich des diametaphysären Übergangs bestehen. Um diese Beobachtungen auch in größeren Kollektiven und prospektiv sowie randomisiert zu untersuchen, sind weitere Studien wünschenswert., The present study deals with the comparison of two minimally invasive surgical treatment strategies of the distal forearm fracture as the most frequent fracture in childhood. The study shows a comprehensive retrospective evaluation of 122 cases of distal pediatric forearm fractures from a period of eleven years, which were treated osteosynthetically in the pediatric surgery department of the University Hospital of Würzburg. The aim of the study was to examine the epidemiology of the injury pattern and the therapeutic success in relation to the selected osteosynthesis technique. The children were treated either by Kirschner wire osteosynthesis of the radius alone or by additional fixation of the ulna using elastic stable intramedullary nailing. Both surgical strategies resulted in a good therapeutic outcome for most patients. With regard to the duration of surgery and hospitalization, there was an advantage for children who were treated osteosynthetically with Kirschner wires only. Complications observed in other studies in the form of secondary dislocation of the fracture even after osteosynthesis or infections in the area of the wire ends were found less frequently in the present collective. The present study shows that, if surgical treatment is indicated, fixation of the radius using Kirschner wires alone is an adequate therapy for distal fractures of the ulna and radius in childhood. Patients who initially have large dislocations of the fracture, concomitant injuries of the arm, or a localization in the area of the diametaphyseal transition could particularly benefit from a combined osteosynthesis using Kirschner wires and ESIN. Further studies are desirable to investigate these observations also in larger collectives, prospectively and randomized.

Mit steigender Nachfrage an minimal-invasiven Therapieoptionen wächst auch das Interesse an innovativen Alternativen im Bereich des arteriellen Gefäßverschlusses nach PVI. Ziel der vorliegenden Arbeit war es, die Effektivität und Sicherheit eines neu auf dem Markt befindlichen, Doppelclip-basierten aktiven VVS zu prüfen. Eine hohe technische Erfolgsrate von 98,8 % bei einer geringen Komplikationsrate von 3,6 % wurde verzeichnet. Bei Anwesenheit der Komorbiditäten Dm und CNI 5 zeigte sich eine signifikante Assoziation zu einem vermehrten Auftreten von Komplikationen. Ein nachgewiesener signifikanter Zusammenhang bestand zudem zwischen einem erhöhten Kalzifikationsgrad der Punktionsstelle bei Vorliegen einer pAVK und eines Dm. Eine erhöhte Gefäßrigidität aufgrund von Komorbiditäten und vaskulären Kalzifikationen, intrinsische Fremdkörperreaktionen, vasoregulatorische Reaktionen oder Produktversagen bieten hypothetische Erklärungsansätze für die einzelnen komplikativen Fälle. Die Nutzung des VVS in spezifischen, bisher nicht beschriebenen Situationen (Unterdimensionierung, Anwendung nach Gefäßpunktionen mit Zugangsschleusen bis 9F, antegrade Punktionsrichtung, anspruchsvolle Eingriffe multimorbider Patienten mit komplexem vaskulärem Status) erwies sich als suffizient. Zur Prävention schwerwiegender Komplikationen während zukünftiger Interventionen wurden die Empfehlung ausgesprochen, eine Durchleuchtungsaufnahme zur Lagekontrolle vor Implantation des proximalen Clips anzufertigen. Eine speziell für Gefäßverschlüsse nach antegrader Punktion konzipierte Zugangsschleuse könnte das Abknicken von Zugangsschleusen bei VVS Applikation verhindern. Zusammenfassend kann das untersuchte aktive VVS mit einzigartigem Wirkmechanismus und spezifischen Design als effektiv und sicher angesehen werden., The need of minimally invasive therapy options is constantly growing and so is the need for alternative options of vascular closure after peripheral vascular interventions. The aim of this thesis was to assess the efficacy and safety of the novel double-clip-based vascular closure device (VCD). The overall technical success rate was 98,8% with an overall low complication rate of 3,6 %. The comorbidities diabetes and chronical kidney disease proof to be significant risk factors for the appearance of complications. There was a significant association between an increased degree of calcification of the puncture site in the presence of peripheral artery disease and diabetes. Increased vascular rigidity due to comorbidities and vascular calcifications, intrinsic foreign body reactions, vasoregulatory reactions, or product failure offer hypothetical explanations for the individual complicative cases. The use of the VCD during previously not analyzed settings e.g. undersizing in relation to the introducer sheath, accesses of up to 8 or 9 Fr with a 7 Fr device, antegrade puncture and challenging patient collective, proofed to be sufficient. To prevent major complications during future application, it is recommended to check the position of the VCD fluoroscopically, before implantation of the proximal wing. A specially designed introducer sheath may be helpful to prevent kinking of the introducer sheath during VCD application in antegrad punctures. In conclusion, the novel VCD proved to be effective and safe.

Hintergrund und Ziele: Trotz hoher Erfolgsraten stellt der mikrovaskuläre Gewebetransfer im Kopf- und Halsbereich weiterhin eine Herausforderung dar. Diese liegt in fehlenden detaillier-ten Erkenntnissen darüber, wie sich der verpflanzte Lappen gegenüber dem umliegenden, angrenzenden Weichgewebe im postoperativen Verlauf verhält. Daneben gilt es zu untersu-chen, inwiefern bestimmte Faktoren wie Transplantattyp, durchgeführter Radio-/Radiochemo-therapie in der Kopf- und Halsregion oder Lappenlokalisation die Empfängerstelle sowie die Transplantatheilung beeinflussen. Somit ist es Ziel der vorliegenden Studie den Einfluss oben genannter Faktoren auf die Empfängerstelle sowie die Transplantatheilung anhand der posto-perativen Untersuchung von Mikrozirkulationsparametern zu bestimmen. Patienten und Methode: Unter Verwendung des O2C-Diagnosegeräts (Oxygen-to-see, LEA-Medizintechnik GmbH, Gießen, Germany) wurden folgende postoperative Vitalitätsparameter der 100 untersuchten Transplantate, sowie deren umgebender Empfängerarealen erfasst: re-lativer Blutfluss (Flow), Blutflussgeschwindigkeit (Velocity), relative Hämoglobinmenge (rHb), Sauerstoffsättigung des Hämoglobins (SO2). Untersucht wurden neben erfolgreich verpflanz-ten auch erfolgreich revidierte Transplantate. Die O2C-Messungen erfolgten an sieben stan-dardisierten Zeitpunkten (1. bis 5., 7. und 14. postoperativer Tag) innerhalb von zwei Wochen. Ergebnisse: Im postoperativen Verlauf wiesen alle vier gemessenen Parameter des Trans-plantatlagers signifikant höhere Werte im Vergleich zu denen des mikrovaskulären Transplan-tates auf. Hinsichtlich des Radialis- und Latissimus-Dorsi-Lappens wiesen die Radialis-Trans-plantate zu jedem Zeitpunkt signifikant höhere Werte auf, mit der einzigen Ausnahme der Hä-moglobinkonzentration am letzten Untersuchungstag. Zusätzlich zeigte der Flow der Empfän-gerstelle des Radialis-Lappens signifikant höhere Werte im Vergleich zu denen des Latis-simus-Dorsi-Lappens. Ein ähnliches Muster zeigte sich auch bei der Velocity. Beim Vergleich der vor- und unbestrahlten Patienten zeigte der Flow der vorbestrahlten Empfängerstelle eine abfallende Tendenz, jedoch war sie nur am fünften und sechsten Untersuchungszeitpunkt sig-nifikant unterschiedlich. Zusätzlich wurde eine Tendenz zu höherem Lappenblutfluss bei den unbestrahlten Patienten im Vergleich zu den vorbestrahlten beobachtet. Dabei gab es jedoch keine statistisch signifikanten Unterschiede. Beim Vergleich von intra- und extraoralen Lappen wiesen die Intraoralen zu jedem Zeitpunkt signifikant höhere Blutflusswerte auf, mit Ausnahme des ersten postoperativen Tags. Die Velocity zeigte nur an den Untersuchungstagen zwei, drei und vier statistisch signifikante Unterschiede. Im Gegensatz dazu wurde beim Blutfluss des Empfängerbettes festgestellt, dass die angrenzende Schleimhaut im Vergleich zur extraoralen Haut-Empfängerregion signifikant deutlich höhere Flow-Werte aufwies, wiederum mit Aus-nahme des ersten postoperativen Tags. Ein ähnliches Muster wurde auch bei der Velocity der Empfängerstellen beobachtet. 2 Schlussfolgerung: Die vorliegende Studie ergab, dass sich die Perfusions- und Oxygenie-rungswerte zwischen Transplantat und Empfängerareal innerhalb der ersten zwei postopera-tiven Wochen signifikant voneinander unterscheiden. Der Radialis-Lappen sowie seine Emp-fängerstelle zeigen im Vergleich zum Latissimus-Dorsi-Lappen bzw. dessen Empfängerstelle signifikant höhere Perfusionswerte. Im Hinblick auf eine vorausgegangene Radio-/Radioche-motherapie lässt sich ein Einfluss sowohl auf die Transplantatlager- als auch die Transplan-tatperfusion feststellen, allerdings sind die Unterschiede statistisch nicht signifikant. Beim Ver-gleich von intra- und extraoral gelegenen Transplantaten ist hervorzuheben, dass die umlie-gende Schleimhaut im Vergleich zum Haut-Empfängerareal signifikant höhere Perfusions-werte zeigt. Da der Flow und die Velocity der intra- und extraoralen Lappen nicht konstant signifikante Unterschiede aufweisen, kann von einer leichten Tendenz zu höherer Durchblu-tung der intraoralen Lappen ausgegangen werden. Die Erfassung der postoperativen Perfusi-onsveränderungen zwischen Empfängerareal und mikrovaskulärem Transplantat, besonders in Anbetracht der anamnestischen Patienten- und Lappencharakteristika, führt zu einem bes-seren Verständnis wie sich der Lappen an seine neue Umgebung anpasst und welche Fakto-ren genau den Heilungsprozess beeinflussen. In diesem Zusammenhang wären weitere Un-tersuchungen hilfreich, die sich mit der Neovaskularisation desselben Lappentyps bei unter-schiedlichen Lappengrößen beschäftigen. Background and aims: Despite the high success rates, microvascular tissue transfer in the head and neck area is a challenge. This lies in the lack of detailed knowledge of how the transplanted flap behaves in relation to the surrounding and adjacent soft tissue in terms of time, postoperatively. It is also not clear how certain factors such as the type of transplant, the application or not of radio-/radiochemotherapy in the head and neck region or flap localization influence the flap healing and the recipient site. It is therefore the aim of the present study to determine the influence of the above-mentioned factors on the recipient site and the process of flap healing, on the basis of the postoperative examination of microcirculation parameters. Patients and methods: Using the O2C diagnostic device (Oxygen-to-see, LEA-Medizintech-nik GmbH, Gießen, Germany), the following postoperative vitality parameters of 100 cases of examined free flaps and their surrounding recipient areas were recorded: relative blood flow (flow), blood flow rate (velocity), relative hemoglobin amount (rHb), oxygen saturation of he-moglobin (SO2). In addition to successfully transplanted flaps, successfully revised flaps were also examined. The O2C measurements were carried out at seven predetermined examination days (1st to 5th, 7th and 14th postoperative day) over a period of two weeks. Results: In the postoperative course, all four parameters of the flap bed measured by O2C (flow, SO2 , rHb, velocity) showed significantly higher values compared to the parameters of the microvascular flap. With regard to the radial forearm and latissimus dorsi flaps, the radial forearm flaps showed significantly higher values at all times, with the only exception being the hemoglobin concentration on the last day of examination. In addition, the blood flow of the recipient site of the radial forearm flap showed higher values compared to that of the latissimus dorsi flap. Blood flow velocity showed a similar pattern. When comparing the pre-irradiated and unirradiated patients, the blood flow of the pre-irradiated recipient site showed a decreasing tendency compared to the unirradiated site, but it was only significantly different when meas-ured during the 5th and 6th examination days. In addition, a trend towards higher flap blood flow was observed in the unirradiated patients compared to the previously irradiated patients, but there were no statistically significant differences. When comparing intraoral and extraoral flaps, the intraoral flaps showed significantly higher blood flow values at all times, with the exception of the 1st postoperative day. The blood flow velocity showed statistically significant differences only on the 2nd, 3rd and 4th examination days. In contrast, it was found that in the blood flow of the recipient bed, the mucosa had significantly higher flow values and differed postoperatively significantly from the skin - again with the exception of the 1st postoperative day. A similar pattern was also observed for the blood flow velocity of the recipient sites. Conclusion: The present study found that the perfusion and oxygenation values between flap and recipient area differ significantly from one another within the first two postoperative weeks. 4 The radial forearm flap and its recipient site show significantly higher perfusion values com-pared to the Latissimus-Dorsi flap or its recipient site. With regard to prior radio-/radiochemo-therapy, an influence on both the flap bed and the flap perfusion is observed, but the differ-ences are not statistically significant. In the comparison between intraoral and extraoral flaps it should be emphasized that the surrounding mucosa shows significantly higher perfusion values compared to the extraoral skin recipient area. Since the flow and velocity of the intraoral and extraoral flaps do not consistently show significant differences, a slight tendency towards increased blood flow to the intraoral flaps can be assumed. The recording of postoperative perfusion changes, especially in view of the patient's history and flap characteristics, leads to a better understanding of how the flap adapts to its new environment, and of which are the exact factors influencing the healing process. In this context, further research would be helpful, focusing on the neovascularization of the same flap type but with different flap sizes.

3 Abstract 3.1 OBJECTIVES The objective of this study is to re-evaluate routine preoperative panendoscopy in patients with oral malignancy as a staging procedure. 3.2 MATERIALS AND METHODS Using a digital operation register (MCC - MEIERHOFER Clinical Competence) all pa-tient data where a preoperative panendoscopy was performed between January 2015 and November 2020 was listed. Consecutively all histopathological results and preoperative CT Imaging Data was analysed. 3.3 RESULTS 460 patients with oral malignancy received a preoperative staging panendoscopy be-tween January 2015 and November 2020 in the Department of Oral and Maxillofacial Surgery Friedrich-Alexander University Erlangen-Nürnberg (FAU). In 53 patients, a bi-opsy was taken (11.5%). In 15 patients, the histopathological result confirmed malig-nancy (3.3%). 28% of all biopsies revealed malignancy. From these 15 malignant re-sults, there was no case where malignancy was not already suspected from CT imag-ing. 3.4 CONCLUSIONS Advancements in imaging technologies in the last decades have led to a high proba-bility of detecting any malignant or otherwise suspicious lesion in a standard contrast staging CT-Scan. The current data suggests, that panendoscopy seldom leads to the detection of synchronous malignancy, and if it does, it was diagnosed or suspected beforehand in CT-imaging.

Intraoperative Flussmessungen mittels Transit-Time Flowmetry und mikrovaskulärer Indocyaningrün Angiographie bei freien DIEP und ms-TRAM Lappenplastiken bei der autologen mikrochirurgischen Brustrekonstruktion. 1.1 Hintergrund und Ziele Die suffiziente Blutversorgung ist ein entscheidender Faktor für die Überlebensrate freier Lappenplastiken. Das Ziel dieser Studie war es Blutflusseigenschaften der Anschluss- und Empfängergefäße bei freien DIEP (= Deep Inferior Epigastric Perforator) und ms-TRAM (= muscle-sparing Transverse Rectus Abdominis Muscle) Lappenplastiken im Rahmen autologer mikrochirurgischer Brustrekonstruktionen zu analysieren und miteinander zu vergleichen. 1.2 Methoden Diese prospektive klinische Studie kombiniert hierfür erstmals die Technologie der Ultraschall-basierten Transitzeit-Flussmessung (TTFM = Transit-Time Flowmetry) mit der mikrovaskulären Indocyaningrün Angiographie zur Messung des Blutflussvolumens, des Gefäßwiderstandes, sowie der intrinsischen Durchflussrate von Lappenplastiken (ITT = Intrinsic Transit Time) bei freien DIEP und ms-TRAM Lappenplastiken. 1.3 Ergebnisse 20 Patientinnen (Durchschnittsalter: 52 Jahre) mit 24 freien Lappenplastiken, wovon 14 DIEP und 10 ms-TRAM Lappenplastiken waren, wurden in diese Studie eingeschlossen. Das mittlere arterielle Blutflussvolumen vor Absetzen des Lappenstiels war mit 11.5 ± 4.8 ml/min bei ms-TRAM Lappenplastiken signifikant höher als bei DIEP Lappenplastiken mit 7.2 ± 1.9 ml/min (p < 0.05). Nach Anastomose mit der Arteria mammaria interna lag das durchschnittliche Blutflussvolumen von ms- TRAM Lappenplastiken bei 13.5 ± 4.2 ml/min und war somit ebenfalls höher als bei DIEP Lappenplastiken mit 9.7 ± 5.6 ml/min (p = 0.07). Im Gegensatz dazu zeigte sich der durchschnittliche arterielle Gefäßwiderstand bei DIEP Lappenplastiken sowohl vor Lappenhebung als auch nach Anastomosierung signifikant höher als bei ms-TRAM Lappenplastiken. Die Intrinsic Transit Time von DIEP Lappenplastiken war mit 52 ± 18 Sekunden signifikant länger als die von ms-TRAM Lappenplastiken mit 33 ± 11 Sekunden. Die Gesamtüberlebensrate aller Lappenplastiken betrug 100%. Die DIEP Lappenplastik mit der längsten Intrinsic Transit Time (77 Sekunden) musste jedoch aufgrund einer arteriellen Thrombose operativ revidiert werden. 1.4 Schlussfolgerung In dieser Studie konnten durch den Vergleich von Blutflusseigenschaften zwischen DIEP und ms-TRAM Lappenplastiken Unterschiede des Blutflussvolumens, dem arteriellen Gefäßwiderstand und der Intrinsic Transit Time erhoben werden. Diese Messungen zu definierten Zeitpunkten sollen als Normwerte und in weiterer Folge als Grundstein für die Erhebung positiv prädiktiver Werte von Durchblutungsstörungen im Rahmen autologer Brustrekonstruktionen mittels DIEP und ms-TRAM Lappenplastiken dienen. Background: Vascular patency is the key element for high flap survival rates. The purpose of this study was to assess and compare the blood flow characteristics of deep inferior epigastric perforator (DIEP) and muscle-sparing transverse rectus abdominis musculocutaneous (ms-TRAM) flaps for autologous breast reconstruction. Methods: This prospective clinical study combined Transit- Time Flowmetry and microvascular Indocyanine Green Angiography for the measurement of blood flow volume, vascular resistance, and intrinsic transit time. Results: Twenty female patients (mean age, 52 years) received 24 free flaps (14 DIEP and 10 ms-TRAM flaps). The mean arterial blood flow of the flap in situ was 7.2 ± 1.9 mL/min in DIEP flaps and 11.5 ± 4.8 mL/min in ms-TRAM flaps (p < 0.05). After anastomosis, the mean arterial blood flow was 9.7 ± 5.6 mL/min in DIEP flaps and 13.5 ± 4.2 mL/min in ms-TRAM flaps (p = 0.07). The arterial vascular resistance of DIEP flaps was significantly higher than that of ms-TRAM flaps. The intrinsic transit time of DIEP flaps was 52 ± 18 s, and that of ms-TRAM flaps was 33 ± 11 s (p < 0.05). The flap survival rate was 100%. One DIEP flap with the highest intrinsic transit time (77 s) required surgical revision due to arterial thrombosis. Conclusion: In this study, we established the blood flow characteristics of free DIEP and ms-TRAM flaps showing different blood flow rates, vascular resistances, and intrinsic transit times. These standard values will help to determine the predictive values for vascular compromise, hence improving the safety of autologous breast reconstruction procedures.

Background: The optimal management strategy for acute aortic dissection remains controversial. Whether a limited primary (index) repair would potentially increase the need for late aortic reintervention is still an open debate. Methods: We conducted a single-center retrospective cohort study using our cardiothoracic database and electronic patient records. From January 2000 through 2020, 396 consecutive adult patients with aortic dissection who underwent cardiac surgery were analyzed. Our research hypothesis was that limited aortic index repair (i.e., isolated aorta ascends replacement without an open distal anastomosis) is associated with a higher incidence of reoperation. Results: Patients with type B were younger. The average age for type A was 57 years, and for type, B was 51 years. The age difference was significant (p = 0.02). Emergency and the life-saving indication were settled in 93% (n=368) of the primary operations. Reoperation was necessary for 16% (n=65), where elective indication was dominant with 90% (n=59 out of 65). In-hospital mortality for the primary operation was 20% (n=79 out of 396). However, in-hospital mortality for the group of patients who had redo surgery was 9% (n=6 out of 65). Patients who tend to have reoperation had a slightly lower EuroSCORE module I. After the initial type, A aortic dissection, reoperation on the aorta was the main indication for a redo intervention, with 37 % (n=24 out of 65) and 18% (n=12 out of 65) for limited and extended repair, respectively. However, the chi-squared test (χ2 test) showed no significant difference with a p-value = 0.569. Prolonged cross-clamp time was an independent predictor of mortality with a p-value = 0.004. Conclusions: In acute settings, when the pathology is limited to the ascending aorta adopting an extended repair strategy won’t decrease the incidence of reoperation and will increase mortality due to the prolongation of cross-clamp time. Zusammenfassung auf Deutsch Hintergrund und Ziele: Die optimale Behandlungsstrategie für eine akute Aortendissektion bleibt umstritten. Die Frage, ob eine begrenzte primäre (Index-)Reparatur möglicherweise die Notwendigkeit einer späten Reintervention an der Aorta erhöhen würde, ist noch offen. Methoden: Wir haben eine retrospektive Kohortenstudie an einem einzigen Zentrum durchgeführt und dabei unsere kardiothorakale Datenbank und elektronische Patientenakten verwendet. Von Januar 2000 bis 2020 wurden 396 konsekutive erwachsene Patienten mit Aortendissektion, die sich einer Herzoperation unterzogen, analysiert. Unsere Forschungshypothese lautete, dass eine begrenzte Aortenindexreparatur (d. h. isolierter Aorta-Ascends-Ersatz ohne offene distale Anastomose) mit einer höheren Inzidenz von Reoperationen verbunden ist. Ergebnisse und Beobachtung: Patienten mit Typ B waren jünger. Das Durchschnittsalter bei Typ A lag bei 57 Jahren, bei Typ B bei 51 Jahren. Der Altersunterschied war signifikant (p = 0.02). Bei 93 % (n=368) der Primäreingriffe wurden der Notfall und die lebensrettende Indikation gestellt. Eine Re-Operation war bei 16% (n=65) notwendig, wobei die elektiven Indikationen im Rahmen einer Re-operation mit 90% (n=59 von 65) dominierten. Die In-Hospital-Mortalität bei der Primäroperation betrug 20 % (n=79 von 396). In der Gruppe der Patienten, die sich einer erneuten Operation unterzogen, lag die Krankenhaussterblichkeit jedoch bei 9 % (n=6 von 65). Patienten, die zu einer Re-Operation neigen, hatten ein etwas niedrigeres EuroSCORE-Modul I. Nach der ersten Aortadissektion vom Typ A war die Re-Operation an der Aorta die Hauptindikation für einen erneuten Eingriff, mit 37 % (n=24 von 65) für eine begrenzte Reparatur, und 18 % (n=12 von 65) für eine erweiterte Reparatur. Der Chi- Quadrat-Test (χ2-Test) ergab jedoch keinen signifikanten Unterschied mit einem p- Wert = 0.569. Eine verlängerte Cross-Clamp-Zeit war ein unabhängiger Prädikator für die Sterblichkeit mit einem p-Wert = 0.004. Schlussfolgerungen: In der Akutsituation, wenn die Pathologie auf die Aorta ascendens beschränkt ist, führt die Anwendung einer erweiterten Reparaturstrategie nicht zu einer Verringerung der Inzidenz von Reoperationen und erhöht die Sterblichkeit aufgrund der verlängerten Abklemmzeit. Die in diesem Datensatz präsentierten Ergebnisse zeigen keinen statistischen Unterschied zwischen den beiden Techniken (d. h. eingeschränkt und erweitert) in Bezug auf die Reoperation, vorausgesetzt, der Primäreingang wird entfernt. Dieser Datensatz zeigt, dass Reoperationen mit einer niedrigen Sterblichkeitsrate durchgeführt werden können. Außerdem konnte die überwiegende Mehrheit der Reoperationen elektiv durchgeführt werden, so dass Zeit für eine sorgfältige Fallplanung zur Verfügung stand, was wahrscheinlich zu den hervorragenden Ergebnissen beitrug. Bei unseren Patienten, bei denen eine begrenzte Reparatur mit Exzision des Primäreingangs durchgeführt wurde, kam es nicht häufiger zu Reoperationen als bei den Patienten, bei denen eine erweiterte Reparatur durchgeführt werden musste. Wenn also eine erweiterte Reparatur durchgeführt wird, obwohl sie nicht notwendig ist, kann dies die Sterblichkeit erhöhen, da es zu einer vermehrten Cross-Clamp-Ziet. In Anbetracht der neuen Definition der Quartärprävention, die lautet: "Eine Maßnahme, die ergriffen wird, um Personen vor einer medizinischen Intervention zu bewahren, die wahrscheinlich mehr Schaden als Nutzen anrichtet", ist es offensichtlich, dass die Anwendung einer erweiterten Reparatur, wenn sie nicht notwendig ist, als schädliche Übermedikalisierung eingestuft werden kann.

Hintergrund und Ziele Mit dem demografischen Wandel steigt die Zahl der versorgungsbedürftigen Schenkelhalsfrakturen. Der minimalinvasive SuperPATH Zugang nutzt die Nische zwischen dem M. gluteus medius und der Piriformissehne. Bei Anwendung dieses Zugangsweges kann eine Luxation des Femurkopfes und die Ablösung der kurzen Außenrotatoren und des Tractus iliotibialis vermieden werden. Patienten mit Coxarthrose ermöglicht dieser Zugang nach dem Einsetzen einer Totalendoprothese eine schnelle postoperative Mobilisation. Ziel dieser Studie ist es, den SuperPATH Zugang für die endoprothetische Versorgung von Schenkelhalsfrakturen bei Patienten hohen Alters zu erproben und Vor- und Nachteile darzulegen. Methoden Es werden 42 Patienten mit Schenkelhalsfraktur über den SuperPATH Zugang mit Duokopf- oder Totalendoprothesen operativ versorgt und verschiedene anamnestische und perioperative Parameter ausgewertet. Ergebnisse und Beobachtungen Die durchschnittliche Schnitt-Naht-Zeit für das Implantieren einer Duokopf Prothese beträgt 65 min und für das Einsetzen einer Totalendoprothese 120 min. 36 % der Patienten benötigen während des Eingriffs eine Bluttransfusion. Intraoperativ kommt es zu 3 Acetabulumfrakturen, insgesamt 4-mal findet eine Konversion zum posterioren Zugang statt. Die Krankenhausaufenthaltsdauer beträgt durchschnittlich 15 Tage. Bei 10 % der Operierten kommt es postoperativ zu einer Luxation. Innerhalb des Beobachtungszeitraums von 3 Monaten versterben 19 % der Studienteilnehmer. Die operateurspezifische Auswertung zeigt signifikante Unterschiede bei der Schnitt-Naht-Zeit (59 vs. 132 min) und den intraoperativen Komplikationsraten. Operateur 1 kommt gut mit dem neuen minimalinvasiven Operationszugang zurecht. Im 120-Tage-Follow-Up sind die Ergebnisse aller Operateure bezüglich der Entwicklung der Mobilität und der Selbstversorgungsmöglichkeiten, dem Durchführen von allgemeinen Tätigkeiten und den körperlichen Beschwerden zufriedenstellend. Schlussfolgerungen und Diskussion Die erhoffte schnellere postoperative Mobilisation von älteren, multimorbiden Patienten durch den gewebeschonenden SuperPATH Zugang konnte in dieser Studie nur bedingt erreicht werden. Dies steht in Zusammenhang mit dem Alterstraumatologischen Konzept und den relativ hohen ASA Klassen der Patienten. Die aufgetretenen intraoperativen Komplikationen waren größtenteils operateurabhängig. Die geringere Übersicht im Operationsfeld und die besondere Anatomie einer dislozierten Schenkelhalsfraktur erfordern neben einer großen operativen Erfahrung auch eine zugangs- und frakturspezifische Schulung. Das Ziel, dieser Patientengruppe sehr hohen Alters durch den SuperPATH Zugang eine möglichst atraumatische Versorgung zu ermöglichen, ist in dieser Studie nur Operateur 1 gelungen, nicht jedoch im Team. Objectives As the demographic structure changes, numbers of femoral neck fractures in need for therapy are increasing. The minimally invasive SuperPATH approach uses the interval between the gluteus medius muscle and the piriformis tendon. During surgical intervention it is not necessary to dislocate the femoral head. The short external rotators and the iliotibial tract can be preserved. For patients with osteoarthritis this approach leads to an early postoperative mobilization after total hip arthroplasty. Purpose of this study is to analyze the SuperPATH approach for hip arthroplasty of femoral neck fractures in elderly patients and to state the advantages and disadvantages. Design and methods 42 patients with femoral neck fracture are treated surgically through SuperPATH approach with hemi or total hip arthroplasty and various perioperative parameters are evaluated. Observation and results Mean operative time for hemiarthroplasty is 65 min, for total hip arthroplasty 120 min. 36 % of the patients receive a blood transfusion. Intraoperatively 3 times the acetabulum fractures, 4 times the SuperPATH incision is extended to the posterior approach. Mean LOS is 15 days. 10 % of the patients suffer a dislocation postoperatively. Within the first 3 months the mortality rate is 19 %. The surgeon specific evaluation shows significant differences in the mean operative time (59 vs. 132 min) and the intraoperative complication rates. Surgeon 1 reports positive outcomes associated with this approach. The 120-days-follow-up results are satisfactory for all surgeons. Conclusions The expected earlier mobilization of especially elderly multimorbid patients, using the tissue-sparing SuperPATH approach, was only partially achieved in this study. This is related to the ATZ concept and the high ASA grading of the study participants. Intraoperative complications were mostly surgeon specific. To ensure good orientation in the small operating field and with the special anatomy of a dislocated femoral neck fracture, practical well experienced surgeons, receiving a fracture specific instruction, are fundamental for success. The aim, to allow this group of elderly patients a treatment as atraumatic as possible, was achieved only by surgeon 1, not within the team.

Das Prostatakarzinom (PCa) ist die am häufigsten auftretende und am dritthäufigsten zum Tode führende Krebserkrankung unter Männern in Deutschland. Das Wachstum des PCa verläuft in allen Stadien androgenabhängig. Die Expression des Androgenrezeptors (AR) und seiner Spleißvarianten (AR-V) ist hierbei ein entscheidender Faktor, deren Bedeutung noch nicht abschließend geklärt ist. Ziel dieser Studie war es, die subzelluläre Proteinexpression des AR und seiner Spleißvariante AR-V7 immunhistochemisch zu charakterisieren und die Ergebnisse mit klinisch-pathologischen Daten und Prognosen zu korrelieren. Es erfolgte die immunhistochemische Anfärbung von Schnittpräparaten aus Tissue Microarrays (TMAs) von Prostatektomiepräparaten von insgesamt 410 PCa-Patienten mittels proteinspezifischer, monoklonaler Antikörper für die Proteine AR und AR-V7. Für die immunhistochemische Auswertung wurde für AR und AR-V7 jeweils ein immunreaktiver Score (IRS) für die zytoplasmatische bzw. nukleäre Expression sowie für AR-V7 der prozentuale Anteil an zytoplasmatischen Granula erhoben. Der nukleäre und zytoplasmatische Nachweis von AR war jeweils nicht mit der Prognose assoziiert. AR V7 wurde lediglich vereinzelt im Zellkern nachgewiesen. Für die Expression von AR-V7 im Zytoplasma bzw. in zytoplasmatischen Granula hingegen zeigte sich in der Kaplan-Meier-Analyse jeweils eine Assoziation mit dem rezidivfreien Überleben (RFS). Während der Nachweis von AR-V7 im Zytoplasma mit einem kürzeren RFS assoziiert war, zeigte sich für den Nachweis von AR-V7 in zytoplasmatischen Granula eine Assoziation mit einem längeren RFS. In einer multivariaten Cox-Regressionsanalyse bestätigte sich lediglich eine negative (

Dazu wurde vorab ein Kollektiv an Versuchs- und Kontrolltieren zur Etablierung eines BRONJ-Tiermodells untersucht [1]. Ziel war es, mögliche Unterschiede in der periimplantären knöchernen Einheilung zwischen Versuchstieren unter Gabe von Bisphosphonaten und einer Kontrollgruppe, die keine Bisphosphonate erhielt, zu analysieren. Methoden: Zwei Versuche wurden mit je zwei Gruppen à sechs Hausschweinen durchgeführt. Eine Gruppe diente hierbei als Kontrollgruppe, die andere als Versuchs- beziehungsweise Experimentalgruppe. Die Versuchstiere erhielten einmal pro Woche, über einen Zeitraum von fünf Wochen intravenös Bisphosphonate (Zoledronat® 4 mg). Versuch 1: 60 Tage nach der ersten Bisphosphonatgabe wurden je vier Implantate, extraoral von sagittal inferior in den rechten Corpus mandibulae zur Angulation des Kieferwinkels hin, gesetzt. Je drei Tiere wurden 30 bzw. 90 Tage nach der Implantation geopfert, um die Knochendichte, den Knochenimplantatkontakt und den Mineralisationsgrad des Knochens zu ermitteln. Versuch 2: Vor der Medikation wurde bei den Tieren der untere zweite rechte Prämolar extrahiert. Nach einer Ausheildauer von 60 Tagen erhielt die Experimentalgruppe Bisphosphonate. In den Bereich der Extraktionsstelle wurde 60 Tage nach der ersten Bishosphonatgabe je ein Implantat inseriert. Die Tiere wurden 30 und 90 Tage nach der Implantation geopfert. Im Anschluss wurden die Knochendichte, der Knochenimplantatkontakt und der Mineralisationsgrad des Knochens gemessen. Ergebnisse: Von den 29 innerhalb der Kontrollgruppe gesetzten Implantaten verblieben bis zur Aufbereitung 13. Dagegen blieben in der Experimentalgruppe von ebenfalls 29 inserierten Implantaten 25 erhalten. Versuch 1: Bei späterem Opferungszeitpunkt war die Knochendichte im Defekt höher. Die Knochendichte im Defekt in der Experimentalgruppe erreichte nach 30 Tagen den geringsten, nach 90 Tagen jedoch den höchsten Wert. Der Knochenimplantatkontakt entpuppte sich zur Kortikalis hin als höher als kortikalisfern. In der Experimentalgruppe konnte ein größerer Kontakt als in der Kontrollgruppe nachgewiesen werden, aber auch die Unterschiede zwischen dem kortikalisnahen und -fernen Knochen zeigten sich hier deutlicher. Mit zunehmendem Alter der Tiere wies der Knochen eine höhere Mineralisation auf. Nach 90 Tagen war ein signifikanter Unterschied im Defekt der Experimentalgruppe gegenüber der Kontrollgruppe festzustellen. Versuch 2: Bei einer Opferung nach 90 Tagen war eine höhere Knochendichte als nach 30 Tagen zu verzeichnen. Die Werte näherten sich dem Signifikanzniveau. Ein Knochenimplantatkontakt konnte in der Kontrollgruppe nicht nachgewiesen werden. In der Experimentalgruppe war er nach 30 Tagen größer als nach 90 Tagen. Der Anteil an nicht-kalzifiziertem Knochen war in der Implantatregion kleiner, unabhängig von der Bisphosphonatgabe. Praktische Schlussfolgerungen: Die beobachteten Ergebnisse aus dem ersten Versuch konnten weitestgehend mit dem zweiten Versuch bestätigt werden: Die gleichzeitige Gabe von Bisphosphonaten bei Implantation führt zu einer stabileren Wundheilung und Implantateinheilung, die jedoch auch den Beginn einer BRONJ darstellen kann. Background: Application of bisphosphonates has long been regarded as a contraindication for the insertion of implants. The present study aims to investigate osseointegration of implants under the application of bisphosphonates in a porcine animal model. In advance, experimental and control animals have been investigated to establish a BRONJ animal model [1]. Here, we compare periimplantate healing in experimental and control animals under application of bisphosphonates. Methods: We conducted two experiments with two groups of animals each, either one consisting of twelve pigs. In each group, six animals served as a control group while the other six served as an experimental group. Pigs in the experimental group received intravenously, bisphosphonates once a week over five weeks. In experiment one, the implants were applied extraorally. In experiment two, teeth were extracted 60 days before the experiments started. After initial healing, the implants were inserted into the cavity. In both experiments, six animals (three of each group) were sacrified 30 days after bisphosphonate application, and the other six after 90 days. For each experiment bone density, bone – implant contact and mineralization were analyzed. Results: In total, 58 implants were placed, 29 in the experimental and control group in both experiments. While 25 implants remained for preparation in the experimental group, only 13 remained in the control group. Experiment 1: Bone density in defect areas rises with the increasing experimental interval. It was lowest in the experimental group after 30 days, but also here, highest 90 days after implantation. The edaphic bone always had higher bone densities than the regions of interest (ROIs). Bone-implant contact turned out to be tighter proximal to corticula than distal. In the experimental group this contact was larger than in the control group, and the differences between nearer to corticula versus with distance to the corticula, higher. Mineralization increased with time. After 90 days the mineralization differs significantly between control and experimental group in the defect. Experiment 2: Bone density is higher after 90 than after 30 days, this result is approaching statistical significance. The edaphic bone of the control region always shows a higher density than the defect area. Here, an exception is found in the control group after 90 days, where values appear more uniform in the bone. There is no bone-implant contact in the control group. In the experimental group, bone-implant contact is more developed after 30 than after 90 days. Mineralization is higher the control region, independent of bisphosphonate application and time. The control group shows significantly more non-mineralized bone tissue than the experimental group. After 90 days, mineralization is highest in the experimental group. Conclusions: In total, results of the second experiment confirm those of the first experiment: Implantation under application of bisphosphonates leads to stronger insertion and wound healing, while it can also be signs of early development of BRONJ.

Minimalinvasive Behandlungsmethoden von Partialrupturen des VKBs wurden seit den 1990 er Jahren diskutiert. Erstmalige Ergebnisse veröffentlichte Richard Steadman 2006 und stellte diese Methode als Alternative zur Kreuzbandersatzplastik vor (Steadman, 2006). Ziel dieser Arbeit war es zu untersuchen, ob die Healing Response Technik eine suffiziente, langfristige Therapieoption bei Partialrupturen des VKBs ist. Bei der Healing Response Technik wird arthroskopisch das verletzte VKB auf seine Stabilität untersucht und die Ruptur anschließend klassifiziert. Bei einer Partialruptur wird der proximale Ansatz des VKBs angemeißelt. Dadurch kommt es zu reaktiven Einblutungen, Einsprossen von mesenchymalen Stammzellen und Wachstumsfaktoren. Erfolgt eine suffiziente Vernarbung kann eine Stabilisierung des VKBs erzielt werden. Das Patientenkollektiv wurde in einem Zeitraum von 2005-2010 im Sporthopaedicum in Straubing mittels Healing Response Technik operativ versorgt. Einschlusskriterien waren ein stattgehabtes Trauma, eine kernspintomografisch beschriebene Ruptur oder Partialruptur des VKB, ein positiver Lachman-Test mit unsicherem oder vorhandenem Anschlag, ein maximal einfach positiver Pivot Shift Test, sowie eine subjektive Instabilität. Es wurden 163 Patienten eingeschlossen. Das initiale Nachuntersuchungsintervall betrug 4,5 Jahre. Die Langzeitergebnisse wurden mittels Mixed-Mode-Befragung mit einem Follow up von 11,4 Jahren erfasst. Insgesamt konnten so 138 Patienten nachuntersucht werden. Die Differenz der Roliometer Messung konnte von präoperativ 10 ± 2 mm auf postoperativ 7 ± 2 mm signifikant verbessert werden. Der postoperative Lyshom Score betrug 90 ±12 Punkte. Der Tegner Activity Score lag bei 5 ± 2. Im Langzeitfollow-up betrug die Revisionsrate 13% mit einem Revisionszeitpunkt von 51 ± 30 Monaten. 8% der befragten Patienten erlitten eine Reruptur. 93% der Patienten beurteilten ihr Kniegelenk subjektiv als stabil. Somit stellt die Healing Response Technik unter Berücksichtigung der Einschlusskriterien und der intraoperativen Befunderhebung eine suffiziente Therapieoption für Partialrupturen des VKBs dar., Minimally invasive treatment options in partial ruptures of anterior cruciate ligament (ACL) have been discussed since the 1990s. First results were published by Richard Steadman in 2006 by presenting this method as an alternative to plastic substitution of anterior cruciate ligament plastic (Steadman, 2006). The purpose of this was to investigate if the healing response technique could serve as a long-term and sufficient therapeutic option in partial ruptures of ACL. Using the healing response technique, the ACL injury is examined during arthroscopy regarding stability. In the case of a partial rupture, the proximal parts of ACL are treated by trephination of the proximal part of the ACL which is followed by reactive bleeding, migration of mesenchymal cells and growth factors. This leads to a sufficient scarring and stabilization of the ACL. Between 2005- 2010, 163 patients underwent healing response technique in Sporthopaedicum Straubing. Inclusion criteria consisted of a trauma, a complete or partial rupture of ACL described in MRT, a positive Lachman-test with an unfirm or clear endpoint, a single positive Pivot shift test and a subjective instability. 163 patients were included. The initial postoperative follow up time was 4.5 years. The long-term results were gained by mixed-mode survey after 11.4 years, so a total of 138 patients could be reexamined after surgery. Postoperative difference in roliometre measurement was significantly improved from 10 ± 2 mm to 7 ± 2 mm. The postoperative Lysholm score was 90 ±12 points and Tegner activity score was 5 ± 2. Regarding long-term follow up, 13 % of the patients were revised after 51 ± 30 months. A re-rupture was observed in 8 % of the patients. A total of 93% of the collective regarded their knee joint as stable. Therefore, the healing-response technique seems to be a sufficient therapeutic option for partial ruptures of ACL regarding inclusion criteria and intraoperative results.

Supernumerary teeth may be encountered as an incidental finding on a radiograph. When impacted, they may be associated with clinical signs related to different problems such as failure of eruption, teeth displacement, root resorption or cystic lesions. They may occur in primary and permanent dentition, in both the maxilla and mandible and can be single or multiple in patients with syndromes. Mesiodens is the most commonly impacted tooth and appears between the central maxillary incisors in pediatric ages. Supernumerary teeth distal to the third molar are rare, usually impacted and referred to as a distomolar. A 46-year-old male consulted with the main complaint of pain on the left side of the maxilla. A panoramic radiograph revealed a right impacted maxillary fourth molar located posterior to the third molar associated with a pericoronal radiolucency. The supernumerary tooth was removed surgically under local anesthesia and the pericoronal lesion enucleated. Histopathological examination was consistent with the diagnosis of a dentigerous cyst associated with an impacted distomolar. Healing was uneventful, and the patients remained asymptomatic. The occurrence of an impacted maxillary distomolar is rare and even rarer the association with a dentigerous cyst.

Background: There is a paucity of data on mid to long-term gait outcomes after total knee arthroplasty. The aims of this longitudinal study were: to assess the evolution of both clinical and gait outcomes before and up to seven years after primary total knee arthroplasty (TKA) in a cohort of patients with knee osteoarthritis.Methods: This study included 28 patients evaluated before and up to seven years after primary TKA with both gait analysis and patient reported outcomes; of these, 20 patients were evaluated one year after surgery as well. Kinematic outcomes during gait (gait velocity, dimensionless gait veolicity, maximal knee flexion and knee range of motion), pain relief, Western Ontario and MacMaster Osteoarthritis Index (WOMAC), quality of life and patient satisfaction were assessed and compared at each visit with the paired Wilcoxon signed rank test (p < 0.05).Results: The significant improvement achieved at one year after TKA was stable up to seven years after surgery, with all clinical and kinematic outcomes unchanged, except for gait velocity, with a significant decrease over time (1.3 (1.1-1.4) m/s one year after TKA versus 1.0 (0.9-1.1) m/s, p < 0.05 up to seven years after).Conclusion: Patients with knee osteoarthritis significantly improve their clinical and kinematic outcomes at one year postoperatively and maintain the gain up to seven years after primary TKA, except for gait velocity which decreases over time, most likely along with ageing.

SUMMARY Background Free flap reconstruction is the gold standard in complex head and neck reconstruction. The branches of the external carotid vessels (ECVs) are considered the most suitable recipients, but they may be unavailable in patients presenting “frozen necks” or “vessel-depleted necks” due to previous treatments. We report our experience using the transverse cervical vessels (TCV) in these situations. Methods Retrospective chart review of microsurgical head and neck reconstructions from 2005 to 2017. We focused our analysis on secondary procedures and compared the complication rate according to whether the TCV or the ECVs were used. Results A total of 97 free flaps were performed for secondary procedures in 89 patients, mainly due to oncological recurrence and fistulae. TCV were used in 14 procedures when external carotid vessel branches were unavailable. The overall complication rate (all grade III Dindo-Clavien) was of 21% versus 35%, respectively, in the TCV and ECVs group. Grade IIIb Dindo-Clavien complications, i.e., microsurgical complications (10%) and flap loss (1%), were only recorded in the ECVs group. Other complications recorded were seroma (7% versus 1%) and hematoma (17% versus 6%) in the TCV and ECVs groups, respectively, and corresponded to grade IIIa Dindo-Claviens. Conclusions The use of TCV is a safe second-line recipient site for microsurgical head and neck reconstruction in vessel-depleted necks. Major advantages are their anatomical position outside the previous surgical and radiation zone, lower affinity for atherosclerotic damage, and similar diameter to the pedicles of the most used flaps.

International audience; To describe the 10-year outcomes of islet transplantation within the Swiss-French GRAGIL Network, in patients with type 1 diabetes experiencing high glucose variability associated with severe hypoglycemia and/or with functional kidney graft. We conducted a retrospective analysis of all subjects transplanted in the GRAGIL-1c and GARGIL-2 islet transplantation trials and analyzed components of metabolic control, graft function and safety outcomes over the 10-year period of follow-up. Forty-four patients were included between September 2003 and April 2010. Thirty-one patients completed a ten-year follow-up. Ten years after islet transplantation, median HbA1c was 7.2% [6.2-8.0] (55 mmol/mol [44-64]) vs 8.0% [7.1-9.1] (64 mmol/mol [54-76]) before transplantation (p\textless0.001). 17/23 (73.9%) recipients were free of severe hypoglycemia, 1/21 patient (4.8%) was insulin-independent and median C-peptide was 0.6 ng/ml [0.2-1.2]. Insulin requirements (UI/kg/day) were 0.3 [0.1-0.5] vs 0.5 [0.4-0.6] before transplantation (p\textless0.001). Median [IQR] β-score was 1 [0-4] (p\textless0.05 when comparing with pre-transplantation values) and 51.9% recipients had a functional islet graft at 10 years. With a 10-year follow up in a multicentric network, islet transplantation provided sustained improvement of glycemic control and was efficient to prevent severe hypoglycemia in almost 75% of the recipients.

Introduction Fracture-related infection (FRI) is a severe post-traumatic complication which is occasionally accompanied by a deficient or even avital soft-tissue envelope. In these cases, a thoroughly planned orthoplastic approach is imperative as a vital and intact soft-tissue envelope is mandatory to achieve fracture union and infection eradication. The aim of this study was, to analyse if soft-tissue reconstruction (STR) without complications is associated with a better long-term outcome compared to FRI patients with STR complications. In particular, it was investigated if primary flap failure represented a risk factor for compromised fracture union and recurrence of infection. Patients and Methods Patients with a lower leg FRI requiring STR (local, pedicled and free flaps) who were treated from 2010-18 at the University Hospital Basel were included in this retrospective analysis. The main outcome measure was the success rate of STR, further outcome measures were fracture nonunion and recurrence of infection. Results Overall, 145 patients with lower leg FRI were identified, of whom 58 (40%) received STR (muscle flaps: n��=��38, fascio-cutaneous flaps: n=19; composite osteo-cutaneous flap: n��=��1). In total seven patients required secondary STR due to primary flap failure. All failures and flap-related complications occurred within the first three weeks after surgery. Secondary STR was successful in all cases. A high Charlson Comorbidity Index Score was a significant risk factor for flap failure (p��=��0.011). Out of the 43 patients who completed the 9-month follow-up, 11 patients presented with fracture nonunion and 12 patients with a recurrent infection. Polymicrobial infection was a significant risk factor for fracture nonunion (p��=��0.002). Primary flap failure was neither a risk factor for compromised fracture consolidation (p��=��0.590) nor for recurrence of infection (p��=��0.508). Conclusion: A considerable number of patients with lower-leg FRI required STR. This patient subgroup is complex and rich in complications and the long-term composite outcome demonstrated a high rate of compromised fracture consolidation and recurrent infections. It appears that secondary STR should be performed, as primary flap failure was neither a risk factor for compromised fracture consolidation nor for recurrence of infection. We propose to monitor these patients closely for three weeks after STR.

Background: Gastrointestinal tumors are among the most common cancer types, and early detection is paramount to improve their management. Cell-free DNA (cfDNA) liquid biopsy raises significant hopes for non-invasive early detection.Aim: To describe current applications of this technology for gastrointestinal cancer detection and screening.Methods: A systematic review of the literature was performed across the PubMed database. Articles reporting the use of cfDNA liquid biopsy in the screening or diagnosis of gastrointestinal cancers were included in the analysis.Results: A total of 263 articles were screened for eligibility, of which 13 articles were included. Studies investigated colorectal cancer (5 studies), pancreatic cancer (2 studies), hepatocellular carcinoma (3 studies), and multi-cancer detection (3 studies), including gastric, oesophageal, or bile duct cancer, representing a total of 4824 patients. Test sensitivities ranged from 71% to 100%, and specificities ranged from 67.4% to 100%. Pre-cancerous lesions detection was less performant with a sensitivity of 16.9% and a 100% specificity in one study. Another study using a large biobank demonstrated a 94.9% sensitivity in detecting cancer up to 4 years before clinical symptoms, with a 61% accuracy in tissue-of-origin identification.Conclusion: cfDNA liquid biopsy seems capable of detecting gastrointestinal cancers at an early stage of development in a non-invasive and repeatable manner and screening simultaneously for multiple cancer types in a single blood sample. Aim: To compare therapeutic benefits of different immunophilin ligands for treating nerve injuries.Materials & methods: Cyclosporine, FK506 and rapamycin, were evaluated firstin vitroon a serum-free culture of embryonic dorsal root ganglia followed by a newin vivomodel of chronic nerve compression.Results: Outcomes of thein vitrostudy have shown a potent effect of cyclosporine and FK506, on dorsal root ganglia axonal outgrowth, comparable to the effect of nerve growth factor. Rapamycin exhibited only a moderate effect. Thein vivostudy revealed the beneficial effects of Cyclosporine, FK506 and rapamycin for neuromuscular regeneration. Cyclosporine showed the better maintenance of the tissues and function.Conclusion: Cyclosporine, FK506 and rapamycin drugs showed potential for treating peripheral nerve chronic compression injuries.

Inhibiting pro-inflammatory cytokine activity can reverse inflammation mediated dysfunction of islet grafts. Human amniotic epithelial cells (hAECs) possess regenerative, immunomodulatory and anti-inflammatory properties. We hypothesized that hAECs could protect islets from cellular damage induced by pro-inflammatory cytokines. To verify our hypothesis, hAEC monocultures, rat islets (RI), or RI-hAEC co-cultures where exposed to a pro-inflammatory cytokine cocktail (Interferon γ: IFN-γ, Tumor necrosis factor α: TNF-α and Interleukin-1β: IL-1β). The secretion of anti-inflammatory cytokines and gene expression changes in hAECs and viability and function of RI were evaluated. The expression of non-classical Major Histocompatibility Complex (MHC) class I molecules by hAECs cultured with various IFN-γ concentrations were assessed. Exposure to the pro-inflammatory cocktail significantly increased the secretion of the anti-inflammatory cytokines IL6, IL10 and G-CSF by hAECs, which was confirmed by upregulation of IL6, and IL10 gene expression. HLA-G, HLA-E and PDL-1 gene expression was also increased. This correlated with an upregulation of STAT1, STAT3 and NF-κB1gene expression levels. RI co-cultured with hAECs maintained normal function after cytokine exposure compared to RI cultured alone, and showed significantly lower apoptosis rate. Our results show that exposure to pro-inflammatory cytokines stimulates secretion of anti-inflammatory and immunomodulatory factors by hAECs through the JAK1/2 – STAT1/3 and the NF-κB1 pathways, which in turn protects islets against inflammation-induced damages. Integrating hAECs in islet transplants appears as a valuable strategy to achieve to inhibit inflammation mediated islet damage, prolong islet survival, improve their engraftment and achieve local immune protection allowing reducing systemic immunosuppressive regimens. Graphical Abstract This study focuses on the cytoprotective effect of isolated hAECs on islets exposed to pro-inflammatory cytokines in vitro. Exposure to pro-inflammatory cytokines stimulated secretion of anti-inflammatory and immunomodulatory factors by hAECs putatively through the JAK1/2 – STAT1/3 and the NF-κB1 pathways. This had protective effect on islets against inflammation-induced damages. Taken together our results indicate that incorporating hAECs in islet transplants could be a valuable strategy to inhibit inflammation mediated islet damage, prolong islet survival, improve their engraftment and achieve local immune protection allowing to reduce systemic immunosuppressive regimens.

We performed a retrospective review of standard anteroposterior and lateral radiographs of the thumb in 80 patients, to compare two radiographic landmarks, in terms of mediolateral bone support, for centering the trapezial component in total joint arthroplasty. On anteroposterior view, we identified the distal articular surface of the trapezium and trapezium width, and defined the two midpoints as radiographic landmarks for positioning a 9-mm trapezial cup. Mean trapezium width was significantly greater than the distal articular surface of the trapezium, and the midpoints did not match. Thus, after positioning simulated 9-mm prosthetic cups centered on each landmark, the residual radial bone distance was significantly greater using the landmark based on trapezium width. The mean value was 33% greater with this landmark, and the minimum value was 2.1 mm, compared to 0.2 mm using the landmark based on the distal articular surface. Our study thus suggested that the midpoint of the trapezium width is the more relevant radiographic landmark for centering the trapezial prosthetic cup in total joint arthroplasty, by preserving better bone stock on the radial side without depleting the ulnar side. On an intraoperative anteroposterior fluoroscopic view, this landmark could be used to check cup positioning. Nous avons effectué une revue rétrospective de radiographies standards de face et de profil du pouce chez 80 patients, afin de comparer deux repères radiographiques et d'identifier la solution la plus cohérente, d'un point de vue du support osseux médio-latéral, pour le centrage la cupule trapézienne dans l'arthroplastie totale trapézo-métacarpienne. Sur les radiographies de face, nous avons identifié la surface articulaire distale du trapèze et la largeur du trapèze. Ensuite, nous avons défini deux repères radiographiques au milieu de ces longueurs pour le positionnement d'une cupule trapézienne de 9 mm. La largeur du trapèze avait une valeur moyenne significativement plus élevée que la surface articulaire distale du trapèze, et leurs milieux ne correspondaient pas. Ainsi, après positionnement des cupules prothétiques simulées avec un diamètre de 9 mm centré sur chaque repère, la distance osseuse résiduelle radiale était significativement plus élevée en utilisant le repère basé sur la largeur du trapèze. La valeur moyenne était 33% plus élevée avec ce repère radiographique et sa valeur minimale était de 2.1 mm alors qu'elle était de 0.2 mm en utilisant le repère basé sur la surface articulaire distale. Notre étude suggère que le milieu de la largeur du trapèze est le repère radiographique le plus cohérent pour centrer la cupule prothétique trapézienne dans l'arthroplastie totale trapézo-métacarpienne, en préservant un meilleur stock osseux du côté radial sans épuiser le côté ulnaire. En effectuant une vue peropératoire de face avec la fluoroscopie, ce repère pourrait être utilisé pour vérifier le bon positionnement de la cupule.

Objective The goal of this study was to monitor the compliance and impact on a protocol change of surgical antimicrobial prophylaxis in cardiac surgery favouring cefazolin instead cefuroxime, initiated by the hospital’s antimicrobial stewardship team. Methods This quality improvement study was performed in a tertiary care hospital in collaboration with the department of cardiothoracic surgery and the hospitals antimicrobial stewardship team following a revision of the standard for surgical antimicrobial prophylaxis including 1029 patients who underwent cardiac surgery. 582 patients receiving cefuroxime and 447 patients receiving cefazolin respectively were compared without altering any other preventative perioperative measures including its postoperative duration of less than 24 h. Adherence and surgical site infections were compiled and analysed. Results A complete adherence was achieved. Overall surgical site infections occurred in 37 (3.6%) of the cases, 20 (3.4%) in cefuroxime patients and 17 (3.8%) in cefazolin patients (p value = 0.754). No statistically significant differences could be found in any of the primary endpoints, but there was a trend towards less deep sternal wound infections in the cefazolin group. Conclusions The study supports the role of antimicrobial stewardship in cardiac surgery and mirrors the success of a multidisciplinary team aiming to minimize adverse events by optimizing antibiotic use. ObjectiveExplore the impact of COVID-19 on patients on the waiting list for liver transplantation (LT) and on their post-LT course.DesignData from consecutive adult LT candidates with COVID-19 were collected across Europe in a dedicated registry and were analysed.ResultsFrom 21 February to 20 November 2020, 136 adult cases with laboratory-confirmed SARS-CoV-2 infection from 33 centres in 11 European countries were collected, with 113 having COVID-19. Thirty-seven (37/113, 32.7%) patients died after a median of 18 (10–30) days, with respiratory failure being the major cause (33/37, 89.2%). The 60-day mortality risk did not significantly change between first (35.3%, 95% CI 23.9% to 50.0%) and second (26.0%, 95% CI 16.2% to 40.2%) waves. Multivariable Cox regression analysis showed Laboratory Model for End-stage Liver Disease (Lab-MELD) score of ≥15 (Model for End-stage Liver Disease (MELD) score 15–19, HR 5.46, 95% CI 1.81 to 16.50; MELD score≥20, HR 5.24, 95% CI 1.77 to 15.55) and dyspnoea on presentation (HR 3.89, 95% CI 2.02 to 7.51) being the two negative independent factors for mortality. Twenty-six patients underwent an LT after a median time of 78.5 (IQR 44–102) days, and 25 (96%) were alive after a median follow-up of 118 days (IQR 31–170).ConclusionsIncreased mortality in LT candidates with COVID-19 (32.7%), reaching 45% in those with decompensated cirrhosis (DC) and Lab-MELD score of ≥15, was observed, with no significant difference between first and second waves of the pandemic. Respiratory failure was the major cause of death. The dismal prognosis of patients with DC supports the adoption of strict preventative measures and the urgent testing of vaccination efficacy in this population. Prior SARS-CoV-2 symptomatic infection did not affect early post-transplant survival (96%).

Background The ‘lockdown’ measures, adopted to restrict population movements in order to help curb the novel coronavirus disease 2019 (COVID-19) pandemic, contributed to a global mental health crisis. Although several studies have extensively examined the impact of lockdown measures on the psychological well-being of the general population, little is known about long-term implications. This study aimed to identify changes in psychiatric emergency department (ED) admissions between two 8-week periods: during and immediately after lifting the lockdown. Methods Socio-demographic and clinical information on 1477 psychiatric ED consultations at the University Hospital of Geneva (HUG) were retrospectively analyzed. Results When grouped according to admission dates, contrary to what we expected, the post-lockdown group presented with more severe clinical conditions (as measured using an urgency degree index) compared to their lockdown counterparts. Notably, after the lockdown had been lifted we observed a statistically significant increase in suicidal behavior and psychomotor agitation and a decrease in behavior disorder diagnoses. Furthermore, more migrants arrived at the HUG ED after the lockdown measures had been lifted. Logistic regression analysis identified diagnoses of suicidal behavior, behavioral disorders, psychomotor agitation, migrant status, involuntary admission, and private resident discharge as predictors of post-lockdown admissions. Conclusions Collectively, these findings can have implications concerning the prioritization of mental health care facilities and access for patients at risk of psychopathological decompensation in time of confinement policies, but above all, provide a foundation for future studies focusing on the long-term impact of the pandemic and its associated sanitary measures on mental health. Busulfan (Bu) is a common component of conditioning regimens before hematopoietic stem cell transplantation (HSCT) and is known for high interpatient pharmacokinetic (PK) variability. This study aimed to develop and externally validate a multicentric, population PK (PopPK) model for intravenous Bu in pediatric patients before HSCT to first study the influence of glutathione‐s‐transferase A1 (GSTA1) polymorphisms on Bu's PK in a large multicentric pediatric population while accounting for fludarabine (Flu) coadministration and, second, to establish an individualized, model‐based, first‐dose recommendation for intravenous Bu that can be widely used in pediatric patients. The model was built using data from 302 patients from five transplantation centers who received a Bu‐based conditioning regimen. External model validation used data from 100 patients. The relationship between body weight and Bu clearance (CL) was best described by an age‐dependent allometric scaling of a body weight model. A stepwise covariate analysis identified Day 1 of Bu conditioning, GSTA1 metabolic groups based on GSTA1 polymorphisms, and Flu coadministration as significant covariates influencing Bu CL. The final model adequately predicted Bu first‐dose CL in the external cohort, with 81% of predicted area under the curves within the therapeutic window. The final model showed minimal bias (mean prediction error, −0.5%; 95% confidence interval [CI], −3.1% to 2.0%) and acceptable precision (mean absolute prediction error percentage, 18.7%; 95% CI, 17.0%–20.5%) in Bu CL prediction for dosing. This multicentric PopPK study confirmed the influence of GSTA1 polymorphisms and Flu coadministration on Bu CL. The developed model accurately predicted Bu CL and first doses in an external cohort of pediatric patients.

Background and purpose — Prior to primary total knee arthroplasty (pTKA), 6–34% of patients have undergone surgical procedure(s) of their knee. We investigated whether history of previous surgeries influences the risk of revision of pTKA, the risk according to the type of previous surgery, and how previous surgery influences specific causes of revision and the time of revision. Patients and methods — This is a prospective cohort study from the Geneva Arthroplasty Registry. All pTKA between 2000 and 2016 were included and followed until December 31, 2019. Outcomes were risk of revision, evaluated using Kaplan–Meier survival and Cox and competing risks regression, the specific causes, and time of revision. Results — Of 3,945 pTKA included (mean age 71 years, 68% women), 21% had a history of previous surgery, with 8.3% revisions vs. 4.3%, at 3–20 years’ follow-up (mean 8.6). 5- and 10-year cumulative failure by previous surgery (yes vs. no) were 6.6% (95% CI 5.1–8.5) vs. 3.3% (CI 2.7–4.0), and 8.4% (CI 6.6–10.6) vs. 4.5% (CI 3.8–5.4). Baseline differences explained only part of the higher risk (adjusted HR 1.5, CI 1.1–2.1). The risk of failure was higher for all causes of revision considered. Patients in the previous surgery group had a higher risk of an early revision. Interpretation — A history of previous surgery adversely affected the outcome with a 1.5 times higher cumulative risk of all-cause revision over the course of up to 20 years after index surgery. The increased risk was seen for all causes of revision and was highest in the first years.

OBJECTIVES: First, establishment and validation of a novel questionnaire documenting the burden of xerostomia and sialadenitis symptoms, including quality of life. Second, to compare two versions regarding the answering scale (proposed developed answers Q3 vs. 0-10 visual analogue scale Q10) of our newly developed questionnaire, in order to evaluate their comprehension by patients and their reproducibility in time. STUDY DESIGN: The study is a systematic review regarding the evaluation of the existing questionnaire and a cohort study regarding the validation of our new MSGS questionnaire. MATERIALS AND METHODS: A Multidisciplinary Salivary Gland Society (MSGS) questionnaire consisting of 20 questions and two scoring systems was developed to quantify symptoms of dry mouth and sialadenitis. Validation of the questionnaire was carried out on 199 patients with salivary pathologies (digestive, nasal, or age-related xerostomia, post radiation therapy, post radioiodine therapy, Sjögren's syndrome, IgG4 disease, recurrent juvenile parotitis, stones, and strictures) and a control group of 66 healthy volunteers. The coherence of the questionnaire's items, its reliability to distinguish patients from healthy volunteers, its comparison with unstimulated sialometry, and the time to fill both versions were assessed. RESULTS: The novel MSGS questionnaire showed good internal coherence of the items, indicating its pertinence: the scale reliability coefficients amounted to a Cronbach's alpha of 0.92 for Q10 and 0.90 for Q3. The time to complete Q3 and Q10 amounted, respectively, to 5.23 min (±2.3 min) and 5.65 min (±2.64 min) for patients and to 3.94 min (±3.94 min) and 3.75 min (±2.11 min) for healthy volunteers. The difference between Q3 and Q10 was not significant. CONCLUSION: We present a novel self-administered questionnaire quantifying xerostomia and non-tumoral salivary gland pathologies. We recommend the use of the Q10 version, as its scale type is well known in the literature and it translation for international use will be more accurate. A 47- year- old female patient with end- stage renal disease secondary to type 1 diabetes underwent a simultaneous pancreas– kidney (SPK) transplantation in 2016, 9 years after a living- donor kidney trans-plantation. The first kidney graft, implanted extraperitoneally in the left iliac fossa, was not removed during SPK. SPK was performed intraperitoneally with the second kidney implanted on the left com-mon iliac artery and the pancreas on the right, with portal- enteric drainage.

Hematoma formation and the need for blood transfusions are commonly reported complications after shoulder arthroplasty. Tranexamic acid (TXA) has been widely used in hip and knee arthroplasty to decrease perioperative blood loss. The role of TXA is still being established in shoulder arthroplasty.We conducted a double-blind randomized controlled trial comparing intravenous TXA vs. placebo in 60 patients undergoing primary anatomic or reverse shoulder arthroplasty. Of these patients, 29 received a placebo whereas 31 received a single dose of 2 g of intravenous TXA. Patient demographic characteristics, as well as drain tube output, blood loss, hematoma formation, transfusion requirement, length of hospital stay, and pain score, were recorded. Patients were followed up for 12 weeks to assess for complications.Patients who received TXA had a lower drain tube output at all time points: 41 mL vs. 133 mL at 6 hours, 75 mL vs. 179 mL at 12 hours, and 94 mL vs. 226 mL at 24 hours (P.001 for all). They also had a higher postoperative hemoglobin (Hb) level (12.3 g/dL vs. 11.4 g/dL, P = .009), lower change in Hb level (1.7 g/dL vs. 2.3 g/dL, P = .011), lower total Hb loss (0.078 g vs. 0.103 g, P = .042), lower blood volume loss (0.55 L vs. 0.74 L, P = .021), higher postoperative hematocrit level (36.7% vs. 34.6%, P = .020), and lower hematocrit change (5.4% vs. 7.6%, P = .022). There was no significant difference in pain score or length of hospital stay, and no patients required a transfusion.A single dose of 2 g of intravenous TXA decreases blood loss and drain tube output in primary anatomic and reverse arthroplasty of the shoulder. No differences were detected in the occurrence of complications, need for transfusion, pain score, or length of hospital stay. With the mounting evidence now available, patients undergoing elective primary shoulder arthroplasty should be given intravenous TXA to decrease perioperative blood loss.

The calcaneal beak fracture is a rare avulsion fracture of the tuber calcanei characterized by a solid bony fragment at the Achilles tendon insertion. Treatment usually requires osteosynthesis. However, lack of biomechanical understanding of the ideal fixation technique persists. A beak fracture was simulated in synthetic bones and assigned to five different groups of fixation: A) 6.5-mm partial threaded cannulated screws, B) 4.0-mm partial threaded cannulated screws, C) 5.0-mm headless cannulated compression screws, D) 2.3-mm locking plate, and E) 2.8-mm locking plate. Different traction force levels were applied through an Achilles tendon surrogate in a material-testing machine on all stabilized synthetic bones. Outcome measures were peak-to-peak displacement, total displacement, plastic deformation, stiffness, visual-fracture-line displacement, and mode of implant failure. The 2.3- and 2.8-mm plating groups showed a high drop-out rate at 100 N tension force and failed under higher tension levels of 200 N. The fracture fixation using 4.0-mm partial threaded screws showed a significantly higher repair strength and was able to withhold cyclic loading up to 300 N. The lowest peak-to-peak displacement and the highest load-to-failure and stiffness were provided by fracture fixation using 6.5-mm partial threaded cannulated screws or 5.0-mm headless cannulated compression screws. As anticipated, large 6.5-mm screw diameters provide the best biomechanical fixation. Surprisingly, the 5.0-mm headless cannulated compression screws yield reliable stability despite the absent screw head and washer. When such large screws cannot be applied, 4.0-mm screws also allow reasonable fixation strength. Plate fixation should be implemented with precaution and in combination with a restrictive postoperative motion protocol. Finally, clinical cases about the surgical application and recovery are included.

Background:. Various prediction tools have been developed to predict biochemical recurrence (BCR) after radical prostatectomy (RP); however, few of the previous prediction tools used serum prostate-specific antigen (PSA) nadir after RP and maximum tumor diameter (MTD) at the same time. In this study, a nomogram incorporating MTD and PSA nadir was developed to predict BCR-free survival (BCRFS). Methods:. A total of 337 patients who underwent RP between January 2010 and March 2017 were retrospectively enrolled in this study. The maximum diameter of the index lesion was measured on magnetic resonance imaging (MRI). Cox regression analysis was performed to evaluate independent predictors of BCR. A nomogram was subsequently developed for the prediction of BCRFS at 3 and 5 years after RP. Time-dependent receiver operating characteristic (ROC) curve and decision curve analyses were performed to identify the advantage of the new nomogram in comparison with the cancer of the prostate risk assessment post-surgical (CAPRA-S) score. Background Solid organ transplantation (SOT) candidates and recipients are highly vulnerable to invasive pneumococcal diseases (IPD). Data on which to base optimal immunization recommendations for this population is scant. The national distribution of IPD serotypes led the Swiss Health Authorities to recommend in 2014 one dose of pneumococcal-13-valent-conjugate-vaccine (PCV13), without any subsequent dose of the 23-valent-polysaccharide-pneumococcal-vaccine (PPV23). Methods This is a retrospective analysis of pneumococcal immunity using a multiplex binding assay, to assess seroprotection rates against a selection of seven PCV13- and seven PPV23-serotypes in SOT-candidates and recipients evaluated and/or transplanted in 2014/2015 in the University Hospitals of Geneva. Seroprotection was defined as serotype-specific antibody concentration greater than 0.5 mg/l and overall seroprotection when this was achieved for ≥ 6/7 serotypes. Results Pre-vaccination and at time of transplant sera were available for 35/43 (81%), and 43/43 (100%) SOT-candidates respectively. At listing, 17/35 (49%) SOT-candidates were seroprotected against PCV13 and 21/35 (60%) against PPV23 serotypes. Following one systematic dose of PCV13 at listing, 35/43 (81%) SOT-recipients were seroprotected at day of transplant against PCV13-serotypes and 34/43 (79%) against PPV23 serotypes, compared to 21/41 (51%) and 28/41 (68%) respectively in the controls transplanted in 2013, before the systematic PCV13-vaccination. Conclusions The systematic vaccination with PCV13 of all SOT candidates without additional PPV23 is a good strategy as it confers seroprotection against a wide range of pneumococcal serotypes. Indeed, one of five PCV13-vaccinated SOT-candidates was nevertheless not seroprotected at time of transplant, reflecting their partial immune competence, and indicating the need for additional dose of pneumococcal vaccines before transplant.
