To evaluate treatment effects in Class II patients using infrazygomatic crest (IZC) miniscrews (MS). A prospective sample of 25 adolescents (14 females and 11 males; mean age: 13.6 ± 1.5 years) who underwent maxillary dentition distalization treatment with IZC MSs were recruited. Lateral cephalograms and digital models at the beginning of treatment (T1) and after Class II molar correction (T2) were obtained. To compare cephalometric and digital model changes, paired t-test and Wilcoxon test were used. A significance level of 5% was used. All patients achieved Class II molar correction over a mean period of 7.7 ± 2.5 months. The IZC MS therapy provided 4 mm of distalization; there was 1.2 mm of intrusion of the first molar with 11.2° distal tipping. The maxillary incisors were retracted 4.7 mm and tipped lingually 13.4°. Overjet and overbite showed a reduction of 3.6 mm and 2.4 mm, respectively. The occlusal plane rotated clockwise 2.8°. The upper lip was retracted by 1 mm and the nasolabial angle increased 5.1°. There was an increase in the interpremolar and intermolar distances. Total arch distalization of the maxillary dentition using IZC MS was effective in the treatment of Class II malocclusions. Objective: This randomized clinical trial aimed to compare the pain intensity in patients treated with orthodontic aligners and conventional fixed appliances.Setting and Sample Population: This study was a randomized clinical trial. The sample comprised 39 patients randomly allocated into 2 groups: OA (orthodontic aligners, n = 20) and FA (Fixed Appliance, n = 19).Material and Methods: The pain intensity was measured by the visual analogue scale (VAS) in the following periods: T0 (baseline), T1 (seven days after appliance placement) and seven days after each return on the first (T2), third (T3) and sixth (T4) months. The following variables were also investigated in the baseline: conditioned pain modulation, anxiety levels, hypervigilance and catastrophizing. The VAS measurements between groups were compared by the Mann-Whitney test. Comparisons between periods within each group were performed by the Friedman test. Data regarding catastrophizing and hypervigilance were compared by the t test. All tests were applied at a significance level of 5%, with 95% confidence interval.Results: Both groups presented similar levels of anxiety, hypervigilance, catastrophizing and conditioned pain modulation. Both groups did not differ concerning the pain intensity in all periods. The intragroup evaluation revealed statistical differences between days in the FA group at all moments evaluated, for the OA group, similar findings between days were found for the T1 evaluation; however, at the 6-month period (T4), the pain levels varied over these days without statistical difference. Higher levels of pain were observed in the first seven days after appliance placement.Conclusion: The pain intensity, usually mild, was not influenced by the appliance design, although different patterns of reported pain seem to occur between groups. Aim: This controlled trial cephalometrically compared soft profile changes during orthodontic treatment with a banded Herbst appliance against a control group. Methods: The sample consisted of late mixed dentition Class II division 1 malocclusion cases. The first group consisted of 29 consecutive patients (14 females and 15 males), selected prospectively and treated solely with a banded Herbst appliance; the second group consisted of 28 nontreated subjects (14 females and 14 males). Two lateral cephalograms were obtained from each patient at the beginning and immediately following the completion of the treatment/observation period (1 year). Soft tissue cephalometric variables were analyzed. Different t tests were used to compare the starting forms of the 2 groups and the changes that occurred after treatment (P < .05). Results: The present study showed a modest decrease in the facial convexity, retrusion of the upper lip, and an improvement in the mentolabial angle. All these changes were statistically significant. The profile improvement in the Herbst group was likely because of the changes observed in the upper lip and, to a minor degree, in the lower lip and soft tissue pogonion. Conclusions: It can be concluded that the changes in the soft tissue profile produced by a banded-type Herbst appliance were statistically different from the control group, but not likely clinically significant. Objectives: To evaluate the influence of text messages on cooperation of Class II patients with the use of intermaxillary elastics. Materials and Methods: The sample consisted of 42 orthodontic patients (20 males and 22 females) aged between 14 and 34 years. They were randomly divided into one of the following two groups: a control group with 21 patients who did not receive messages and an experimental group with 21 patients who received motivational and reminder text messages. Messages were sent twice a week for a period of 3 months. The patients were instructed to wear the elastics all day, removing them only during meals and replacing them daily. All patients were instructed regarding the importance of cooperation. Measurements were performed with a digital caliper on plaster models at the beginning of elastics wear (T1) and 3 months later (T2). For intragroup and intergroup comparisons between T1 and T2, paired and unpaired t-tests, respectively, were used with a significance level of 5%. Results: Statistically significant differences were observed in the intra- and intergroup comparisons between T1 and T2. Both groups showed a decrease in the sagittal distance between upper and lower arches from T1 to T2, demonstrating the effective use of elastics. However, the experimental group showed a Class II correction that was 3.7 times greater than the control group (P = .001). Conclusions: Text messages had a positive influence on the cooperation of patients regarding the use of intermaxillary elastics in the orthodontic treatment of Class II malocclusion. Objectives: To assess reliability and reproducibility of the individual assessment of midpalatal suture maturation in computed tomography among orthodontists and radiologists for potential diagnosis application. Materials and Methods: Sixty axial slices from cone-beam computed tomography and multi-slice CT scans of patients aged between 11 and 21 years old (33 females and 27 males) were selected. For the investigation of reliability and reproducibility of the method, two groups of examiners were established. The first group consisted of 11 orthodontists and the second consisted of 10 radiologists. Each group examined the images and performed individual assessment of the midpalatal suture maturation method twice within an interval of 21 days. During the first and second analyses, the sequence of images was randomized to reduce potential bias. Weighted Cohen's kappa was performed to assess inter- and intra-examiners' agreement. The percentage of perfect agreement and the number of stages apart for each disagreement were calculated. The significance level was P < .05. Results: The overall inter-examiner agreement was satisfactory in the first (kappaw: 0.37) and the second (kappaw: 0.34) analyses. Intra-examiner agreement outcomes were similar between orthodontists (kappaw: 0.44) and radiologists (kappaw: 0.41). The percentage of perfect agreement was 43.2%. Conclusions: The method for individual assessment of midpalatal suture maturation revealed potential reliability and reproducibility. Objective: Evaluate the facial profile changes of orthodontic treatment with extraction of two upper first premo-lars, from the perspective of orthodontists, dentists and lay people. Methods: Facial profiles of radiographs taken before and after treatment of 70 patients with Class II, division 1 malocclusion were traced. The silhouettes of the 70 patients were randomly assembled in an album with, being two profiles on each sheet of the same patient. Then, 30 orthodontists, 30 dentists and 30 lay people chose the more esthetic facial profile (A or B), and the amount of change they perceived between the two profiles before and after treatment, according to a visual analog scale (VAS). Results: The results revealed that 83 examiners preferred the post-treatment profiles, and only three dentists and four lay people chose the profiles pre-treatment more frequently. Thus, the orthodontists often chose the profiles after treatment, followed by dentists, with no statistically significant differences found between dentists and lay people. There were significant differences within groups in the preference of pre- and post-treatment profile. Furthermore, the three groups of evaluators indicated that pre and post-treatment profiles did not differ substantially. Conclusions: The treatment of Class II, division 1 malocclusion with extraction of two first premolars has a positive effect on facial profile esthetics. [ABSTRACT FROM AUTHOR]

Objectives: To determine the prevalence of malocclusion in a group of 3,466 children aged 7 to 12 years enrolled in public schools in the cities of Lins and Promissão, in the state of São Paulo, Brazil. Methods: The sagittal relationships between dental arches, the transverse relationship between arches, and the vertical and horizontal relations of incisors were analyzed. The prevalence of diastemas, crowding and tooth losses were evaluated. Results: Among the types of malocclusion, 55.25% of the children had a Class I molar relationship, 38%, Class II, and 6.75%, Class III. The analysis of incisor relationships revealed 17.65% of open bite, followed by 13.28% of deep bite and 5.05% of anterior crossbite; 13.3% of the children had a posterior crossbite. The analysis of relationships between arches showed that 31.88% of the children had diastemas, 31.59%, crowding, and 4.65%, tooth losses. [ABSTRACT FROM AUTHOR]

Introduction: Obstructive sleep apnea and/or hypopnea syndrome (OSAHS) is characterized by repeated obstruction of the airway or reduction of breathing during sleep. This syndrome is caused by pharyngeal collapse against increased respiratory effort during sleep. SAHOS leads to deterioration of sleep quality associated with clinical complaints of excessive daytime sleepiness and unfavorable effects on the cardiovascular system. Orofacial features such as mandibular retrognathism and/or associated maxillary retrognathism, Class II malocclusion, with or without open bite and reduction of the oropharyngeal airway characterize this syndrome. Objective: It is reported an orthosurgical treatment of a 31 years old male patient, with OSAHS characteristics, Class II malocclusion, mandibular deficiency, posterior crossbite and anterior open bite, lower anterior facial height decrease, deepening of the labial sulcus, little exposure of the lower lip and unbalanced face. Report and Results: Non-invasive treatments for OSAHS pass through positive continuous nasal pressure devices in the airways or intra-oral appliances in order to maintain the mandible in an anterior position during sleep. In the invasive treatments protocol, mandibular advancement surgery prevents rotation in the anti-clockwise direction of the maxillomandibular complex with an increase in the oropharyngeal space of the middle and lower airways. In cases with great skeletal dentofacial impact associated with the degree of obstruction of the airway will be necessary an ortho-surgical decompensatory treatment, to allow a respiratory and aesthetic restoration of the face. Conclusion: The decompensatory treatment with bimaxillary surgical advancement corrected the malocclusion, increasing the air space contemplating the respiratory, functional and aesthetic expectation. Introduction: synthetic intermaxillary elastics have emerged as an alternative for orthodontic use in patients with latex sensitivity, but differences can be observed when compared to natural latex elastics. Objective: compare the amount of force degradation of synthetic intermaxillary elastics and latex when distended in artificial saliva in order to find clinical parameters for the use of a protocol on the frequency of exchange of these elastics in orthodontic therapies in patients with latex sensitivity or not. Method: for this, two groups containing 30 elastics were used. Group 1 with 30 medium strength and diameter 5/16 "Morelli latex elastics and group 2, 30 synthetic latex-free elastics, also of the medium strength and 5/16" diameter Morelli brand. They were stretched 4cm and submerged in artificial saliva at 37 degrees Celsius and had strength measured with orthodontic dynamometer at the beginning and at the intervals of 24, 48, 72 and 120 hours after immersion. The force degradation assessment was performed by calculating the percentage of force lost in relation to the initial force at each time interval, for each group tested separately and also between them. Results and discussion: the mean elastic values of both silicone and latex had a similar percentage for the degradation of forces, since after the first 24 hours the greatest force decline occurred, of approximately 11% both elastics. After 48 hours this percentage did not change significantly. Conclusion: elastics should be changed within 48 hours so that the force applied remains at acceptable rates. [ABSTRACT FROM AUTHOR]

This study analyzed the impact of the Covid-19 pandemic on awake bruxism (BV) behavior in patients undergoing orthodontic treatment. The sample consisted of 31 patients aged between 15 and 30 years old. The frequency of BV was evaluated before and during the pandemic, along with psychosocial variables such as stress level and anxiety. The results showed that there was no significant difference in the frequency of BV between the two periods, but levels of hypervigilance and catastrophizing decreased during the pandemic, while the level of concentration was low. It was concluded that the pandemic did not have a significant impact on awake bruxism habits, but it did affect the patients' concentration. [Extracted from the article]

Os elásticos correntes são usados no fechamento de espaços entre os dentes, por isso devem liberar forças suaves e contínuas para obter um maior controle sobre os movimentos dentários. Apesar da sua grande utilização, existem desvantagens como a degradação da força interferindo na movimentação dentária e na efetividade do tratamento. O objetivo deste projeto é comparar a quantidade de força inicial e sua degradação de 4 diferentes marcas comerciais de elásticos corrente estéticos. Foram avaliados 10 elásticos corrente estético de força média com 6 elos das marcas Morelli, Orthometric, Eurodonto e 3M. Os elásticos foram distendidos o dobro de seu comprimento e submersos em saliva artificial. Dessa forma, foram mensuradas as força com dinamômetro ortodôntico ao início e nos intervalos de 1, 7, 14, 21 e 30 dias após a imersão. A degradação da força foi realizada calculando o percentual de força perdida em relação à força inicial em cada intervalo de tempo, para cada grupo testado separadamente e também entre eles. A avaliação da alteração de cor foi observada por meio de fotografias dos elásticos antes e após o término do experimento. Foi adotado o teste ANOVA de medidas repetidas, tempo e grupos e para as análises de múltiplas comparações utilizou- se o teste de Tukey. Após 7 dias os grupos degradaram em torno de 50% da força inicial. Na avaliação da alteração de cor, os elásticos da marca Eurodonto apresentaram menores alterações de cor. Todas as marcas comerciais apresentaram manchamento, com os melhores resultados para os elásticos da Eurodonto. [ABSTRACT FROM AUTHOR]

Objective: This study aimed at assessing the relationship between facial morphological patterns (I, II, III, Long Face and Short Face) as well as facial types (brachyfacial, mesofacial and dolichofacial) and obstructive sleep apnea (OSA) in patients attending a center specialized in sleep disorders., Methods: Frontal, lateral and smile photographs of 252 patients (157 men and 95 women), randomly selected from a polysomnography clinic, with mean age of 40.62 years, were evaluated. In order to obtain diagnosis of facial morphology, the sample was sent to three professors of Orthodontics trained to classify patients' face according to five patterns, as follows: 1) Pattern I; 2) Pattern II; 3) Pattern III; 4) Long facial pattern; 5) Short facial pattern. Intraexaminer agreement was assessed by means of Kappa index. The professors ranked patients' facial type based on a facial index that considers the proportion between facial width and height., Results: The multiple linear regression model evinced that, when compared to Pattern I, Pattern II had the apnea and hypopnea index (AHI) worsened in 6.98 episodes. However, when Pattern II was compared to Pattern III patients, the index for the latter was 11.45 episodes lower. As for the facial type, brachyfacial patients had a mean AHI of 22.34, while dolichofacial patients had a significantly statistical lower index of 10.52., Conclusion: Patients' facial morphology influences OSA. Pattern II and brachyfacial patients had greater AHI, while Pattern III patients showed a lower index.
