11. Can transcranial direct current stimulation (tDCS) over the motor cortex increase endurance running performance? a randomized crossover-controlled trial.

Abstract

Transcranial direct current stimulation (tDCS), a non-invasive brain stimulation technique, has been shown to increase exercise performance in strength and cycling studies but its effects on running endurance remain unclear. The objectives of this randomized sham-controlled crossover trial were to assess tDCS efficacy on submaximal treadmill running time to exhaustion (TTE). Forty-five healthy male runners aged between 18 and 32 years (mean maximal oxygen consumption: 46.6 mL/min/kg) performed two constant-load tests at 90% of their maximal aerobic speed preceded by 20 minutes of active or sham multichannel (5 anodes, 3 cathodes) tDCS applied over the bilateral motor cortex with a total intensity of 4 mA. Ratings of perceived exertion (RPE), blood lactate, VO2, and heart rate were monitored every five minutes until volitional exhaustion. The median [IQR] TTE was similar following active (12.2 [10.5, 16.1] minutes) or sham (12.5 [10.2, 15.1] minutes) tDCS (p = 0.96). Likewise, there were no significant differences between active and sham conditions for RPE, blood lactate, final VO2, and final heart rate (all p ≥0.05). No difference in TTE was found when stratifying groups according to their VO2max (i.e., VO2max ≥ 45 mL/min/Kg, p = 0.53; VO2max < 45 mL/min/Kg, p = 0.45) but there was a trend for a significant correlation between VO2max and change in TTE (p = 0.06). TDCS applied over the bilateral motor cortex did not improve endurance performance in a large sample of trained runners. Characterization of individual tDCS responsiveness deserves further consideration. In our experimental conditions, tDCS had no ergogenic effect on endurance running performance. Abstract

The cardiac response to physical exertion is linked to factors such as age, work intensity, and fitness levels. The primary objective of this study was to characterize within-session changes in cardiac response to running in Canadian football athletes, which may indicate physical fatigue. Performance profiles were collected from GPS and heart rate (HR) sensors worn by 30 male Canadian varsity football players (20–26 years old) over 28 games and practices. Running efforts with 60–180 s of rest were detected, and the maximum HR (HRmax) and peak HR recovery (HRRpk) during rest were extracted. Additionally, a new metric of cardiovascular cost (CVC) was developed to reflect the efficiency of the HR response to physical workload. HRmax was higher in games (p < 0.001) and in linemen (p < 0.001), and it increased over time (p < 0.001). HRRpk was higher in skilled players (p < 0.001) and changed over time (p < 0.001) depending on the rest period. CVC was higher in linemen (p < 0.001) and increased over time (p < 0.001). This study demonstrated the utility of HR response metrics to quantify ongoing fatigue experienced by Canadian football athletes and proposed a novel fatigue metric capable of monitoring an athlete's fatigue state in real time. Abstract

Objective: Temporomandibular disorders (TMD) are characterised by chronic pain and dysfunction in the jaw joint and masticatory muscles. Repetitive transcranial magnetic stimulation (rTMS) has emerged as a potential non‐invasive treatment for chronic pain; however, its effectiveness in individuals with TMD has not been thoroughly investigated. This study aimed to evaluate the immediate and sustained (over seven consecutive days) effects of a single session of active rTMS compared to sham stimulation on pain intensity and pain unpleasantness in individuals with TMD. Methods: A randomised, double‐blind, sham‐controlled trial enrolled 41 female participants with chronic TMD. Pain intensity and pain unpleasantness were assessed immediately pre‐ and post‐intervention, as well as twice daily for 21 days using electronic diaries. Secondary outcomes included pain interference, sleep quality, positive and negative affect and pain catastrophizing. Adverse effects were monitored. Repeated measures ANOVA and multilevel modelling regression analyses were employed for data analysis. Result: Active rTMS demonstrated a significant immediate mild reduction in pain intensity and pain unpleasantness compared to sham stimulation. However, these effects were not sustained over the 7‐day post‐intervention period. No significant differences were observed between interventions for pain interference, sleep quality and negative affect. A minority of participants reported minor and transient side effects, including headaches and fatigue. Conclusion: A single session of active rTMS was safe and led to immediate mild analgesic effects in individuals with TMD compared to sham stimulation. However, no significant differences were observed between interventions over the 7‐day post‐intervention period. Based on this study, rTMS stimulation appears to be a promising safe approach to be tested in TMD patients with longer stimulation protocols. [ABSTRACT FROM AUTHOR]

Abstract

This study aimed to use a data-driven approach to identify individualized speed thresholds to characterize running demands and athlete workload during games and practices in skill and linemen football players. Data were recorded from wearable sensors over 28 sessions from 30 male Canadian varsity football athletes, resulting in a total of 287 performances analyzed, including 137 games and 150 practices, using a global positioning system. Speed zones were identified for each performance by fitting a 5-dimensional Gaussian mixture model (GMM) corresponding to 5 running intensity zones from minimal (zone 1) to maximal (zone 5). Skill players had significantly higher (p < 0.001) speed thresholds, percentage of time spent, and distance covered in maximal intensity zones compared to linemen. The distance covered in game settings was significantly higher (p < 0.001) compared to practices. This study highlighted the use of individualized speed thresholds to determine running intensity and athlete workloads for American and Canadian football athletes, as well as compare running performances between practice and game scenarios. This approach can be used to monitor physical workload in athletes with respect to their tactical positions during practices and games, and to ensure that athletes are adequately trained to meet in-game physical demands. 33. Long-Term Effects of Concussions on Psychomotor Speed and Cognitive Control Processes During Motor Sequence Learning

Abstract.In asymptomatic multiple-concussion athletes, studies evidenced long-term impairments in psychomotor speed, motor sequence learning, and cognitive control processes, as indexed by the Error Negativity (Ne), also commonly referred to as the Error-related Negativity (ERN). In healthy controls, motor sequence learning during a Serial Reaction Time (SRT) task is associated with an increase in Ne/ERN amplitude. The objective of this paper is to investigate whether concussion effects on cognitive control are associated with sequence learning changes in asymptomatic multi-concussion athletes. Thirty-seven athletes (18 nonconcussed; 19 concussed) completed a SRT task during which continuous electroencephalographic (EEG) activity was recorded. Ne/ERN amplitude modulation from early to late learning blocks of the task was measured. Median reaction times (RTs) were computed to assess psychomotor speed and motor sequence learning. Psychomotor speed was significantly reduced in concussed athletes. Accentuated Ne/ERN amplitude from early to late learning blocks significantly correlated with motor sequence learning in nonconcussed athletes. In contrast, Ne/ERN amplitude was found to decrease significantly with task progression in concussed athletes who nonetheless achieved normal motor sequence learning. Multiple concussions detrimentally affect psychomotor speed. Unlike nonconcussed athletes, motor sequence learning in multi-concussion athletes was not associated with Ne/ERN amplitude modulation, indicating that cognitive control processes do not centrally contribute to learning of a motor sequence after repeated concussions.
