Abstract: The CARMENES instrument was conceived to deliver high-accuracy radial velocity (RV) measurements with long-term stability to search for temperate rocky planets around a sample of nearby cool stars. The broad wavelength coverage was designed to provide a range of stellar activity indicators to assess the nature of potential RV signals and to provide valuable spectral information to help characterise the stellar targets. The CARMENES Data Release 1 (DR1) makes public all observations obtained during the CARMENES guaranteed time observations, which ran from 2016 to 2020 and collected 19,633 spectra for a sample of 362 targets. The CARMENES survey target selection was aimed at minimising biases, and about 70% of all known M dwarfs within 10 pc and accessible from Calar Alto were included. The data were pipeline-processed, and high-level data products, including 18,642 precise RVs for 345 targets, were derived. Time series data of spectroscopic activity indicators were also obtained. We discuss the characteristics of the CARMENES data, the statistical properties of the stellar sample, and the spectroscopic measurements. We show examples of the use of CARMENES data and provide a contextual view of the exoplanet population revealed by the survey, including 33 new planets, 17 re-analysed planets, and 26 confirmed planets from transiting candidate follow-up. A subsample of 238 targets was used to derive updated planet occurrence rates, yielding an overall average of 1.44+/-0.20 planets with 1 M_Earth < M sin i < 1000 M_Earth and 1 d < P_orb < 1000 d per star, and indicating that nearly every M dwarf hosts at least one planet. Abstract: Statistical analyses from exoplanet surveys around low-mass stars indicate that super-Earth and Neptune-mass planets are more frequent than gas giants around such stars, in agreement with core accretion theory of planet formation. Using precise radial velocities derived from visual and near-infrared spectra, we report the discovery of a giant planet with a minimum mass of 0.46 Jupiter masses in an eccentric 204-day orbit around the very low-mass star GJ 3512. Dynamical models show that the high eccentricity of the orbit is most likely explained from planet-planet interactions. The reported planetary system challenges current formation theories and puts stringent constraints on the accretion and migration rates of planet formation and evolution models, indicating that disc instability may be more efficient in forming planets than previously thought., Comment: Manuscript author version. 41 pages, 11 figures

Abstract: Context. Teegarden's Star is the brightest and one of the nearest ultra-cool dwarfs in the solar neighbourhood. For its late spectral type (M7.0V), the star shows relatively little activity and is a prime target for near-infrared radial velocity surveys such as CARMENES. Aims. As part of the CARMENES search for exoplanets around M dwarfs, we obtained more than 200 radial-velocity measurements of Teegarden's Star and analysed them for planetary signals. Methods. We find periodic variability in the radial velocities of Teegarden's Star. We also studied photometric measurements to rule out stellar brightness variations mimicking planetary signals. Results. We find evidence for two planet candidates, each with $1.1M_\oplus$ minimum mass, orbiting at periods of 4.91 and 11.4 d, respectively. No evidence for planetary transits could be found in archival and follow-up photometry. Small photometric variability is suggestive of slow rotation and old age. Conclusions. The two planets are among the lowest-mass planets discovered so far, and they are the first Earth-mass planets around an ultra-cool dwarf for which the masses have been determined using radial velocities., Comment: A&A 627, A49. 26 pages, 17 figures, 6 tables. Abstract: We report on the first star discovered to host a planet detected by radial velocity (RV) observations obtained within the CARMENES survey for exoplanets around M dwarfs. HD 147379 ($V = 8.9$ mag, $M = 0.58 \pm 0.08$ M$_{\odot}$), a bright M0.0V star at a distance of 10.7 pc, is found to undergo periodic RV variations with a semi-amplitude of $K = 5.1\pm0.4$ m s$^{-1}$ and a period of $P = 86.54\pm0.06$ d. The RV signal is found in our CARMENES data, which were taken between 2016 and 2017, and is supported by HIRES/Keck observations that were obtained since 2000. The RV variations are interpreted as resulting from a planet of minimum mass $m_{\rm p}\sin{i} = 25 \pm 2$ M$_{\oplus}$, 1.5 times the mass of Neptune, with an orbital semi-major axis $a = 0.32$ au and low eccentricity ($e < 0.13$). HD 147379b is orbiting inside the temperate zone around the star, where water could exist in liquid form. The RV time-series and various spectroscopic indicators show additional hints of variations at an approximate period of 21.1d (and its first harmonic), which we attribute to the rotation period of the star., Comment: accepted for publication as A&A Letter

Abstract: The CARMENES radial velocity (RV) survey is observing 324 M dwarfs to search for any orbiting planets. In this paper, we present the survey sample by publishing one CARMENES spectrum for each M dwarf. These spectra cover the wavelength range 520--1710nm at a resolution of at least $R > 80,000$, and we measure its RV, H$\alpha$ emission, and projected rotation velocity. We present an atlas of high-resolution M-dwarf spectra and compare the spectra to atmospheric models. To quantify the RV precision that can be achieved in low-mass stars over the CARMENES wavelength range, we analyze our empirical information on the RV precision from more than 6500 observations. We compare our high-resolution M-dwarf spectra to atmospheric models where we determine the spectroscopic RV information content, $Q$, and signal-to-noise ratio. We find that for all M-type dwarfs, the highest RV precision can be reached in the wavelength range 700--900nm. Observations at longer wavelengths are equally precise only at the very latest spectral types (M8 and M9). We demonstrate that in this spectroscopic range, the large amount of absorption features compensates for the intrinsic faintness of an M7 star. To reach an RV precision of 1ms$^{-1}$ in very low mass M dwarfs at longer wavelengths likely requires the use of a 10m class telescope. For spectral types M6 and earlier, the combination of a red visual and a near-infrared spectrograph is ideal to search for low-mass planets and to distinguish between planets and stellar variability. Abstract: Context: The main goal of the CARMENES survey is to find Earth-mass planets around nearby M-dwarf stars. Seven M-dwarfs included in the CARMENES sample had been observed before with HIRES and HARPS and either were reported to have one short period planetary companion (GJ15A, GJ176, GJ436, GJ536 and GJ1148) or are multiple planetary systems (GJ581 and GJ876). Aims: We aim to report new precise optical radial velocity measurements for these planet hosts and test the overall capabilities of CARMENES. Methods: We combined our CARMENES precise Doppler measurements with those available from HIRES and HARPS and derived new orbital parameters for the systems. Bona-fide single planet systems are fitted with a Keplerian model. The multiple planet systems were analyzed using a self-consistent dynamical model and their best fit orbits were tested for long-term stability. Results: We confirm or provide supportive arguments for planets around all the investigated stars except for GJ15A, for which we find that the post-discovery HIRES data and our CARMENES data do not show a signal at 11.4 days. Although we cannot confirm the super-Earth planet GJ15Ab, we show evidence for a possible long-period ($P_{\rm c}$ = 7025$_{-629}^{+972}$ d) Saturn-mass ($m_{\rm c} \sin i$ = 51.8$_{-5.8}^{+5.5}M_\oplus$) planet around GJ15A. In addition, based on our CARMENES and HIRES data we discover a second planet around GJ1148, for which we estimate a period $P_{\rm c}$ = 532.6$_{-2.5}^{+4.1}$ d, eccentricity $e_{\rm c}$ = 0.34$_{-0.06}^{+0.05}$ and minimum mass $m_{\rm c} \sin i$ = 68.1$_{-2.2}^{+4.9}M_\oplus$. Conclusions: The CARMENES optical radial velocities have similar precision and overall scatter when compared to the Doppler measurements conducted with HARPS and HIRES. Abstract: PurposeTo correlate the functional outcomes with histologic findings following transplantation of fetal retinal sheets in rd mice, and to investigate the mechanisms of visual function restoration.MethodsTwenty-one postnatal day 31-38 rd/rd (C3H/HeJ) mice were transplanted in one eye with retinal sheets (1.0 x 0.4 mm) obtained from embryonic day (E) 17 enhanced-green-fluorescent protein (eGFP) mice. Five mice underwent sham surgery without insertion of tissue. Four to five weeks after transplantation, visual responses to a light flash were recorded across the superior colliculus (SC) in seven eyes of seven transplanted mice that had clear corneas and lenses, and in all five sham surgery mice. Following the SC recording, the eyes were enucleated and processed for immunohistochemistry and examined using confocal microscopy.ResultsIn three out of the seven eyes (43%), positive responses were recorded in the SC in an area topographically corresponding to the placement of the transplant in the host retina. No responses were recorded in the untreated eyes of 5-week-old and 9-week-old rd/rd mice, and in the 9-week-old sham surgery mice. In contrast, visual responses were recorded over the entire SC in normal eyes. The response onset latencies of the 3 transplanted mice with responses were similar to those of normal control mice. The organization of the graft did not appear to correlate as expected with the electrophysiology results, as eyes with well-organized, laminated grafts showed no response whereas the three light-responsive eyes had rosetted or disorganized grafts. All three light-responsive eyes demonstrated much higher levels of recoverin immunoreactivity in the host retina overlying the graft compared with untreated age-matched rd/rd mice.ConclusionRestoration of the SC visual response does not appear to depend on a well-organized transplant in the rd mouse. Increased recoverin-staining in the host retina in light-responsive animals suggested that host cone rescue was the likely mechanism of vision restoration in this transplant model.

15. Gradual Increase in Environmental Light Intensity Induces Oxidative Stress and Inflammation and Accelerates Retinal Neurodegeneration

Abstract: PURPOSE: Retinitis pigmentosa (RP) is a blinding neurodegenerative disease of the retina that can be affected by many factors. The present study aimed to analyze the effect of different environmental light intensities in rd10 mice retina. METHODS: C57BL/6J and rd10 mice were bred and housed under three different environmental light intensities: scotopic (5 lux), mesopic (50 lux), and photopic (300 lux). Visual function was studied using electroretinography and optomotor testing. The structural and morphological integrity of the retinas was evaluated by optical coherence tomography imaging and immunohistochemistry. Additionally, inflammatory processes and oxidative stress markers were analyzed by flow cytometry and western blotting. RESULTS: When the environmental light intensity was higher, retinal function decreased in rd10 mice and was accompanied by light-dependent photoreceptor loss, followed by morphological alterations, and synaptic connectivity loss. Moreover, light-dependent retinal degeneration was accompanied by an increased number of inflammatory cells, which became more activated and phagocytic, and by an exacerbated reactive gliosis. Furthermore, light-dependent increment in oxidative stress markers in rd10 mice retina pointed to a possible mechanism for light-induced photoreceptor degeneration. CONCLUSIONS: An increase in rd10 mice housing light intensity accelerates retinal degeneration, activating cell death, oxidative stress pathways, and inflammatory cells. Lighting intensity is a key factor in the progression of retinal degeneration, and standardized lighting conditions are advisable for proper analysis and interpretation of experimental results from RP animal models, and specifically from rd10 mice. Also, it can be hypothesized that light protection could be an option to slow down retinal degeneration in some cases of RP.

16. New Nrf2-Inducer Compound ITH12674 Slows the Progression of Retinitis Pigmentosa in the Mouse Model rd10

Abstract: BACKGROUND/AIMS: It is well established that oxidative stress and inflammation are common pathogenic features of retinal degenerative diseases. ITH12674 is a novel compound that induces the transcription factor Nrf2; in so doing, the molecule exhibits anti-inflammatory, and antioxidant properties, and affords neuroprotection in rat cortical neurons subjected to oxidative stress. We here tested the hypothesis that ITH12674 could slow the retinal degeneration that causes blindness in rd10 mice, a model of retinitis pigmentosa. METHODS: Animals were intraperitoneally treated with 1 or 10 mg/Kg ITH12674 or placebo from P16 to P30. At P30, retinal functionality and visual acuity were analyzed by electroretinography and optomotor test. By immunohistochemistry we quantified the photoreceptor rows and analyzed their morphology and connectivity. Oxidative stress and inflammatory state was studied by Western blot, and microglia reactivity was monitored by flow cytometry. The blood-brain barrier permeation of ITH12674 was evaluated using a PAMPA-BBB assay. RESULTS: In rd10 mice treated with 10 mg/Kg of the compound, the following changes were observed (with respect to placebo): (i) a decrease of vision loss with higher scotopic a- and b-waves; (ii) increased visual acuity; (iii) preservation of cone photoreceptors morphology, as well as their synaptic connectivity; (iv) reduced expression of TNF-a and NF-?B; (v) increased expression of p38 MAPK and Atg12-Atg5 complex; and (vi) decreased CD11c, MHC class II and CD169 positive cell populations. CONCLUSION: These data support the view that a Nrf2 inducer compound may arise as a new therapeutic strategy to combat retinal neurodegeneration. At present, we are chemically optimising compound ITH12674 with the focus on improving its neuroprotective potential in retinal neurodegenerative diseases.

METHODS: This retrospective analysis followed the results of 274 patients treated with the Argus II Retinal Prosthesis System between June 2007 and November 2017, including 30 subjects from the US and European clinical trials, and 244 patients in the postapproval phase. Results were gathered for incidence of a serious adverse event, incidence of conjunctival erosion, occurrence sites, rates of erosion, and erosion timing. RESULTS: Overall, 60% of subjects in the clinical trial subjects versus 83% of patients in the postapproval phase did not experience device- or surgery-related serious adverse events. In the postapproval phase, conjunctival erosion had an incidence rate of 6.2% over 5 years and 11 months. In 55% of conjunctival erosion cases, erosion occurred in the inferotemporal quadrant, 25% in the superotemporal quadrant, and 20% in both. Sixty percent of the erosion events occurred in the first 15 months after implantation, and 85% within the first 2.5 years. CONCLUSION: Reducing occurrence of conjunctival erosion in patients with the Argus II Retinal Prosthesis requires identification and minimization of risk factors before and during implantation. Implementing inverted sutures at the implant tabs, use of graft material at these locations as well as Mersilene rather than nylon sutures, and accurate Tenon's and conjunctiva closure are recommended for consideration in all patients.

Abstract: The innate immune Toll-like receptor (TLR) family plays essential roles in cell proliferation, survival and function of the central nervous system. However, the way in which TLRs contribute to the development and maintenance of proper retinal structure and function remains uncertain. In this work, we assess the effect of genetic TLR4 deletion on the morphology and function of the retina in mice. Visual acuity and retinal responsiveness were evaluated in TLR4 knockout and wild type C57BL/6J control mice by means of an optomotor test and electroretinography, respectively, from P20 to P360. Retinal structure was also analyzed in both strains using confocal and electron microscopy. ERG data showed impaired retinal responsiveness in TLR4 KO mice, in comparison to wild type animals. The amplitudes of the scotopic a-waves were less pronounced in TLR4-deficient mice than in wild-type animals from P30 to P360, and TLR4 KO mice presented scotopic b-wave amplitudes smaller than those of age-matched control mice at all ages studied (P20 to P360). Visual acuity was also relatively poorer in TLR4 KO as compared to C57BL/6J mice from P20 to P360, with significant differences at P30 and P60. Immunohistochemical analysis of retinal vertical sections showed no differences between TLR4 KO and C57BL/6J mice, in terms of either photoreceptor number or photoreceptor structure. Horizontal cells also demonstrated no morphological differences between TLR4 KO and wild-type mice. However, TLR4 KO mice exhibited a lower density of bipolar cells (15% less at P30) and thus fewer bipolar cell dendrites than the wild type control mouse, even though both confocal and electron microscopy images showed no morphologic abnormalities in the synaptic contacts between the photoreceptors and second order neurons. Microglial cell density was significantly lower (26% less at P30) in TLR4 KO mice as compared to wild-type control mice. These results suggest that TLR4 deletion causes functional alterations in terms of visual response and acuity, probably through the loss of bipolar cells and microglia, but this receptor is not essential for the processing of visual information in the retina.

Abstract: Surveys have shown that super-Earth and Neptune-mass exoplanets are more frequent than gas giants around low-mass stars, as predicted by the core accretion theory of planet formation. We report the discovery of a giant planet around the very-low-mass star GJ 3512, as determined by optical and near-infrared radial-velocity observations. The planet has a minimum mass of 0.46 Jupiter masses, very high for such a small host star, and an eccentric 204-day orbit. Dynamical models show that the high eccentricity is most likely due to planet-planet interactions. We use simulations to demonstrate that the GJ 3512 planetary system challenges generally accepted formation theories, and that it puts constraints on the planet accretion and migration rates. Abstract: Context. Teegarden's Star is the brightest and one of the nearest ultra-cool dwarfs in the solar neighbourhood. For its late spectral type (M7.0 V), the star shows relatively little activity and is a prime target for near-infrared radial velocity surveys such as CARMENES. Aims. As part of the CARMENES search for exoplanets around M dwarfs, we obtained more than 200 radial-velocity measurements of Teegarden's Star and analysed them for planetary signals. Methods. We find periodic variability in the radial velocities of Teegarden's Star. We also studied photometric measurements to rule out stellar brightness variations mimicking planetary signals. Results. We find evidence for two planet candidates, each with 1.1 M minimum mass, orbiting at periods of 4.91 and 11.4 d, respectively. No evidence for planetary transits could be found in archival and follow-up photometry. Small photometric variability is suggestive of slow rotation and old age. Conclusions. The two planets are among the lowest-mass planets discovered so far, and they are the first Earth-mass planets around an ultra-cool dwarf for which the masses have been determined using radial velocities.© ESO 2019.

Abstract: The design and construction of CARMENES has been presented at previous SPIE conferences. It is a next-generation radial-velocity instrument at the 3.5m telescope of the Calar Alto Observatory, which was built by a consortium of eleven Spanish and German institutions. CARMENES consists of two separate échelle spectrographs covering the wavelength range from 0.52 to 1.71¿m at a spec-tral resolution of R < 80,000, fed by fibers from the Cassegrain focus of the telescope. CARMENES saw ¿First Light¿ on Nov 9, 2015. During the commissioning and initial operation phases, we established basic performance data such as throughput and spectral resolution. We found that our hollow-cathode lamps are suitable for precise wavelength calibration, but their spectra contain a number of lines of neon or argon that are so bright that the lamps cannot be used in simultaneous exposures with stars. We have therefore adopted a calibration procedure that uses simultaneous star / Fabry Pérot etalon exposures in combination with a cross-calibration between the etalons and hollow-cathode lamps during daytime. With this strategy it has been possible to achieve 1-2 m/s precision in the visible and 5-10 m/s precision in the near-IR; further improvements are expected from ongoing work on temperature control, calibration procedures and data reduction. Comparing the RV precision achieved in different wavelength bands, we find a ¿sweet spot¿ between 0.7 and 0.8¿m, where deep TiO bands provide rich RV information in mid-M dwarfs. This is in contrast to our pre-survey models, which predicted comparatively better performance in the near-IR around 1¿m, and explains in part why our near-IR RVs do not reach the same precision level as those taken with the visible spectrograph. We are now conducting a large survey of 340 nearby M dwarfs (with an average distance of only 12pc), with the goal of finding terrestrial planets in their habitable zones. Abstract: The CARMENES radial velocity (RV) survey is observing 324 M dwarfs to search for any orbiting planets. In this paper, we present the survey sample by publishing one CARMENES spectrum for each M dwarf. These spectra cover the wavelength range 520¿1710 nm at a resolution of at least R >80 000, and we measure its RV, H¿ emission, and projected rotation velocity. We present an atlas of high-resolution M-dwarf spectra and compare the spectra to atmospheric models. To quantify the RV precision that can be achieved in low-mass stars over the CARMENES wavelength range, we analyze our empirical information on the RV precision from more than 6500 observations. We compare our high-resolution M-dwarf spectra to atmospheric models where we determine the spectroscopic RV information content, Q, and signal-to-noise ratio. We find that for all M-type dwarfs, the highest RV precision can be reached in the wavelength range 700¿900 nm. Observations at longer wavelengths are equally precise only at the very latest spectral types (M8 and M9). We demonstrate that in this spectroscopic range, the large amount of absorption features compensates for the intrinsic faintness of an M7 star. To reach an RV precision of 1 m s¿1 in very low mass M dwarfs at longer wavelengths likely requires the use of a 10 m class telescope. For spectral types M6 and earlier, the combination of a red visual and a near-infrared spectrograph is ideal to search for low-mass planets and to distinguish between planets and stellar variability. At a 4 m class telescope, an instrument like CARMENES has the potential to push the RV precision well below the typical jitter level of 3-4 m s-1. © ESO 2018.

Abstract: We report on the first star discovered to host a planet detected by radial velocity (RV) observations obtained within the CARMENES survey for exoplanets around M dwarfs. HD 147379 (V = 8.9 mag, M = 0.58 ± 0.08 M), a bright M0.0 V star at a distance of 10.7 pc, is found to undergo periodic RV variations with a semi-amplitude of K = 5.1 ± 0.4 m s and a period of P = 86.54 ± 0.06 d. The RV signal is found in our CARMENES data, which were taken between 2016 and 2017, and is supported by HIRES/Keck observations that were obtained since 2000. The RV variations are interpreted as resulting from a planet of minimum mass m sin i = 25 ± 2 M, 1.5 times the mass of Neptune, with an orbital semi-major axis a = 0.32 au and low eccentricity (e < 0.13). HD 147379 b is orbiting inside the temperate zone around the star, where water could exist in liquid form. The RV time-series and various spectroscopic indicators show additional hints of variations at an approximate period of 21.1 d (and its first harmonic), which we attribute to the rotation period of the star.© ESO, 2018.

Abstract: Context. The main goal of the CARMENES survey is to find Earth-mass planets around nearby M-dwarf stars. Seven M dwarfs included in the CARMENES sample had been observed before with HIRES and HARPS and either were reported to have one short period planetary companion (GJ 15 A, GJ 176, GJ 436, GJ 536 and GJ 1148) or are multiple planetary systems (GJ 581 and GJ 876). Aims. We aim to report new precise optical radial velocity measurements for these planet hosts and test the overall capabilities of CARMENES. Methods. We combined our CARMENES precise Doppler measurements with those available from HIRES and HARPS and derived new orbital parameters for the systems. Bona-fide single planet systems were fitted with a Keplerian model. The multiple planet systems were analyzed using a self-consistent dynamical model and their best fit orbits were tested for long-term stability. Results. We confirm or provide supportive arguments for planets around all the investigated stars except for GJ 15 A, for which we find that the post-discovery HIRES data and our CARMENES data do not show a signal at 11.4 days. Although we cannot confirm the super-Earth planet GJ 15 Ab, we show evidence for a possible long-period (P = 7030 d) Saturn-mass (msini = 51.8M) planet around GJ 15 A. In addition, based on our CARMENES and HIRES data we discover a second planet around GJ 1148, for which we estimate a period P = 532.6 days, eccentricity e = 0.342 and minimum mass msini = 68.1M. Conclusions. The CARMENES optical radial velocities have similar precision and overall scatter when compared to the Doppler measurements conducted with HARPS and HIRES. We conclude that CARMENES is an instrument that is up to the challenge of discovering rocky planets around low-mass stars.© ESO, 2018.

Abstract: © 2015 American Academy of Ophthalmology. Purpose: Retinitis pigmentosa (RP) is a group of inherited retinal degenerations leading to blindness due to photoreceptor loss. Retinitis pigmentosa is a rare disease, affecting only approximately 100 000 people in the United States. There is no cure and no approved medical therapy to slow or reverse RP. The purpose of this clinical trial was to evaluate the safety, reliability, and benefit of the Argus II Retinal Prosthesis System (Second Sight Medical Products, Inc, Sylmar, CA) in restoring some visual function to subjects completely blind from RP. We report clinical trial results at 1 and 3 years after implantation. Design: The study is a multicenter, single-arm, prospective clinical trial. Participants: There were 30 subjects in 10 centers in the United States and Europe. Subjects served as their own controls, that is, implanted eye versus fellow eye, and system on versus system off (native residual vision). Methods: The Argus II System was implanted on and in a single eye (typically the worse-seeing eye) of blind subjects. Subjects wore glasses mounted with a small camera and a video processor that converted images into stimulation patterns sent to the electrode array on the retina. Main Outcome Measures: The primary outcome measures were safety (the number, seriousness, and relatedness of adverse events) and visual function, as measured by 3 computer-based, objective tests. Results: A total of 29 of 30 subjects had functioning Argus II Systems implants 3 years after implantation. Eleven subjects experienced a total of 23 serious device- or surgery-related adverse events. All were treated with standard ophthalmic care. As a group, subjects performed significantly better with the system on than off on all visual function tests and functional vision assessments. Conclusions: The 3-year results of the Argus II trial support the long-term safety profile and benefit of the Argus II System for patients blind from RP. Earlier results from this trial were used to gain approval of the Argus II by the Food and Drug Administration and a CE mark in Europe. The Argus II System is the first and only retinal implant to have both approvals.

Abstract: OBJECTIVE To develop and characterize an infusion cannula that reduces the intraocular jet forces created during fluid-air exchange. METHODS A new infusion cannula was manufactured by adding a baffle 1.0 mm perpendicular to the distal opening of a 20-gauge infusion. The forces generated by the modified and standard cannulas were measured at a constant air infusion pressure of 50 mmHg by using a force sensor coupled to a schematic eye with eight radial positions from 0 degrees to 180 degrees along the circumference. Five different orientations relative to the baffle support of the modified cannula were evaluated at each position to assess the turbulent flow. RESULTS The standard cannula produced a maximum force of 0.14 mmHg at 180 degrees from the sensor. The modified cannula generated a maximum pressure of 0.017 mmHg at 67.5 degrees from the sensor. Overall, the force from the modified cannula was an order of magnitude less than the standard infusion cannula (P < 0.01). CONCLUSIONS The modifications of the new infusion cannula are passive with respect to its surgical performance. However, the additional baffle transforms laminar air currents into turbulent ones and allows dispersion of the jet forces, which should reduce barotrauma and desiccation of the retina during fluid-air exchange.

Abstract: To assess subjective levels of eye pain, nausea, and sedation following vitreoretinal surgery performed with intravenous sedation and retrobulbar anesthesia.One hundred eighty-five consecutive patients who underwent vitreoretinal surgery and had access to a standard postoperative analgesic regimen prospectively quantified levels of eye pain, nausea, and sedation 2 hours and 5 hours after surgery by using a standard visual analog scale. Analgesic requests were recorded. Responses were statistically analyzed.Fifty-six percent of patients had some eye pain after vitreoretinal surgery; 48% of patients requested an analgesic within 5 hours after surgery. Twenty-seven percent of patients required narcotic analgesia. There was a significant relationship between the presence of eye pain and surgery duration of2 hours (P0.02). Sixteen percent of patients had postoperative nausea, which more likely occurred in those who received a narcotic analgesic (P0.02). Eighty percent of patients had postoperative sedation, which more likely occurred in those who received a narcotic analgesic (P0.02).One half of individuals undergoing vitreoretinal surgery, especially those who have lengthy procedures (2 hours), will request pain medication within 5 hours after surgery; one half of these patients will need narcotic analgesia for pain control. Narcotics may result in nausea and sedation.

Abstract: • Background: Recent studies have suggested that tyrosine kinase pathways that are activated by angiogenic growth factors may play a role in corneal neovascularization. • Methods: Corneal neovascularization was induced in rat corneas by chemical cauterization. At 6, 24, 48, 96, and 168 h after chemical cauterization the rat corneas without the corneal epithelium were prepared for gel electrophoresis. Total protein profiles of the corneal samples were examined by staining gels with Coomassie brilliant blue. Tyrosine-phosphorylated proteins, three angiogenic growth factors (basic fibroblast growth factor, vascular endothelial growth factor, and platelet-derived growth factor-B chain), and three intracellular signal proteins in the tyrosine kinase pathways (phospholipase Cγ, SHC, and mitogen-activated protein kinase) in the corneal samples were examined by western blot analysis. A topical treatment of genistein eye drop (5 mg/ml) was used for inhibition of corneal neovascularization after chemical cauterization in rats. • Results: In total protein profiles, three bands in the corneal samples were increased after cauterization. Overall tyrosine-phosphorylated proteins and all three angiogenic growth factors increased with progression of corneal neovascularization. The tyrosine-phosphorylated forms of three intracellular signal proteins were also increased after cauterization. Treatment with topical genistein was effective in inhibiting corneal neovascularization in rats. • Conclusion: Protein tyrosine phosphorylation was involved in inflammation-induced corneal neovascularization. Tyrosine kinase inhibitors may have utility as inhibitors of corneal neovascularization.

Abstract: It has been proposed that basic fibroblast growth factor (basic FGF) mediates the neovascular response in a variety of conditions, including diabetic retinopathy and branch retinal vein occlusion. To test the hypothesis that basic FGF was released from retinal stores as a result of retinal ischaemia, transient retinal ischaemia was induced, followed by 48 h of reperfusion, in the rat by combined central retinal vasculature and optic nerve ligation. The immunolocalization of basic FGF was studied in the retina. We found that basic FGF in the normal retina is present around the deeper retinal vessels and in the neuronal tissue of the outer plexiform layer. In the eyes that had ischaemia followed by reperfusion, there was moderate cellular oedema with retinal swelling, and mitoses in the inner nuclear and plexiform layers. There were no changes evident at the immunohistochemical level either in the intensity or distribution of stores of basic FGF. We conclude from these data that stores of basic FGF are not altered dramatically under the conditions of transient experimental ischaemia and reperfusion in the rat, despite the presence of cellular proliferation.

El objetivo de este estudio es evaluar desde ambas perspectivas el equilibrio ácido-base en pacientes en hemodiafiltración (HDF) crónica. Material y métodos: se estudian 35 pacientes (24 hombres y 11 mujeres, con una edad media de 67,2 ± 15,7 años y con un peso seco de 72,8 ± 19,2 kg. La duración media de la hemodiálisis (HD) fue de 253,6 ± 40,5 minutos. Se analizan los parámetros gasométricos (pH, pCO2, HCO3- y exceso de bases) y Na+, K+, Cl-, Ca++, Mg++ y lactato.
