Abstract

Variation in the NOS3 gene has been related to the development of restenosis. The Glu298Asp polymorphism has previously been investigated for its effect on NO levels and the development of restenosis. However, the variability of findings gave rise to the hypothesis that the functional significance of this polymorphism may only become manifest under conditions of endothelial dysfunction. Since patients with the metabolic syndrome are known to have endothelial dysfunction, we aimed to investigate if the significance of NOS3 polymorphisms may depend on the presence of the metabolic syndrome. We examined the impact of the -949 A/G, the -716 C/T and the Glu298Asp polymorphisms in the NOS3 gene on the risk of clinical restenosis in a previously described subpopulation of the GENDER-study, a multicenter prospective study design that enrolled consecutive patients after successful PCI. This subpopulation consisted of 901 patients of whom sufficient data were available to establish absence or presence of the metabolic syndrome. Of these patients, 448 had the metabolic syndrome. Clinical restenosis, defined as target vessel revascularization (TVR), was the primary endpoint. We demonstrated that the minor -949G, -716T and 298Asp alleles were associated with a significantly increased risk of TVR in patients with the metabolic syndrome (HR: 1.58 [95% CI: 1.04-2.40], HR: 1.95 [95% CI: 1.02-3.70] and HR: 1.67 [95% CI: 1.09-2.54], respectively). In the group without the metabolic syndrome we observed no association between the three polymorphisms and TVR. We suggest that the metabolic alterations present in patients with the metabolic syndrome influence the functional significance of these polymorphisms and increase the susceptibility of carriers of one of these variations in the NOS3 gene to develop restenosis after PCI.

Abstract

Objective: To perform a patient-pooled analysis of a routine invasive versus a selective invasive strategy in elderly patients with non-ST segment elevation acute coronary syndrome. Methods: A meta-analysis was performed of patientpooled data from the FRISC IIeICTUSeRITA-3 (FIR) studies. (Un)adjusted HRs were calculated by Cox regression, with adjustments for variables associated with age and outcomes. The main outcome was 5-year cardiovascular death or myocardial infarction (MI) following routine invasive versus selective invasive management. Results: Regarding the 5-year composite of cardiovascular death or MI, the routine invasive strategy was associated with a lower hazard in patients aged 65-74 years (HR 0.72, 95% CI 0.58 to 0.90) and those aged ≥75 years (HR 0.71, 95% CI 0.55 to 0.91), but not in those aged less than65 years (HR 1.11, 95% CI 0.90 to 1.38), p=0.001 for interaction between treatment strategy and age. The interaction was driven by an excess of early MIs in patients less than65 years of age; there was no heterogeneity between age groups concerning cardiovascular death. The benefits were smaller for women than for men (p=0.009 for interaction). After adjustment for other clinical risk factors the HRs remained similar. Conclusion: The current analysis of the FIR dataset shows that the long-term benefit of the routine invasive strategy over the selective invasive strategy is attenuated in younger patients aged less than65 years and in women by the increased risk of early events which seem to have no consequences for long-term cardiovascular mortality. No other clinical risk factors were able to identify patients with differential responses to a routine invasive strategy. Abstract

The pharmacological responsiveness of the coronary collateral circulation in humans has been studied only by indirect means. Patients with one-vessel disease and recruitable (n = 14) or spontaneously visible (n = 24) collateral vessels were studied during coronary angioplasty. Collateral flow in the recipient coronary artery was determined with a 0.014-in Doppler guide wire during balloon coronary occlusion and expressed as the diastolic blood flow velocity integral (dVi). Collateral blood flow velocity, mean aortic pressure (Pao), and coronary wedge pressure (Pw) were used to calculate the collateral vascular resistance index: Rcoll = (Pao-Pw)/ dVi (mm Hg/cm) and the peripheral vascular resistance index of the recipient coronary artery: R4 = Pw/dVi (mm Hg/cm). Adenosine (12 to 18 micrograms) and nitroglycerin (0.2 mg) were injected as a bolus in the donor coronary artery during subsequent balloon inflations to assess their effect on these hemodynamic variables. The administration of adenosine or nitroglycerin in patients with recruitable collateral vessels did not induce a change in dVi and Pw/Pao ratio. In patients with spontaneously visible collateral vessels, dVi increased from 8.0 +/- 4.5 to 10.8 +/- 8.0 cm (P = .01) after adenosine and from 7.4 +/- 4.5 to 10.3 +/- 6.9 cm (P = .003) after nitroglycerin. The Pw/Pao ratio remained unchanged after adenosine and nitroglycerin. Rcoll decreased from 10.3 +/- 9.5 to 8.6 +/- 8.5 mm Hg/cm (P = .01) after adenosine and from 11.6 +/- 10.4 to 8.3 +/- 8.9 mm Hg/cm (P < .001) after nitroglycerin. R4 decreased from 7.7 +/- 5.5 to 5.9 +/- 5.1 mm Hg/cm (P < .001) after adenosine and from 8.4 +/- 6.6 to 7.1 +/- 7.2 mm Hg/cm (P = .01) after nitroglycerin. Coronary collateral blood flow can be increased with adenosine and nitroglycerin in patients with one-vessel disease and spontaneously visible collateral vessels, which is in contrast to patients with recruitable collateral vessels. This effect is the result of a reduction in the collateral vascular resistance and peripheral vascular resistance of the recipient coronary artery

43. Estimation of a decreasing hazard of patients with acute coronary syndrome.

Abstract

The Kaplan-Meier, Nelson-Aalen and Breslow estimators are widely used in the analysis of right-censored time to event data in medical applications. These methods are fully non-parametric and do not put any restriction on the shape of the hazard curve. In some applications, this leads to implausible estimates of the hazard course over time. With non-parametric shape-constrained estimation techniques, one can facilitate an increasing or decreasing hazard and thus generate estimators that better match the biological reasoning, without being as restrictive as parametric methods. We illustrate the advantage of such techniques in the analysis of a large clinical trial in cardiology. Simulation results show that in case the true hazard is monotone, the non-parametric shape-constrained estimators are more accurate than the traditional estimators on the hazard level. Abstract

Aims The acute phase reactant C-reactive protein is an important prognostic risk factor in patients with both stable and unstable coronary artery disease. The potential prognostic implications of an abnormal pre-procedural C-reactive protein concentration in patients undergoing elective coronary angioplasty may be relevant for subsequent treatment.Methods and Results Pre-procedural plasma levels of C-reactive protein were measured in 501 patients with stable coronary artery disease undergoing elective coronary angioplasty. The incidence of death or myocardial infarction during a 2-year follow-up was 10·6% (24/227) in patients with an increased C-reactive protein level (>3mg.l−1) and 2·9% (8/274) in patients with a normal C-reactive protein level (RR 3·9, 95% CI 1·7–8·9). Survival without death, myocardial infarction, urgent revascularization or hospital admission for unstable angina was significantly lower in patients with an increased C-reactive protein vs patients with a normal C-reactive protein (log-rank 14·62, P<0·0001). Logistic regression analysis identified an increased C-reactive protein level as a strong independent predictor of event-free survival (RR 2·54, 95% CI: 1·44–4·47, P=0·001).Conclusion Pre-procedural C-reactive protein levels are increased in 45% of patients undergoing elective coronary angioplasty. An increased C-reactive protein level is a powerful independent prognostic indicator for subsequent cardiac events, suggesting that late clinical outcome is markedly influenced by pre-procedural systemic activation of inflammation. Aims We studied the diagnostic value for acute myocardial infarction of serial creatine kinase-MBmassmeasurements on admission and at 7h after the onset of symptoms.Methods and Results Patients presenting to our chest pain unit with symptoms of <5-h duration were eligible. Patients were kept under observation at least until 12h after onset of symptoms. Blood samples were drawn on admission and 7 and 10h after onset of symptoms. Creatine kinase-MBmass>7·0μg.l−1(upper reference limit for acute myocardial infarction), or an increase >2·0μg.l−1(reference change value) between admission and at 7h was considered abnormal. Of a total of 470 patients, 248 patients had acute myocardial infarction: 100 out of the 248 patients had a single creatine kinase-MBmass>7·0μg.l−1on admission (sensitivity 40%, 95% CI:34–46%), 234/248 patients at 7h (sensitivity 94%, 95% CI:91–97%), and 240/248 at 10h (sensitivity 97%, 95% CI:94–99%). At 7h, 246/248 patients had either a single creatine kinase-MB >7·0μg.l−1or a significant increase between admission and 7h (sensitivity 99%, 95% CI:98–100%). Of 222 patients without acute myocardial infarction, 214 had a normal serial creatine kinase-MBmass(specificity 96%, 95% CI:93–98%).Conclusion In patients with symptoms of <5-h duration, acute myocardial infarction can be ruled out using serial creatine kinase-MBmasstaken on admission and at 7h. Abstract

Erythrocytes have been suggested to bind chemokines, thus limiting their effective concentration in the circulation. We investigated binding of the chemokines IL-8 and RANTES to erythrocytes following endotoxin challenge. Endotoxin caused a transient increase of IL-8 in plasma, and a gradual increase of erythrocyte-bound IL-8, remaining elevated for at least 24 h in vivo. Binding of IL-8 to erythrocytes was confirmed by incubation of freshly isolated erythrocytes with endotoxin stimulated plasma. Incubation for 1 h led to a decrease in plasma concentrations of IL-8 with a concomitant increase in erythrocyte-bound IL-8 levels. Erythrocyte-bound RANTES was induced early following endotoxin infusion and our data may suggest competition between the two chemokines for the same binding site on erythrocytes in vivo.
