In our 2021 article published in this journal, we described the development, historical significance, and impact of the National Library of Medicine (NLM) Michael E. DeBakey fellowship in the History of Medicine. This article focuses on a key part of the fellowship, the NLM Michael E. DeBakey Lecture in the History of Medicine, by explaining how this annual program advances historical scholarship and promotes awareness of DeBakey’s legacy and his support of the world’s largest biomedical library, whose collections are appreciated by researchers worldwide. The annual DeBakey Lecture provides a platform for a selected DeBakey fellow to share and expand on their fellowship research, connecting that research and the fellow’s story with a global audience through a videocast, a permanently and freely available archived lecture, a research-based blog post, and an associated blog interview. The lectures have covered topics about DeBakey himself, his influence on the world, and new research that reflects his historical interests. The library’s support of this impactful program, like the Michael E. DeBakey fellowship overall, testifies to its commitment to expanding the legacy of DeBakey hand in hand with its commitment to serving scientists and society in the 21st century. Acute aortic dissection is a medical emergency associated with high mortality, so an early diagnosis based on a detailed clinical history, physical examination findings, and imaging studies is essential for its diagnosis. The treatment depends on its location, with the recommendation of an urgent surgical approach for acute type A dissections and mainly pharmacological treatment in uncomplicated type B aortic dissection. We present the case of a Mexican male patient who developed uncomplicated type B aortic dissection, that was treated with pharmacological treatment and endovascular aortic repair, and presented an adequate clinical evolution without complications at discharge or during follow-up. Aortic dissection is a life-threatening emergency that occurs when the aortic wall layers are separated. It is important to recognize that aortic dissections can have an atypical presentation with neurological deficits and not typical findings of chest pain or thoracic symptoms. Our patient presented with headache, vertigo, and transient neurologic deficits. However, imaging revealed no acute stroke. Our patient had a prior history of DeBakey type 1 aortic dissection repaired four years before. Imaging did not demonstrate an acute aortic dissection however there was an increase in size of the aneurysmal components. However, it is unclear if this contributed to the transient neurological deficits. Further research is needed to determine if there is a correlation between aortic aneurysmal enlargement and transient neurological deficits., Competing Interests: Human subjects: Consent was obtained or waived by all participants in this study. Ce premier article se consacrera aux inventeurs des pinces de Debakey, Adson, Kocher, Péan, Tuffier et Halsted. The history of cardiothoracic surgery at Baylor College of Medicine begins in 1948, when Michael E. DeBakey was recruited as chair of the Department of Surgery. With the addition of Denton A. Cooley, E. Stanley Crawford, and other surgical legends, Baylor College of Medicine quickly rose to the forefront of cardiothoracic surgery. Highlights in its history include treatment of aortic aneurysm and dissection, development of synthetic aortic substitutes, early experience with cardiac replacement valves, the first successful use of coronary artery bypass grafts to revascularize the heart, and the cardiac transplantation and total artificial heart program, which led to the creation of the left ventricular assist device. Baylor's Division of Cardiothoracic Surgery continues to build on its history and legacy to remain a vital contributor to the specialty in the 21st century. [ABSTRACT FROM AUTHOR]

Acute aortic syndrome is made up of three entities: aortic dissection, intramural hematoma, and penetrating ulcer. The common denominator for this pathology is the disruption of the middle layer with bleeding along the wall of the aorta resulting in separation of the layers or in some cases break trauma. Early diagnosis of aortic dissection is essential to improve the outcome, since mortality of natural evolution in the fi rst 24 hours is 20% and 7 days of 62%. One of the most significant diagnostic errors is to confuse the dissection of the aorta with a heart attack, especially if thrombolytic therapy is indicated. The aim of our event is to describe type A aortic dissection Stanford and I DeBakey in 30 year old patient who presented to the emergency department of the hospital Cardiology XXI Century, Mexican Institute of Social Security, with an emphasis on diagnosis and treatment methods to be established in the emergency department in order to decrease mortality in these patients.
