We showed that six widely prevalent biomarkers affecting the atherosclerotic process were also involved in DAS, suggesting a strong osteogenic and pro-inflammatory profile, indicating that aortic valve calcification is a multifactorial biological process. Aim. To study the relationship between the concentration of lipoprotein (a) (Lp (a)) and autotaxin (ATX) in patients with and without degenerative aortic valve stenosis (AoS) in the presence of coronary artery disease (CAD).Material and methods. The study included 461 patients (mean age, 66±11 years, men, 323), 354 of whom had CAD with stenosis ≥50% in at least one coronary artery according to angiography. Degenerative AoS was diagnosed with ultrasound. The control group consisted of 107 patients without CAD and degenerative AoS. Concentrations of Lp (a), ATX, lipids and blood cells were measured for all patients.Results. CAD without degenerative AoS (group 1) was diagnosed in 307 patients, while 47 patients had CAD and degenerative AoS (group 2). Patients in both groups were older than patients in the control group (66±10, 74±8, and 61±13 years, respectively). Background: Although degenerative aortic valve stenosis (DAS) is the most prevalent growth-up congestive heart valve disease, still little known about relationships between DAS severity, vascular stiffness (VS), echocardiographic parameters, and serum biomarkers in patients undergoing transcatheter (TAVR) or surgical aortic valve replacement (SAVR). The objective of this study was to identify biomarkers associated with DAS severity, and those that are associated with cardiovascular death (CVD) and episodes of chronic heart failure (CHF) exacerbation. Methods: A total of 137 patients with initially moderate-to-severe DAS were prospectively evaluated for the relationship between DAS severity, baseline VS, and serum biomarkers (uPAR, GDF-15, Gal-3, IL-6Rα, ET-1, PCSK9, RANTES/CCL5, NT-proBNP, and hs-TnT), and were followed-up for 48 months. The prognostic significance of each variable for CVD and CHF risk was measured by hazard ratio of risk (HR), which was calculated by Cox’s proportional hazard model. Results: DAS severity showed correlations with IL-6Rα (r = 0.306, p < 0.001), uPAR (r = 0.184, p = 0.032), and NT-proBNP (r = −0.389, p < 0.001). Levels of ET-1 and Gal-3 were strongly correlated with VS parameters (r = 0.674, p < 0.001; r = 0.724, p < 0.001). Out of 137 patients, 20 were referred to TAVR, 88 to SAVR, and 29 to OMT. In TAVR patients, the highest levels of ET-1, Gal-3, and VS were found as compared to other patients. The highest incidence of CVD was observed in patients who underwent TAVR (35%), compared to SAVR (8%) and OMT (10.3%) (p = 0.004). In a multivariate analysis, ET-1 occurred predictive of CVD risk (HR 25.1, p = 0.047), while Gal-3 > 11.5 ng/mL increased the risk of CHF exacerbation episodes requiring hospital admission by 12%. Conclusions: Our study indicated that ET-1 and Gal-3 levels may be associated with the outcomes in patients with DAS.

The aim of the present work was to investigate hemodynamic, hormonal, metabolic determinants and their correlations in patients with aortic valve stenosis and nonobstructive aortal calcification. 212 patients elder than 60 years with arterial hypertension were under observation. Were shown that hyperuricemia and hypothyroids to be significantly frequent detected in patients with nonobstructive aortic calcification if compared with patients with aortic valve stenosis. Possible mechanisms of organization degenerative aortic valve stenosis including parathyroid glands hyperfunction and genetic determinants were discussed.

Introduction: Currently, Cardiology Centres are overfilled with patients with degenerative aortic valve stenosis (DAS), usually eldery, with severe concommittant comorbidities, who are referred for further decisions and possible intervention. Aim: To evaluate changes in the risk profile of patients with severe DAS admitted to the cardiology department a decade ago compared with patients currently being admitted. Material and methods: We retrospectively evaluated all patients admitted with confirmed severe DAS, hospitalized during 2005.2006 (group I: 140 patients) and in 2016 (group II: 152 patients), admitted for aortic valve intervention. A standard transthoracic echocardiogram, cardiovascular symptom and risk factor distribution, perioperative risk with the logistic EuroSCORE II and STS mortality scores were obtained. Results: Patients in group II were significantly older (p < 0.001), had more cardiovascular risk factors, and more often presented with atrial fibrillation (27% vs. 11.4%, p = 0.001), renal impairment (34.9% vs. 22.8%; p = 0.024), severe lung disease (17.1% vs. 2.1%, p < 0.001), and extracardiac arteriopathy (40.1% vs. 17.8%, p < 0.001). The aortic valve area (AVA) (p = 0.356), mean-transvalvular pressure gradient (p = 0.215), and left ventricular ejection fraction (p = 0.768) were similar in both groups. However, the prevalence of pulmonary hypertension, severe mitral regurgitation, and low-flow, low-gradient DAS were 3.1-, 8.4- and 1.84-fold more frequent in group II than group I. The percentages of subjects with EuroSCORE II and STS scores . 4% in 2005.2006 were 7.1% and 6.4%, as compared to 27% and 26.3% in 2016 (both p < 0.001). 22% of patients in 2016, as compared to 31% in 2005/2006, were considered ineligible for DAS intervention. Conclusions: In just a decade, the risk profile of patients admitted with DAS has increased hugely, mainly due to older age, accumulation of comorbidities and more advanced disease at presentation. Although transcatheter aortic valve intervention has expanded the indications for intervention in high-risk patients, the number of patients disqualified from interventional treatment remains high. Objective: To investigate the effect of Dan Lou tablet on the efficacy and expression of serum inflammatory factors in elderly patients with degenerative aortic calcification. Methods: A total of 34 cases of aortic valve calcification patients were randomly divided into two groups, the treatment group and the control group, 22 patients in Dan Lou tablet group, 12 patients in the control group. The efficacy was compared between two groups according to the Seattle angina questionnaire in Chinese version, SF-36 scale of quality of life, TCM clinical syndrome score. The serum TNF-α, IL-6 and HCY levels were detected by ELISA before and after treatment. Results: The improvement of angina pectoris degree and the degree of satisfaction were superior in the treatment group than those of the control group, Dan Lou Tablet could enhance the quality of life, improve syndrome and reduce serum inflammatory factors levels of patients. Conclusions: Dan Lou tablet could slow down the progression of aortic valve calcification，which might be related to attenuate the inflammation. [ABSTRACT FROM AUTHOR]

The purpose was to investigate a frequency of hyperuricemia and its relations with risk factors of degenerative pathology at patients who are elder than 60 years. Methods and result. We examined 212 patients with arterial hypertension and degenerative pathology of aortic valve, of which 76 with nonobstructive aortal calcification and 136 patients with aortic valve stenosis. In blood glucose level, lipids, uric acid, TTH and free T4 were defined. Results: A frequency of hyperuricemia was 30.7 %, overt hypothyroidism 11,3 %, high normal TTH level 11.8 %. In women a combination of hyperuricemia and overt hypothyroidism was 39.3 %, in men it was 8.1 %. A negative correlation was found between glucose level and hyperuricemia.

The introduction of transcatheter aortic valve implantation (TAVI) has resulted in a paradigm shift in the treatment of patients with severe aortic stenosis. Data from the recent U.S CoreValve Trial suggest, for the first time, that TAVI is associated with a significantly higher rate of survival at one year compared to surgical aortic valve replacement (SAVR) in the treatment of high-risk patients affected by severe aortic stenosis. Degenerative aortic stenosis (AS) is an active inflammatory process similar to that in atherosclerosis plaque. The aim was to evaluate the atherosclerotic risk factors, concomitant diseases and risk of cardiovascular death using the SCORE system in degenerative AS-patients aged 60 and over with preserved left ventricle ejection fraction related to severity of AS. Material: The database contains retrospectively consecutive series of 126 patients (mean age 69). Patients were analyzed related to severity of AS: severe AS with mean gradient of 64.6±8.7mmHg (n=79) and non-severe AS with mean gradient of 24.4±8.4mmHg (n=47). There were analyzed: age, gender, BMI, laboratory data, presence of hypertension (HA), diabetes, CAD, smoking cigarettes, positive family history, results of 12-lead ECG and coronary arteriography. There was estimated 10 year risk of cardiovascular death using the SCORE system. Results: Both studied populations presented high 10-year risk of cardiovascular death with higher SCORE in non-severe AS (5.64+/-3.3% vs. 6.95+/-3.13%, P=0.006). Multivariate analysis revealed that only HA was an independent parameter determining higher SCORE in non-severe AS (P<0.05). Conclusions: The non-severe degenerative AS-population had higher 10 years risk of cardiovascular death based on SCORE system than severe AS-patients, what was probably resulted from arterial hypertension. The rapid development and refinement of techniques over the past decade have led to the realization that a minimally invasive approach enables aortic valve surgery to be performed with results, at the very least, equivalent to those of traditional (open) valve surgery done in experienced centres. Minimally invasive aortic valve replacement (MIAVR) has now evolved into a safe, efficient treatment option providing greater patient satisfaction and fewer complications. For rapidly ageing population of industrialized countries, aortic valve replacement (AVR) has become the most frequent heart valve surgery. However, transcatheter aortic valve implantation techniques and sutureless aortic valve prostheses recently introduced into clinical practice are challenging now the results of MIAVR in certain high surgical risk patients. Right anterior minithoracotomy results in excellent exposure and a safe conduct of AVR. The minithoracotomy is performed via the second intercostal space with a 6-7&emsp14;cm long skin incision and no bone transection. After direct aortic and peripheral venous cannulation, aortic valve is exposed in a conventional manner, and further conduct of the intervention is not different from the sutureless AVR in median sternotomy. Herein, we discuss the indications, surgical technique and initial results of sutureless AVR through right anterior minithoracotomy.
