This article, published in the journal Pediatric Anesthesia, presents a case report of an infant with a congenital coronary artery anomaly. The patient initially presented with symptoms of heart failure and was diagnosed with anomalous left coronary artery from pulmonary artery (ALCAPA) based on preoperative investigations. However, during intraoperative transesophageal echocardiography (TOE) and surgical inspection, the diagnosis was changed to a normal left coronary artery with a coronary arteriovenous fistula between the left main stem coronary artery and pulmonary artery. International audience; Eilandvlei forms part of the Wilderness Lakes system located along the southern Cape coast of South Africa (Figure 1A, B). These lakes are located behind Pleistocene dune ridges that run parallel along the coastline. Situated within South Africa's aseasonal rainfall zone, climate is influenced by both temperate and tropical circulation systems. 9. Late Holocene environmental and climate dynamics along the southern Cape coast of South Africa: high resolution multi-proxy records from the wilderness embayment

It is well documented that the South African palaeoenvironmental record is relatively limited in terms of both quantity and quality. This is mainly due to the region's highly seasonal rainfall regimes and generally arid to semi-arid environments which are not conducive to the preservation of sedimentary sequences and associated proxy records. The climate along the southern Cape coast is influenced by both tropical and temperate climate systems, and the region hosts highly diverse vegetation including fynbos and thicket elements and includes the Knysna Afrotemperate Region – the most extensive forest complex in southern Africa. The mechanisms controlling these tropical and temperate systems have responded to changing global boundary conditions and these changes have significantly impacted the regional vegetation mosaic. This ephemeral nature of the region's climate and vegetation suggests it is particularly sensitive to climate change, making it an ideal area to evaluate changes in these systems and how they interact over time. For this study, four sets of records were produced from three wetlands along the southern Cape coast. The Eilandvlei palynological and microcharcoal records span the last ~3000 years with the pollen and microcharcoal records from adjacent Bo Langvlei covering the last ~1300 years. The most outstanding feature in these records are the time periods covering the Medieval Climate Anomaly (MCA; c. AD 950 – 1250) and the Little Ice Age (LIA; c. AD 1300 – 1850). The evidence indicates that conditions in the region during the MCA chronozone were relatively dry and perhaps slightly cooler than present. The most durable phase of forest expansion, and likely more humid conditions, occurred during the transition between the MCA and core cooling of the LIA with the LIA clearly identified as a period of cool, dry conditions between c. AD 1600 to c. AD 1850. In addition, the Eilandvlei pollen record demonstrates the effects of external physiographic dynamics on pollen accumulation and deposition within the lake basin. A complementary set of geochemical and sedimentological records have been generated for Bo Langvlei incorporating the last ~4200 years. These records suggest that the late Holocene evolutionary history of Bo Langvlei comprised of three phases: a marine/lagoonal phase extending until c. 1270 cal yr BP, a short transitional phase between c. 1270 and 1200 cal yr BP, and IV the more recent lacustrine phase. A ~650 year pollen and microcharcoal record have been obtained from nearby Vankervelsvlei. Although discontinuous, this is the first palynological record from this unique waterbody covering this period. In terms of climate, the mechanisms driving the observed changes in the records taken as a whole appear to relate to changes in temperature and dynamics in the influence of tropical systems, perhaps transmitted at least in part via the Agulhas Current and the development of localised precipitation systems. The findings further reinforce the proposed importance of summer rainfall in regulating moisture availability along the south coast of South Africa. The records also highlight the significant impacts of fluctuating sea levels and changes in dune morphology in shaping the embayment, and more recently, the effects of accelerated anthropogenic activities in the area

11. Training, knowledge, experience and perceptions regarding cardiopulmonary resuscitation of doctors at Universitas Academic Hospital

Introduction: High-quality CPR is proven to improve immediate survival and survival to hospital discharge in patients having a cardiac arrest in hospital. Evidence shows that without frequent retraining in CPR, health-care providers lose their skill and knowledge earlier than the current recommendation i.e. to attend CPR retraining every two years. The purpose of this study was to determine the current competencies of doctors at Universitas Academic Hospital regarding CPR training, knowledge, experience and perceptions. Methods: A questionnaire designed by the researcher and reviewed by CPR providers was distributed to interns, medical officers, registrars and consultants obtaining information regarding CPR training, CPR exposure and perceptions regarding CPR retraining and CPR knowledge. The knowledge aspect of the questionnaire consisted of questions on basic, advanced cardiac, paediatric, neonatal and obstetric life support. Results: Of the 245 participants only 22,5 % achieved competency (a mark ≥ 80%) for the knowledge aspect of the questionnaire. The majority of participants had not had retraining after two years although 96,7 % of participants felt that keeping up to date with CPR guidelines improved patient outcomes. The most common reasons given for not feeling confident in performing CPR was training related. Conclusion: Doctors at Universitas Academic Hospital are currently not adequately trained in CPR and it reflects in their lack of CPR knowledge. Lack of training seems to be the most common reason for not feeling confident and being too busy to attend these retraining courses was reported as the most common reason. From this study it also seems that very little of the departments have CPR training for their doctors. Implementing a regular CPR training program within the hospital is suggested to improve CPR knowledge of doctors.

12. 4000 years of environmental and climate change at Eilandvlei : a palynological investigation into the late Holocene palaeoenvironment of the Wilderness Embayment

It is a well-known fact that the South African palaeoenvironmental record is fairly limited, mainly because the environmental conditions here do not lend themselves to the preservation of proxy data sources. This being said, new evidence is emerging from the Wilderness Embayment along the Southern Cape coast of South Africa. This area is of particular interest from a palaeoclimate perspective due to its location within the small year round rainfall zone of South Africa, while the presence of both Fynbos and Afrotemperate forest within this region further highlights the importance of studies in the area. This study presents a new high resolution late Holocene pollen and charcoal record from Eilandvlei, extending from c. 3 800 cal yr BP to present. The record is marked by several significant periods -- largely corresponding to the timing of the three events that typify the late Holocene: the Neoglacial, the Medieval Climate Anomaly and the Little Ice Age. The record also encompasses the arrival of European colonists in the region ca. 1800 AD, marked by the appearance of Pinus, with subsequent notable fluctuations in other taxa, e.g. Podocarpus and Stoebe-type, possibly the expression of the anthropogenic effect on the landscape. This high resolution record thus highlights significant, albeit short term, fluctuations in climate and vegetation patterns along the Southern coast of South Africa and adds to the inadequate records available for the region thus far.

This research cruise with the RV Meteor into South African waters was realized in December 2013. The cruise was embedded in the German South African cooperation within the framework of the SPACES program and provided a very important extension and contribution to the works planned in the RAIN (Regional Archives for Integrated Investigations) and WCLL (Wild Coast Living Laboratory) projects as well as a valuable additional data set to the GENUS-II project. The focus of the cruise was on the origin and dynamics of the Agulhas current and its interplay with estuarine systems along South Africa's east coast, and the dynamics of ESACW and SACW and its impact on the OMZ in the northern Benguela upwelling system. The focus of the geological work was on the investigation of paleo lagoons off Mossel Bay and Durban. A strong focus was put on training and capacity building activities in physical, biological, biogeochemical and geological marine research. A total of 15 berths were provided for post-graduate or PhD students and young scientists to learn state of the art methods in marine science. The joint work at sea of ten South African and five German trainees is seen as an important element to foster intercultural exchange and provide a common basis for potential future cooperation. To reach the scientific goals of the cruise we worked up 44 stations along the Southern African coast and 4 south of Madagascar. We realised 30 CTD casts, 16 vertical and 26 towed multi net hauls, and 18 Ring Trawls. A total of 10 drifters and floats were deployed around Madagascar to investigate the currents in the Mozambique channel. Parasound was used along a track of 62 km to map sea bottom structure off Durban and Mossel Bay and search for potential sites to deploy Gravity Corer and Vibro Corer at a total of 11 stations. Underway measurements could be performed on pCO2 distribution. Lectures were hold regularly on different topics and trainees were supervised to prepare the cruise report., METEOR-Berichte
