Objective: To assess differences in right ventricular function between the primary angioplasty and pharmacoinvasive groups using echocardiographic parameters. Method: Observational, retrospective, and cross-sectional study conducted on 111 patients in a cardiac care unit, where ventricular function was evaluated through transthoracic echocardiography. Results: There were no significant differences in right ventricular function parameters (TAPSE, FAC, S wave, RV Tei index, right V/A coupling) between both groups. As a notable finding, the pharmacoinvasive group exhibited better LVEF and smaller volumes at the end of diastole and systole. Conclusions: In patients with STEMI undergoing the pharmacoinvasive strategy compared to those receiving primary angioplasty, there is no significant difference in the main right ventricular function parameters assessed by transthoracic echocardiography. Massive pulmonary thromboembolism (PT) is a rare acute vascular event characterized by obstructive shock associated with high mortality. The objective of this report is to emphasize the echocardiographic-guided thrombolytic therapy of a patient with massive PT and cardiovascular arrest. We report the case of a 48-year-old male patient admitted with an open fracture of the left fibula, which was not surgically intervened and not prophylactic anticoagulated, who developed sudden onset of dyspnea, chest pain, and cardiovascular arrest two times; the last one in the ICU. After returning to spontaneous circulation, he developed hypotension, tachycardia, hypoxemia, and a Glasgow coma scale of 4 points. The EKG showed sinus tachycardia, S1Q3T3 pattern, and complete right bundle block. The echocardiography showed a >1 ratio between the right and left ventriculus, McConell positive signs, displacement of the ventricular septum, systolic excursion of the tricuspid ring of 10mm, inferior cava vein of 23mm with <50% collapse. He received the highest doses of noradrenaline, vasopressin, and thrombolysis with 100mg recombinant tissue plasminogen activator followed by sequential echocardiographic monitoring, achieving cardiopulmonary stabilization and resolution of the obstructive shock. Objectives: Arrhythmogenic cardiomyopathy (ACM) is a complex cardiac disorder associated with ventricular arrhythmias. Understanding the relationship between mechanical uncoupling and cardiac structural changes in ACM patients is crucial for improved risk stratification and management. Methods: In this study, we enrolled 25 ACM patients (median age 34 years, 72% men) based on the 2019 Modified Task Force and Padua criteria. Patients were categorized by the presence or absence of clinically relevant ventricular tachycardia (crVT), necessitating emergency interventions. Right ventricular-arterial coupling (VAC) was assessed using echocardiography. Low-rank regression splines were employed to model left ventricular ejection fraction (LVEF) and right ventricular ejection fraction (RVEF) in relation to VAC. Results: Positive associations were observed between VAC and LVEF (= 0.472, p = 0.023), RVEF (ρ = 0.522, p = 0.038), and right ventricular (RV) indexed stroke volume (ρ = 0.79, p < 0.001). Patients with crVT exhibited correlations with RV shortening, reduced RVEF (39.6 vs. 32.2%, p = 0.025), increased left ventricular (LV) mass (38.99 vs. 45.55, p = 0.045), and LV end-diastolic volume (LVEDV) (56.99 vs. 68.15 mL/m2, p = 0.045). Positive associations for VAC were noted with LVEDV (p = 0.039) and LV mass (p = 0.039), while negative correlations were observed with RVEF by CMR (p = 0.023) and RV shortening by echocardiography (p = 0.026). Conclusions: Our findings underscore the significance of right VAC in ACM, demonstrating correlations with RV and LVEF, RV stroke volume, and clinically relevant arrhythmias. Insights into RVEF, LV mass, and end-diastolic volume provide valuable contributions to the understanding of ACM pathophysiology and may inform risk assessment strategies. [ABSTRACT FROM AUTHOR]

Sepsis-induced cardiomyopathy is a clinical entity rarely reported in the literature. It lacked an objective and agreedupon definition, and although there are clinical criteria, there are no diagnostic guidelines or specific treatment; its identification, approach, and management imply a challenge for the clinician. Its presence increases mortality by up to 70%. It is considered an acute onset complication that manifests with alterations in systolic and diastolic function, finally showing reversibility in a period of seven to ten days. When this entity is suspected, the gold standard for diagnosis is the echocardiogram. Infection control, life support, and hemodynamic measures associated with the use of vasopressors and inotropes are the main therapeutic tools that have shown a decrease in mortality. However, this entity usually requires advanced organic support, even requiring ventricular assistance devices such as an intraaortic balloon pump (IABP) or extra corporeal membrane oxygenation (ECMO), which have shown promising results in the treatment of this pathology. Despite this, mortality is high, and the prognosis continues to be unfavorable without early identification, hence the importance of unifying criteria, establishing risk factors, validated diagnostic tools as well as differentiating it from other clinical entities, thereby improving its outcome and prognosis. Resumo Fundamento A ecocardiografia é essencial para avaliação do coração transplantado. No entanto, os valores de normalidade no transplante cardíaco (TC) não estão claramente definidos. Objetivos: Comparar parâmetros ecocardiográficos convencionais e pela técnica de Speckle Tracking entre pacientes transplantados cardíacos sem rejeição e uma população de indivíduos saudáveis. Métodos Foram estudados prospectivamente pacientes adultos, com menos de 1 ano de TC, que realizaram biópsia endomiocárdica de vigilância seguido de ecocardiograma transtorácico (ETT). Medidas convencionais de ETT acrescidas da avaliação de mecânica cardíaca por meio do Strain pelo Speckle Tracking foram realizadas e comparadas com um grupo de voluntários saudáveis. A significância estatística adotada para o estudo foi de 5%. Resultados Avaliou-se 36 pacientes transplantados sem rejeição, os quais foram comparados com 30 indivíduos saudáveis. RESUMEN La insuficiencia mitral funcional auricular (IMFA) ha sido recientemente el foco de atención de muchos artículos y revisiones originales. El envejecimiento de la población, el aumento de la prevalencia de insuficiencia cardiaca con fracción de eyección preservada y fibrilación auricular así como el desarrollo de las técnicas transcatéter para la reparación de la válvula mitral han resaltado la IMFA, un fenotipo de insuficiencia mitral que presenta desafíos diagnósticos específicos y para el cual actualmente las guías no tienen recomendaciones sólidas para su tratamiento. Existe una importante acumulación de datos que sugieren que el pronóstico de los pacientes con IMFA grave es malo si no se tratan. Sin embargo, las nuevas terapias para la insuficiencia cardiaca y las técnicas mínimamente invasivas pueden tener un impacto positivo en el prognóstico de esos pacientes.

15. Assessment of subtle cardiac dysfunction induced by premature ventricular contraction using two-dimensional strain echocardiography and the effects of successful ablation

Introduction and objectives: We aimed to assess the effects of successful ablation on impaired left ventricular global longitudinal strain (LV-GLS) in patients with frequent premature ventricular contractions (PVCs). We also evaluated the potential risk factors of impaired LV-GLS. Methods: Thirty-six consecutive patients without any structural heart disease, who were treated with radiofrequency (RF) ablation due to frequent PVCs, were included in the study. All patients were evaluated with standard transthoracic and two-dimensional speckle tracking echocardiography. Results: Mean LV-GLS before ablation was 17.3 ± 3.7 and 20.5 ± 2.6 after ablation; the difference was statistically significant (p−16% and those ≤16%. Low PVC E flow/post-PVC E flow and PVC SV/post-PVC SV ratios were associated with impaired LV-GLS. Conclusion: In symptomatic patients with frequent PVCs and normal left ventricular ejection fraction, we observed significant improvement in LV-GLS value following successful RF ablation. Patients with impaired LV-GLS more often display non-ejecting PVCs and post-extrasystolic potentiation (PEP) compared to patients with normal LV-GLS. Resumo: Introdução e objetivos: O nosso objetivo foi avaliar os efeitos de ablação eficaz na deformação longitudinal global do ventrículo esquerdo (LV-GLS) em doentes com extrassístoles ventriculares (PVCs). Também avaliamos os potenciais fatores de risco de LVGLS anormal. Métodos: Trinta e seis doentes consecutivos sem doença cardíaca estrutural tratados com ablação por radiofrequência (RF) devido a PVCs frequentes foram incluídos no estudo. Todos os doentes foram avaliados com ecocardiografia transtorácica com speckle tracking bidimensional (2D-STE). Resultados: Os valores médios de LVGLS antes da ablação foram de 17,3 ± 3,7. Esse valor foi observado como 20,5 ± 2,6 após a ablação e a diferença foi estatisticamente significativa (p−16% e ≤16%. Baixo valor de PVC E flow/post-PVC E flow e PVC SV/post-PVC SV associaram-se com LV-GLS anormal. Conclusão: Em doentes sintomáticos com PVCs frequentes e fração de ejeção do ventrículo esquerdo (FEVE) normal, observamos melhoria significativa no valor LV-GLS após ablação por RF bem-sucedida. Doentes com LV-GLS anormal apresentam frequentemente nonejecting PVCs e potencialização pós-extrassistólica (PEP) em comparação com doentes com LV-GLS normal.

Resumo Fundamento: A cardiomiopatia associada à lipodistrofia parcial (LP) ainda não foi bem descrita. Objetivo: Caracterizar a morfologia e a função cardíaca na LP. Métodos: Pacientes com LP e controles foram avaliados prospectivamente por ecocardiografia transtorácica e ecocardiografia por speckle-tracking (Strain Longitudinal Global, SLG). A relação entre as variáveis ecocardiográficas e o diagnóstico de LP foi testada com modelos de regressão, considerando o efeito da pressão arterial sistólica (PAS). Adotou-se um nível de significância de 5%. Resultados: Vinte e nove pacientes com LP foram comparados com 17 controles. Eles não se diferiram quanto à idade (p=0,94), sexo ou índice de massa corporal (p= 0,05). Os pacientes com LP apresentaram PAS estatisticamente mais alta (p=0,02) em comparação aos controles. Ainda, os pacientes com LP apresentaram maior dimensão do átrio (37,3 ± 4,4 vs. 32,1 ± 4,3 mm, p= 0,001) e maior volume atrial (30,2 ± 7,2 vs. 24,9 ± 9,0 mL/m2, p=0,02), massa do Ventrículo Esquerdo (VE) (79,3 ± 17,4 vs. 67,1 ± 19,4; p=0,02), e parâmetros sistólicos reduzidos do VE (E’ lateral, p= 0,001) (E’ septal, p= 0,001), (razão E/E’, p= 0,02). A fração de ejeção do VE (64,7 ± 4,6 vs. 62,2 ± 4,4 %, p = 0,08) e o SLG não foram estatisticamente diferentes entre os grupos (-17,1±2,7 vs-18.0 ± 2,0%, p= 0,25). Os inibidores da enzima fosfodiesterase tipo 5 (PDE-5) são usados no tratamento da DE, e as respostas dos pacientes a esse tratamento podem variar. Resumo Fundamento: Seguimento de coorte retrospectiva de 30 anos que se aproxima da história natural dos tumores cardíacos diagnosticados no feto uma vez que nenhum caso foi submetido à interrupção da gestação. Objetivo: Avaliar a morbidade e mortalidade perinatal e em longo prazo em fetos com diagnóstico de tumor cardíaco. Como objetivo secundário avaliar os fatores que influenciaram os resultados perinatais e pós-natais. Método: Estudo de coorte retrospectiva envolvendo 74 gestantes com diagnóstico ecocardiográfico fetal de tumor cardíaco acompanhadas em dois serviços de referência no período de maio de 1991 a novembro de 2021. Foi realizada análise descritiva dos dados por meio de frequências absolutas (n) e relativas (%), mediana e intervalos interquartis. Para avaliar a associação entre as características ecocardiográficas e as manifestações clínicas com os resultados perinatais e pós-natais, foi aplicado o teste exato de Fisher. O cálculo da sobrevida global foi realizado pelo método de Kaplan-Meier e a comparação de curvas pelo teste de log-rank. Papillary fibroelastoma is a benign cardiac tumor generally found on valve surfaces, although involvement of a cardiac chamber endocardium has been rarely reported. They are usually asymptomatic, but embolic events are highly associated presenting with neurological clinic. We present a case of a patient with recurrent stroke presenting with aphasia, right hemiparesis and left oculocephalic deviation, in which a mobile mass was observed in the transesophageal echocardiography at the ligament of Marshall, being a very unusual location for the tumor. Surgical resection was performed uneventfully and histopathological examination confirmed the diagnosis. Resumen: El fibroelastoma papilar es un tumor cardiaco benigno que se encuentra generalmente en la superficie valvular, siendo muy infrecuente la afectación del endocardio de las cámaras cardiacas. Suelen ser asintomáticos, aunque eventos embólicos están muy asociados manifestándose con clínica neurológica importante. Presentamos el caso de una paciente con ictus embólico recurrente que presenta afasia, hemiparesia derecha y desviación oculocefálica izquierda, en la que se observó en el ecocardiograma transesofágico una masa móvil a nivel del ligamento de Marshall, una localización muy infrecuente del tumor. Se realizó resección quirúrgica completa y sin incidencias, obteniendo posteriormente el diagnóstico histopatológico de fibroelastoma papilar.

Resumo A comunicação do septo atrial (CIA) representa, aproximadamente, de 6%-10% dos defeitos cardíacos congênitos, com incidência de 1 em 1.500 nascidos vivos.1 Forame oval patente (FOP) é mais comum e está presente em mais de 20%-25% dos adultos.2 Síndromes clínicas associadas a CIA e FOP são variáveis, com implicações abrangendo a medicina pediátrica e adulta, neurologia e cirurgia. O interesse adicional na anatomia do septo interatrial (SIA) aumentou substancialmente nas últimas duas décadas, com evolução simultânea dos procedimentos percutâneos envolvendo cardiopatia estrutural do lado esquerdo e procedimentos eletrofisiológicos. Idealmente, essas intervenções baseadas em cateter requerem rota direta para o átrio esquerdo (AE) através do SIA, necessitando completo entendimento de sua anatomia. Atualmente, tecnologias de imagem sofisticadas e não invasivas como ecocardiografia transesofágica bidimensional (ETE 2D) e tridimensional (ETE 3D), ressonância cardíaca (RMC) e tomografia computadorizada (TC) passaram por um extraordinário desenvolvimento tecnológico, fornecendo detalhes anatômicos das estruturas cardíacas visualizadas em formato 2D e 3D e são essenciais para diagnóstico e tratamento de pacientes com doenças cardíacas. A avaliação da anatomia e anormalidades do SIA, portanto, requer abordagem padronizada e sistemática, integrando modalidades diagnósticas e fornecendo avaliação adequada e uniforme para terapias cirúrgicas e transcateter.

Hypoplastic left heart syndrome (HLHS) is characterized by underdevelopment of the left-sided heart structures. The prenatal diagnosis of this congenital heart disease is crucial because a newborn with undiagnosed HLHS often presents with clinical signs of low cardiac output once the ductus arteriosus begins to close. With that in mind, the aim of this article was to perform a nonsystematic review focusing on the key ultrasound features that can be used in the prenatal diagnosis of HLHS. Severe forms of HLHS are characterized by a markedly abnormal four-chamber view of the fetal heart (small left atrium, hypoplastic left ventricle, or abnormal mitral valve). The left ventricular outflow tract view allows the degree of hypoplasia in the tract to be evaluated and the diameter of the ascending aorta to be measured. The Z-scores are intended to aid in the diagnosis and follow-up of HLHS. In mild forms of HLHS, a right ventricle/left ventricle length ratio > 1.28 was the strongest predictor of a univentricular outcome. [ABSTRACT FROM AUTHOR]

The definition of mitral annular disjunction (MAD) is still controversial. In the different areas of research, anatomical, clinical, and imaging studies, the separation of the junction points between the mitral valve annulus and the left ventricular wall (greater than 5 mm) has been proposed as a definition, which has been observed in patients with mitral valve prolapse, ventricular arrhythmias, and sudden death. Many of them presented myxomatous degeneration of the valve, a pathology that generates a weakening of the tissue and changes in its normal function. Changes in the geometry of the left ventricle are accompanied by changes in its function, modifying the valve apparatus and generating abnormal dynamics of the mitral annulus. Given the proximity to the anatomical structures of the conduction system, it is likely that these changes have an impact on the it, generating alterations such as arrhythmias, related to sudden death. Material and methods: 30 human hearts preserved with the Kaiserling technique were used. The dissections were performed with microsurgery and standard cardiovascular surgery forceps and scissors with the naked eye, with magnifying glasses and with an optical microscope. A cut was made at the level of the posterior leaflet of the mitral valve, between both papillary muscles, with part of the left atrium and ventricle, to assess whether there is disjunction or not. Results: We have managed to identify MAD in approximately 10% of cases. Conclusions: It is proposed to establish the measurement of MAD parameters in routine controls to perform cardiological follow-up of those patients who present them, their early detection may contribute to the prevention, monitoring and treatment of the aforementioned pathologies. Thoracic compartment syndrome in the post operatory of cardiac surgery is a rare entity caused by an increase in pressure within the osteofibrous thoracic cavity. This is due to an increase in the interior volume or a decrease in the space that contains the mediastinal structures. As a result, it leads to a marked decrease in preload and cardiac index causing cardiogenic shock. We present the case of a 86-year-old patient with multiple comorbidities, who underwent aortic valve replacement and reconstruction of the ascending thoracic aorta. Consequently, presented with cardiogenic shock associated to thoracic compartment syndrome in the immediate postoperative period. The patient underwent surgical reintervention twice to relieve cardiogenic shock where the closure of the thoracic cavity was deferred for three days. During that time there was decompression of cardiac cavities and complete recovery of the ventricular function. Thoracic compartment syndrome in the post operatory of cardiac surgery is a rare and potentially lethal condition. It can present in the early or late postoperative period and it requires high clinical suspicion to be diagnosed. Early recognition is crucial in the management and prognosis. [ABSTRACT FROM AUTHOR]

Introduction: The electrocardiogram (ECG) is a useful initial tool for the detection of left ventricular hypertrophy (LVH) despite its low sensitivity. However, there are no data in Colombian population on the behavior of the different electrocardiographic criteria. Materials and method: A cross-sectional study was carried out in two specialized cardiology centers in Medellín since 2011, where ECGs of 51 patients who met the inclusion criteria were analyzed. Six criteria were calculated, including the Peguero-Lo Presti, while LVH was defined based on two-dimensional transthoracic echocardiography and M mode. Results: 18 patients with electrocardiographic criteria of LVH and confirmed with echocardiography were analyzed and were compared with 33 controls. LVH was found more frequently in older age, arterial hypertension (HTN) and chronic kidney disease. ROC curves were made determining that the best criteria for the electrocardiographic diagnosis of LVH identified by echocardiography is the Peguero-Lo Presti criteria (ROC: 0.81). The specificities of all the criteria were 81-93%, with no significant differences between them. Conclusion: The electrocardiographic criteria with the best diagnostic performance to identify LVH diagnosed by echocardiography is the Peguero-Lo Presti criteria, compared to the traditional criteria used in the evaluated population. Background and aim: Acute kidney injury (AKI) conditions several short- and long-term complications. The aim of the present study was to analyse the impact of cardiac function and structure in the cardiovascular prognosis after an in-hospital AKI episode. Material and methods: This is an observational retrospective cohorts study including all in-hospital AKI episodes in 2013 and 2014 in our centre. At baseline, epidemiological values, comorbidities and echocardiography parameters were collected. During a follow-up of 49 ± 28 months, cardiovascular events (CVE) were collected, and associated factors were analysed. Results: 1255 patients were included (55% male, age 75 ± 13 years). Of the 676 (54%) that had a previous echocardiogram, 46% had left ventricular hypertrophy, 38% pulmonary hypertension, 38% diastolic dysfunction and 22% systolic dysfunction. During the follow-up, 484 (39%) developed a CVE. Associated factors to VCE were male sex, age, diabetes mellitus, hypertension, dyslipidemia, coronary heart disease, heart failure, atrial fibrillation, neoplasia and chronic kidney disease (also, glomerular filtration rate at baseline and after the AKI episode). Survival curves demonstrated that all the echocardiographic parameters were associated to CVE. An adjusted Cox regression model showed that age (HR 1.017), diabetes (HR 1.576) and diastolic dysfunction (HR 1.358) were independent predictors for CVE. Conclusion: Diastolic dysfunction is an independent predictor for long-term cardiovascular events after an in-hospital acute kidney injury episode. Resumen: Antecedente y objetivo: El desarrollo de un fracaso renal agudo (FRA) condiciona complicaciones a corto, medio y largo plazo. El objetivo de nuestro estudio fue analizar el impacto de las alteraciones cardiacas en el pronóstico cardiovascular de pacientes que presentan un episodio de FRA. Materiales y métodos: Realizamos un estudio observacional de cohortes retrospectivo incluyendo a todos los pacientes con FRA en 2013 y 2014. Basalmente recogimos variables epidemiológicas, comorbilidades y parámetros ecocardiográficos. Seguimos a los pacientes tras el ingreso durante una media de 49 ± 28 meses, recogiendo la incidencia de eventos cardiovasculares (ECV) y los factores asociados a los mismos. Resultados: Se incluyeron 1255 pacientes (55% varones, edad 75 ± 13 años). De los 676 (54%) pacientes que disponían de un ecocardiograma previo, el 46% tenían hipertrofia de ventrículo izquierdo, el 38% hipertensión pulmonar, el 38% disfunción diastólica y el 22% disfunción sistólica. Tras la hospitalización por FRA, 484 (39%) tuvieron un ECV. Los factores asociados a presentar un ECV fueron el sexo (varón), la edad, diabetes mellitus, hipertensión arterial, dislipidemia, cardiopatía isquémica, insuficiencia cardiaca, fibrilación auricular, neoplasia previa y enfermedad renal crónica (y el filtrado glomerular estimado basal y tras el FRA). El análisis de supervivencia demostró que todos los parámetros ecocardiográficos se asociaban a ECV. Un modelo de regresión de Cox ajustado demostró que la edad (HR 1,017), la diabetes mellitus (HR 1,576) y la disfunción diastólica (HR 1,358) eran predictores independientes de ECV. Conclusiones: La disfunción diastólica es un predictor independiente de ECV a largo plazo tras un episodio hospitalario de FRA.

La insuficiencia congénita de la válvula auriculoventricular derecha se define como displasia tricuspídea. La alteración se presenta en hojuelas valvares septal y mural, en cuerdas tendinosas o en músculos papilares. La malformación provoca regurgitación tricuspídea y presentación de signos clínicos asociados a insuficiencia cardíaca derecha congestiva. Las lesiones macroscópicas incluyen nodulación o engrosamiento localizado o difuso de hojuelas valvares, cuerdas tendinosas cortas o alargadas, con desarrollo incompleto y fusión o unión parcial, músculos papilares con inserción anormal o separación incompleta a la pared ventricular derecha o ausencia de uno o más músculos. El objetivo del presente trabajo es aportar información y casuística con fines diagnósticos de una patología de baja prevalencia en animales de compañía. Entre 2011 y 2021 se atendieron 24 caninos con diagnóstico definitivo de displasia tricuspídea. La patología fue más frecuente en perros de talla pequeña a mediana, Caniche (4), Dachshund (3), Bulldog francés (3), Yorkshire terrier (2), Bull terrier (1), Bóxer (1) y 10 mestizos, 3 menores a 12 kg y 7 entre 12 y 30 kg de peso, de talla mediana a grande. Los hallazgos son distintos, raza y talla, a los observados por otros autores. El engrosamiento nodular asociado a cuerdas tendinosas cortas fue la malformación valvar más común. La displasia tricuspídea se asoció con la mitad (13 de 24) de los casos con estenosis pulmonar. Otras anomalías congénitas, comunicación interventricular e interauricular, también se presentaron relacionadas. El diagnóstico preciso de la anormalidad congénita sobre el aparato valvular tricuspídeo sólo puede ser realizado por ecocardiografía. Las alteraciones valvares, hojuela septal, caracterizados por nodulación, engrosamiento y unión al septum interventricular, fueron las más habituales. La exploración doppler espectral continuo permitió determinar gravedad, presiones en cámaras atrial y ventricular derecha y establecer efectos hemodinámicos del proceso y su implicancia en el pronóstico y tratamiento del paciente.

Introduction: Proportionality of secondary mitral regurgitation (sMR) may be a key factor in deciding whether a patient may benefit from mitral intervention. The aim of this study was to evaluate the prognostic value of two different concepts of proportionality and assess their ability to improve MR stratification proposed by the American Society of Echocardiography (ASE) guidelines. Methods: We conducted a retrospective analysis in patients with reduced left ventricular ejection fraction (LVEF) (0.14; b) Lopes et al. – disproportionate sMR whenever measured EROA>theoretical EROA (determined as 50%×LVEF×LVEDVMitralVTI). Primary endpoint was all-cause mortality. Results: A total of 572 patients (69±12 years; 76% male) were included. Mean LVEF was 33±9%, with a median left ventricular end-diastolic volume of 174 mL [136;220] and a median effective regurgitant orifice area of 14 mm2 [8;22]. During mean follow-up of 4.1±2.7 years, there were 254 deaths. Background: Although chemotherapy-induced cardiotoxicity is an emerging problem, limited information is available on the effects of chemotherapy on left ventricular (LV) mechanical functions in patients with non-small cell lung cancer (NSCLC). Objective: We aimed to explore chemotherapy-induced alterations in cardiac mechanical functions in patients with NSCLC using speckle tracking echocardiography (STE). Methods: Seventy-one patients with NSCLC and 34 age and sex matched control subjects were consecutively included. Based on their good performance status (Eastern Cooperative Oncology Group performance status), 39 patients were treated with paclitaxel plus carboplatin (PC) regimen and 32 patients were treated with vinorelbine plus cisplatin (VC) regimen. All patients and controls underwent conventional two-dimensional echocardiography and STE at baseline to assess their LV functions. The echocardiographic examinations of NSCLC patients were repeated after the chemotherapy regimens. Results: None of the NSCLC patients developed any signs or symptoms of clinical heart failure during or after the chemotherapy. There were not any significant differences in LV ejection fraction between NSCLC patients and controls before and after chemotherapy. There were not any significant differences in baseline LV global longitudinal strain (GLS), radial strain (RS), and circumferential strain (CS) between NSCLC patients and controls. However, all LV GLS, RS and CS significantly decreased in patients treated with the PC regimen resulting in a significant difference compared to both VC group and controls while no significant decreases were observed in strain measures in VC group. Conclusion: Paclitaxel plus carboplatin, but not VC, may induce subclinical cardiotoxicity in patients with NSCLC, which may be detected by STE. Resumo: Introdução: Embora a cardiotoxicidade induzida pela quimioterapia seja um problema emergente, existe informação limitada sobre os efeitos da quimioterapia na função do ventrículo esquerdo (VE) em doentes com cancro do pulmão não de pequenas células (CPNPC). Objetivo: Avaliar alterações induzidas pela quimioterapia na função cardíaca em doentes com CPNPC utilizando a ecocardiografia de speckle tracking (EST). Métodos: Foram incluídos consecutivamente 71 doentes com CPNPC e 34 indivíduos caso controlo emparelhados por idade e género. Com base no seu desempenho (Eastern Cooperative Oncology Group performance status), 39 doentes foram tratados com regime de paclitaxel e carboplatina (PC) e 32 doentes foram tratados com regime de vinorelbina e cisplatina (VC). Todos os doentes e casos controlo foram submetidos a ecocardiografia bidimensional convencional (2DE) e a EST na fase basal para avaliar a função VE. As avaliações ecocardiográficas dos doentes com CPNPC foram repetidas após quimioterapia. Resultados: Nenhum dos doentes com CPNPC desenvolveu qualquer sinal ou sintoma clínico de insuficiência cardíaca durante ou após a quimioterapia. Não houve diferenças significativas na fração de ejeção do VE entre os doentes com CPNPC e os casos controlo antes e depois da quimioterapia. Não houve diferenças significativas nos valores basais da deformação longitudinal global do VE (GLS), deformação radial (RS) e deformação circunferencial (CS) do VE entre os doentes com CPNPC e os casos controlo. No entanto, todos os valores de deformação miocárdica VE diminuíram significativamente nos doentes tratados com o regime CP, resultando numa diferença significativa em comparação tanto com o grupo CV como com os casos controlo, embora não se tenham observado diminuições significativas nas medidas de strain no grupo VC. Conclusão: Apenas a PC, mas não a VC, pode induzir cardiotoxicidade subclínica em doentes com CPNPC, o que pode ser detetado por ecocardiografia de speckle tracking.

A quadricuspid aortic valve is a rare congenital anomaly whose diagnosis is mostly incidental. The overall incidence in the population is less than 0.00033% in autopsies and less than 0.0065% in transthoracic echocardiography. Most patients present with valvular pathologies, the most prominent of this being aortic regurgitation. The following case report presents a 60-year-old female patient with severe aortic insufficiency. Her symptoms were rapidly progressing and she had a clear indication for valvular replacement. In the preoperative transthoracic and transesophageal echocardiography there is no evidence of a four aortic cusp. However, during the surgical procedure a native aortic quadricuspid valve was found. During surgery a fourth aortic cusp is identified. The native aortic valve was exchanged for a biological prosthetic valve. Postoperative ecography demostrates a prosthetic aortic valve with normal function. A quadricuspid aortic valve is a rare anatomic variation that can result in aortic insufficiency due to mechanical valvular anomalies. [ABSTRACT FROM AUTHOR]
