2. IMPORTANCE OF HYPERBILURUBINEMIA IN DIFFERENTIATION OF PRIMARY AND SECONDARY HEMOPHAGOCYTIC LYMPHOHISTIOCYTOSIS IN PEDIATRIC CASES

Background and objective: Hemophagocytic lymphohistiocytosis (HLH) is a life threatening hyper inflammatory disease. It is difficult to differentiate between primary and secondary HLH based on clinical findings at the onset of disease. We aimed to find parameters that can help to differentiate primary and secondary HLH at initial diagnosis especially for physicians working in developing countries. Patient and Method: We retrospectively analyzed data of 38 HLH patients who were admitted to the Pediatric Hematology Department of Gaziantep University between January 2009 and December 2013. Results: Of 38 patients, 20 were defined as primary and 18 were secondary HLH. The average age of primary and secondary HLH patients was 31±9 and 81±14 months, respectively (p=0.03). We found consanguinity rates significantly higher in primary HLH patients compared to secondary HLH patients (p=0.03). We found that total and direct bilirubin levels significantly increased in primary HLH patients compared to secondary HLH patients (p=0.006, p=0.044). Also, CRP levels were found markedly increased in secondary HLH patients compared to primary ones (p=0.017). Conclusion: We showed that cholestasis and hyperbilurubinemia findings of HLH patients at the initial diagnosis should be considered in favor of primary HLH and increased level of CRP should be considered in favor of secondary HLH.

3. EVALUATION OF THE PLASMA MICRO RNA EXPRESSION LEVELS IN SECONDARY HEMOPHAGOCYTIC LYMPHOHISTIOCYTOSIS

Background: Hemophagocytic lymphohistiocytosis (HLH) is a life threatening hyper inflammatory disease. Micro RNAs (miRNA) are about 22 nucleotide-long, small RNAs encoded with genes, and they have regulatory functions in immune response. Objective: To determine the miRNA expression levels of 11 secondary HLH patients, we evaluated the associations of miRNA levels with pathogenesis, clinical presentation, and prognosis of the disease. Patients and Methods: Patients who were diagnosed with secondary HLH from January 2011 to December 2012 were included in this study. We profiled the expressions of 379 miRNAs in plasma of both HLH patients and healthy controls. Patients were evaluated regarding with age, clinical findings, miRNA expresions, laboratory data, treatment, and prognosis, by using descriptive statistics. Results: A total of 11 secondary HLH patients and 11 healthy children were included in this study. miR-205-5p was expressed in all case and controls and expression level of miR-205-5p was found 6.21 fold higher than control group (p=0.01). We detected the second highest expression percent in miR-194-5p with 81% of cases and controls. Expression level of miR-194-5p was found to have 163 fold higher than controls (p= 0.009). miR-30c-5p showed 77% expression percent in cases and controls together. The expression level of this miRNA was detected 9 fold decreased in HLH patients compared to healthy children (p= 0.031). Conclusion: We showed that miR-205-5p, miR-194-5p and miR-30c-5p could be useful plasma biomarkers for HLH. Further research is needed in larger and homogenous study groups, especially for these miRNAs as biomarkers for HLH.

4. Clinicopathological Evaluation of Childhood Sacrococcygeal Germ Cell Tumors: A Single-Center Experience

- Abstract
We aimed to evaluate the cases of sacrococcygeal germ cell tumors diagnosed in our hospital between 2006 and June 2021.We evaluated 38 sacrococcygeal germ cell tumors cases in our series in terms of age, sex, clinical complaints, localization, macroscopy, tumor size, histopathological diagnosis, surgical, postoperative complications, treatment, recurrence, and prognosis.The cases ranged from 1 day to 16 years of age; 14 cases were diagnosed with routine ultrasonographic examination during prenatal period while the rest of the cases most frequently presented with complaints of constipation. In terms of localization, 6 cases were type 1, 11 cases were type 2, 6 cases were type 3, and 15 cases were type 4. In the pathological evaluation, 25 cases were mature teratoma, 8 cases were immature teratoma, and 5 cases were pure yolk-sac tumor. In terms of complications, temporary colostomy was performed as a result of rupture during birth in 2 cases, disseminated intravascular coagulation at birth in 1 case, and colon injury in 2 cases. There was a recurrence in 2 of our cases. Thirty-seven of our cases were alive and 1 died. Alpha-fetoprotein level was high in 28 of our cases.In our series, type 4 cases were observed more frequently, contrary to the literature. We recommend to use a routine ultrasonography to patients who come to the clinic with complaints of constipation and inability to urinate and if a mass is detected, asking for alphafetoprotein for further follow-up. Sacrococcygeal germ cell tumors are ultimately a disease that can be successfully treated with multidisciplinary approach, accurate diagnosis in the antenatal and postnatal period, appropriate surgical intervention, and regular follow-up.

The main goal of the research is to determine the perception of managers working in food production companies in Eagean Region on the information on business analysis obtained from the human resources (HR) information system on information health. The survey form of the study was applied face to face interview or through e-mail to 63 managers working at food production companies in Aegean Region between March 2016-June 2016. According to research results, business analysis factors of data / information derived from HR information systems “leadership, motivation, communication, organizational relations, cooperation, audit of the work, mission, responsibility, jurisdiction, performance standards, mental and physical effort, way of doing work, technology equipment tools materials used in work, work information, to be open to learning, job related skills, relations with other works, education, working conditions and capabilities” effect information health positively. Efficient use of information obtained from HR information systems can provide strategic advantages to businesses.

Entrepreneurship is a concept with an increasing emphasis on the health sector. Mobbing is the most important formation that affects the motivation of health personnel in the negative direction. In this study, it is aimed to examine the effect of the mobing in the workplace on the entrepreneurship activities of the individual. Articles on entrepreneurship are searched by key words such as mobbing, entrepreneurship, in PubMed, Medline, EBSCOHost, PsycINFO, Turkish Medline, Cukurova Index Databases and Google Scholar and it has benefited from the work that is appropriate for review. Today, as in other fields, mobbing is also mentioned in the field of health and there is serious loss of workplace performance in health personnel exposed to mobinge. Individuals can have more devastating consequences than other types of stress. However, systematic violence within an institution may cause an individual to live anxiety, threat, insecurity about the future. This may jeopardize the entrepreneurship of the individual as it can affect the decision-making power, autonomy, in business life. In health institutions, administratively, initiatives to prevent mobiling can help to support the entrepreneurial traits of the staff and improve their performance.

Transformational leadership qualities are thought to be in harmonywith the relational management style of women managers. In this context, femalemanagers think that their job performance is increasing.

PubMedID: 30706992 Aim: Since the beginning of the Syrian civil war, more than 3.5 million Syrians have been under temporary protection status in Turkey. Because beta-thalassemia (BT) is a prevalent disorder in the Mediterranean countries, we decided to estimate the prevalence of and make an overview of the demographic, socioeconomic, medical characteristics, and healthcare problems of refugee children with BT. Patients: Eighteen Turkish Pediatric Hematology Oncology Centers (PHOC) with 318 refugee children from 235 families participated in the study. The mean age of the patients was 8.1 ± 4.8 years (0.5–21 years). The mean time after immigration to Turkey was 2.5 ± 1.5 years (range, 0.1–7 years). Seventy-two (22.6%) of them were born and diagnosed with BT in Turkey. On physical examination, 82 patients (26%) were underweight and 121 patients (38%) were stunted. The appearance of a thalassemic face was reported for 207 patients (65.1%). Hepatomegaly and splenomegaly were reported in 217 (68.2%) and 168 (52.8%) patients, respectively. The median ferritin level was 2508 ng/mL (range, 17–21 000 ng/mL) at the first admission, and 2841 ng/mL (range, 26–12 981 ng/mL) at the last visit after two years of follow-up in a PHOC (P > 0.05). The most frequently encountered mutation was IVSI-110 (G>A) (31%). Before immigration, only 177 patients (55.6%) reported the use of chelators; after immigration it increased to 268 (84.3%). Conclusion: Difficulties in communication, finding a competent translator capable in medical terminology, nonregular use of medications, and insensitivity to prenatal diagnosis were preliminary problems. The current extent of migration poses emerging socioeconomic and humanitarian challenges for refugee patients with BT. © 2019 Wiley Periodicals, Inc.

Introduction: Beta-thalassemia is the most common autosomal recessive disease. More than 200 differentmutations determined in beta-globin gene. Beta thalassemia disease has a severe clinical picture. Treatment of thediseases should be maintained properly otherwise quality of life and life period can be affected. In this study, we triedto reveal inherited beta-thalassemia mutations in surrounding areas of Gaziantep.Material and Methods: In this study, we included 208 patients who applied with beta -thalassemia suspicion to thepediatric hematology clinic (age range 4-14). In that study, 138 patients with homozygous mutations and 70 patients’heterozygous mutations were identified. HBB gene was sequenced by Sanger DNA Sequencing method.Results: Most common homozygote mutations are IVS I-110 G>A, IVS I-1 G>A, IVS II-1 G>A, the heterozygotemutations are IVS I-110 G>A, IVS II-1 G>A, codon 8 (AA) del. The incidence of IVS 1.110 (G>A) mutation is 24%, 28in homozygote patients, 29.7% in heterozygote patients. The 8% homozygote patients and 19.7% heterozygotepatients were effected with IVS 1.110 (G>A) mutation.Conclusion: The consanguinity marriages are very abundant particularly in our region due to fact that we are able tosee many autosomal recessive diseased in our region. Reducing incidence of autosomal recessive disease by givengenetic counseling could help the solution but it is still a major problem. J Clin Exp Invest 2016; 7(4): 265-268

With this study, it is aimed to review the innovative approaches in nursing education and practices. The articles on innovation in nursing, which are included in various databases, have been exploited by relevant key words and studies that are appropriate for oversight. Nursing services in order to provide planning, strategies and Innovation, the use of the service quality is among important factors that affect directly. Because nurses are responsible for knowing what they are doing and why they are doing while offering complex and important services such as patient care in the existing health care system. At the same time, nurses take responsibility for being a more efficient, higher-quality and cost-effective, when their services are given how and in which ways. Changing and evolving health care system, people basic health needs of people, and therefore also affect the nursing education. Nurses are required to keep pace with development and change in order to achieve effective and desired results when delivering services and to combine innovative approaches with their services. For this reason, the concepts of simulation, technology and knowledge should be integrated into the content of vocational training. Nurse managers should facilitate the creative efforts of teaching staff in their teaching and clinical practice and innovative programs.

11. Health services workers determination of the workplace stresses

This study was planned as descriptive of the purpose of detecting stressors caused by working environment of human resources working in the operating rooms. The data were collected between September-October 2016. The sample of the study consisted of 150 health personnel (doctors, nurses, technicians and technicians) working in the surgical departments and operating theaters. The average age of the health personnel participating in the study was 33.94 ± 8.37. 37% are nurses, 14.5% are physicians, 31.2% have been working for 6-10 years; 16.8% have a chronic disease. (39%), working in an artificial light environment and inadequate number of assistant personnel (37%), working as a stress source (49%), lack of rewarding (39% , 6) inadequacy of salary and supplementary payments, and the least stressful factor (12.1%) was the inadequacy of the knowledge level of the team. Positive work environment should be established in closed units such as the operating room.

Hemophilia A is an inherited, X-linked, recessive disorder caused by deficiency of clotting factor VIII. Central nervous system hemorrhage is an uncommon complication in patients with hemophilia. We report the case of a 5-year-old child who had intraspinal hematoma with nonsurgical, conservative management. It should be kept in mind that the optimal management is decided according to patient's condition because of high morbidity and mortality.

Visceral leishmaniasis is an infectious disease that infects and multiplies in macrophages of the liver, spleen, and bone marrow. The most common clinical features are fever, splenomegaly, and anemia. Anemia, leucopenia, and thrombocytopenia are the main hematologic abnormalities commonly seen in visceral leishmaniasis. These findings can be seen in several types of hematologic disorders. The findings are similar to most hematologic disorders and so may make diagnosis problematic. It is difficult to confirm when it is seen except in epidemiologic areas. It can be fatal if it is not treated and appropriate treatment can be lifesaving. In this article a 12 year-old male patient who was followed-up with diagnosis of acute lymphoblastic leukemia and received maintenance therapy while being under remission after BFM-ALL-2000 treatment protocol and diagnosed with hemophagocytic lymphohistiocytosis due to Kala-azar during this period was presented.

The results of our study confirm that entrepreneurs show signs of addictive behavior such as obsessive thoughts, withdrawal-condensation period, and negative emotion. Similar to the behavior as addictive as gambling or the Internet, serial entrepreneurs may face negative consequences resulting from the urge to continue. MicroRNAs (miRNAs) are small RNAs which are found in cells and circulation, and play a role in protein synthesis and regulation. In this study, we aimed to determine a biomarker for childhood ITP comparing the plasma miRNA levels of children having ITP with healthy children. A total of 86 patients with ITP and 56 healthy children followed up by the Department of Pediatric Hematology and Oncology in University of Gaziantep since July 2011 were enrolled in the study. The 86 patients with ITP were evaluated in two groups as 43 acute ITP (aITP) and 43 chronic ITP (cITP) patients. Plasma expression levels of 379 miRNAs were investigated by RT-PCR (quantitative RT-PCR) technique and they were compared between aITP, cITP, and control groups. For all miRNAs, the average of raw quantification cycle values of three groups separately in the analysis chip was accepted as the reference gene value, and normalization was done according to this value. Statistically significant differences were detected in seven miRNAs (miR-302c-3p, miR-483-5p, miR-410, miR-544a, miR-302a-3p, miR-223-3p, and miR-597) investigated between the groups with respect to the expression levels. The expression rates were found to be over 95% in miR-302c-3p and miR-483-5p, over 75% in miR-410, and over 40% in miR-544, miR-302a-3p, and miR-223-3p in all three groups. The detection of significant differences between plasma miRNA levels of aITP and cITP patients and healthy children may provide useful information in the prediction of the course of disease, determination of disease etiopathogenesis, and the development of new therapeutic modalities.

Despite the use of new broad-spectrum anti-bacterial and anti-fungal agents, infections still represent the major cause of morbidity and mortality in patients with prolonged neutropenia after intensive chemotherapy. The aim of this study is to assess the effect and safety of granulocyte transfusions (GTs) for the treatment of severe life-threatening infections in pediatric patients with febrile neutropenia. In this study, 13 pediatric patients with high-risk febrile neutropenia, who received 24 GTs, were included. GTs were well tolerated in all patients. Upon 24 h post-transfusion, neutrophil and platelet counts increased significantly, when compared to the baseline values. The clinical response and hematologic response rates were 69.2 % respectively. In conclusion, GT is safe and effective in controlling life-threatening infections. Furthermore, randomized controlled studies with long-term follow-up are needed to assess the exact role of GT in the outcome of patients with neutropenia.

The questionnaire prepared on this subject was applied between January 2015 - June 2015 to the 103 (30 opticianry graduate, 73 non opticianry graduate) optical store employees in Izmir. Within the scope of the research, it is expected that there is a correlation between the administrative applications in the optical stores and the business motivation of the optical store employees. These administrative applications that are adequate wage level, adequate benefits, adequate wage increase, unity of purpose, balance of authority and responsibilities, giving the initiative, to appreciate and financially reward, respectful behavior, ensuring the vocational training and development, provision of leadership, increase in provision charges based on performance, emphasis on employee health and safety, improving and increasing employee relations communications, increase the motivation level of employees in optical stores. If otherwise, the motivation level can be neutral or decrease. At the end of the research, it is found that four factors; personal rights (f1), supporting employees I (f2), supporting employees II (f3), employee relations (f4), effects business motivations in the positive way. These four factors affect the opticians' business motivation in the positive way more than non-optician employees. Also four factors have positive correlations at the medium-high level with each other.

Method: For this article, in international and national databases were reviewed. Findings: The harmony between the work to be done and the individual's personality is an important subject which needs a focus in terms of people and organizations. In the situation of unability to select appropriate individual to the work, the additional costs stem from the re-recruitment process, the time spent for adjustment process of new employee and the negative results of occupational-work accidents cause extra costs for organization. Furthermore there are different personality types. A type personalities are aggressive, impatient and business-oriented even B type personalities are experiences less conflict with other individuals and time and also they are more stable. Discussion: It is supported with scientific researches that there is a significant relationship between personality characteristics of employees and organizational variables such as job satisfaction, leadership, conflict management styles. Result: In order to create a positive atmosphere within the organization to employees, the regular training programs such as communication, dealing with stress, self-knowledge, and personality features are very important for the welfare of the organization.

Recently, entrepreneurship is emerging as an important concept in creating economic growth and employment. The human factor is the structure should be evaluated primarily on entrepreneurship. This study aimed to examine the relationship between entrepreneurial individual personality characteristics. Method: The articles that are related with entrepreneurship, personality, neurosis, extroversion were searched by key words in PubMed, Medline, EBSCOHost, PsycINFO, Science Direct, Turkish Medline, Cukurova Index Databases and in Google Scholar and those appropriate for this review were used by authors. Expected Results: Conclusion: Our study revealed the influence of individual differences on entrepreneurship. studies showing that impact on the behavior of personality traits, will be a guide to future entrepreneurs. Personality include sustainability aspects of individual's mental, emotional, social and physical. Meta-analytic studies showed that individual differences effect on entrepreneurial orientation. Entrepreneurs were determined to be risk-taking, innovative, high sense of responsibility, high communication skills individuals. Especially in biological studies, it was determined that extraversion and neurotic structure increase entrepreneurship.

Companies apply innovative activities to be different in their products and services, in the production and management processes, marketing approaches. In addition, innovative activities are important to increase the commercial performances of the export companies. In dried fruit production and world trade, Turkey has the comparative advantage. The share of exports in this sector of Turkey is around 11%. The share of dried figs and exported from Aegean Region is quite big in this success. Therefore, application in our study has been applied to manufacturing and exporting companies in dried fruit sector in Aegean Region. The aim of our study is to identify innovative activities and to determine the differences presence of some features of the companies in this sector related to innovation activities. Therefore questions on R&D department, patent, utility model, know-how, support projects have been asked to the dried fruit exporter companies and data on innovative activities of the companies were collected by questionnaire. The research has been applied to dried fruit manufacturing and exporting companies registered to Aegean Exporters' Union. Data was analysed using SPSS version 18.0. According to the results, it has been observed that companies involved in this research have high innovation average on product, process, marketing and organizational structure. It is concluded that the businesses without R&D department have more tendency in points of innovative activities. The businesses that not have patent, utility model, know-how are shown to be more tendency to a process innovation. At the end of the research in the 20 scope proposition collected in the eight factors; it was determined that within the framework of innovative features, women managers ownership of the mission and vision, care about career development (f1), to see opportunities, to be brave, to be open to learning (f2), to be conciliatory and to be convincing (f3), to be prescience and to be creative (f4), to be solution-oriented and to be open to innovation (f5), to care about cooperation, research and high communication (f6), to be emotional and abstract thinking (f7), high confidence and to be competitive (f8), affected business performances of them in the advanced positive way. Background and objective: Hemophagocytic lymphohistiocytosis (HLH) is a life threatening hyper inflammatory disease. It is difficult to differentiate between primary and secondary HLH based on clinical findings at the onset of disease. We aimed to find parameters that can help to differentiate primary and secondary HLH at initial diagnosis especially for physicians working in developing countries.Patient and Method: We retrospectively analyzed data of 38 HLH patients who were admitted to the Pediatric Hematology Department of Gaziantep University between January 2009 and December 2013.Results: Of 38 patients, 20 were defined as primary and 18 were secondary HLH. The average age of primary and secondary HLH patients was 31±9 and 81±14 months, respectively (p=0.03). We found consanguinity rates significantly higher in primary HLH patients compared to secondary HLH patients (p=0.03). We found that total and direct bilirubin levels significantly increased in primary HLH patients compared to secondary HLH patients (p=0.006, p=0.044). Also, CRP levels were found markedly increased in secondary HLH patients compared to primary ones (p=0.017).Conclusion: We showed that cholestasis and hyperbilurubinemia findings of HLH patients at the initial diagnosis should be considered in favor of primary HLH and increased level of CRP should be considered in favor of secondary HLH.

Background: Hemophagocytic lymphohistiocytosis (HLH) is a life threatening hyper inflammatory disease. Micro RNAs (miRNA) are about 22 nucleotide-long, small RNAs encoded with genes, and they have regulatory functions in immune response. Objective: To determine the miRNA expression levels of 11 secondary HLH patients, we evaluated the associations of miRNA levels with pathogenesis, clinical presentation, and prognosis of the disease. Patients and Methods: Patients who were diagnosed with secondary HLH from January 2011 to December 2012 were included in this study. We profiled the expressions of 379 miRNAs in plasma of both HLH patients and healthy controls. Patients were evaluated regarding with age, clinical findings, miRNA expresions, laboratory data, treatment, and prognosis, by using descriptive statistics. Results: A total of 11 secondary HLH patients and 11 healthy children were included in this study. miR-205-5p was expressed in all case and controls and expression level of miR-205-5p was found 6.21 fold higher than control group (p=0.01). We detected the second highest expression percent in miR-194-5p with 81% of cases and controls. Expression level of miR-194-5p was found to have 163 fold higher than controls (p= 0.009). miR-30c-5p showed 77% expression percent in cases and controls together. The expression level of this miRNA was detected 9 fold decreased in HLH patients compared to healthy children (p= 0.031). Conclusion: We showed that miR-205-5p, miR-194-5p and miR-30c-5p could be useful plasma biomarkers for HLH. Further research is needed in larger and homogenous study groups, especially for these miRNAs as biomarkers for HLH.

Although ACF is a minor anomaly, it should not be forgotten that it can be accompanied by major congenital anomalies and if this finding is detected, other anomalies should be investigated.

29. Investigation of Epidemiologic Factors in Children with Acute Lymphoblastic Leukemia

In this study, we aimed to investigate epidemiologic factors in children with Acute Lymphoblastic leukemia (ALL). The parents of 105 children diagnosed and treated as ALL between the years 1997 –2007 in our Clinic of Pediatric Hematology-Oncology were questioned and results were compared with control group that consisted of 102 healthy children with similar age and gender. The mean age of the patients was 8.57 ± 3.95 years. The mean age at diagnosis was 5.87 ± 3.73 years. There was no significant difference between the groups in terms of type of delivery, birth weight, asphyxia at birth, resuscitation with high-concentration oxygen and ventilatory support (p>0.05). There was also no significant difference between the groups in terms of duration of breast feeding. The rate of exposure to infections prior to diagnosis was higher in control group (p:0.003). Besides that, the rate of going to nursery was also significantly higher in control group (p:0.014). There was no difference between the groups in terms of X-ray exposure (p>0.05). In conclusion over-protection from infectious agents in and delay in meeting with some specific agents seems to increase ALL risk. Infections in early infantile period can be protective against leukemia. There has to be more extended studies on this subject.
