Aim: The aim of this study was to compare the effect of two calcium silicate-based and an epoxy resin-based root canal sealers on postoperative pain and analgesic intake following single-visit root canal treatment. Materials and Method: Ninety patients with at least one first or second molar tooth diagnosed as symptomatic irreversible pulpitis and symptomatic apical periodontitis were selected and allocated into three groups (n=30) according to the sealer used. Root canals were prepared using Protaper Gold instruments (Dentsply Sirona) in a crown down technique and irrigated with 2.5% NaOCl (Calyx, India) and saline solution. Root canal filling was then accomplished with a single cone obturation technique and treated in a single visit by the same endodontist. Patients were told to use a Visual Analog Scale (VAS) to rate their postoperative pain severity as none, minimal, moderate, or severe after 6 h, 24 h, 48 h, 5 days and 7 days following obturation using the appropriate sealers. The need for analgesic intake was also recorded. The data were statistically analyzed. Results: Results showed a significant difference among the studied groups. Bio-C Sealer Ion+ reported the least pain score followed by Nishika Canal Sealer BG and AH plus sealer at all the time intervals recorded. The intergroup analysis, revealed was a significant difference in postoperative pain at 6 h (p=0.000) and 24 h (p = 0.028), but not at 48 h, 5 day or 7 days (P > 0.05). VAS ratings for all the three groups decreased over time. Also, there were significant differences between the means of analgesic intake among 3 groups (p=0.022). Analgesic intake in group BIO-C Sealer Ion+ is significantly lesser than AH Plus and Nishika Canal Sealer BG group. Conclusion: Calcium silicate-based sealer (Nishika Canal Sealer BG and Bio-C Sealer Ion+) resulted in significantly lower levels of pain as compared to epoxy resin-based sealer (AH Plus) at 6h and 24-h interval, there was no significant difference in postoperative pain occurrence at 48-h, 5 day and 7-day period. Aim: The purpose of this study was to evaluate and compare the influence of ultrasonic and sonic activation of bio-ceramic and epoxy resin-based root canal sealer on the push-out bond strength of root dentine. Materials and Method: Ninety extracted human single rooted teeth with single canal were selected and decoronated at cemento-enamel junction. The working length was established using #10 K-file, keeping 1mm short of anatomical apex followed by canal preparation using ProTaper Universal till F3. Teeth specimen were randomly divided into three groups on the basis of sealer placement technique (n=30): Group I – Ultrasonic, Group II – Sonic and Group III – Lentulospiral (Control group). On the basis of the sealer used, each group was further divided into two subgroups (n=15): Subgroup A – Ceraseal and Subgroup B – Adseal. The obturation was done using F3 master cone and accessory cones with selected sealers, using lateral compaction technique. After incubation, each specimen was sectioned at 2mm, 5mm and 8mm from the root apex and subjected to push-out bond strength using Universal Testing Machine. Result: Among all the sealer placement methods, ultrasonic (136.18±85.74) gave the best result with both Ceraseal and Adseal, when compared to sealers placed using sonic activation (129.41±70.64) and lentulospiral (121.19±60.60). Regarding different root third, the apical third (207.33±55.28) showed best Push-out bond strength in comparison to middle third (79.90±55.28) and coronal third (99.55±50.87). Conclusion: Within the limitation of study ultrasonic activation of Ceraseal exhibited best push-out bond strength compared to sonic activation and lentulospiral. Pendahuluan: Kebocoran apikal dianggap sebagai penyebab umum kegagalan endodontik. Kebocoran dapat terjadi antara sealer dan gutaperca. Penelitian ini bertujuan untuk menganalisis perbedaan kebocoran mikro pada saluran akar dengan teknik pengisian single cone menggunakan dua sealer berbeda, sealer berbahan dasar zinc oxide eugenol dan sealer berbahan dasar epoxy resin. Metode: Jenis penelitian adalah analitik dengan desain eksperimental laboratorium. Obyek penelitian ini menggunakan 32 premolar rahang bawah. Objek dibagi secara acak menjadi 2 kelompok masing-masing terdiri dari 16 gigi. Kelompok I adalah pengisian saluran akar dengan sealer berbahan dasar zinc oksida eugenol dan kelompok II adalah pengisian saluran akar dengan sealer berbasis epoxy resin. Seluruh permukaan akar dilapisi dengan dua lapis cat kuku kecuali apikal 1 mm dan direndam dengan larutan metilen biru 2% selama 1 minggu pada suhu 37°C. Background/purpose: A perfect sealing of root canal system is essential for preventing ingress of bacteria from the oral environment. The purpose of this study was to evaluate the apical sealing ability of bioceramic (EndoSequence BC Sealer®) and epoxy resin-based (AH Plus®) sealers at 24 h, 7 days and 4 weeks. Materials and methods: Forty two extracted human upper anterior teeth were sectioned to leave the root 15-mm long, then all the roots were instrumented using a set of ProTaper® rotary instruments. Four roots were selected randomly as controls, and the remaining 38 roots were randomly divided into 2 groups of 19 roots each: group 1: EndoSequence BC Sealer® and gutta-percha, and group 2: AH Plus® and gutta-percha using a multiple wave condensation technique. The apical sealing ability of the filled root canal was measured using the fluid filtration method with 200 mmHg (26.67 KPa) above atmospheric pressure at 24 h, 7 days and 4 weeks. Background: The lateral canals are pathways for bacteria and their products to pass between the root canal and the periradicular tissue. Objectives: The present study aims to compare the filling ability of the lateral canals with three calcium silicate based sealers, including MTA Fillapex, Endoseal MTA and Sure-Seal Root, and AH26 epoxy resin sealer in the presence and absence of the smear layer. Material and methods: Six lateral canals were prepared using an engine reamer in 80 single-rooted human teeth. The root canals were cleaned, then the teeth were randomly divided into two groups. In group A, the smearlayer was removed using 17% EDTA and 5.25% NaOCl, and in group B, the canals were irrigated with normal saline. Groups A and B were divided into four subgroups each, according to the sealer used: A1, B1 (MTA Fillapex sealer), A2, B2 (Endoseal MTA sealer), A3, B3 (Sure-Seal Root sealer), and A4, B4 (AH26 sealer). Obturation of canals was conducted by the warm vertical technique and then teeth were incubated for 72 hours. Teeth were made clear and the filling of lateral canals were evaluated under stereomicroscope. Results: In group A, the highest lateral canals filling rate was in subgroup A2 and the lowest in subgroup A1, and the difference was statistically significant. However, in both groups, the highest lateral canals filling rate was in subgroups B2 and B3 and the lowest in subgroup B1. However, the difference was not statistically significant. In addition, no significant difference was observed in the lateral canals filling rate considering their position in the root. Conclusion: Providing the removal of the smear layer, calcium silicate-based sealers, Endoseal MTA and Sure-Seal Root had a higher ability to fill the lateral canals than AH26 and MTA fillapex sealers. The objective of this study was to evaluate the efficacy of reciprocating instruments in the removal of bioceramic and epoxy resin-based sealers using micro-CT analysis. Root canals of 40 extracted human teeth were instrumented with a size R25 Reciproc instrument. Specimens were randomly divided into two groups (n = 20) according to the root canal filling material. In the first group, root canals were obturated with AH Plus sealer and Reciproc R25 gutta-percha. In the second group, a combination of bioceramic gutta-percha (TotalFill BC) and bioceramic sealer (TotalFill BC) was used. After one week, the retreatment of all specimens was performed using Reciproc instruments. To analyze the differences in the filling remnants, specimens were scanned in a micro-CT device after obturation and after the retreatment procedure. Statistical analysis was performed using the Mann–Whitney U test (p < 0.05). A combination of bioceramic sealer and bioceramic gutta-percha was more effectively removed from canals using a reciprocating instrument, with a filling remnants volume of 4.01 ± 3.13 mm3, in comparison to the combination of epoxy resin-based sealer and gutta-percha (6.96 ± 2.70 mm3) (p < 0.05). A reciprocating instrument was more effective in removing bioceramic sealers than epoxy resin-based sealers, although none of the root canal filling materials were completely removed from the root canals.

This study evaluated the effect of residual substances derived from irrigant solutions and intracanal medications on the rheological properties of a silicone, gutta-percha, and bioactive glass–based sealer in comparison with an epoxy resin–based sealer. Thin films of contaminants (0.5 μL) (3% sodium hypochlorite [NaOCl], chlorhexidine digluconate [CHX], 17% EDTA, calcium hydroxide, 40% citric acid, or distilled water [DW]) were applied along with freshly mixed GuttaFlow Bioseal (GFB; Coltene/Whaledent, Langenau, Germany) or AH Plus Jet (Dentsply DeTrey, Konstanz, Germany) on the lower plate of a dynamic rheometer. Rheological measurements were performed in oscillating mode at the strain amplitude, angular frequency, and constant temperature of 0.01%, 10 rad/s, and 37°C, respectively. Samples without contaminants served as controls. All measurements were performed 5 times. The Kolmogorov-Smirnov test and 1-way analysis of variance followed by the Student-Newman-Keuls test were performed to analyze the results (P <.05). The contaminants did not alter the setting time of GFB compared with the controls; however, CHX and calcium hydroxide decreased its complex viscosity (ƞ*) (P <.05). The setting time of AH Plus Jet was increased by DW, CHX, citric acid, and EDTA (P <.05). All contaminants decreased its ƞ* at the setting point (P <.05). NaOCl led to the greatest alteration of its rheological parameters. Irrigant solutions and intracanal medications adversely affected at least 1 of the tested rheological properties of both sealers, except for DW and 3% NaOCl, which did not alter any rheological property of GFB. The aim of this study was to evaluate the biocompatibility of calcium silicate-based sealers (CeraSeal and EndoSeal TCS) and epoxy resin-based sealer (AH-Plus) in terms of cell viability, inflammatory response, expression of mesenchymal phenotype, osteogenic potential, cell attachment, and morphology, of human periodontal ligament stem cells (hPDLSCs). hPDLSCs were acquired from the premolars (n = 4) of four subjects, whose ages extended from 16 to 24 years of age. Flow cytometry analysis showed stemness of hPDLSCs was maintained in all materials. In cell viability test, AH-Plus showed the lowest cell viability, and CeraSeal showed significantly higher cell viability than others. In ELISA test, AH-Plus showed higher expression of IL-6 and IL-8 than calcium silicate-based sealers. In an osteogenic potential test, AH-Plus showed a lower expression level than other material; however, EndoSeal TCS showed a better expression level than others. All experiments were repeated at least three times per cell line. Scanning electronic microscopy studies showed low degree of cell proliferation on AH-Plus, and high degree of cell proliferation on calcium silicate-based sealers. In this study, calcium silicate-based sealers appear to be more biocompatible and less cytotoxic than epoxy-resin based sealers.

Introduction The aim of this study was to compare the effect of resin-based and bioceramic root canal sealers on the occurrence and intensity of postoperative pain in patients with asymptomatic apical periodontitis (AAP). Methods Patients presenting with AAP in previously endodontically treated teeth were included in this split-mouth blinded randomized controlled trial. For each patient, 2 single-rooted teeth were retreated and obturated using the warm vertical condensation technique and different obturation materials (ie, a gutta-percha point with resin-based sealer and a bioceramic-coated gutta-percha point with bioceramic sealer). Treatment of 1 root canal was performed in a single visit. Postoperative pain was recorded by a visual analog scale (VAS) at 24 hours, 48 hours, 72 hours, and 7 days after obturation. Results Of the 61 included patients, 57 individuals presenting 114 teeth completed the study. There was no statistically significant difference between the tested root canal sealers regarding postoperative pain at any time points assessed ( P > .05). In total, 20 (35%) patients perceived pain. Only 1 patient reported severe pain. VAS scores of 80 and 70 were reported in the AH Plus (Dentsply Maillefer, Ballaigues, Switzerland) and Total Fill (FKG Dentaire SA, La Chaux-de-Fonds, Switzerland) groups, respectively. Pain intensity decreased about 2-fold in both groups at 48 hours after treatment. There were no reports of pain since 72 hours after obturation. The odds ratio for pain occurrence in the lower premolars was 7.2 (95% confidence interval, 1.708–30.352) compared with the upper front teeth. Conclusions AH Plus and Total Fill perform similarly in terms of the occurrence and intensity of postoperative pain in teeth with AAP with no material extrusion beyond the apex. Objectives: The objective of the study was to compare the retreatability of three calcium silicate-containing sealers (BioRoot RCS, MTA Fillapex, Endo C.P.M.) and an epoxy resin-based sealer (AH Plus) with different root canal instruments (Hedström files, Reciproc R40, Mtwo retreatment file R 25/.05 + Mtwo 40/.06, and F6 SkyTaper) concerning sealer remnants and retreatment time.Materials and methods: Root canals of 192 teeth were instrumented with Reciproc R40. All root canals were obturated using the single-cone technique with Reciproc R40 gutta-percha and one of the sealers (n = 48 per sealer). Two months later, retreatment was performed using one of the mentioned instruments (n = 12 per instrument and sealer). The roots were split longitudinally, and both halves were investigated using light microscopy. The percentage of sealer remnants covering the root canal wall was evaluated using the software ImageJ. The time required for retreatment was recorded. Statistical analysis was performed using two-way ANOVA and Student-Newman-Keuls post hoc test.Results: Regarding the percentage of root canal filling remnants as well as retreatment time, two-way ANOVA indicated that the results were significantly affected by the sealer (p < 0.001) and by the instrument used (p < 0.05). Overall, the use of AH Plus was associated with significantly more remnants compared to all other sealers (p < 0.001) and F6 SkyTaper instruments allowed significantly faster retreatment than the other instruments (p < 0.05).Conclusion: The retreatability of calcium silicate-containing sealers was better compared to AH Plus as less sealer remnants and shorter retreatment times were observed. Retreatment with engine-driven NiTi instruments was superior compared to hand instrumentation.Clinical relevance: Engine-driven NiTi instruments are better suited to remove root canal fillings than stainless steel Hedström files. Aim: The objective was to evaluate the retrievability of bioceramic sealers in comparison with epoxy resin-based sealers assessed using microcomputed tomography., Materials and Methods: Four reviewers independently conducted a systematic literature search in four electronic databases. The risk of bias was assessed using modified Joanna Briggs Institute Critical Appraisal tools for Quasi-Experimental Studies., Results: Ten articles were included in this review and all had a moderate risk of bias. The results across the included articles were inconsistent with some indicating easier/difficult removal of bioceramic sealers, while others indicate no difference. The studies varied in terms of the type of bioceramic sealer used, root canal anatomy, and retreatment techniques followed.
