Introducción: el compromiso inmunoinflamatorio gingival en áreas interradiculares establece una lesión furcal. Sus terapéuticas tienen como objetivo mantener la salud, la función y la estética del diente. Objetivo: determinar las terapéuticas empleadas en lesiones furcales y la relación con su gravedad. Método: se realizó un estudio descriptivo, con historias clínicas dadas de altas en las consultas de Periodoncia y Estomatología General Integral de la Facultad de Estomatología, del municipio Plaza de la Revolución, entre enero 2009 y mayo 2011. Los 119 molares afectados en la furca conformaron el universo que se estudió en su totalidad. Variables estudiadas: grado de afectación furcal según la clasificación de Carranza y Newman, agrupados por gravedad y su tratamiento. Para la asociación y relación entre variables cualitativas fue utilizado el Chi cuadrado. Resultados: el colgajo con implante, la hemisección y la bicuspidización fueron terapéuticas pobremente representadas con 7,6%, 2.5% y 0,8% respectivamente, mientras que en las más aplicadas estuvieron: colgajo convencional (53,1%), raspado y alisado (26%) y la exodoncia (11,8%). El raspado y alisado (90,3%) obtuvo la supremacía para los grados I y II y la exodoncia (57,1%) para los grados III y IV. p= 0,0004. Conclusiones: las terapéuticas predominantes fueron: el colgajo convencional, el raspado y alisado radicular y la exodoncia. El raspado y alisado fue la terapéutica favorecida para los grados I y II y la exodoncia lo fue en los grados III y IV.Introduction: the destruction of the support tissues in inter-radicular areas of multi radicular tooth is known as furcation lesion. The goals of those therapies are maintaining the teeth health, function and esthetics. Objective: to determine the therapy employed in furcation lesions and its relation with their seriousness. Methods: a transversal study was carried out, with the dental records of patients discharged from the consultations of Periodontics and Integral General Dentistry of the Faculty of Dentistry of the municipality Plaza de la Revolución from January 2009 to May 2011 . The universe was formed by all the 119 molars registered with furcation lesions. The studied variables were: furcation lesion degree according to the Carranza y Newman classification, grouped by seriousness and treatment. The association and relationship among qualitative variables were carried out with Chi square. Results: the flap with implant, the hemisection and the bicuspidizatión were poorly represented with 7, 6%, 2.5% and 0, 8%, respectively while among the most applied ones were the conventional flap (53, 1%), root planning (26%) and the exodontia (11, 8%). Root planning obtained the supremacy (90, 3%) in the lesions grades I and II and the exodontia (57, 1%) was associated to grades III y IV. p= 0,0004 Conclusions: the predominant therapies were: the conventional flap, root planning and the exodontia. Root planning was the favored therapy for the grades I and II, while the dental extraction was the favored one in grades III and IV.

3. Long-term periodontal tissue outcome in regenerated infrabony and furcation defects: a systematic review.

Objectives: This study aimed to systematically evaluate the long-term results of periodontal tissues in regenerated infrabony and furcation defects. Material and methods: A search of the Medline database was performed (1960 to 2012), complimented by an additional hand search. For inclusion in the review, clinical trials had to include the long-term evaluation of infrabony and furcation defects submitted to periodontal regeneration for up to 3 years of follow-up. Changes in clinical attachment level (CAL) from pre-surgery, at the first post-surgical, and final evaluations were used to analyze the effects of regenerative treatments over time. Results: A total of 866 articles were found using the descriptors employed. Of these, 216 papers were selected for abstract reading by two evaluators. One hundred and eighty-nine were excluded for not fulfilling the eligibility criteria. Twenty-seven papers were selected for the analysis of the full texts, and 13 were excluded. Two studies were included after a manual search. Finally, 16 papers were selected for the present review. Conclusions: Regenerative techniques for the treatment of infrabony and furcation defects resulted in improved CAL during long-term follow-up periods, even when attachment losses occurred during this period. This improvement should be carefully evaluated with regard to its clinical relevance for dental longevity. Clinical relevance: The improvement in clinical attachment level achieved by regenerative techniques with biomaterials demonstrated satisfactory results over the long-term. However, regenerative techniques still present different success rates with regard to gain in clinical attachment level. [ABSTRACT FROM AUTHOR]

5. Prevalence of furcation involvements in patients referred for periodontal treatment.

The aim of this study was to evaluate the prevalence of furcation-involved molars in a patient sample referred for periodontal treatment. A total of 222 patients aged, 14-73 years (mean age 45 years), and with signs of destructive periodontal disease in at least 2 quadrants of the dentition were included in the study. The clinical examination involved assessments of oral hygiene status, gingival conditions, probing pocket depth and presence/degree of furcation involvement. In addition, a full mouth intraoral radiographic examination was performed. Results revealed that 4% of the patients presented with all 12 molars, while 3% had lost all molars. 52% of the individuals had at least 8 molars; 95% of subjects < 30 years of age and 19% in the age 60+ years. In patients aged ≥40 years, every 2nd molar showed advanced periodontal destruction in the furcation area. The prevalence of furcation involved molars was higher in the maxilla than in the mandible. From the age of 30 years, about 50% of the 1st and 2nd molars in the maxilla showed at least 1 furcation site with deep involvement, while in the mandible a similar prevalence was observed first after the age of 40 years. The highest frequency of furcation involvement was found at the distal site of the maxillary 1st molar (53%), and the mesial aspects of the maxillary 2nd molar showed the lowest frequency (20%). Furcation sites with a probeable trunk region were observed at a frequency of 17-22% at the various tooth sites. It was concluded that tooth morphology may be an important factor that accounts for the variability in prevalence of molar furcation involvement. [ABSTRACT FROM AUTHOR]
