Šio straipsnio tikslas – pristatyti retą klinikinį pakartotinio gimdos plyšimo po buvusios cezario pjūvio operacijos ir laparoskopinės gimdos miomos pašalinimo operacijos atvejį ir apžvelgti literatūrą, nagrinėjančią gimdos plyšimo nėštumo metu aktualumą, rizikos veiksnius, klinikinius požymius bei gydymą.

4. Vasa praevia: a case report and literature review.

Background. The aim of this article is to present a rare clinical case of vasa praevia as well as to assess the relevance of the problem by reviewing the latest literature sources. Materials and methods. In this report we present a case of a 33-yearold woman diagnosed with vasa praevia at 33 weeks of pregnancy, after hospitalisation with preterm rupture of membranes following the delivery of a live healthy baby through a lower segment Caesarean section during 33rd week of gestation at Vilnius University Hospital Santariškių Clinics. We investigated all the documentation of the patient before and after delivery. Results and conclusions. Vasa praevia is a rather rare pathology which is likely to occur during pregnancy, may result in heavy bleeding and be particularly threatening to the fetus life. A timely diagnosis for these women is essential. The gold standard for vasa praevia diagnosis is the fetal ultrasound scan. Vasa praevia pathology is found during the routine second trimester ultrasound check-up. The selection of proper tactics applied during pregnancy care is essential. At the gestational age of 28-32, it is advisable to mature fetal lungs as well as the fetus condition should be investigated by a perinatologist. The mode of delivery is the C-section which tends to reduce the frequency of possible complications. [ABSTRACT FROM AUTHOR]

Gimdymo takų traumas, kuomet reikalingas minkštųjų audinių siuvimas, patiria apie 50 proc. natūraliais takais gimdančių moterų. Lietuvoje po nekomplikuoto gimdymo natūraliais takais moterys stacionare praleidžia vidutiniškai 2–3 paras, tokio laiko ne visada pakanka įgyti gebėjimams pasirūpinti ne tik naujagimiu, bet ir prižiūrėti gimdymo takų žaizdas. Slaugos tęstinumas, sugrįžus gimdyvei iš stacionaro, ypač svarbus, siekiant išvengti tokių komplikacijų pogimdyminiu periodu kaip karščiavimas, infekcijos žaizdos srityje, žaizdos kraštų išsiskyrimas, dujų ar išmatų nelaikymas, fistulių susidarymas, skausmingi lytiniai santykiai ir šeimos planavimo atidėjimas ateityje. Tyrimo tikslas. Įvertinti moterų, patyrusių gimdymo takų traumų, slaugos tęstinumo svarbą. Tyrimo metodai: išanalizavus mokslinę literatūrą, nuo 2020 m. lapkričio iki 2021 m. sausio atliktas kiekybinis tyrimas, anoniminė apklausa, per pastaruosius dvejus metus natūraliais takais gimdžiusių ir patyrusių įvairių gimdymo takų minkštųjų audinių traumų, moterų apklausa. Tyrimo duomenys išanalizuoti „Microsoft Office Excel 2016“ duomenų apdorojimo programa. Tyrimas atliktas laikantis etikos principų. Tyrimo rezultatai: tyrime dalyvavo 110 moterų. Didžioji dalis respondenčių gimdė pirmą kartą ir patyrė I–II laipsnio tarpvietės ar makšties plyšimus. Daugiau nei pusė moterų akcentavo specialistų informacijos trūkumą apie gimdymo takų priežiūrą namuose, lytinį gyvenimą po gimdymo, tolimesnį šeimos planavimą. Išvados: natūraliais takais pagimdžiusios moterys, patyrusios minkštųjų audinių gimdymo takų traumų, pogimdyminiu laikotarpiu jaučia skausmą žaizdos srityje, kuris riboja judesius vaikštant, sėdint, keliantis iš gulimos padėties, jaučia nerimą ir baimę dėl minkštųjų audinių sužalojimo tuštinantis. Gimdyvės nepakankamai informuojamos apie gimdymo takų žaizdos priežiūrą namuose ir pageidautų tęstinės priežiūros pirminėje sveikatos priežiūroje., Around 50% of women who give birth naturally have trauma to the birthing tract requiring soft tissue suturing. In Lithuania, women spend an average of 2-3 days in hospital after an uncomplicated natural delivery, which is not always enough time to acquire the skills to take care of the new-born as well as to care for the delivery wound. Continuity of care after the return of the mother from hospital is particularly important to avoid complications in the postnatal period such as fever, wound infections, resolution of wound edges, incontinence of gases or faeces, formation of fistulae, painful sexual intercourse, and delay of future family planning. Aim of the study: to assess the importance of continuity of care for women with obstetric trauma. Methods: A quantitative study, an anonymous survey of women who have given birth naturally in the last two years and who have experienced various soft tissue injuries of the birthing tract between November 2020 and January 2021, following an analysis of the scientific literature, was conducted. The survey data were analysed using Microsoft Office Excel 2016 data processing software. The study was carried out in accordance with ethical principles. Results: 110 women participated in the study. Most of the respondents were first-time mothers with grade I-II perineal or vaginal tears. More than half of the women highlighted the lack of information from professionals about home care of the birth canal, postnatal sexuality, and family planning. Conclusions: Women with natural childbirth who have suffered soft tissue injuries to the birthing tract experience pain in the wound area in the postnatal period, which limits movement when walking, sitting, getting up from a lying position, and feel anxiety and fearof soft tissue injury when defecating. Mothers are not sufficiently informed about home care of the obstetric wound and would prefer follow-up care in primary care.

6. Pregnancy and delivery after augmentation enterocystoplasty: a case report.

Background. The aim of this article is to present a rare clinical case of pregnancy and surgical delivery after bladder augmentation surgery in the past. Methods and materials. In this report we present one case of a 23-yearold woman who became pregnant 3 years later after bladder augmentation surgery for congenital microcystis leading to hydronephrosis and nephrosclerosis and delivered a live healthy baby through a lower segment Caesarean section at 38 weeks of gestation in Vilnius University Hospital Santariškių Clinics. We investigated all documentation of the patient before and after bladder augmentation enterocystoplasty. Results and conclusions. Bladder augmentation is mostly performed for women in their childhood or young age and most of them reach reproductive age. There is no contraindications for these women to become pregnant and deliver, both vaginally or surgically. It is advisable to consult urologists regularly, make regular bacteriological urine analysis, take monthly blood samples for kidney function evaluation and perform renal ultrasonography. Multiple consultations in a Perinatology Center are also necessary to follow the fetus condition and prevent preeclampsia. Antibacterial treatment or prophylaxis should be used during pregnancy if necessary, intermittent self-catheterization is mostly performed routinely. The presence of a urologist is suggested during the Caesarean section. [ABSTRACT FROM AUTHOR]

Multiple sclerosis (MS) is a chronic disorder of central nervous system that is a major course of the disability of young adults. MS is twice as prevalent in females as in males, and has a typical onset between the ages of 20 and 40 years. Accordingly, MS most commonly affects women of childbearing age. Young women who have a chronic illness as MS often have increased concerns about pregnancy, the childbirth, and the future health. They may express worries about whether the illness or its treatment can affect the foetus or whether a child will inherit the disease. This article reviews the data from existing literature about gender and epidemiology, natural course of the disease, genetic factors, impact of oral contraceptives, diagnosis of the disease during pregnancy, effect of pregnancy on the course of MS, the influence of pregnancy on disability of MS, outcome of pregnancy, therapy during pregnancy (immunomodulators, immunosupresants, symptomatic therapy), labour and delivery, breast-feeding. Background: The European Union Statistics on Income and Living Conditions (EU-SILC) are increasingly used in demographic analysis, due to their large country coverage, the availability of harmonized socioeconomic measures, and the possibility to merge partners. However, so far there exists no comprehensive analysis of the representativeness of the fertility behavior reported by EU-SILC. Objective: This paper quantifies the quality of periodic fertility measures in EU-SILC. Methods: We compare periodic fertility measures obtained with EU-SILC to unbiased measures from the Human Fertility Database (HFD) for several European countries, by applying a cross-sectional perspective. Results: We show that EU-SILC measures of periodic fertility are biased downward, mainly due to attrition, while births of order one for ages 20‒29 are particularly underreported. However, we find no evidence of socioeconomic differentials in attrition. Conclusions: Our results suggest that for the majority of European countries, EU-SILC can be used for the analysis of childbearing behavior when respecting the measures of precaution mentioned in this article. Contribution: These precautions contain, for example, applying a retrospective approach and differentiating by rotation groups when calculating aggregate measures of periodic fertility.

Tyrimo tikslas yra įvertinti, ar moters savijauta nėštumo metu, subjektyvus gimdymo sunkumo vertinimas ir gimdymo metu gauta parama siejasi su dvejų metų amžiaus vaikų elgesio ir emociniais sunkumais ir ar motinos suvokiamas saviveiksmingumas gali būti tarpinis šios sąsajos veiksnys. Metodika. Tyrime dalyvavo 63 mamos ir jų vaikai. Duomenys apie nėštumo ir gimdymo psichologines aplinkybes buvo surinkti anketavimo būdu, 2–3 dieną po gimdymo, motinų saviveiksmingumas vertintas Motinos saviveiksmingumo skale vaikui sulaukus 1,5 m. amžiaus, o dvejų metų amžiaus vaikų elgesio ir emociniai sunkumai vertinti Vaiko elgesio aprašu. Rezultatai atskleidė, kad vaikai, gimę emociškai nepalankaus gimdymo metu, motinų manymu, turi daugiau tiek emocinių, tiek elgesio sunkumų. Prielaida, kad saviveiksmingumas yra tarpinis šios sąveikos veiksnys, nepasitvirtino. Išvados. Psichologiniai nėštumo ir gimdymo veiksniai yra susiję su motinų savo dvimečių vaikų emocinių ir elgesio sunkumų vertinimais. The aim of this study is to examine associations between pregnancy and childbirth psychological factors and 2-year-old children’s behavioural and emotional problems and to assess whether maternal self-efficacy could be a mediator of this association. Methods. Mothers and their 63 children were involved in the study. The data about psychological factors of pregnancy and childbirth were collected on 2-3rd day after delivery. The Child Behavior Checklist (CBCL/1½-5) was used for the assessment of behavioural and emotional problems of 2-year-old children. The maternal self-efficacy measured and analysed with The Parental Efficacy Questionnaire. Results revealed that psychological factors of pregnancy and childbirth are significantly associated with 2-year-old children’s emotional and behavioural problems. The assumption that maternal self-efficacy could mediate the relationship of the aforementioned association was not proved. The effect of psychological factors of pregnancy and childbirth on children’s emotional and behavioural problems remains significant after adjusting sociodemographic factors, mode of delivery, and maternal self-efficacy.
