5. Nuevos conceptos en la fisiopatología de la hipertensión arterial. Receptores purinérgicos.

- Abstract
Hypertension is a major risk of morbidity and mortality in patients when it is uncontrolled. In spite of improved therapies currently available for blood pressure control, their complications are far away from being accomplished. Therefore, chronic renal failure is frequently observed in hypertensive patients. Thus, insights on mechanisms that may contribute to arterial pressure control should be studied to prevent life-threatening cardiovascular disorders. Purinergic receptors have been recognized in the physiopathology of hypertension; this review summarizes their participation in the renal abnormalities of the kidney in hypertension. Several studies have suggested the activation of renal purinergic receptors under an elevated interstitial ATP milieu as a fundamental pathway that leads to generation and maintained hypertension. Elevated ATP concentration alters fundamental mechanisms involved in the long-term control of blood pressure such as pressure natriuresis, autoregulation of glomerular filtration rate and renal blood flow, as well as increased tubule-glomerular feedback responses, overall, these alterations decrease sodium excretion; in addition, the expression of ATP receptors is modified. Under a genetical background, ATP induces the production of vasoactive compounds, decreases renal function and induces tubulointerstitial injury before glomerular damage. Simultaneously, a deleterious interaction between angiotensin II and purinergic receptors lead to the progression of renal damage. [ABSTRACT FROM AUTHOR]

This review provides an overview of the efficacy and safety of renal sympathetic denervation as a therapeutic approach for resistant hypertension. While the initial enthusiasm was sparked by the results of early clinical trials, it was dampened by the findings of the Symplicity HTN-3 study. However, recent advances in catheter technology and more refined patient selection criteria have yielded more promising results. Subsequent studies, such as SPYRAL HTN-OFF MED and RADIANCE II, demonstrated significant reductions in blood pressure, even in patients with mild to moderate hypertension. Despite the lack of robust data on major clinical outcomes, investigations into the time in therapeutic range for patients undergoing renal sympathetic denervation suggested potential cardiovascular benefits. Nevertheless, further research is needed to thoroughly understand the long-term impact, assess cost-effectiveness, and accurately identify which patient subgroups may derive the greatest benefits from this therapy. 12. Determinación social de la obesidad, la diabetes y la hipertensión arterial desde las narrativas de mujeres de una comunidad indígena en el sur de Morelos, México.

- Abstract
Objetivo : comprender los procesos críticos (PC) de determinación social de la obesidad, la diabetes y la hipertensión (ODH) en una comunidad nahua de México. Metodología : estudio cualitativo de registros de un taller de fotovoz, donde las participantes fotografiaron su entorno y analizaron las causas y posibles soluciones a la ODH. Para analizar los PC de la ODH utilizamos como método la investigación narrativa y, como referente teórico, la epidemiología crítica. Resultados : la ODH se reproduce social e históricamente a través de PC destructivos vinculados con las relaciones de producción global y de género. Estas determinan modos de vida deteriorantes que limitan la atención a la salud, comprometen la salud mental, producen contaminación y diferenciación de uso de espacios, y reducen oportunidades para alimentarse nutritivamente y realizar actividad física. Todo ello se expresa como ODH y problemas de salud mental. Los PC protectores ante estas expresiones incluyen la atención estatal, las oportunidades de trabajo, y la promoción de dispositivos culturales y comunitarios. Conclusiones : nuestros resultados aportan a la discusión global sobre cómo las condiciones históricas de vida son parte de la determinación social de la ODH. Comprender los PC y sus expresiones locales puede orientarnos hacia la descolonización de la forma de pensar y hacer promoción de la salud. [ABSTRACT FROM AUTHOR]

La hipertensión sigue siendo un importante desafío de la salud pública en todo el mundo porque aumenta el riesgo de enfermedad cardiovascular. Se ha demostrado que el manejo eficaz de la presión arterial reduce la incidencia de Acv, ataque cardiaco e Insuficiencia cardiaca. La prevalencia actual en nuestro país de Hta es del 36,3%. En el estudio Renata 2 cuatro de cada diez pacientes Hta, desconocen su condición. Esta observación no es ajena a lo que sucede globalmente. En el estudio Pure, solo se logra controlar al 13% de los hipertensos. El 30% tuvo tensión arterial óptima y el otro 30% se encontraba en prehipertensión Del 40%de los pacientes con Hta, el 40% recibían tratamiento, pero solo el 13% tenían controlada su enfermedad. Este trabajo de investigación que se realiza en el Servicio de Cardiología del Hospital Nacional de Clínicas de la ciudad de Córdoba, tiene como objetivos evaluar la prevalencia de Hta y frecuencia de pacientes controlados que concurren a consultorio externo. de Cardiología entre el mes de abril de 2021-febrero 2022. Los datos serán obtenidos de una muestra al azar mediante la utilización de historias clínicas entre el mes de abril 2021-febrero 2022.Un año de Pandemia. La hipótesis el comprobar el insuficiente control de la hipertensión arterial más allá de lo encontrados en los resultados de los estudios antes mencionados, en nuestra población de la Ciudad de Córdoba, y en un año totalmente diferente, un año de Pandemia. En donde el acceso a la salud se encuentra limitada por el COVID 19.

14. Comportamiento de la Hipertensión arterial en el consultorio 5 de Puerto Padre

Introducción: la hipertensión arterial es una enfermedad crónica que a su vez constituye un factor de riesgo para la derivación en enfermedades cardiovasculares, reconocida como una de las principales causas de decesos en los últimos años. Objetivo: describir el comportamiento clínico-epidemiológico de la hipertensión arterial en el consultorio 5. Métodos: se realizó un estudio observacional, descriptivo, transversal en 342 pacientes hipertensos del CMF 5 de Puerto Padre durante el primer cuatrimestre del 2023. La información se obtuvo del Análisis de la Situación de Salud, de las historias clínicas y de un cuestionario elaborado por los autores. Las variables estudiadas fueron: factores de riesgo no modificables, comorbilidades, factores de riesgo modificables, tratamiento médico preventivo, tratamiento farmacológico, adherencia al tratamiento, complicaciones. En el procesamiento de la información se empleó la estadística descriptiva. Resultados: predominaron el sexo femenino y el grupo etario comprendido entre 65-74 años. El 82,75 % tenía antecedentes familiares de HTA. La diabetes mellitus y obesidad fueron las comorbilidades presentes con predominio en las mujeres. En los hombres predominó el tabaquismo. El 75,15 % cumplía el tratamiento farmacológico y un bajo porciento llevaba a la práctica acciones encaminadas a un estilo de vida saludable. Entre las complicaciones se identificaron la cardiopatía isquémica y la insuficiencia cardíaca con mayor número de afectados. Conclusiones: se describió el comportamiento de la HTA en el consultorio 5 donde es necesario el accionar de los profesionales del sector para elevar la responsabilidad individual y colectiva.

Contexto: el síndrome metabólico (SM) se define como un conjunto de factores de riesgo que aumentan la probabilidad de padecer enfermedad cardiovascular. El síndrome poscovid es una complicación a largo plazo de la infección por COVID-19, definida por la Organización Mundial de la Salud (OMS) como la perpetuación de los síntomas de la enfermedad aguda más allá de 12 semanas sin otra causa aparente. La asociación entre las enfermedades cardiometabólicas y las complicaciones de la infección aguda por SARS-CoV-2 ha estado clara desde el principio, no siendo así con el síndrome poscovid. Objetivo: determinar la asociación existente entre la presencia de enfermedades cardiovasculares en el momento de la infección aguda por SARS-CoV-2 y el desarrollo de síndrome poscovid. Metodología: estudio observacional de cohortes y retrospectivo, con un total de 278 pacientes, 140 en la cohorte expuesta y 138 en la cohorte no expuesta, para establecer la asociación entre la presencia de enfermedades cardiovasculares en la fase aguda de infección por COVID-19 y el síndrome poscovid. Resultados: en la cohorte expuesta, el 74,1?% presentó el síndrome poscovid, mientras que en la cohorte no expuesta el 25,9?% presentó dicho desenlace. Luego del análisis multivariado, se estableció una asociación estadísticamente significativa entre el diagnóstico de síndrome metabólico en la fase aguda de la infección por COVID-19 y el desarrollo de síndrome poscovid, con un RRa de 3,18, IC 95?% 1,42-7,14; p = 0,005. Conclusiones: se logró establecer una asociación clara entre la presencia del síndrome metabólico en la fase de infección aguda por COVID-19 y la aparición de síndrome poscovid.

16. Predictive model of macrovascular complications at the time of diagnosis of patients with type 2 diabetes

Introduction: Cardiovascular disease is the main cause of mortality in people with type 2 diabetes mellitus. Objective: Design a predictive model of macrovascular complications in patients with type 2 diabetes at the time of diagnosis. Methods: An observational, of cases and controls, retrospective study was carried out. The population studied were all patients diagnosed with type 2 diabetes mellitus in the "José Martí" health area of the province of Santiago de Cuba, during the period from September 2018 to December 2022. 40 cases with macrovascular complications and 80 controls were studied. randomly selected from the same population, without complications. To build the model, binary logistic regression was applied and those that were associated with macrovascular complications in a previous bivariate analysis were used as independent variables. Results: The variables that were included in the model, after the binary logistic regression analysis, were arterial stiffness, hyperlipidaemia, and arterial hypertension. Hosmer-Lemeshow test= 6.027; p= 0.644. The sensitivity of 77.50 % and specificity of 90.00 %; with a positive predictive value of 79.49 and negative predictive value of 88.89; and validity index equal to 85.83. The observed area under the Receiver Operating Characteristic curve is 0.933, with a significance associated with the calculated statistic of 0.000. Conclusions: The designed model is a good predictor of macrovascular complications in patients with type 2 diabetes at the time of diagnosis.

This study addresses arterial hypertension (AHT) as a critical public health problem in Ecuador; it examined its epidemiology, diagnosis, treatment and risk factors. A literature and documentary review methodology was used to analyze publications between 2018 and 2024, which included scientific articles, books, and theses available on recognized platforms such as Google Scholar, PubMed, ScienceDirect, and Dialnet. This method ensured the accuracy and relevance of the information by applying strict inclusion and exclusion criteria, with a specific focus on studies on epidemiology, diagnosis, treatment, and risk factors of arterial hypertension in Ecuador. The findings reveal significant variability in the prevalence of HT among different regions and demographic groups, highlighting the need for culturally tailored management and treatment approaches. The importance of early detection and effective management of HT was highlighted to prevent serious complications in vital organs and reduce associated mortality. The conclusions underscore the urgency of improving diagnostic and treatment strategies, integrating both pharmacological interventions and lifestyle modifications. It identifies the need for more personalized and culturally sensitive public health policies to address variability in the prevalence and management of HTN. The study also recommends strengthening public education and awareness of HT to encourage earlier detection and effective management, with a view toward future research that will delve deeper into the causes of the observed disparities and develop more effective intervention methods that consider genetic and environmental factors. 18. Prevalence of secondary arterial hypertension in patients with acute renal failure in a secondary-level pediatric hospital in Northwestern Mexico.

Background: The worldwide prevalence of arterial hypertension in pediatric patients is 3.5%, and it has repercussions at renal, cardiovascular, neurological, and lifestyle levels. This study aimed to estimate the prevalence of arterial hypertension, mortality, and follow-up in patients with acute renal failure in the nephrology outpatient clinic at a second-level hospital in Northwestern Mexico. Methods: We conducted a descriptive, retrospective, and observational study. Men and women aged 1-18 years diagnosed with acute kidney injury were analyzed from January 1, 2012, to December 31, 2021. The medical and electronic records of the candidate patients were analyzed, and nutritional data, laboratory analysis, most frequent etiology, and follow-up in the pediatric nephrology clinic were collected. Those with exacerbated chronic kidney disease and previous diagnosis of high blood pressure were excluded. Results: One hundred and seventy-four patients were evaluated, and only 40 were eligible for the study (22.98%), predominantly males with a mean age of 9.9 years. The degree of arterial hypertension was 50% for grade I and 50% for grade II (p = 0.007); the mortality rate was 32%. One hundred percent of hypertension cases were controlled at 6 months after discharge (p = 0.000080). Conclusions: Our results were similar to those reported in other studies. Follow-up and early detection of arterial hypertension in children need to be strengthened. [ABSTRACT FROM AUTHOR]

21. Relación del Sobrepeso y obesidad con la hipertensión arterial en personal administrativo y académico de la Universidad Salvadoreña Alberto Masferrer.
- Abstract
Currently, overweight and obesity have become public health problems worldwide, since they are conditions that cause the population to develop other diseases such as Diabetes mellitus and arterial hypertension. This research was carried out with the purpose of identifying the relationship between obesity and overweight, with arterial hypertension and demonstrating the degree of correlation between both variables, as well as its intensity. Simple linear regression analysis was performed to confirm this relationship and predict the behavior of the dependent variable. These results allowed us to propose strategies to prevent chronic diseases in employees. Materials and methods: A descriptive observational study with a correlational scope was carried out at the Universidad Salvadoreña Alberto Masferrer. The population consisted of 240 employees; the sample was made up of 146, calculated in Open Epi at a 95% confidence interval. The participants were selected by applying non-probabilistic sampling by quotas. A questionnaire, taking anthropometric measurements and taking blood pressure were used as a data collection instrument. For data analysis, descriptive statistics were used such as frequencies, percentages, mode and arithmetic mean, normality tests and inferential statistics, obtaining that, for Spearman’s correlation, the value of R=0.063 with p=0.780 > 0.05, therefore, there is a positive correlation between the variables of overweight, obesity and arterial hypertension. Objective: We conducted a cross-sectional study to identify, characterize, and compare the presence of cognitive impairment in patients with diabetes mellitus (DM) and patients with high blood pressure (HBP) who were seen in primary care. Methods: Consecutive patients over the age of 65 with DM or HBP who attended outpatient consultation at the Family Medicine Unit No. 51 of the Mexican Institute of Social Security in Guadalajara were included in the study. The Montreal Cognitive Assessment (MoCA) test was applied to detect the presence of cognitive impairment. Results: A total of 177 patients (59.9% men) with DM (59.9%) or HBP (40.1%) were included in the study. It was found that 44.1% of the participants had cognitive impairment. Conclusions: The prevalence of cognitive impairment in older adults with DM or HBP is higher than in older adults without these comorbidities. The use of a quick and easy screening test such as the MoCA test in this group of patients could allow for early detection and impact on timely referral, approach, and management. Introducción: En Latinoamérica uno de cada tres adultos padece de hipertensión arterial. En Ecuador se reporta como la tercera causa de muerte en el año 2019. En tal virtud, es importante intervenir en los factores de riesgo modificables de salud por medio de la atención primaria. Sin embargo, se evidencia en los pacientes la falta de adherencia a las prácticas preventivas en salud y recomendaciones médicas prescritas. Objetivo: Evaluar la efectividad de una estrategia educativa en salud para mejorar la adherencia terapéutica en los pacientes hipertensos del centro de salud N° 3 del cantón Riobamba. Métodos: El presente estudio se basa en un diseño cuasi experimental, analítico – sintético, deductivo, con una población de 140 pacientes hipertensos pertenecientes al centro de salud N°3 del cantón Riobamba, provincia de Chimborazo, diciembre 2021 – febrero 2022. Resultados: Los resultados del primer proceso (pre-test) reportan: 9 pacientes (6.40%) con buena adherencia al tratamiento prescrito. Posterior a la intervención educativa en el segundo proceso (post-test); se evidencia un aumento significativo a 96 pacientes (68.60%) con buena adherencia al tratamiento frente a esta patología. Conclusiones: La implementación de una estrategia educativa, resulta efectiva para incrementar la adherencia al tratamiento farmacológico, asistencia a control médico y cambio en el estilo de vida en los sujetos de estudio.

Primary hyperaldosteronism (PAH) is an important cause of secondary hypertension (HTN). The study of the same requires a high clinical suspicion in addition to a hormonal study that confirms hormonal hypersecretion. It is important to start the appropriate treatment once the diagnosis is confirmed, and for this is necessary to demonstrate whether the hormonal hypersecretion is unilateral (patients who could be candidates for surgical treatment) or bilateral (patients who are candidates for pharmacological treatment only).At the Hospital del Mar since 2016 there has been a multidisciplinary work team in which Nephrologists, Endocrinologists, Radiologists and Surgeons participate to evaluate cases with suspected hyperaldosteronism and agree on the best diagnostic-therapeutic approach for these patients, including the need for adrenal vein sampling, which is a technique that in recent years has become the gold standard for the study of PAH.In the present study we collect the experience of our centre in performing AVC and its usefulness for the management of these patients. Resumen: El hiperaldosteronismo primario (HAP) es una causa importante de hipertensión arterial (HTA) secundaria. El estudio del mismo precisa de una alta sospecha clínica además de un estudio hormonal que confirme la hipersecreción hormonal. Es importante iniciar el tratamiento adecuado una vez se confirma el diagnóstico y para ello es preciso demostrar si la hipersecreción hormonal es unilateral (pacientes que podrían ser tributarios a tratamiento quirúrgico) o bilateral (pacientes que son tributarios a tratamiento únicamente farmacológico).En el Hospital del Mar desde el año 2016 existe un equipo de trabajo multidisciplinar en el que participan Nefrólogos, Endocrinólogos, Radiólogos y Cirujanos para evaluar los casos con sospecha de hiperaldosteronismo y consensuar el mejor abordaje diagnóstico-terapéutico de estos pacientes, incluyendo la necesidad de cateterismo venoso adrenal que es una técnica que en los últimos años se ha erigido como gold standard para el estudio de HAP.En el presente estudio recogemos la experiencia de nuestro centro en la realización de CVA y en la utilidad de este para el manejo de estos pacientes.

Circadian blood pressure variations are classified into different phenotypes that provide additional knowledge about patients’ morbidity and prognosis of patients. Ambulatory blood pressure monitoring (ABPM) is the ideal tool to obtain a record of these variations. The etiology of non-dipper hypertension is multifactorial, including dysregulations in the autonomic nervous system, neuroendocrine axes, and mineral turnover system, with complex pathophysiological behavior. There are many associations between altered circadian phenotypes (such as the non dipper phenotype) and adverse cardiovascular outcomes such as cardiovascular mortality, coronary and cerebrovascular disease, chronic kidney disease, and heart failure. Introduction: The ischemic cerebrovascular disease has a high frequency due to the population aging mainly. Objective: To compare clinical characteristics of patients with ischemic cerebrovascular of two age groups. Methods: A descriptive, cross-sectional, retrospective study was carried out in the Neurology and Neurosurgery Institute in Havana, from January to December, 2017 in patients with ischemic cerebrovascular disease; 36 individuals of both age groups. In this regard, demographic variables, risk factors, clinical manifestations, coma scale and neurological deficiency, etiology and localization of the ischemic ictus were analyzed. Results: The 65 years group had a significant increase of hypertensive patients (88.9%). The average of the National Institute of Health stroke scale was superior in these patients (median [10-90 percentile]: 9.5 [4-19]). There was statistical increment of over 65 years patients with partial paralysis of the look and ataxia, but monoparesis and visual extinction in the age under 65 years. Such a scale had a statistical increase in the atherothrombotic and cardioembolic ictus in comparison with other etiologies in both patient groups. The over 65 years patients with just one risk factor or and those with hypertension had a higher punctuation of the scale. Conclusions: The degree of neurological affectation was higher in over 65 years patients that had a risk factor and in those with hypertension. As a result it could be suggested that the molecular and pathophysiolologic mechanisms of these patients vary with the age. [ABSTRACT FROM AUTHOR]

Introducción: Los receptores purinérgicos participan activamente en las alteraciones renales que ocurren en la hipertensión arterial. La elevación del ATP en el intersticio renal, activa los receptores purinérgicos, lo que constituye una vía fundamental en la generación y la persistencia de hipertensión arterial. Objetivo de la revisión: Este artículo es una revisión narrativa con el objetivo de mostrar la importancia de los nuevos conceptos en la fisiopatología de la hipertensión arterial. Puntos importantes de la revisión: Los receptores purinérgicos son receptores de ATP, a mayor ATP hay sobreexpresión de receptores purinérgicos. Las concentraciones elevadas de ATP alteran mecanismos fundamentales relacionados al control de la presión arterial, como el mecanismo de natriuresis de presión y la excreción renal de sodio, la regulación de la filtración glomerular y el mecanismo de retroalimentación túbulo-glomerular, la alteración de estos mecanismos disminuye la excreción urinaria de sodio. Ya sea bajo la influencia de alteraciones genéticas, o bien, inducida por péptidos vasoconstrictores, se produce una disminución de la función renal y lesión tubulointersticial antes de que se lesione el glomérulo. La interacción entre la angiotensina II y los receptores purinérgicos favorece la progresión del daño renal. Conclusión: El daño renal en hipertensión arterial es muy importante, por lo que se debe tratar de controlar la hipertensión para evitar la progresión del daño renal que llega a producir insuficiencia renal. El desarrollo de antagonistas de los receptores purinérgicos para su uso en clínica es una posibilidad para obtener un mejor control de la presión arterial.

INTRODUCION: La enfermedad aterosclerótica es una enorme carga para la salud en todo el mundo, y su prevención está basada en gran medida en el control de los factores de riesgo tradicionales, a pesar de la eficacia limitada en la prevención de eventos cardiovascular (ECV). La mejoría en la estratificación del riesgo se puede lograr a través de la detección de progresión del área total de la placa (TPA) medida por ecografía carotidea, la cual ha demostrado duplicación del riesgo basal en estos pacientes en un intervalo de 1 año. Nuestra hipótesis es que el control de la presión arterial y el colesterol LDL sérico en valores objetivo (según guías clínicas actuales) son insuficientes para reducir la progresión de la aterosclerosis en personas sin eventos previos, con alto riesgo cardiovascular. METODOS Y RESULTADOS: Estudio observacional prospectivo de 742 participantes con alto riesgo cardiovascular en un programa de prevención primaria cardiovascular. Se determinaron dos mediciones de ultrasonido de TPA para cada participante durante al menos un año. Incluimos en el análisis 57 participantes que mantuvieron una presión arterial por debajo de 130/80 mmHg y un colesterol sérico de lipoproteínas de baja densidad (LDL-C) por debajo de 100 mg/dl durante todo un año. Los participantes con progresión de la placa de TPA definida como aumento sobre el basal mayor de 5 mm2 se compararon con aquellos con cambios de TPA de 5 mm2 o menos (grupo sin progresión) mediante análisis de regresión logística multivariable. Después de una media de estadía en programa de casi 8 años, con Presión Arterial de 120.4 + 9/68.6 + 8 mmHg y LDL colesterol de 81 + 25 mg/dl, identificamos progresión de TPA en 22 de 57 (39%) participantes. No detectándose diferencias para ninguna covariable al comparar progresión versus no progresión. CONCLUSIÓN: La progresión de TPA ocurre hasta en el 39% de los individuos a pesar de mantener la presión arterial por debajo de 130/80 y el LDL-C sérico por debajo de 100 mg/dl. La evaluación de TPA puede ayudar a resolver las limitaciones de las pautas establecidas para la prevención de eventos de ECV en personas de alto riesgo.

Introduction: The ischemic cerebrovascular disease has a high frequency due to the population aging mainly. Objective: To compare clinical characteristics of patients with ischemic cerebrovascular of two age groups. Methods: A descriptive, cross-sectional, retrospective study was carried out in the Neurology and Neurosurgery Institute in Havana, from January to December, 2017 in patients with ischemic cerebrovascular disease; 36 individuals of both age groups. In this regard, demographic variables, risk factors, clinical manifestations, coma scale and neurological deficiency, etiology and localization of the ischemic ictus were analyzed. Results: The 65 years group had a significant increase of hypertensive patients (88.9%). The average of the National Institute of Health stroke scale was superior in these patients (median [10-90 percentile]: 9.5 [4-19]). There was statistical increment of over 65 years patients with partial paralysis of the look and ataxia, but monoparesis and visual extinction in the age under 65 years. Such a scale had a statistical increase in the atherothrombotic and cardioembolic ictus in comparison with other etiologies in both patient groups. The over 65 years patients with just one risk factor or and those with hypertension had a higher punctuation of the scale. Conclusions: The degree of neurological affectation was higher in over 65 years patients that had a risk factor and in those with hypertension. As a result it could be suggested that the molecular and pathophysiolologic mechanisms of these patients vary with the age.

Se estudia el caso de una paciente femenina de 33 años, conocida hipertensa desde los 17 años, con reciente tratamiento antihipertensivo, con historia de dos gestas sin antecedente de preeclamsia, quien es remitida de la consulta de medicina familiar y comunitaria al tercer nivel de atención por hipertensión resistente a tratamiento y quien refiriendo cefalea ocasional, edema periférico matutino y palpitaciones autolimitadas, así como una hipokalemia leve e intermitente, por lo cual la paciente se encontraba en tratamiento con 4 medicamentos antihipertensivos. Debido al alto indice de sospecha clínica de hiperaldosteronismo primario se realizó un cribado con la relación aldosterona / renina, el cual se reportó de 29.2 en reposo y 36.7 de pie, asociado a una tomografía axial computarizada de abdomen en el que se reportó una imagen nodular en la glándula suprarrenal izquierda, siendo altamente sugestiva de un adenoma productor de aldosterona. Se decide ingresar y realizar adrenalectomia izquierda, cuya biopsia reportó una masa en la glándula suprarrenal de color amarillo y aspecto heterogéneo, concordante con un adenoma cortical izquierdo productor de aldosterona.

Foundation: arterial hypertension is the most common of all conditions internationally and one of the main risk factors that contributes to premature mortality. Objective: to describe the epidemiological behavior of arterial hypertension and some elements of the care process at the Ángel Arturo Aballi Polyclinic, in the Old Havana municipality, from September 2019 to May 2020. Method: a descriptive and cross-sectional study was carried out in 159 people aged 18 years and over classified as hypertensive, to whom a questionnaire was applied to explore sociodemographic aspects, personal history and others related to the care process. Three blood pressure measurements were made at different times. The results were expressed in absolute and relative frequency. Results: 62.3 % of hypertensive people were older than 60 years, 61.6 % female. The most frequent comorbidities were: diabetes mellitus with 21.4 % and chronic obstructive pulmonary disease and asthma in 9.4 %. 54.7 % of these people did not feel the need for care in the last year and only 45.3 % were seen in a follow-up consultation. 81.8 % had not had their blood pressure measured for more than a year, 89.3 % had indicated drug treatment, 86.2 % were controlled and 74.6 % adhered to the treatment. Conclusions: the attention that these people received was not adequate; there were difficulties with dispensing, compliance with the program, in addition to a low perception of risk in these people.

Foundation: cerebrovascular diseases are one of the main causes of mortality in the world. In the Americas they constitute the third cause of death and their incidence is increased in type 2 diabetic patients. Objective: to characterize type 2 diabetic patients with acute ischemic cerebrovascular disease. Method: a descriptive study was carried out in diabetic patients who were admitted to the Calixto García Hospital with a diagnosis of acute ischemic cerebrovascular disease in the period between January and December 2022. The universe consisted of all type 2 diabetic patients who were admitted to the Institution and the population of 148 patients. The variables analyzed were: sex, skin color and age; blood pressure and body mass index; cholesterol, triacylglycerides and glycemia; type of cerebrovascular disease and its severity and comorbidities. Descriptive statistical methods such as absolute frequency and percentage were used. The results were presented in tables created for the purpose. Results: the male sex, the white skin color and the age group between 60 and 69 years predominated with 58.1, 43.9 and 40.5 % respectively. Hypertensive and overweight patients related to the greater severity of cerebrovascular disease, hyperglycemic patients with high cholesterol and triacylglyceride values predominated, and in them the cerebral ischemic event was more severe. Arterial hypertension and ischemic heart disease prevailed in the studied population with 81.8 and 52.7 respectively. Conclusions: cerebrovascular disease was more frequent in males, white skin color and elderly patients. Arterial hypertension, overweight, hyperglycemia as well as high cholesterol and triglyceride values predominated in the sample and were related to the greater severity of the ischemic cerebrovascular event.

Introducción: La hipertensión arterial es la comorbilidad cardiovascular más prevalente, en los pacientes infectados, por la COVID-19. Objetivo: Describir los mecanismos de la hipertensión arterial, con la gravedad de la infección viral por SARS-CoV-2. Métodos: Las bases de datos seleccionadas fueron: PubMed, Scientific Electronic Library Online, Scielo y Google Académico. Se realizó una revisión independiente y se propusieron 50 referencias. No se pusieron restricciones en cuanto al tipo de trabajo, en idioma español e inglés. Se seleccionaron 44 referencias de la propuesta inicial, por mayoría simple. Conclusiones: La hipertensión arterial aumenta el riesgo de hospitalización y muerte, por la COVID-19. Se explica la enfermedad e infección por SARS-CoV-2 que coexiste con la hipertensión, entre los que se destacan el papel de la enzima convertidora de angiotensina 2, los trastornos del sistema renina-angiotensina-aldosterona, la disfunción inmune con excesiva inflamación y la disfunción gastrointestinal.

Introducción: Paraguay asumió el reto de lograr cobertura universal mediante redes basadas en Atención Primaria de la Salud con Unidades de Salud de la Familia (USF) en el primer nivel de atención. Un desafío es la atención integral ante enfermedades no transmisibles, principal causa de mortalidad en el país. Objetivo: analizar la capacidad de las USF para la atención de personas con hipertensión arterial y diabetes en el sistema nacional de salud. El diseño fue no experimental, cuantitativo, transversal, descriptivo con componente analítico. Metodología: Incluyó a 761 USF de 12 regiones sanitarias agrupados en 4 ejes territoriales. Se adaptó el método de evaluación SARA de la OMS con 75 variables, aplicando un cuestionario a profesionales de salud entre noviembre y diciembre de 2022. Se calculó índices de disponibilidad y preparación así como un índice que los integra. Introducción: La prevalencia de HTA se encuentra entre 11,9 % y 49 %, la prevalencia de DM2 es de 10,5 %. La prevalencia de trastornos depresivos en diabéticos va en el rango de 10 % a 15 %. Objetivo: determinar la asociación entre el tiempo de enfermedad de los pacientes con HTA y DM2 con los síntomas de depresión. Metodología: Estudio analítico transversal. La base secundaria analizada fue la Encuesta Demográfica y de Salud del Perú (ENDES) de los años 2019, 2020 y 2021. Se utilizó el Cuestionario de Salud del Paciente-9 (PHQ-9). Se utilizó la prueba del chi cuadrado para cada posible factor asociado a la depresión y las asociaciones de interés se evaluaron con una distribución de Poisson. Resultados: El 21,59 % tenían síntomas de depresión al momento del estudio. El 51,40 % eran del sexo femenino. El 16,58 % presentaba mayor a 60 años. La HTA estuvo presente en el 5.9%, mientras que la DM2 en el 2,09 %. En el análisis bivariado, todas las variables se asociaron con la presencia de síntomas depresivos, excepto la variable que fuma a diario (p=0.275). Discusión: Los resultados señalan una asociación entre el tiempo de enfermedad desde el diagnóstico de HTA y DM2 con los síntomas depresivos. En el caso de ambas patologías el tener menos de un año de diagnóstico aumentaban en 51 % y 44 % respectivamente la probabilidad de presentar síntomas depresivos.

La Carga Ácida Potencial Renal (CAPR) constituye la capacidad de un alimento para producir precursores ácidos o bases. Las proteínas incrementan la producción de ácidos, mientras que las frutas y hortalizas constituyen precursores alcalinos. Dietas con CAPR elevada genera acidosis metabólica de bajo grado y contribuyen al desarrollo de enfermedades cardiometabólicas. Objetivo: Investigar la asociación entre la CAPR de la dieta y la prevalencia de hipertensión arterial (HTA), diabetes tipo 2 (DM2) y obesidad en pacientes hospitalizados en el Centro Médico Docente La Trinidad. Método: Estudio epidemiológico, descriptivo, corte transversal, fuente directa que incluyó 27 pacientes (edad media 53,85 años). Se determinó el consumo de energía, macronutrientes, frutas y hortalizas mediante un cuestionario de frecuencia de consumo y la CAPR mediante el método de Remer y Manz. Resultados: Prevalencia para sobrepeso/obesidad, HTA, y DM2: 78%, 48, y 11% respectivamente. El consumo promedio de calorías y proteínas fue superior, y el de frutas y hortalizas inferior a los requerimientos recomendados. Las dietas tuvieron una CAPR promedio de 37meq/día. El 66,66% de los pacientes con sobrepeso/obesidad y el 61,53% con HTA, tuvieron una CAPR>30 mEq/l. No hubo correlación entre la CAPR y las patologías registradas. Conclusión: Todos los pacientes tuvieron CAPR elevada inducida por un consumo elevado de proteínas y una ingesta deficiente de hortalizas. No se encontró correlación significativa entre CAPR y la prevalencia de obesidad/sobrepeso, HTA y DM2. Se recomienda implementar estrategias educativas para concientizar a los pacientes acerca de la importancia de vigilar el aporte ácido de sus dietas.

To describe the prevalence of non-alcoholic fatty liver disease (NAFLD), the association between Liver fibrosis 4 score (FIB4) and ultrasound findings, and the clinical characteristics of psoriatic arthritis patients. We carried out an observational cross-sectional study of patients seen in the outpatient clinic from January 1st, 2020, to November 30th, 2020, with psoriatic arthritis. Of the 90 patients studied, the prevalence of NAFLD was 56.67%. FIB4 presents an association with ultrasound findings (p =.030), the absence of enthesitis (p =.036), and longer duration of disease (Rho.213 p =.042). It also presents an association with hypertension (p =.027) and alcohol consumption (p =.021). However, biological treatment can be considered as a protective factor (p =.005). FIB4 acts as a NAFLD predictor with 69.2% sensitivity and 70.4% specificity. The prevalence of NAFLD was higher in our sample than in the standard population. FIB4 index may be useful in screening for silent liver damage in psoriatic arthritis in clinical practice. [ABSTRACT FROM AUTHOR]

48. Factores asociados con defunción en pacientes positivos a COVID-19 en Tamaulipas, México.

- Abstract
OBJECTIVE: To identify the associated factors to death in COVID-19 positive patients in Tamaulipas, Mexico. MATERIALS AND METHODS: Retrospective, descriptive, cross-sectional and observational study that used information obtained from the Single Epidemiological Surveillance Information System (SINOLAVE) from January 2020 to January 2022 of the Mexican Social Security Institute. A hypothesis test of difference of proportions for independent samples based on the z statistic and a logistic regression model was used. RESULTS: The sample used was 12,704 positive patients. Arterial hypertension (61.2%), diabetes mellitus (50.6%) and obesity (30.2%) were the main comorbidities, but chronic kidney disease was the major contributor to the probability of death. In addition, lethality increased significantly after the age of 55 years. CONCLUSIONS: The behavior of lethality and the causes that contribute to increase the probability of death by SARS-CoV-2 virus infection depend on the characteristics of each population, for example, sex, age, presence of comorbidities such as arterial hypertension, diabetes mellitus, obesity or chronic kidney disease, among others. This reflects the current lifestyles that are professed in each place and that have undoubtedly shown the public health problems of the country. [ABSTRACT FROM AUTHOR]

The article presents the results of a study conducted in Spanish community pharmacies during May 2019, with the aim of understanding the situation of blood pressure in the Spanish population and estimating the prevalence of masked hypertension. It was found that almost one in four participants had elevated blood pressure values, and the most related risk factors were overweight, diabetes, and age. Additionally, it was confirmed that 40% of suspected cases of masked hypertension were confirmed through self-measurement of blood pressure. The importance of regular blood pressure measurement and the need to improve the management of hypertension in the population are highlighted. [Extracted from the article]

Introduction: Arterial hypertension (AH) is a global risk factor for morbidity and mortality and the main cause of premature death, largely affecting women. The purpose of the study was to understand how migrant women (MW) with AH configure their therapeutic itineraries (TI) in the search for health care in Colombia, from the health-disease-care process. Methods: Qualitative study with a grounded theory approach. 16 interviews and two focus groups were conducted with MW with AH in Popayán Colombia. Intentional theoretical sampling was carried out until reaching theoretical saturation and the abstraction of emerging categories through open, axial and selective coding. Results: Open coding generated 1,135 codes. The main emerging category was that TI in AH care are determined by social, cultural, political, and economic dynamics. As related subcategories emerged: the risks of getting sick from AH configure TI; heterogeneity of formal and informal therapeutic resources arises to attend to the disease; there are determinants of access to the health system; and mobilization of various practices for monitoring and control of the disease. Conclusion: Women in a situation of migration with AH configure TI to achieve health care not only from the state health system, but from various elements of the social, cultural, political and economic order. To this extent, AH goes beyond morbid nominations exclusive to biomedicine and the doctor-patient relationship, to address and transcend towards a condition that requires resolving contingencies of the context.
