Beta-blockers are used for a wide range of cardiovascular diseases (CVD) - from arterial hypertension to heart failure. Recently, however, a number of analyzes and information have appeared that question the role of beta-blockers in some indications. We provide an overview of the controversies that have arisen around beta-blockers in the treatment of heart failure, hypertension, arrhythmias, in the secondary prevention of ischemic heart disease, and in perioperative administration to patients during non-cardiac operations. The definition and diagnostics of prehypertension (PHT), hypertension (HT) and their prevalence in children and adults are presented. Attention was drawn to the PHT and HT programming already in the prenatal and early postnatal period. An overview of very high salt consumption in the Czech Republic and in the world which is connected with PHT, HT and later with complications of cardiovascular diseases (CVD). The concept of salt sensitivity/ resistance is explained. Briefly presented are opinions on the pathophysiology of hypertension. Summarized are the facts about salt consumption in history and the present and possibility of CVD prevention. Recently, the risk of PHT/HT is currently associated with the increasing incidence of overweight/obesity in children and adolescents, who are often associated with high consumption of not only salt, but also sugar. Summarized are principles of primary prevention of PHT/HT and complications of CVD with emphasis on early non-pharmacological intervention. Prevention of PHT/ HT should start already at the age when the individual health care is provided by pediatricians. [ABSTRACT FROM AUTHOR]

Posterior reversible encephalopathy syndrome (PRES) is a rare pathological entity, in which the neuroimaging, especially MRI, is essential for the diagnosis. The disease is characterized by the typical distribution of cerebral vasogenic edema. Several risk factors are known to be associated with the PRES development. This case report describes PRES in a 46-yearold woman treated in the past for acute myeloid leukemia with numerous concomitant complications. The clinical manifestations of PRES is usually sudden and include a various neurological symptoms. The presence of uncontrolled hypertensive disease is a frequent finding. The treatment of PRES usually focusses on the underlying disease. Although the prognosis of the PRES is generally considered to be favorable, the disease may not be completely reversible and may also be fatal. [ABSTRACT FROM AUTHOR]

25. Léčba arteriální hypertenze v době epidemie COVID‑19.

Currently, the Czech Republic, like most other Western countries, is affected by the pandemic of COVID‑19, caused by the SARS‑CoV‑2 virus. Studies to date did not show increased risk of developing COVID‑19 or greater severity of its course with medication of renin‑angiotensin system inhibitors. Patients taking angiotensin converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) should continue treatment with these products, including during hospitalization for COVID‑19. We should continue to follow the current guidelines for the treatment of arterial hypertension. The goal office blood pressure is <140/90 mm Hg for all hypertensive patients, in case of good tolerance of treatment around 130/80 mm Hg or lower. In pharmacotherapy we prefer the combinations of an ACE‑inhibitor or angiotensin receptor blocker with a calcium channel antagonist and/or a diuretic, beta blockers remain reserved for specific indications. In most patients, combination therapy should be initiated from the beginning of treatment, optimally in the form of a fixed drug combination. Ischemic strokes are frequent neurological diseases. Annual incidence in Europe varies from 100 to 350/100.000 inhabitants per year. Strokes are leading cause of long-term invalidity and 3rd cause of death worldwide. Reperfusion therapy represents therapeutic keystone. In the year 2015, positive results of five randomized studies showed the superiority and efficacy of mechanical thrombectomy compared to standard treatment in acute ischemic stroke due to large vessel occlusion. Positive results of DAWN and DEFUSE-3 trials published in 2017/2018 showed benefit for endovascular therapy beyond standard therapeutic time window in strictly selected patients based on imaging of ischemic penumbra using CT perfusion or MRI. Thus, "tissue" window is becoming a limiting factor for mechanical thrombectomy now instead of previous "time" window. Use of aspiration thrombectomy and choice of suitable anesthesia caused increase of procedural efectivity of intervention. There are 3 possible positive outcome predictors which can be influenced in acute care. Blood pressure seems to be the key variable in acute phase. Lowering the values is associated with better outcome up to systolic blood pressure under 100 torr where growth of complications is observed. Oxygen stress protective effect of normoglycaemia appears to be more important in case of mechanical thrombectomy. Body temperature seems to be good predictor of outcome in relation to other characteristics of the patient. Its' sole decrease via therapeutic hypothermia is in the cases of acute ischaemic stroke associated with significantly higher incidence of severe complications. [ABSTRACT FROM AUTHOR]

Hypertension is a serious cardiovascular risk factor, and because of its high incidence in adults (30-40 %), its treatment is also a major medical and economic problem. Sympathicotonia plays a key role in the etiopathogenesis of hypertension. Currently, pharmacotherapy is the mainstay of hypertension treatment. Historically, however, the first truly effective treatment of hypertension, including malignant hypertension, was surgical sympathectomy. Unfortunately, surgical sympathectomy was often complicated by serious or fatal complications, and was abandoned with the advent of modern pharmacotherapy. Catheter-based renal denervation is based on the concept of mini-invasive partial renal sympathectomy. Introduced for the first time in 2009, renal denervation has undergone its historical development: from inadequate initial euphoria to subsequent rejection to contemporary renaissance. Currently, renal denervation appears to be an effective adjunctive non-pharmacological treatment in a selected group of patients with true primary resistant hypertension, in whom "maximum combined pharmacotherapy alone" fails to achieve target blood pressure levels. The incidence of civilization diseases in the population is increasing every year due to changing environmental conditions and modern lifestyles. These diseases reduce the quality of life, lead to health complications or even death of patients. This can be prevented by monitoring health status. Circulating free DNA (cfDNA) has the potential to be introduced as a non-invasive biomarker. Possible mechanisms of cfDNA release are apoptosis, necrosis, NETosis, pyroptosis and active secretion. The occurrence of these processes in some civilization diseases allows the use of cfDNA as a biomarker. The aim of this work was to summarize the current knowledge on the possibility of using cfDNA in the diagnosis, monitoring of the development and course of some diseases of civilization, namely obesity, diabetes mellitus, atherosclerosis, hypertension, myocardial infarction and stroke. From the available literature it can be concluded that cfDNA could be used in clinical practice for the purposes described above. Remote ischemic perconditioning (RIPerC) stimulated by brief nonlethal periods of ischemia and reperfusion of a remote organ or a tissue applied during myocardial infarction, is a powerful cardioprotective maneuver. Because of its easy realization, for example, through inflation and deflation of blood pressure cuff, this method has a big potential for translation to clinical settings. The main purpose of this thesis was to investigate the cardioprotective effect of RIPerC as a reduction of infarct size and the incidence and severity of ischemic and reperfusion arrhythmias and to examine, whether this protective effect will be confirmed also in rats with partial deletion of a transcriptional factor of hypoxia-inducible factor-1α (HIF-1α). HIF-1α is a main regulator of hypoxic intracellular signalization and its role was previously indicated in cardioprotection by local ischemic preconditioning. Anesthetized rats were subjected to 20 minutes of the left anterior descending coronary artery occlusion followed by 3 hours of reperfusion. RIPerC was performed by 3 cycles of 4 minutes of ischemia and 2 minutes of reperfusion with a pressure cuff placed on both hind-limbs. Turner syndrome (TS) is the most common chromosomal disorder in women, which is caused by absence or structural abnormality of X chromosome. The most common features of TS are short stature, gonadal dysgenesis and congenital heart and kidney diseases. Spectrum of congenital heart diseases is highly varied and most often the left side of the heart is affected. Bicuspid aortic valve and coarctation of the aorta are the most frequent of them. Bakalářská práce se zabývá problematikou informovanosti pacientů s hypertenzí. Teoretická část bakalářské práce je zaměřena na charakteristiku onemocnění, klasifikaci, klinický obraz, diagnostiku, léčbu arteriální hypertenze, doporučení správného měření krevního tlaku a zdravý životní stylu u pacientů s hypertenzí. Průzkumná část vyplývá z rozdaných dotazníků pacientům, kteří se léčí s arteriální hypertenzí v nemocnici krajského typu. K získání informací o informovanosti pacientů o problematice hypertenze bylo použito dotazníkové šetření kvantitativního typu. Ze získaných dat vyplývá, že pacienti s arteriální hypertenzí jsou poměrně dobře informování o problematice hypertenze. Nejčastější zjištěnou chybou u respondentů bylo chybné vymezení hodnot značících hypertenzi, nesprávné informace o zásadách měření krevního tlaku. Na základě zjištěných chyb byl vytvořen informativní leták vlastí tvorby., The bachelor's thesis deals with the issue of awareness of patients with hypertension. The theoretical part of the bachelor's thesis is focused on the characteristics of the disease, classification, clinical picture, diagnosis, treatment of arterial hypertension, recommendations for correct blood pressure measurement and healthy lifestyle in patients with hypertension.The research part results from questionnaires distributed to patients who are treated for arterial hypertension in a regional hospital. A quantitative survey was used to obtain information about patients' awareness of the issue of hypertension. The obtained data show that patients with arterial hypertension are relatively well informed about the issue of hypertension. The most common fault found among the respondents was incorrect definition of values indicating hypertension, incorrect information about the principles of blood pressure measurement. The article provides an overview of the effects of massage at various diagnoses and clinical states. The positive effects of massage on the human body are documented by many studies. It is advisable to indicate massage as a complementary procedure, or as a preparation for more specific and effective methods of therapy. A review of proven mass effects outlines a wide range of uses. The author of this article, however, sees methodological reserves in some experiments in which the massage was used. Turner syndrome (TS) is the most common chromosomal disorder in women, which is caused by absence or structural abnormality of X chromosome with classical traits that include growth retardation, gonadal dysgenesis and infertility. Acquired cardiovascular disease is a major cause of premature morbidity and mortality in TS. Congenital heart disease and acquired cardiovascular disease can reduce life expectancy by up to 13 years. Ischemic heart disease, stroke and aortic dissection belong to acquired cardiovascular disease with high prevalence. This review is focused on issues of risk factors for acquired cardiovascular disease such as arterial hypertension, glucose metabolism disorders, dyslipidemia and obesity in patients with TS. 1 Abstract My bachelor's thesis focuses on the intake of table salt and other dietary approaches and habits in patients with primary aldosteronism, an extreme form of salt-sensitive hypertension. The theoretical part deals with the definition of the term and risk factors associated with primary aldosteronism, summarizes the diagnosis and treatment procedures especially dietary measures which are very important in the treatment. The practical part was based and developed using an anonymous questionnaire form. The questionnaire consisted of 66 questions ranging from sociodemographic to awareness, education, dietary habits and dietary preferences in relation to salt intake. The purpose of this study was thereby to determine the dietary habits and sodium chloride intake of patients with primary aldosteronism. The total number of patients who were approached was 15, mostly males which were 11 and females were 4. The diet was fairly regular, but with more than half prioritizing lunch and dinner rather than breakfast. When they had a higher intake of cold cuts, salty bars, chips compared to fruit and vegetables. Most of the patients were educated about salt restriction diet and diet in hypertension due to the characteristics of the cohort, but few of them actually follow it. This bachelor thesis deals with nurse education in the primary care of patients suffering from chronic diseases, specifically diabetes mellitus, hypertension and obesity in paediatric patients. The thesis consists of two parts, a theoretical and an empirical. The theoretical part first focuses on education, basic concepts, the process of education, design and preparation for education, implementation and evaluation. It also deals with primary care, its goals and scope, and finally education for diabetes mellitus, hypertension and obesity in paediatric patients. The empirical part has been carried out in the form of qualitative research through interviews with nurses from GPs for adults and GPs for children and adolescents. The aim was to find out how nurses in primary care actually educate patients with selected chronic diseases. The interviews have been conducted according to pre-prepared questions in which the nurses participated. The research group consisted of five nurses from a general practitioner's office for adults and five nurses from a general practitioner for children and adolescents. The results of the research survey have been categorized into seven categories. The result of the research survey points out that education is most often carried outintheform of a short interview, recommendations and subsequent publication of information leaflets. The nurses cannot rely on whether these leaflets are effective and patients really take them home and study them. The results of the thesis can serve as an aid for orientation in the education of patients with chronic diseases. They can be beneficial to nurses in primary care and can also serve as information material for the public.

Introduction: Obstructive sleep apnea (OSA) is characterized by repeated, complete or partial, obstructions of upper respiratory tract while asleep with accompanied apneic pauses. Interruption of ventilation and the subsequent waking reactions cause fragmented and low-quality sleep in the long term and the simultaneous decline in oxygen saturation of hemoglobin causes development of intermittent hypoxia, which leads to serious complications in patients with sleep apnea. One of the most important etiopathogenic factors in development of obstructive sleep apnea is obesity, mainly central and android obesity. It is estimated that obese patients with BMI ≥ 40 kg/m2 suffer from OSA in over 40-60 % cases and 70 % of patients with OSA are obese. According to current findings, both the maladies influence each other and form a positive feedback loop, which further complicates the whole situation. Aim: The aim of this thesis is to describe the relationship between obesity and obstructive sleep apnea on a set of patients, who were hospitalized in department D3 of III. Many studies prove that developed countries have an over-exaggerated intake of salt. The World Health Organization recommends an intake of 5g per person per day. Daily doses are, however, exceeded multiple times. It has been proven that an excessive intake of salt has a negative impact on our organism. It is a risk factor for the occurrence of hypertension, obesity, osteoporosis. The objective of my thesis has been to find out what amount of salt the food of older schoolchildren contains. The thesis is divided into a theoretical part and a practical part. The theoretical part deals with the issue of salt, it describes its history, kinds, meaning and use, the occurrence of salt in different foods and compensation options in diet. Futher, it focuses on the physiology of sodium, potassium and chlorine and the illnesses caused by an excessive intake of salt - specifically hypertension, obesity and osteoporosis. The conlcusion of the theoretical part is devoted to carbohydrates, fats, and protein in the diet of older schoolchildren. The practical part evaluated the weekly diet plans and urine analyses from the 12 respondents of older school age. The content of salt in their diet was being analysed via the Nutriservis programme. Urine analyses were being done in clinical laboratories. The acquired information on body height, weight and blood pressure was assessed using percentile graphs. The results confirm the fact that children of an older school age consume a higher than recommended amount of salt. In ten out of twelve respondents a higher than average daily amount of 5 g salt was discovered. This was confirmed by the value of sodium waste levels in their urine. The link between a higher salt intake and obesity or hypertension was not proven. There were no significant differences between girls and boys.

Aim: The aim of our study was to establish whether or not renal sympathetic denervation (RSD) may help control blood pressure (BP) levels in patients with severe hypertension refractory to pharmacological therapy. Method: Out of a group of 12 patients, candidates for RSD, with uncontrolled hypertension (systolic BP over 190-200 mm Hg on repeated measurements despite optimal medication) four patients were excluded for multiple renal arteries and one for hyperaldosteronism. Seven patients had RSD using a Symplicity device (5 M, 2 F) with a mean age of 64.9 years. While all were followed up for a minimum of 6 months, follow-up duration in the majority of them was substantially longer (12-20 months). Results: At six months post-RSD, six of the seven patients showed a decrease in systolic BP by at least 20 mm Hg while receiving the same or fewer doses of antihypertensive agents. A similar response was seen in diastolic BP. The BP decrease was maintained throughout whole follow-up. Conclusion: In a small group of patients we demonstrated that renal sympathetic denervation is capable of reducing blood pressure even in patients with severe hypertension. [ABSTRACT FROM AUTHOR]
