2. 肝移植术后 VTE 风险评估与个体化护理干预效果观察.

- Abstract
Objective To investigate the effect of risk assessment and individualized nursing interventions for venous thromboembolism (VTE) after liver transplantation. Methods A total of 74 patients who underwent liver transplantation in this hospital from February 2022 to April 2023 were selected and divided into the observation group and the control group using the randomized numerical table method, with 37 patients in each group. The control group received traditional care, and risk of VTE of the observation group was graded the according to the Caprini model on the basis of traditional care, and the care level was graded according to the risk level. The screening results, occurrence of VTE, postoperative recovery and nursing care satisfaction of patients in the two groups were compared. Results The incidence [2. 70% (1/37) vs. 10. 81% (4/37) ] of VTE and the maximum width of thrombus[ (5. 71±1. 12) mm vs. (7. 13±1. 35) mm] of patients in the observation group were significantly lower than that of the control group, chest tube placement [ (3. 56±0. 74) d vs. (4. 01±1. 04) d] and hospitalization time[ (8. 66±1. 23) d vs. (9. 46±1. 86) d] were significantly shorter than that of the control group, and the scores of ability to perform activities of daily living [ (86. 23±5. 23) scores vs. (83. 78±4. 73) scores] and satisfaction with nursing care were significantly higher than that of the control group, and the differences were all statistically significant (P<0. 05) . Conclusion The Caprini thrombosis risk assessment model and graded interventions are effective in preventing VTE after liver transplantation and reducing the maximum width of postoperative thrombus and the incidence of VTE, which is beneficial in promoting the postoperative recovery situation. Objective: To investigate the impact of Traditional Chinese Medicine (TCM) physique classification and targeted, individualized nursing intervention on athletes' mental health in the context of sports. Methods: From February 2018 to February 2019, we randomly assigned 154 athletes to two groups (n=77) for this study. The control group received standard sports-related nursing care, while the observation group received personalized nursing interventions based on their TCM physique constitution. We assessed various parameters, including mental health indicators and sports performance metrics, before and after the nursing intervention. The assessment included psychological wellbeing scores, performance metrics, and the incidence of any adverse effects. Results: After the nursing intervention, athletes in the observation group showed significant improvements in psychological well-being, including lower levels of anxiety and depression (all P<0.05). Additionally, their sports performance metrics, including strength, endurance, and recovery, demonstrated remarkable enhancement compared to the control group (P<0.05). The overall quality of life scores for athletes in the observation group also showed significant improvements, particularly in physiological function, mental health, emotional function, and social function (P<0.05). In terms of adverse effects, the observation group experienced significantly fewer issues compared to the control group (P<0.05). Conclusion: Utilizing TCM physique classification and tailored nursing interventions for athletes can lead to improved mental health outcomes, enhanced sports performance, and an overall better quality of life. This approach holds promise for optimizing athletes' well-being and performance and deserves further consideration and application in the realm of sports and athletes' mental health. This paper reviewed the clinical data of 5 newborn infants with acute peripheral vascular lesion, and summarized individualized surgical treatment and nursing management. The types of peripheral vascular lesion included mural thrombus at initiation site of left external iliac vein in 2 cases, false aortic aneurysm of posterior tibial of left leg in 1 case, thrombophlebitis of left leg in 1 case, false aortic aneurysm of at left elbow in 1 case. Based on the analysis of cause analysis, implementation of individualized surgical treatment and nursing strategy may achieve good clinical outcomes in the treatment of acute peripheral vascular lesion in neonates. Objective To evaluate the application of individualized nursing interventions for patients with geriatric diseases. Methods Totally 100 patients with geriatric diseases were randomly divided into control group and study group by using the lottery method, with 50 cases in each group. All patients received routine care during the treatment, and those in the study group were given individualized nursing interventions. The patients’ mental status and quality of life were measured before and after intervention. Results Scores of Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale (SDS) decreased in both two groups after intervention, and were lower in the study group than those in the control group (P<0. 05). The score of general quality of life inventory (GQOLI-74 ) increased in both two groups after intervention, and was higher in the study group than that in the control group (P<0. 05). Conclusion The individualized nursing may relieve the negative emotions and improve the quality of life in patients with geriatric diseases. (目的 探讨个性化护理干预在老年病患者中的应用效果。方法 选取2021年1月—12月收治的老年病患者100例为研究对象, 依据随机抽签法将其分为对照组和研究组, 各50例。对照组给予常规护理干预, 研究组在对照组基础上开展个性化护理干预。比较两组干预前后心理状态和生活质量。结果 干预后, 两组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均降低, 生活质量综合评定量表(GQOLI-74)各维度评分升高, 且研究组评分优于对照组, 差异有统计学意义(P<0. 05)。结论 个性化护理干预在老年病患者中的应用效果显著, 能有效改善患者心理状态, 提升生活质量。)

The aim of the present study was to compare the effects of individualized nursing on improving the satisfaction of elderly patients with lung cancer undergoing thoracoscopic lobectomy. A total of 72 elderly patients with lung cancer undergoing thoracoscopic lobectomy in the First Hospital of Qinhuangdao (Qinhuangdao, China) were randomly allocated to a control group (n=36) or an observation group (n=36). The patients in the control group received routine nursing, and the patients in the observation group received individualized nursing. The patients' compliance with respiratory function exercise, the complications after surgery, and nursing satisfaction were recorded. The patients' compliance with respiratory rehabilitation exercise and the patients' satisfaction in the observation group were significantly higher than that in the control group. The length of hospital postoperative stay, the time of drainage tube indwelling, and the incidence of postoperative complications in the observation group were significantly lower than in the control group. Thus, an individualized nursing model can accelerate the rehabilitation of elderly patients undergoing video-assisted thoracoscopic lobectomy and improve patient satisfaction. [ABSTRACT FROM AUTHOR]

The aim of the study was to investigate the application of hydrocolloid dressings in the prevention and treatment of infant diaper rash. A total of 210 infants with diaper rash were included in the study and randomized into 3 groups of 70 infants. Infants in group A received hydrocolloid dressings and individualized nursing; infants in group B received mupirocin plaster and topical application of pearl powder as well as routine nursing; and infants in group C received zinc oxide plaster and routine nursing. The clinical efficacy, incidence of adverse events, time to resolution of diaper rash, hospitalization duration, mean cost and satisfaction of nursing were compared between the 3 groups. After 1 cycle of treatment and nursing, the difference in the healing rate of mild diaper rash between groups A, B and C was significant (P<0.05), with the best healing rate in group A. Differences in the healing rate of grade I and II diaper rash between groups A, B and C was significant (P<0.05), with the best healing rate in group A. The incidence of adverse events in infants between groups A, B and C were significantly different (P<0.05), with the lowest incidence in group A (2.9%). The time to resolution of diaper rash, hospitalization duration and cost in infants were not significantly different (P<0.05). The satisfaction rate of nursing for the parents of groups A, B and C was 98.6, 87.1 and 80.0%, respectively (P<0.05). Hydrocolloid dressings combined with individualized nursing may prevent and treat infant diaper rash effectively, decrease the incidence of adverse reactions significantly, shorten time to resolution of diaper rash and hospitalization duration, reduce mean hospitalization cost, improve parent satisfaction and promote nursing-patient harmony. To report a descriptive study of nursing in facilities for short-term stay aiming to outline what 'knowing the patient' means in an endoscopic outpatient clinic. Background. 'Knowing the patient' is indispensable to the effort of tailoring nursing to the individual patient's needs. Structural changes in the practice environments, however, reduce the amount of time a nurse spends getting to know the patient. Despite recent years' focus on the subject, no uniform description of 'knowing the patient' in facilities for short-term stay exists. Design. A fieldwork study influenced by practical ethnographic principles was performed in a high-technology endoscopic outpatient clinic during 2008 - 2010. Methods. Data were collected using participant observation for 12 weeks and semi-structured interviews with eight patients and four nurses. Findings. Findings were summarized into two categories 'What to know?' and 'How to get to know?' The former concerned practical issues in relation to gastroscopy and was described in terms of the patient's level of anxiety, wish for medication and previous experiences. The latter 'How to get to know?' concerned instruments employed in getting to know the patient and was described in terms of the use of communication and sensing. Conclusions. 'Knowing the patient' in the endoscopic outpatient clinic was understood in a very practical sense. Conversation and the use of the eyes and physical touch enabled a situational awareness.
