2. A Case of Lung Cancer Presenting with Paraneoplastic Pruritus.

Pruritus, defined as an unpleasant sensation that compels scratching, is a common occurrence in dermatological disorders. Chronic pruritus is defined as itching that persists for a period exceeding six weeks. Lichen simplex chronicus (LSC) is a chronic dermatological condition that presents with dry, scaly, and thickened skin lesions, which typically arise from habitual scratching. Additionally, paraneoplastic pruritus may serve as an early indicator of the presence of solid tumours. This case report outlines the diagnostic and therapeutic process of a 73-year-old male diagnosed with lichen simplex chronicus (LSC), confirmed by skin biopsy. Subsequent malignancy screening revealed the presence of solid nodules in the lungs and multiple metastatic masses in the liver. Biopsies of the lung nodule and liver mass demonstrated positivity for TTF-1 and synaptophysin, leading to a diagnosis of mixed component lung adenocarcinoma and small cell lung cancer. In order to ascertain the underlying cause of the patient's chronic itching, a comprehensive investigation was conducted, which considered a range of potential factors, including haematological diseases, endocrine disorders, liver diseases, infections, drug-related factors, and neurological and psychiatric causes. Despite a comprehensive evaluation, the precise etiology of the chronic itching remained elusive. The initiation of carboplatin and etoposide therapy for lung cancer resulted in a significant reduction in the patient's itching, which may suggest a possible link between chronic itching and malignancy. Unfortunately, the patient succumbed to hypoxia, pneumonia, and multiple organ failure two months post-diagnosis. This case serves to illustrate the potential of chronic pruritus as a symptom of cancer. While paraneoplastic pruritus is often associated with haematological malignancies, this report highlights that it can also occur with solid tumours. Atopic dermatitis (Ad), an important dermatological problem for humans, similar to dogs. Although it is known that leaky gut-related increase in intestinal permeability and deterioration of the intestinal microbiota play a role in the disease, there are still unknowns regarding in term of both diagnosis and treatment. In this study, based on the relationship between atopic dermatitis and intestinal microbiota; 1) determination of whether Lactobacillus plantarum and Lactbacillus paracasei and probiotic enema can be used in the treatment of itching, which is the leading symptom, 2) aimed to evaluate the administration of one of the new, up-to-date probiotherapy options that are promising for role model treatment by rectal enema, also known as proctolysis, in contrast to problems of absorption in oral administration. In our study, a total of 12 dogs with Ad as different ages, breeds, and both sexes were included. Cases which had no previous treatment history, showed clinical findings compatible with Favrot criteria and atopy accompanying with increasing allergen-specific IgE level, were determined in the light of allergen-specific IgE levels (before treatment), CADESI-04 scores and clinical findings, and then rectal enema with nutraceuticals. Probiotic enema with Lactobacillus plantarum and Lactbacillus paracasei was applied 10-15 cm forward from the rectum via foley catheter or rectal catheter. Pruritus of 12 dogs with Ad applied probiotherapy by rectal enema were ceased significantly between 0 and 10 days. In conclusion, it has been observed that L. plantarum and L. paracasei strains, which can be evaluated in antipruritic lactic acid bacteria, can be used for both clinical improvement and relief of itching in dogs with atopic dermatitis at the recommended and applied dose/time in this study. Objective: Intrahepatic Cholestasis of Pregnancy (ICP) is a pregnancy complication characterized by pruritus, elevation in liver enzymes and concentration of serum bile acids (SBA). In the current study, we aimed to investigate the relationship after treatment of ursodeoxycholic acid (UDCA) in patients with ICP between pruritus and the level of alanine aminotransferase (ALT) and SBA. Methods: A total of 30 pregnant diagnosed with ICP in the Gastroenterology outpatient clinic of Kanuni Sultan Süleyman Training and Research Hospital were enrolled in the study in 2018. Data including patient age, pregnancy week, delivery week, pruritus score, level of SBA and ALT levels were recorded. Results: The mean age of pregnant was 29.9±6.5 and the mean week of pregnancy was 31.8±3,74. There was no significant difference in terms of ALT level between before and after period of UDCA treatment (106.13 ± 91.27 vs. 45.8 ± 35.76 U / L, p = 0.87, respectively). A significant difference was observed in terms of ALT level between postpartum period and post-UDCA treatment (22.2 ± 5.79 vs. 45.8 ± 35.76 U / L, p = 0.002, respectively). There was a significant difference in terms of visual analog scores between before and after period of UDCA treatment (6.6 ± 1.44 vs. 2.03 ± 1.49, p = 0.02, respectively). Conclusion: The treatment of UDCA in ICP positively effects on level of pruritus, but does not contribute to regression of ALT level. [ABSTRACT FROM AUTHOR]

Bu çalışmada, deri problemlerine sebep olan enfektif ve non-enfektif hastalıkların tedavisinde sahada yaygın olarak kullanılmakta olan tedavi protokolleri ve preperatlara alternatif olarak doğal bir pestisit olan Odun Sirkesi’nin etkinliğinin araştırılması amaçlanmıştır. Sürüler de bulunan hayvanlar kaşıntı, tüy döküntüsü, deri lezyonları gibi deri problemleri yönünden klinik olarak muayeneleri yapılarak çalışmaya dâhil edilmiştir. Deri problemi görülen koyunlar rastgele seçilerek, odun sirkesi tedavisi uygulanan (n=26), Flutick uygulanan (n=16) ve Dectomax uygulan (n=14) olmak üzere üç gruba ayrıldı. Çalışmaya dahil edilen hayvanlardan hemogram ve kan gazı bulgularını değerlendirmek için steril kan tüplerine kan örnekleri alındı. Parazitolojik muayene için deri lezyonlarının olduğu bölgelerden steril petri kaplarına deri kazıntısı ve kıl örnekleri alındı. Histopatolojik yönden incelenmek üzere 21 hayvandan deri biyopsisi alındı. Alınan numunelerden elde edilen sonuçların istatistiki değerlendirmeleri ve klinik olarak iyileşen hayvanlara bakılarak yapılan değerlendirmelere göre odun sirkesi grubunda %96,2 gibi yüksek bir iyileşme oranı olduğu belirlenmiştir. In this study, it was aimed to investigate the effectiveness of Wood Vinegar, which is a natural pesticide, as an alternative to the treatment protocols and preparations that are widely used in the field in the treatment of infective and non-infective diseases that cause skin problems. The animals in the herd were included in the study by clinical examination in terms of skin problems such as itching, hair rash and skin lesions. Sheep with skin problems were randomly selected and divided into three groups as wood vinegar treatment (n=26), Flutick applied (n=16) and Dectomax applied (n=14). Blood samples were taken from the animals included in the study into sterile blood tubes to evaluate the hemogram and blood gas findings. For parasitological examination, skin scrapings and hair samples were taken from the areas with skin lesions in sterile petri dishes. Skin biopsies were taken from 21 animals for histopathological examination. According to the statistical evaluations of the results obtained from the samples taken and the evaluations made by looking at the clinically recovered animals, it was determined that there was a high recover rate of 96.2% in the wood vinegar group. 13. Efficacy of Topical Curcuma longa and Nigella sativa Combination for Feline Head and Neck Dermatitis: An Open Pilot Study.

Two medicinal plants, Curcuma longa (turmeric) and Nigella sativa (black cumin), have generated growing interest for their therapeutic antiinflammatory action. We hypothesize that commercially available topical C. longa and N. sativa cream (Dermacumin cream®) will significantly decrease both the Feline Dermatitis Extent (FDE) and Severity Index (SI) and the pruritus scores after 3 weeks of twice-daily treatment of feline head and neck dermatitis. Ten client-owned cats with feline head and neck dermatitis which had received no previous treatment. FDE and SI scores gradually decreased on days 7 (P<0.001) and 28 (P<0.0001) compared to day 0. Similarly, decreased Visual Analog Scale (VAS) scores were detected on days 7 and 28 (P<0.0001). Cats receiving the herbal cream for 3 weeks all had significantly decreased FDE and SI scores, from 171.8 to 0 (P<0.0001), and VAS scores from 6.3 to 0 (P<0.0001). A positively elevated correlation (r=0.62, P<0.001) between FDE and SI and VAS scores for each case through evaluation period was revealed. 14. Factors Affecting the Etiology of Intractable Pruritus in Hospitalized Patients Without Primary Skin Lesions.

Introduction: Pruritus is a common symptom that has dermatologic, systemic, neurological, psychogenic, mixed, and unknown causes. The aim of this study was to identify the underlying origin of pruritus (UOP) in hospitalized patients having intractable pruritus and presenting with secondary scratch lesions (SSLs) and to assess the factors affecting UOP. Methods: Data of 95 patients (male/female: 47/48) presenting with SSLs were examined retrospectively. Demographic and clinical characteristics, diagnostic procedures, and treatment agents were recorded. UOP was defined as dermatological and non-dermatological factors. Results: The median (range) age and disease duration were 61 years (11-91) and 2 months (0.06-120), respectively. Pruritus was related to dermatological and non-dermatological reasons in 78% and 22% of patients. Univariate analysis revealed that factors showing an association between patients with and without underlying dermatological origin of [pruritus (UDOP)] were age, disease duration, disease onset, ≥3 months of continuous drug use, intake of drugs attributed to lead to pruritus, accompanying systemic diseases, polypharmacy, renal function tests, and the presence of sleep disorders (p=0.001, p=0.001, p=0.001, p=0.03, p=0.045, p=0.02, p=0.004, p=0.047 and p=0.01, respectively). Multivariate analyses revealed that acute onset of pruritus increased the risk of UDOP by 15.28 times (p=0.005, 95% confidence interval (CI): 2.30-101.67) compared to patients with chronic onset. The lack of sleep disorders increased the risk of UDOP by 8.22 times (p=0.01, 95% CI: 1.67-40.56) compared to patients who had sleep disorders. Conclusion: Acute onset of pruritus and lack of sleep disorders were independent predictors of UDOP in patients with SSLs. Dış kulak yolu yabancı cisimleri hemen her gün kulak burun boğaz uzmanları tarafından karşılaşılan bir acil sorundur. Yabancı cisimler işitme kaybı, ağrı, şiddetli kaşıntı, kulak akıntısı, çınlama ve baş dönmesine neden olabilir. Aynı zamanda yabancı cismin çıkarılması sırasında da komplikasyonlar açısından büyük riskler vardır. 82 yaşında demans hastalığı olan ve işitme kaybı, ağrı, şiddetli kaşıntı, kulak akıntısı rahatsızlığı çeken erkek hasta kliniğimize acil vaka olarak yönlendirildi. Otoskopik muayenesinde her iki kulakta metalik olmayan beyaz/sarımsı renkte pamuksu görünümü olan yabancı cisimler tespit edildi. Tüm pamuk yabancı cisimler cerrahi odasında herhangi bir anestezik ilaç kullanmaksızın aligatör yardımıyla çıkarıldı. Kulakta yabancı cisim olan hastalara yaklaşım komplikasyonların azaltılması ya da ileri hasarların önlenmesi açısından tecrübeli eleman yada KBB uzmanı tarafından uygun ekipman kullanılarak yapılmalıdır.

Inflammatory linear verrucous epidermal nevus (ILVEN) is usually accompanied by persistent and severe pruritus. It is mostly located in the lower left limb as hyperpigmented or erythematous hyperkeratotic linear papules and/or plaques. Genital/perigenital involvement is extremely rare. It varies in size from very small lesions to the involvement of the whole extremity. The differential diagnosis is broad, including but not limited to lineer psoriasis, lichen striatus, verrucous epidermal nevus, lichen simplex chronicus, linear lichen planus and linear Darier disease and, prognosis is poor. In this report, we present a case of vulvar ILVEN that responded to topical nonsteroidal anti-inflammatory cream. [ABSTRACT FROM AUTHOR]

Tüm insanlar hayatlarında bir kez de olsa pruritusu yaşamışlardır. Akut kaşıntı, böcek ısırması gibi, saniyelerden haftaya kadar değişen sınırlı süre kaşıntı yaşanmasıyken, kronik kaşıntı ise 6 haftadan uzun kaşıntılara verilen addır. Kronik pruritus dünya genelinde bir problemdir. Her yaşta görülebilir ve yaşam kalitesine büyük etkisi vardır. Kaşıntı için akut ve kronikten başka sınıflamalar da vardır. IFSI (International Forum for the Study of Itch: Kaşıntı Çalışması Uluslararası Forumu) sınıflaması kaşıntılı bireyleri 3 gruba ayırır; hastalıklı deride kaşıntısı olanlar (grup 1), hastalık olmayan deride kaşıntısı olanlar (grup 2), deri kaşıma lezyonlarına sekonder kaşıntısı olanlar (grup 3). Ayrıca altta yatan kaşıntılı hastalıklara göre de 6 spesifik kategoriye ayırır: dermatolojik, sistemik, nörolojik, psikojenik, miks ve diğer pruritus.

- Abstract
Prurigo pigmentosa is a rare skin disease of unknown etiology. Prurigo pigmentosa is seen frequently in young Japanese women. It is characterized by pruritic, erythematous macular, papular, papulovesicular lesions and reticular pigmentation. Lesions often localize back, neck, and chest. Dapsone and minocycline are most commonly used in the treatment of prurigo pigmentosa. Here we report a 19 years-old women diagnosed with prurigo pigmentosa by clinical and histopathological findings and treated with doxycycline. Effectiveness of doxycycline in the treatment of prurigo pigmentosa was discussed in the light of literature. [ABSTRACT FROM AUTHOR]

23. Kolestaz ve Kaşıntı: Progresif Familyal İntrahepatik Kolestaz?

- Abstract
Progressive familial intrahepatic cholestasis (PFIC) has a significant place among the childhood cholestasis. It is the second most frequent liver transplant indication met in some liver transplant institutions. It constitutes approximately 10-15% of all cases of cholestasis and liver transplant. PFIC is an autosomal recessively inherited disorder and causes hepatocellular-originated cholestasis. In the period from infancy to adulthood it may cause death due to liver failure. PFIC has three types and the most frequently observed clinical symptoms of all three types are; pruritus, hepatitis, growth retardation, hepatomegaly, and splenomegaly. The prognosis of these three types of PFIC is different. The complications of PFIC are; portal hypertension, liver failure, cirrhosis, hepatocellular carcinoma, and extrahepatic symptoms. Although medical and surgical treatments are applied in order to improve the patients' life quality, most of the cases require liver transplantation in the early stage. [ABSTRACT FROM AUTHOR]

Purpose: Chronic urticaria is a skin disease with unknown etiology. Autoimmunity and psychogenic factors have been suggested to play a role in pathogenesis. Fibromyalgia is a rheumatic disease that may be associated with psychogenic factors. The aim of the study was to determine the frequency and severity of fibromyalgia and the effect of fibromyalgia coexistence on clinical features and dermatology life quality index in urticaria patients.Materials and Methods: One hundred patients with chronic urticaria and no history of any systemic disease and 100 age and sexmatched healthy controls were included. Urticaria severity was evaluated by the dermatology life quality index (DLQI). Clinical severity and disability status were evaluated by the visual analog scale (VAS) and fibromyalgia impact questionnaire (FIQ).Results: The mean age of the chronic urticaria patient group was 37.7±1.32, and that was 40.7±11.2 in the control group (p=0.09). Fibromyalgia frequency (45%) in the chronic urticaria group was significantly higher than in the control group (13%) (p=0.00). The duration of chronic urticaria was significantly higher in the patients with FMS than in the patients without FMS (p=0.001). There was a statistically significant correlation between FIQ and DLQI (p=0.019). No significant differences in terms of FMS durations, VAS, and FIQ values were detected between groups (p=0.432, p=0.201, p=0.332).Conclusion: High frequency of fibromyalgia in chronic urticaria patients was found. This may be related to the chronic effect of neuromediators involved in chronic urticaria pathogenesis., Amaç: Kronik ürtiker, nedeni tam olarak aydınlatılamamış kaşıntılı bir deri hastalığıdır. Otoimmünite ve psikojenik faktörlerin de patogenezde rolü olabileceği düşünülmektedir. Fibromiyalji sendromu (FMS) psikojenik nedenlerle ilişkili olabileceği düşünülen bir romatizmal hastalıktır. Çalışmamızda kronik ürtiker hastalarında FMS sıklığı ve şiddetini araştırmayı ve kronik ürtikere eşlik eden fibromiyalji varlığının klinik özelliklere ve dermatoloji yaşam kalitesine etkisini değerlendirmeyi planladık.Araçlar ve Yöntem: Çalışmaya kronik ürtiker tanısı almış ve sistemik hastalık öyküsü olmayan 100 hasta, yaş ve cinsiyet açısından eşleştirilmiş 100 sağlıklı kontrol dahil edildi. Ürtiker şiddeti dermatoloji yaşam kalite indeksi skorları (DYKI) hesaplanarak değerlendirildi. Klinik şiddet ve fonksiyonel disabilite durumları vizuel analog skala (VAS) ve fibromiyalji etki anketi (FIQ) ile değerlendirildi.Bulgular: Hasta grubunun yaş ortalaması 37.7±1.32, kontrol grubunun ise 40.7±11.2 olarak saptandı (p=0.09). Hasta grubundaki FMS sıklığı (%45) kontrol grubundan (%13) istatistiksel olarak anlamlı şekilde yüksekti (p=0.00). Kronik ürtiker süresi FMS'li hastalarda istatistiksel olarak anlamlı derecede yüksek bulundu(p=0.001). FIQ ve DLQI değerleri arasında istatistiksel olarak anlamlı korelasyon saptandı (p=0.019). Hasta ve kontrol grubunda FMS süreleri, VAS ve FIQ açısından istatistiksel olarak anlamlı fark saptanmadı (p=0.432, p=0.201, p=0.332).Sonuç: Kronik ürtiker hastalarında FMS sıklığının artmış olduğunu görülmektedir. Bu sonuç kronik ürtiker patogenezindeki nöromediatörlerin kronik dönemdeki etkileriyle ilişkili olabilir.

Imatinib mesylate is a small-molecule tyrosine kinase inhibitor (TKi) designed to target c-ABL and BCR-ABL, approved for the treatment of chronic myeloid leukemia and gastrointestinal stromal tumors. Adverse cutaneous reactions induced by imatinib are frequent, generally moderate, and dose-dependent. The aim of this work was to investigate the possible contribution of interleukin (IL)-33 and IL-31, cytokines involved in disorders associated with itching, in the pathogenesis of pruritus in a patient undergoing imatinib mesylate treatment. His IL-31 and IL-33 serum levels were significantly higher than in the control group (respectively 96.6 pg/mL vs. 7.623±7.681 pg/mL and 27.566 pg/mL vs. 6.170±7.060 pg/mL). In light of these findings, imatinib mesylate-related symptoms of dermatologic toxicities might be related to the release of IL-31 and IL-33. In particular, it is supposable that TKi usage could cause keratinocyte injury, the release of IL-33, and the consequent interaction with its receptor on mast cells that induces the secretion of several factors capable of causing skin manifestations, including IL-31, a known pruritus-inducing cytokine. This report, to the best of our knowledge, is the first work describing the possible involvement of the IL-31/IL-33 axis in the pathogenesis of skin side effects related to imatinib mesylate treatment. Amaç: Bu araştırmanın amacı, karaciğer hastalığı olan bireylerde aromaterapi masajının kaşıntı, konfor, deri pH düzeyi ve deri nemliliğine etkisini belirlemektir. Gereç ve Yöntem: Araştırmanın örneklemini; 13.02.2020-03.05.2021 tarihleri arasında "Ege Üniversitesi Hastanesi Gastroenteroloji Kliniği"nde yatarak tedavi olan, kaşıntısı olan ve örnekleme alınma kriterlerine uyan 36 birey [aromaterapi (n=11), plasebo (n=12) ve kontrol (n=13)] oluşturmuştur. Bu araştırma, randomize kontrollü deneysel bir çalışma olarak yapılmıştır. Araştırmanın verileri "Birey Tanılama Formu", "5-D Kaşıntı Ölçeği" ve "Genel Konfor Ölçeği" kullanılarak toplanmıştır. Aromaterapi grubundaki bireylere %2 oranında dilüe edilmiş çay ağacı yağı ile plasebo grubundaki bireylere ise tatlı badem yağı ile kaşıntısı olan bölgelere altı kez yaklaşık 15 dakika masaj yapılmıştır. Kontrol grubundaki bireylere, herhangi bir uygulama yapılmamıştır. Uygulama öncesi, ilk uygulamadan sonra ve altıncı uygulamadan sonra tüm gruplardaki hastalara veri toplama formları uygulanmış ve hastaların deri ph düzeyi ile deri nemliliği düzeyi ölçülmüştür. İstatistiksel analizlerde sayı ve yüzde değerleri, t testi, ANOVA, Kruskal Wallis, Wilcoxon ve Friedman testleri kullanılmıştır. Sonuçlar %95'lik güven aralığında, p0.05). Aromaterapi grubunun uygulama sonrası kaşıntı skorları plasebo grubundan daha düşüktür. Hastaların konfor düzeyleri incelendiğinde ilk uygulamadan ve altıncı uygulamadan sonra kontrol grubunda anlamlı değişiklik yokken aromaterapi ve plasebo grubunun değerlerinin anlamlı şekilde artmakta olduğu saptanmıştır (p0.05). Material and Method: The sample of the research, consisted of 36 individuals [aromatherapy (n=11), placebo (n=12) and control (n=13)] who received inpatient treatment at the "Ege University Hospital Gastroenterology Clinic" between 13.02.2020-03.05.2021, had itching and met the sampling criteria. The individuals in the aromatherapy group were massaged with 2% diluted tea tree oil and the individuals in the placebo group were massaged with sweet almond oil six times for approximately 15 minutes.The individuals in the control group did not receive any intervention. Before the application, after the first application and after the six application data collection forms were applied to the patients in all groups and the skin pH level and skin moisture level of the patients were measured. Number and percentage values, t test, ANOVA, Kruskal Wallis, Wilcoxon and Friedman tests were used in statistical analysis. Results: The mean age of the patients participating in the study was 65.19±11.64, and 52.8% of them were women. While there was a significant decrease in the itching levels of the patients in the aromatherapy (p=0.000) and placebo groups (p=0.018) before and after the sixth application, it was not significant in the control group (p>0.05). The post-application itching scores of the aromatherapy group were lower than the placebo group. When the comfort levels of the patients were examined, it was determined that while there was no significant change in the control group after the first application and the sixth application, the values of the aromatherapy and placebo groups increased significantly (p0.05). When the skin moisture levels of the patients were examined, the skin moisture levels of the back, arms, legs and abdomen did not change in the control group after the first application and after the sixth application, but increased significantly in the aromatherapy and placebo groups (p

- Abstract
Foreign bodies in the external canal of the ear are an emergency problem encountered by the otolaryngologist almost every day. Foreign bodies may cause hearing loss, pain, severe itching, ear discharge, tinnitus and vertigo. There is also great risk for complications during the removal of foreign bodies. An 82 years old male patient with dementia suffering from hearing loss, pain, itching and ear discharge in both ears was referred to our clinic as an emergency case. Otoscopic examination revealed multiple nonmetallic foreign bodies, whitish/yellowish cottony material, at both ears. All cotton foreign bodies were removed with alligator in the surgery room without use of any anesthetic agents. The management of patients with ear foreign bodies must be carried out by the otorhinolaryngologist or skilled professional with the use of suitable material to minimise complications or prevent further damages. Itching is a situation which has biological, psychological and sociological dimensions, accrue as a multidimensional symptom in many diseases, especially in skin diseases. Researches shows that, nonpharmacologic methods are reduced using drugs related to itching and the severity of itching. One of the roles of health professionals is to advise individuals, who are living on this problem, using the nonpharmacologic methods in checking the itching. Objective: Reasons for application to emergency departments may vary, in dermatology as well as in other branches, according to the geographic regions, climatic and socio-cultural structures. The aim of this study is to gain epidemiological knowledge about the identification, clinical features, treatment and follow-up procedures of patients who attended with dermatologic problems to the emergency department of an university hospital in an island with a subtropical climate in Mediterranean area. Materials and Methods: In this retrospective descriptive study, patients who were admitted to the emergency department with a dermatological complaint during an 18-month period between July 2010 and December 2011 in the Emergency Department of Near East University Hospital, Nicosia, TRNC were evaluated. Descriptive statistics were evaluated as mean and percentage. Results: 1.8% (206 of total 11441) of applications to the emergency department were dermatological diseases. Urticaria/angioedema was the most common cause of applications, and was followed by drug reactions, insect bites and cellulitis respectively. Conclusion: Results of our study indicate that the most common reason for application to the emergency department in North Cyprus is urticaria, and this finding is consistent with other studies in the world. Objectives: We aimed to determine possible changes in pruritus, quality of life, depression and sleep quality in pruritic hemodialysis (HD) patients with gabapentin therapy. Patients and Methods: Fourteen adult HD patients (7 men, 7 women; mean age 59.7±17.2 years; range 41 to 88 years) with history of pruritus of more than eight weeks were assigned to receive 8-week gabapentin (300 mg per day) therapy. The daily pruritus were recorded using visual analogue scale for each period of the study during one week preceding the trial, the active treatment phase, the placebo phase and the intervening 1-week washout period. Sleep quality was determined with a modified post-sleep inventory, quality of life with a short form of Medical Outcomes Study (SF-36), depression using the Beck Depression Inventory. Results: The mean pruritus score and total of post-sleep inventory were decreased significantly with gabapentin therapy (p=0.01 and p=0.002 respectively). Physical and mental component scales of SF-36 increased, whereas cognitive and somatic depression index decreased with gabapentin. Conclusion: We concluded that beneficial effects of gabapentin therapy on pruritus, quality of life, depression and sleep quality are clinically important in HD patients with pruritus. Gabapentin therapy should be taken into account as an important choice of therapy in pruritic HD patients. [ABSTRACT FROM AUTHOR]

Hepatitis A is an acute infectious disease of the liver caused by the hepatitis A virus. Many cases have little or no symptoms. In our case, a nineteen year old female patient referred our hospital because of exhaustion, jaundice and diffuse itching complaints which existed for one week. In dermatological examination, there were diffusely erythematous, excoriated papules with crusts on the body as more significant on the lower extremity. The patient was diagnosed with acute viral hepatitis A and generalized pruritus according to present findings. Cholestyramine 16 g/day and acrivastine tablet as 1x1 were started to administrate for hyperbilirubinemia and itching. The patient whose complaints regressed partially at the eighth day of the treatment was discharged by her own will. Consequently, patients with generalized pruritus who do not have any previous primary cutaneous disorders could be tested for HAV infection. [ABSTRACT FROM AUTHOR]

Kaşıntı ve ağrı, ortak birçok özelliği paylaşan iki rahatsız edici edici histir. Ağrılı durumlarda endokannabinoid yıkıcı enzimlerin inhibisyonunun antinosiseptif etki göstermesini dikkate alarak, endokannabinoid modülasyonuyla, yani yağ asidi amid hidrolazı, monoaçilgliserol lipazı veya endokannabinoid uptake'ini inhibe ederek kaşınma davranışını azaltmaya çalıştık. Kaşıntı davranışı, Balb/c farelere intradermal serotonin enjeksiyonu ile oluşturulmuştur. Yağ asid amid hidrolaz inhibitörü olan URB 597 (10 mg/kg, i.p.), monoaçilgliserol lipaz inhibitörü olan JZL 184 (16 mg/kg, i.p.) ve endokannabinoid transport inhibitörü AM 404 (10 mg/kg, i.p.) endokannabinoid modülasyonunun kaşıma yanıtları üzerindeki etkilerini değerlendirmek için verilmiştir. Daha sonra kannabinoid reseptörlerinin endokannabinoid sistem modulatörlerinin antipruritik etkilerine aracılık edip etmediğini belirlemek için kannabinoid-1 reseptörünün antagonisti AM 251 (1mg/kg, i.p.) ve kannabinoid-2 reseptörünün antagonisti SR 144528 (1mg/kg, i.p.) uygulandı. URB 597 ve JZL 184 serotonin ile oluşturulan kaşıntıyı azalttı fakat AM 404'ün ise bir etkisi olmadı. URB 597'nin inhibe edici etkisi, SR 144528 tarafından tersine çevrildi, fakat kannabinoid reseptör antagonistlerinin, inhibitörlerin modülasyonu üzerinde başka hiçbir etkisi yoktur. Considering the antinociceptive effects of blockade of endocannabinoid degrading enzymes in pain states, we attempted to reduce scratching behavior by endocannabinoid modulation, i.e. by inhibiting fatty acid amide hydrolase, monoacylglycerol lipase, or cellular uptake of endocannabinoids. Scratching behavior was induced by intradermal injection of serotonin to Balb/c mice. URB 597 (10 mg/kg, i.p.), afatty acid amide hydrolase inhibitor, JZL 184 (16 mg/kg, i.p.), a monoacylglycerol lipase inhibitor, and AM 404 (10 mg/kg, i.p.), an endocannabinoid transport inhibitor, were given to evaluate the effects of endocannabinoid modulation on scratching responses. Then, the cannabinoid-1 receptor antagonist, AM 251 (1 mg/kg, i.p.), and the cannabinoid-2 receptor antagonist, SR 144528 (1 mg/kg,i.p.), were administered to determine whether cannabinoid receptors mediate these effects. URB 597 and JZL 184, but not AM 404, attenuated serotonin-induced scratches. The inhibitory effect of URB 597 was reversed by SR 144528, but cannabinoid receptor antagonists had no other effects on modulation by the inhibitors. We propose that augmenting the endocannabinoid tonus by inhibition of degradative enzymes, fatty acid amide hydrolase and monoacylglycerol lipase, but not cellular uptake, may be a novel target for the development of antipruritic agents. 62

Termination of the TDF therapy because of side effects is very rare. Pruritus is not taken into account as a reason of termination in the literature. Itchiness is the desire for mechanical irritation of the skin and it may result from various dermatological and sometimes neurological diseases. Neurological causes for itch are relatively unknown and frequently overlooked. Here, we report a case of possible acute inflamatory polyradiculopathy who had facial pruritus and diplopia as the initial symptoms. Diagnosis of our patient was delayed because initial diplopia and pruritus complaints were considered as nonspecific and unrelated to the nervous system. We suggest that pruritus may be a remarkable neurological finding especially when it is accompanied by other neurological symptoms. [ABSTRACT FROM AUTHOR]

Creutzfeldc-Jacob's disease is a rare and fatal neurodegenerative disease characterized by the accumulation of abnormal prion-like proteins in the central nervous system. Clinical features, electroencephalography (EEG), magnetic brain resonance imaging (MRI) and protein 14.3-3 is used in diagnosis. Protein 14.3.3 may be negative in the early or late stages of the disease. Presentation with psychiatric symptoms and itching is not typical in the onset of the disease. Here we present a patient who was first admitted to the psychiatry clinic due to his psychiatric symptoms and had severe itching resistant to antihistamine treatment. Foreign bodies in the external canal of the ear are an emergency problem encountered by the otolaryngologist almost every day. Foreign bodies may cause hearing loss, pain, severe itching, ear discharge, tinnitus and vertigo. There is also great risk for complications during the removal of foreign bodies. An 82 years old male patient with dementia suffering from hearing loss, pain, itching and ear discharge in both ears was referred to our clinic as an emergency case. Otoscopic examination revealed multiple nonmetallic foreign bodies, whitish/yellowish cottony material, at both ears. All cotton foreign bodies were removed with alligator in the surgery room without use of any anesthetic agents. The management of patients with ear foreign bodies must be carried out by the otorhinolaryngologist or skilled professional with the use of suitable material to minimise complications or prevent further damages., Dış kulak yolu yabancı cisimleri hemen her gün kulak burun boğaz uzmanları tarafından karşılaşılan bir acil sorundur. Yabancı cisimler işitme kaybı, ağrı, şiddetli kaşıntı, kulak akıntısı, çınlama ve baş dönmesine neden olabilir. Aynı zamanda yabancı cismin çıkarılması sırasında da komplikasyonlar açısından büyük riskler vardır. 82 yaşında demans hastalığı olan ve işitme kaybı, ağrı, şiddetli kaşıntı, kulak akıntısı rahatsızlığı çeken erkek hasta kliniğimize acil vaka olarak yönlendirildi. Otoskopik muayenesinde her iki kulakta metalik olmayan beyaz/sarımsı renkte pamuksu görünümü olan yabancı cisimler tespit edildi. Tüm pamuk yabancı cisimler cerrahi odasında herhangi bir anestezik ilaç kullanmaksızın aligatör yardımıyla çıkarıldı. Kulakta yabancı cisim olan hastalara yaklaşım komplikasyonların azaltılması ya da ileri hasarların önlenmesi açısından tecrübeli eleman yada KBB uzmanı tarafından uygun ekipman kullanılarak yapılmalıdır.

Scabies is an infestation caused by Sarcoptes scabiei and characterised by polymorphous lesions that may include burrows, papules, pustules, crusts and excoriations. Several pruritic diseases may be confused with scabies. Herein, we present a case of scabies with lesions resembling perforating folliculitis diagnosed on the basis of both clinical and histopathological view. A 72-year-old man with type 2 diabetes mellitus and receiving hemodialysis for ten years due to end-stage renal disease was admitted to our dermatology department with a 6-month history of severe pruritus. Based on the results of skin biopsy revealing Sarcoptes scabiei in the epidermis, the patient was diagnosed as scabies and was successfully treated with 5% permethrin. This case is presented to emphasize that scabies should be considered in the differential diagnosis in cases of chronic pruritus. [ABSTRACT FROM AUTHOR]

Kısa etki süresi ve ender yan etkileri nedeniyle propofol özellikle günübirlik cerrahide yaygın kullanılan bir anesteziktir. . Propofol uygulaması sonrası gelişen anormal hasta hareketleri ve nöbet benzeri aktiviteler bildirilmiştir. Propofol ile indüksiyonu yapılan elektif tüp ligasyonu planlanmış 28 yaşındaki kadın hastada indüksiyon amaçlı propofolün tamamı intravenöz (IV) olarak yavaş yavaş verildikten hemen sonra baş boyun bölgesinde sanki boyunda kaşıntı varmış gibi hızlı ritmik şekilde başın sağa sola hareketleri başladı. Operasyondan sonra hasta serviste sorgulandığında omuzlarından başlayan tüm boynu çene altına kadar kapsayan şiddetli bir kaşıntı hissettiğini belirtti. Biz bu olgu sunumunda, kaşıntı ile birlikte fokal bir bölgede ritmik istenmeyen hareketleri tartışmak istedik., Propofol with anesthesia is widely used especially for short duration of action and rare side effects of especially in day surgery. Patients with an abnormal movements after propofol administration and seizure-like activity have been reported.Induction with propofol for elective tubal ligation 28-year-old female patient after administration of propofol intravenously slowly redness in the head and neck region and the fast rhythmic movements of the head fro occurred.Patient go to after surgery service that we see covering all the neck under the chin stated that he felt a severe itching. In the present study we wanted to discuss along with itching in a focal region with rhythmic unwanted movements.

Bu yüzden tırmalama, yalama, emme, ısırma ve çiğneme gibi hayvanda görülen bu gibi davranışların kaşıntı olabileceğinden şüphelenmekteyiz. Kaşıntı sekunder bakteriyel deri enfeksiyonları (piyoderma) ve sekunder mantar enfeksiyonları dahil olmak üzere diğer cilt hastalıkları ile ilişkili olup, allerjiler ve deri parazitleri gibi cilt hastalıklarının da ana semptomudur. In fact, no one strictly knows the actual sensation concerning of pruritus in dogs, so we just suspect pruritus from their behavior while grooming, scrathing, licking, sucking, biting, and chewing. Pruritus is associated with other skin diseases, including secondary bacterial skin infections (pyoderma) and secondary yeast infections, and it is the main symptom of skin conditions like allergies and skin parasites.

All people have pruritus even once in their lives. Acute pruritus, like insect bite, is having pruritus during a limited duration changing from seconds to weeks. Chronic pruritus is a kind of pruritus with a longer duration than 6 weeks. Chronic pruritus is a worldwide problem. It is encountered in all ages and a has great effect to quality of life. Other classifications exist other than acute or chronic pruritus. International Forum for the Study of Itch (IFSI)classifies itch into three groups: pruritus in diseased skin(group 1), pruritus in non-diseased skin(group 2), pruritus secondary to scratching lesions (group 3). Besides, it is classified into six specific groups according to the underlying disease: dermatologic, systemic, neurologic, psychogenic, mixed and others., Tüm insanlar hayatlarında bir kez de olsa pruritusu yaşamışlardır. Akut kaşıntı, böcek ısırması gibi, saniyelerden haftaya kadar değişen sınırlı süre kaşıntı yaşanmasıyken, kronik kaşıntı ise 6 haftadan uzun kaşıntılara verilen addır. Kronik pruritus dünya genelinde bir problemdir. Her yaşta görülebilir ve yaşam kalitesine büyük etkisi vardır. Kaşıntı için akut ve kronikten başka sınıflamalar da vardır. Hastalığın tanısı klinik, elektroensefalografi (EEG), kraniyal MRG ve 14.3.3 proteini incelemeleri ile konur. Protein 14.3.3, hastalığın erken veya geç fazında negatif olabilmektedir. Psikiyatrik bulgular ve kaşıntı ile klinik başlangıç hastalık için tipik değildir. Bu yazıda psikiyatrik bulguları nedeniyle psikiyatri kliniğine yatırılan ve antihistaminik ile tedaviye dirençli kaşıntı şikayeti olan bir hasta sunulmuştur.

TENS cihazı kaşıntılı deri hastalıklarında etki mekanizması net olmamakla birlikte antipruritik etki sağlayabilmektedir. Böylelikle TENS cihazının kullanımıyla liken simpleks kronikus, jeneralize pruritus, atopik dermatit, prurigo nodülaris, notaljiya parestetika, mikozis fungoides ve hematolojik hastalıkların neden olduğu kaşıntılarda; ayrıca reaktif perforan kollajenozların ve yanıkların neden olduğu kaşıntıların giderilmesinde kullanılmış ve yarar sağlandığı daha önceden rapor edilmiştir. Klinik yan etkisiyle ilgili bildirilmiş kanıtlar yoktur. Derlememizde kaşıntılı deri hastalıklarının tedavisinde standart tedavilere alternatif olarak transkutanöz elektriksel sinir uyarımı (TENS) kullanımının yararını göstermeyi amaçlıyoruz. TENS device can provide antipruritik effects although its mechanism of action is not clear on skin diseases. By this way, TENS device is used for lichen simplex chronicus, generalized pruritus, atopic dermatitis, prurigo nodularis, notalgia paraesthetica, mycosis fungoides “and pruritus due to hematological diseases,and also” itch caused by reactive perforating collagenosis and burns; in which these benefits of TENS device have been reported previously.There is no reported evidence related to clinical side effects of the device. In this review , we aim to indicate the benefit of the use of transcutaneous electrical nerve stimulation (TENS) as an alternative to standard therapies in the treatment of itchy skin diseases.

Malassezia dermatitisi, çoğunlukla sağlıklı köpeklerin deri ve dış kulak yolundan isole edilen ortakçı lipofilik bir maya olan Malassezia pachydermatis (pytirosporum canis) tarafından meydana getirilen bir enfeksiyondur. Yüz, karın altı, kulaklar, dış kulak kanalı ve interdigital bölgeler en yaygın olarak etkilenen kısımlardır. Malassezia dermatitisi’nin köpeklerdeki başlıca semptomları şiddetli kaşıntı, likenifikasyon, eritem ve hiperpigmentasyondur. Sağaltıma cevap vermeyen şiddetli kaşıntı, otitis eksterna, dermatitis, eritem, likenifikasyon ve hiperpigmentasyon durumlarında Malassezia dermatitisi’nin göz önünde bulundurulması son derece önemlidir. Malassezia dermatitis is infection that occurs by Malassezia pachydermatis (Pytirosporum canis) is a commensal lipophilic yeast which is frequently isolated from the external ear canal and from the skin of healthy dogs. Face, ventrum, ears, external ear canal and interdigital zones are the most affected areas. The major symptoms Malassezia dermatitis in dogs are severe pruritis, lichenification, erythema, and hyperpigmentation. İt was extremely important that Malasseszia dermatitis should be considered in non- responsive dermatological cases characterized with intense pruritis, otitis externa, dermatitis, erythema, lichenification,hyperpigmentation.

Progresif familyal intrahepatik kolestaz PFİK , çocukluk çağı kolestazları içinde önemli bir yer tutmaktadır. Bazı karaciğer nakil merkezlerinde en sık karşılaşılan ikinci karaciğer nakil endikasyonudur. Tüm kolestazlı ve karaciğer nakli yapılan olguların yaklaşık %10-15’ini oluşturmaktadır. PFİK, otozomal resesif geçişlidir ve hepatosellüler kaynaklı kolestaza yol açmaktadır. Süt çocukluğundan erişkin döneme kadar olan dönemde karaciğer yetmezliğine yol açarak ölüme neden olabilmektedir. PFİK’nin üç tipi vardır ve tüm tiplerde en sık gözlenen klinik bulgular kaşıntı, sarılık, büyüme geriliği, hepatomegali, splenomegalidir. PFİK’nin üç tipinin de prognozu farklıdır. PFİK’nin komplikasyonları portal hipertansiyon, karaciğer yetmezliği, siroz, hepatosellüler karsinom ve ekstrahepatik bulgulardır. Olguların hayat kalitesini yükseltecek medikal ve cerrahi tedaviler uygulanmakla birlikte çoğu olguya erken dönemde karaciğer nakli gerekmektedir, Progressive familial intrahepatic cholestasis PFIC has a significant place among the childhood cholestasis. It is the second most frequent liver transplant indication met in some liver transplant institutions. It constitutes approximately 10-15% of all cases of cholestasis and liver transplant. PFIC is an autosomal recessively inherited disorder and causes hepatocellular-originated cholestasis. In the period from infancy to adulthood it may cause death due to liver failure. PFIC has three types and the most frequently observed clinical symptoms of all three types are; pruritus, hepatitis, growth retardation, hepatomegaly, and splenomegaly. The prognosis of these three types of PFIC is different. The complications of PFIC are; portal hypertension, liver failure, cirrhosis, hepatocellular carcinoma, and extrahepatic symptoms. Although medical and surgical treatments are applied in order to improve the patients’ life quality, most of the cases require liver transplantation in the early stage
