Aim: Despite medical advancements, heart failure (HF) maintains high mortality rates. Our research delves into examining the relationship between the serum creatinine/albumin ratio and one-year mortality in patients with decompensated systolic HF. Methods: During the period from October 2014 to October 2015, we enrolled 80 patients (comprising 37 females) who had been diagnosed with acute systolic decompensated heart failure and had a left ventricular ejection fraction (LVEF) of ≤ 40%. These patients were divided into two cohorts depending on whether they experienced all-cause mortality within the span of one year. Results: Among the 80 participants, 31 (39%) experienced mortality within the first year. The average age of the deceased group was 69±14 years, with 38.7% (n=12) being female. In contrast, the surviving group had an average age of 66±12 years, with 51% (n=25) being female. The HF group with mortality exhibited significantly higher levels of serum creatinine-albumin ratio, urea, and creatinine values, along with a higher prevalence of pretibial edema (p<0.01).Furthermore, the deceased HF group exhibited significantly lower LVEF, albumin levels, lymphocyte counts, and systolic and diastolic blood pressure values. Statistical analysis revealed a significant difference in the serum creatinine/albumin ratio between the deceased group (0.68±0.27) and the surviving group (0.38±0.18), with a p-value of less than 0.01. Using a cut-off value of 0.45 for the creatinine/albumin ratio, the sensitivity and specificity for predicting one-year mortality in HF patients were 81% and 78%, respectively. Conclusion: The conjunction of heightened creatinine levels, diminished albumin levels, and an augmented creatinine/albumin ratio could potentially function as straightforward, economical prognostic indicators for forecasting one-year mortality in systolic decompensated HF patients. Background: To evaluate the prognostic value of blood urea nitrogen/creatinine ratio (BUN/SCr) and cystatin C (Cys C) in patients with renal cell carcinoma (RCC) after radical nephrectomy. Methods: The study analysed 348 patients with RCC who underwent radical nephrectomy. The optimal cut-off was obtained based on the ROC of specific survival outcomes and the maximum Youden index. The patients were divided into four groups: Group 1 (low BUN/SCr-low Cys C), Group 2 (low BUN/SCr-high Cys C), Group 3 (high BUN/SCr-low Cys C), and Group 4 (high BUN/SCr-high Cys C). The primary endpoint was cancer-specific survival (CSS), and the secondary endpoint was disease-free survival (DFS). Results: A strong positive correlation was shown between BUN/SCr value and Cys C level. Patients with a higher BUN/SCr ratio (17.41) and Cys C level (3.98 mg/L) had poorer survival outcomes. Notably, patients in group 4 showed the worst CSS and DFS rates, while patients in groups 1 and 2 had better survival outcomes with no significant difference between the two groups. A higher BUN/SCr ratio (17.41) and high Cys C serum level (3.98 mg/L) were independent predictors of CSS and DFS, in addition to preoperative tumour size and pathological T (pT) stage. Conclusion: This study provides the first evidence of the independent prognostic importance of BUN/SCr ratio and Cys C in patients with RCC after radical nephrectomy. [ABSTRACT FROM AUTHOR]

A 62-year-old female patient with a history of cystic bronchiectasis was brought to the emergency department with complaints of weakness, nausea, and shortness of breath. The patient was hospitalized at the internal medicine service for the accompanying acute renal failure, hypocalcemia, hypokalemia, and hypomagnesemia. The patient was started on oral active vitamin D therapy. Intravenous calcium treatment was given to the patient with a corrected calcium value of 6.4 mg/dl. In addition, intravenous potassium and intravenous magnesium replacement were performed. The patient's fluid intake and output were monitored to prevent fluid overload. The creatinine value of the patient decreased to the normal limits after five days of treatment. The patient, who had no electrolyte imbalance and whose complaints regressed, was discharged on the 8th day of hospitalization with recommendations. Clinicians should be careful about the potential risk of accompanying renal failure and electrolyte imbalance in patients with cystic bronchiectasis. [ABSTRACT FROM AUTHOR]

Aim: Rhabdomyolysis is a rare clinical entity. Its manifestation might be acute or delayed, with an acute episode frequently accompanied by severe clinical signs, while delayed rhabdomyolysis presentation has a more benign progression. Case report: Here we report a case of a 70-year-old patient with combined rhabdomyolysis, brachial plexus injury, second-degree burn injuries and acute kidney injury with an unusual cause of injury and subsequent mild clinical course. Conclusion: Even with a benign clinical course, one must think of rhabdomyolysis in uncertain injury mechanisms or questionable circumstances, and prompt laboratory diagnosis and fluid supplementation should be undertaken. Objective: For end-stage renal disease, kidney transplantation is the best therapy. Early graft function is critical for kidney transplant success. The purpose of this study was to see how mean arterial pressure (MAP) affected early graft function and biochemical outcomes. Methods: This was a retrospective study of transplanted patients from live-related donors at the Transplant Center Hospital, Tirana, Albania, between January 2007 and December 2022.The patients divided based on their mean MAP. One group had MAP above 100 mmHg (68 patients). The second group had MAP between 85-100 mmHg (74 patients) and the other group MAP less than 85 mmHg (52 patients). Serum creatinine decrease was observed during the first three post-operative days and the sixth day. For statistical computation, correlation analysis, analysis of variance test (ANO-VA), and multivariate analysis technique were used in this study. Results: On the first, second, and third postoperative days, the first and second groups’ mean serum creatinine values were comparable to the third group’s mean values. The mean MAP at the time of declamping was 108.64 mmHg for the first group, 95.28 mmHg for the second group, and 80.18 mmHg for the third group. The mean MAP ranged from 78 to 118 mmHg. There was no significant difference between the first two groups on the first, second, third, and sixth postoperative days, but there were significant differences between the first and second groups compared to the third group on the first, second, and third postoperative days. Acute kidney injury (AKI) is one of the important complications of the perioperative period, and associated with increased risk of chronic kidney disease, renal replacement therapy requirements, increased cost, and risk of mortality. In this overview, we summarized baseline confounders and surgical procedure related risk factors contributing to the perioperative AKI, which may serve as risk scores, improve early diagnosis, contribute to prevention, and early management of AKI. There are immediate needs for context specific clinical prediction scores and novel biomarkers to very early diagnose AKI. Preventive guidance provided by Kidney Disease: Improving Global Outcomes is a helpful practical tool for clinicians. Potential roles of novel biomarkers and their context specific contributions require further exploration and close attention. Perioperative hemodynamics and oxygenation appear to contribute to AKI. Therefore, while their optimization can be recommended, their detailed and context specific roles need further explored. Aim: In recent years, an increasing number of evidence suggests that the inflammation plays a significant role in the pathophysiology of ST-segment elevation myocardial infarction (STEMI). The association between inflammatory markers and renal functions in STEMI prognosis has been previously reported. No studies have investigated the ability of the serum albumin to creatinine ratio (sACR), defined as a prognostic score, to predict long term mortality in patients experiencing STEMI. Material and Method: This retrospective study included 1133 patients experiencing STEMI. The study population was divided into two groups according to survival and analyzed whether the sACR was an independent predictor of long-term mortality. Results: Out of 1133 patients, death was observed in 112 patients (9%) an average follow-up of 55 months. During the total follow-up period, patients were divided into two groups according to survival. Compared to the survival group, long-term mortality group was older, had a higher SYNTAX score and a lower sACR (p<0.05). Independent predictors of long-term mortality were found to be age, smoking, LVEF, and sACR (HR: 0.627 95% CI: 0.533–0.737; p<0.001). Receiver operating characteristic (ROC) curve comparisons for long-term mortality demonstrated that the sACR was a better predictor than both albumin and creatinine, separately. Conclusion: The present study revealed that sACR is an independent predictor of long-term mortality in patients experiencing STEMI. As a marker which can be easily obtained and calculated, sACR can be an effective parameter used for prognosis estimation of STEMI. [ABSTRACT FROM AUTHOR]

Amaç: Hastanemiz iç hastalıkları polikliniklerine başvuran 65 yaş ve üzerindeki tip 2 diyabetes mellitusun eşlik ettiği hastalarda anemi sıklığını saptamayı ve aneminin morfolojik olarak dağılımını değerlendirmeyi amaçladık. Yöntem ve gereçler: Dereli İlçe Devlet Hastanesi iç hastalıkları polikliniğine 01.01.2021– 01.01.2022 tarihleri arasında başvuran 65 yaş ve üzerindeki tip 2 dm’nin eşlik ettiği hastalar, retrospektif olarak incelenerek çalışmaya alındı. Hastaların hemogram ve biyokimya tetkikleri değerlendirildi. Hastaların anemi sıklığı ile yaş, cinsiyet, kronik renal yetmezlik ilişkileri değerlendirildi. Veriler SPSS programına kaydedilip, istatistiksel analizler yapıldı. Bulgular: Çalışmamızda anemi sıklığı %24,4 olarak saptandı. Anemisi olan hastalar MCV değerlerine göre %26’ sı (26) mikrositer, %72’ si (72) normositer, %2’ si (2) makrositer anemi olarak sınıflandırıldı. Çalışmamızda kadınlarda, erkeklere göre anemi sıklığı açısından istatistiksel anlamlı farklılık tespit edildi (p=0,016). Artan yaş grupları ile anemi sıklığı arasında istatistiksel anlamlı farklılık gözlenmedi (p=0,088). Geriatrik diyabetik hastalarda kronik renal yetmezlik ile anemi arasında istatistiksel anlamlı farklılık tespit edildi (p=0,008). Sonuç: Çalışmamızdaki hastalarda anemi sıklığı literatürle uyumlu olarak %24,4 olarak saptandı. Geriatrik diyabetik hasta popülasyonu gittikçe artmaktadır. Ülkemizde geriatrik gruptaki diyabetik hastalarda anemi ile ilgili çalışmalar kısıtlı olup, çok merkezli prospektif çalışmalara gereksinim duyulmaktadır.

Lipopolysaccharide (LPS) is a constituent of Gram-negative bacterial cell walls, thus LPS injection has been widely used as a model of experimental acute kidney injury associated with sepsis. LPS-induced sepsis is caused by excessive secretion of proinflammatory mediators and reactive oxygen species (ROS). The neurohormone melatonin, which is mainly secreted by the pineal gland, regulates the circadian rhythm, has an anti-inflammatory and immunoregulatory role. Melatonin and its metabolites have been shown to scavenge various free radicals and reactive oxygen intermediates. The aim of this study was to evaluate the effect of melatonin in preventing endotoxemia-induced kidney damage caused by LPS, by analysing the concentration of urea and creatinine in the blood serum of rats. Twenty-eight Wistar albino rats were randomly divided into four groups (n = 7 per group) as follows: 1) Control group, 2) MLT group (50 mg/kg, per os), 3) LPS group (10 mg/kg, i.p.) and 4) LPS + MLT group (10 mg/kg + 50 mg/kg). Serum levels of creatinine and urea were significantly higher (p<0.05) in the LPS-treated animals compared with values in the control group. Co-application of LPS and MLT significantly reduced an increase in serum creatinine and urea levels (p<0.05). It can be concluded that oral administration of melatonin significantly alleviates LPS-induced acute nephrotoxicity in rats. It is likely that the beneficial effects of melatonin are related to its known antioxidant effects on kidney tissue, and possibly to some other less known/studied effects. [ABSTRACT FROM AUTHOR]

Background: COVID-19 is a new pandemic that has infected millions of people worldwide and caused a high morbidity and mortality rate. COVID-19 may have a harmful effect on organs, especially the kidneys. Aims: The main aim of our research is to study the association between the severity of COVID-19 disease and biochemical parameters related to kidney function and to investigate certain risk factors of COVID-19-associated kidney disease. Methods: A total of 174 individuals, 121 COVID-19 positive and 53 COVID-19 negative, were enrolled in this study. The relation between COVID-19 infection, severity, kidney function test, and hematological indicators were examined. Results: The most prominent symptoms among COVID-19 were fever (95%) and fatigue (92%). Regarding biochemical parameters, median creatinine, MPV, and CRP were significantly higher in COVID-19 patients, whereas median eGFR, Na+, WBC, MCH, MCHC, and eosinophil percentages were significantly lower in this group. Severely infected patients were observed to have higher urea, creatinine, neutrophils, and NLR. However, median sodium, eGFR, hemoglobin, hematocrit, RBC, lymphocytes, and platelet count were significantly lower in the severe group. Urine examination of the severe group showed a significantly lower specific gravity, while urine pH, protein, and glucose were significantly higher. Conclusions: Our analysis indicates that COVID-19 infection affects kidney function, mainly creatinine level, urea, eGFR, Na+ and urine protein. Additionally, comorbidities such as older age (≥65), hypertension, taking medications, and CRP (≥33.55 mg/L) are considered risk factors that are more likely to contribute to kidney impairment in COVID-19 positive patients. Objectives: Cerebral oxygenation may decrease due to hypoxia during one lung ventilation (OLV) in thoracic surgery. Diabetic patients are at increased risk of cerebral and renal damage after major surgery due to microangiopathy. The primer aim of our study is to evaluate whether cerebral oxygenation is affected during OLV with near infrared spectroscopy (NIRS) and arterial/central venous oxygen saturation of the central venous blood (ScvO2 ) blood gases in diabetic patients. The secondary aim of our study is to investigate whether there is a relationship between cerebral oxygenation and changes in renal functions in diabetic patients. Methods: Diabetic patients underwent lung resection between 2018 and 2021 were included in our prospective, case-controlled study. Regional oxygen saturation (rSO2 ) and hemodynamic parameters of the cases were recorded every 15 min from the pre-operative period to extubation. Central venous and arterial blood gas samples were taken from the patients during single lung ventilation and double lumen ventilation. They were recorded simultaneously with rSO2 and hemodynamic data. Pre-, post-operative creatinine values were checked. Results: Fifty-two patients were included in the study. It was completed with 19 diabetic and 22 normal blood sugar patients. rSO2 decreased according to baseline measurements in both groups at 30 min during OLV. While rSO2 values were lower in OLV 60 min and throughout the DLV in diabetic patients, it was around basal values in control group. Hemodynamic data and ScvO2 values were similar. A correlation was observed between rSO2 values and creatinine values during OLV in diabetic patients. Conclusion: In our study, it was shown that cerebral oxygen values measured by NIRS during OLV in lung surgery decreased more in diabetic patients than control group, but were not within the pathological limits. Purpose: In 2021, the CKD-EPI-creatinine and CKDEPI-creatinine-cystatin-C combined equations were revised again by the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) and the National Institute of Diabetes & Digestive and Kidney Diseases (NIDDK) and it was reported that its accuracy was increased. The main object of this study is to compare the diagnostic accuracy and performance of 2009 CKD-EPI eGFRcr, 2012 CKD-EPI eGFRCysC, 2012 CKD-EPI eGFRcr-CysC, MDRD, and revised 2021 CKD-EPI to provide a better estimation. Materials and Methods: The study design was prospective. The sample consisted of 111 CKD patients and 35 healthy individuals who applied to the nephrology clinic. All participants were evaluated by a nephrologist. The participants were divided into study groups according to their KDIGO classification. Five variables were used: age, race, gender, serum creatinine, and serum cystatin-C. Results: 2012 CKD-EPI eGFR cystatin-C out performed existing equations in terms of accuracy, specificity, and sensitivity (AUC:0.988). Furthermore, when both creatinine and cystatin-C are included in an equation, it consistently improves diagnostic values over formulas containing only creatinine (2009CKD-EPI eGFRcr AUC:0.953, 2012CKD-EPI eGFRcr-CysC AUC: 0.985, 2021CKD-EPI eGFRcr AUC:0.954, MDRD AUC: 0.953, 2021CKD-EPI eGFRcr-CysC AUC: 0.985). Conclusion: The new formula has not been confirmed to be superior to other equations in its ability to estimate eGFR values, particularly at higher levels of chronic kidney disease. When compared with existing equations, 2012 CKD-EPI eGFRCysC had higher specificity and sensitivity. Combined formulas containing both blood tests had higher diagnostic values than equations containing only creatinine. Amaç: Çalışmamızda alkol/madde kullanım bozukluğu (AMKB) tanılı hastalarda bazı laboratuvar parametrelerini hem birbirleri arasında hem de sağlıklı kontrol grubu ile karşılaştırarak incelemeyi amaçladık. Materyal ve Metot: Çalışmaya 25 Aralık Gaziantep Devlet Hastanesi AMATEM (Alkol ve Madde Bağımlılığı Tedavi Merkezi) servisinde yatarak tedavi gören DSM-5 kriterlerine göre opioid, metamfetamin, alkol, çoklu madde kullanım bozukluğu tanılı hastalar alındı. Toplamda 729 kişi; 612 kişi hasta grubu ve 117 kişi sağlıklı kontrol grubu olarak dahil edildi. Katılımcıların hastane sisteminden geriye dönük olarak laboratuvar parametreleri incelendi. Bulgular: Çalışmamıza alınan hastalardan 23 kişi kadın iken 706 kişi erkek idi. Genel yaş ortalaması 40.91±29.58 yıl olarak hesaplandı. Laboratuvar parametreleri incelendiğinde kan elektrolit düzeylerinden sodyum değeri opioid, metamfetamin, çoklu madde kullanım bozukluğu olan hastalarda sağlıklı kontroller ve alkol kullanım bozukluğu olan hastalara göre daha yüksekti (F=14.651, P

Introduction: renal injury occurs as one of the most common causes of morbidity and mortality in patients with multiple myeloma (MM), as patients with MM are characterized by increased production of light chains of immunoglobulins. Aim: evaluation of correlation between potential parameters for early detection of renal injury in MM. Materials and methods: this cross-sectional study involved 62 patients with MM. They were characterized in three different stage groups of the disease using Salmon-Durie classification and by International Staging System. Blood samples were drawn to measure concentration of Cystatin C, Immunoglobulins, Free light chains (FLC), C-reactive protein (CRP), polyclonal chains, while urine is collected to measure creatinine level. All measurements were evaluated using the ELISA and nephelometry methods. Spearman correlation test was used to examine the relationship between the variables. Results: correlations between tested parameters in all patients regardless of MM classification were: Cystatin C significantly correlate with creatinine (p<0.001) and CRP (p=0.004), Cystatin C significantly correlated with serum kappa chains (p<0.05), and kappa chains significantly correlated with CRP (p <0.05) and serum λ chains (p=0.002). In the initial phase of MM, Cystatin C was observed to correlate with CRP (p<0.001). In MM at steady phase, statistically significant correlation was found between Cystatin C and creatinine (p<0.001) and creatinine and serum kappa and λ chain ratio (p<0.05). In MM at relapse phase, statistically significant correlation was found between Cystatin C and creatinine (p<0.001), Cystatin C and C reactive protein (p<0.05), Cystatin C and serum kappa chains (p<0.05) and creatinine clearance and serum kappa and λ chain ratio (p<0.05). In ISS stage 1, statistically significant correlation was found between Cystatin C and creatinine (p<0.05). In ISS stage 2, statistically significant correlation was found between Cystatin C and creatinine (p<0.05), Cystatin C and serum kappa chains (p<0.05) and Cystatin C and serum kappa and lambda chain ratio (p<0.05). In ISS stage 3, statistically significant correlation was found between Cystatin C and creatinine (p<0.05), Cystatin C and CRP (p<0.05) and CRP and creatinine (p<0.001). Conclusion: this study confirms that serum Cystatin C significantly correlates with creatinine level as well as with elevated serum κ chains prior to increase of serum creatinine levels, as a potential parameter of early detection of renal dysfunction and injury. Objective: The current study investigates the development of acute kidney injury (AKI) in patients undergoing major abdominal surgery via earlier determination of the rise in neutrophil gelatinase-associated lipocalin (pNGAL) compared to that in creatinine. Methods: In this prospective observational study, 60 patients aged between 18 and 65 undergoing major abdominal surgery were selected for the investigation after obtaining ethics committee approval. Patients who did not meet the age criteria, had liver and kidney failures, severe cardiac and respiratory distress, used angiotensin-converting enzyme inhibitors and nonsteroidal anti-inflammatory drugs, developed postoperative respiratory failure, cardiogenic and septic shock, relapse in the 1st 24 h, and those who underwent emergency surgery were excluded from the study. Leukocyte and creatinine values (preoperatively and at 24 and 48 h postoperatively), pNGAL (at 0 h intraoperatively and 6 h and 24 h postoperatively), lactate values (at 0 h intraoperatively and 24 h and 48 h postoperatively), and urine output value (at 24 and 48 h postoperatively) were evaluated. Results: Of the 60 patients, 9 (15%) showed increased pNGAL values at 6 h postoperatively; however, increased creatinine values and decreased urine output values were observed at 48 h postoperatively. Thus, AKI development was detected at an early stage. The change in leukocyte and lactate values was not statistically significant. Background/Aim. Renal transplantation is the best and preferred way of treating patients with end-stage renal disease, as it offers improved survival and better quality of life compared to dialysis. The aim of this study was to present single-center (Military Medical Academy in Belgrade, Serbia) results of the kidney allograft and patient survival from 1996 to 2017. Methods. A retrospective 22-year cohort study was conducted. Variables of interest were graft and patient survival in kidney transplanted patients. Age, gender, serum creatinine levels, and induction therapy after transplantation were analyzed in this group of patients as well. Results. Among 386 transplanted patients, 316 had a living donor and 70 patients had a deceased donor. Pre-emptive renal transplantation was done in 29 (7.5%) patients and AB0-incompatible kidney transplantation in 21 (5.4%) patients. One-year, 5-year, 10-year, and 20-year overall patient survival after kidney transplantation in the observed group was 97.7%, 95.3%, 93.8%, and 91.7%, respectively. One-year, 5-year, 10-year, and 20-year graft survival in our patients were 93.8%, 85.5%, 78.5%, and 73.3%, respectively. Conclusion. Objective: We aimed this study, investigated the changes in pre- and post-treatment hematological parameters, liver and kidney function parameters in children that were diagnosed with epilepsy and initiated on levetiracetam (LEV) therapy. Material and Methods: The study population consisted of 114 children (6-18 years) had normal growth percentiles, that were treated for new-onset epilepsy with LEV monotherapy. In each patient, hematological parameters, B12, ferritin, liver and kidney function parameters were measured before and one/three years after the initiation of the therapy. Results: The hemoglobin (Hgb) and hematocrit (Htc) levels showed a significant increase and the absolute lymphocyte count (ALC), absolute neutrophil count (ANC) (p=0.000), monocyte percentage (p=0.032), and mean platelet volume (MPV) (p=0.000) levelsshowed a significant decrease. The ALC levels decreased significantly in 6 (5.3%) children. The mean drug dose at three years of treatment was 30.0±5.6 mg/kg/day. No significant difference was found between preand post-treatment platelet (PLT) counts and no significant correlation was found between gender and hematological parameters (p>0.05 for both). Conclusion: Although LEV monotherapy led to changes in the hematological parameters of the epilepsy patients, no significant change was observed in liver function. We suggest that when evaluating the hematological parameters in children with epilepsy, the community's predisposition should be considered and also children should be examined for iron deficiency and vitamin B12 deficiency anemia before initiating the LEV therapy. Medical record data were analyzed for serum creatinine level in dogs to determine useful associations between the data. Differences in sex, age, and breed were analyzed using Fisher's exact test, and multiple factors such as sex were analyzed by contingency table analysis. Of the 3347 dogs that were tested for serum creatinine level, 243 dogs had creatinine over 1.4 mg/dL more than once. The overall rate of renal dysfunction in all breeds was 7.3%, but the rate for cavalier King Charles Spaniels was 14.1% (P<0.05), and for Shetland Sheepdogs it was also 14.1% (P<0.05), both significantly higher than the overall rate. Aim: Hemiarthroplasty is an appropriate treatment option for hip fractures that provides early mobilization, and good functional results. In the available literature, there are few studies that particulary investigate the risk factors affecting the 30-day mortality after hemiarthroplasty in geriatric patients with hip fractures. This study aimed to determine patient-related factors and biochemical predictors, which were easily accessible, inexpensive, and routinely examined in the perioperative period of 30-day mortality in geriatric patients after hemiarthroplasty for femoral neck fractures. Methods: We enrolled 169 patients in our study, retrospectively. Age, body mass index, hospitalization time from admission to surgery, total hospitalization time, comorbidities, American Society of Anesthesiologists scores, blood transfusions, anticoagulant medications, albumin and plasma replacements, preoperative and postoperative hemogram, and biochemical tests were analyzed to determine the major predictors of 30-day mortality. Results: The 30-day mortality rate was 14.2%. Multivariate analysis indicated increased age (>80 years of age) (1.095 odds ratio, p = 0.029), American Society of Anesthesiologists score (3.584 odds ratio, p = 0.007), and postoperative creatinine level (2.845 odds ratio, p = 0.001) as the major predictors of 30-day mortality after hemiarthroplasty for femoral neck fractures in geriatric patients. Conclusion: Older age (>80 years of age), higher American Society of Anesthesiologists scores (ASA score 3 or 4) and increased postoperative creatinine levels were associated with an increased risk of 30-day mortality.

Objective: In our study, we aimed to investigate some laboratory parameters in patients diagnosed with alcohol / substance use disorder (ASUD) by comparing with a healthy control group. Material and Methods: Patients diagnosed with opioid, methamphetamine, alcohol, and multiple substance use disorders according to DSM-5 criteria who were hospitalized in Gaziantep State Hospital Alcohol and Substance Addiction Treatment Center (AMATEM) service on December 25 were included in this study. Study group consisted of 612 patients while the control group consisted of 117 healthy subjects. Laboratory parameters of the participants were analysed retrospectively from the hospital data. Results: Among the participants included in our study, 23 patients were female, while 706 were male. Mean age was 40.91 ± 29.58 years. Blood electrolyte levels of sodium were higher in patients with opioid, methamphetamine, and multiple substance use disorders compared to healthy controls and patients with alcohol use disorder (P <0.001). In patients with opioid, methamphetamine, and multiple substance use disorder, potassium levels were higher than healthy controls and patients with alcohol use disorder (P <0.001). In patients with alcohol use disorder, liver enzymes (aspartate aminotransferase (AST), alanine aminotransferase (ALT), gamma glutamyl transferase (GGT)) were found to be higher than other groups, while urea value was found to be low. Conclusion: Our results showed that liver enzymes, kidney functions, electrolyte levels, and blood lipid profiles of patients with alcohol / substance use disorder were affected. Based on the results we obtained, it should be taken into consideration in treatment schedules that patients with alcohol / substance use disorder may have single or multiple organ damage and/or structural and functional effects. Our aim was to validate a mass spectrometric creatinine method and compare this method with Jaffe and enzymatic serum creatinine methods. 90 samples were included. The levels were classified into three groups according to serum creatinine results as Group 1: Lower (n=30) (0.16–0.59 mg/dL), Group 2: Normal (n=30) (0.62–1.18 mg/dL) and Group 3: Higher (n=30) (1.33–3.88 mg/dL). Jaffe and enzymatic creatinine measurements were performed on the Beckman Coulter AU5800 autoanalyzer. Serum creatinine was linear from 0.039 up to 10 mg/dL, CV and bias values were ranged between 1.9–3.8% and 2–15%. Correlation coefficients were 0.990 (95% confidence interval 0.984–0.993), 0.992 (95% confidence interval 0.988–0.995) and 0.994 (95% confidence interval 0.991–0.996) for LC-MS/MS-Enzymatic, LC-MS/MS-Jaffe and Enzymatic-Jaffe, respectively. Although, Jaffe method for serum creatinine measurement is still much more practical and cheap, so in use for routine practice, tandem mass spectrometric detection of serum creatinine can be used as an accurate and specific method for verification of discordant clinical results, existence of possible interferences and serum levels under 0.5 mg/dL creatinine results such as pediatric or pregnant populations. Background/Aim. Contrast-induced acute kidney injury (CI-AKI) is a common complication after the percutaneous coronary intervention, associated with a prolonged hospital stay, increased medical costs, and risk of adverse clinical outcomes. The aim of this study was to compare changes in levels of serum creatinine (sCr) and cystatin C (sCyC) 24 h after coronary angiography as an early indicator of CI-AKI. Methods. The study included 45 patients with chronic renal failure grade I-III scheduled for coronary angiography. Levels of sCr and sCyC were measured a day before and 24 h after coronary angiography. CI-AKI was defined as a 25% and 10% increase of sCr and sCyC levels from baseline within 24 h from contrast media exposure, in the absence of alternative causes. Results. Mean sCr and sCyC concentrations were 86.4 ± 22.6 µmol/L and 1.18 ± 0.52 mg/dL, respectively before contrast administration, and 90.6 ± 24.1 µmol/L and 1.24 ± 0.65 mg/dL, respectively 24 h after contrast media exposure. sCr-based CI-AKI occurred in 4 patients (8.89%) and sCyC-based CI-AKI was detected in 19 patients (42.22%) after the contrast procedure (p < 0.001). Objective: Acute Kidney Injury (AKI) and subsequent renal failure are the leading causes of morbidity and mortality in the intensive care unit (ICU). In this study, it was planned to compare Neutrophil Gelatinase-associated Lipocalin (NGAL) and creatinine values in patients diagnosed with AKI and to determine the effect of renal dose dopamine use on renal blood flow, development of chronic renal failure (CRF) and mortality. Methods: This prospective study was planned with 35 patients developed AKI in the ICU of Bakırköy Dr. Sadi Konuk Training and Research Hospital. The patients were randomized into 2 groups as 18 patients who received dopamine treatment with the recommendation of the cardiology clinic and 17 patients who did not receive dopamine treatment. Urea, creatinine and NGAL plasma levels were compared between groups. Results: There was no difference between the groups in terms of age, gender and AKI stage. The 0th, 24th hour results and 24-hour changes of urea, creatinine and NGAL values of dopamine patient, who took dopamine, were found to be similar to those of patients who did not take dopamine. A significant positive correlation was found between the 24-hour change in creatinine value and the 24-hour change in NGAL (r=0.374; p<0.05). There was no significant change in the diameter and flow of renal arteries between measurements in patients who received dopamine. The rates of patients who regain normal kidney functions, develop CRF or develop mortality between the two groups were found to be similar. Conclusion: Treatment results of AKI developing in ICU are not satisfactory. Low-dose dopamine treatment has no effect on patient outcomes in these patients. NGAL is a biomarker that has the ability to show renal damage at an early stage. Serial measurement of NGAL concentration during ICU stay may benefit the clinician in early diagnosis and follow-up of AKI. [ABSTRACT FROM AUTHOR]

Purpose: Serum cystatin C level is a specific marker to estimate the glomerular filtration rate (GFR). Due to this specificity, we assume that GFR estimations based on cystatin C have higher diagnostic performances than GFR estimations using other biomarkers. In this study, we aimed to compared the diagnostic performances of CKDEPI cystatin C equation, CKD-EPI creatinine equation and 24-hour creatinine clearance to estimate GFR. Materials and Methods: A total of 130 participants who consisting of 101 acute and chronic renal disease patients and 29 healthy volunteers, who applied to the Nephrology Clinic, have been included in our study. Their urine creatinine levels and serum creatinine levels were measured by the colorimetric-Jaffe method, and cystatin C was by the nephelometric method. Results: There was a statistically significant relationship between CKD-EPI cystatin C, between CKD-EPI creatinine and standard creatinine clearance. The Area Under the Receiver Operating Characteristics curve (AUROC) was found to be 1, 0.995, and 0.954 respectively. Conclusion: According to the results, we think that serum cystatin C levels will present earlier findings in GFR decrease and may be a more effective marker than serum creatinine and standard creatinine clearance in the early period. Objective: Patients with chronic kidney disease (CKD) often show an increased serum free light chains concentration (FLC) accompanied by changes in the representation of both forms, i.e., kappa and lambda chains, and consequently by changing their ratio (FLC ratio). The aim of this study was to verify the reference limits of this ratio in patients with chronic renal failure, determined by commercial IVD of Binding Site. Design: Methodological study. Settings: Institute of Laboratory Diagnostics, Department of Clinical Biochemistry, University Hospital Ostrava. Material and methods: The study included 150 patients with CKD and 17 controls. The concentration of creatinine, FLC kappa, FLC lambda was determined, the estimate of glomerular filtration according to CKD-EPI and serum FLC ratio were calculated according to the KDIGO classification, divided into groups G2 to G5. To exclude the presence of monoclonal FLC, serum protein electrophoresis, immunofixation electrophoresis and isoelectric focusing in agarose gel followed by affinity immunoblotting with antibodies against free kappa and lambda light chains (IEF / AIB) were used. Excel, MedCalc and Project R software were used for statistical data processing. Results: Statistically significant differences were observed between concentrations of FLC kappa, FLC lambda and FLC ratio in individual groups (P < 0.0001). These parameters also statistically significantly correlated with creatinine concentration and with the estimate of glomerular filtration according to CKD EPI (P < 0.0001). In accordance with CLSI guideline EP28-A3C, the following reference limits were established for the FLC ratio in patients with CKD: a lower limit of 1.01 and an upper limit of 3.65. Conclusion: Practical application of reference limits for the FLC ratio may increase the specificity of the FLC test for the detection of monoclonal FLC production in patients with CKD. The aim of the study is to evaluate some hematological parameters in healthy Gurcu bucks and female Gurcu goats. The number of studies in this area is very low. In the study, healthy, 2-4 years old, 15 male and 15 female animals were used. From the animals in the study, 10 mL blood samples were taken with the help of holder from vena jugularis and transferred to K3EDTA and gel tubes. Complete blood count was measured in a complete blood count device within 30 minutes. Total leukocyte (WBC) was determined as 11.95 x103/µL in females and 8.18 x103/µL in males. Hemoglobin was measured as an average of 7.32 g/dL in females and 8.75 (g/dL) in males. The total protein concentration was determined as an average of 7.40 g/dL in females, 7.26 g/dL in males and a total average of 7.33 g/dL. In addition, the total average concentrations of creatinine, urea nitrogen, albumin, calcium, phosphorus, magnesium and glucose, respectively; 0.66 mg/dL, 25.92 mg/dL, 2.56 g/dL, 8.62 mg/dL, 4.37 mg/dL, 2.77 mEq/L and 47.04 mg/dL. As a result, some blood parameters of healthy Gurcu bucks and does that are traditionally pasture feeding were determined and reference values were obtained with this study. [ABSTRACT FROM AUTHOR]

Purpose: Delaying the cord clamping for at least 30 seconds in infants who did not require cardiopulmonary resuscitation after birth has been recommended in the revised guidelines of American Academy of Pediatrics in 2015. We aimed to evaluate the effect of delayed cord clamping on hematological and biochemical parameters, need for hospitalization, oxygen, mechanical ventilation, phototherapy, and on sepsis, hyperbilirubinemia rate in late preterm and term infants. Material and Methods: This prospective study included 86 infants with a gestational age of 34 to 41 weeks. Infants were randomly included in one of two groups. While cords of 43 infants were clamped immediately after birth, cords of the other 43 infants were clamped after one minute. Hematological and biochemical values were evaluated both in cord blood and venous blood samples on 7th day of life. Results: Hemoglobin and hematocrit values were similar in two groups both in cord blood and on 7th day of life. Delayed cord clamping resulted in statistically significant decrease in the leukocyte and platelet coun and creatinine levels in the cord blood. Mechanical ventilation requirement was significantly lower in the delayed cord clamping group. Hospitalization rate, sepsis rate and hyperbilirubinemia rate were similar in both groups. Conclusion: Delayed cord clamping caused lower leucocyte, platelet counts and lower creatinine levels in cord blood. Aim: It has been proposed that elevated levels of uric acid (UA) has a protective effect against the development of Idiopathic Parkinson's Disease (IPD), however, since IPD has been found to be more common in gout patients, their relationship became controversial. Our aim was to investigate the relationship of UA level and UA-to-creatinine ratio (UCR) with IPD stages and their effects on disease severity. Methods: Patients were grouped as early stage (ES) (Hoehn-Yahr (HY); 1-2), middle stage (MS) (HY; 2.5-3) and advanced stage (AS) (HY; 4-5). UA levels and UCR were compared between the groups. After age and L-dopa dose were corrected, differences between the groups were recalculated. Results: There were 39 ES, 33 MS, 28 AS patients. The mean UA level was 4.9±1.26 in ES, 5.02±1.22 in MS and 5.1±1.64 in AS (p=0.85). The mean UCR was 5.91±1.78 in ES, 6.03±1.66 in MS and 5.72±1.73 in AS (p=0.79). When corrected age was 67.42, there was no difference in UA (p=0.93), creatinine (p=0.92) and UCR (p=0.91). When corrected L-Dopa dose was 561.18, there was no difference in UA (p=0.99), creatinine (p=0.51) and UCR (p=0.21). When age and L-Dopa were corrected, there was no difference in UA (p=0.96), creatinine (p=0.82) and UCR (p=0.21). Conclusion: Despite corrected age and L-Dopa dose, UA and UCR did not differ between the stages. UA and UCR used as biomarkers of disease severity may not always yield accurate results. Background: To investigate the effect of changes in laboratory light intensity on chemistry and whole blood analysis. Materials and Methods: The light intensity of the laboratory environment was measured and chemical and whole blood analysis was performed on 20 patient blood samples. The light intensity was then increased using projectors and re-measured, and the chemical and whole blood analyses were repeated. The values of the tests pre- and post-light increase were compared by statistical analysis using the Wilcoxon test. Results: Increasing light from 195 to 1,168 lux significantly altered the results of the lipase, alkaline phosphatase, creatinine, and iron chemistry tests, (p<0.001 [11.3%], p=0.003 [2.2%], p=0.001 [2%] and p=<0.001 [1.2%], respectively). There was also a significant difference in platelet count (p=<0.001 [188%]). Conclusions: We show that the platelet count is sensitive to changes in laboratory light intensity at clinically unacceptable levels. The lipase, alkaline phosphatase, creatinine and iron tests are also sensitive to changes in laboratory light intensity, but at clinically acceptable levels. [ABSTRACT FROM AUTHOR]

Objective: Rosacea, a chronic inflammatory skin disease, is known to be associated with various systemic disorders. However, limited data are available on the association between rosacea and renal diseases. The objective of this study was to evaluate serum creatinine, cystatin C and neutrophil gelatinase-associated lipocalin (NGAL) levels in patients with rosacea without any systemic diseases. Material and Methods: A prospective, case-control study was planned to investigate the relationship between rosacea and renal involvement by evaluating serum creatinine, cystatin C and NGAL levels. Results: A total of 38 patients with rosacea and 45 healthy controls were included. Statistically significant differences in serum creatinine and cystatin C levels were noted between the patient and control groups (respectively p=0.012, p<0.001). No statistically significant difference was shown in the values of NGAL between the two groups (p=0.235). A positive correlation was noted between serum creatinine and serum cystatin C levels (r=0.434; p<0.001). The serum levels of cystatin C were positively correlated with disease duration (r=0.511, p<0.001). Conclusion: Serum creatinine and cystatin C levels may provide early detection of renal dysfunction in rosacea. [ABSTRACT FROM AUTHOR]
