Background & Aims The shortage of liver donations demands the use of suboptimal grafts with steatosis being a frequent finding. Although ≤30% macrovesicular steatosis is considered to be safe the risk for primary non-function ( PNF) and outcome after re-transplantation (re- OLT) is unknown. Methods Among 1205 orthotopic liver transplantations performed at our institution the frequency, survival and reason of re- OLT were evaluated. PNF (group A) cases and those with initial transplant function but subsequent need for re- OLT (group B) were analysed. Histopathology and clinical judgement determined the cause of PNF and included an assessment of hepatic steatosis. Additionally, survival of fatty liver allografts (group C) not requiring re- OLT was considered in Kaplan-Meier and multivariate regression analysis. Results A total of 77 high urgency re- OLTs were identified and included 39 PNF cases. Nearly 70% of PNF cases were due to primary fatty liver allografts. The 3-month in-hospital mortality for PNF cases after re- OLT was 46% and the mean survival after re- OLT was 0.5 years as compared to 5.2 and 5.1 years for group B, C, respectively, ( P<.008). In multivariate Cox regression analysis only hepatic steatosis was associated with an inferior survival ( HR 4.272, P=.002). The MELD score, donor BMI, age, cold ischaemic time, ICU stay, serum sodium and transaminases did not influence overall survival. Conclusions Our study highlights fatty liver allografts to be a major cause for PNF with excessive mortality after re-transplantation. The findings demand the development of new methods to predict risk for PNF of fatty liver allografts. [ABSTRACT FROM AUTHOR]

Background Long-term results for patients being managed for ruptured compared to elective abdominal aortic aneurysms ( AAA) are unclear. We hypothesize that patients who survive 30 days or more following repair of ruptured AAA ( RAAA) performed by open technique have a life expectancy no different to those patients surviving 30 days or more following elective AAA repair, or compared to a general age-matched population. Methods Between 1987 and December 2014, 620 consecutive patients were treated by the principal author for aortic aneurysmal disease. Two subgroups were selected from this population, elective open abdominal repair (215) and RAAA open repair (105). Comparable age-matched life curves with the general population were used from the Australian Bureau of Statistics for each patient according to gender, age and date of presentation. Statistical comparison was by Kaplan-Meier survival analysis. Results Both the open and RAAA groups were well matched for age and sex. There was no statistical difference between RAAA survival and an age-matched population P = 0.23, or was there any difference between open repair and an age-matched population, P = 0.1. Survival curves for RAAA and open repair were similar, P = 0.98. For elective open repair 1-, 5-, 10-, 15- and 20-year survival was 93.6, 71.2, 40, 17 and 2% respectively. Corresponding results for RAAA were 92.5, 74, 36.7, 13.5 and 5% respectively. Conclusion Open AAA repair for RAAA or elective aneurysm treatment restores predicted life expectancy for those patients surviving 30 days or more and is therefore a durable method of treatment for this condition. Background Long-term data on clinical outcomes of restorations attached to implants via zinc oxide ( ZnO) cement have been sparse. Purpose The purpose of this study was to retrospectively investigate decementation rates and peri-implant tissue status of implant-supported fixed restorations retained by ZnO cement. Materials and Methods Between 1989 and 2003, 63 partially edentulous patients received 93 implants. Of these, 27 patients received 36 screwed single crowns ( SC) and served as a control ( C group). The other 36 patients had their restorations cemented using ZnO cement. They were subdivided into a SC group and a fixed dental prosthesis ( FDP) group. After between 10 and 23 years (mean: 13.22 ± 3.21), all decementation events and peri-implant soft tissue status were evaluated using patient records. Results Decementation was assessed in 57 implants supporting 30 SCs and 16 FDPs. Five cases of decementation (8.77%) were recorded after a mean of 9.27 ± 7.05 years (range: 4.81-21.79). In the control group of vertically screwed SCs, five events of screw loosening (13.89%) were recorded in five patients (18.52%) after a mean of 5.84 ± 5.96 years (range: 0.56-15.05) within a 14.30 year observation period. No cases of peri-implantitis were observed in any group. The mean values of periodontal probing depths and bleeding on probing (BOP+) were 3.74 mm and 31.58%, respectively, for ZnO-cemented restorations, versus 3.76 mm and 25%, respectively, for the C group. No significant correlations regarding technical/biologic complications between the groups were detected. Conclusions Within the limitations of this study, we conclude that the use of ZnO cement provides sufficient retention of implant-supported fixed restorations over long periods without biologic complications in form of peri-implantitis. Background Calcium phosphate ceramic coatings have the potential to compensate for challenging bone conditions such as delayed or impaired bone healing and low bone quantity or density. Thus, the increasing universal prevalence of subjects with such challenging bone conditions might be paralleled by an enhanced global use of calcium phosphate ceramic-coated dental implants. However, it is speculated that the long-term clinical survival of calcium phosphate-coated dental implants might be adversely affected by coating delamination. Objective The aims of the current review were (1) to systematically appraise and (2) to meta-analyse long-term survival data of calcium phosphate-coated dental implants in clinical trials. Materials and methods An extensive search in the electronic databases of the National Library of Medicine (http://www.ncbi.nlm.nih.gov), The Cochrane Central Register of Controlled Trials and the ISI Web of Knowledge, was carried out for articles published between January 2000 and November 2011 to identify randomized controlled clinical trials, prospective clinical trials as well as retrospective analysis of cases ( RA) presenting survival data on the topic of calcium phosphate-coated dental implants. Only publications in English were considered, and the search was narrowed to studies in humans with a follow-up of at least 5 years only. Furthermore, the reference lists of related review articles and publications selected for inclusion in this review were systematically screened. The primary outcome variable was percentage annual failure rate ( AFR), and the secondary outcome variable was percentage cumulative survival rate ( CSR). Results The electronic search in the database of the National Library of Medicine, The Cochrane Central Register of Controlled Trials and the ISI Web of Knowledge, resulted in the identification of 385 titles. These titles were initially screened by the two independent reviewers for possible inclusion, resulting in 29 publications suitable for further consideration. Screening the abstracts led to 20 full-text articles. From these articles, 15 reports were excluded. Finally, five of these original research reports could be selected for evaluation. No additional publications were identified by manual search. Thus, a total of five articles were included for analysis. Meta-analysis revealed that neither AFRs of calcium phosphate-coated dental implants increased progressively nor that long-term CSRs for calcium phosphate-coated dental implants were inferior to survival rates of noncoated implants. Conclusion We conclude that (1) published long-term survival data for calcium phosphate-coated dental implants are very limited, (2) AFRs of calcium phosphate-coated dental implants do not increase progressively, and (3) long-term CSRs for calcium phosphate-coated dental implants are comparable to survival rates of noncoated implants. Abstract: Introduction:: Solitary adrenal metastases occur in about 1.2–10% of renal cell cancer patients. Since the vast majority of intraadrenal lesions can be detected preoperatively, we and others have recently recommended to renounce a routine adrenalectomy during surgery of renal cell cancer. However, the impact of adrenalectomy on the patients’ clinical prognosis in case of a solitary metastatic lesion within the adrenal gland remains an issue of controversial discussion. Whereas some authors suggest adrenalectomy as a potentially curative treatment option in these cases, others compare its clinical value with that of a mere lymphadenectomy. Patients and method:: Between 1981 and 2000, 648 patients (440 males and 208 females) underwent nephrectomy in combination with adrenalectomy in our clinic for the diagnosis of renal cell cancer. The median age at first diagnosis was 59 (range 33–84) and 60 (range 20–85) years for male and female patients, respectively. The median postoperative follow - up was 2.4 years (0.2–18 years). According to the TNM – classification system (2003) tumor stages were classified as follows: T1, 228 pat. (37%); T2, 70 pat. (11%); T3, 287 pat. (46%); T4, 37 pat. (6%). In total, 339 patients revealed regional lymph node or distant metastases at the time of the surgical treatment. Although metastases of the adrenal gland were diagnosed in 48 patients, solitary intraadrenal metastases without further systemic spread were observed in only 13 cases. Several patients’ and tumor characteristics (age, tumor stage and size, the presence of regional lymph node metastases, the presence of metastatic lesions at different organ sites as well as the detection of solitary intraadrenal metastases) were correlated with the patients’ overall survival by univariate and multivariate statistical analysis (logistic Cox regression analysis). Results:: The median long – term survival was 4.8 years for the entire cohort of patients investigated. The median long – term survival was 13.8 years and 11.7 years for patients with no evidence of metastatic spread as well as for patients with a solitary intraadrenal metastatic lesion, respectively. Accordingly, the long – term survival rates at 5 and 10 years after surgery were 66%/50% and 51%/51% for patients with no evidence of metastatic spread or isolated intraadrenal metastases. This difference was not statistically significant. In contrast, for patients revealing lymph node or distant metastases at other organ sites, the median long – term survival was significantly decreased (lymph node metastases: 0.7 years; distant metastases: 1.2 years). Discussion:: For patients with a solitary intraadrenal metastatic lesion, adrenalectomy is a potentially curative treatment option. The observation that the long – term survival of the latter patients is comparable to that of patients with organ – confined disease might suggest the establishment of a separate TNM – category for patients revealing a solitary metastasis within the adrenal gland and no hint at further systemic metastatic spread.
