Neuromuscular diseases are a prevalent cause of prolonged and severe suffering for patients, and with the global population aging, it is increasingly becoming a pressing concern. To assess muscle activity in NMDs, clinicians and researchers typically use electromyography (EMG), which can be either non-invasive using surface EMG, or invasive through needle EMG. Surface EMG signals have a low spatial resolution, and while the needle EMG provides a higher resolution, it can be painful for the patients, with an additional risk of infection. The pain associated with the needle EMG can pose a risk for certain patient groups, such as children. For example, children with spinal muscular atrophy (type of NMD) require regular monitoring of treatment efficacy through needle EMG; however, due to the pain caused by the procedure, clinicians often rely on a clinical assessment rather than needle EMG. Magnetomyography (MMG), the magnetic counterpart of the EMG, measures muscle activity non-invasively using magnetic signals. With super-resolution capabilities, MMG has the potential to improve spatial resolution and, in the meantime, address the limitations of EMG. This article discusses the challenges in developing magnetic sensors for MMG, including sensor design and technology advancements that allow for more specific recordings, targeting of individual motor units, and reduction of magnetic noise. In addition, we cover the motor unit behavior and activation pattern, an overview of magnetic sensing technologies, and evaluations of wearable, non-invasive magnetic sensors for MMG.

Muscles are the actuators of all human actions, from daily work and life to communication and expression of emotions. Myography records the signals from muscle activities as an interface between machine hardware and human wetware, granting direct and natural control of our electronic peripherals. Regardless of the significant progression as of late, the conventional myographic sensors are still incapable of achieving the desired highresolution and non-invasive recording. This paper presents a critical review of state-of-the-art wearable sensing technologies that measure deeper muscle activity with high spatial resolution, so-called super-resolution. This paper classifies these myographic sensors according to the different signal types (i.e., biomechanical, biochemical, and bioelectrical) they record during measuring muscle activity. By describing the characteristics and current developments with advantages and limitations of each myographic sensor, their capabilities are investigated as a super-resolution myography technique, including: (i) non-invasive and high-density designs of the sensing units and their vulnerability to interferences, (ii) limit-of-detection to register the activity of deep muscles. Finally, this paper concludes with new opportunities in this fast-growing super-resolution myography field and proposes promising future research directions. These advances will enable next-generation muscle-machine interfaces to meet the practical design needs in reallife for healthcare technologies, assistive/rehabilitation robotics, and human augmentation with extended reality. Objective: To record and characterize features of levator ani muscles (LAM) activity in pregnancy and postpartum using non-invasive and novel Magnetomyography (MMG) technique with amplitude and spectral parameters. Methods: Nulliparous women with uncomplicated pregnancies participated in the MMG data collection during rest and voluntary LAM contractions (Kegels) with modulated intensity in third trimester and approximately 2 months postpartum (PP). Simultaneous surface electromyography was recorded to document the recruitment of accessory muscles. Moderate strength Kegel (MK) MMG trials were analyzed. Amplitude and spectral parameters including root-mean square (RMS) amplitude, power spectrum density (PSD) and normalized PSD (rPSD) in three frequency bands (low, middle, high) were computed on MK epochs. Statistical comparisons between pregnancy and postpartum were calculated. Results: MMG recordings were measured from 10 pregnant women. Results showed decreased RMS and power between third trimester and postpartum, trending towards significance. rPSD values in the low frequency band decreased significantly (p = 0.028) from third trimester to postpartum, while significant increase was observed in the middle frequency band (p = 0.018). Conclusions: This study shows that MMG as non-invasive tool has the ability to detect and characterize changes of LAM activity with amplitude and spectral parameters during pregnancy and postpartum.

Objective: The purpose of this study was to determine if uterine electrophysiological signals gathered from 151 non-invasive biomagnetic sensors spread over the abdomen were associated with successful induction of labor (IOL). Study design: Uterine magnetomyogram (MMG) signals were collected using the SARA (SQUID Array for Reproductive Assessment) device from 33 subjects between 37 and 42 weeks gestational age. The signals were post-processed, uterine contractile related MMG bursts were detected, and parameters in the time and frequency domain were extracted. The modified Bishop score calculated at admission was used to determine the method of IOL. Wilcoxon's rank-sum test was used to compare IOL successes and failures for differences in gestational age (GA), parity, modified Bishop's score, maximum oxytocin, and electrophysiological parameters extracted from MMG. Results: The average parity was three times (3x) higher (1.53 versus 0.50; p = 0.039), and the average modified Bishop score was 2x higher (3.32 versus 1.63; p = 0.032) amongst IOL successes than failures, while the average GA and maximum oxytocin showed a small difference. For the MMG parameters, successful IOLs had, on average, 3.5x greater mean power during bursts (0.246 versus 0.070; p = 0.034) and approximately 1.2x greater mean number of bursts (2.05 versus 1.68; p = 0.036) compared to the failed IOLs, but non-significant differences were observed in mean peak frequency, mean burst duration, and mean duration between bursts. Conclusion: The study showed that inductions of labor that took less than 24 h to deliver have a higher mean power in the baseline electrophysiological activity of the uterus when recorded prior to planned induction. The results are indicative that baseline electrophysiological activity measured prior to induction is associated with successful induction.

While electromyography (EMG) and magnetomyography (MMG) are both methods to measure the electrical activity of skeletal muscles, no systematic comparison between both signals exists. Within this work, we propose a novel in silico model for EMG and MMG and test the hypothesis that MMG surpasses EMG in terms of spatial selectivity, i.e. the ability to distinguish spatially shifted sources. The results show that MMG provides a slightly better spatial selectivity than EMG when recorded directly on the muscle surface. However, there is a remarkable difference in spatial selectivity for non-invasive surface measurements. The spatial selectivity of the MMG components aligned with the muscle fibres and normal to the body surface outperforms the spatial selectivity of surface EMG. Particularly, for the MMG's normal-to-the-surface component the influence of subcutaneous fat is minimal. Further, for the first time, we analyse the contribution of different structural components, i.e. muscle fibres from different motor units and the extracellular space, to the measurable biomagnetic field. Notably, the simulations show that for the normal-to-the-surface MMG component, the contribution from volume currents in the extracellular space and in surrounding inactive tissues, is negligible. However, recent advancements in non-cryogenic quantum sensors, such as optically pumped magnetometers (OPMs), enable interesting possibilities to flexibly record biomagnetic signals. Yet, a magnetomyographic investigation of muscular fatigue is still missing. Here, we simultaneously used sEMG (4 surface electrode) and OPM-based magnetomyography (OPM-MMG, 4 sensors) to detect muscle fatigue during a 3 × 1-min isometric contractions of the left rectus femoris muscle in 7 healthy participants. Both signals exhibited the characteristic spectral compression distinctive for muscle fatigue. OPM-MMG and sEMG slope values, used to quantify the spectral compression of the signals, were positively correlated, displaying similarity between the techniques. Additionally, the analysis of the different components of the magnetic field vector enabled speculations regarding the propagation of the muscle action potentials (MAPs). Magnetomyography (MMG) with superconducting quantum interference devices (SQUIDs) enabled the measurement of very weak magnetic fields (femto to pico Tesla) generated from the human skeletal muscles during contraction. However, SQUIDs are bulky, costly, and require working in a temperature-controlled environment, limiting wide-spread clinical use. We introduce a low-profile magnetoelectric (ME) sensor with analog frontend circuitry that has sensitivity to measure pico-Tesla MMG signals at room temperature. It comprises magnetostrictive and piezoelectric materials, FeCoSiB/AlN. Accurate device modelling and simulation are presented to predict device fabrication process comprehensively using the finite element method (FEM) in COMSOL Multiphysics. The fabricated ME chip with its readout circuit was characterized under a dynamic geomagnetic field cancellation technique. The ME sensor experiment validate a very linear response with high sensitivities of up to 378 V/T driven at a resonance frequency of fres = 7.76 kHz. Measurements show the sensor limit of detections of down to 175 pT/√Hz at resonance, which is in the range of MMG signals. Such a small-scale sensor has the potential to monitor chronic movement disorders and improve the end-user acceptance of human–machine interfaces. [ABSTRACT FROM AUTHOR]

Magnetism‐based systems are widely utilized for sensing and imaging biological phenomena, for example, the activity of the brain and the heart. Magnetomyography (MMG) is the study of muscle function through the inquiry of the magnetic signal that a muscle generates when contracted. Within the last few decades, extensive effort has been invested to identify, characterize and quantify the magnetomyogram signals. However, it is still far from a miniaturized, sensitive, inexpensive and low‐power MMG sensor. Herein, the state‐of‐the‐art magnetic sensing technologies that have the potential to realize a low‐profile implantable MMG sensor are described. The technical challenges associated with the detection of the MMG signals, including the magnetic field of the Earth and movement artifacts are also discussed. Then, the development of efficient magnetic technologies, which enable sensing pico‐Tesla signals, is advocated to revitalize the MMG technique. To conclude, spintronic‐based magnetoresistive sensing can be an appropriate technology for miniaturized wearable and implantable MMG systems. [ABSTRACT FROM AUTHOR]

Aims: To characterize levator ani muscle (LAM) activity in nulligravidas using magnetomyography (MMG) and define MMG characteristics associated with LAM activity with and without accessory muscle contributions. Methods: MMG data were collected from eight nulligravidas during rest and voluntary LAM contractions (Kegels) of varying intensity. We utilized simultaneous vaginal manometry and surface electromyography (sEMG) to evaluate for accessory muscle recruitment. Moderate Kegel (MK) MMG trials were sub‐selected based on the presence or absence of accessory muscle interaction. Amplitude and spectral‐related indicators were calculated across MK epochs: root‐mean square (RMS) amplitude, percentage amplitude relative to rest, and relative power spectrum density (rPSD) in three frequency bands (low, middle, high). Ternary diagram characterized rPSD from selected Kegels and ROC analysis was performed to identify cut‐points to differentiate MKs from interacting MKs. Results: Nineteen MMG recordings were obtained. Amplitude and spectral parameters were significantly different between isolated and interacting MK epochs. Mean RMS and power values of the isolated MK were, respectively, 120.66 ± 43.8 fT and 1.72 ± 1.44 (T2/Hz)*10−28. Amplitudes of MK were 64% and 117 higher than baseline activities for the isolated and interacting epochs, respectively. ROC curves reveled cut‐off points on low and middle frequency bands that achieved perfect separation (ROC‐AUC = 1.0) between isolated and interacting MK. Conclusions: Our study demonstrates that MMG, a novel biomagnetic technique, allows precise detection and characterization of normal female pelvic floor function. Results show that isolated moderate voluntary contraction of the LAMs produces distinct MMG amplitude and spectral characteristics compared with Kegels involving co‐activation of other muscle groups. The article considers the potential use of mechano or vibro or acceleromyography (MMG) in skeletal muscle physiology studies. Topics discussed include the clinical and research applications of MMG, the use of magnetic fields from skeletal muscles as a valuable physiological measurement tool, and the methodological and cost constraints that restrict a larger use of MMG.

The objective of the study was to perform serial magnetomyographic examinations in order to detect changes in synchrony characteristics of myometrial electrophysiological activity as women approach labor. Of the total of 56 patients recruited, the results of 149 examinations from 29 patients were included in the analysis. The global synchrony across each sensor spread over the abdomen was computed and quantified as synchronization index. The mean and the median value of the global synchrony were computed and correlated with time to active labor from the last recording. Overall, synchrony increased as the patient approached active labor (P = .035). Furthermore, mean synchronization index increased twice as fast in the nonnulliparous group compared to the nulliparous group (P = .039). The changes in synchrony of uterine electrophysiological activity near term could aid in prediction of labor. [ABSTRACT FROM AUTHOR]

Objective: The purpose of this study was to characterize the noninvasive magnetic field recordings of the uterine electrophysiological activity in patients reporting onset of uterine contractions. Study design: Transabdominal magnetomyographic (MMG) recordings were performed with the use of the SARA system's 151 primary magnetic sensors (CTF Systems Inc, Coquitlam, British Columbia, Canada). Eleven term and 4 preterm patients participated in the study. On all patients, cervical dilation and outcomes were recorded. Results: Of 8 patients having a peak MMG activity exceeding 8 pT (pico Tesla), all but 1 delivered within 48 hours of our recordings. Of the 7 patients with peak activity below 8 pT, 5 failed to deliver within 48 hours of the recordings. This observation reflects an increase in the electrophysiologic activity of the myometrium as labor progresses. Conclusion: MMG evaluation provides a new noninvasive method for the prediction of labor. Further studies are in progress to determine the temporal relationship of MMG changes with the onset of labor. [ABSTRACT FROM AUTHOR]

Aim: This study aims to simultaneously record the magnetic and electric components of the propagating muscular action potential.Method: A single-subject study of the monosynaptic stretch reflex of the musculus rectus femoris was performed; the magnetic field generated by the muscular activity was recorded in all three spatial directions by five optically pumped magnetometers. In addition, the electric field was recorded by four invasive fine-wire needle electrodes. The magnetic and electric fields were compared by modelling the muscular anatomy of the rectus femoris muscle and by simulating the corresponding magnetic field vectors.Results: The magnetomyography (MMG) signal can reliably be recorded following the stimulation of the monosynaptic stretch reflex. The MMG signal shows several phases of activity inside the muscle, the first of which is the propagating muscular action potential. As predicted by the finite wire model, the magnetic field vectors of the propagating muscular action potential are generated by the current flowing along the muscle fiber. Based on the magnetic field vectors, it was possible to reconstruct the pinnation angle of the muscle fibers. The later magnetic field components are linked to the activation of the contractile apparatus. Interpretation MMG allows to analyze the muscle physiology from the propagating muscular action potential to the initiation of the contractile apparatus. At the same time, this methods reveals information about muscle fiber direction and extend. With the development of high-resolution magnetic cameras, that are based on OPM technology, it will be possible to image the function and structure of the biomagnetic field of any skeletal muscle with high precision. Muscular activity recording is of high basic science and clinical relevance and is typically achieved using electromyography (EMG). While providing detailed information about the state of a specific muscle, this technique has limitations such as the need for a priori assumptions about electrode placement and difficulty with recording muscular activity patterns from extended body areas at once. For head and face muscle activity, the present work aimed to overcome these restrictions by exploiting magnetoencephalography (MEG) as a whole head myographic recorder (head magnetomyography, hMMG). This is in contrast to common MEG studies, which treat muscular activity as artifact in electromagnetic brain activity. In a first proof‐of‐concept step, participants imitated emotional facial expressions performed by a model. Exploiting source projection algorithms, we were able to reconstruct muscular activity, showing spatial activation patterns in accord with the hypothesized muscular contractions. Going one step further, participants passively observed affective pictures with negative, neutral, or positive valence. Applying multivariate pattern analysis to the reconstructed hMMG signal, we were able to decode above chance the valence category of the presented pictures. Underlining the potential of hMMG, a searchlight analysis revealed that generally neglected neck muscles exhibit information on stimulus valence. Results confirm the utility of hMMG as a whole head electromyographic recorder to quantify muscular activation patterns including muscular regions that are typically not recorded with EMG. This key advantage beyond conventional EMG has substantial scientific and clinical potential. We present an innovative method called head magnetomyography (hMMG), which exploits magnetoencephalography (MEG) as a whole head electromyographic (EMG) recorder. Differently from the typical EMG recording, which needs an a priori selection of the placement of the electrodes, hMMG is able to detect muscular activity from many regions of the face and head simultaneously, including typically overlooked muscles. Abstract: Objective: The objective was to extract, quantify and characterize the uterine magnetomyographic (MMG) signals that correspond to the electrophysiological activity of the uterus. Methods: Transabdominal MMG recordings with high spatial–temporal resolution were performed with the use of the 151 non-invasive magnetic sensor system. The extraction, quantification and characterization procedures were developed and applied to representative MMG signals that were recorded from a pregnant woman at regular intervals starting at 37 weeks of gestation until the subject reached active labor. Results: Multiple MMG recordings were successfully performed on the subject before she went into active labor. The extracted MMG burst activity showed a statistically significant correlation (r =0.2; p <0.001) with the contractile events perceived by mothers. The time–frequency analysis of the burst activity showed a power shift towards higher-frequency at 48h before the subject went into active labor as compared to earlier recordings. Further there was a gradual increase in the synchrony in the higher-frequency band as the subject reached close to active labor. Conclusions: The non-invasive recording of the magnetic signals of pregnant uterus with high spatial–temporal resolution can provide an insight into the preparatory phase of labor and has the potential of predicting term and preterm labor.
