The study investigates the effect of reflective thinking practices on retention scores using a mixed meta-method approach, which involves analyzing quantitative and qualitative data obtained through document reviews with meta-analysis and meta-thematic analysis. For the meta-analysis, nine studies that met the inclusion criteria in specific databases were identified. Using CMA and Meta Win programs, the meta-analysis revealed a significant effect size of g=1.550 across all nine studies, indicating that reflective thinking practices positively influence retention scores. The meta-thematic analysis, which included content analysis of 13, revealed the following four themes: contribution of reflective thinking practices to the learning process, benefits for students, obstacles associated with these practices, and suggested solutions to overcome them. The findings from both the meta-analysis and the meta-thematic analysis consistently support the conclusion that reflective thinking practices have a positive effect on retention. Research on factors influencing information credibility judgment is increasing, whereas their findings are mixed. This study conducted a meta-analysis of 85 empirical studies, synthesizing the effects of 12 frequently examined source, content, channel, and receiver factors on information credibility perception. Results reveal that message quality, source credibility, and message fluency have large correlations with credibility perception, while other factors show small-to-medium correlations. Personal topic knowledge does not have a significant relationship with credibility perception. Gender composition, information channel, topic, and research method significantly moderate the aggregated relationships. The theoretical and practical implications are discussed. Context: Several studies have shown that emotional regulation (ER) is a transdiagnostic construct of emotional disorders. Therefore, if therapy improves ER, it would improve psychological distress. Objective: This review assesses and compares the changes in ER due to psychological treatment in different therapies. Methods: A systematic review and meta‐analysis of RCTs published in the databases PubMed, PsycINFO and Web of Science was performed. It was registered in PROSPERO under the number CRD42023387317. Two independent experts in the field reviewed the articles. Results: A total of 18 articles met the criteria for inclusion in the review. Analysis of these studies suggests that in unified protocol (UP), cognitive behaviour therapy, dialectical behaviour therapy (DBT) and mindfulness, there is evidence to support that a moderate effect occurs during treatment. Furthermore, in mindfulness and DBT, the effect was moderate in the follow‐up period, while in UP, it was high. Limitations: Given the heterogeneity of the applied interventions and the methodological limitations found in the reviewed trials, the results should be interpreted with caution. Conclusions: UP, cognitive behaviour therapy, DBT and mindfulness can improve ER after therapy, while UP, DBT and mindfulness in the follow‐up period. Other therapies, such as SKY or Flotation REST, require more research. [ABSTRACT FROM AUTHOR]

Background: PTSD is a significant mental health problem worldwide. Current evidence-based interventions suffer various limitations. Ketamine is a novel agent that is hoped to be incrementally better than extant interventions. Objective: Several randomized control trials (RCTs) of ketamine interventions for PTSD have now been published. We sought to systematically review and meta-analyse results from these trials to evaluate preliminary evidence for ketamine’s incremental benefit above-andbeyond control interventions in PTSD treatment. Results: Omnibus findings from 52 effect sizes extracted across six studies (n = 221) yielded a small advantage for ketamine over control conditions at reducing PTSD symptoms (g = 0.27, 95% CI = 0.03, 0.51). However, bias-correction estimates attenuated this effect (adjusted g = 0.20, 95%, CI = −0.08, 0.48). Bias estimates indicated smaller studies reported larger effect sizes favouring ketamine. The only consistent timepoint assessed across RCTs was 24-hours post-initial infusion. Effects at 24-hours post-initial infusion suggest ketamine has a small relative advantage over controls (g = 0.35, 95% CI = 0.06, 0.64). Post-hoc analyses at 24-hours post-initial infusion indicated that ketamine was significantly better than passive controls (g = 0.44, 95% CI = 0.03, 0.85), but not active controls (g = 0.24, 95% CI = −0.30, 0.78). Comparisons one-week into intervention suggested no meaningful group differences (g = 0.24, 95% CI = 0.00, 0.48). No significant differences were evident for RCTs that examined effects two-weeks post initial infusion (g = 0.17, 95% CI = −0.10, 0.44). Conclusions: Altogether, ketamine-for-PTSD RCTs reveal a nominal initial therapeutic advantage relative to controls. However, bias and heterogeneity appear problematic. While rapid acting effects were observed, all control agents (including saline) also evidenced rapid acting effects. We argue blind penetration to be a serious concern, and that placebo is the likely mechanism behind reported therapeutic effects. [ABSTRACT FROM AUTHOR]

COVID-19 pandemic , *HARM (Ethics) , *COVID-19 , *MENTAL illness , *MENTAL health , *POST-traumatic stress disorder - Abstract
Background: During the COVID-19 pandemic, health-care workers (HCWs) may have been confronted with situations that may culminate in moral injury (MI). MI is the psychological distress that may result from perpetrating or witnessing actions that violate one’s moral codes. Literature suggests that MI can be associated with mental health problems. Objective: We aimed to meta-analytically review the literature to investigate whether MI is associated with symptoms of posttraumatic stress disorder (PTSD), anxiety, depression, burnout, and suicidal ideation among active HCWs during the COVID-19 pandemic. Method: We searched eight databases for studies conducted after the onset of the COVID-19 pandemic up to 18 July 2023, and performed random-effects meta-analyses to examine the relationship between MI and various mental health outcomes. Results: We retrieved 33 studies from 13 countries, representing 31,849 individuals, and pooled 79 effect sizes. We found a positive association between MI and all investigated mental health problems (rs = .30–.41, all ps < .0001). Between-studies heterogeneity was significant. A higher percentage of nurses in the samples was associated with a stronger relationship between MI and depressive and anxiety symptoms. Samples with a higher percentage of HCWs providing direct care to patients with COVID-19 exhibited a smaller effect between MI and depressive and anxiety symptoms. We observed a stronger effect between MI and PTSD symptoms in US samples compared to non-US samples. Conclusion: We found that higher MI is moderately associated with symptoms of PTSD, anxiety, depression, burnout, and suicidal ideation among HCWs during the COVID-19 pandemic. Our findings carry limitations due to the array of MI scales employed, several of which were not specifically designed for HCWs, but underscore the need to mitigate the effect of potentially morally injurious events on the mental health of HCWs. OPIOIDS , *PSYCHIATRIC drugs , *DISSOCIATIVE disorders , *PUBLICATION bias , *PSYCHOTHERAPY - Abstract
The clinical guidelines for the treatment of dissociation focus primarily on psychotherapy. However, different psychoactive drugs are used in clinical practice. The use of opioid antagonists has been proposed as a therapeutic option based on the theory that dissociation might be a phenomenon mediated by dysregulation of the endogenous opioid system. To review and meta-analyse the available evidence on the efficacy of the opioid antagonists naltrexone, naloxone, and nalmefene as treatments for dissociative symptoms and disorders. The PRISMA guidelines were followed, and this review was registered in Prospero with reference number CRD42021280976. The search was performed in the PubMed, Scopus, Web of Science, EMBASE, PsycINFO, and PubPsych databases. 1,798 citations were obtained. After removing duplicates and applying inclusion and exclusion criteria, we included 5 comparative studies with 9 dissociation measures that had included a total of 154 participants, of whom 134 had been treated with an opioid antagonist. The results of the meta-analysis showed a treatment effect for dissociation when using opioid antagonists [pooled d = 1.46 (95% CI: 0.62–2.31)]. However, the studies we included were very heterogeneous [Q = 66.89 (p <.001)] and there may have been publication bias. Although more research is needed and the results must be interpreted with caution because of the limited amount of data and heterogeneity in the studies and their methodological qualities, opioid antagonists (particularly naltrexone) are promising candidates for the treatment of dissociative symptoms and showed a moderate – large effect size in reducing these symptoms. POST-traumatic stress disorder , *BIOFEEDBACK training , *DEFAULT mode network , *ALPHA rhythm , *SALIENCE network , *RANDOMIZED controlled trials - Abstract
Background: Posttraumatic stress disorder (PTSD) is a debilitating condition affecting millions of people worldwide. Existing treatments often fail to address the complexity of its symptoms and functional impairments resulting from severe and prolonged trauma. Electroencephalographic Neurofeedback (NFB) has emerged as a promising treatment that aims to reduce the symptoms of PTSD by modulating brain activity. Objective: We conducted a systematic review and meta-analysis of ten clinical trials to answer the question: how effective is NFB in addressing PTSD and other associated symptoms across different trauma populations, and are these improvements related to neurophysiological changes? Method: The review followed the Preferred Reporting Items for Systematic Reviews and Meta analyses guidelines. We considered all published and unpublished randomised controlled trials (RCTs) and non-randomised studies of interventions (NRSIs) involving adults with PTSD as a primary diagnosis without exclusion by type of trauma, co-morbid diagnosis, locality, or sex. Ten controlled studies were included; seven RCTs and three NRSIs with a total number of participants n = 293 (128 male). Only RCTs were included in the meta-analysis (215 participants; 88 male). Results: All included studies showed an advantage of NFB over control conditions in reducing symptoms of PTSD, with indications of improvement in symptoms of anxiety and depression and related neurophysiological changes. Meta-analysis of the pooled data shows a significant reduction in PTSD symptoms post-treatment SMD of −1.76 (95% CI −2.69, −0.83), and the mean remission rate was higher in the NFB group (79.3%) compared to the control group (24.4%). However, the studies reviewed were mostly small, with heterogeneous populations and varied quality. Conclusions: The effect of NFB on the symptoms of PTSD was moderate and mechanistic evidence suggested that NFB leads to therapeutic changes in brain functioning. Future research should focus on more rigorous methodological designs, expanded sample size and longer follow-up. Neurofeedback (NFB) was found to have moderate beneficial effects on PTSD symptoms, and positive effects on secondary outcomes such as depression and anxiety, according to a meta-analysis of seven randomised controlled trials (RCTs). The beneficial effects of NFB were observed across diverse populations, including those with different types of trauma (military and civilians) and from different ethnic backgrounds. Results suggest that modulation of alpha rhythm might be a viable NFB protocol in patients with PTSD, as changes in neurophysiological functioning, such as connectivity in the Default Mode Network (DMN) and Salience Network (SN), were observed post-NFB and were correlated with a decrease in PTSD severity. POLITICAL refugees , *INTERNALLY displaced persons , *MENTAL health , *FORCED migration , *MENTAL health promotion - Abstract
Background: People forced to leave their homes, such as refugees and internally displaced persons, are exposed to various stressors during their forced displacement, putting them at risk for mental disorders. Objective: To summarize evidence on the efficacy of psychosocial interventions aiming to promote mental health and/or to prevent mental symptoms by fostering transdiagnostic skills in forcibly displaced persons of all ages. Method: Four databases and reference lists were searched for randomized controlled trials on interventions in this population on 11 March 2022. Thirty-six studies were eligible, 32 studies (comprising 5299 participants) were included in random-effects multilevel meta-analyses examining the effects of interventions on mental symptoms and positive mental health (e.g. wellbeing) as well as moderators to account for heterogeneity. OSF Preregistration-ID: 10.17605/OSF.IO/XPMU3 Results: Our search resulted in 32 eligible studies, with 10 reporting on children/adolescents and 27 on adult populations. There was no evidence for favourable intervention effects in children/adolescents, with 44.4% of the effect sizes pointing to potentially negative effects yet remaining non-significant. For adult populations, our meta-analyses showed a close-to-significant favourable effect for mental symptoms, M(SMD) = 0.33, 95% CI [–0.03, 0.69], which was significant when analyses were limited to high-quality studies and larger for clinical compared to non-clinical populations. No effects emerged for positive mental health. Heterogeneity was considerable and could not be explained by various moderators (e.g. type of control, duration, setting, theoretical basis). Certainty of evidence was very low across all outcomes limiting the generalizability of our findings. Conclusion: The present review provides at most weak evidence for an effect favouring transdiagnostic psychosocial interventions over control conditions for adult populations but not for children and adolescents. Future research should combine the imperative of humanitarian aid in face of major crises with studying the diverse needs of forcibly displaced persons to improve and tailor future interventions. This review is the first to examine the efficacy of transdiagnostic interventions for mental health promotion and prevention of mental disorders in forcibly displaced persons of all ages. Overall, we found no favourable effect of transdiagnostic interventions in both children/adolescents and adults. Excluding studies at high risk of bias, there was weak evidence for a small favourable effect in adults, but not in children and adolescents. Thus, so far, there is weak evidence for transdiagnostic interventions in forcibly displaced persons. Research efforts need to match care needs: While most people live and need care in low-income countries, the majority of research has been conducted in high-income countries. [ABSTRACT FROM AUTHOR]

SELF medication , *COLOMBIANS , *DRUGS , *MEDICAL care , *CITIES & towns , *ANTIPARASITIC agents - Abstract
Objectives: The reported frequency of self-medication in Colombia ranges from 32.3% to 84.7%, depending on the study sample and time frame. This study aimed to estimate a pooled prevalence of self-medication and its associated factors in Colombian youth and adults. Method: A comprehensive systematic literature review and meta-analysis of the studies on self-medication in Colombia published from January 2000 - June 2022 was conducted. Results: Twelve studies (n=5,668) from urban areas were included, and a pooled prevalence of self-medication of 64.2% (95% confidence interval [CI] 50.8%-77.5%) was found. However, the prevalence was lower when self-medication was assessed during the last 30 days (32.3%; 95%CI 25.4%-39.3%) than when longer time frames were used. Female university students had a higher frequency of self-medication (OR= 1.72; 95% CI 1.17-2.53) than males. The most common medications were analgesics (37.7%), anti-inflammatories (33.2%), antihistamines (14.8%), and antibiotics/antiparasitics (12.1%). Lack of time and delays in medical care were reported in 35.2% (95% CI 25.6%-44.7%) of the cases. Conclusions: The reported frequency of self-medication in urban areas of Colombia changed across the studies depending on the time frame used; therefore, this should be considered when conducting and comparing studies on selfmedication prevalence. Although over-the-counter drugs were the most frequently involved (85.7%), the reported use of prescription drugs such as antibiotics/antiparasitics was 12.1%. Considering that lacking time and delays in medical care were reported in a third of cases of self-medication, shortening the long waiting times for healthcare services might contribute to the proper use of medications. Evidence reviews are widely used to summarize findings from existing studies and, as such, are an important base for policy analysis. Over the past 50 years, three waves of evidence reviews have emerged: (1) the meta‐analysis wave, (2) the mixed‐methods synthesis wave, and (3) the core components wave. The present article first describes these waves and reflects on the benefits and limitations of each wave in the context of policy analysis. Informed by this historical account, the article then identifies and discusses three trends that are likely to influence future directions of evidence reviews: (1) using data science tools, (2) embedding an equity focus, and (3) translating research into practice. The concluding discussion connects these developments to public policy, identifying how evidence from systematic evidence reviews informs—or could better inform—policy decisions.

- Abstract
Meta-analysis consists of a set of statistical techniques that aims to combine the results of two or more primary studies. It enables the calculation of point and interval estimates of some population parameter, usually a measure of effect size. The aim of this article is to introduce fundamental concepts of meta-analysis and its applications for psychologists and psychology students. The article: (1) introduces the logic of meta-analysis, its uses and common criticisms levied against it; (2) presents two computational models of meta-analysis commonly used by researchers; and (3) addresses two issues associated with the correct interpretation of results from meta-analyses: heterogeneity and subgroup analysis. A worked example illustrates the concepts throughout the article. The Supplementary Materials contain a worked example of the models presented in the text, a script in R language that allows the reader to reproduce the results, and a commented list of additional sources. This study is to investigate the effect of home-based cardiac rehabilitation (HBCR) on quality of life, functional capacity, and readmission rates in patients with heart failure. Randomized controlled trials (RCTs) were screened from Cochrane Library, CINAHL, EMBASE, and MEDLINE. The intervention group received a standardized HBCR or a comprehensive rehabilitation strategy that included HBCR. The participants in the control group received CR at a medical center or usual care without CR intervention. The main outcome measurements included quality of life, exercise capacity, mortality and re-hospitalization. This meta-analysis included 20 RCTs, in which 16 studies compared HBCR with usual care, and 4 studies compared HBCR with center-based CR. In comparison with the usual care, HBCR improved the total quality of life score [MD=-5.85, 95 % CI (-9.76, - 1.94), P=0.003, I2=75 %]. Patients with HBCR and usual care were significantly different in VO2 max [MD=1.05 mL/kg/min, 95 % CI (0.35, 1.75), P=0.003, I2=46 %]. However, VO2 max of patients with HBCR was not significantly different from those with center-based CR [MD=0.08 mL/kg/min, 95 % CI (-1.29, 1.44), P=0.91, I2=0 %]. There was statistically significant difference in the 6-min Walk Distance between usual care and HBCR (for distance [MD=11.84, 95 % CI (7.41, 16.28), P<0.00001, I2=0 %]; and for feet [MD=98.93, 95 % CI (26.79, 171.08), P=0.007, I2=56 %]). However, there was no significant difference in 6-min Walk Distance between patients with HBCR and center-based CR [MD=12.45, 95 % CI (-9.81, 34.72), P=0.27, I2=0 %], or in anxiety and depression between patients with usual care and HBCR (for anxiety, [MD=-0.25, 95 % CI (-0.56, 0.05), P=0.11, I2=0 %]; for depression, [MD=-0.18, 95 % CI (-0.51, 0.16), P=0.30, I2=0 %] . No significant difference was found in death number [RR=1.04, 95 % CI (0.55, 1.98), P=0.90, I2=0 %] or in the number of re-hospitalization [RR=0.88, 95 % CI (0.66, 1.18), P=0.40, I2=0 %] between usual care and HBCR. For patients with heart failure, compare with usual care and center-based CR, HBCR can improve the total quality of life. Compare with usual care, HBCR can improve VO2max and 6-min Walk Distance, but compare with centerbased CR, there are no differences in mortality, re-hospitalization rate or incidence of anxiety and depression. Additionally, centerbased CR and HBCR showed similar outcomes and medical costs. [ABSTRACT FROM AUTHOR]

RANDOM effects model , *DRUG therapy , *MEDICAL specialties & specialists , *STANDARD deviations , *STATISTICAL models - Abstract
Introduction: cardiology is one of the medical specialties with the most systematic reviews and meta-analyses. Studying the methodology of the reviews and analyzing their statistical heterogeneity is essential to guarantee their scientific validity. Objective: to describe the comparison of association measures, statistical models and degree of heterogeneity in meta-analyses of systematic reviews of pharmacological interventions in cardiology, published between 2000-2005 and 2011-2016. Methodology: analytical study based on the description and comparison of statistical methods of systematic reviews of pharmacological interventions in cardiology, published in the Cochrane library. For the qualitative variables, absolute and relative frequencies were estimated, while for the quantitative ones, means and standard deviations, or medians and interquartile ranges, were determined, depending on their distribution. The Student's t test was used to establish the difference in means and the Chi square for the difference in proportions. Results: 54 systematic reviews were included, with a total of 1.053 meta-analyses, 6 reviews with 240 meta-analyses between 2000-2005, and 48 reviews with 813 metaanalyses between 2011-2016. Most meta-analyses used standard treatment as the comparison group (56.6%), measured nominal qualitative outcomes (86.3%), determined relative risks (63.3%), and applied fixed-effect models (57.8%). In the 2011-2016 period, an average of the Higgins Index was found to be 17.5 lower than in the 2000-2005 (p<0.05). Conclusion: there was evidence of a decrease in statistical heterogeneity and an increase in the implementation of random effects models, which accounts for greater rigor when it comes to demonstrating truly significant results. Aim: To evaluate the quality of meta-analyses reviewing patient safety when residents are involved in patient care compared to specialists. Material and methods: In May 2023, a systematic review of meta-analysis was carried out using the search terms "residents and patient safety" in the PubMed and Embase databases and with these terms in Spanish in Lilacs, without filtering by publication date. The finally selected metaanalyses were scored with the AMSTAR 2 assessment tool. Results: Three meta-analyses were extracted for qualitative analysis. Scoring with the AMSTAR 2 assessment tool found that the overall confidence of its results was critically low. Conclusions: The meta-analyses evaluated were consistent in reporting the longest operative time of residents. Also about patient safety when the resident performs the surgery. Assessment with the AMSTAR 2 assessment tool showed that overall confidence in the results of the meta-analyses was critically low. [ABSTRACT FROM AUTHOR]

DATABASE searching , *ATTEMPTED suicide , *MENTAL illness , *DRUG addiction , *MOOSE , *SELF-destructive behavior - Abstract
Background: The CS-DS (Chronic Self-Destructiveness Scale) was designed and validated in 1985 and to date its applications in research studies are unknown. Objective: To analyze the uses of the CS-DS scale through a systematic review and compare scores of indirect self-destructive in different population groups in the period 1985-2019. Methods: Systematic review in five multidisciplinary databases with three search strategies following Cochrane recommendations, the phases of the PRISMA guide and the MOOSE guide criteria. The selection of the studies and the extraction of the variables was carried out by two researchers guaranteeing reproducibility. The methodological quality was evaluated and meta-analysis of indirect measures was performed by type of population studied, random effects meta-analysis for difference of means according to sex and meta-regression by scale dimensions. Results: were included 23 studies with 6,382 individuals. The lowest self-destructiveness was found in a healthy population and psoriasis, the highest in drug addiction, followed by suicide attempts, delinquency, anxiety or depression and schizophrenia. In healthy population, no differences were found according to sex and in people with suicidal intent there was greater self-destructiveness in men in the dimensions of personal and social negligence, lack of planning and impotence and passivity in the face of problems. Conclusion: The CS-DS scale has been applied basically in the United States and Poland, in different populations of healthy and sick people. CS-DS demonstrates discriminating ability to profile self-destructiveness in different populations, mainly with mental disorders, while its dimensions have a differential profile by sex. - Abstract
The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) statement, published in 2009, was designed to help systematic reviewers transparently report why the review was done, what the authors did, and what they found. Over the past decade, advances in systematic review methodology and terminology have necessitated an update to the guideline. The PRISMA 2020 statement replaces the 2009 statement and includes new reporting guidance that reflects advances in methods to identify, select, appraise, and synthesise studies. The structure and presentation of the items have been modified to facilitate implementation. In this article, we present the PRISMA 2020 27-item checklist, an expanded checklist that details reporting recommendations for each item, the PRISMA 2020 abstract checklist, and the revised flow diagrams for original and updated reviews. EQUINE infectious anemia , *META-analysis - Abstract
The aim of this study was to determine, through a systematic review and meta-analysis, the combined prevalence of equine infectious anemia (EIA) in domestic equids in South America, Central America and the Caribbean regions, in the period 2010-2021. Information was selected from articles published in the electronic databases PubMed/ PubMed Central, Science Direct, Scielo, Ebsco and Google Scholar. Original articles and postgraduate theses, with full text available, were considered in the study, including domestic equids, using serological techniques recommended by the OIE and providing the sample size and number of positive samples. A meta-analysis to estimate the combined prevalence of the disease and a subgroup analysis were carried out to determine the effect of the geographic regions (South America, Central America and the Caribbean), the animal species (horses, donkeys and mules) and the period of publication of the articles (2010-2016 and 2017-2021). The effect of the groups on the combined prevalence was further assessed by meta-regression analysis. Forty-nine articles were included in the study, from which a combined prevalence of 11.8 % was determined (95 % CI: 10.3-13.5). The multivariate meta-regression analysis showed that there was an association of the combined prevalence of the disease with the study period and the animal species studied. It is concluded that there are few studies regarding the behavior of this disease in the regions studied, besides there was an association of the combined prevalence of the disease with the study period and the species studied. - Abstract
Introduction and Objectives: Multivessel percutaneous coronary intervention (MV-PCI) is recommended in patients with ST-segment elevation myocardial infarction (STEMI) and multivessel coronary artery disease (CAD) without cardiogenic shock. The present network meta-analysis investigated the optimal timing of MV-PCI in this context., Methods: We pooled the aggregated data from randomized trials investigating stable STEMI patients with multivessel CAD treated with a strategy of either MV-PCI or culprit vessel-only PCI. The primary outcome was all-cause death. The main secondary outcomes were cardiovascular death, myocardial infarction, and unplanned ischemia-driven revascularization., Results: Among 11 trials, a total of 10 507 patients were randomly assigned to MV-PCI (same sitting, n=1683; staged during the index hospitalization, n=3460; staged during a subsequent hospitalization within 45 days, n=3275) or to culprit vessel-only PCI (n=2089). The median follow-up was 18.6 months. In comparison with culprit vessel-only PCI, MV-PCI staged during the index hospitalization significantly reduced all-cause death (risk ratio, 0.73; 95%CI, 0.56-0.92; P=.008) and ranked as possibly the best treatment option for this outcome compared with all other strategies. In comparison with culprit vessel-only PCI, a MV-PCI reduced cardiovascular mortality without differences dependent on the timing of revascularization. MV-PCI within the index hospitalization, either in a single procedure or staged, significantly reduced myocardial infarction and unplanned ischemia-driven revascularization, with no significant difference between each other., Conclusions: In patients with STEMI and multivessel CAD without cardiogenic shock, multivessel PCI within the index hospitalization, either in a single procedure or staged, represents the safest and most efficacious approach. The different timings of multivessel PCI did not result in any significant differences in all-cause death. Introduction: Human vitamin D levels have been increasingly related to a wide range of clinical outcomes. There is a large amount of reports on its associations, especially with obstetric complications, including preeclampsia and gestational diabetes. These results are scarcely consistent and there is still a lack of quality intervention studies to confirm the role of vitamin D in those outcomes. Objective: To review the available scientific evidence on the role of maternal vitamin D in the development of preeclampsia. Materials and methods: The methodology used followed the recommendations of the Cochrane guide for the preparation of systematic reviews, and for metaanalysis, the Guide of the Metaanalysis of Observational Studies in Epidemiology group (MOOSE). The search included both observational studies and controlled clinical trials. Results: Low vitamin D levels, measured by the 25-hydroxyvitamin D test, are common in pregnancy. The results of this systematic review and metaanalysis suggest an inverse ratio between vitamin D levels and the development of preeclampsia. There was heterogeneity among the studies with regard to the design, population, geographic location, definitions of exposure, and the outcome. We excluded randomized controlled trials from this meta-analysis. Conclusion: The inverse association we found suggests that the higher the levels of vitamin D the lesser the probability of developing preeclampsia, in spite of the heterogeneity of the global measurement in this type of analysis.

Background: Animal-assisted interventions (AAI) are increasingly applied for people with post-traumatic stress disorder (PTSD) symptoms albeit its effectiveness is unclear. Objectives: To examine the effectiveness of AAI for treating PTSD symptoms.Method: We searched 11 major electronic databases for studies reporting quantitative data on effects of AAI for children and adults with PTSD symptoms. Of 22ʹ211 records identified, we included 41 studies with 1111 participants in the systematic review comprising eight controlled studies with 469 participants in the meta-analysis. We conducted random-effects meta-analyses with all controlled studies based on standardized mean differences (SMD), and calculated standardized mean change (SMC) as effect sizes for studies with a pre-post one-group design. Two independent researchers assessed the quality of the included studies using the NIH Study Quality Assessment Tools. The primary outcome was PTSD or depression symptom severity measured via a standardized measurement at pre- and post-intervention assessments. Results: There was a small but not statistically significant superiority of AAI over standard PTSD psychotherapy (SMD = −0.26, 95% CI: −0.56 to 0.04) in reducing PTSD symptom severity while AAI was superior to waitlist (SMD = −0.82, 95% CI: −1.56 to 0.08). Getting a service dog was superior to waiting for a service dog (SMD = −0.58, 95% CI: −0.88 to −0.28). AAI led to comparable effects in reducing depression as standard PTSD psychotherapy (SMD = −0.03, CI: −0.88 to 0.83). Pre-post comparisons showed large variation for the reduction in PTSD symptom severity, with SMCs ranging from −0.38 to −1.64, and for depression symptom severity, ranging from 0.01 to −2.76. Getting a service dog lowered PTSD symptoms between −0.43 and −1.10 and depression with medium effect size of −0.74.Conclusions: The results indicate that AAI are efficacious in reducing PTSD symptomatology and depression. Future studies with robust study designs and large samples are needed for valid conclusions. We conducted a systematic review with meta-analysis to provide an up-to-date summary on the state of evidence regarding the effectiveness of animal-assisted interventions in treating people with PTSD symptoms. Results are promising but high-quality research is lacking to date. [ABSTRACT FROM AUTHOR]

POST-traumatic stress disorder , *SEROTONIN uptake inhibitors , *VENLAFAXINE , *PRAZOSIN , *PAROXETINE - Abstract
Background: Pharmacological approaches are widely used for post-traumatic stress disorder (PTSD) despite uncertainty over efficacy. Objectives: To determine the efficacy of all pharmacological approaches, including monotherapy, augmentation and head-to-head approaches (drug versus drug, drug versus psychotherapy), in reducing PTSD symptom severity. Method: A systematic review and meta-analysis of randomised controlled trials were undertaken; 115 studies were included. Results: Selective serotonin reuptake inhibitors (SSRIs) were found to be statistically superior to placebo in reduction of PTSD symptoms but the effect size was small (standardised mean difference −0.28, 95% CI −0.39 to −0.17). For individual monotherapy agents compared to placebo in two or more studies, we found small statistically significant evidence for the antidepressants fluoxetine, paroxetine, sertraline, venlafaxine and the antipsychotic quetiapine. For pharmacological augmentation, we found small statistically significant evidence for prazosin and risperidone. Conclusions: Some medications have a small positive effect on reducing PTSD symptom severity and can be considered as potential monotherapy treatments; these include fluoxetine, paroxetine, sertraline, venlafaxine and quetiapine. Two medications, prazosin and risperidone, also have a small positive effect when used to augment pharmacological monotherapy. There was no evidence of superiority for one intervention over another in the small number of head-to-head comparison studies. Our review found evidence of a small positive effect for a handful of medications, which can reduce the symptom severity in post-traumatic stress disorder. • These were fluoxetine, paroxetine, sertraline, venlafaxine and the antipsychotic quetiapine when used as monotherapy, prazosin and risperidone for augmentation. SEXUAL dimorphism , *PATELLA , *BONES , *WIDTH measurement , *MAGNETIC resonance , *META-analysis - Abstract
Sexual dimorphism is one of the most important ways to identify bone remains in mass disasters. Many of them have been used for this purpose; mainly skull, pelvis and long bones. However, only a few studies using the patella have been done and, to our knowledge, there are no assessments of previous results in the literature. Meta-analysis provides a useful strong tool to test, in a systematic way, the most relevant information about a given research field. The aim of this study is to apply the meta-analytic technique to assess the major studies concerning sexual dimorphism in the patella by measuring classical metric traits: maximum height and maximum width, with different techniques, such as caliper, radiography, tomography and magnetic resonance. The 17 papers found, involving a total sample size higher than 2600 patellae, showed a very high heterogeneity-around 93 % of I2 value, for height and width measurements when all the studies were analyzed together. Homogeneity increased when each study was classified according to the techniques used. In this case, a statistical difference appeared, among the several subgroups of techniques for the two measurements, suggesting the importance of the methodology used. Maximum height and maximum width were all showed to be statistically relevant in distinguishing both sexes. [ABSTRACT FROM AUTHOR]

Although research suggests that people with physical disabilities experience impacts to their health-related quality of life (HRQL), there are no previous reviews or meta-analyses of studies that use appropriate assessment instruments in this population. The objective of this project was to conduct a systematic literature review and meta-analysis of studies that use the World Health Organization Quality of Life Questionnaire (WHOQOL-BREF) to evaluate HRQL in people with physical disabilities. We searched the PsycINFO, Medline, CINAHL and The Cochrane Library databases, identifying articles published through February 2020. The methodological quality of the studies was analyzed, and the pooled mean scores for each domain of the WHOQOL-BREF were calculated. Ten studies involving 2,048 people with physical disabilities were included. Their methodological quality was adequate. The pooled mean scores were 49.5 for the physical domain, 57.7 for the psychological domain, 58.2 for the social relations domain, and 55.5 for the environmental domain. The findings of this work confirm an impact of physical disabilities on the four HRQL domains in this population. [ABSTRACT FROM AUTHOR]

TRUTH commissions , *HUMAN rights violations , *DICTATORSHIP , *META-analysis , *COLLECTIVE representation - Abstract
This article analyses the assessments of Truth Commissions (TCs) and their association with positive socio-emotional climate and trust in institutions in six South American countries The sample consisted of 4,442 participants (53.3% women), 13 to 90-years-old (M = 37.4, SD = 15.7). Participants answered a questionnaire comprised of scales that evaluated their knowledge, evaluation, emotions and social sharing about the TC, as well as the socio-emotional climate and the trust in institutions. Comprehensive Meta-analysis procedure and multiple linear regression were carried out. Positive evaluation of the TCs was related to trust in institutions (r̅ =.23, random model) and a positive socio-emotional climate (r̅ =.31, random model). Political positioning and age showed limited effects. Finally, the associations of positive TC evaluation with social climate and trust in institutions were found to be positively moderated by contextual factors. [ABSTRACT FROM AUTHOR]

ADVERSE childhood experiences , *LUNG diseases , *ADULTS , *CHRONIC diseases , *DISEASE risk factors , *ASTHMA , *OBSTRUCTIVE lung diseases - Abstract
Background: Adverse childhood experiences (ACE) affect physical and mental health and may appear as risk factors for the development of different conditions in adult life. Objective: To perform a literature review and meta-analysis on risk indicators for the development of chronic lung diseases in adulthood associated with ACE. Method: We conducted a systematic literature review according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines using the online databases PubMed, PsycINFO, and Web of Science. Quantitative studies involving male and female adults were included. Fixed- and random-effect models were used in the estimation of meta-analytical measures. The heterogeneity between studies was assessed using I2 statistics and Cochran's Q test. Results: A total of 19 studies were selected for the meta-analysis. The analyses showed statistically significant associations between ACE and lung diseases in general (OR = 1.41; CI 95%: 1.28–1.54), besides specific associations with asthma (OR = 1.32; CI 95%: 1.13–1.50) and COPD (OR = 1.44; CI 95%: 1.13–1.76). When the mediating effect of smoking was assessed separately we found a significant – although not quite expressive – association (OR = 1.06; CI 95%: 1.02 to 1.10), which weakens the hypothesis that a direct relationship exists between childhood trauma and the occurrence of lung diseases. Conclusions: ACE are an important risk factor for the development of lung diseases in adulthood, whether through direct or indirect contribution to this outcome, which highlights the relevance of increasing the awareness of health staff for the early detection and intervention in situations of vulnerability or risk in childhood as an important preventative measure. Objective. To estimate the general prevalence of Anaplasma spp. and specific prevalence of A. platys and A. phagocytophilum in canines, through studies published between 2000 and 2018. Material and methods. Systematic review with 14 search strategies, guaranteeing completeness and reproducibility according PRISMA. Quality was evaluated with STROBE. The global prevalence and the specific ones were estimated according to country, period and diagnostic test, with their confidence intervals of 95%. Forest Plot was performed for the individual and global prevalence of A platys or A. phagocytophilum according to PCR, ELISA and IFI, which were compared based on Statistic Z. Results. Thirty studies were included with 18 472 canines, mostly from Brazil, United States and Germany. In IFI the prevalence was 39.0% (95% CI = 37.0-41.0), in ELISA 9.3% (95% CI = 8.8-9.8) and in CRP 7.1% (95% CI = 6.4-7.8). The prevalence based on CRP was statistically greater in America with 11.9% (95% CI = 10.5-13.3) compared to 5.5% in Africa (95% CI = 1.2-9.7), Asia 4.1% (95% CI = 3.1-5.1) and Europe 3.5% (95% CI = 2.5-4.5). The prevalence of A. platys with CRP was 16.1% (IC95% = 14.2-17.9) and of A. phagocytophilum 3.7% (95% CI = 2.8-4.6). Conclusions. A high prevalence of global infection was found, with greater importance of A. platys, in a low number of publications worldwide and with a high heterogeneity according to the country, the diagnostic technique and the species involved. Introduction and objectives Coronary artery calcium (CAC) score improves the accuracy of risk stratification for atherosclerotic cardiovascular disease (ASCVD) events compared with traditional cardiovascular risk factors. We evaluated the interaction of coronary atherosclerotic burden as determined by the CAC score with the prognostic benefit of lipid-lowering therapies in the primary prevention setting. Methods We reviewed the MEDLINE, EMBASE, and Cochrane databases for studies including individuals without a previous ASCVD event who underwent CAC score assessment and for whom lipid-lowering therapy status stratified by CAC values was available. The primary outcome was ASCVD. The pooled effect of lipid-lowering therapy on outcomes stratified by CAC groups (0, 1-100, > 100) was evaluated using a random effects model. Results Five studies (1 randomized, 2 prospective cohort, 2 retrospective) were included encompassing 35 640 individuals (female 38.1%) with a median age of 62.2 [range, 49.6-68.9] years, low-density lipoprotein cholesterol level of 128 (114-146) mg/dL, and follow-up of 4.3 (2.3-11.1) years. ASCVD occurrence increased steadily across growing CAC strata, both in patients with and without lipid-lowering therapy. Comparing patients with (34.9%) and without (65.1%) treatment exposure, lipid-lowering therapy was associated with reduced occurrence of ASCVD in patients with CAC > 100 (OR, 0.70; 95%CI, 0.53-0.92), but not in patients with CAC 1-100 or CAC 0. Results were consistent when only adjusted data were pooled. Conclusions Among individuals without a previous ASCVD, a CAC score > 100 identifies individuals most likely to benefit from lipid-lowering therapy, while undetectable CAC suggests no treatment benefit.

This review describes essential concepts of network meta-analysis and indirect comparisons and their application to clinical science. There are an increasing number of systematic reviews and meta-analyses providing direct comparisons between different interventions, although this is often not feasible when there is a lack of evidence related to all possible comparisons. Thus, important statistical tools that help make indirect comparisons based on previously existing comparisons have been developed. Network meta-analyses are an innovative tool that could assist doctors, researchers, and governmental organizations when making clinical and public health decisions.
