Background. Efficacy the narrow-band UVB phototherapy in patients with atopic dermatitis varies greatly. An important condition for achieving optimal therapeutic effect is the identification of factors that can impact on the efficacy of therapy and considering their influence when prescribing treatment. Aims. The present study aimed to identify the factors which affect the efficacy of narrow-band phototherapy in patients with atopic dermatitis Methods. A prospective, open-label trial was conducted to evaluate the efficacy and safety of narrow-band UVB phototherapy for patients with moderate-to-severe atopic dermatitis. All patients were treated with narrow-band UVB phototherapy four times weekly for 5 weeks. Disease severity was evaluated by SCORing of the Atopic Dermatitis Index (SCORAD) and Eczema Area and Severity Index (EASI). Distribution of patients by the severity of therapeutic effect was evaluated. To compare the efficacy of therapy depending on initial atopic dermatitis severity, initial and cumulative irradiation doses, skin phototype, and smoking status patients were divided into subgroups. Results. 40 patients with moderate-to-severe atopic dermatitis received course of narrow-band UVB phototherapy. After NB-UVB therapy SCORAD and EASI scores reduced from 45.6 ± 11.4 at baseline to 22.6 ± 12.4 (p 0,05) and from 14.4 ± 7.2 at baseline to 4.1 ± 3.9 (p 0,05) respectively demonstrating the efficacy of narrow-band UVB phototherapy in patients with atopic dermatitis. Our investigation showed that tobacco smokers had definitely lower efficacy of NB-UVB phototherapy in comparison with non-smokers. Narrow-band UVB phototherapy had definitely higher efficacy when it is started with an initial dose 0.2–0.3 J/cm2 chosen in compliance with results of MED determing in comparison with an initial dose 0.05–0.15 J/cm2 selected according to skin phototype. Conclusions. Factors that impact on the efficacy of narrow-band UVB phototherapy in patients with atopic dermatitis were identified. It was determined that using higher initial dose is associated with higher efficacy of therapy. The obtained data suggest the opportunity of decrease in efficacy of therapy in smokers with atopic dermatitis.

The paper presents the results of a comparative study of various immunosuppressive treatment methods of medium severity vulgar psoriasis (PASI from 10 to 20). The parameters used were PASI indices, p53 apoptosis marker expression indices, and expression indices of T-regulatory cell marker Foxp3. The study involved 96 patients, from 18 to 60 years old, divided by randomization into three groups. Patients in the 1st group (n = 32) received narrow-band phototherapy according to a 4-day regimen. We used the Waldmann UV-7002K instrument and F79/120W-TL01 lamps generating radiation with a maximum emission at a wavelength of 311 nm. We started with a dose of 0.05–0.1 J/cm2, subsequently increased by 0.05–0.1 J/cm2 for each procedure, in the absence of erythema. In total, patients received 12–15 procedures, with a course dose of 4.7–7.65 J/cm2. In the 2nd group (n = 32), patients were treated with the sodium salt of the synthetic gamma-D-glutamyl-D tryptophan dipeptide. Patients of the 3rd group (n = 32) underwent combination therapy, which included both methods described above. As a result of treatment, the greatest decrease in the PASI index, as well as the level of expression of the marker of Foxp3 T-regulatory cells, was observed among patients who received the combined regimen. The data obtained indicate the high therapeutic efficacy of narrow-band phototherapy in combination with the sodium salt of the synthetic gamma-D glutamyl-D tryptophan synthetic dipeptide. It was also noted that the immunohistochemical marker p53, reflecting the cellular renewal of psoriatic papule keratinocytes, is an informative molecular-cellular indicator of the effectiveness of narrow-band phototherapy.

Currently, phototherapy has been widely used in the treatment of various skin diseases, including mycosis fungoides. The article provides a literature review on the use of various spectra ultraviolet therapy in the treatment of patients with early stages of mycosis fungoides. The mechanisms of action of medium-wave ultraviolet therapy (UVB-311 nm) and PUVA therapy, causing the immunosuppressive, anti-inflammatory and anti-proliferative effect, are described. The results of retrospective studies of PUVA therapy and UVB-311 nm efficacy in patients with cutaneous T-cell lymphoma are presented, as well as modern literature evidence of the need for standardization of phototherapy methods for patients with mycosis fungoides.

Aim: research the effectiveness of UVB 311 nm phototherapy in patients with the "escape effect" that developed in the 44th — 56th weeks of the ustekinumab use. Materials and Methods. The examination and treatment of 9 patients with extensive psoriasis vulgaris receiving ustekinumab by hypodermic injection at a dose of 45 mg if body weight of the patient not more than 100 kg, and 90 mg of body weight over than 100 kg, on the zero week, the 4th week and then every 12 weeks. Among them 6 (66,7%) men and 3 (33,3%) women aged from 28 to 65 years. All patients in the 44th — 56th weeks of ustekinumab observed effect of "escape" in connection with which he has been appointed narrowband phototherapy. Results. The combined use of ustekinumab and UVB 311 nm phototherapy in patients with "escape effect" contributes to the marked regression of clinical symptoms and allows you to control the process for long, which is confirmed by the dynamics of the index PASI, BSA and DLQI. Conclusion. In the case of the effect of "escape" in the application of biological therapy drugs (ustekinumab) possible appointment of a narrow-band medium wave phototherapy course 25-30 procedures.
