Introduction: With a high mortality rate, diabetes mellitus (DM) is the result of insulin deficiency during its secretion or action, more common in patients over 40 years old, being responsible for 95% of morbidity cases in Brazil. Well-planned physical therapy, with efficient interventions and exercises, aims to improve mobility and balance in individuals with peripheral diabetic neuropathy (PDN). Objective: The objective of this article is to describe the physical therapy interventions in patients with diabetic neuropathy. Methodology: The present work is a literature review, with data collection between August 2021 and May 2022, through a search in PubMed, SciELO and PEDro databases. Results and discussion: Considering the clinical repercussions of patients, professionals use treatment techniques with the objective of preventing the progression of symptoms and degeneration of neural function. Final Considerations: An isolated treatment considered the gold standard to be applied in patients with PDN was not found in the literature, transcutaneous electrical stimulation does not present among scholars a concession of applicability. Introducción: La Neuroartropatía de Charcot puede ser mal diagnosticada. Objetivo: Describir los elementos básicos para el diagnóstico de la Neuroartropatía de Charcot en personas con Diabetes mellitus. Material y Métodos: Se realizó una revisión bibliográfica descriptiva. Se utilizó como motor de búsqueda a Google Académico. Se empleo información de diferentes bases de datos. Las palabras clave utilizadas fueron: Diabetes mellitus; neuropatía diabética; neuroartropatía de Charcot; artropatía neuropática; pie de Charcot; pie diabético (en inglés y español). Se evaluaron artículos, que en general tenían menos de 10 años de publicados, en los idiomas referidos y que por el título trataban el tema de estudio. Fueron excluidos los que no abordaran la relación entre Diabetes mellitus y la neuroartropatía de Charcot. Se referenciaron 50 artículos. Desarrollo: La hiperglucemia persistente es causa de alteraciones neurotróficas, vasculares, inflamatorias e inmunológicas, que contribuyen a la aparición de la Neuroartropatía de Charcot, la cual se debe sospechar en personas con factores de riesgo que condicionan su aparición. Su diagnóstico es eminentemente clínico y puede evolucionar desde un estadio agudo (activo) a su etapa crónica (menos activa o inactiva). Las imágenes de Rx convencional y resonancia magnética de la zona afectada, pueden ayudar al diagnóstico positivo y diferencial. Conclusiones: El diagnóstico de la Neuroartropatía de Charcot, se realiza a través de la verificación de la existencia de factores de riesgo que condicionan su aparición en una persona con Diabetes mellitus. Su diagnóstico es predominantemente clínico y la imagenología, pueden ayudar al diagnóstico positivo y diferencial.

6. Neuropatía autonómica cardíaca en diabéticos; estudio transversal analítico.

- Abstract
Introduction: Cardiac autonomic neuropathy is a little known and underdiagnosed entity in diabetic patients, characterized by damage to autonomic nerve fibers causing symptoms such as exercise intolerance and postural hypotension; the prevalence and risk factors in the Colombian diabetic population are unclear and understudied. Objective: To determine the prevalence of cardiac autonomic neuropathy and associated factors in diabetic patients in a Colombian population. Materials and method: Analytical cross-sectional study, in a population of 107 patients with a diagnosis of diabetes mellitus type I and type II, who consulted a second level of care hospital in Colombia, between April and September 2022. Diagnosis was performed using the Ewing test of cardiovascular autonomic reflex. Statistical analyses were performed at descriptive and association level by logistic regression calculating odds ratio and 95% confidence intervals. Results: The study population had a mean age of 62 years, 56.1% of whom were women. 94.4% (CI 95%: 89.9-98.6) of the participants presented a positive evaluation for cardiac autonomic neuropathy; incipient status was 6.5%, 26.2% confirmed involvement and 61.7% severe involvement; age is associated with cardiac autonomic neuropathy presentation, ORa: 1.07 (CI 95%: 1.03-1.11). Conclusions: The current study found high prevalence of cardiac autonomic neuropathy (94.4%) when using the gold standard of diagnosis. La diabetes mellitus desencadena entre otras complicaciones la neuropatía periférica diabética, que tiene compromiso nervioso periférico, entre sus consecuencias principales es la disminución de la sensibilidad, la fuerza y la funcionalidad. El Objetivo de este estudio fue establecer un programa de ejercicios terapéuticos para restablecer la funcionalidad y sensibilidad de tobillo y pie en personas con neuropatía periférica diabética. De un total 34 pacientes, se realizaron las evaluaciones antes y posterior a la intervención fisioterapéutica que fueron ejercicios de equilibrio y fuerza, se evaluaron con el Test de Michigan Neuropathy Screening Instrument (MNSI) que es para valorar los síntomas y una exploración física, la prueba de lo Monofilamentos de Sammes-Weinstein (10g-MF) que sirve para evaluar la sensibilidad y para la valorar la fuerza con la Dinamometría Manual (HHD). Solo 17 pacientes culminaron este estudio, donde mostraron el test de Michigan no se encontró resultados estadísticamente significativos, pero si clínicamente, en cambio para la valoración con la prueba 10G-MF y HHD si se encontró cambios significativos. Los resultados obtenidos fueron insertados en una base de datos con el objetivo de crear un repositorio para futuros estudios en el Ecuador. Para insertar la información se desarrolló un sistema de gestión que permite registrar los datos sociodemográficos de los pacientes, registrar las patologías, la intervención fisioterapéutica implementada, y los resultados alcanzados para cada caso, en determinados periodos de tiempos que también son registrados. Este estudio mostró que los ejercicios implantados en tobillo y pie son beneficiosos para mejorar la sensibilidad y fuerza de los pacientes con DPN.

- Abstract
Diabetes mellitus, a chronic and progressive condition, was the third most common cause of death in Mexico between January and June 2021. Its most frequent chronic complication is diabetic neuropathy, which has a major impact on the nervous system. A multidisciplinary group of experts met in Mexico City to establish a treatment algorithm considering the symptomatic and etiopathogenic aspects of diabetic neuropathy. A real-time Delphi method with two rounds of interative questions was used. The implementation of the proposed algorithm will allow a comprehensive approach to the diabetic patient with painful and non-painful neuropathy, both in terms of symptoms and etiopathogenesis. This approach provides the opportunity to improve quality of life and achieve reintegration into family and work life. The expert panel recommends thioctic acid as the first line etiopathogenic treatment for diabetic neuropathy. [ABSTRACT FROM AUTHOR]

Introduction: diabetes mellitus leads to many associated complications, including diabetic neuropathy and diabetic angiopathy, both with a high incidence in the onset diabetic foot and including also the diabetic retinopathy disease cause of blindness. Objective: determine if there is any correlation between the onset diabetes foot and whether or not diabetic retinopathy present in patients treated at the Hospital Provincial, María Eugenia Neto, Zaire, Republic of Angola, from September 2020 to September 2020. Method: a descriptive study was carried out in 181 patients treated in the diabetic foot department of the aforementioned hospital; variables described were as follow: age, sex, associated chronic diseases, presence or absence of diabetic retinopathy, lower limb ulcers, limb amputation and years with diabetes. Bivariate correlation tests were performed, and the Pearson´s correlation coefficient was analyzed. Results: the average age was 59.3 years, the age group over 70 years (28.2%) and the female sex (57.5%) predominated. The 41.8% of patients presented previous amputations and 40.1% had ulcers. The 65.19% of patients had irregular treatment patterns, statistically strong and significant correlation between diabetic foot and diabetic retinopathy. Conclusions: there is evidence of a strong and significant relationship between patients with diabetic foot, who may suffer from a certain degree of diabetic retinopathy, more frequent in females after their sixth decade of life that cause a longer evolution of his disease.

13. Procedimientos de enfermería para la valoración de la neuropatía periférica diabética. Revisión bibliográfica

- Abstract
Diabetes, a chronic metabolic condition that raises blood glucose levels, carries serious health consequences. Among its most significant complications is diabetic peripheral neuropathy (DPN), common in both type 1 and type 2 diabetes. Research in the literature indicates that around 50% of people with diabetes suffer from this condition, frequently manifested at through symptoms such as numbness, tingling, pain and weakness in the extremities. Properly evaluating and monitoring this condition is essential for its treatment. With the aim of investigating the methods of assessment of diabetic peripheral neuropathy, a systematic review was carried out following the PRISMA protocol. This review covered various academic and scientific databases, initially identifying 60 studies, of which, after applying specific criteria, 21 scientific articles were selected for analysis. The results showed that diabetic neuropathy manifests with specific symptoms such as pain, tingling and itching. Diabetes mellitus is positioned as the main risk factor associated with this condition, accompanied by elements such as age, hyperlipidemia, hypertension, smoking, alcoholism and overweight. Worldwide, the prevalence of this disease exceeds 50%. As for nursing procedures, these focus on patient care through the use of drugs, glucose control, physical therapy, podiatric care, and alternative approaches such as acupuncture, laser therapy, and electrical stimulation.

Introduction: foot examination is essential in patients with diabetes mellitus (DM). The correct evaluation of the foot in the patient who attends diabetes control is key to establish risk factors for the development of ulcers, detect injuries, take preventive measures, make an early referral and educate in foot care. Objectives: in this study, the feet of people with DM were examined during the Diabetic Foot Awareness and Prevention Campaign on November 1, 2019 in order to assess the presence of symptoms and signs related to neuropathy, peripheral vascular disease, prevalence of the same and know the risk. Materials and methods: 165 patients were analyzed in four centers: Sanatorio Güemes (Diabetes and Endocrinology Service), PREDIGMA (Preventive Medicine Center, Posadas, Misiones), Central Hospital of San Isidro, Nexo Medical Center (Junín City) and Hospital Municipal of General Viamonte (Province of Buenos Aires).

Se efec-tuó una búsqueda en español e inglés de artículos originales, revisiones sistemáticas y meta-análisis en las bases de datos PubMed, Google Scholar, Scielo y Science Direct con las siguientes palabras clave: Neuropatía diabética, marcadores electrofisiológicos, exploración física y pie diabético. Se eligieron 64 publicaciones entre 1981 y 2019. Se encontró una mayor posibilidad de tamizaje mediante el uso de pruebas electrofisiológicas en combinación con pruebas de evaluación mixta, ya que permiten un abordaje objetivo, mínimamente invasivo y reproducible

16. Síndrome de mano diabética: un diagnóstico clínico frecuentemente olvidado.

BACKGROUND: Diabetic hand syndrome is a frequent complication and is little valued by health personnel. Classically it is described as osteoarticular abnormalities related to long-standing diabetes, poor metabolic control, and microvascular complications. However, other disorders, such as skin involvement, infection of the hand and entrapment of peripheral nerves are part of this illness. CLINICAL CASE: A 66-year-old man, with a history of long-standing type 2 diabetes mellitus, undergoing intensive insulin therapy, consulted for presenting non-painful blisters on the fingers and palms of both hands, associated with weakness, changes in sensitivity and rigidity of the same. He had microvascular complications due to nephropathy, retinopathy and diabetic neuropathy, his glycated hemoglobin was 10.9%. Physical examination documented semiological findings compatible with diabetic bullosis and diabetic cheiroarthropathy. CONCLUSIONS: Osteoarticular, dermatological and neuropathic involvement of the hand should be recognized by health personnel as a specific complication of long-standing diabetes. Its early detection is of great value to prevent its progression and guide the search for microvascular complications. The diagnosis of this entity is clinical; therefore, it is advisable to carry out a routine evaluation of the hands of the diabetic patient, in order to install multidisciplinary management that will improve the outcomes. Objective: To evaluate the proportion of evidence of neuropathy and peripheral arterial disease, and to identify its relationship with sociodemographic variables, lifestyle and clinical factors with the classification of risk related to the foot of people with Diabetes Mellitus. Methods: Cross-sectional, descriptive, quantitative study. Sample for convenience 60 people in three Basic Health Units. It was used a sociodemographic form, clinical form for the assessment and screening of neuropathic pain, loss of protective sensitivity and peripheral arterial disease in primary care, a questionnaire for evidence of diabetic neuropathy. Pearson's test was applied in the relationship study. Results: Female predominance, mean age 58.1 (± 12.6) years, diagnosis time 12 years (± 9.6) and casual blood glucose 211.3mg/dl (± 91.7). Five participants presented evidence for neuropathy and none presented evidence for peripheral arterial disease. Significant correlation between risk classification, scores for neuropathic impairment (p <0.05) and monofilament (p <0.002). Conclusion: There was a low proportion of neuropathy and peripheral arterial disease, neuropathic impairment score and sensitivity to monofilament, were associated with risk classification. These relationships represent the need for intervention through the practice of screening for complications, which may contribute to the prevention of diabetic foot with amputation. This article describes the construction of classification models by applying machine learning algorithms, in the kinesiological domain of neuropathy diagnosis, in patients with diabetes. The process is developed using the Cross Industry Standard Process for Data Mining guide (CRISP-DM), according to the general process of Knowledge Discovery in Databases (KDD). The main objective of the research is to build a model for classifying the condition of diabetic neuropathy in patients, using the data recorded by the expert and, mainly, by using a Wii Balance Board to capture the postural variations of a subject undergoing a series of clinical evaluations. In this research, the results obtained in each phase of the process are analyzed and the degree of achievement of objectives and performance metrics in the evaluation of machine learning models are measured. The results obtained will be expanded with the analysis of time series and the extraction of features, under the hypothesis that the center of pressure (COP) is a sufficient and reliable predictor to discriminate against the existence of a neuropathy. Background: Cardiovascular autonomic neuropathy (CAN) is a common complication of diabetes and is associated with cardiovascular morbidity and mortality. Despite its prevalence, it is usually underdiagnosed. Objective to assess frequency of CAN in type 2 diabetes, treated at the Diabetology Service at Cordoba Hospital, Argentina.Methods: Cross-sectional study was conducted from May 2015 to May 2016. Cardiovascular Autonomic Tests (Ewing´s battery) were performed: response to Valsalva maneuver, expiration-to-inspiration ratio (E/I ratio), standing to lying flat and postural hypotension.Results: 100 type 2 diabetes patients, 53% females. The mean age and diabetes duration 52 years old and 8.4 years, respectively. Hemoglobin A1c level of 8.7% and 47% were insulin-requiring type 2 diabetes patients. 29% had early CAN, 54% confirmed CAN (10% severe CAN) and 17% normal cardiovascular tests. Confirmed CAN was higher in those with longer duration of diabetes; longer duration of insulin therapy, older patients and severe hypoglycemia events was higher in those with confirmed CAN versus without CAN or early CAN (p 0.003). The risk factors associated were hypertension OR 2.55 (p 0.02), peripheral neuropathy OR 11.7 (p 7% OR 2.57 (p 0, 03).Conclusions: frequency of CAN was high, was associated with hypertension, microvascular complications, older age, poor metabolic control, longer duration of disease and insulin therapy. Patients with a higher risk of developing CAN should be identified, in order to reduce the impact of this complication.

Introduction: strategic interventions intended to reduce the amputation rates and enhance the survival among patients with diabetes mellitus starts from the prevention based on educating the patients, identifies risk factors and multidisciplinary treatment of the wounds. Objective: To describe the effective interventions to prevent the amputation in patients with diabetic foot. Methodology: It is a comprehensive review of literature, including articles in Spanish, English and Portuguese coming from any country and conducted under any design that were published between 2015 and 2020 in the data bases: Clinicalkey, BVS biblioteca virtual en salud, Scopus, EBSCOHost, Pubmed y Cuiden. Selection was filtered based on abstract, title and full text. Articles were sorted according to evidence level and recommendation. Finally, a critical reading was carried out after the scales of Amstar, Consort, Strobe, CASPe y Trend. Results: At the end 29 article were selected and strategies were organized in to 3 topics according to the prevention level: Level one: promotion, prevention and mitigation of damage. Level two: identification, diagnosis and treatment, and level three: rehabilitation. Conclusions: Regular evaluation of diabetic foot is crucial and must be done by an interdisciplinary team. The diabetic neuropathy is a severe complication resulting from Diabetes Mellitus (DM), which can cause ulcerations in the feet and lead to amputations in the lower limbs (LL). The aim of the present study is to evaluate the feet, self-care and the neuropathy impairment of LL in individuals with Type 2 DM (DM2), as well as to correlate impairment with age, diagnosis time and glycemic control. Socio-demographic and anthropometric data were collected, and fasting blood glucose and glycated hemoglobin (HbA1C) tests were conducted, as well as clinic examination was applied to the feet. Plantar protective sensitivity was assessed, as well as the Neuropathy Impairment Score (NIS), the Neuropathy Symptom Score (NSS) and the Diabetes Self-Management Questionnaire (DSMQ). Results were descriptively shown and the Spearman correlation test was adopted to correlate DM2 diagnosis time with NIS, NSS, age and HbA1C (p<0.05). Sixty-six individuals, at the age group 62 years (56-65) were evaluated. The physical examination showed signs of neuropathic foot and arterial insufficiency. There was change in the plantar protective sensitivity (48.5%), in the neuropathy impairment (48.5%) and in the neuropathy symptoms (59%). The combination between NIS and NSS diagnosed 33% of patients with peripheral diabetic neuropathy. There were weak positive correlation between age and DM2 diagnosis time, age and NIS, HbA1c and NSS, and among DM2 diagnosis time, NIS and NSS. The self-management items concerning the feet presented good adhesion. Conclusion: We found signs of neuropathic foot and insufficient arterial, these predictive factors of future foot ulcerations, weak positive correlation between age and time of diagnosis with neuropathic changes and good adherence to self-care with feet. Neuropathic pain (NP) ¡s the most frequent reason for consultation in patients with peripheral neuropathy, an especially frequent condition in patients with diabetes mellitus. Distal symmetric polyneuropathy is the most prevalent presentation of diabetic neuropathy (DNP), which tends to feature NP in up to a third of the patients, representing a significant detriment in their quality of life. Several drugs and strategies have been designed to correctly approach patients with NP. Among conventional treatments, anticonvulsants such as pregabalin and gabapentin are central alternatives, followed by serotonin and noradrenalin reuptake inhibitors such as duloxetine and venlafaxine, as well as tricyclic antidepressants. Opioids are more controversial and have not been universally recommended. Various novel molecules are currently under study, focused on various therapeutic targets at different points in neuronal function. The objective of this review is to describe the management of NP in patients with DNP, and examine new recently proposed treatment strategies. [ABSTRACT FROM AUTHOR]

Introducción: la diabetes mellitus produce complicaciones, dentro de las cuales se describen la neuropatía diabética y la angiopatía diabética, que en presencia de estas puede llegarse a un cuadro de pie diabético sin dejar de mencionar la pérdida visual por retinopatía diabética. Objetivo: determinar la correlación existente entre la aparición de pie diabético y la presencia o no de la retinopatía diabética en pacientes del Hospital Provincial "María Eugenia Neto", Zaire, República de Angola, en el período comprendido entre septiembre de 2020 a septiembre de 2022. Método: se realizó un estudio descriptivo en 181 pacientes de la consulta de pie diabético del hospital antes dicho, donde se describieron variables, tales como: edad, sexo, enfermedades crónicas asociadas, presencia o no de retinopatía diabética, úlceras en miembros inferiores, amputación en miembros inferiores y años con diabetes. Se realizaron pruebas de correlación bivariable, se analizó el coeficiente de correlación de Pearson. Resultados: el promedio de edad fue de 59,3 años, predominó el grupo etario de más de 70 años (28,2 %) y el sexo femenino (57,5 %). Presentaron amputaciones previas un 41,8 % y úlceras un 40,1 %. Sin tratamiento estable el 65,19 %, correlación estadísticamente fuerte y significativa entre pie diabético y la retinopatía diabética. Conclusiones: se evidencia una fuerte y significativa relación existente entre los pacientes portadores de pie diabético, los cuales pueden padecer determinado grado de retinopatía diabética, más frecuentes en el sexo femenino después de la sexta década de la vida, con los cuales presentan mayor tiempo de evolución de su enfermedad.

A Diabetes Mellitus (DM) é uma síndrome metabólica de origem múltipla, decorrente da falta de insulina e/ou da incapacidade de a insulina exercer adequadamente seus efeitos. Sendo então, a neuropatia diabética a complicação mais prevalente da diabetes, que afeta os nervos periféricos, principalmente das mãos e dos pés, podendo haver presença de dor, falta de sensibilidade no local, formigamentos e falta de força, entre outros sintomas mais graves, devido a alterações no sistema nervoso somatossensorial. O tratamento dessa dor neuropática inclui o controle da glicemia e uso de medicamentos para controlar os sintomas, desse modo, o uso médico de canabinoides demonstra inegáveis efeitos analgésicos para o tratamento de dor neuropática e inflamação, podendo melhorar a eficácia terapêutica. Tendo em vista esse importante assunto, esse trabalho objetiva compreender o uso de canabidiol para o alívio da dor neuropática em casos de neuropatia diabética. De maneira mais especifica, objetiva compreender o que é a neuropatia diabética e as consequências da dor neuropática, assim como avaliar se há benefício no uso de canabidiol para o manejo da dor em pacientes com neuropatia diabética.

A 10% of the patients with diabetes exhibit neuropathy at the moment they were diagnoses with diabetes, while that percentage elevates to 40%-60%, depending on the use of electrophysiology or clinical criteria.

35. Differential effects in CGRPergic, nitrergic, and VIPergic myenteric innervation in diabetic rats supplemented with 2% L-glutamine

ABSTRACT The objective of this study was to investigate the effects of 2% L-glutamine supplementation on myenteric innervation in the ileum of diabetic rats, grouped as follows: normoglycemic (N); normoglycemic supplemented with L-glutamine (NG); diabetic (D); and diabetic supplemented with L-glutamine (DG). The ileums were subjected to immunohistochemical techniques to localize neurons immunoreactive to HuC/D protein (HuC/D-IR) and neuronal nitric oxide synthase enzyme (nNOS-IR) and to analyze varicosities immunoreactive to vasoactive intestinal polypeptide (VIP-IR) and calcitonin gene-related peptide (CGRP-IR). L-Glutamine in the DG group (i) prevented the increase in the cell body area of nNOS-IR neurons, (ii) prevented the increase in the area of VIP-IR varicosities, (iii) did not prevent the loss of HuC/D-IR and nNOS-IR neurons per ganglion, and (iv) reduced the size of CGRP-IR varicosities. L-Glutamine in the NG group reduced (i) the number of HuC/D-IR and nNOS-IR neurons per ganglion, (ii) the cell body area of nNOS-IR neurons, and (iii) the size of VIP-IR and CGRP-IR varicosities. 2% L-glutamine supplementation exerted differential neuroprotective effects in experimental diabetes neuropathy that depended on the type of neurotransmitter analyzed. However, the effects of this dose of L-glutamine on normoglycemic animals suggests there are additional actions of this beyond its antioxidant capacity.

Diabetic neuropathy (DN) is the most common and early microvascular alteration associated to diabetes mellitus (DM); however, this event is most missed and less diagnosed. At the same time, it is widely known that metabolic syndrome (MS) is highly prevalent and the relationship between this status and macrovascular commitment is documented. The Diabetic Neuropathy Committee is focused in analyzing the neuropathic effect within the framework of MS and in one of its components such as non-diabetic disglycemia. The under diagnosis and increase of morbidity and mortality risk of these patients alert the medical community of the existence of this microvascular alteration exceeding the DM framework and is also present in MS and disglycemia either at fasting and postprandial.

Introduction. Up to 76% of patients who present for consultation for diabetes mellitus (DM) have reported having one or more gastrointestinal symptoms, while 50-55% of patients report having upper gastrointestinal symptoms. The autonomic neuropathy explains to a large extent the disorders of digestive motility in these subjects, however, it is possible that other factors are involved. Our objective was to determine the prevalence and relationship between gastrointestinal symptoms in patients with diabetes mellitus and its associated factors. Material and Methods. a prospective, cross-sectional and comparative study, in which the PAGI SYM questionnaires (validated in Spanish) were applied to subjects with type 2 diabetes mellitus that presented to two reference centers in 2013. Descriptive statistics and SPSS V. 22 were used. Results. 157 subjects were included (69.4% women), with an average age of 55.8 years. The average time of evolution of the DM was 10.9 ± 9.1 years and the average glycosylated hemoglobin (HbA1c) was 13.5 ± 31.5. The most common complication was diabetic neuropathy with 27.40%. 83.4% of the patients had at least one upper GI symptom. The presence of the most common GI symptoms according to the PAGI SYM were: heartburn in 38.9%, postprandial fullness with 38.2% and 31.8% nocturnal heartburn. The overall presence of symptoms compatible with gastroparesis was 71%. Discussion. More than 80% of patients with type 2 DM have at least one GI symptom, the most frequent being symptoms associated with gastroparesis. The factors associated with the presence of GI symptoms were glycemic control and the presence of neuropathy and retinopathy. [ABSTRACT FROM AUTHOR]

Introducción: el pie diabético constituye un problema de salud en los pacientes portadores de diabetes mellitus y se registra como la primera causa de amputación no traumática de los miembros. El conocimiento de sus particularidades clínico-epidemiológicas resulta de sumo interés. Objetivo: caracterizar clínica y epidemiológicamente a los pacientes con pie diabético neuroinfeccioso en el Hospital "Dr. Joaquín Castillo Duany" de Santiago de Cuba. Método: se realizó un estudio descriptivo retrospectivo. El universo estuvo constituido por 96 pacientes diagnosticados con pie diabético durante el periodo comprendido entre el 2011 y el 2015. Se emplearon variables demográficas y relacionadas con factores de riesgo generales y específicos del pie diabético. Resultados: el pie diabético predominó entre la cuarta y sexta décadas de vida (24,7 %) y en pacientes del sexo femenino (59,7 %). La diabetes mellitus tipo 2 estuvo representada en el 52,5 %. Mientras el factor de riesgo general de pie diabético más frecuente fue la obesidad (56,7 %), la neuropatía diabética, como factor de riesgo específico se presentó en el 98,9 % de los enfermos estudiados. Conclusiones: el pie diabético predominó en la cuarta y sexta décadas de la vida y en el sexo femenino. El mayor porcentaje de los pacientes era portador de diabetes mellitus tipo 2. La obesidad, la neuropatía diabética periférica, el sobrepeso, y la hiperglucemia constituyeron los factores de riesgo prevalecientes.

Objetivo: Determinar mediante un estudio de costo efectividad cual de tres intervenciones en Xoxocotla, Morelos es la màs costo efectiva para disminuir la prevalencia de neuropatìa diabètica secundaria a Diabetes Mellitus.Material y Mètodos: En una muestra de 556 personas se realizó el cálculo del coeficiente costoefectividad y se aplicó la regla de decision. Resultados: Se obtuvo que la intervención A, llamada “Equilibra tu vida”, basada en actividad física, consulta nutricional y taller a familiar fue la más costo efectiva, obteniendo un costo total $311,841.00 con un porcentaje de efectividad del 30%, y un coeficiente de efectividad de 10.39 Conclusiones: No necesariamente una intervención màs costosa, es la intervención màs efectiva, y asì dar respuesta congruentes y concretas a la necesidades de salud.

OBJETIVO: Construir o perfil epidemiológico dos idosos com DMT2 cadastrados na Estratégia de Saúde da Família a partir das características de saúde, descrição do autocuidado e correlação entre alterações sensitivas e equilíbrio funcional. METODOLOGIA: Após aprovação do Comitê de Ética e Pesquisa da Universidade Estadual do Piauí (nº 462.651), a população do estudo compôs-se de 32 idosos com DMT2, idade média de 70,16(±6,74), atendidos pela Unidade Básica de Saúde do Bairro Poty Velho em Teresina, Piauí. RESULTADOS/CONCLUSÃO: Observou-se que 81,10% da amostra pertencem ao gênero feminino, 12,50% já apresentaram úlcera plantar, 84,40% com diagnóstico de polineuropatia e 59,40% enquadrados no estágio 3 de comprometimento sensitivo. Ao correlacionar-se o gradiente de alterações sensitivas com a classificação do Timed Up and Go Test e com a presença de neuropatia diabética, percebeu-se correlação significativa apenas no segundo caso. A adesão ao autocuidado foi maior quanto à terapia medicamentosa e mínima à monitorização da glicemia e prática de atividade física. Verificou-se que o autocuidado pode ser moldado através da reflexão sobre como realizavam os cuidados.

La diabetes representa un síndrome de enfermedades metabólicas complejas caracterizada por la presencia de la hiperglicemia, que muestra diversos orígenes (deficiencia y/o resistencia de insulina y consecuencias sistémicas del embarazo). Se sabe que la morbilidad y mortalidad de la diabetes es debida al desarrollo de las complicaciones macrovasculares y microvasculares. Sin embargo, la neuropatía diabética genera más hospitalizaciones que las otras complicaciones y es la causa más frecuente de amputaciones y falla autonómica. En ese sentido, la neuropatía diabética es heterogénea por sus síntomas, comprende patrones neurológicos, tiempo, riesgo y alteraciones patológicas que llevan a mecanismos que conducen al daño del nervio.

46. Encuesta transversal sobre los procesos de cribaje de pie diabético y pie de riesgo en atención primaria

Objetivo: El objetivo principal del presente estudio fue conocer la situación real de la atención del pie diabético y de riesgo en los centros sanitarios de Atención Primaria de España. Como objetivos secundarios, se estableció determinar si los profesionales de Atención Primaria identifican los procesos de cribado del pie diabético y de riesgo y, si los procesos de cribaje y diagnóstico, precisan implementar estrategias y acciones de mejora en su manejo. Diseño: Estudio cuantitativo, descriptivo y transversal realizado mediante encuesta electrónica autocumplimentada, diseñada ad hoc a través de la aplicación Google Forms, integrada en la plataforma de Google. Marco: Centros sanitarios de Atención Primaria en España. Participantes: Médicos/as y enfermeras/os colegiados/as de Atención Primaria en España. Se obtuvo un tamaño muestral final de 328 encuestados. Variables principales: Se midieron aspectos sociodemográficos y de formación; patologías tratadas; atención prestada por el profesional sanitario a los problemas y afecciones de los pies; procesos de cribado de la neuropatía periférica diabética; sistemas de clasificación de úlceras diabéticas; toma de muestras para cultivo microbiológico y antibiograma y opinión sobre la inclusión del podólogo en el Sistema Público de Salud. Resultados: El 30,8% de los participantes nunca ha recibido formación en pie diabético. La duración de ésta es inferior a 10 horas en casi el 45,0% de los casos. El 57,9% desconoce los cuestionarios de signos y síntomas utilizados para la valoración de la neuropatía periférica diabética. El cuestionario de síntomas positivos, Neuropathy Symptoms Score es el más conocido. El 65,9% de los encuestados reconoce no aplicar ninguno. Conclusión: Se observan déficits de formación relacionados con el nivel de identificación en los procesos de cribado en pie diabético y pie de riesgo. [Resumen] La neuropatía diabética constituye la complicación más habitual derivada de la diabetes mellitus llegando a afectar al 50% de estos pacientes con más de 10 años de evolución. Esta afección es considerada el principal factor de riesgo para el desarrollo de úlceras y por tanto es la responsable del 75% de las amputaciones que se llevan a cabo en personas diabéticas. Para su correcto diagnóstico es fundamental realizar una historia clínica detallada junto con una valoración de la sensibilidad vibratoria y de la sensibilidad protectora con el diapasón graduado de Rydel – Seiffer de 128 Hz y con el monofilamento de 10 gramos respectivamente. Objetivos: Los objetivos principales de este proyecto de estudio son, por un lado, determinar la prevalencia de neuropatía diabética en una muestra de diabéticos tipo 2 del área sanitaria de Ferrol, y por otro, determinar la concordancia entre los dos métodos diagnósticos empleados e incluidos en los protocolos de actuación clínica de la Asociación Americana de diabetes. De forma secundaria, se determinará la relación entre la neuropatía diabética y determinados factores clínicos y sociodemográficos de los participantes. Metodología: Este proyecto de estudio contempla la realización de dos tipos de estudio diferentes. For its correct diagnosis, it is essential to carry out a detailed clinical history along with an assessment of vibratory sensitivity and protective sensitivity with the 128 Hz Rydel-Seiffer graduated tuning fork and with the 10-gram monofilament, respectively. Objectives: The main objectives of this study project are, on the one hand, to determine the prevalence of diabetic neuropathy in a sample of type 2 diabetic patients from the health area of Ferrol, and on the other hand, to determine the concordance between the two diagnostic methods used and included in the clinical protocols of action of the American Diabetes Association. Secondarily, the relationship between diabetic neuropathy and certain clinical and sociodemographic factors of the participants will be determined. Methodology: This study project contemplates the realization of two different types of study. On the one hand, a prospective cross-sectional observational study on the prevalence of diabetic neuropathy and on the other hand, a study on the concordance of diagnostic tests in patients diagnosed with type 2 diabetes mellitus with more than 10 years of evolution under supervision in the Endocrinology and Nutrition Service of the Hospital Complex in Ferrol without previous diagnosis of diabetic neuropathy.

La diabetes es una enfermedad crónica que se desencadena cuando el páncreas no produce sufi ciente insulina o cuando el organismo no la puede utilizar con efi cacia, derivando en serias complicaciones e incrementando el riesgo de muerte. Según el último informe de la OMS se aprecia un marcado aumento de la carga global de diabetes pasando de 180 millones de personas con esta enfermedad en 1980 a 422 millones en 2014, por lo cual esta organización recomienda mejorar la vigilancia, prevención y control de la diabetes y sus complicaciones. De acuerdo con la metodología, se realizó una búsqueda sistemática en Medline. Las palabras clave empleadas fueron: “Neuropatía diabética”, “nefropatía diabética”, “diabetes mellitus”, “enfermedad cardiovascular” y “pie diabético” En la búsqueda inicial fueron seleccionados 218 artículos originales y de revisión publicados en los últimos cinco años de los cuales 83 cumplían los criterios establecidos, su análisis permitió el desarrollo de un artículo de fácil comprensión enfocado en la fi siopatología y diagnóstico clínico de las complicación anteriormente mencionadas que responde a la problemática mundial en la cual la demanda de pacientes diabéticos supera la oferta de especialistas y en donde el médico de atención primeria juega un papel fundamental en el manejo de estos pacientes.
