1. Practice and Reflection Regarding the Development of Featured Brand Community-based Specialty Care: a Case Study of Chu's Traumatology

Abstract

Public healthcare institutions are facing challenges to upgrade the level of services to satisfy gradually increased healthcare needs of residents as the economy develops. To address such challenges, Shanghai Huangpu District Waitan Community Health Center (HDWCHC) has explored a way for the development of featured brand community-based specialty care services, and developed a standardized operation toolkit that can be reproduced and promoted based on a summary of the practice of building Chu's traumatology, the first brand specialty built by the HDWCHC. We reviewed the process and achievements of building Chu's traumatology, hoping to provide methodological guidance for the development of featured brand community-based specialty care services in other areas of China. The brand building of Chu's traumatology includes four stages covering seven parts: standards formulation (diagnostic standards formulation, intervention standards formulation, referral standards formulation) , internal promotion (determination of the inheritor of Chu's traumatology, service team building) , standardized single disease diagnosis and treatment and building the standardized clinic room (expanding to all accessible sites in primary care) and external promotion (building a standardized, community-based operation toolkit featured by the brand of Chu's traumatology) . The exploratory practice of HDWCHC has enabled the development of featured services delivered by a standardized process mode transformed from individual mode (from single-person mode to multi-person cooperation mode, or from single-institution mode to multi-institution cooperation mode) , enhancing the transferability, promotion and reproducibility of featured services as well as bettering their allocation, facilitating the formation of ideas of integrated development of multiple techniques with Chu's traumatology services as the core, thereby improving the effectiveness of healthcare services.

Abstract

Our study aimed to identify the incidence of patients with vancomycin-resistant enterococci (VRE) colonization in the orthopedics and traumatology clinic and to present an approach for these patients. The anal swab samples of 781 patients applying to and inpatient in the orthopedic and traumatology clinic from August 2013-December 2013, taken by the infection control committee under the auspices of the VRE infection surveillance program, were investigated. Of the 781 samples investigated, 14 patients (1.79%) were found to have VRE colonization. In 4 patients there was a history of diabetes mellitus, 5 patients had dirty wounds and 5 patients had repeated inpatient stay and surgery history. Cultures taken from the patients produced E. species and E. casseliflavus/gallinarum. In Turkey, the first vancomycin-resistant E. faecium strain was reported from Akdeniz University in 1998. Since this date, VRE is encountered more and more often in the hospital environment. Resistant bacteria easily propagate in the hospital environment and this increases the possibility of colonization and infection incidence. Early identification of resistant enterococci colonization in inpatients is important for control of enterococcal infections. Abstract

The background and the aim of the work: The Department of Orthopedics and Traumatology of the “Carlo Poma” Hospital (Social Territorial Health Authority of Mantova), has pointed out in 2017, through the questionnaires survey over the citizens satisfaction, an appreciation decrease compared to the previous years. The obtained data were not sufficiently explanatory of the reasons for that kind of deterioration and also not enough specific to define possible corrective measures. The aim of this work was to identify the patients’ perception regarding the hospitalization phases (from booking to follow up), taking into account five kind of operations and pathologies: 1st knee, shoulder and tibio-talar arthroscopy; 2nd hip and knee prosthesis; 3rd upper limb traumatology; 4th lower limb traumatology and 5th orthogeriatrics. Methods: The research is based on 29 narrations resulted from orthopedic patients between 30 and 80 days after the time of discharge. Results: The phases of care path which get the highest level of satisfaction are those concerning the operation and the outpatient visit followed by rehabilitation and assistive continuation. The most negative phase was the discharge but, also the needs assistance respond, the reception, the microclimate and the pre-operative medical assessment resulted contradictory. At the same time the three most significant areas of improvement were: the organization (critical for upper limb traumatology, arthroscopy and prosthetics); the health features (critical for the lower limb, orthogeriatrics and traumatology) and medical information (the most critical issues were those concerning the upper limb traumatology while the less were the orthogeriatrics ones). Conclusion: Use the narration to go into the orthopedic patient needs and perceptions allows to activate appropriate and customized organizational and professional changes in order to answer adequatly to the patient’s needs to limit litigation and defence medicine expences.
