Plexiform Fibrohistiocytic Tumour (PFHT) is an intermediate malignant tumour of soft tissue. It is a rarely metastasising dermal subcutaneous neoplasm. It affects children and young adults. The age range of presentation is from birth to age 77 years. The tumour is most commonly located on the upper extremities, whereas the involvement of the frontal region is rare. The differential diagnosis includes benign fibrous histiocytoma, fibromatosis, giant cell tumour and neurofibroma. Plexiform fibrohistiocytic tumour is a slow-growing mass and which is considered a low grade malignant tumour. Treatment is simple excision. It can exhibit aggressive behaviour and rate of local recurrence is 12-38%. A 45-year-old female was presented with a solid mass on the frontal region of the head which was diagnosed as a plexiform fibrohistiocytic tumour with underlying frontal bone invasion on histopathological examination of an excised mass in the Pathology Department. Plexiform Fibrohistiocytic Tumour (PFHT) is a mesenchymal neoplasm that shows a multinodular growth pattern and which is composed of fibroblasts, histiocyte like cells and multinucleated osteoclast like giant cells. Clinically, it is usually a slow-growing mass with frequent local recurrence and rare regional lymphatic and systemic metastasis.

Background: Giant cell tumour (GCT) is a primary benign, locally aggressive bone tumour. Treatment is surgical however, denosumab has been increasingly used as a treatment in recurrent or unresectable locally advanced GCT because of its effect in reducing tumour size. Denosumab treated GCT can exhibit a wide array of histopathological features resulting in diagnostic problems for pathologists. Our study aimed at identifying these histopathological features to aid pathologists in reaching a correct diagnosis. Methods: Our study included 20 patients of biopsy-proven GCT cases treated with denosumab. We received specimens for histopathological examination in our institution from January 2018 to March 2020. The demographic and clinical data of these patients were retrieved from the Health Management Information System (HMIS). The slides were retrieved and studied for histopathological changes. Results: The mean age of patients in our study was 29.5 years. There were 11 males and 9 females. Distal radius was most commonly involved bone. On histopathological examination, ovoid to spindle mononuclear stromal cells were seen. Total absence of osteoclast like giant cells was seen in 10 (50%) cases, whereas 3 (15%) cases showed a marked reduction in osteoclast like giant cells. Conclusion: Denosumab treated GCT of bone can exhibit a wide spectrum of histopathological features. Pathologists need to be mindful of these features. Correlation of histopathological findings with clinical history and radiological features is important to prevent erroneous diagnosis. Pleomorphic lobular carcinoma (PLC) of the breast with osteoclast like giant cells (OGC) is a very rare variant of lobular carcinoma associated with tumour-associated macrophages (TAM) in the microenvironment. High content of TAM in breast cancer is associated with poor survival rates. Using genomic testing for the individual risk assessment in breast cancers with special features of their microenvironment might be skewed by the fact, that most recommended tests investigate proliferation and oestrogen receptor associated genes of tumour cells in their risk stratification but do not take into consideration the microenvironment. So far, no disease-specific molecular alteration could be identified in NST and lobular invasive carcinoma with OGC.

Anaplastic thyroid carcinoma (ATC) accounts for 5%–10% of primary thyroid malignancies. Cytodiagnosis of osteoclast-like giant cells (OLGCs) variant of ATC is very rare with only few cases reported in literature. We report a case of ATC with OLGC variant in an 85-year-old female who presented with swelling in the right lobe of thyroid and scalp nodule, which on radiological evaluation and subsequent fine-needle aspiration biopsy (FNAB) was diagnosed as ATC having numerous OLGC with metastases in skull bones. Cytologically, thyroid aspirate revealed sheets of epithelioid neoplastic cells admixed with many OLGC. Aspirate from the scalp nodule showed neoplastic cells with follicular differentiation. She also had radiological evidence of lung metastasis. Such a presentation of this rare variant being diagnosed by FNAB is rarely reported in literature and highlights the importance of this simple procedure in diagnosing and planning management of this rare condition. Giant Cell Tumour of Bone (GCTB) is a benign but locally aggressive osteolytic skeletal neoplasm of young adults consisting of giant cells expressing RANK (Receptor Activator of Nuclear Factor-κB) and mesenchymal spindle-like stromal cells expressing RANKL (RANK ligand). The interaction of these cells leads to bone resorption. Recently, the RANKL inhibitor, denosumab, has demonstrated activity against giant-cell tumours. The current article reports a case of a Giant cell tumour of left distal femur with pathological fracture. A 34- year-old male patient presented with history of on and off dull aching pain in the left knee for 4 months followed by a history of trivial fall. He sustained a closed injury in the left knee, following which he was unable to bear weight and developed pain and swelling in left knee. Conventional radiographs and Computerized tomography (CT) was done which showed the presence of a left distal femoral osteolytic lesion and a histological analysis of a biopsy specimen confirmed the diagnosis of GCTB. The patient was treated with neoadjuvant denosumab therapy which resulted in successful downstaging of the tumour followed by extended curettage of the lesion with high speed burr and argon laser cautery. The post-curettage microscopic examination revealed the absence of osteoclast-type giant cells.

Malignant pigmented villonodular synovitis (MPVNS) is a rare lesion whose existence may be debatable. We studied 10 cases of malignant pigmented Villonodular synovitis among the age group of 20-40yrs. Females dominated males in the incidence & wrist was the commonest joint involved presenting with pain & swelling in most of our cases & only 2 paitents had restricted movements. In 3 cases MPVNS was found to be arising from the previous cases of benign PVNS & 7 cases were primary with histological features similar to that of secondary. Most of the MPVNS showed histological features like nodular, solid infiltrative pattern of the large, plump, round or oval cells with deep eosinophilic cytoplasm and indistinct borders, large nuclei with prominent nucleoli, and necrotic areas. Atypical mitoses were frequent about more than 10 /10hpf. 3 cases showed immunoreactivity for CD68, CD163, and vimentin. 3 cases recurred & died of pulmonary metastasis & inguinal lymph node metastasis in a span of 2 yrs. The malignant nature of this lesion, the histologic architecture similar to that of PVNS, and the fibrohistiocytic appearance of the cells suggest that malignant PVNS is an entity. Thus corroborative evidence of clinical, pathological, radiological & immunohistochemical findings are essential in arriving at a definitive diagnosis & treatment of MPVNS. [ABSTRACT FROM AUTHOR]

Undifferentiated carcinoma with osteoclast-like giant cells (UC-OGC) is a rare malignant neoplasm accounting for <1% of pancreatic masses. Very few case reports and small series have described the cytomorphological features of this entity. We report a case of UC-OGC arising in the pancreas presenting with liver metastasis in a 56-year-old man diagnosed by guided fine-needle aspiration cytology (FNAC). A characteristic biphasic pattern comprising of malignant mononuclear cells with scattered bland giant cells were the hallmark features for cytological diagnosis. Mammary carcinoma with osteoclast-like giant cells is rare, and comprises less that 2% of breast carcinoma cases. Herein, we present a case of a 45-year-old woman who underwent breast lumpectomy and sentinel lymph node biopsy for a solitary well defined breast tumor. Histological examination revealed an invasive tumor composed of ducts, small nests and cribriform formations intermixed with a prominent osteoclast like giant cell component. The background stroma is hemorrhagic with conspicuous hemosiderin deposition. The paper will outline the clinico-pathologic characteristic features of this uncommon subtype as well as the current understanding on the pathogenesis of the osteoclast-like giant cells. The invasive carcinoma and the osteoclast-like giant cells staining patterns using immunohistochemical stains for estrogen receptor, progesterone receptor, human epidermal growth factor receptor 2, receptor activator of nuclear-kB, RANK ligand, and matrix metalloproteinase 1 are reported.
