Introduction: This study aimed to explore the preferences of end-stage cancer patients regarding spending their final days at home or in a hospital, focusing on the impact of cancer types and symptoms necessitating palliative care admission during this period.Methods: The study was conducted with 274 patients diagnosed with end-stage cancer and admitted to the palliative service of Dr. İFC City Hospital in Tekirdağ province. Opinions were collected from all patients while they were fully conscious. The relationships between patients' preferences for their place of death and their age, gender, socioeconomic status, duration of stay in palliative care, admission complaints, and diagnoses were examined. A significance level of p lt;0.05 was considered statistically meaningful.Results: Approximately one-third of the participating patients preferred to die at home under home care supervision. It was found that these preferences were not influenced by the patients' age, gender, socioeconomic status, or duration of stay in palliative care but were affected by their cancer diagnoses and symptoms leading to admission. Patients with pancreatic cancer preferred to die in the hospital, while those with prostate and breast cancer were more likely to choose home death. Conclusion: If a system equipped with an effective planned home care program, managed by a multidisciplinary team in palliative care services, can be provided to meet the needs of patients and caregivers when they approach hospitals in their final stages, it could enable the right group of patients to spend their end-of-life at home, in accordance with their preferences.

Introduction: Euthanasia, which allows individuals with an incurable disease to end their lives in an honourable way, is a long-debated practice. It is related to various issues such as culture, philosophy, religion, ethics, public value, and efficient use of health resources. While some advocate for the right of individuals to choose euthanasia, others criticize it on ethical, legal, and religious grounds. Methods: In this study bibliometric analysis was used and the data were obtained from the Web of Science Core Collection database on July 26, 2024. Only English publications and articles were included using keywords such as "euthanasia," "end-of-life decisions," "medical assistance in dying," "physician-assisted death," and "physician-assisted suicide" without time restrictions. VOSviewer software was used to create network and density graphs, and SciMAT software was used to create strategic diagrams.Results: A total of 2,230 studies published between 1980 and 2024 were analyzed. The author with the most publications is Deliens, L, with 86 studies. The majority of studies were published in the USA, Netherlands, and Canada (692, 336, 225 respectively). The most frequently used keywords are euthanasia, palliative care, and ethics. The highest number of publications (149) was in the year 2021. In the strategic diagram for the first period (between 1980 and 2001), the themes of care and ethics were prominent, while for the second period (between 2002 and 2024), the clusters of nurse and shared decision-making were notable. Conclusion: After its legalization in the Netherlands, euthanasia has been accepted and implemented in some countries over the years, with variations in scope. Some countries, like the Netherlands and Belgium, also accept health problems related to old age and mental illness as euthanasia requests. The discussion around euthanasia often revolves around the principles of medical ethics, including autonomy, beneficence, non-maleficence, and justice. Studies have focused on the role of physicians and nurses in the process, as well as end-of-life decision-making by patients. Recently, Canada defined the role of nurses in the euthanasia process, marking a significant development.

Background: It was aimed to evaluate the patients who were admitted to Samsun Training Research Hospital (STRH), palliative care service (PCS) since it was started to be admitted by family-medicine clinic. Materials and Methods: Cross-sectional, retrospective study consisted of all PC-patients admitted to STRH-PCS between 01/12/2019-01/12/2020. Gender, age, occupation, blood type, diagnosis, where we were consulted from, how many days of received, how many hospitalizations were made, number of deaths, reason for hospitalization, requests for imaging, specific interventional procedures etc. was collected with the help of automation-system and analyzed in SPSS. Results: The mean age of the participants in the study was 76.3±12.9 years, and the female ratio was 51.3%. The majority of the people's blood groups were found to be A Rh+ with 36.9%. The average hospitalization period of the individuals was 12.0±10.4 days, and it was determined that the most hospitalizations were made in winter with 46.7%. While 62.8% of the individuals were discharged from the hospital, it was determined that 21.7% of them died. It was determined that nearly half of the patients were hospitalized due to nutritional deficiencies. The primary diagnosis distributions of the individuals Lung-cancer (15.8%), Cerebrovasculer disease (CVD) (14.8%) and Alzheimer’s disease (14.1%), Conclusion: More consultation was requested from the emergency department and intensive care departments, and patients who were asked to have direct radiography as an imaging need died more. This study was conducted to examine the postgraduate theses on palliative care in nursing in Türkiye and to reveal the nursing practices used. The population of the retrospective descriptive study consisted of theses in the field of nursing registered in the National Thesis Center of the Council of Higher Education, while the sample consisted of 55 theses between 2010 and 2022 that met the inclusion criteria among the theses in the field of nursing as a result of the screening made within the scope of PRISMA rules. It was seen that the first thesis on palliative care in nursing was published in 2010. Of the theses obtained, 13 were doctoral (23.6%) and 42 were master's theses (76.4%), 60% were published between 2019 and 2022, 92.8% were conducted with quantitative research methods, 76.5% of these theses used at least one of the descriptive designs and 15.7% were randomized controlled experimental studies. Additionally, it was determined that 36.4% of the theses were sampled by healthcare personnel working in palliative care clinics, and 72.7% did not use a sampling method. Among the interventions implemented by nurses; the most common ones were palliative care training, simulation training/practice, and theory-model based nursing care practice. However, neonatal palliative education, psychoeducation, application of the case management model and face-to-face fan practice were rarely included. In recent years, complementary practices such as ear acupuncture, aromatherapy massage and relaxation exercise, as well as red-white light application, were observed. It should be emphasized that thesis studies in palliative care and nursing have increased in recent years, but mostly at the master's level and descriptive type, the number of doctoral level studies is insufficient, and the need for randomized controlled intervention studies to provide evidence for palliative care nursing should be emphasized. Introduction: This study aimed to evaluate the depression symptoms and quality of sleep of palliative care patients’ family caregivers. Methods: A total of 120 patients were hospitalized at the palliative care center and primary caregivers were included. Depression symptoms of caregivers was assessed by the Beck Depression Inventory (BDI) and quality of sleep with Pittsburgh Sleep Quality Index (PSQI).Results: The mean age of 120 caregivers included in the study was 47.2±12.8 years. The mean BDI score was 10.4±7.2, and the mean PSQI score was 4.6±2.9. Significantly higher rates of depression symptoms were observed in singles than in married people (p=0.037). The healthy sleep rate of the patients whose caregiving period was 6-10 years was significantly lower (p=0.018). While 78.5% of those who received support from their families had a healthy sleep, this rate was 52.4% in those who did not receive support from their families (p=0.034). A significant difference was found between PSQI and BDI categories (p=0.004). People with moderate depressive symptoms were more likely to have poor sleep quality and long-term sleep disorders.Conclusions: Family caregivers of patients receiving palliative care are at risk for poor sleep quality and depression symptoms. We think that the evaluation of these people in terms of the presence of depression symptoms and poor sleep quality may provide early intervention to alleviate the symptoms.

Introduction: We aimed to determine whether there is a difference in care burden, anxiety, and depression between caregivers of patients living at home and those in the palliative care unit.Methods: The study was cross-sectional in the palliative care and home health units. The study population consisted of 1,879 patients. Different from home care, caregivers in the palliative unit were educated by the nurses on patient care during their stay in the hospital. A total of 186 caregivers were enrolled in the study. Unlike those receiving home care, caregivers of the patients in the palliative care unit received training on patient care while they were in the hospital. The care burden, anxiety, and depression levels of the caregivers in both groups were measured using the Zarit Caregiver Burden and Hospital Anxiety and Depression scales.Results: The mean age of the participants was 51.44±12.11 years. The caregivers' mean burden and anxiety scores were higher in palliative care compared to the homecare group. Both burden and anxiety scores were significantly higher in the palliative care group (p lt;0.001 and p =0.031, respectively).Conclusion: One of the distinguishing features of patients in the palliative care unit is the opportunity to provide ongoing medical support, social support, and education to the caregivers. However, the interventions at the palliative unit were not enough to compensate for the difference in the burden of care between the two groups.

Objective: Pressure ulcers (PU) are common in patients admitted to palliative care clinics (PCC). Pregressive nutritional deficiency, incontinence, and limitation of movement increase the risk of new PU formation. Purpose: To examine the risk of PU in patients hospitalized in PCC andthe rate of wounds on admission. Materials and Methods: We retrospectively analyzed patients hospitalized between 2017 and 2019 in a PCC integrated into a tertiary health institution. Patients with demographic and clinical data, data on the region, number, and stage of PU, and Norton and Braden pressure risk scores were enrolled. Patients with scores 11 on the Norton scale and 18 on the Braden scale were considered to be at higher risk for PU. Results: The final analysis included 566 patients [mean age: 68 years, male: 328 (58%)]. On admission, PU was recorded in 181 patients (32%) (sacrum: 80%). According to the Norton scale, 269 (47.5%) of all cases were under the risk of PU and 48.7% (n=131) of them already had PU. According to the Braden scale, 475 (83.9%) patients were at risk of PU, and 37.5% (n=178) of them already had PU. Among subjects with a low Norton score (n=297), 16% (n=50) already had PUs. Among subjects with PUs, the Norton score was not high 27.6%. Only 3.3% (n=3) of 91 patients whose Braden score was not low already had a PU. The sensitivity and specificity of the Norton score in the prediction of PU were 48.9% and 83.2%, respectively. The sensitivity and specificity of the Braden score in the prediction of PU were 66.9% and 99.0%, respectively. Conclusion: Physical PU examination on admission to PCC and during follow-up can identify a significant number of cases. The utility of Norton and Braden scores to estimate the occurrence of PUs may be limited in the palliative care setting or patients. [ABSTRACT FROM AUTHOR]

Objective: The benefit of medical interventions applied in terminal patients who have no hope of recovery and whose death is thought to be approaching is still a controversial issue. In this study, it was aimed to analyze the terminal stage patients hospitalized in the intensive care unit. Materials and Methods: The general characteristics and laboratory results of 61 end-stage patients admitted to the adult intensive care unit of university hospital between December 2016 and may 2017 were examined. Life expectancy, mortality rates, factors affecting mortality, and costs per patient were evaluated according to the palliative prognostic index (PPI) and palliative care admission score (PCAS) of the patients. Results: Exitus patients had an average of 13.83 days of intensive care stay and patients who survived had a mean 30.69 days (p<0.05). The PCAS was not statistically significant between ex and alive patients. Exitus patients had a PPI of 6.48, whereas survivors had a PPI of 3.73, (p<0.05). Exitus patients had a mean palliative life expectancy of 3.78 days, whereas the palliative life expectancy of surviving patients was 71.42 days (p<0.05). The most common primary disease for intensive care admission is malignancy, with 33 patients. The average cost of treatment for exitus patients was $3654,50, while the cost of treatment for surviving patients was $7053,38 (p<0.05). Conclusion: End-stage patients should be admitted to the palliative care unit or hospice using prognostic scoring systems. The bed capacity in intensive care units should be used for patients who have a chance to be treated. It is thought that unnecessary health expenditures can be avoided by hospitalizing terminal patients in palliative care units and hospices. [ABSTRACT FROM AUTHOR]

Introduction: Otolaryngologists are required to possess the knowledge, skills, and confidence necessary to manage consultations requested by palliative care services. However, the inclusion of palliative care needs in otolaryngology residency training is limited. The goal of this study was to enhance awareness of otolaryngologists' palliative care knowledge and skills by evaluating otolaryngology consultations requested by tertiary palliative care services between 2016 and 2021.Methods: This retrospective cross-sectional study included patients who consulted the otolaryngology department from the palliative care service of Kutahya Health Sciences University between January 1, 2016 and September 31, 2021. A total of 80 otolaryngology consultations were requested between 2016 and 2021. Patients were evaluated for age, gender, diagnosis of palliative admission, reason for consultation, date of consultation, interventions, and additional investigations.Results: There were 27 female patients (33.7%) and 53 male patients (66.2%). The mean age was 65.0 (range: 22-97) years. It was found that the volume of consultations did not increase significantly over a five-year period (p=0.502) and 78.8 % of consultations were requested during working hours (08:00-16:00). In addition, the response time (minutes) for consultations during working hours (08:00-16:00) was significantly lower than outside working hours (16:01-07:59) (p lt;0.001). The reasons for requesting a consultation and the procedures performed were significantly different between patients under and over 65 years of age (p=0.019 and p=0.008, respectively).Conclusions: This is the first study to evaluate otolaryngology consultations requested by a tertiary palliative care service. Aim: The aim of this study was to determine the most important barriers and supportive behaviors perceived by intensive care nurses while providing end-of-life care. Material and Methods: The sample of this cross-sectional descriptive study consisted of 278 intensive care unit nurses who have been working at the Ankara City Hospital. Results: The three most highly perceived barriers to providing endof-life care by intensive care unit nurses are: "Families not accepting poor patient prognosis", "Family not with the patient when he/she is dying", "Family not understanding the term life-saving measures and its implications" and the three most highly perceived supportive behaviors to providing end of life care by intensive care unit nurses are: "Physicians meet in person with the family after the patient's death", "Staff compiles all paperwork to be signed by the family before they leave the unit", "Nurses take care of patients while affected nurse gets away for a moment after the death of a patient". Conclusion: The highly perceived barriers to providing end-of-life care by intensive care unit nurses are related to the patient's families, and the highly perceived supportive behaviors are related to healthcare professionals. It is recommended that families should be provided with opportunities (psychological support, etc.) to facilitate their acceptance of the poor prognosis of their patients and that research be conducted to understand the conditions that enable the information provided by physicians and to ensure their continuity. The study was conducted to evaluate the care burden and burnout levels of caregivers for patients hospitalized in palliative care. This descriptive study was conducted with 76 caregivers who were hospitalized in the palliative care services of a university and state hospital between 02.12.2019 and 02.06.2022 in Turkey. After obtaining the permissions of the ethics committee and the institution, the data were collected with the Descriptive Characteristics Form, Palliative Performance Scale, Caregivers Burden Inventory, and Maslach Burnout Inventory. The total score of the Caregivers Burden Inventory was 50.31±15.37, sub-dimensions of timedependency burden, developmental burden, physical burden, social burden, and emotional burden scores were 18.30±3.23, 10.94±6.03, 13.54±5.99, 4.09±4.68, and 3.42±3.89, respectively. The Maslach Burnout Inventory sub-dimensions were emotional exhaustion, depersonalization, and personal accomplishment, with mean scores of 24.78±10.33, 11.59±4.20, and 28.64±5.67, respectively. The mean Palliative Performance Scale score of the patients was 32.76±16.86. As the care burden of caregivers increased, so did their exhaustion scores. Caregivers had moderate care burdens, high time dependency and physical burdens, high emotional exhaustion, and low personal accomplishment burnout. All patients who were cared for were bedridden. The presence of incontinence in the patient affected the time dependency care burden of the caregivers, and the presence of chronic disease in the caregiver and living with the patient also affected the caregivers' emotional exhaustion. These results show that studies must be conducted to develop multidisciplinary interventions to reduce caregivers’ care burden and burnout. [ABSTRACT FROM AUTHOR]

Background/Aims : Urinary tract infections are an important cause of mortality, especially in geriatric patients. The effectiveness of urine culture and appropriate antibiotic use in geriatric patients followed in primary care is unknown. We aimed to compare the urine cultures and antibiotic susceptibility of patients over the age of 65 who were treated at home by the Home Health Services (ESH) and inpatients in the Palliative Care Service (PBS). Methods: Our study retrospectively compared the culture-antibiogram results of urine samples of patients aged 65 years and older with urinary infection complaints and followed and treated by PBS and ESH between January 1, 2019 and January 1, 2020. Results: Of the 60 patients included in the study, 30 had PBS, 30 ES Services patients had a mean age of 78.0±12.7 years, and PBS patients had a mean age of 80.7±9.8 years. According to the culture-antibiogram results of the urine samples of the patients, E.coli was the most common microorganism in both groups (p=0.003). When we look at the antibiotic sensitivity of the patients in the ESH group, Amikacin (96.7%), Cefoxitin (83.3%), Fosfomycin (73.3%), Nitrofurantoin (70%) sensitive and Ampicillin (76.7%) Cefuroxime (36%) ,7) Ceftazidime(40%) was found resistant to Ciprofloxacin(33%). PBS group is sensitive to Amikacin(60%), Cefoxitin(23.3), Fosfomycin(23.3%), Nitrofurantoin(16.6%) and Ampicillin(40%), Cefuroxime(26.7%), Ceftazidime( 33.3% were found to be resistant to Ciprofloxacin(53.3%). In both groups, the highest resistance rates were found in Ampicillin, 76.7% in ESH patients and 40% in PBS patients, and the highest sensitivity rates were in Amikacin, 96.7% in ESR and 60% in PBS. Conclusion: In this study conducted over the age of 65, empirically in ESR in urinary system infections; on the other hand, it would be rational to use oral Fosfomycin or Nitrofurantoin or Amikacin; It is thought that due to high antibiotic resistance in PBS, caution should be exercised.

Palliative psychiatry is an approach that aims to improve the quality of life of patients and their families by trying to prevent or alleviate suffering through timely evaluation and treatment when faced with physical, mental, social, and spiritual problems associated with serious life-threatening mental illnesses. The fact that individuals with serious mental illness are a disadvantaged group in accessing health services, the rate of physical diseases that are difficult to treat compared to the general population is quite high in this population, their physical diseases are diagnosed later and their compliance with treatment is more difficult, this special group continues to live with a low quality of life and it results in death at an earlier age of 15-20 years. Therefore, considering the burden of serious mental illness on both the family and society, it is clear that palliative care is needed. However, the need for palliative care for individuals with psychiatric illnesses remained in the background, and discussion of the need for palliative care became possible at the beginning of the 21. century. This study was written to raise awareness about where we are in benefiting from palliative care services in our country and in the world and to draw attention to the role of psychiatric nurses, whose role in patient advocacy is at the forefront, in psychiatric palliative care. Medicine has several goals, including preventing and curing diseases, relieving pain and suffering, and promoting and maintaining the mental and physical health of the person from birth to death. Thanks to medical and technological developments in the last century, medicine has started to provide the opportunity to live longer. However, whether a longer life leads to a better life is debatable. Before these developments, death was regarded as a natural and inevitable end and mostly handled with religious approaches, such as Ars moriendi. However, these medical and technological advances have caused a misleading perception that medicine has the power to achieve every desired outcome. Under the influence and pressure of this misleading situation, people began to demand aggressive treatments, like resuscitation, mechanical ventilation, artificial nutrition and hydration, and chemotherapy. Of course, these medical interventions can be lifesaving for some patients. However, in the case of incurable conditions, such as metastatic cancers, Alzheimer's disease, or advanced organ failure, aggressive treatment methods often fail to produce promising results; on the contrary, they place a variety of burdens on patients, families, and society, as significantly reducing patients' quality of life, causing unrealistic hopes, and exhausting limited health resources. In this context, the article aims to briefly examine the change in people's perception of death and the dying process and to evaluate its de facto and ethical impacts. This study shows that medical and technological breakthroughs have significantly changed people's comprehension, acceptance, and perception of death, which made death no longer a natural end, but a medical failure. In other words, a medical interventions-based death, called medicalized death, has replaced a religion-based death. As a result of this change, the social, spiritual, and emotional aspects of death and the dying process are ignored for the sake of excessive and ineffective medical interventions. This situation brings about a critical shift in where, how, and when to die, and produces various ethical problems regarding the principles of respect for autonomy, non-maleficence, and justice. The present article demonstrates that most people want to die at home and spend their last days with their families. However, due to medical interventions, many die in healthcare facilities while on medical treatments and machines. In this context, this study recommends home-based palliative care services encompassing social, emotional, and spiritual support instead of aggressive medical interventions at end-of-life. Home-based palliative care will enable individuals to spend the last days of their lives at home with their loved ones, receive emotional and spiritual support, and prevent themselves, their families, and society from unnecessary and futile medical expenses. In conclusion, in light of the principles of respect for autonomy, non-maleficence, and justice, in end-of-life issues, medicine should focus on improving patients' quality of life, providing patients with social, emotional, and spiritual support, and ensuring that patients have the opportunity to spend their last days of life with their family members, which can most suitable be achieved through home-based palliative care. Palliative care is a specialty that aims to prevent and reduce the distress of patients and their families who encounter life-threatening diseases. Especially, it aims to eliminate the problems caused by the cancer itself or the treatment methods. Patients diagnosed as having cancer experience many problems such as nausea, vomiting, anxiety, depression and sleep disturbance due to the disease and its treatment. Patients can search for complementary treatment methods such as aromatherapy in order to reduce or eliminate these symptoms and increase their well-being and quality of life. Aromatherapy is defined as the use of essential oils obtained from plants for therapeutic purposes. Essential oils used in aromatherapy are applied in the form of massage, bath or inhalation. Background: The blood type is known to be associated with the risk of cancer. This study aimed to evaluate the relationship between oncological diagnoses and blood groups of inpatients receiving palliative care. Material and methods: This retrospective study was conducted with patients receiving palliative care at our hospital between December 2019 and December 2020. The records of patients receiving comprehensive palliative care during this 1-year period were obtained from the hospital archive, and patients diagnosed with a malignant disease were included in the study. Results: Overall, 176 patients were included in the study. No association was found between the cancer type and ABO/Rh blood groups. The incidence of central nervous system (CNS) and breast cancers was lower in patients with blood group A than in those with other blood groups (p = 0.028). Moreover, the focus and site of metastasis and the ABO/Rh blood group were not related. In addition, the comparisons between blood groups in terms of the presence and site of metastasis revealed no significant relationship. Conclusions: Patients with blood group A had a lower incidence of CNS and breast cancers than patients with other blood groups, and there was no relation between metastases status and ABO/Rh blood groups. Aim: Palliative medicine provides holistic care to increase the quality of life of patients. Predicting prognosis is critical for a personalized treatment plan. Therefore, we aimed to investigate the survival prediction properties of routine biochemistry tests, complete blood count (CBC) and neutrophil/lymphocyte ratios, in addition to biomarker-based indices (the mGPS, PI, and PNI). Materials and methods: The laboratory parameter values, prognostic factor scores, diagnoses and survival time of 139 palliative care patients in the last 5 weeks of their life were evaluated retrospectively. Results: Ninety-one (65.5%) patients were male and the average age was 65.9 years (28–91 years). Results of all three prognostic indices (the mGPS, PI, and PNI) investigated in this study were found to have a statistically significant relationship with the survival time of our patients. In addition, NLR, neutrophil percent, hemoglobin, serum albumin and CRP values were seen to have a statistically significant relationship with the scores of all three prognostic indices. Moreover, hand it is detected that WBC, NLR, albumin, and CRP values were associated with overall survival. Conclusion: The results of our study, which showed that these indices have prognostic markers in palliative care patients, will guide the management of the clinical process. We would like to emphasize the importance of palliative care clinicians in developing personalized treatment plans for patients using prognostic tools for survival prediction.

The foreign body aspiration into the respiratory system is an emergency situation with a high mortality rate, when diagnosis and treatment are delayed which can cause complications and repeated hospitalizations. Although it is especially common in children under the age of five, it can be seen at any age in chronically ill children with neurological problems. However, it is rarely seen in pediatric patients with tracheostomy and dependent on a home mechanical ventilator due to their nutritional style and severe neurological retardation. In this study, foreign body aspiration that developed in a pediatric patient with tracheostomy who was followed up with a home mechanical ventilator and fed from gastrostomy was presented.

Introduction: Determining the awareness of family physicians working in primary care about palliative care, to determine their attitudes and behaviors towards patients and their relatives, and their approaches are aimed in this study.Methods: The universe of the cross-sectional and descriptive study consists of 194 family physicians in Tokat in January 2020. However 23 physicians who refuse to participate and 15 physicians who could not reached in detected dates were excluded from the study. 156 physicians were applied prepared questionnaire. The necessary permissions were obtained from the Provincial Health Directorate and the clinical research ethics committee. The data were evaluated with IBM Statistics 20.0 SPSS statistical software. Chi-square test was used to test the differences, and p lt;0.05 was accepted statistically significant.Results: Regarding participants 98 (62,8%)were male, 58 (37,2 %) female, mean age 40,5±9,8 and mean working years 15,8. 6.4% (n: 10) of the participants were family medicine specialists and 93.6% (n: 146) general practitioners. The proportion of respondents who state the correct answer as ‘palliative care deals with life-threatening diseases’ was 60.3%. The proportion of those who helped palliative care patients about nutrition was 29.5% (n: 46) and 60 physicians supported for psychiatric problems (38.5%). 51.3% of the physicians prescribed opioid group drugs. The rate of those who want to follow patients in the family health center as integrated with the palliative care units 20.5%, and this rate was found to be statistically significantly higher in female physicians (p: 0.03). 11.5% of the physicians received palliative care training, while 61.5% stated that they wanted to receive regular training in this regard.26.9 % of participants want to specialize in palliative care. 74.3% of physicians think that palliative care practices should not be included into performance system in the family medicine practices.Conclusions: In the current system, it was found that family physicians did not have sufficient training and equipment related to palliative care. Introduction: Early palliative care interventions and structured advance care planning can improve symptom control and quality of life in patients with a brain tumor. In this study, we aimed to investigate symptoms, hospital discharge conditions, and the length of stay of patients with primary and metastatic brain tumors in our palliative care center (PCC). Material and Method: Ninety-one patients; who had been followed-up in the PCC with the diagnosis of a primary or metastatic brain tumor were included in this retrospective study. Demographic characteristics, Glasgow Coma Scale (GCS) scores, Karnofsky Performance Scale (KPS) scores, hospital discharge status, and symptoms of the patients included in the study were compared. Results: The mean age of the patients was 60.67 years; 59.3% were male, and the mean length of hospital stay was 29.26 days. The rates of PEG, tracheostomy, seizures, and paresis symptoms were significantly higher in patients with a primary brain tumor (p lt;0.05). The rates of death in the hospital were significantly higher in patients with metastatic tumors and low GCS scores (p=0.032 and p=0.00, respectively). Conclusion: We observed differences in clinical findings and prognoses between primary and metastatic brain tumor patients during the follow-up in PCC. Further to advances in treatment methods, we believe that identifying the need for palliative care and appropriate symptom management will improve the quality of life in brain tumor patients with poor prognosis.

Objective: It is important to evaluate the nutrition of patients in palliative care centers. Most patients are unrecognized and lacking treatment, as there is no consensus on ways to scan, diagnose, treat and follow the malnutrition, and the lack of adequate awareness and training of healthcare professionals. This study aims to evaluate the approaches and awareness of healthcare professionals in Turkey who support nutrition in Palliative Care Centers. Methods: A survey, prepared by researchers, conducted to assess the effectiveness of healthcare professionals in patient nutrition, was conducted on a virtual basis in all healthcare professionals who were fully employed, accessible and volunteered with palliative care patients. The data was evaluated through descriptive and analyzing statistical methods. Results: The average age of the 105 medical professionals who participated in our study was 36.4±9.3 (25-59), of whom 68 (64.8%) were women and 37 (35.2%) were men. Sixty-seven (63.8%) of the participants, most of whom were physicians, were specialists, 30 (28.6%) were assistants and general practitioners, and 8 (7.6%) were nurses. When asked how many of their patients they started feding products, 38 people (36.2%) said, with 41-60%. If you think you've got enough recognition of nutritional content, there were 28 people who said yes (26.7%), 30 people who said no (28.6%), and 47 people (44.8%) who said sometimes. When asked what they considered the most when planning a feeding product, 95 people (90.5%) said that they cared about the patient's request, compared to the calories and content of the product. Conclusions: Our study found that participants were largely absent from knowledge and awareness, and observed differences in patient attitudes. This systematic review was conducted to demonstrate the effectiveness of the use of reiki therapy in cancer pain management and to draw attention to the availability of this application in palliative care units. The studies on cancer pain management, both in English and Turkish, of which full text versions were accessible, were systematically analyzed, and the studies without full text and ongoing studies were not included. The study was conducted by reviewing Cumulative Index to Nursing and Allied Health Literature, EBSCOhost, MEDLINE, ScienceDirect, Ovid, ProQuest, Web of Science, and ULAKBİM National DataBases. There were no year restrictions in screening. Keywords such as "cancer pain", "cancer pain management", "reiki" and "palliative care" were used to access the studies. Nineteen research papers were accessed and 5 of them were found to be compatible with the inclusion criteria of our study. Cohrane was established as the guideline to determine levels of evidence. Of the included studies, one was a randomized controlled study, three were experimental studies, the other one was a systematic review. In our day, randomized controlled studies examining the effect of reiki therapy on pain in cancer patients who receive palliative care are limited. In particular, studies have shown that reiki is even more effective in reducing pain when used in addition to pharmacological treatment and conventional nursing care. It is observed that the studies are carried out with a small sample number and generally with a single group. Pain assessments are usually performed with visual scale and numeric scale, reiki sessions are performed in the range of 10-90 minutes and 2-6 sessions per week. When we examine the effect of reiki on physical and psychological recovery in cancer patients treated in palliative care, it is reported that reiki has a positive effect on reducing pain. However, there are gaps in matters such as which hand position to use in reiki therapy, the duration and frequency of the reiki session, and the application period. Introduction: Palliative Mobile Health Unit started to serve in in February 2019 at University of Health Sciences Samsun Training and Research Hospital. In our study, it is aimed to evaluate the symptoms and ongoing status of the registered patients by mobile health services of palliative care unit.Methods: The population of this descriptive, cross-sectional study consists of individuals who are registered in Samsun Training and Research Hospital's palliative mobile unit and discharged from the palliative service. In our study, all patients enrolled in mobile services were evaluated. The data were obtained from computer records. Pearson chi-square and Mann-Whitney U tests were used to evaluate the data. p lt;0.05 was considered statistically significant.Results: Since the introduction of palliative mobile services for discharged patients in our hospital, 112 patients have been reached. At the time of this study, 46.42% of these patients are still provided with palliative mobile service. The average age of the participants is 65.60 ± 16.34 years, and 54.35% of the patients are women. 64.20% of the patients are fully bedridden. The patients we see most frequently (64.24%) are patients with a diagnosis of oncology. Foley catheter is the most performed procedure as an invasive procedure.Conclusion: Our patients enrolled in the palliative mobile unit were mostly bedridden and oncology patients. With palliative mobile services, it is aimed to improve the quality of life of patients who encounter problems arising from life-threatening diseases after discharge and to evaluate symptom management. It may be appropriate to disseminate palliative mobile services across the country by evaluating the results.

Kanser, önemli bir halk sağlığı problemidir. Kanser türleri ve kanser nedeniyle tedavi gören birey sayısı her geçen gün artmaktadır. Sayısı artan kanser türlerinin büyük bir bölümünü çocukluk çağı kanserleri oluşturmaktadır. Çocuklarda görülen kanserlerin büyüme ve gelişme, psikososyal durum gibi birçok alanda olumsuz etkilerinin olduğu bilinmektedir. Çoğu zaman ölümle eşdeğer görülen bu durum çocuk ve ailesi üzerinde birtakım zorlayıcı etkiler meydana getirebilmektedir. Tedavi sürecinin uzun olması, tekrar oranlarının yüksekliği ve meydana gelen fiziksel ve ruhsal değişiklikler tedavi gören çocukların baş etmesi güç birtakım etkiler arasındadır. Kanser tedavisi süresince hastalara ve ailelerine mümkün olan en iyi yaşam kalitesini sağlamak amacıyla fiziksel ve psikolojik tüm semptomların erken teşhisi ve tedavisini amaçlayan palyatif bakım uygulamaları başarılı bir şekilde yıllardır uygulanmaktadır. Palyatif bakım alan çocuklarda fiziksel olumsuzlukların giderilmesine ek olarak ruhsal destek uygulamalarının da etkili olduğu bilinmektedir. Pediatrik onkolojide hastalık sürecine ve tedavi yöntemlerine hem aileyi hem de çocuğu adapte etmek, oluşabilecek komplikasyonlar hakkında bilgilendirmek hastalık süreçlerini yönetmeyi kolaylaştırabilmektedir. Bu derlemede pediatrik onkolojide palyatif bakım alan çocuklara psikolojik destek uygulamalarının etkilerinin literatür kapsamında incelenmesi amaçlanmış ve hemşirelik uygulamalarına yansımaları üzerinde durulmuştur.

Aim: This study aimed to evaluate the effect of education given to nursing students on their palliative care opinion, knowledge, and attitudes. Material and Method: The study used a single group pre-test/posttest method. The study was carried out between 11--26 December 2017 with the participation of second-grade nursing students enrolled at a university. The study sample consisted of 105 students before the training and 98 students after the training. The study data were collected using a palliative care knowledge, opinion form, and the Frommelt Attitudes Toward Care of the Dying Scale (FATCOD). In the analysis of the data, McNemar chi-square for dependent groups, t-test, and Pearson's correlation test were used. Results: The mean age of the students was 19.71±1.01 years. The percentage of the students who had clinical practice experience in the palliative care unit during their education was 11.4%, while 32.4% were determined to have already been given palliative care. Before and after the training, the students stated that the place of effective/quality palliative care service was mostly palliative care units. The students reported that the most challenging symptoms in palliative care management would be delirium, pain, dyspnea, and fatigue. The proportion of the students who wanted to work in palliative care after graduation decreased after the training (p>0.05). The students' mean pre-training palliative care knowledge score was 9.95±2.00, and it was found to increase to 12.29±2.42 significantly after the training (t/p=-7.881/0.000). The mean FATCOD score of the students who wanted to work in palliative care after graduation (114.66±8.76) was determined to be higher compared to the students who did not want to (108.24±8.88) (t=3.468, p=0.001). In addition, there was a positive but weak correlation between students' mean post-training palliative care knowledge score and their mean FATCOD score (r=0.285, p=0.004). Conclusion: The education given to the students about palliative care was found to create positive differences in their knowledge and attitudes. Amaç: Araştırma, hemşirelerinin ve palyatif bakım hakkındaki bilgi düzeylerini ve iyi ölüme yönelik algılarını belirlemek amacıyla yapıldı. Yöntem: Tanımlayıcı ve kesitsel tipte gerçekleştirilen araştırma, Şubat - Ağustos 2018 tarihleri arasında, İstanbul’da bir kamu hastanesinde çalışan 237 hemşire ile yürütüldü. Veriler, Tanıtım Formu, Palyatif Bakım Bilgi Testi ve İyi Ölüm Ölçeği kullanılarak toplandı. Verilerin analizinde, ortalama ve yüzdelik dağılım kullanıldı. Bulgular: Çalışmaya katılan hemşirelerin yaş ortalaması 29.52±6.42 yıl olup; çoğunluğunun kadın (%78.5), lisans mezunu (%70) ve çalışma yılı ortalamasının 7.57±7.45 yıl olduğu belirlendi. Hemşirelerin %83.1’inin palyatif bakım hakkında bilgisi olduğu ve %66’sının bu bilgileri lisans eğitimi sırasında aldıkları saptandı. Hemşirelerin Palyatif Bakım Bilgi Testi genel puan ortalaması 10.37±1.79 ve İyi Ölüm Ölçeği puan ortalaması 54.65±8.03 olarak bulundu. Sonuç: Hemşirelerin palyatif bakım bilgi düzeylerinin orta ve iyi ölüme yönelik algılarının yüksek olduğu belirlendi.

Objective: Physicians’ approaches are important in refering patients to palliative care centers (PCC). We aimed to evaluate the perceptions and attitudes of general practitioners and anesthesiologist on patient referral. Methods: Anesthesiologist and general practitioners were included in a face to face or telephone interview method survey; predefined written forms were used to collect data and results were analyzed. Results: The research was completed with 207 participants. Groups were different regarding their knowledge about PC, most of the general practitioners responded “I know a litle” and anesthesiologists responded “I know what it is, I can explain it to someone else” (p=0.002). The answers to the question of which patients should be referred to PCC (“End-stage cancer patients” [p=0.018], “Patients referred home care services” [p=0.003] and “Patients having chronic ilness without adequate symptom control” [p=0.004], “Patients having total parenteral nutrition” [p=0.001], “Precence of pressure ulcer” [p<0.001], “Patients refusing discharge” [p=0.037] ) were different between anesthesiologists and general practitioners. Conclusion: Anesthesiologists and general practitioners refer patients with different diagnoses and conditions to PCC and knowledge about patient referral to PC is different. [ABSTRACT FROM AUTHOR]
