2. Paraplegia Following Type A Aortic Dissection After Open Cardiac Surgery: An Uncommon Complication.

Type A aortic dissection is an infrequent occurrence following open-cardiac surgery, and the development of paraplegia as a complication is even rarer. This case report presents a unique instance of Type A aortic dissection in a patient who experienced paraplegia subsequent to undergoing coronary artery bypass grafting and mitral valve replacement. The causes of paraplegia after aortic dissection are diverse, with cerebrovascular diseases and spinal cord ischemia being uncommon contributors. Despite its rarity, paraplegia in this context carries substantial mortality and morbidity risks. This case emphasizes the importance of considering such complications in the postoperative management of cardiac surgery patients, highlighting the need for vigilance and early intervention. 4. Effectiveness of Locomat Therapy in Neurological Rehabilitation.

Objective: To retrospectively analyze the demographic characteristics of patients receiving robotic rehabilitation treatment and examine various parameters such as walking distance, speed, and time before and after treatment. Material and Method: Between 2019 and 2021, 136 patients who received robotic rehabilitation therapy were analyzed. 136 patients were divided into 3 groups-hemiplegia, cerebral palsy, and spinal cord injury groups. Walking distance, speed and time recorded in the Lokomat device on the first and last day of treatment were recorded. Results: Of the 136 patients, 62 were male (45.5%) and 74 (54.5%) were female. The number of patients treated with hemiplegia, cerebral palsy and spinal cord injury was 46 (33.8%), 30 (22%) and 60 (44.1%), respectively. In all the three diagnostic groups, a significant increase was observed in the walking time, distance, and speed recorded in the Lokomat device on the first and last day of treatment (p <0.001). No significant difference was observed between the groups in terms of walking parameters (p >0.05). Conclusion: We believe that this treatment approach is effective in the neurological rehabilitation group [ABSTRACT FROM AUTHOR]

Pediatrik çağda spinal tutulum gösteren kitlelerin çoğunluğunu sarkomlar oluşturmaktadır. Granülositik sarkom, immatür granülositlerin oluşturduğu nadir rastlanan bir tümördür. En sık akut myelositer löseminin seyri sırasında ortaya çıkmakta olup, bazen de ilk bulgusu granülositik sarkom olabilmektedir. Akut gelişen paraparezi nedeniyle acil kliniğimize başvuran 16 yaşındaki erkek olgunun yapılan tetkikleri sonucunda torakal 3-7 vertebraları arasında ekstradural spinal kitle saptanması üzerine olgu, acil olarak ameliyat edildi. Patoloji sonucu granülositik sarkom olarak geldi. Yapılan kemik iliği muayenesi ve flow sitometri sonucu, akut myelositer lösemi tanısı kondu.

Spontan mesane rupturu nadir gorulen klinik bir durumdur ve genellikle uriner retansiyon, uriner sistem enfeksiyonları, mesane ici basıncı arttıran sebepler, alkol intoksikasyonu sonrasında mesane duvarındaki zayıflama sebebiyle oluşurlar. Daha nadir olarak foley kateter erozyonu, inflamasyon (eozinofilik sistit, interstisyel sistit), dev mesane taşları, intraarteriyel kemoterapi ya da aterosklerotik emboli sonrasında perforasyon gorulebilir. Norojenik mesaneye bağlı olgularda literaturde yer almaktadır. Bu yazımızda sepsis kliniği ile başvuran, yoğun bakıma alınarak takip edilen ve yapılan laparotomide spontan mesane perforasyonu tanısı alan, paraplejik hastayı sunmak ve deneyimimizi paylaşmak istedik. Glucocorticoid induced osteoporosis can cause vertebral fractures. However, neurological complications such as spinal cord compression are rare. Here, we report a case study of a 55-year-old asthmatic male patient, who was on high doses of oral as well as inhaled steroids for uncontrolled bronchial asthma, was presented to the emergency room with paraplegia and urinary incontinence for a week. He showed the clinical features of cord compression with a sensory level at T10. His workup revealed a fracture of vertebrae T6, T8, T12 and L2 with cord contusion and edema at multiple levels on MRI. Osteomalacia was present as well and DXA scan revealed the lowest individual T-score of -3.4 at the lumbar spine. Surgical intervention was deferred due to fragile nature of the fractured spine and patient was treated with teriparatide (recombinant PTH), oral steroid dose was gradually reduced, and supportive care was given. Amaç: D vitamini eksikliği dünya çapında büyüyen bir problem olduğundan, omurilik yaralanmalı (OY) hastalarda da yetersiz D vitamini seviyeleri bildirilmiştir. D vitamini düzeylerinin, OY hastalarda fonksiyonel durumun dolaylı bir göstergesi olabileceği belirtilmiştir. Bu çalışmanın amacı, subakut dönemdeki OY hastalarda D vitamini düzeyleri ile mobilizasyon, fonksiyonel durum, el kavrama gücü ve solunum fonksiyon testi (SFT) parametreleri arasındaki ilişkiyi araştırmaktır. Yöntemler: Subakut motor komplet torakal (T) ve lomber OY 58 hasta [46 erkek, 12 kadın; yaş ortalaması 32.0 (11.2) yıl] çalışmaya dahil edildi. Herhangi bir ortostatik semptom olmadan tilt masası ile vertikalizasyon programını tamamlama süresi “mobilizasyon zamanı” olarak değerlendirildi. El kavrama gücü dinamometre ile, SFT spirometre ile, fonksiyonel durum ise Fonksiyonel Bağımsızlık Ölçeği (FBÖ) ile değerlendirildi. D vitamini düzeylerini belirlemek için 25-hidroksi-vitamin D [25(OH)D] ölçümü yapıldı ve 20ng/mL’nin altındaki değerler eksiklik olarak değerlendirildi. Hastalar 25(OH)D seviyeleri 20 ng/mL olan ve 25(OH)D seviyeleri ?20 ng/mL olan olmak üzere iki gruba ayrıldırlar. Demografik özellikler, mobilizasyon, SFT, el kavrama gücü ve FBÖ skorlarları 25(OH)D düzeylerine göre karşılaştırıldı. Hastalar nörolojik seviyelerine göre de farklı iki gruba ayrıldılar: Seviye T6-T10 arası ve seviye T11 ve altı. Sonrasında 25(OH)D düzeylerine göre grup içi karşılaştırmalar yapıldı. Bulgular: Hastaların ortalama 25(OH)D düzeyleri 19.8 (8.3) ng/mL olarak bulundu. Tüm hastalar değerlendirildiğinde, 25(OH)D eksikliği olan grupta diğer gruba göre “mobilizasyon zamanı” daha uzundu ve FBÖ skorları daha düşüktü (sırasıyla P0,001 and P=0,038). Hastalar nörolojik düzeylerine göre ayrı ayrı değerlendirildiğinde; hem lezyon seviyesi T6-T10 arası olan grupta hem de lezyon seviyesi T11 ve altı olan grupta; 25(OH)D eksikliği olan hastalarda “mobilizasyon zamanı” daha uzundu (sırasıyla P0,001 and P=0,009). Hastalar n, Aim: Since vitamin D deficiency is a growing problem worldwide, insufficient levels of vitamin D were reported in patients with spinal cord injury (SCI). It was stated that levels of vitamin D may be an indirect indicator of functional status in patients with SCI. The aim of this study was to investigate the relationship between vitamin D levels and mobilization, functionality, grip strength, and pulmonary function test parameters in patients with subacute SCI. Methods: Fifty-eight patients with subacute motor complete thoracic (T) and lumbar SCI injuries [46 males, 12 females; mean age 32.0 (11.2) years] were included. The time to complete the verticalization program on the tilt table without any orthostatic symptoms was considered as the ‘time of mobilization.’ Grip strength (GS) was measured using a dynamometer, pulmonary function test (PFT) parameters were measured using a spirometer, and functional status was measured using the Functional Independence Measure (FIM). For determining the levels of vitamin D, 25-hydroxy-vitamin D [25(OH)D] was measured and levels below 20 ng/mL were considered as deficiency. The patients were divided into two groups according to 25(OH)D levels, 20 ng/mL and ?20 ng/mL. The demographic features, mobilization, PFT, GS, and FIM scores were compared according to the levels of 25(OH)D. The patients were divided into two other groups according to neurologic levels: levels between T6-10 and levels T11 and below, and then intragroup comparisons according to the levels of 25(OH)D were performed. Results: The mean 25(OH)D level of the patients was found as 19.8 (8.3) ng/mL. When all patients were evaluated, time of mobilization was longer and FIM scores were lower in the 25(OH)D deficient group than in the other group (P0.001 and P=0.038, respectively). Aim: Since vitamin D deficiency is a growing problem worldwide, insufficient levels of vitamin D were reported in patients with spinal cord injury (SCI). It was stated that levels of vitamin D may be an indirect indicator of functional status in patients with SCI. The aim of this study was to investigate the relationship between vitamin D levels and mobilization, functionality, grip strength, and pulmonary function test parameters in patients with subacute SCI.Methods: Fifty-eight patients with subacute motor complete thoracic (T) and lumbar SCI injuries [46 males, 12 females; mean age 32.0 (11.2) years] were included. The time to complete the verticalization program on the tilt table without any orthostatic symptoms was considered as the ‘time of mobilization.’ Grip strength (GS) was measured using a dynamometer, pulmonary function test (PFT) parameters were measured using a spirometer, and functional status was measured using the Functional Independence Measure (FIM). For determining the levels of vitamin D, 25-hydroxy-vitamin D [25(OH)D] was measured and levels below 20 ng/mL were considered as deficiency. The patients were divided into two groups according to 25(OH)D levels, Amaç: D vitamini eksikliği dünya çapında büyüyen bir problem olduğundan, omurilik yaralanmalı (OY) hastalarda da yetersiz D vitamini seviyeleri bildirilmiştir. D vitamini düzeylerinin, OY hastalarda fonksiyonel durumun dolaylı bir göstergesi olabileceği belirtilmiştir. Bu çalışmanın amacı, subakut dönemdeki OY hastalarda D vitamini düzeyleri ile mobilizasyon, fonksiyonel durum, el kavrama gücü ve solunum fonksiyon testi (SFT) parametreleri arasındaki ilişkiyi araştırmaktır.Yöntemler: Subakut motor komplet torakal (T) ve lomber OY 58 hasta [46 erkek, 12 kadın; yaş ortalaması 32.0 (11.2) yıl] çalışmaya dahil edildi. Leriche's syndrome represents an arterial occlusion of the aortic bifurcation with ischaemic symptoms of both legs. A case of Leriche's syndrome as a complication of end stage renal failure has not yet been reported in the litarature. A 50-year-old woman was taken into the emergency service with severe abdominal and leg pain, loss of movement of both legs, and dyspnoea. On physical examination, there was abdominal tenderness, coldness and pallor of both legs, and pulselessness of the upper and lower extremities. No flow was seen in either femoral artery with superficial ultrasonography probe in the emergency department. Dynamic thorax and abdominal computed tomography was planned and total occlusion of the infrarenal level of the abdominal aorta was detected. Within the first hour of the diagnosis of acute thrombosis of the aorta bifurcation, bilateral femoral embolectomy was performed by the Cardiavascular Surgery Clinic. After 1 week, the paitent was discharged without any complication. Chronic renal disease patients, especially those referred to the emergency department with acute complaints, must be evaluated carefully and quickly, primarily for cardiovascular diseases. Principally, infrarenal aortic occlusion must be considered and femoral pulses must be examined in the differential diagnosis for patients presenting with paraplegia and severe pain. The aim of this report describes the identification of an interesting spinal cord lesion developing in a 2 years old, male, hybrid dog due to being injured by gunshot. In the direct radiography of the patient diagnosed with clinic paraplegia was detected a bullet in the 9th-10th intercostal space at a distance of 2-3 cm away from the corpus vertebrae. While no abnormal findings were determined from direct and contrast radiography (myelography), through MRI, an area with a lesion at the T9-T10 level was identified on the spinal cord. At necropsy of the dog dying on the 10th day of case, a damaged area of 0.5 cm compatible with MRI findings was determined while examining the injured spinal cord. Besides, myelomalacia on spinal cord was recognized during histopathology evolution. As a result, this case report is presented so that it would be beneficial in the identification of both diagnosis and prognosis of the disease in similar cases our colleagues may come across. Paraparesia, tetraplegia and radiculopathy caused by direct compression on spinal cord or ischemic changes are observed in approximately 1% of all hydatid cyst cases. A 13 year-old female patient who had been operated for lung hydatid cyst nearly 2.5 years earlier in another medical facility applied to our out-patient department with back pain, weakness in both lower extremities, paraplegia especially in the left lower extremity and paraparesia in the right lower extremity that had started 20 days earlier. During radiological examination, the reason of paraplegia was detected as extradural intraspinal hydatid cyst causing spinal cord compression and the cyst was removed surgically. The said patient, whose neurological symptoms were observed to improve during postoperative early term, is being reported here along with literature reference. [ABSTRACT FROM AUTHOR]

In a patient operated for left femoropopliteal bypass under epidural anesthesia, 3 ml of 3% glutaraldehyde solution was administered through the epidural catheter at the postoperative sixth hour accidentally. Following glutaraldehyde administration, the patient developed paraplegia in addition to systemic symptoms such as hypotension, tachycardia, nausea and vomiting. 27. Tüberküloz Menenjite Bağlı Parapleji Olgusu.

- Abstract
Giriş: Tüberküloz gerek ülkemizde gerek dünyada morbidite ve mortalite açısından önemli bir sorun olma özelliğini korumakta olup tüberküloz menenjit (TBM) tüberküloz tipleri arasında en ağır seyreden formdur. Olgumuzu TBM’nin nadir gelişen komplikasyonlarından olan paravertebral apsenin önemini vurgulamak amacıyla sunmaktayız. Olgu: Bilinen kronik hastalığı olmayan 30 yaşında kadın hasta, yaklaşık bir aydır olan son 15 gündür şiddetlenen baş ağrısı ve iki gündür olan bilinç bulanıklığı şikayetleriyle acil servise başvurdu. Hastanın ateşi 37,6 ˚C idi, diğer vitalleri stabildi. Fizik muayenesinde ense sertliği mevcuttu. Hastaya beyin BT ve difüzyon MR çekildi. Patoloji saptanmadı. Hastaya tarafımızca lomber ponksiyon yapıldı. BOS direkt bakısında 130 lenfosit, 5 PMNL izlendi. Mikroprotein: 107, glukoz: 28, kan şekeri: 119 olarak ölçüldü. Hasta santral sinir istemi enfeksiyonu tanısıyla kliniğimize yatırıldı. BOS örneklerinden Gram, ARB, BOS kültürü, HSV PCR, TBC PCR, tüberküloz kültürü gönderildi. PPD testi yapıldı. Ampirik olarak seftriakson 2x2 gr IV, deksametazon 4x1 amp IV, nexium 1x1 amp IV, mannitol 4x125 mg IV order edildi. Hastanın takiplerinde baş ağrısı şiddetlendi, şuuru açıldı alınan biyokimya tetkiklerinde sodyum 125 olarak geldi. Hiponatremisi ve subakut kliniği olan hastaya PPD testinin de 17 mm gelmesi üzerine tarafımızca ampirik olarak 4’lü antitüberküloz tedavi başlandı. Kontrastlı MR çekildi. Leptomeningeal kontrastlanma artışı izlendi. TBC PCR pozitif olarak geldi. İzlemin 32. gününde BOS tüberküloz kültüründe Mycobacterium tuberculosis üredi. Hastanın takibinde bel ağrısı olması üzerine torakolomber MR çekildi. MR raporu “T11-12 seviyesinde 3 cm’lik kitlesel lezyon, leptomeningeal kontrastlanma artışı mevcut olup bulgular leptomeningeal metastaz lehine değerlendirilmiştir” şeklinde sonuçlandı. Takiplerinde parapleji gelişen hasta beyin cerrahi kliniğine operasyon açısından danışıldı ve opere edildi, kitlesel lezyon eksize edilerek patolojiye gönderildi. Beyin cerrahi tarafından kitlesel lezyon makroskobik olarak glioblastom lehine değerlendirildi. Hastanın patoloji raporu “kesitlerde geniş nekroz alanları, fokal alanlarda nekrotizan granülom benzeri yapılanmalar izlemiştir” şeklinde sonuçlandı. Hastada TBM’ye bağlı paravertebral apse gelişti. Hastanın steroid tedavisi azaltılarak kesildi. Walker yürüteçle yürümeye başlayan hasta ikili antitüberküloz tedavi ile taburcu edildi. Sonuç: Olgumuzda TBM’nin bir komplikasyonu olabilen paravertebral apse meydana gelmiştir. TBM’li olgularda %10’dan daha az görülebilen ve paraplejiye neden olabilen paravertebral apseler açısından dikkatli olunmalı bu paravertebral apselerin kitle ve metastaz ile karışabileceği akılda tutulmalıdır. Spinal tuberculosis (Pott's disease) is still an important problem in many countries and may result in severe neurological deficits. Pott's paraplegia can occur in the early period of the disease or many years later. Pott's disease usually occurs in the thoracic vertebrae and may cause neurological symptoms as a consequence of bone destruction and spinal cord compression. In this article, we present the case of a 73-year-old diagnosed with Pott's paraplegia who was referred to our clinic with back pain. He had been previously diagnosed with pulmonary tuberculosis and had received anti-TB therapy. After five weeks of an intensive rehabilitation program, the patient could walk independently and was discharged with minimal dependency. We conclude that there should be more awareness of the possibility of non-traumatic spinal cord injuries in elderly patients. [ABSTRACT FROM AUTHOR]

Spinal epidermoid tumors are rare tumors, constituting about 1.5-2% of the spinal tumors. Cervical and thoracic spinal cord epidermoid tumors are exceedingly rare. We reported a case of a 28-year-old patient with an intradural extramedullary epidermoid cyst at the thoracic region presented with urinary incontinence and paraparesis to our outpatient clinic. First symptoms of patient started 16 years ago and diagnosed as peroneal muscular atrophy and he couldn't be investigated sufficiently. Because of the delayed diagnosis and treatment, paraparesis and urinary incontinence were persistent in spite of successful surgery and proper physical therapy. Spinal epidermoid tumors have a good prognosis, as they are histologically benign. When diagnosed early, complete surgical resection offers the patient good neurologic outcome. It is important to investigate extensively patients with neurological deficits and to make diagnosis as soon as possible. [ABSTRACT FROM AUTHOR]

Plazma hücre neoplazileri plazma hücre miyelomu, plazmasitom, immunglobulin depo hastalıkları, POEMS sendorumu (osteoklastik myelom) ve hafif zincir hastalıkları olmak üzere sınıflandırılır. Soliter plazmasitom plazma hücre neoplazilerinin % 10 unu oluşturan, malign plazma hücrelerinden oluşan lokal primer kemik tümörüdür. Olgumuz nadir görülen vertebra lokalizasyonlu plazmasitomun akut paraplejiye yol açması açısından önem arz ediyordu. 80 yaşında erkek hasta acil servise alt ekstremitelerde ani başlayan güçsüzlük ile başvurdu. Travma tanımlamayan hastanın 20 yıl önce kostal soliter plazmasitom tanısı olmakla birlikte takiplerinde şikayeti yoktu. Komorbiditesi de bulunmuyordu. Bilinç açık, oryante, koopereydi. T6 vertebra seviyeli ASİA A spinalkord hasarı tespit edildi. Ön tanıda aort diseksiyonu düşünülmedi. Kranyal BT normal olarak bulundu. Servikal, torasik ve lomber MR görüntülemelerde T2 sekansında T6 vertebra bölgesinde spinal korda posteriordan bası yapan intradural ekstramedüller izointens kitle saptandı. Kitle eksize edildi. Patoloji sonucu plazmasitoid diffüz tümöral proliferasyon olarak rapor edildi. Çekilen PETBT de multipl lokalizasyonlu, BT karşılıkları izlenmeyen, normalden yoğun artmış FDG tutulum odakları saptandı. Sırt ağrısı, ekstremitelerde güçsüzlük durumunda etyolojide çok uzun yıllara uzanan ve şifa bildirilen tümoral nedenler de göz önünde bulundurulmalıdır., Plasma cell neoplasms are classified as plasma cell myelomas, plasmacytomas, immunglobulin deposit diseases, POEMS syndrome (osteoclastic myeloma) and light chain diseases. Solitary plasmacytoma is a local primary bone tumor composed of malignant plasma cells, forming 10% of the plasma cell neoplasms. Our case was important because of rarity of vertebral localized plasmacytoma causing acute paraplegia. An 80-year-old male patient was admitted to the emergency room with sudden onset weakness in the lower extremities. The patient, who had no trauma, had no complaints on his follow-up with the diagnosis of costal solitary plasmacytoma 20 years ago. He did not have comorbidities either. Patient was conscious, oriented and cooperated. Diagnose was T6 vertebra leveled ASIA-A spinal cord injury. Aortic dissection was not considered at the diagnosis. Cranial CT was usual. In cervical, thoracic and lumbar MRI images, an intradural extramedullary isointense mass was found that posteriorly compresses spinal cord at the T6 vertebral region in the T2 sequence. The mass was removed. Pathologic result of the mass was reported as plasmacytoid diffuse tumor proliferation. In PET-CT: foci of multiple localization, intense, increased FDG involvement without computerized tomography correspondence were detected. In cases with back pain and weakness in lower exremities tumoral stuctures even diagnosed long ago and accepted as recovery should be considered.

After five weeks of an intensive rehabilitation program,the patient could walk independently and was discharged with minimal dependency. We conclude that there should be more awareness of the possibility of non-traumatic spinal cord injuriesin elderly patients. Spinal tüberküloz (Pott hastalığı) günümüzde halen bir çok ülkede önemli bir sağlık sorunudur ve ağır nörolojik kayıplara neden olabilmektedir. Pott hastalığına bağlı parapleji, hastalığın erken dönemlerinde yada yıllar sonra ortaya çıkabilmektedir. Genellikle torasik vertebralarda ortaya görülmekte, kemik yıkımı ve spinal kord basısına bağlı olarak da nörolojik bulgulara sebep olabilmektedir. Bu makalede, kliniğimize bel ağrısı ile yönlendirilen, Pott paraplejisi tanısı konan 73 yaşındaki bir hasta sunulmaktadır. Hasta daha öncesinde pulmoner tüberküloz tanısı ile takip edilmekte ve anti-tüberküloz tedavi almaktaydı. Hasta beş haftalık yoğun rehabilitasyon programından sonra bağımsız olarak yürüyebildi ve minimal bağımlı olarak taburcu edildi. Sonuç olarak yaşlı hastalarda travmatik olmayan spinal kord yaralanmaları ihtimali konusunda dikkatli olunmalıdır.

34. Impact of vitamin D on mobilization, pulmonary function tests, grip strength and functionality in patients with spinal cord injury: A cross-sectional study

Aim: Since vitamin D deficiency is a growing problem worldwide, insufficient levels of vitamin D were reported in patients with spinal cord injury (SCI). It was stated that levels of vitamin D may be an indirect indicator of functional status in patients with SCI. The aim of this study was to investigate the relationship between vitamin D levels and mobilization, functionality, grip strength, and pulmonary function test parameters in patients with subacute SCI. Methods: Fifty-eight patients with subacute motor complete thoracic (T) and lumbar SCI injuries [46 males, 12 females; mean age 32.0 (11.2) years] were included. The time to complete the verticalization program on the tilt table without any orthostatic symptoms was considered as the ‘time of mobilization.’ Grip strength (GS) was measured using a dynamometer, pulmonary function test (PFT) parameters were measured using a spirometer, and functional status was measured using the Functional Independence Measure (FIM). For determining the levels of vitamin D, 25-hydroxy-vitamin D [25(OH)D] was measured and levels below 20 ng/mL were considered as deficiency. The patients were divided into two groups according to 25(OH)D levels, Amaç: D vitamini eksikliği dünya çapında büyüyen bir problem olduğundan, omurilik yaralanmalı (OY) hastalarda da yetersiz D vitamini seviyeleri bildirilmiştir. D vitamini düzeylerinin, OY hastalarda fonksiyonel durumun dolaylı bir göstergesi olabileceği belirtilmiştir. Bu çalışmanın amacı, subakut dönemdeki OY hastalarda D vitamini düzeyleri ile mobilizasyon, fonksiyonel durum, el kavrama gücü ve solunum fonksiyon testi (SFT) parametreleri arasındaki ilişkiyi araştırmaktır. Yöntemler: Subakut motor komplet torakal (T) ve lomber OY 58 hasta [46 erkek, 12 kadın; yaş ortalaması 32.0 (11.2) yıl] çalışmaya dahil edildi. Herhangi bir ortostatik semptom olmadan tilt masası ile vertikalizasyon programını tamamlama süresi “mobilizasyon zamanı” olarak değerlendirildi.
