Aim: The aim of our study is evaluate the association between serum vitB12 levels at time of diagnosis and clinical and laboratory signs in multiple (MM) myeloma patients Patients and Methods: Patienst diagnosed MM between 2010-2020 were evaluated. Patients were divided into two groups; those had low and normal levels of vitB12. Groups were analized by using independent simple T, Mann Whitney U and chi-square tests considering by distrubition characteristics of data. Results: Two hundred-and-twelve patients' records were examined retrospectively. Forty-one (32%) patients had low vitB12 levels. In patients with low vit B12 levels, bone marrow plasma cell ratios were also significantly lower than those found normal vit B12 levels (median %30 vs %50, respectively) (p: 0,024). There was no statistically significant difference between the two groups in terms of age, lytic bone lesion numbers, and serum levels of total protein, lactate dehydrogenase, IS2-microglobulin, calcium, creatinin, albumin. However, 11 patients (%26,8) in low vitB12 group had anemia, while 44 (%50,5) patients in normal vitB12 group had. So, anemia was more often in vitamin B12 normal group patients (p: 0,019). No difference was found in terms of myeloma ISS and R-ISS stages in two patient groups. Conclusions: Bone marrow median plasma cell ratio was also low in the group with low vit B12 levels. Our data could not show its clinical reflection. Further studies with larger groups are required to evaluate the response to treatment, survival and recurrence rate. [ABSTRACT FROM AUTHOR]

Clinical Practice Points • AL amyloidosis is classically associated with plasma cell disorders. Albeit rarely, it has also been described in association with hematological disorders. • We describe here a case of a patient with gastrointestinal AA amyloidosis, which was likely the result of long-standing biclonal gammopathy IgM-κ and IgG-λ. • We believe that this finding can be instrumental to raise awareness about the possibility that long-standing gammopathies can result in AA amyloidosis.

B cells play a pivotal role in adaptive immune system, since they maintain a delicate balance between recognition and clearance of foreign pathogens and tolerance to self. During maturation, B cells progress through a series of developmental stages defined by specific phenotypic surface markers and the rearrangement and expression of immunoglobulin (Ig) genes. To get insight into B cell proteome during the maturation pathway, we studied differential protein expression in eight human cell lines, which cover four distinctive developmental stages; early pre-B, pre-B, plasma cell and immature B cell upon anti-IgM stimulation. Our two-dimensional differential gel electrophoresis (2D-DIGE) and mass spectrometry based proteomic study indicates the involvement of large number of proteins with various functions. Notably, proteins related to cytoskeleton were relatively highly expressed in early pre-B and pre-B cells, whereas plasma cell proteome contained endoplasmic reticulum and Golgi system proteins. Our long time series analysis in anti-IgM stimulated Ramos B cells revealed the dynamic regulation of cytoskeleton organization, gene expression and metabolic pathways, among others. The findings are related to cellular processes in B cells and are discussed in relation to experimental information for the proteins and pathways they are involved in. Se reportan cinco casos de enfermedad de Castleman (EC) ingresados en el Hospital "Hermanos Ameijeiras". Cuatro casos correspondieron a la variedad histológica hialinovascular y uno a la plasmocelular. Todos los casos de la variedad hialinovascular fueron localizados y uno de ellos se complicó con un sarcoma de células foliculares dendríticas. La variedad plasmocelular fue del ltipo multicéntrico. De los cuatro casos localizados dos fueron retroperitoneales y recibieron sólo tratamiento quirúrgico; el otro de localización cervical recibió radioterapia y esteroides; y el cuarto de localización axilar, fue tratado con exéresis quirúrgica seguido de radioterapia. El último caso, de tipo multicéntrico y plasmocelular, recibió sólo tratamiento con prednisona. Todos los casos se mantienen vivos en la actualidad, y sólo el de la variedad plasmocelular continua con síntomas. It has been reported five cases of Castleman's disease. They were admited at "Hermanos Ameijeiras" Surgical-Clinic Hospital. In four out of the five cases belonged to hyaline-vascular histological variety and one case to plamocelular variety. All the cases which belonged hyaline-vascular variety were located and one of them was complicated with sarcoma of dendritic follicular cells. The plasmacelular variety was of the multicentric type. Of the four located cases two of them were retroperitoneals and they received surgical treatment only; the case of cervical location was treated with radiotherapy and steroids and the forth one of axillary location was treated with surgical removal followed of radiotherapy. The last one case, of plasma-cell and multicentric type, was only treated with prednisone. All the patients are still alive and only the one with plasmacelular variety has symptoms yet.
