ABSTRACT Introduction. Cardiopulmonary resuscitation (CPR) has a low survival rate after cardiac arrest. According to the Spanish Out-of-Hospital Cardiac Arrest Registry (OHSCAR), the incidence is 24.2 per 1,000,000 inhabitants, with only a 9.8% survival rate at discharge. Since 2005, international resuscitation organizations have recommended focusing on performing high-quality CPR and developing new tools to optimize it. Objective. To evaluate the usefulness of capnography and cerebral oximetry in improving the quality of CPR and predicting clinical and neurological outcomes. Methodology. Articles were reviewed in Science Direct, Medline, and the Cochrane Library, selecting primary research and systematic reviews published between 2011 and 2024. The search included terms related to end-tidal carbon dioxide and cerebral oximetry in CPR. Results. Capnography allows real-time monitoring of ventilation and cardiac performance, serving as a key predictor of the return of spontaneous circulation (ROSC). Values above 10 mmHg are associated with a higher probability of ROSC. Cerebral oximetry measures brain oxygen saturation and indicates CPR effectiveness, with values above 40% correlated with better outcomes. Discussion. Capnography and cerebral oximetry are essential tools in CPR. Although cerebral oximetry shows potential in predicting neurological damage, more studies are needed for its implementation. Integrating these measurements could improve CPR outcomes.

Introduction: Every year, five million people around the world experience an out-of-hospital cardiac arrest (OHCA) and less than 40 % receive any assistance before the arrival of the Emergency Medical Services (EMS). Ambulance operators (AO) take care of people experiencing an OHCA, stabilize and then transfer them. In Medellín, Colombia, there is a public AO and several private providers, but the information about an OHCA and the operational characteristics during the response to the event are limited. Objective: To estimate the incidence of OHCA and to explore the factors associated with survival after the event in Medellín city. Methods: Retrospective, population-based cohort study. All the medical records of patients experiencing an OHCA who were assessed and treated by ambulance operators, (AO) of the Emergency Medical Services (EMS) and private agencies in Medellin city were reviewed. Descriptive statistics were used for data analysis and the annual incidence of the event standardized for the general population was estimated. Potential survival-associated factors reported as OR with their corresponding 95% CI were explored. Results: A total of 1,447 patient records with OHCA between 2018 and 2019 were analyzed. The event incidence rate for the number of cases assessed was 28.1 (95 % CI 26.0-30.3) and 26.9 (95 % CI 24.9- 29.1) cases per 100,000 inhabitants/year for 2018 and 2019, respectively; the incidence rate of treated OHCA was 2.6 (95 % CI 2.0-3.3) and 3.2 (95 % CI 2.5-4.0) per 100,000 inhabitants/year, for 2018 and 2019. Survival on arrival at hospitals of treated cases was 14.2 % (95 % CI 5.5-22.8) and 15.5 % (95 % CI 7.4-23.5) for 2018 and 2019, respectively. Conclusions: This study portrays the operating and care characteristics of the population experiencing OHCA in Medellín city. The incidence rate of the event and the survival were lower than those reported in the literature. [ABSTRACT FROM AUTHOR]

AUTOMATED external defibrillation , *CARDIAC arrest , *CARDIOTONIC agents , *CARDIOPULMONARY resuscitation , *DEFIBRILLATORS , *DIAGNOSIS , *EMERGENCY medical services , *EMERGENCY medicine - Abstract
Sudden cardiac death is a common occurrence. Out-of-hospital cardiac arrest is a global public health problem suffered by ≈3.8 million people annually. Progress has been made in the knowledge of this disease, its prevention, and treatment; however, most events occur in people without a previous diagnosis of heart disease. Due to its multifactorial and complex nature, it represents a challenge in public health, so it led us to work in a consensus to achieve the implementation of cardioprotected areas in Mexico as a priority mechanism to treat these events. Public access cardiopulmonary resuscitation (CPR) and early defibrillation require training of non-medical personnel, who are usually the first responders in the chain of survival. They should be able to establish a basic and efficient CPR and use of the automatic external defibrillator (AED) until the emergency services arrive at the scene of the incident. Some of the current problems in Mexico and alternative solutions for them are addressed in the present work. [ABSTRACT FROM AUTHOR]

Sudden Cardiac Death (SCD) and Out-of-Hospital Cardiac Arrest (OHCA) are global public health problems suffered by ≈3.8 million people annually; they represent a challenge in public health, which leads us to work on regulations, legislation, and consensus to implement the formation of protected Cardio areas in Mexico. The increase in cardio-protected spaces and the use of Automatic External Defibrillators (AED) in the world have contributed to improving the survival of OHCA. The use of AEDs and early public access defibrillation requires training for non-medical personnel, who are generally the first to assist and start the chain of survival, with basic and efficient Cardiopulmonary Resuscitation (CPR) until emergency services arrive at the scene of the incident. In this article, we present a structured guide to the steps that must be followed to accredit a cardioprotected space in Mexico and distinguish between public access defibrillation and a cardio-protected area. ENDOVASCULAR surgery , *HOSPITAL mortality , *WOUNDS & injuries , *BALLOON occlusion , *TRAUMA centers , *CARDIOPULMONARY resuscitation , *DEATH rate , *BLOOD transfusion - Abstract
Introduction: The objective is to describe the utility of the Critical Administration Threshold (CAT) as a tool in hemostatic resuscitation in patients with severe trauma and REBOA. Methods: Retrospective review between January 2015 and June 2020 of adult patients with hemorrhage secondary to trauma with or without REBOA in a level I trauma center in Cali, Colombia. Demographic variables, trauma severity, clinical status, transfusion needs, time to CAT+ and number of CAT achieved (1, 2 or 3) were recorded. Results: Ninety-three patients were included, in which REBOA was used in 36 and traditional management in 57. The REBOA group had a higher bleeding volume (3000 ml), IQR: 1950-3625 ml vs the control group (1500 ml, IQR: 700-2975 ml) (p<0.001) and a higher rate of PRBC units transfused in the first 6 hours (median 5, IQR: 4-9); p=0.015 and in the first 24 hours (median 6, IQR: 4-11); p=0.005. There were no statistically significant differences in the number of CAT+ patients between groups or time to CAT+. However, CAT+ status during the first 30 minutes of surgery was higher in the REBOA Group (24/36, 66.7%) vs. the control group (17/57, 29.8%; p=0.001), having this group a higher in-hospital mortality rate vs. CAT-patients. Conclusion: CAT is a useful tool in the hemostatic resuscitation of patients with trauma and REBOA that could predict early mortality. [ABSTRACT FROM AUTHOR]

In clinical practice, it is not rare to encounter situations in which parents and families are asked to leave the child alone with the health care team in rooms full of devices throughout the performance of procedures, which at times may give rise not only to conflicts but, more importantly, emotional sequelae in children or adolescents. We conducted a narrative review of the literature by searching the digital library of the public health care system of Andalusia for articles concerning the experiences of health care professionals and families with the accompaniment of paediatric patients during health care procedures. We restricted the search to studies published in Spanish or English and conducted in humans. The review evinced the need to humanise care in order to improve care quality. The need to accompany minors is supported by the evidence from works that have analysed the factors involved in the persistence of these behaviours and attitudes in both professionals and parents. We consider it necessary to develop institutional policies and appoint mediators to compile the statements of different national and international societies, taking into account legal aspects but, above all, the pertinent values from a health care ethics perspective, and in pursuit of the best interests of the child.

HEART assist devices , *CARDIOPULMONARY resuscitation , *HEART failure - Abstract
Advanced heart failure is becoming more frequent and, in this population, left ventricular assist devices have emerged in the last thirty years as bridge-to-transplant or destination therapy for those with a contraindication for transplant. These devices are continuous flow, contrary to normal hemodynamic physiology, which leads to associated complications with the interaction between the device and the patient, a phenomenon known as “haemocompatibility”. One of its implications is the absence of a central pulse and the need to measure blood pressure through infrequent methods such as Doppler. Various complications are frequently associated with this and can be grouped under the mnemonic A2B2C2D2, including those specific and related to the device. Each of these must be recognized for early and prompt diagnosis and treatment, including prompt cardiopulmonary resuscitation, if indicated, to impact the survival of these patients. The number of air travelers has increased over the past years, and after the limitations imposed by the COVID-19 pandemic, are expected to increase further. Given these conditions, the possibility for a health professional to find himself needing to assist an in-flight cardiac arrest is also increased. The present work describes some of the most common problems in managing an inflight cardiac arrest and emphasizes the relevance of cardiopulmonary resuscitation (CPR) performance and automated external defibrillators (AED) use on board of commercial airliners. Introduction: Basic life support training in school age is a topical issue because, with adequate training, any person can help save a life. Methods: Cluster clinical trial with data collection through an ad hoc self-administered, semi-structured questionnaire. The target population encompassed the students aged 4–6 years enrolled in 49 educational centres. The centres were randomly allocated to the intervention or control group. The intervention group was trained with the RCParvulari® methodology, consisting of theoretical and practical training on the first link of the chain of survival. The control group only received theoretical training. We evaluated participants before and immediately after the intervention and between 3 and 12 months post intervention by means of the questionnaire. We assessed the acquisition and retention over time of the knowledge and skills covered in the training compared to previous trainings in both groups. Results: A total of 1327 schoolchildren (79% of the target population) participated. The level of knowledge acquired immediately after training and after 3–12 months compared to baseline was significantly better (P

- Abstract
Introduction: Training in cardiopulmonary resuscitation (CPR) and correct use of the automated external defibrillator (AED) allows the intervention of lay citizens in situations of out-of-hospital cardiac arrest (EHPA). In Paraguay, Law No. 5,578/2016 establishes the mandatory use of the AED in mass attendance centers (CCM). Methodology: quasi-experimental before/after study in CCM representatives (Universities, shopping malls, Social/Sports Clubs, Gyms, Automotive Dealership, Residential Complexes and Supermarkets). Data were obtained from universities of the Ministry of Education and Sciences, municipal sources, and the Chamber of Commerce. Between 1-3 representatives/MCCs were trained through theoretical-practical workshops on basic CPR and correct use of the AED in HCWP with a validated questionnaire (Cronbach's alpha: 0.75). Data were compared with ?² and Mann-Whitney t-test or U test (p-significant value =<0.05). Results: Of 194 participants, the mean age was 29±5; 60.31% (117) were male, 38.65% (70) were shopping mall representatives, 60.82% (118) had a university education, and 45.36% (88) worked in customer service. 59.228% (115) had not previously received CPR training and 86.08% (167) had no AED training. A statistically significant difference was observed between the before and after score, with a difference of 4.791(1.012 points (p=0.0001). Discussion: The intensive training program is effective in raising the level of knowledge and practice in basic CPR and AED use among lay CCM representatives., Introducción: La capacitación en reanimación cardiopulmonar (RCP) y uso correcto del desfibrilador externo automático (DEA) permite la intervención de los ciudadanos legos en situaciones de paro cardíaco extrahospitalario (PCEH).

Introduction: As there are advances in medicine, there are advances in the learning modalities of this science, such as simulated interventions. Various studies demonstrate it’s importance for professional development, being fundamental in addition, in the strengthening and improvement of teamwork in its multiple stages and contexts. Objective: the aim of this study is to evaluate the development of teamwork skills through the use of simulation workshops in neonatal resuscitation in undergraduate students. Material and method: In 2017, a survey was applied to 80 students of third and fifth year of the Obstetrics undergraduate course at the University of Chile, to measure various aspects of teamwork during a neonatal resuscitation simulation, which includes activities such as recognition of the physical plant and material, and application of knowledge in resuscitation situations where the roles of each student alternate. The applied survey corresponds to the translation of internationally validated instruments; it was applied at the beginning and at the end of the intervention. Results: There was a general increase in the value of each aspect, with significant changes in 20% of them. The areas of “resource allocation”, “goal setting” and “transparent thinking” stand out significantly. Conclusions: The neonatal resuscitation simulation workshops were able to improve the students’ perception of their teamwork skills in fundamental aspects for teamwork in health, however they don’t represent a significant benefit at global level., Introducción: A medida que existen avances en la medicina, existen progresos en las modalidades de aprendizaje de esta ciencia, como son las intervenciones simuladas.

- Abstract
- Abstract
Background: Neonatal mortality is a health problem affecting Latin American countries, it can be prevented by providing adequate health care at all levels (1). One of the protocols to prevent neonatal death is the application of cardiopulmonary resuscitation (2). Objectives: To determine the knowledge and attitudes of nursing professionals on neonatal cardiopulmonary resuscitation in two hospitals in the Austro region of Ecuador, 2023-2024. Methods: The method used was a quantitative, descriptive, cross-sectional study. A 48-question questionnaire on the level of knowledge and attitudes about neonatal cardiopulmonary resuscitation (CPRN) was administered to nurses working at the Hospital de Especialidades "José Carrasco Arteaga" and the Hospital "Homero Castanier Crespo". The information was condensed in a database in Microsoft Excel 2019 and analyzed in the IBM SPSS Statistics version 18 program according to the type of variable; for qualitative variables, absolute frequencies and percentages were used; and for quantitative variables, measures of central tendency and dispersion were used. Results: Of all the nursing professionals who participated in the study, 19.8% had a good level of knowledge of CPRN, 75.3% had fair knowledge and 4.9% had poor knowledge. In terms of attitude, 95.4% have a positive attitude and 4.6% have a negative attitude. Conclusion: The nursing professionals studied have a fair level of knowledge and a positive attitude towards CPRN.

- Abstract
The global incidence and outcomes of out-of-hospital cardiorespiratory arrest (OHCA) vary significantly worldwide. Bystander actions play a crucial role in influencing the survival and outcomes of OHCA victims because when CPR was initiated by bystanders before the arrival of emergency medical services (EMS) resulting in a survival rate twice as high as when initiated by EMS. Unfortunately, individuals who could assist in such emergencies may lack the necessary knowledge, attitudes, and basic life support (BLS) skills, leading to up to 65% of OHCA events going unattended until ambulance arrival. Bystander intervention is also vital in other out-of-hospital emergencies, such as unplanned childbirths, which can be challenging when they occur far from healthcare facilities or suddenly in multiparous women. The 35% of the unplanned out-of-hospital childbirths receiving no medical care and being attended by family members or witnesses.

- Abstract
Objective: To validate the efficacy of brief CPR training with dual feedback to maintain the ability to perform quality chest compressions., Materials and Methods: Quasi-experimental study with two groups to evaluate a brief theoretical training followed by a practice with manikin with feedback; Participants: 155 health and non-health professionals from 5 primary care health centers of urban area (43 losses); Main measurements: Characteristics of compressions that were measured before and after the brief training and their maintenance at 3 and 6 months according to the study group. The effect of training and maintenance of skills were analyzed using multiple linear regression models., Results: 155 participants were included, mean age 39.7 years (SD=12.0) with 82.7% female. The training effect had an improvement in mean compression depth (pre-post difference: 3.5, P<.001), total compressions with adequate depth (pre-post difference: 0.2, P<.001) and Total Compressions with Adequate Rhythm (pre-post difference: 0.4, P<.001). The second phase was completed by 112 participants (72.2%). Compression skills declined at 3 months and were lower at 6 months, although the loss of skills was not statistically significant between the two groups., Conclusions: An individualized, brief training action with feedback immediately improves the quality of compressions. The progressive loss of skills from 3 to 6 months is not relevant., (Copyright © 2024 Sociedad Española de Médicos de Atención Primaria (SEMERGEN). Publicado por Elsevier España, S.L.U. - Abstract
Objective: To analyze if the implementation of a multidisciplinary extracorporeal cardiopulmonary resuscitation (ECPR) program in a tertiary hospital in Spain is feasible and could yield survival outcomes similar to international published experiences., Design: Retrospective observational cohort study., Setting: One tertiary referral university hospital in Spain., Patients: All adult patients receiving ECPR between January 2019 and April 2023., Interventions: Prospective collection of variables and follow-up for up to 180 days., Main Variables of Interest: To assess outcomes, survival with good neurological outcome defined as a Cerebral Performance Categories scale 1-2 at 180 days was used. Secondary variables were collected including demographics and comorbidities, cardiac arrest and cannulation characteristics, ROSC, ECMO-related complications, survival to ECMO decannulation, survival at Intensive Care Unit (ICU) discharge, survival at 180 days, neurological outcome, cause of death and eligibility for organ donation., Results: Fifty-four patients received ECPR, 29 for OHCA and 25 for IHCA. Initial shockable rhythm was identified in 27 (50%) patients. The most common cause for cardiac arrest was acute coronary syndrome [29 (53.7%)] followed by pulmonary embolism [7 (13%)] and accidental hypothermia [5 (9.3%)]. Sixteen (29.6%) patients were alive at 180 days, 15 with good neurological outcome. Ten deceased patients (30.3%) became organ donors after neuroprognostication., Conclusions: The implementation of a multidisciplinary ECPR program in an experienced Extracorporeal Membrane Oxygenation center in Spain is feasible and can lead to good survival outcomes and valid organ donors., (Copyright © 2024 Elsevier España, S.L.U. and SEMICYUC. Purpose: To validate the content and adequacy of the «Rescube» training material that includes adapted information from the chain of survival. Material and methods: The study included three steps: (i) material development by 7 experts, following Delphi method; (ii) assessment of training material by 11 experts by means of a Likert score and calculation of content validity; and (iii) pilot study in two groups of 5–8 years-old: Rescube group (GR; n = 60) and Traditional group (GT; n = 60). GR was trained with Rescube and a Teddy bear, while GT was traditionally trained with a pediatric manikin. Participants were individually assessed at baseline, and one week and one month after training. Results: All content validity indexes calculated are above the recommended cut-off for analysis with more than 9 experts (≥0,80). Children’s learning results were positive, with percentages equal or higher than 80% in all registered variables at the first (one week) evaluation and equal or higher than 67% when evaluated one month after training. No significant differences were detected between groups. Conclusion: The Rescube training tool based on infantile pictures is valid and useful to train young schoolchildren in the chain of survival.
