Abstract Background In Canada, as in many other countries, private dental insurance addresses financial barriers to a great extent thereby facilitating access to dental care. That said, insurance does not guarantee affordability, as there are issues with the quality and level of coverage of insurance plans. As such, individuals facing barriers to dental care experience poorer oral health. Therefore, it is important to examine more keenly the socio-demographic attributes of people with private insurance to particularly identify those, who despite having insurance, face challenges in accessing dental care and experience poorer oral health. Methods This study is a secondary data analysis of the most recent available cycle (2017-18) of the Canadian Community Health Survey (CCHS), a national cross-sectional survey. Univariate analysis was conducted to determine the characteristics of Ontarians with private insurance (n = 17,678 representing 6919,814 Ontarians)—bivariate analysis to explore their financial barriers to dental care, and how they perceive their oral health. Additionally, logistic regressions were conducted to identify relationships between covariates and outcome variables. Results Analysis shows that the majority of those with private insurance do not experience cost barriers to dental care and perceive their oral health as good to excellent. However, specific populations, including those aged 20–39 years, and those earning less than $40,000, despite having private dental insurance, face significantly more cost barriers to access to care compared to their counterparts. Additionally, those with the lowest income (earning less than $20,000 annually) perceived their oral health as “fair to poor” more than those earning more. Adjusted estimates revealed that respondents aged 20–39 were six times more likely to report cost barriers to dental care and ten times more likely to visit the dentist only for emergencies than those aged 12–19. Additionally, those aged 40–59 were two times more likely to report poorer oral health status compared to those aged 12–19. Conclusion Given the upcoming implementation of the Canadian Dental Care Plan, the results of this study can support in identifying vulnerable populations who currently are ineligible for the Plan but can be benefitted from the coverage.

Introduction: Diabetes, a key chronic non-communicable disease, poses a substantial public health burden. The role of oral health as a determinant in the epidemiology of diabetes mellitus, particularly in the Central Eastern European region, remains underexplored. This research aims to examine the impact of specific oral health parameters, including gum bleeding, active dental caries, tooth mobility, and tooth loss, on diabetes prevalence. Additionally, it seeks to clarify the moderating effects of socio-demographic and lifestyle variables on this relationship. Materials and methods: Data were extracted from the 2014 and 2019 datasets of the Hungarian European Health Interview Survey, comprising a combined nationally representative sample of 11,429 participants. Descriptive statistics were presented as weighted proportions and unweighted counts, and weighted Pearson's chi-squared tests were employed for assessing associations and goodness-of-fit. Significant predictors were integrated into weighted multiple logistic regression models for analysis. Sensitivity analysis was then conducted to confirm the robustness of the findings. Results: The study identified 'Bad' self-perceived oral health as a diabetes risk (OR=1.35; 95% CI: [1.04-1.75]), with filled teeth being protective (0.65 [0.51-0.84]). Subgroup analysis revealed higher diabetes odds among individuals with primary education (1.41 [1.02-1.96]) and rural residents with tooth loss from decay (3.54 [1.36-9.19]). The bootstrap analysis with 1,000 iterations reaffirmed the model's stability and predictive accuracy for diabetes. Discussion: Enhanced oral health is associated with lower risk factors for diabetes. Abstract Background Frailty contributes to adverse outcomes in older adults and places a heavy burden on healthcare resources. Dysphagia is associated with frailty, but the mechanisms by which dysphagia affects frailty in older adults are unclear. This study aimed to investigate a serial mediating effect of self-perceived oral health and self-reported nutritional status in the relationship between dysphagia and frailty among hospitalized older patients in China. Methods This cross-sectional study included 1200 patients aged ≥ 65 years in the Department of Geriatrics, Shaanxi Provincial People’s Hospital. A structured face-to-face interview was used to survey the following questionnaires: General Information Questionnaire, Tilburg Frailty Indicators (TFI), Eating Assessment Tool-10 (EAT-10), 30mL Water Swallow Test (WST), Geriatric Oral Health Assessment Index (GOHAI), and Short-Form Mini-Nutritional Assessment (MNA-SF). A total of 980 participants with complete data were included in the analysis. Statistical analysis was performed using SPSS 26.0 and Amos 28.0 software. Spearman’s correlation analysis was used for correlation analysis of study variables. Dysphagia was found to directly affect frailty (β = 0.161, 95%CI = 0.089 to 0.235) and through three significant mediation pathways: (1) the path through self-perceived oral health (β = 0.169, 95%CI = 0.120 to 0.221), accounting for 36.98% of the total effect; (2) the path through self-reported nutritional status (β = 0.050, 95%CI = 0.023 to 0.082), accounting for 10.94% of the total effect; (3) the path through self-perceived oral health and self-reported nutritional status (β = 0.077, 95%CI = 0.058 to 0.102), accounting for 16.85% of the total effect. The total mediation effect was 64.77%. Conclusions: This study indicated that dysphagia was significantly associated with frailty. Self-perceived oral health and self-reported nutritional status were serial mediators of this relationship. Objectives: This study examined the dental insurance coverage, dentist visits, self-perceived oral health status, and dental problems among Asian immigrant women of childbearing age in contrast to Canadian women of childbearing age and non-Asian immigrant women of childbearing age. Potential barriers to dental care services among Asian immigrant women were explored. Methods: This analysis utilized data from the combined Canadian Community Health Survey from 2011 to 2014. The analytical sample consisted of 5737 females whose age was between 20 and 39 years. Multivariable logistic regression models assessed immigrant status and other factors in relation to the indicators of dental health (i.e., dental visit, self-perceived oral health, acute teeth issue, and teeth removed due to decay). Results: Amongst Asian women immigrants of childbearing age, there was a significantly lower frequency of dentist visits compared to non-immigrant counterparts (OR = 0.53; 95% CI: 0.37–0.76). The most commonly reported reason for not seeking dental care in the last three years was that the "respondent did not think it was necessary". Relative to Canadian born women of same age bracket, Asian women of childbearing age reported fewer acute teeth issues (OR = 0.67; 95% CI: 0.49–0.91) and had a greater risk of tooth extracted due to tooth decay (OR = 3.31; 95% CI: 1.64–6.68). Furthermore, for Asian women immigrants, their major barriers to dental care included low household income (≤$39,999 vs. $40,000–$79,999 OR = 0.26) and a lack of dental insurance (no vs. yes OR = 0.33). Conclusions: Asian immigrant women showed lower utilization of dental services than non-immigrant women. A perceived lack of necessity, lower household income, and dental insurance coverage were major barriers to professional dental usage for most Asian immigrants of childbearing age. Background and aims. On March 16, 2020, the state of emergency was established with drastic measures of isolation and quarantine. Among the measures taken in the context of the state of emergency was the closure of dental offices throughout the country. The aim of our questionnaire-based cross-sectional study was to identify the psycho-affective implications of the Romanian citizens induced by the COVID-19 pandemic period as well as their opinion related to the self-perceived oral health status during this period. Methods. An original questionnaire was designed consisting of 41 questions, which were distributed to the general population via the Internet. The first part of the questionnaire investigated the respondents' opinion regarding the impact of this COVID-19 pandemic period of isolation on their oral health and the attention paid by the subjects to oral hygiene during restrictions. The second part refers to their addressability to the dental services during this pandemic and the third part interrogates different characteristics, namely the presence of comorbidities, possible anxiety, and stress. Results. A total of 769 participants answered the questionnaire online. Conclusion. According to their responses most of the respondents were not afraid of the possibility of contracting Sars-Cov-2 during dental treatments, but were in certain periods deprived of accessing dental services due to lockdown restrictions. Background: Self-perceived oral health is related to clinical and subjective oral factors, socioeconomic factors, perceived stress, and oral health behavior. However, limited studies have examined whether self-perceived oral health is related to dry mouth, salivary viscosity, or lifestyle factors. Accordingly, this study aimed to verify the effect of self-perceived oral health and stress levels on subjective oral symptoms and lifestyle., Methods: The responses of 644 university students who agreed to participate in the study were analyzed. A chi-square test was used to determine whether self-perceived oral health and stress levels showed significant differences based on subjective oral symptoms and lifestyle. Logistic regression was used to analyze the effect of subjective oral symptoms and periodontal disease diagnosis on self-perceived oral health., Results: Subjective oral symptom factors that showed significant differences depending on self-perceived oral health included gingival bleeding or pain, dry mouth, and saliva viscosity, and depending on lifestyle factors included the frequency of tooth brushing and beverage consumption. Additionally, subjective oral symptoms influenced self-perceived oral health. Self-perceived oral health was negative when there was gingival bleeding, pain (odds ratio (OR)=0.594, p=0.002), and dry mouth (OR=0.577, p=0.001)., Conclusions: This study's results showed that self-perceived oral health significantly impacts gingival bleeding, pain, and dry mouth. Objective: This study investigated the role of self-perceived oral health and normative need on dental services utilization by US adults aged ≥30 years from the 2011-2014 National Health and Nutrition Examination Survey (NHANES).Methods: Data on 5,806 dentate adult NHANES participants were analyzed. Key independent variables were self-perceived oral health and normative need assessed by a dentist. The dependent variable was dental visit within the past 12 months. Select sociodemographic variables were adjusted for in logistic regression models using SAS survey procedures.Results: Overall, 33.8 percent of adults did not have a dental visit in the past 12 months. About 26.9 percent reported they were unsatisfied with their oral health and 53.4 percent needed to seek care. Individuals unsatisfied with their oral health were less likely to have a dental visit in the past 12 months compared to those who were satisfied with their oral health (57 versus 25 percent). Overall, participants who were unsatisfied with their oral health received a recommendation to seek care. These participants had more normative needs and were less likely to have a dental visit in the past 12 months; however, this relationship varied by race/ethnicity. For Asian-Americans, there was no association between those unsatisfied with their oral health and dental visits. Also, there was no association across all race/ethnicities between number of teeth present and dental visits.Conclusion: Both self-perceived oral health and normative need are associated with dental utilization. Self-perceived oral health is affected not only by awareness of the clinical status but also by comparisons with people of a similar age. This study explored the relative contributions of clinical variables assessing caries, periodontal status, and prosthetic status to self-perceived oral health within two age groups. Data of 891 adults (35-44 yr of age) and 760 older people (65-74 yr of age) from the Fourth German Oral Health Study ( DMS IV, 2005) were evaluated. Self-perceived oral health was obtained from questionnaires. Numbers of decayed, filled, and unreplaced teeth, mean attachment loss, bleeding on probing ( BOP), the presence of a fixed denture, and the presence of a removable denture were assessed. Multinomial logistic regression models were developed for both age groups, separately, using stepwise methods. For adults, unreplaced teeth, filled teeth, decayed teeth, the presence of a removable denture, and mean attachment loss were added to the final model. For older people, the presence of a removable denture, unreplaced teeth, decayed teeth, mean attachment loss, filled teeth, and BOP were included in the final model. Awareness of the relative contributions of clinical variables to self-perceived oral health is important for obtaining a clearer understanding of patients' subjective and objective self-perceptions of oral health. Community Dent Oral Epidemiol 2010; 38: 68–76. © 2009 John Wiley & Sons A/S Global self-ratings of oral health (GSROH) have been used in population surveys to assess oral health status. The aim of this study was to assess various candidate predictors for each response level (very poor, poor, fair, and good/very good) of GSROH among Korean elders. Data from a national representative sample of Korean adults aged 65 years and over was analyzed. The mean age of the 1173 subjects was 72.7 years and 60.2% were women. After bivariate analysis, a generalized multinomial logit model with individual slope parameters was performed. Only 29.5% of elders rated their oral health as good or very good, while 53.1% assessed their oral health as poor or very poor. Significant determinants for GSROH for elders (including dentate and edentulous) were difficulty chewing, concern about oral health, the number of missing teeth, and denture state. Income and education were not significant determinants. The number of decayed teeth was an additional determinant in the model for the dentate only subgroup. The indicator demarcating response at a good/very good oral health level compared with poorer oral health levels for all (dentate and edentulous) elders was not reporting having much difficulty chewing. Experiencing loss of 10 or more teeth and being concerned about their oral health distinguishes those in the poor or very poor GSROH levels from the others. For dentate elders, having untreated dental caries was also distinguishing for those reporting very poor oral health. Public health programs aimed at correcting significant chewing functional difficulties in the elderly, by restoring or replacing missing teeth, may alleviate the oral health concerns and improve the overall oral health of the elderly. To determine if psychosocial factors explain the socioeconomic disparities in self-perceived oral health that persist after controlling for oral status variables. Data came from the participants in the Canadian Community Health Survey 2003 who were residents in the city of Toronto. Oral health variables included self-rated oral health, a 13-item oral health scale, denture wearing, and having a tooth extracted in the previous year. The last two measures were regarded as proxy indicators of tooth loss. Psychosocial variables included a self-esteem scale, a depression scale, and single items measuring life satisfaction, life stress, and sense of cohesion. Socioeconomic status was assessed using total annual household income. Interviews were completed with 2,754 dentate persons aged 20 years and over. Bivariate analyses confirmed that there were income gradients in self-rated oral health and scores on the oral health scale. Linear regression analyses confirmed that these persisted after controlling for age, gender, denture wearing, and having a tooth extracted in the previous year. In the model predicting self-rated oral health self-esteem, life satisfaction, stress, a sense of cohesion, and depression also contributed to the model, increased its explanatory power, and reduced the strength of but did not eliminate the association between income and self-rated oral health. Broadly, similar results were obtained when the oral health scale score was used as the dependent variable. In both analyses and all models, denture wearing had the strongest and most enduring effect. Psychosocial factors partly but do not wholly explain the socioeconomic disparities in self-perceived oral health in this population after controlling for tooth loss and denture wearing. Objective: To investigate the association between routine visits for dental checkup and self-perceived oral health. Methods: Cross-sectional data from a study of university employees in Rio de Janeiro – The Pró-Saúde Study. Self-perceived oral health and the reported pattern and frequency of visits to the dentist were obtained through a multidimensional self-administered questionnaire. Results: Data were obtained from 3252 participants. When compared with individuals who reported self-perceived oral health as good (‘very good’, ‘good’ or ‘fair’) individuals who reported self-perceived oral health as bad (‘bad’ or ‘very bad’) were significantly more likely to be older, male, less educated, poorer; they also reported more frequently to have lost more teeth and not visiting the dentist for routine dental ‘checkup’. Among those who reported visiting for dental checks at least annually, 3% reported bad oral health, as opposed to 15% among those who reported visiting the dentist only when in trouble. Compared with those who reported visiting the dentist at least annually, odds ratio of bad oral health was 3.9 (95% CI, 2.68–5.67) for subjects who reported visiting only when in trouble, 2.6 (95% CI, 1.51–4.62) who reported visiting for dental checks less frequently than once every 2 years, and 1.4 (95% CI, 0.77–2.52) for subjects who reported visiting for dental checks once every 2 years, after controlling for sex, age, education, income and tooth loss. Conclusions: Not visiting the dentist for a routine dental check increased the chance of reporting one's own oral health as bad. In any case, the habit of visiting for dental ‘checkup, once per year or once every 2 years was associated with nearly all the individuals perceiving his/her oral health positively. However, in order to gather more solid scientific data to guide public polices it is necessary to perform longitudinal studies, especially experiments in different populations focused mainly on the socioeconomic characteristics and dental clinical conditions.
